4 Effect of Sofia distal access catheter aspiration in the treatment of acute arterial occlusion

 Objective To investigate the effect of Sofia distal access catheter aspiration in the treatment of acute arterial occlusion.Methods From January 2019 to June 2020,60 patients with acute arterial occlusion who received thrombectomy treatment in Jingdezhen No.1 People′s Hospital, Jiangxi Province were selected as the research objects. They were divided into the control group and the study group by the random number table method, with 30 cases in each group,and they were given stent thrombus removal treatment and Sofia distal access catheter aspiration treatment respectively.The surgical treatment, incidence of complications, the National Institutes of Health stroke scale (NIHSS) score, and the modified Rankin scale (MRS) score commonly used to judge the condition and disability by neurosurgeon and nurses were compared in the two groups. Results The time from puncture to successful recanalization in the study group was shorter than that in the control group ([14.30±2.10 vs. 29.55±4.56] min), the difference was statistically significant (P<0.05). There were on statistically significant differences in the time from onset to successful recanalization ([255.00±69.00 vs. 260.00±79.00] min), first-time success recanalization rate (73.33% vs. 63.33%), and proportions of DSA perfusion score (mTICI) 2b/3 (66.67% vs. 53.33%) and mTICI 3 (40.00% vs. 33.33%) between the study group and the control group (P>0.05). There was no significant difference in the total incidence of surgical complications between the study group and the control group (23.33% vs. 26.67%)(P >0.05). There was no significant difference in the NIHSS score before operation between the study group and the control group ([11.26±3.90 vs. 11.28±3.88] points)(P>0.05). The NIHSS scores of the two groups at 24 and 48 h after surgery were lower than those before surgery, and the differences were statistically significant (P<0.05). There were no statistically significant differences in the NIHSS scores at 24 h after surgery ([10.61±3.68 vs. 10.59±3.70] points) and 48 h after surgery ([9.85±3.44 vs. 9.87±3.42]points) between the study group and the control group (P>0.05). At 90 days postoperatively, there were 24 cases with MRS score <2 points and 6 cases with MRS score ≥2 points in the study group, and in the control group, 25 cases with MRS score <2 points, and 5 cases with MRS score ≥2 points, there was no significant difference in MRS score between the two groups (P>0.05). Conclusion After comparison of Sofia distal access catheter aspiration and stent thrombus removal, the safety and curative effect of the two procedures are similar. Both of them can effectively improve the neurological deficit and prognosis quality of patients. But the time from puncture to successful recanalization in the former is shorter, and the operation is simpler. In the clinical practice, the medical staff should make reasonable choices according to the actual situation of the patients.

8 Risk factors analysis of abnormal lung function in children with dust mite allergic rhinitis

Objective To investigate the risk factors of abnormal lung function in children with dust mite allergic rhinitis (AR). Methods A total of 128 children with dust mite AR without asthma who were diagnosed in the Department of Pediatrics in Zhongshan Bo′ai Hospital Affiliated to Southern Medical University from November 2019 to April 2020 were selected as the research objects. Patients were divided into abnormal pulmonary function group (40 cases) and normal group (88 cases) according to the results of pulmonary function. The relation between abnormal lung function and gender, age, total immunoglobulin E (IgE) levels, dust mite specific IgE (sIgE) levels and the number of allergens were analyzed. Results Univariate analysis results showed that there were significant differences in the course of disease, onset time and severity of symptoms between the abnormal pulmonary function group and the normal group (P<0.05). Multivariate analysis showed that the course of disease was 3-5 years (β=1.186, OR=3.273, 95%CI=1.123-9.536), the course of disease was 5 years (β=1.551, OR=4.714, 95%CI=1.374-16.175), and the severity of symptoms was moderate and severe (β=1.531, OR=4.625, 95%CI=1.303-16.414) and persistent onset of symptoms (β=1.019, OR=2.769, 95%CI=1.144-6.701) were the influential factors of abnormal lung function (P<0.05). Conclusion Some children with dust mite AR have abnormal lung function. Children with long course, persistent, moderate-severe AR are higher probability to have abnormal lung function. There are no significant correlation between abnormal lung function and total IgE levels, sIgE levels and the number of positive SPTs.

12 Study on the effect of different doses of Ginkgo biloba extract on monocrotaline-induced pulmonary arterial hypertension

 Objective To study the effect of different doses of Ginkgo biloba extract (GBE) in monocrotaline-induced pulmonary arterial hypertension, and to measure the expression of endothelin-1 (ET-1) and nitric oxide synthase(eNOS) in lung tissue. Methods According to the random number table method, 50 male SD rats were divide into five groups: normal control group (group C), pulmonary arterial hypertension model group (group M), GBE low-dose treatment group(group G1,40 mg/kg),GBE medium-dose treatment group(group G2,60 mg/kg)and the GBE high-dose treatment group (group G3, 80 mg/kg), 10 rats in each group. The right atrial pressure (RAP), right ventricular systolic pressure (RVSP), mean pulmonary artery pressure (mPAP) and right ventricular hypertrophy index (RVHI) in rats were measured. The histological structure of small pulmonary arteries was observed by HE staining, and the percentage of mesothelial thickness to the outer diameter of the vessel (MA%) and ratio of the mesothelial area to the total area of the vessel (MV%) for 50-150 μm diameter pulmonary arteries were calculated. The expression of ET-1 and eNOS in lung tissue of rats was determined by immunohistochemistry. Results Compared with the group M, the RAP, mPAH, RVSP,RVHI, MA% and MV% of the rats in the three treatment groups decreased, and the differences were statistically significant (P<0.05). Among the three treatment groups, RAP, mPAH and MV% in group G3 were statistically significant compared with group G1 (P<0.05). In eNOS, the expression of eNOS in five groups: group C>group G3>group G1>group G2>group M, the expression levels of eNOS in the three treatment groups were higher than those in the group M, and the differences were statistically significant (P<0.05). The expression of eNOS in the G3 group was compared with the group G1 and group G2, and the differences were statistically significant (P<0.05). In ET-1, the expression of ET-1 in five groups: group M>group G1>group G2>group G3>group C, the expression levels of ET-1 in the three treatment groups were lower than those in the group M, and the differences were statistically significant (P<0.05). The expression of ET-1 in the G3 group was compared with the group G1 and group G2, and the differences were statistically significant (P<0.05). Conclusion GBE can delay the formation of pulmonary arterial hypertension, which is related to inhibiting the release of ET-1 and maintaining the synthesis of eNOS. Compared with low and medium doses, high doses of GEB could enhance the delaying effect formation of pulmonary arterial hypertension in a dose-dependent manner.

17 Effect of Ticagrelor combined with percutaneous coronary intervention in the treatment of patients with acute ST segment elevation myocardial infarction

Objective To observe the clinical efficacy and safety of Ticagrelor combined with percutaneous coronary intervention (PCI) in patients with acute ST segment elevation myocardial infarction. Methods A total of 100 patients with acute ST segment elevation myocardial infarction who were treated in Huludao Central Hospital from January to October 2019 were selected and divided into the experimental group and the control group according to the random number table method. The control group was treated with Clopidogrel combined with PCI, and the experimental group was treated with Ticagrelor combined with PCI. The clinical efficacy and safety of treatment were compared between the two groups. Results After treatment, the platelet count and platelet aggregation rate of the experimental group were lower than those of the control group, the differences were statistically significant (P<0.05); the values of endothelial cell specific molecule-1 (ESM-1), interleukin-6 (IL-6) and high sensitivity C-reactive protein (hs-CRP) of the experimental group were lower than those of the control group, the differences were statistically significant (P<0.05). After treatment, the values of creatine kinse isoenzymec (CK-MB) and cardiac troponin I (cTn I) in the experimental group were lower than those in the control group, and the differences were statistically significant (P<0.05); the LVEF score in the experimental group was higher than that in the control group, and the difference was statistically significant (P<0.05); the adverse cardiovascular events in the experimental group during treatment was lower than the control group,the difference was statistically significant (P<0.05). Conclusion Ticagrelor combined with PCI for patients with acute ST segment elevation myocardial infarction has significant effect and high safety, which can be widely used.

21 Effect observation of the alternating treatment of sequential high-flow nasal cannula and non-invasive positive pressure ventilation in patients with chronic obstructive pulmonary disease after invasive mechanical ventilation

Objective To explore the clinical effect of the alternating treatment of sequential high-flow nasal cannula(HFNC)-non-invasive positive pressure ventilation (NIPPV) in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with type 2 respiratory failure after invasive mechanical ventilation. Methods A total of 98 patients with AECOPD with type 2 respiratory failure after invasive mechanical ventilation admitted to the First Affiliated Sanming Hospital of Fujian Medical University from January 2018 to January 2020 were selected as research objects. To take the "pulmonary infection control window" as the switching point, the patients were divided into noninvasive group (n=49) and joint group (n=49) according to the random number table method, non-invasive group was treated with NIPPV , and the joint group was treated with alternating treatment of HFNC-NIPPV. The PaO2 and PaCO2 indexes, viscosity of sputum, degree of comfort, intensive care unit stay time, reintubation rate, incidence of adverse reactions and mortality were compared between the two groups. Results There were no significant differences between the two groups in PaO2 and PaCO2 before treatment (P>0.05). After treatment, the PaO2 index of the joint group was higher than that before treatment and that of the non-invasive group, and the PaCO2 index of the joint group was lower than that before treatment and that of the non-invasive group, the differences were statistically significant (P<0.05). There was no significant difference in the viscosity of sputum before treatment between the two groups (P>0.05). After treatment, the viscosity of sputum of the joint group was higher than that before treatment and that of the non-invasive group, the differences were statistically significant (P<0.05). After treatment, the joint group comfort scores were higher than the noninvasive group, and the differences were statistically significant (P<0.05). After treatment, the intensive care unit stay time, reintubation rate, incidence of adverse reactions and mortality in the joint group were lower than those in the non-invasive group, and the differences were statistically significant (P<0.05). Conclusion For AECOPD patients with type 2 respiratory failure after invasive mechanical ventilation, compared with NIPPV treatment, alternating treatment of HFNC-NIPPV improves the patient′s hypoxia and carbon dioxide retention while improving the degree of comfort, reduces sputum viscosity, incidence of adverse reactions and mortality, shortening the patient′s intensive care unit stay time. The security is high, the clinical curative effect is good, has the medical value, it is worth popularizing vigorously.

25 Research progress in the investigation of inhibitory effect of common Chinese herbal extracts and major components on Cytochrome P450 2C9

Chinese herbs and Western medicine are frequently applied in combination for disease treatment and prevention currently. The complicated components of Chinese Herbs may affect the activity of drug metabolism enzymes,and thus lead to herb-drug interaction (HDI). However, HDI has been neglected. Cytochrome P450 2C9 (CYP2C9), a phase I drug metabolism enzyme, is responsible for the metabolism of more than 15% drugs, including several narrow therapeutic window drugs. Evaluation of the CYP2C9 activity alteration is essential for HDI prediction. The current article reviewed the inhibitory effect on CYP2C9 of the common Chinese herbal extracts and major components, such as Ginkgo Biloba, Ginseng, Danshen, Ginger and Garlic. The aim is to provide theoretical basis for rational use of Chinese herbs and Western medicine in clinic.

32 The Research Progress of microRNA-34a in Cardiovascular Diseases

microRNA (miRNA) is widely existed in a variety of organisms. As a member of miRNA family, miRNA-34a is abnormally expressed in a variety of cardiovascular diseases. Based on the overview of miRNA-34a and related cardiovascular literature, this study found that miRNA-34a is positively correlated with clinical grade of hypertension,involved in regulating the process of atherosclerosis, promoting plaque formation, and increased after ischemia reperfusion injury. The addition of inhibitors can improve ventricular remodeling and delay cardiac hypertrophy. It also participates in the regulation of the pathophysiological process of atrial fibrillation. These results suggest that miRNA-34a is indeed closely related to a variety of common cardiovascular diseases, and can be used as a new target for the diagnosis and treatment of these cardiovascular diseases, providing reference for clinical prediction and diagnosis and treatment.

36 Comparison of the effect of laparoscopic surgery and traditional laparotomy in the treatment of children with complicated appendicitis

Objective To compare the effect of laparoscopic surgery and traditional laparotomy in the treatment of children with complicated appendicitis. Methods From January 2015 to December 2019, 52 cases of children with complicated appendicitis treated in Marina Bay Center Hospital of Dongguan City were selected as the research objects, and they were divided into the minimally invasive group (26 cases) and the laparotomy group (26 cases) by random number table method. The minimally invasive group was treated with laparoscopic surgery, and the laparotomy group was treated with laparotomy surgery. The surgical treatment condition, inflammatory factor levels (C reactive protein [CRP], interleukin-6[IL-6],procalcitonin [PCT]), immune function index such as peripheral blood CD3, CD4 and complications were observed and compared between the two groups. Results The anal exhaust time and hospitalization time in the minimally invasive group were shorter than those in the laparotomy group, and the intraoperative blood loss in the minimally invasive group was less than that in the laparotomy group,the differences were statistically significant (P<0.05).The levels of CRP, IL-6 and PCT of the two groups after operation were higher than those before operation, and the levels of CRP, IL-6 and PCT after operation in the minimally invasive group were lower than those in the laparotomy group, the differences were statistically significant(P<0.05).The levels of CD3 and CD4 of the two groups after operation were lower than those before the operation,and the levels of CD3 and CD4 in the minimally invasive group after operation were higher than those in the laparotomy group, the differences were statistically significant (P<0.05). The total incidence of postoperative complications in the minimally invasive group was lower than that in the laparotomy group,and the difference was statistically significant (P<0.05). Conclusion For children with complicated appendicitis surgery, compared with traditional laparotomy, laparoscopic surgery has less postoperative inflammatory reaction, less impact on immune function,faster recovery and lower incidence of complications.

40 Application of apheresis fresh frozen plasma in preparation of cryoprecipitated coagulation factor

Objective To explore the application effect of apheresis fresh frozen plasma in preparation of cryoprecipitated coagulation factor. Methods A total of 48 bags of plasma collected from Zhaoqing Central Blood Station from April to October 2020 were selected and divided into experimental group (24 bags) and control group (24 bags) according to the random number table method. The experimental group used apheresis fresh frozen plasma to prepare cryoprecipitated coagulation factor, and the control group used fresh frozen plasma prepared from whole blood to prepare cryoprecipitated coagulation factor. The content of factors Ⅷ (FⅧ), the fibrinogen (Fg) content, and the pass rate of cryoprecipitated coagulation factor were compared between the two groups. Results The FⅧcontent in the experimental group was higher than that in the control group, the difference was statistically significant (P<0.05). There was no significant difference in Fg content between the two groups (P>0.05). The pass rate of cryoprecipitated coagulation factor in experimental group was higher than that in control group,the difference was statistically significant(P<0.05). Conclusion The application of apheresis fresh frozen plasma in preparation of cryoprecipitated coagulation factor is valuable, the activity and the quality of coagulation factor is better.

43 Effect of Weisu Granules combined with quadruple therapy in the treatment of Hp positive chronic atrophic gastritis of the spleen and stomach with gas stagnation

Objective To investigate the effect of Weisu Granules combining with quadruple therapy in the treatment of Helicobacter pylori (Hp) positive chronic atrophic gastritis of the spleen and stomach with gas stagnation. Methods A total of 116 cases of Hp positive chronic atrophic gastritis of the spleen and stomach with gas stagnation admitted to Hospital of Traditional Chinese Medicine of Boshan District of Zibo City from October 2017 to April 2020 were selected and divided into the control group(n=58)and the observation group(n=58)according to the random number table method.The control group was treated with quadruple therapy, while the observation group was treated with Weisu Granules on the basis of the control group. The treatment effect, the improvement of traditional Chinese medicine of clinical symptom score and the total incidence of adverse reactions were observed and compared between the two groups. Results After treatment, the total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). The traditional Chinese medicine of symptom score of the observation group was lower than that of the control group, the difference was statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Weisu Granules combined with quadruple therapy in the treatment of Hp positive chronic atrophic gastritis of the spleen and stomach with gas stagnation has better effect and high safety, which is worthy of popularization and application.

46 Observation on the effect of high volume hem of iltration in the treatment of severe septic shock and the improvement of organ function

Objective To explore the effect of high volume hem of iltration in the treatment of severe septic shock and the improvement of organ function. Methods A total of 168 patients with severe septic shock admitted to Mindong Hospital of Ningde City were selected from January 2017 to October 2020 as research objects and divided into group A,group B and group C, with 56 cases in each group. Group A was treated with high volume hem of iltration under 6 hours of fluid resuscitation, group B was treated with high volume hem of iltration after 6 hours of fluid resuscitation,group C was treated with routine fluid resuscitation. The organ function, multiple organ dysfunction syndrome (MODS)score, acute physiology and chronic health evaluation (A-PACHEⅡ) score, serum levels of inflammatory factors before and after treatment and ICU hospitalization days, total hospitalization days, and 28 days mortality were compared among the three groups. Results After treatment, the alanine aminotransferase (ALT) and blood urea nitrogen (BUN) of group A were lower than those of group B and group C, and the arterial oxygenation index of group A was higher than that of group B and group C, the differences were statistically significant (P<0.05). After treatment, the MODS score and A-PACHEⅡscore of group A were lower than those of group B and group C, the differences were statistically significant (P<0.05). The levels of tumor necrosis factor-α (TNF-α) and C reactive protein (CRP) of group A were lower than those of group B and group C, the differences were statistically significant (P<0.05). ICU hospitalization days and total hospitalization days of group A were shorter than those in group B and group C, and the differences were statistically significant (P<0.05). There was no statistically significant difference of the 28 days mortality between the three groups(P>0.05). Conclusion Early high-volume hem of iltration to treat severe septic shock could promote the improvement of organ function, reduce the body′s inflammatory response, and promote the patient′s speedy recovery.

50 Clinical effect of panretinal photocoagulation combined with Ranibizumab treating severe non-proliferative diabetic retinopathy

Objective To explore the clinical effect of panretinal photocoagulation combined with Ranibizumab treating severe non-proliferative diabetic retinopathy (DR). Methods A total of 96 patients with severe non-proliferative DR admitted to Shangrao Aier Eye Hospital from August 2019 to August 2020 were selected as the research objects and divided to the control group (48 cases) and the study group (48 cases) according to the random ballot method. The control group was treated with panretinal photocoagulation, and the study group was treated with panretinal photocoagulation combined with Ranibizumab. The clinical efficacy, best corrected visual acuity (BCVA), intraocular pressure (IOP),central macular thickness (CMT) and adverse reactions were compared between the two groups. Results The total effective rate in the study group was higher than that in the control group, the difference was statistically significant (P<0.05).There was no significant difference in BCVA between the two groups before treatment (P>0.05). After treatment, the BCVA of the study group lower than that of the control group, the difference was statistically significant (P<0.05). There were no significant differences in IOP and CMT between the two groups before treatment (P>0.05). After treatment, the IOP and CMT of the study group were lower than those of the control group, the differences were statistically significant(P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Panretinal photocoagulation combined with Ranibizumab could effectively improve the vision of patients with severe non-proliferative DR, reduce IOP and CMT. It has certain clinical effect.

53 Effect of Atropine combined with hemoperfusion in the treatment of severe organophosphorus pesticide poisoning

Objective To investigate the clinical effect of Atropine combined with hemoperfusion in the treatment of severe organophosphorus pesticide poisoning. Methods A total of 90 patients with severe organophosphorus poisoning admitted to the Emergency Internal Medicine Department of Quanzhou Guangqian Hospital from February 2018 to June 2020 were selected and randomly divided into the anticholinergic group (44 cases) and the hemoperfusion combined with anticholinergic group (46 cases) by drawing lots. The anticholinergic group was treated with Atropine on the basis of routine treatment, and the hemoperfusion combined with anticholinergic group was given hemoperfusion therapy on the basis of the anticholinergic group. The curative effect of the two groups were compared, and the changes of blood gas analysis indexes and rehabilitation of the patients were tracked. Results After 3 days of treatment, pH value (7.48±0.20)and arterial partial pressure of oxygen(PaO2)([71.97±5.68]mmHg)in the hemoperfusion combined with anticholinergic group were higher than those in the anticholinergic group [7.20±0.14], [61.25±6.87] mmHg), the differences were statistically significant (t=7.662, 8.083; P<0.05). The arterial partial pressure of carbon dioxide (PaCO2) in the hemoperfusion combined with anticholinergic group ([36.49±5.13] mmHg) was lower than that of the anticholinergic group ([44.72±4.19] mmHg), and the difference was statistically significant (t=8.314, P<0.05). The total effective rate in the hemoperfusion combined with anticholinergic group (91.30%) was higher than that in the anticholinergic group (72.73%), and the total incidence of complications in the hemoperfusion combined with anticholinergic group (13.04%) was lower than that in the anticholinergic group (31.82%), and the differences were statistically significant (χ2=5.310, 4.586; P<0.05).Conclusion Atropine combined with hemoperfusion in the treatment of severe organophosphorus pesticide poisoning can quickly improve the liver function of patients, reduce the risk of complications, improve the severity of the disease,and promote the patients to recover and leave the hospital as soon as possible. It is of high value in the treatment of severe organophosphorus pesticide poisoning.

57 Efficacy and safety of Cyclosporin A combined with γ Globulin in the treatment of severe aplastic anemia

Objective To analyze the efficacy and safety of cyclosporin A combined with γ globulin in the treatment of severe aplastic anemia. Methods Sixty-two patients with severe aplastic anemia admitted to the Department of Hematology, Liaoning Provincial People′s Hospital from January 2018 to December 2019 were selected as the research subjects. They were divided into control group (n=31) and observation group (n=31) by odd/even number method. Patients in the control group were treated with Cyclosporin A, and patients in the observation group were treated with Cyclosporin A combined with γ Globulin. The clinical efficacy, safety, biochemical indexes, helper/inducible T lymphocytes (CD4), helper/inducible T suppressor cells (CD8) and ratio of T helper cells to T suppressor cells (CD4/CD8) between the two groups were compared. Results The total effective rate of observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). The difference of the treatment safety between the two groups was not statistically significant (P>0.05). The levels of the hemoglobin (Hb), red blood cell (RBC), peripheral reticulocyte (RC), white blood cell (WBC), absolute neutrophil count (ANC), platelet (PLT) in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).The levels of CD4 and CD4/CD8 in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The level of CD8 in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion For patients with severe aplastic anemia, combination of Cyclosporine A and γ Globulin can effectively improve the spinal cord′s hematopoietic function and greatly alleviate anemia in a favorable clinical effect. The safety is high during treatment, which is worthy of clinical promotion and application.

60 Clinical effect of Maqin Xiaoke Granules in the treatment of acute bronchitis

Objective To explore the clinical effects of Maqin Xiaoke Granules in the treatment of acute bronchitis.Methods A total of 114 patients with acute bronchitis admitted to Liaocheng City Central Hospital from May 2018 to January 2020 were selected as research subjects. According to equal single-blind method, patients were divided into routine group (57 cases) and study group (57 cases). The routine group was treated with glucocorticoid, and the study group was treated with glucocorticoid combined with Maqin Xiaoke Granules. The traditional Chinese medicine symptom scores, lung function, total effective rate of treatment and total incidence of adverse reactions were compared between the two groups. Results There were no significant differences in the total score of traditional Chinese medicine symptom scores and the indexes of lung function between the two groups before medication (P>0.05). After medication,the total score of traditional Chinese medicine symptom scores of the two groups was lower than that before medication,and the total score of traditional Chinese medicine symptom of the study group was lower than that of the routine group,the difference was statistically significant (P<0.05). The forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) of the two groups were higher than those before medication, the FVC,FEV1 and PEF of the study group were higher than those of the routine group, the differences were statistically significant (P<0.05). The total efficiency of the study group was higher than that of the routine group, the difference was statistically significant (P<0.05). The total incidence of adverse reactions was compared between the two groups with no statistical significance (P>0.05). Conclusion The application of Maqin Xiaoke Granules in the treatment of acute bronchitis can not only guarantee the safety of treatment, but also improve the therapeutic effect and improve the lung function of patients. The clinical treatment value is remarkable.

64 Clinical effect of Broussonetia Papyrifolia herb bath combined with Capotriol Betamethasone Ointment in the treatment of psoriasis vulgaris

Objective To investigate the clinical effect of Broussonetia Papyrifolia herb bath combined with Capotriol Betamethasone Ointment in the treatment of psoriasis vulgaris. Methods A total of 69 patients with psoriasis vulgaris admitted to Air Force Medical Center of PLA from March 2020 to February 2021 were selected as the research objects.And the patients were randomly divided into the treatment group (32 cases) and the control group (37 cases) according to drawing lots method. The observation period was 4 weeks. The treatment group received Broussonetia Papyrifolia herb bath combined with Capotriol Betamethasone Ointment in the first two weeks, and only topical Capotriol Betamethasone Ointment in the following two weeks. The control group was only given topical Capotriol Betamethasone Ointment for four weeks. The pruritus score, psoriasis area and severity index (PASI) score and total effective rate were compared after 2 and 4 weeks of treatment between the two groups.Results Two weeks after the treatment,the pruritus score and PASI score of the treatment group were lower than those of the control group,and total effective rate of the treatment group was higher than that of the control group, the differences were statistically significant (P<0.05). Four weeks after the treatment, the pruritus score of the treatment group was lower than that of the control group, the difference was statistically significant(P<0.05). Four weeks after the treatment, there were no statistically significant differences in PASI score and total effective rate between the two groups (P>0.05). There were no serious adverse reactions in two groups. Conclusion Broussonetia Papyrifolia herb bath combined with Capotriol Betamethasone Ointment in treating psoriasis vulgaris has remarkable clinical effect, and it is safe and reliable.

69 Influence of Levosimendan on left ventricular function and hemodynamics in elderly patients with chronic heart failure

Objective To investigate the influence of Levosimendan on left ventricular function and hemodynamics in elderly patients with chronic heart failure (CHF).Methods A total of 60 elderly CHF patients admitted to Fushun Petrochemical General Hospital from January 2017 to January 2020 were selected as the research subjects. They were divided into the control group (n=30) and the observation group (n=30) according to the random number table method. Patients in the control group were provided with conventional treatment and Tolvaptan, and patients in the observation group were added with Levosimendan Injection on the basis of the control group. The left ventricular functions including left ventricular ejection fraction (LVEF) and left ventricular end diastolic diameter (LVEDD), B-type natriuretic peptide (BNP) and hemodynamic related indexes, such as pulmonary arterial mean pressure (PAMP), pulmonary capillary wedge pressure (PCWP) and systemic vascular resistance (SVR), were compared between the two groups after 7 days of treatment. Results In the observation group, the levels of LVEDD, BNP, PAMP, PCWP and SVR after 7 days of treatment were lower than those in the control group, the LVEF level was higher than that in the control group, with statistical significances (P<0.05). Conclusion Levosimendan therapy for elderly patients with CHF can effectively improve left ventricular function and hemodynamics, and it is worthy of promotion.

72 Application of PDCA circulation in shortening the door-to-balloon time in the patients with acute ST-segment elevation myocardial infarction

Objective To analyze the application situation of PDCA circulation in shortening the door-to-balloon time(D-to-B time) in the patients with acute ST-segment elevation myocardial infarction (STEMI). Methods The monthly D-to-B time and the compliance rate ≤90 minutes were retrospectively analyzed about 66 patients with STEMI who underwent emergency percutaneous coronary intervention (PCI) in the Department of Cardiology of Tongren City People′s Hospital from January to June in 2019. After the implementation of PDCA circulation management, the monthly time of D-to-B time and the compliance rate ≤90 minutes of 79 patients with STEMI who underwent emergency PCI from January to June in 2020 were analyzed, and the situation before and after the implementation was compared. Results After the implementation of PDCA cycle, the monthly D-to-B time was shorter than that before the implementation,and the compliance rate of the monthly D-to-B time ≤90 minutes after the implementation was higher than that before the implementation (P<0.05).Conclusion The application of PDCA circulation has a significant effect on shortening D-to-B time in patients with STEMI, and can further optimize the diagnosis and treatment process of patients with chest pain.

76 Stroke risk factor screening of stroke incidence of high-risk groups and the impact of onset-to-door time

 Objective To investigate the effect of screening and intervention of stroke risk factors on the incidence of stroke in high-risk population and onset-to-door time (ODT) through cross-sectional investigation. Methods From January 2019 to November 2019, the population sampling method was adopted in Ganzhou Municipal Hospital, and the 2 main risk factors and 8 general risk factors for stroke were applied in 12 natural villages around Zhanggong District of Ganzhou city to detect the high-risk population of stroke. The high-risk groups were further screened: blood pressure,blood glucose, blood lipid and carotid plaque were examined, and follow-up of 1, 3, 6, 9 and 12 months was recorded.The incidence and ODT of stroke high-risk population at 1, 3, 6, 9 and 12 months after intervention were observed. Results The detection rate of high-risk group was higher in male than female, and higher in the elderly than in the middle-aged, with statistical significance (P<0.05). Screening of high-risk population for stroke, detection of various risk factors and the cumulative incidence of stroke in high-risk population at 1, 3, 6, 9 and 12 months after intervention were 1.71%, 2.73%, 3.41%, 4.09% and 5.12%, respectively. The ODT of patients at high risk of stroke after screening,follow-up and intervention was shorter than that of patients admitted to hospital. Conclusion The incidence of stroke can be reduced by conducting repeated health education on the knowledge related to stroke among the high-risk population screened early in various forms. For the high-risk group with stroke related knowledge and health education,stroke can be correctly identified early, seek medical treatment as soon as possible, shorten the ODT time, so as to effectively reduce the incidence and disability rate of stroke patients, so that patients can return to their families and society.

79 Influencing factors of clinical prognosis in patients with acute ischemic stroke treated with intravenous thrombolysis

Objective To investigate the clinical prognostic factors of patients with acute ischemic stroke treated with intravenous thrombolysis. Methods A total of 260 patients with acute ischemic stroke admitted to People′s Hospital of Nanhai District from October 2017 to October 2019 were retrospectively selected as the research subjects. According to the improved clinical outcome of thrombolysis, the patients were divided into good prognosis group (130 cases) and poor prognosis group (130 cases), and both groups were treated with Alteplase. The nerve defect scores of the two groups were compared, and the factors affecting the prognosis of acute ischemic stroke were analyzed. Results After 2 d,1 week and 1 month of treatment, neurological deficit scores in the poor prognosis group were higher than those in the good prognosis group, the difference was statistically significant (P<0.05). Univariate analysis results indicated that age,weight, history of hypertension, length of hospital stay, history of diabetes mellitus, history of atrial fibrillation and systolic blood pressure before thrombolysis were compared between the two groups, the differences were statistically significant (P<0.05). Multivariate analysis indicated that age≥60 years old (β=1.457,OR=4.257,95%CI=2.365-6.954), body weight ≥65 kg (β=0.684,OR=5.236,95%CI=1.028-5.687), length of hospital stay≥10 d (β=0.747,OR=5.326,95%CI=4.395-7.893), history of hypertension (β=1.442,OR=4.091,95%CI=3.235-4.573), history of diabetes (β=1.573,OR=6.413,95%CI=5.691-8.461), history of atrial fibrillation (β=1.131,OR=4.574,95%CI=1.496-9.481) and systolic blood pressure before thrombolysis ≥140 mmHg (β=1.215,OR=3.591,95%CI=2.491-6.324) were risk factors for poor prognosis in patients with acute ischemic stroke (P<0.05). Conclusion The clinical effect of intravenous thrombolysis in the treatment of acute ischemic stroke is significant, which can reduce the nerve defect in patients. The risk factors for poor prognosis in patients with acute ischemic stroke need active intervention to improve the quality of life of patients

82 Study on the correlation between homocysteine level and bone metabolism markers in patients with fist-episode schizophrenia

 Objective To investigate the correlation between homocysteine (HCY) level and bone metabolic markers in patients with first-episode schizophrenia.Methods A total of 69 patients with first-episode schizophrenia who were admitted to Zhongshan Third People′s Hospital from May 2016 to April 2020 were selected as the research objects according to the International Classification of Diseases 10th edition Mental and Behavioral Disorders. Patients were divided into high HCY group (HCY>15.4 μmol/L, with 31 cases) and normal HCY group (HCY≤15.4 μmol/L, with 38 cases) according to the HCY level of patients.General data,HCY,bone metabolism markers(osteocalcin[OCN], β-collagen breakdown product [β-CTX], 25-hydroxy vitamin D [VitD-T], procollagen type ⅠN propeptide [PⅠNP] and parathyroid hormone [PTH]) and cerebrovascular-related indexes (triacylglycerol [TG], total cholesterol [TC], high-sensitivity C-reactive protein [hs-CRP], high-density lipoprotein [HDL], low density lipoprotein [LDL], creatine kinase [CK], creatine kinase isoenzyme [CK-MB]) in the two groups were collected, and their differences and correlation were analyzed. Results The levels of CK, CK-MB and PⅠNP in the high HCY group were higher than those in the normal HCY group, and the differences were statistically significant (P<0.05). There were no significant differences in the levels of hs-CRP, TG, TC,HDL, OCN, β-CTX, VitD-T, PTH and LDL between the two groups (P>0.05). Correlation analysis showed that HCY level was positively correlated with CK, CK-MB and HDL levels in schizophrenia patients (P<0.05). HCY was positively correlated with PⅠNP (P<0.05). Conclusion HCY level has a certain correlation with bone metabolism markers in patients with first-episode schizophrenia, and also has a certain correlation with cerebrovascular-related indexes.

86 Application comparison of selective arterial thrombolysis combined with arterial stent angioplasty and intravenous thrombolysis in the treatment of patients with ultra-early acute cerebral infarction

Objective To explore the clinical application effect of selective arterial thrombolysis combined with arterial stent angioplasty and intravenous thrombolysis in the treatment of patients with ultra-early acute cerebral infarction.Methods A total of 60 patients with early acute cerebral infarction who admitted to Ji′an Intergration of Traditional Chinese and Western Medicine Hospital from October 2019 to August 2020 were selected as the research objective.They were divided into the experimental group and the control group using the data parity arrangement method, with 30 cases in each group. Patients in the control group were treated with intravenous thrombolysis, and patients in the experimental group were treated with selective arterial thrombolysis and arterial stent angioplasty. Coagulation indexes,total response rate and National Institutes of Health stroke scale (NIHSS) scores were compared between the two groups.Results After 6 h, 3, 7 d treatment, the thrombin time (TT) and prothrombin time (PT) in experimental group were longer than those in control group, and the differences were statistically significant (P<0.05), after 6 h, 3, 7 d, the fibrinogen (FIB) in the experimental group were lower than those of the control group, and the differences were statistically significant (P<0.05). The total effective rate and NIHSS score after 7 d of treatment in experimental group were higher than those in control group, and the differences were statistically significant (P<0.05). Conclusion The implementation of selective arterial thrombolysis and arterial stent angioplasty in the treatment of patients with ultra-early acute cerebral infarction is beneficial to the optimization of the treatment effect, and the treatment effectiveness of the patient is improved, which has clinical promotion value.

89 Effect of Levodopa and Benserazide Hydrochloride Tablets combined with Pramipexole in the treatment of Parkinson′s disease

Objective To observe the effect of Levodopa and Benserazide Hydrochloride Tablets combined with Pramipexole in the treatment of Parkinson′s disease. Methods Ninety-four patients with Parkinson′s disease admitted to the Internal Medicine Department of Dianbai District People′s Hospital of Maoming City, Guangdong Province from October 2018 to June 2020 were selected as the research subjects. They were divided into a control group (n=47) and an observation group (n=47)using the odd/even number distribution method.Patients in the control group were treated with Levodopa and Benserazide Hydrochloride Tablets.Patients in the observation group were provided with Levodopa and Benserazide Hydrochloride Tablets combined with Pramipexole. The Webster comprehensive rating scale (Webster) score and the Parkinson′s disease rating scale Ⅲpart motor examination evaluation (UPDRS Ⅲ) score before and after medication were compared between the two groups, and the total effective rate of the two groups was compared. Results Webster and UPDRS Ⅲscores before medication in the two groups were compared, and the differences were not statistically significant (P>0.05). After two months of medication, the Webster and UPDRS Ⅲscores of the two groups were lower than those before medication, and the differences were statistically significant (P<0.05). The Webster and UPDRS Ⅲscores of the observation group after two months of medication were lower than those of the control group, the differences were statistically significant (P<0.05). The total effective rate of observation group (89.36%) was higher than that of control group (61.70%), and the difference was statistically significant (P<0.05). Conclusion The effectiveness of the treatment of patients with Parkinson′s disease is significantly improved in the combination of Levodopa and Benserazide Hydrochloride Tablets and Pramipexole compared with Levodopa and Benserazide Hydrochloride Tablets alone. It can effectively improve the clinical symptoms of patients and control the development of their disease. It has high medicinal value and is worthy of clinical application.

93 Application study of ultrasound-assisted technique in craniotomy and clipping of distal intracranial aneurysm

Objective To explore the application effect of ultrasound-assisted technology in the treatment of intracranial distal aneurysms (PIA) with craniotomy and clipping. Methods A total of 100 PIA patients admitted to Jiangmen Central Hospital of Guangdong Province from September 2017 to July 2020 were selected for retrospective analysis. Accorded to whether ultrasound assisted technology was carried out, they were divided into control group (50 cases) and observation group (50 cases), those who did not perform the ultrasound-assisted technique for craniotomy and clipping were the control group, and those who performed the ultrasound-assisted technique for the craniotomy and clipping were the observation group. The effect of craniotomy and clipping, operation time, hospital stay and postoperative complications were compared between the two groups. Follow-up 2 months, the Glasgow Outcome Score (GOS) of the two groups was evaluated at discharge, 1 month after discharge, and 2 months after discharge. Results The complete clipping rate of the observation group (100.00%) was higher than that of the control group (90.00%), and the difference was statistically significant (P<0.05); the operation time and hospital stay of the observation group were shorter than those of the control group, and the differences were statistically significant (P<0.05); the total incidence of complications in the observation group (4.00%) was lower than that of the control group (20.00%), the difference was statistically significant(P<0.05); the observation group was discharged from the hospital, 1 month after discharge, and 2 monthafter discharge,the GOS scores were higher than those of the control group, and the differences were statistically significant (P<0.05).Conclusion The application of ultrasound-assisted technology in craniotomy and clipping surgery for PIA can improve the clipping effect, shorten the operation time and hospital stay, reduce postoperative complications, and improve the prognosis.

96 Analysis of the effect of keyhole drainage and traditional craniotomy in the treatment of epidural hematoma after brain trauma

Objective To compare and analyze the clinical effect of minimally invasive keyhole drainage and craniotomy for epidural hematoma after traumatic brain injury. Methods A total of 60 cases of traumatic epidural hematoma patients in Anshan Central Hospital from January to December 2019 were selected as the research object, according to the order of admission, they were divided into observation group (30 cases) and control group (30 cases) using alternate grouping method. The control group was treated with craniotomy, and the observation group was treated with minimally invasive keyhole drainage. The operation time, hospital stay, intraoperative blood loss, incidence of craniocerebral defects, incidence of rebleeding, incidence of infection, and medical treatment were compared between the two groups.Results The amount of bleeding during surgery in the observation group was less than that in the control group, and the operation time and hospital stay were shorter than those in the control group, the hospitalization expense of the observation group was lower than that of the control group, and the differences were statistically significant (P<0.05); the incidence rates of craniocerebral defects, rebleeding, and infection in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Minimally invasive keyhole drainage is used in the treatment of patients with brain injury and extradural hematoma, the treatment effect is not only the same as that of craniotomy, but also can reduce the traumatic surface of patients, shorten the recovery time of patients, reduce the incidence of postoperative complications, and reduce the economic burden of patients. Therefore, the minimally invasive keyhole drainage is of great significance in clinical promotion and it is worth promoting and applying.

99 Clinical effect of Olanzapine on sleep disorder in patients with depression

Objective To explore the effect of Olanzapine on sleep disorder in patients with depressionn. Methods A total of 102 cases of patients with depression and sleep disorders and who received treatment in Ganzhou People′s Hospital from February 2018 to November 2020 were retrospectively collected and divided into the observation group and the control group according to the different treatment methods, with 51 cases in each group. The observation group were given Olanzapine drug treatment, the control group were given conservative psychological treatment. The improvement of clinical symptoms and sleep before and after treatment in the two groups were observed. Results Before treatment,there was no significant difference in sleep status between the two groups (P>0.05). After treatment, the light sleep time, deep sleep time and total sleep time of the observation group were shorter than those of the control group, and the differences were statistically significant (P<0.05). The total effective rate of observation group was higher than control group after treatment, and the difference was statistically significant (P<0.05). Conclusion The Olanzapine treatment for depression patients with sleep disorders can improve the sleep condition of patients and ensure their sleep quality. After drug treatment, the clinical symptoms are also relieved, and the treatment effect is better, thus it is recommended to be popularized in clinical practice.

102 Effect of neuroendoscopic microtrauma on hematoma clearance rate and neurological function in patients with hypertensive cerebral hemorrhage

Objective To explore the effect of neuroendoscopic microtrauma (NM) on hematoma clearance rate (HCR)and neurological function (NF) in patients with hypertensive cerebral hemorrhage (HCH). Methods A total of 40 patients with HCH admitted to Wafangdian Central Hospital from March 2018 to February 2020 were retrospectively collected and divided into the control group (n=17) and the observation group (n=23) according to different surgical treatments. The control group were treated with craniotomy minimally invasive hematoma removal and the observation group were treated with NM. HCR, stress response, score of NF, and the incidence of complications were compared between the two groups. Results HCR in the observation group was higher than in the control group, and the total incidence of complications was lower than that of the control group, the differences were statistically significant (P<0.05). There were no statistical significance in the preoperative NF score or oxidative stress between the two groups (P>0.05). The levels of endothelin (ET) and cortisol (Cor) 24 hours after operation in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). Six months after operation, the Chinese stroke scale(CSS) score of the observation group was lower than that of the control group, the difference was statistically significant(P<0.05). Objective To investigate the diagnostic value and clinical significance of detecting serum M-type phosphatase A2 receptor (PLA2R) combined with renal tissue immunoglobulin 4 (IgG 4) in idiopathic membranous nephropathy (IMN) in adults. Methods The clinical data of 70 patients with membranous nephropathy admitted to Xinyu People′s Hospital from April 2017 to April 2019 were retrospectively collected, and divided into IMN group (30 cases) and secondary membranous nephropathy group (40 cases) according to the etiology. All patients received serum PLA2R and renal tissue IgG 4 detection, and the diagnostic value of serum PLA2R combined with renal tissue IgG 4 detection for IMN was analyzed. Results The level of serum PLA2R in IMN group was higher than that in SMN group, and the difference was statistically significant (P<0.05). The ROC curve was drawn with serum PLA2R level as the test variable and the incidence of IMN disease as the dependent variable (1=IMN, 0=SMN). The diagnostic threshold of IMN was 25.641 RU/mL of serum PLA2R, when serum PLA2R>25.641 RU/mL was identified as IMN. At this time, the diagnostic AUC was 0.919 (P<0.05). The diagnostic accuracy of serum PLA2R and renal tissue IgG 4 combined detection for IMN was 93.33%, which was higher than that of serum PLA2R and renal tissue IgG 4 alone detection (81.43%,85.71%), the differences were statistically significant (P<0.05). The serum levels of PLA2R and renal tissue IgG 4 alone in detecting IMN were in good consistency with renal biopsy (Kappa=0.622, 0.706). Combined detection of IMN and renal biopsy showed great consistency (Kappa=0.912). Conclusion The detection of serum PLA2R combined with renal tissue IgG 4 has a high diagnostic value for IMN disease. Targeted preventive measures can be made in time according to the detection results of serum PLA2R combined with renal tissue IgG 4 in clinical practice.

108 Clinical study of single channel percutaneous nephrolithoscope combined with flexible ureteroscope in the treatment of complicated renal calculi

Objective To investigate the clinical value of single channel percutaneous nephrolithotomy (PCNL) combined with flexible ureteroscopy (RIRS) in the treatment of complex renal calculi. Methods A total of 120 patients with complex renal calculi admitted to Yichun People′s Hospital from January 2019 to May 2020 were selected as the research subjects. According to random number table method, they were divided into combined group, single-channel PCNL group and multi-channel PCNL group, with 40 cases in each group. The combined group was treated with single-channel PCNL combined with RIRS, the single-channel PCNL group was treated with single-channel PCNL, and the multi-channel PCNL group was treated with multi-channel PCNL. Clinical indicators, stone clearance rate and complications were compared among the three groups. Results The operation time of the combined group was longer than the single-channel PCNL group and the multi-channel PCNL group; the intraoperative blood loss was less than the multi-channel PCNL group, the hospital stay was shorter than the single-channel PCNL group and the multi-channel PCNL group, the differences were statistically significant (P<0.05). The stone clearance rate of the combined group was higher than that of the single-channel PCNL group, and the difference was statistically significant (P<0.05), while there was no statistically significant difference between the combined group and the multi-channel PCNL group (P>0.05). There was no statistical significance in the total incidence of complications among the three groups (P>0.05).Conclusion Single channel PCNL combined with RIRS is safe and feasible in the treatment of patients with complex renal stones, which has the advantages of high stone clearance rate, small injury and few complications, and can promote the early postoperative recovery of patients. Therefore, it can be used as the preferred method for the treatment of complex kidney stones in clinical promotion.

112 Mid-term effect of color ultrasound-guided percutaneous transluminal angioplasty in the treatment of arteriovenous fistula stenosis

Objective To investigate the mid-term effect of color ultrasound-guided percutaneous transluminal angioplasty (PTA) in the treatment of arteriovenous fistula (AVF) stenosis. Methods A total of 60 cases of patients with AVF stenosis treated in Shangrao People′s Hospital of Jiangxi Province from January 2017 to June 2018 were selected as the research objects, and they were divided into two groups by the random number table method, with 30 cases in each group. The observation group was treated with PTA, and the control group was treated with percutaneous thrombectomy.The clinical indexes before and after operation of the two groups were recorded, and the occurrence of adverse events and the patency of internal fistula at 18 months after operation were counted. Results The internal diameter of the internal fistula stenosis in both groups after operation was larger than that before operation, the natural flow of the internal fistula, and the blood flow of dialysis were all higher than those before operation, and those in the observation group were higher than those in the control group, with statistical significant differences (P<0.05). The total incidence of postoperative adverse events in the observation group was higher than that in the control group,the patency rate of internal fistula at 18 months after operation was higher than that in the control group, with statistical significant differences (P<0.05).Conclusion PTA in treatment of patients with AVF stenosis can improve the clinical indexes, which has certain safety.

115 Effect of Metformin combined with Glimepiride or Canagliflozin on pancreatic islet function, serum FFA, Asprosin levels in elderly patients with diabetes

 Objective To explore the effect of Metformin combined with Glimepiride or Canagliflozin on pancreatic islet function, serum free fatty acids (FFA), Asprosin levels in elderly patients with diabetes. Methods A total of 108 elderly diabetic patients admitted to the Health Management Center, the Second People′s Hospital of Foshan from June 2018 to June 2020 were selected as the research objects and divided into the control group (54 cases) and the observation group (54 cases) according to the random number table method. The control group was treated with Metformin, while the observation group was treated with Metformin combined with Glimepiride or Canagliflozin. Blood glucose (fasting plasma glucose [FPG], 2 h postprandial blood glucose [2 h PBG], glycosylated hemoglobin [HbA1c]), pancreatic islet function indicators (homeostatic model assessment of β-cell function [HOMA-β], homeostasis model assessment of insulin resistance [HOMA-IR]), serum FFA and Asprosin levels, lipid metabolism indicators(total cholesterol [TC],triglycerides [TG], low-density lipoprotein cholesterol [LDL-C], high-density lipoprotein cholesterol [HDL-C]), oxidative stress indicators (malondialdehyde [MDA], superoxide dismutase [SOD]), and the total incidence of adverse reactions were compared between the two groups. Results After treatment, the levels of FPG, 2 h PBG, HbA1c, TC, TG,LDL-C, HOMA-IR, FFA, Asprosin, MDA, and SOD of the observation group were lower than those of the control group,while the levels of HDL-β and HOMA-IR of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). There was no significant difference in the total incidences of adverse reactions between the two groups (P>0.05). Conclusion Metformin combined with Glimepiride or Canagliflozin could control blood glucose, improve pancreatic islet function and lipid metabolism, and lower serum FFA and Asprosin levels in elderly diabetic patients.

119 Effect of small incision thyroidectomy in the treatment of thyroid nodules and analysis of intraoperative bleeding

 Objective To explore the effect of small incision thyroidectomy for the treatment of thyroid nodules and analyse intraoperative bleeding. Methods A total of 180 patients with with thyroid nodules admitted to Panjin Central Hospital from May 2019 to May 2020 were selected as the research objects and divided into the control group (90 cases) and the study group (90 cases) according to the alternate grouping method. The control group used traditional surgery, and the observation group used small incision resection. Total clinical efficacy, intraoperative bleeding, postoperative scar diameter, inflammatory index levels (tumor necrosis factor-α [TNF-α], interleukin-6 [IL-6]), reflux symptoms, voice disturbance index, and beauty satisfaction were compared between the two groups. Results There was no significant difference in total clinical efficiency between the two groups before treatment(P>0.05).The intraoperative bleeding volume in the observation group was lower than that in the control group, and the postoperative scar diameter of the observation group was smaller than that of the control group, the differences were statistically significant (P<0.05). After treatment, the TNF-α and IL-6 levels of the observation group were higher than those of the control group, the differences were statistically significant(P<0.05).The scores of reflux symptom index and voice disturbance index in the observation group were lower than those in the control group, and the beauty satisfaction in the observation group was higher than in the control group, the differences were statistically significant (P<0.05). Conclusion The treatment of thyroid nodule patients with small incision could achieve the ideal therapeutic effect. It is helpful to reduce intraoperative bleeding and stress response, reduce postoperative complications and improve aesthetic degree.

122 Clinical application of laryngeal mask ventilation general anesthesia in thoracoscopic surgery

Objective To explore the clinical application of laryngeal mask ventilation general anesthesia in thoracoscopic surgery. Methods A total of 100 patients with thoracoscopic surgery who admitted to the Department of Anesthesiology, Thoracic Hospital of Jiangxi Province from January 2017 to February 2019 were selected as the research objects and divided into the control group and the observation group by lotting, with 50 cases in each group. The control group received general anesthesia with double lumen tube ventilation, and the observation group received general anesthesia with laryngeal mask spontaneous ventilation. The waking time, spontaneous recovery breathing time, time to get out of bed and the total incidence of complications were compared between the two groups. Results The postoperative awakening time, spontaneous recovery breathing time and getting out of bed time in the observation group were shorter than those in the control group, and the differences were statistically significant (P<0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The application of laryngeal mask airway general anesthesia in thoracoscopic surgery can shorten the time of waking up and postoperative getting out of bed, which is worthy of clinical promotion.

125 Perioperative effect of ultrasonic iliohypoabdominal and ilioinguinal nerve block on pelvic fracture patients

Objective To investigate the perioperative effects of ultrasound iliac inferior abdominal nerve (IHN) and iliac ventral sulcus nerve (IIN) block on pelvic fracture surgery. Methods A total of 90 patients with pelvic fractures who underwent internal fixation surgery in the First People′s Hospital of Shaoguan City from January 2018 to December 2020 were selected as the research subjects. According to random number table method, they were divided into the ultrasound group and the control group, with 45 cases in each group. Patients in the ultrasound group received ultrasound-guided IHN and IIN block combined general anesthesia, while patients in the control group received routine general anesthesia. Hemodynamics, intraoperative anesthesia dosage, postoperative pain in the operative area and the incidence of adverse reactions were compared between the two groups at different time points. Results There were no statistically significant differences in mean arterial pressure (MAP) and heart rate (HR) in the ultrasound group at 5 min before surgery (T0), the beginning of surgery (T1), 30 min (T2), 60 min (T3), at the end of surgery (T4), and 15 min after extubation under general anesthesia (T5) (P>0.05). MAP and HR of the control group at T1 and T2 were lower than T0,the differences were statistically significant (P<0.05). There were no statistically significant differences in MAP and HR at T3, T4 and T5 between the two groups (P>0.05). The dosage of Propofol and Remifentanil in the ultrasound group were less than those in the control group, and the differences were statistically significant (P<0.05). There were no statistically significant differences in the dosage of Sufentanil and Rocuronium between the two groups (P>0.05). The postoperative pain scores in the ultrasound group at 2, 12, 24 and 48 h were lower than those in the control group, and the total incidence of postoperative adverse reactions was lower than that in the control group, and the differences were statistically significant (P<0.05). Conclusion Ultrasoundguided IHN and IIN block combined with general anesthesia can effectively maintain hemodynamic stability during pelvic fracture surgery, reducing the use of narcotic drugs, to relieve postoperative pain, and reduce the occurrence of adverse reactions.

129 Effect of low dose Dexmedetomidine on postoperative stress response and delirium in patients undergoing laparoscopic radical resection of colon cancer

Objective To analyze the effect of low dose Dexmedetomidine on postoperative stress response and delirium in patients undergoing laparoscopic radical resection of colon cancer. Methods A total of 68 patients with colon cancer treated in Xinfeng People′s Hospital from February 2018 to March 2020 were selected as the research objects, and they were divided into two groups according to the principle of randomization, with 34 cases in each group. Both groups underwent laparoscopic radical resection of colon cancer under general intravenous anesthesia. Patients in the observation group received low-dose Dexmedetomidine during the induction period and continued to be pumped during the operation, and patients in the control group received 0.9% normal saline with equal volume. The stress response indexes and postoperative delirium (POD) were compared between the two groups. Results Plasma adrenocorticotropic hormone(ACTH), angiotensin-Ⅱ(Ang-Ⅱ) and cortisol (Cor) levels after induction of anesthesia, during extubation, and 1 hour after extubation in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). The incidence of POD in the observation group was 11.76%, which was lower than 32.35%in the control group, and the difference was statistically significant (P<0.05). The duration of POD in the observation group was shorter than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Low dose Dexmedetomidine can reduce stress response and incidence of POD in patients undergoing laparoscopic radical resection of colon cancer.

132 Comparison of echocardiographic indexes and the incidence of myocardial infarction in elderly patients of different ages

Objective To explore the indexes of echocardiography and the incidence of myocardial infarction in elderly patients of different ages. Methods A total of 120 elderly patients with myocardial infarction who were treated in Tieling Central Hospital from September 2018 to October 2019 were selected as the research objects. They were divided into group A,group B and group C according to different ages.Group A(35 cases)aged between 60 and 70 years old,Group B (42 cases) aged between >70 and 80 years old, and Group C (43 cases) aged over 80 years old were selected. The distribution of myocardial infarction site and the distribution of ultrasonic examination indexes were compared among the three groups. Results There was no significant difference in the incidence of anterior wall, inferior wall, posterior inferior wall and anterior wall plus inferior wall among the three groups (P>0.05); the incidence of anterior and inferior myocardial infarction in group C was higher than that of posterior inferior wall and anterior wall plus inferior wall, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of centrifuge infarction among the three groups in the left ventricular diameter ≥50 mm, left atrial diameter ≥39 mm, left ventricular diameter <50 mm and left atrial diameter <39 mm (P>0.05); the incidence of myocardial infarction in group C patients with left ventricular diameter ≥50 mm and left atrial diameter ≥39 mm was higher than that in group C patients with left ventricular diameter <50 mm and left atrial diameter <39 mm, and the difference was statistically significant (P<0.05). Conclusion The diagnostic effect of echocardiography on myocardial infarction in the elderly with the age of patients, the higher the diagnostic value, the better the diagnostic effect for patients over 80 years old, which has clinical significance.

135 Value of vaginal ultrasound and MRI in the diagnosis of cesarean scar pregnancy

Objective To investigate the value of vaginal ultrasound and MRI in the diagnosis of cesarean scar pregnancy (CSP). Methods The clinical data of 86 patients with CSP confirmed by surgery and pathology in Huai′an Maternal and Child Health Hospital from January 2019 to October 2020 were retrospectively analyzed, all patients underwent vaginal ultrasound and MRI examination, and the value of two methods in CSP diagnosis was analyzed. Results The coincidence rate of combined diagnosis of vaginal ultrasound and MRI (100.00%) was higher than that of vaginal ultrasound (91.86%) and MRI (94.19%), and the differences were statistically significant (P<0.05). There was no statistically significant difference in the diagnosis coincidence rate between vaginal ultrasound and MRI (P>0.05). Conclusion Both vaginal ultrasound and MRI have high compliance rate in the diagnosis of CSP, and the combined diagnosis of the two has higher diagnostic value and has important promotion value.

138 Clinical effect of Tacrolimus combined with Cyclophosphamide pulse therapy in the treatment of steroid-dependent nephrotic syndrome in children

 Objective To observe the clinical effect of Tacrolimus combined with Cyclophosphamide pulse therapy in the treatment of steroid-dependent nephrotic syndrome in children. Methods A total of 160 cases of children with steroid-dependent nephrotic syndrome treated in the Central Hospital Affiliated to Shenyang Medical College from March 2014 to March 2018 were selected as the research subjects and were divided into observation group and control group according to random number table method, with 80 cases in each group. On the basis of oral Prednisone treatment, the control group was given Cyclophosphamide pulse therapy, and the observation group was given Tacrolimus combined with Cyclophosphamide pulse therapy, both groups were treated for 12 months. The 24-hour urinary protein,blood urea nitrogen (BUN), serum creatinine (SCR), retinol binding protein (RBP), blood and urine β2-microglobulin(β2-MG), serum albumin (ALB), cholesterol (Chol), Cystatin C (CysC), D-dimer and fibrinogen (FIB) levels were observed before and 12 months after treatment, and the recurrence rate and incidence of adverse reactions were recorded.Results After treatment, 24 h urinary protein level, BUN, Scr, RBP, β2-MG, ALB, Chol, D-dimer and FIB levels in both groups were lower than before treatment, while CysC level was higher than before treatment, the differences were statistically significant (P<0.05), but there was no significant difference between the two groups (P>0.05). There was no statistical significance in the incidence of adverse reactions such as gastrointestinal reactions, decreased peripheral white blood cells and elevated liver transaminase between the two groups (P>0.05). The recurrence rate of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Compared with Cyclophosphamide pulse therapy alone, Tacrolimus combined with Cyclophosphamide pulse therapy in the treatment of children with steroid-dependent nephrotic syndrome has better clinical effect, the recurrence rate was low and the incidence of adverse reactions was not increased, which is worthy of clinical promotion.

Objective To investigate the effect of clinical pharmacist intervention on prophylactic application of antibiotics in type I incision surgery of traumatic orthopedics. Methods A total of 287 patients who received aseptic operation and prophylactic antibiotics in the Department of Orthopedics, Bayannur City Hospital from November 2017 to October 2018 were selected as the control group, and a total of 336 patients receiving aseptic operation and prophylactic antibiotics from November 2018 to October 2019 were selected as the intervention group. The perioperative prophylactic use of antibiotics in the control group was determined by the physician, while the clinical pharmacist in the intervention group went to the Department of Orthopedics for the intervention of irrational use. The type I incision operation and perioperative prophylactic use of antibiotics in the two groups were analyzed. The selection of antimicrobial varieties, rational rate of usage and dosage, duration of administration and postoperative infection were compared between the two groups. Results The rational rate of variety selection, usage and dosage of prophylactic antibiotics in the intervention group were higher than that in the control group, and the duration of administration was shorter than that in the control group, the differences were statistically significant (P<0.05). There was no significant difference in infection rate between the two groups (P>0.05). Conclusion Clinical pharmacist intervention can effectively improve the rational use of prophylactic antibiotics in type I incision surgery.

He has great attainments in the research and application of the theory of five evolutive phases and six climatic. Professor Gu Zhi-shan believes that five evolutive phases and six climatic is a theory in traditional Chinese medicine to explore the periodic law of natural changes and its impact on human health and diseases, and is the basis and origin of the basic theory of traditional Chinese medicine. In order to study the origin and development of five evolutive phases and six climatic, we should base on the perspective of Chinese culture, condense the law of yin and Yang into "three yin and three yang Taiji phase diagram", interpret "three generate all things" from yin and yang, and look at the function of six meridians dynamically. He insisted on a rigorous academic attitude and opposed to the metaphysical and simplification of the five evolutive phases and six climatic. The control group was given Glucosamine Sulfate Granules orally, and the observation group was given Shiwei Gugu Decoction on the basis of control group. Visual analogue scale (VAS) score, Western Ontario and McMaster University knee arthritis (WOMAC) index before treatment, 2 and 6 weeks after treatment, total response rate and the incidence of adverse reactions were compared between the two groups. Results There was no significant difference in VAS score and WOMAC index between the two groups before treatment (P>0.05). After 2 and 6 weeks of treatment, the VAS score and WOMAC index in both groups were lower than before treatment, and the VAS score and WOMAC index in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05).The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). There were no obvious adverse reactions in the two groups during treatment. Conclusion Shiwei Gugu Decoction has a good therapeutic effect on KOA of liver and kidney deficiency type, and its safety is high,which is worthy of clinical promotion.

156 Detection of illegal addition of Buformin and Glibornuride in chinese medicine and health foods

Objective A fast and accurate method was developed for the determination and conformation of buformin and glibornuride illegally added in traditional Chinese medicines and health foods by ultra-performance liquid liquidtandem mass spectrometry (UHPLC-MS/MS). Methods The samples were extracted with methanol by ultrasonic processing and separated on a ACES C18 (150 mm×2.1 mm, 1.7 μm) column with 0.1% formic acid-10 mmol/L ammonium acetate (A) and acetonitrile (B) solution by gradient elution. The chemical drugs were determined by UHPLC-MS/MS in positive ionization ESI and selected reaction monitoring mode. The obtained molecularions, fragment ions, and retention time for SRM channels were used to identify the two kinds of drugs, and quantitative analysis was conducted with external standard method. Results This method had a good separation effect, the two kinds of chemical drugs, including Buformin and Glibornuride showed good linearity in the range of 50-2000 ng/mL with the correlation coefficients (r≥0.9997). The limits of detection were 0.65 μg/g and 0.56 μg/g. The recoveries were 98.06% and 97.54%, and the RSDs were 2.43% and 3.84%. Conclusion This method is simple, accurate with high sensitivity, which was suitable for rapid determination of the two kinds of chemical drugs which illegally added in traditional Chinese drugs and health foods.

160 Clinical application study on external application of Chinese medicine in treatment of lactation mastitis during mass stagnation stage

Objective To explore the clinical application value of external application of Chinese medicine in the treatment of lactation mastitis during mass stagnation stage. Methods A total of 200 cases of lactation mastitis patients during mass stagnation stage in the First Affiliated Hospital of Nanchang University from August 30, 2017 to August 30, 2019 were selected as the research objects. The patients were divided into the traditional treatment group and the Chinese medicine external application group by the random number table method, with 100 patients in each group. The traditional treatment group was treated with milking of traditional Chinese medicine method and antibiotics, and the Chinese medicine external application group was treated with Ruyi Golden Powder on the basis of the traditional treatment. The total response rate, visual analogue scale (VAS) score before and after treatment, mass size and rate of breast reflux were compared between the two groups. Results The total effective rate in the Chinese medicine external application group was higher than that in the traditional treatment group, and the difference was statistically significant (P<0.05). The VAS score in the Chinese medicine external application group was lower than that in the traditional treatment group, and the difference was statistically significant (P<0.05). The mass range of the Chinese medicine external application group was lower than that of the traditional treatment group, and the difference was statistically significant(P<0.05). The rate of breast reflux of the Chinese medicine external application group was lower than that of the traditional treatment group, and the difference was statistically significant (P<0.05). Conclusion It has good clinical application value for patients with lactation mastitis during mass stagnation stage to add local external application of Chinese medicine treatment on the basis of traditional treatment, which can effectively relieve the pain of patients, improve the mass range of patients, and reduce the rate of milk regurgitation.

163 Clinical effect of acupuncture combined with Halitum on patients with lumbar intervertebral disc herniation

Objective To explore the clinical effect of acupuncture and moxibustion combined with Halitum in the treatment of patients with lumbar disc herniation. Methods A total of 60 patients with lumbar intervertebral disc herniation admitted to Yichun Hospital of Traditional Chinese Medicine from February 2019 to October 2020 were selected as the research objects and divided into control group (30 cases) and observation group (30 cases) randomly according to the touch ball method. The control group was treated with single acupuncture and moxibustion, while the observation group was treated with acupuncture and moxibustion combined with Halitum. Total effective rate, traditional Chinese medicine symptom score and lumbar function score were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). There was no significant difference in lumbar functional scores between the two groups before treatment (P>0.05).After 15 and 30 days of treatment, the lumbar functional scores of the two groups were higher than before treatment,and the lumbar functional scores of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). Before treatment, there was no significant difference in traditional Chinese medicine symptom scores between the two groups (P>0.05). After treatment, the traditional Chinese medicine symptom scores of the two groups after treatment were lower than before treatment, and the traditional Chinese medicine symptom scores of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). Conclusion Acupuncture and moxibustion combined with Halitum in the treatment of lumbar disc herniation can effectively enhance the total effective rate of disease treatment, improve the function of the lumbar spine, relieve the symptoms of traditional Chinese medicine, and has clinical application value.

166 Application of acupoint application combined with auricular point pressing under the guidance of Ziwu Liuzhu in patients with hypertension

Objective To explore the effect of acupoint application combined with ear acupoint pressing under the guidance of Ziwu Liuzhu on blood pressure control and traditional Chinese medicine (TCM) syndromes in hypertensive patients. Methods A total of 88 hypertensive patients admitted to Zhanjiang First Hospital of Traditional Chinese Medicine of Guangdong Province from November 2018 to October 2020 were selected as subjects and divided into the control group and the observation group by random number table method, with 44 patients in each group. The control group took conventional western medical care, and the observation group was intervened with acupoint application and auricular pressure under the guidance of Ziwu Liuzhu for 4 weeks. Blood pressure, TCM symptom score and total satisfaction rates were compared between the two groups. Results There were no statistically significant differences in blood pressure levels and TCM syndrome scores between the two groups before intervention (P>0.05). After intervention, the diastolic blood pressure, systolic blood pressure, and TCM syndrome scores of the observation group ([81.55±5.16],[127.39±6.85] mmHg, [8.51±2.06] points) were lower than those in the control group ([86.24±5.11], [133.52±7.04] mmHg,[13.63±2.92] points), and the differences were statistically significant (P<0.05). The total satisfaction rate of the observation group (93.18%) was higher than that of the control group (77.27%), and the difference was statistically significant (P<0.05). Conclusion Under the guidance of Ziwu Liuzhu, acupoint application combined with ear acupoint pressing can enhance the blood pressure control effect of patients with hypertension, improve the symptoms of traditional Chinese medicine, and increase patient satisfaction.

170 Clinical effect of Jiawei Huangqi Fangji Decoction combined with external air wave in the treatment of upper arm swelling and pain after breast cancer surgery

 Objective To compare the clinical effect of single external air wave and Jiawei Huangqi Fangji Decoction combined with external air wave in the treatment of upper arm swelling and pain after breast cancer surgery. Methods Sixty patients with postoperative upper arm swelling and pain of breast cancer admitted to the Jingdezhen Third People′s Hospital, Jiangxi Province from January 2018 to July 2020 were selected as the research subjects. The patients were divided into control group and experimental group by random number table method, with 30 cases in each group. Patients in the control group were treated with single external air wave, and children in the experimental group were treated with Jiawei Huangqi Fangji Decoction on the basis of the treatment in the control group. Before and after treatment, the differences of upper extremity circumference between the affected side and the healthy side, the pain situation and the improvement of depression were compared between the two groups, and the clinical efficacy of the two groups was compared. Results Before treatment, there were no statistical significances in the differences of upper extremity circumference between the affected side and the healthy side, visual analogue scale (VAS) scores and Hamilton depression scale (HAMD) scores between the two groups (P>0.05). After treatment, the difference of upper extremity circumference between the affected side and the healthy side in the experimental group was lower than that in the control group, and the VAS score and HAMD score in the experimental group were lower than those in the control group, the differences were statistically significant (P<0.05). The total effective rate of the experimental group was 90.00%, which was higher than that of the control group (66.67%), and the difference was statistically significant (P<0.05). Conclusion Compared with the single external application of air wave treatment, the combined application of Jiawei Huangqi Fangji Decoction and external air wave in the treatment of upper arm swelling and pain after breast cancer surgery, can achieve more significant clinical effect, and at the same time can help patients relieve pain and depression, which is worthy of promotion and publicity.

174 Clinical effect of Shuanghuanglian Granules combined with Ceftazidime in the treatment of children with acute upper respiratory tract infection

Objective To investigate the clinical effect of Shuanghuanglian Granules combined with Ceftazidime in the treatment of children with acute upper respiratory tract infection. Methods A total of 80 children with acute upper respiratory tract infection treated in Ruichang People′s Hospital from January 2019 to July 2020 were selected as subjects.According to the random number table method, they were divided into two groups, with 40 cases in each group. Children in the two groups were given routine treatment, the control group was given Ceftazidime for treatment on the basis of routine treatment, and the observation group was given Shuanghuanglian Granules combined with Ceftazidime on the basis of conventional treatment. The levels of inflammatory markers (C-reactive protein [CRP], interleukin-6 [IL-6], interleukin-10 [IL-10]) were compared between the two groups before treatment and 4 days after treatment. The improvement time of symptoms (cough, wheezing, lung rales, fever) between the two groups was compared. The total incidence of adverse reactions (nausea and vomiting, loss of appetite, abdominal pain and dizziness) was compared between the two groups. Results Before treatment, there were no significant differences in serum levels of CRP, IL-6 and IL-10 between the two groups (P>0.05). After treatment, the levels of CRP and IL-6 in both groups were lower than those before treatment, and the levels of IL-10 in both groups were higher than those before treatment, the differences were statistically significant (P<0.05). The levels of CRP and IL-6 in the observation group were lower than those in the control group, and the level of IL-10 in the observation group was higher than that in the control group, with statistical significances (P<0.05). The disappearance time of cough, wheezing, lung rales and fever abatement time in the observation group were shorter than those in the control group, the differences were statistically significant (P<0.05). During the treatment, the total incidence of adverse reactions in the observation group was 12.50%, while that in the control group was 7.50%. There was no statistical significance in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Shuanghuanglian Granules combined with Ceftazidime in the treatment of acute upper respiratory infection has a significant effect, can improve the inflammatory state of children, shorten the time for symptom improvement, worthy of clinical application

178 Clinical effect of Taoren Honghua Decoction combined with Ivabradine in the treatment of persistent atrial fibrillation of blood collateral stasis type

Objective To observe and explore the clinical effect of Taoren Honghua Decoction combined with Ivabradine in the treatment of persistent atrial fibrillation of blood collateral stasis type. Methods A total of 120 patients with persistent atrial fibrillation of blood collateral stasis type admitted to the Department of Cardiology of Hubei Integrated Traditional and Western Medicine Hospital from April to August 2020 were selected as the research subjects. According to the different treatment methods,the patients were divided into treatment group (65 cases) and control group (55 cases).Patients in both groups were given basic symptomatic treatment. Patients in the control group were treated with Ivabradine alone, and patients in the treatment group were treated with Taoren Honghua Decoction combined with Ivabradine.The clinical efficacy of the two groups was compared. The indexes of dynamic electrocardiograph ventricular rate (resting ventricular rate, 24 h mean ventricular rate, 24 h fastest ventricular rate, 24 h slowest ventricular rate), the 6 min walking distance, cardiac function indexes (left atrial diameter [LAD1], left ventricular ejection fraction[LVEF]), serological indexes (troponin [cTn], high-sensitive C-reactive protein [hs-CRP], N-terminal B-type natriuretic peptide precursor [NT-proBNP], aldosterone [Ald]) before and after treatment were compared between the two groups.The results of safety monitoring were compared between the two groups. Results After treatment, the total effective rate of the treatment group was 92.31%, which was higher than that of the control group (78.18%), the difference was statistically significant (P<0.05). After treatment, the resting ventricular rate, 24 h mean ventricular rate, 24 h fastest ventricular rate and 24 h slowest ventricular rate in the two groups were lower than those before treatment, and the 6 min walking distance was longer than that before treatment, with statistically significant differences (P<0.05). After treatment, the resting ventricular rate, 24 h mean ventricular rate, 24 h fastest ventricular rate and 24 h slowest ventricular rate in the treatment group were lower than those in the control group, and the 6 min walking distance was longer than that in the control group, with statistically significant differences (P<0.05). After treatment, LAD1 in the treatment group was less than that before treatment, and LVEF was greater than that before treatment, with statistically significant differences (P<0.05). After treatment, LAD1 in the treatment group was lower than that in the control group, and LVEF in the treatment group was greater than that in the control group, with statistically significant differences (P<0.05). After treatment, the levels of hs-CRP, NT-proBNP, Ald and cTn in the two groups were all lower than those before treatment, with statistically significant differences (P<0.05). The levels of hs-CRP, NT-proBNP, Ald and cTn in the treatment group were all lower than those in the control group after treatment, with statistically significant differences (P<0.05). There was no statistical significance in the incidence of adverse drug reaction between the two groups (P>0.05).Conclusion The clinical effect of Taoren Honghua Decoction combined with Ivabradine in the treatment of patients with persistent atrial fibrillation of blood collateral stasis type is better than that of Ivabradine alone, which is worthy of clinical application.

184 Analysis on hazard cognition and handling status of mercury leakage incident among nursing staff of different qualifications

Objective To analyze the hazard cognition and handling status of mercury leakage incident among nursing staff of different qualifications. Methods A total of 320 clinical nursing staff in Jiujiang No.1 People′s Hospital were selected as the research subjects. According to their nursing qualifications, they were divided into senior nursing group(titled as chief nurse, n=80), intermediate nursing group (titled as junior nurse, n=80), primary nursing group (titled as nurse, n=80) and practice nursing group (practice nurse, n=80). The handling status of mercury leakage incident was assessed among the four groups, and awareness of the hazard cognition of nursing staff towards mercury leakage incident (hazards of mercury, intrusion mode into the human body, occupational precautions), on-site simulation operation ability and handling effect of mercury leakage incident were compared among the four groups. Results There were significant differences in the awareness of hazard cognition of nursing staff towards mercury leakage incident, qualification rate of on-site simulation operation ability and satisfaction rate of handling mercury leakage incident among the four groups (P<0.05). There were no significant differences between senior nursing group and intermediate group and between intermediate nursing group and primary nursing group in terms of mercury awareness of hazard cognition, on-site simulation operation ability and satisfaction rate of handling (P>0.05). The mercury awareness of hazard cognition, onsite simulation operation ability and satisfaction rate of handling in primary group were lower than those in senior nursing group, and the above indexes in practice nursing group were lower than those in primary nursing, intermediate nursing and senior nursing groups, with statistically significant differences (P<0.05). Conclusion Hazard cognition and handling ability of mercury leakage incident of intermediate and senior nursing staff are significantly higher than those of junior and practice nursing staff, but the indexes have not reached the ideal standard. It is necessary to conduct clinical teaching and training for all nursing staff according to the actual situation to reduce the occurrence of mercury leak incident in the hospital and timely make effective treatment.

The research demand were obtained through questionnaire survey, and the items with composition ratio ≥70% were selected for analysis. Results The clinical medical technicians in Beijing Obstetrics and Gynecology Hospital, Capital Medical University had the requirements of time and energy, scientific research platform, scientific research design, scientific research methodology training, lack of ideas and topic selection. Age, professional title and occupation were the main influencing factors. Conclution Hospitals should start from the research training system, young talent training, discipline leaders, research time and other aspects to meet the demand of scientific research, and actively improve the level of scientific research in hospitals.

191 Cluster analysis of hot spots in fall care for elderly patients based on bibliometrics

Objective Through the research and analysis of domestic journal literature in the field of elderly patients′ fall care in China,to provide reference for the research of elderly patients′fall care in China.Methods By searching the fulltext database of academic journals of China National Knowledge Infrastructure,the literatures on nursing care of elderly patients with falls from 2010 to 2019 were obtained. Bibliographic Items Co-occurrence Matrix Builder (BICOMB) was used for bibliometric analysis. gCLUTA 2.0 software was used for visualization to reveal the research hotspots in this field. Results A total of 1132 related documents were obtained, including 1037 valid papers mainly from 241 journals with 913 high-frequency subject terms.Through two-way cluster analysis,domestic research on patients′ fall care can be summarized into seven clustering hotspots. The content mainly involves the factors of internal medicine and psychiatric inpatient falls, the effect of detailed nursing and comprehensive care on elderly patients′ falls, and the evaluation of related nursing methods in elderly patients after falls, etc. Conclusion The study reveals the hot spots in the field of elderly patients′ fall care in China and provides some references for the development direction of this field in China.

195 Investigation on influencing factors of medication compliance of pulmonary tuberculosis patients among floating population in Zhongshan City

 Objective To investigate the influencing factors of medication compliance of pulmonary tuberculosis (PT)patients among floating population in Zhongshan City. Methods The data of 143 cases of PT among floating population in Zhongshan City from June 2018 to June 2020 were collected. The relevant information was obtained through questionnaire survey. Morisky medication adherence scale-8 (MMAS-8) was used to evaluate medication compliance, and the influencing factors of medication compliance of PT patients among floating population were analyzed. Results Single factor analysis results showed that the medication compliance of PT patients among floating population was not affected by gender, residence distance from hospital and occupation (P>0.05). However, the medication compliance of PT patients among floating population might be affected by age, educational level, catastrophic expenditure of tuberculosis and knowledge of pulmonary tuberculosis (P<0.05). Logistic regression analysis showed that age ≥60 years old, education level of junior middle school or below, having catastrophic expenditure of tuberculosis and lacking knowledge of pulmonary tuberculosis treatment were the influencing factors for poor medication compliance of PT patients among floating population (P<0.05). Conclusion Age ≥60 years old, education level of junior high school or below, having catastrophic expenditure of tuberculosis and lacking knowledge of pulmonary tuberculosis treatment may be the influencing factors for poor medication compliance of PT patients among floating population. Relevant programs can be developed clinically to improve medication compliance according to the above factors.

198 Effect of mixed synchronous learning on clinical teaching of medical undergraduates and teachers′ teaching ability

Objective To explore the influence of mixed synchronous teaching on the learning effect of clinical medical undergraduates. Methods A total of 134 undergraduate students majoring in 2015 clinical medicine in Qiqihar Medical University in February 2019 were selected as the control group, another 142 undergraduate students majoring in 2016 clinical medicine of Qiqihar Medical University in February 2020 were selected as the observation group. The control group was given the traditional teaching method, the observation group was given mixed synchronous teaching. The theoretical scores and case analysis scores of the two groups were observed. After the end of teaching, the two groups were given final assessment and teaching satisfaction survey. The final assessment included theoretical assessment and case analysis assessment. Before the beginning of school in January 2020 and after the final examination in July 2020, the teaching ability and job burnout scale of the teachers were evaluated. The teaching ability included communication ability and empathy ability. The job burnout scale was divided into three dimensions: emotional exhaustion, personal non achievement and depersonalization. Results The theoretical score and case analysis examination of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05); and there was no significant difference in the teaching satisfaction score between the two groups (P>0.05). The communication ability and empathy ability of teachers in the teaching group in July 2020 were higher than those in January 2020, and the score of emotional exhaustion in July 2020 was lower than that in January 2020, the differences were statistically significant (P<0.05). There was no significant difference in personal non achievement and depersonalization of teachers between January 2020 and July 2020 (P>0.05).Conclusion Based on the hybrid synchronous teaching method can improve the theoretical achievement and case analysis ability of medical undergraduates. At the same time, it can mobilize and improve teachers′ communication ability,empathy ability, and improve teachers′ emotion.

202 Effect analysis of health education on primiparous women on a consistent WeChat platform

Objective To explore the effect of a consistent WeChat platform health education on primiparas. Methods A total of 100 primiparas from February to May 2020 in Dalian Women and Children Medical Center who were examined in the obstetric clinic after 36 weeks of gestation and gave birth at term in hospital were selected. They were divided into a control group and an observation group by a random number table method, with 50 cases in each group. In the control group, regular health education was used, and in the observation group, a consistent WeChat platform for health education was adopted. The 36th week of pregnancy was led to the WeChat platform for health education until the 42-day postpartum physical examinations. Primiparous satisfaction with nursing, and their mastery of postpartum nursing, breastfeeding, and parenting knowledge were compared in the two groups. Results The overall satisfaction with nursing and the knowledge of postpartum nursing, breastfeeding, and childcare scores in the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). Conclusion The consistent health education on WeChat platform not only increases the satisfaction of primiparas with nursing, but also improves the maternal mastery of their own postpartum nursing and parenting knowledge.

208 Application of positive regard technology combined with Maslow′s hierarchy of needs in patients with chemotherapy for lung cancer

Objective To explore the application effect of positive regard technology combined with Maslow′s hierarchy of needs theory in patients with chemotherapy for lung cancer. Methods A total of 117 patients with chemotherapy for lung cancer in Jiangxi Tumor Hospital from December 2017 to December 2019 were selected, random number table method was used to divide them into the control group (n=59) and the observation group (n=58). The control group was intervened by Maslow′s hierarchy of needs theory, the observation group was used positive regard technology combined with Maslow′s hierarchy of needs for intervention, all patients were continuously intervened until they were discharged from hospital. The hospitalization time of patients in the two groups was compared, and compared the psychological status [general hospital anxiety and depression scale (HADS) and the quality of life [lung cancer life quality assessment scale (FACT-L) of patients in the two groups at admission and after discharge of hospital, and counted and compared the nursing satisfaction of patients in the two groups. Results The hospitalization time of the observation group was shorter than the control group, with a statistical significant difference (P<0.05); The scores of depression and anxiety of patients in the observation group were lower than those of the control group at discharge, with statistical differences (P<0.05); the scores of physiology, social/family, emotion, function and the total score of scale of patients in the observation group were lower than those of the control group at discharge, with statistical differences (P<0.05); the total satisfaction of the observation group was higher than that of the control group, with statistical difference (P<0.05). Conclusion Positive regard technology combined with Maslow′s hierarchy of needs theory has a better application effect in patients with chemotherapy for lung cancer, which can effectively improve the mental state of patients, improve their quality of life and satisfaction, shorten the hospitalization time.

212 Intervention of holistic nursing on efficacy of freezing condyloma acuminatum in anal canal

Objective To investigate the effect of holistic nursing intervention on efficacy of condyloma acuminatum in anal canal. Methods A total of 60 patients with condyloma acuminatum in anal canal treated in Jiangxi Provincial Dermatology Hospital from March 2019 to March 2020 were selected as the research subjects, and were divided into two groups according to the random number table method, 30 cases in each group. The control group was used routine nursing intervention, and the observation group was used holistic nursing intervention under the basis of routine nursing, all of them were nursing for 12 weeks. The rehabilitation effect, negative emotions, treatment compliance and relapse rate of the two groups were compared. Results After 12 weeks of nursing, the total recovery efficiency of the observation group was higher than that of the control group, and the scores of the self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were lower than those of the control group, and the differences were statistically significant (P<0.05); after 4 and 12 weeks of nursing, the treatment compliance score of the observation group was higher than that of the control group, and the recurrence rate of the observation group was lower than that of the control group after cure, the differences were statistically significant (P<0.05). Conclusion Holistic nursing can improve the efficacy of condyloma acuminatum in anal canal, significantly improve or eliminate negative emotions of patients,improve treatment compliance, and reduce the rate of disease recurrence.

215 Influence of CGA-based continuing nursing care on the cognitive status and self-care ability of patients with Alzheimer′s disease

 Objective To explore the impact of continuing nursing care based on comprehensive Geriatric Assessment(CGA) on the cognitive status and self-care ability of patients with Alzheimer′s disease (AD). Methods a total of 88 patients with AD admitted to the Third People′s Hospital of Ganzhou City, Jiangxi Province from April 2018 to March 2020 were selected and divided into two groups by random number table method, 44 cases in each group. The control group received routine follow-up care, and the observation group received CGA-based continuation care for a period of 3 months. The cognitive status, daily self-care ability and accident rate were compared between the two groups. Results The two groups of AD assessment scale-cognitive subscale (ADAS-Cog), minimental state examination (MMSE),Barthel index(BI)scores comparison before nursing,the differences were not statistically significant (P>0.05).The scores of ADAS-Cog, MMSE, BI in the control group were compared before and after nursing, the differences were not statistically significant (P>0.05); The scores of ADAS-Cog, MMSE, and BI in the observation group after nursing were higher than those before nursing, and the observation group was higher than the control group, the differences were statistically significant (P<0.05). the total incidence of accidents in the observation group (6.82%) was lower than that of the control group (22.73%), the difference was statistically significant (P<0.05). Conclusion CGA-based continuing nursing care can improve the cognitive status of patients with AD, improve their self-care ability in daily life, and reduce the occurrence of accidents.

218 Application of group perinatal health nursing in parturient

Objective To explore the influence of group health care on postpartum nursing satisfaction rate, postpartum breastfeeding rate and postpartum anxiety/depression mood. Methods A total of 89 cases of parturient treated in Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from May 2019 to June 2020 were selected as subjects of the study. According to the random number table method, they were divided into the experimental group(48 cases) and the control group (41 cases).The experimental group was given group perinatal health nursing and the control group was given routine nursing intervention, nursing satisfaction rate, postpartum breastfeeding rate and postpartum anxiety/depression mood were compared between the two groups. Results The total satisfaction of postpartum care in the experimental group (93.75%) was higher than that in the control group (70.73%), and the difference was statistically significant (P<0.01).The rate of postpartum breastfeeding in the experimental group (73.33%) was higher than that in the control group (51.22%), and the difference was statistically significant (P<0.05).The scores of self-rating anxiety scale (SAS), self-rating depression scale (SDS) in the experimental group and the control group after intervention were lower than those before intervention, the scores of SAS, SDS in the experimental group were lower than those in the control group after intervention, and the differences were statistically significant (P<0.05). Conclusion Group perinatal health nursing can effectively improve the satisfaction of postpartum nursing, improve the rate of postpartum breastfeeding, relieve postpartum anxiety and depression mood, and has clinical application value.

221 Analysis of the application value of post-abortion care in the nursing of threatened abortion patients

 Objective To analyze the application value of post-abortion care (PAC) in the nursing of threatened abortion patients. Methods A total of 400 cases threatened abortion patients admitted to Jiangxi Maternal and Child Health Hospital from January 2019 to June 2020 were selected as research subjects. They were divided into the observation group and the control group by the random number table method, 200 cases in each group. Patients in the observation group were provided with PAC and routine care was used for the patients in the control group. The patients′ satisfaction, outcome of fetus protection, hospital stay, and score of negative emotions were compared between the two groups.Results The total satisfaction of patients in the observation group after nursing was higher than that of the control group, and the difference was statistically significant (P<0.05). The success rate of fetus preservation after nursing in the observation group was higher than that in the control group, with statistical difference (P<0.05). The hospital stay in the observation group was shorter than that in the control group, and the difference was statistically significant (P<0.05).The pain score of the observation group after nursing was lower than that of the control group, and the difference was statistically significant (P<0.05). The negative emotion score of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Compared with routine care, PAC intervention for patients can reduce the probability of patients presenting with anxiety, shorten the hospital stay, and the outcome of maternal fetal preservation is good, which can increase the satisfaction of patients with care and is worthy of promotion.

225 Effect of group psychological intervention based on knowledge, attitude and practice theory on self perceived burden and quality of life of lung cancer patients

 Objective To explore the effect of group psychological intervention based on knowledge attitude practice(KAP) theory on self perceived burden and quality of life of lung cancer patients. Methods A total of 86 patients with lung cancer treated in Jiujiang Third People′s Hospital in Jiangxi Province from April 2019 to July 2020 were selected and divided into two groups according to random number table method, 43 cases in each group. The control group was given routine nursing,and the observation group was given group psychological intervention based on KAP theory.The self perceived burden and quality of life of the patients before and 1 month after the intervention were compared. Results Before the intervention, there were no significant difference in the scores of self perceived burden and quality of life between the two groups (P>0.05). After the intervention, the scores of self perceived burden in the two groups were lower than those before the intervention, and the scores of physiological, social/family, emotional and functional conditions in the quality of life were higher than those before the intervention, with statistical differences (P<0.05); after the intervention, the scores of self perceived burden in the observation group were lower than those in the control group, and the scores of physiological, social/family, emotional and functional conditions in the quality of life were higher than those in the control group, the differences were statistically significant (P<0.05). Conclusion Group psychological intervention based on KAP theory can reduce the self perceived burden of lung cancer patients and improve their quality of life.

228 Effect of peer education on the psychological pressure and self-efficacy of elderly patients with type 2 diabetes mellitus

Objective To explore the effect of peer education on the psychological pressure and self-efficacy of elderly patients with type 2 diabetes mellitus (T2DM). Methods A total of 128 elderly patients with T2DM treated in the Fourth Affiliated Hospital of Nanchang University from June 2018 to June 2020 were selected as study subjects. The patients were divided into control (n=64) and observation (n=64) groups by random number table method. The control group was given routine education, and the observation group was given joint peer education on the basis of the control group, with intervention for 2 months in both groups. The psychological stress and self-efficacy changes were compared between the two groups. Results After 2 months of intervention, the self-rating anxiety scale (SAS) and self-rating depressin scale (SDS) of the two groups were lower than those before the intervention, and the SAS and SDS scores of the observation group were lower than those of the control group, with statistically significant differences (P<0.05). After 2 months of intervention, the scores of clinical treatment, exercise, diet and foot management in the two groups were higher than those before the intervention, while the scores of clinical treatment, exercise, diet and foot management in the observation group were higher than those in the control group, with significant differences (P<0.05). Conclusion Peer education for elderly T2DM patients can effectively relieve their psychological pressure and improve the selfefficacy, which is worthy of promotion.

231 Application effect of 3D printing individualized protective support in patients with skull defect

Objective To explore the application effect of 3D printing individualized protective support in patients with skull defect. Methods A total of 30 patients with skull defect after intracranial hematoma removal admitted to Yichun People′s Hospital from January 2019 to September 2020 were selected and divided into the control group and the observation group according to random number table method, with 15 cases in each group. The control group used conventional nursing care, and the observation group used 3D printed individualized protective braces related nursing care on the basis of the control group, and continued observation until 1 month after surgery. The total incidence of complications, self-care ability, exercise ability, quality of life and patient satisfaction were compared between the two groups.Results There was no statistically significant between the two groups in the total incidence of complications (P>0.05).One month after surgery, the Barthel index (BI), Fugl-Meyer assessment (FMA) score of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). One month after surgery,the scores of quality of life questionnare-core 30 (QLQ-C30) and Newcastle Satisfaction with nursing scales (NSNS) of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05).Conclusion The design of the 3D printing individualized protective brace can improve the protective effect of the skull defect area, reduce the occurrence of local complications, promote the recovery of patients′ self-care ability and exercise ability, improve their quality of life, and increase nursing satisfaction.

234 Application of retention enema of traditional Chinese medicine combined with auricular point sticking and pressing in nursing of elderly patients with constipation

Objective To investigate the application of retention enema of traditional Chinese medicine combined with auricular point sticking and pressing in nursing of elderly patients with constipation. Methods Retrospective analysis was performed on 82 elderly patients with constipation who were treated in Zhonghe Hospital of Yichuan City from April 2017 to December 2019. According to the treatment methods, they were divided into the observation group (42 cases) and the control group (40 cases). The control group was treated with auricular point sticking and pressing, while the observation group was treated with retention enema of traditional Chinese medicine combined with auricular point sticking and pressing. The curative effect, TCM symptom score and nursing satisfaction were compared between the two groups. Results The total effective rate after nursing of the observation group was higher than that of the control group,the difference was statistically significant (P<0.05). There was no significant difference between the two groups in the scores of anorexia, fatigue, dry stool, low back pain and back pain (P>0.05). After treatment, the scores of poor appetite,fatigue, dry stool, back pain and total score after nursing in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05); the total satisfaction of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). Conclusion Retention enema of traditional Chinese medicine combined with auricular point sticking and pressing can decrease the scores of clinical symptoms of elderly patients with constipation, and improve the clinical symptoms, improve the treatment effect and nursing satisfaction.

238 Effect of Omaha system nursing model on depression degree and well-being index score of primipara

Objective To explore the effect of Omaha system nursing mode on the depression degree and well-being index score of primipara. Methods A total of 90 primiparas who received prenatal diagnosis and delivery in Fuzhou Maternal and Child Health Hospital from January 2019 to January 2020 were randomly divided into the study group and the control group according to computer drawing method, with 45 cases in each group. The control group adopted routine nursing mode, and the study group adopted Omaha system nursing mode on the basis of the control group. After 8 weeks of intervention, the Edinburgh postpartum depression scale (EPDS) was used to evaluate the improvement of the depression degree of primiparas in the two groups, the changes of well-being index were evaluated by the general well-being scale (GWB), and the nursing satisfaction of primiparas was investigated by nursing satisfaction questionnaire. Results There was no significant difference in EPDS score between the two groups before intervention (P>0.05).After intervention, the EPDS score of the two groups was lower than that before intervention and that of the study group was lower than that of the control group, the differences were statistically significant (P<0.05). There was no significant difference in GWB score between the two groups before intervention (P>0.05). After intervention, the GWB score of the two groups was higher than that before intervention and that of the study group was higher than that of the control group, the differences were statistically significant (P<0.05). The total nursing satisfaction of the study group was higher than that of the control group, the difference was statistically significant (P<0.05). Conclusion The application of Omaha system nursing mode is helpful to improve the degree of postpartum depression of primipara, enhance the subjective well-being and nursing satisfaction of primipara.

Systematic nursing intervention was used in the experimental group. The control group adopted the conventional nursing mode, nursing satisfaction, nursing intervention 1, 3 days after the creatinine, contrast nephropathy incidence were compared. in the two groups. Results After nursing intervention 3 d, the total nursing satisfaction of the experimental group was higher than that of the control group, the creatinine value of the experimental group on day 1 and 3 was lower than that of the control group, and the incidence of contrast nephropathy of the experimental group was lower than that of the control group, the differences were statistically significant (P<0.05). Conclusion The application of systematic nursing intervention can effectively prevent the incidence of enhanced CT contrast nephropathy and improve nursing satisfaction.

Reference group C adopted conventional emergency nursing procedures, and study group Y adopted optimized emergency nursing procedures. Terms of emergency time, hospital stay, stay (emergency) time, evaluation (triage) time, rescue success rate, and adverse event rate between two groups were compared.Results Study group Y the emergency time was (26.55±5.22)min,hospitalization time was (9.54±2.24)d, stay time was (52.61±9.12) min, evaluation time was (2.96±0.69)min, shorter than those of the reference group C of (38.94±5.97) min, (13.12±2.94) d, (69.47±9.66) min, (4.66±0.85) min, the differences were statistically significant (P<0.05); Study group Y total composition the power was 97.22%, which was higher than 74.29% in the reference group C, and the difference was statistically significant (P<0.05); during the treatment period,the total incidence of adverse events in the study group Y was 11.11% lower than that of the reference group C,31.43%, the difference was statistically significant (P<0.05). Conclusion Applying optimized emerency nursing procedures to the rescue of patients with acute chest pain can effectively shorten the patient′s emergency time, hospital stay,stay time, evaluation time, etc. improve the success rate of patient rescue, reduce the incidence of adverse events, and help improve the prognosis.

249 Application effect of nursing intervention by regulating meridian and Zang-organs in patients with type 2 diabetic peripheral neuropathy

Objective To explore the application effect of nursing intervention by regulating the meridian and Zang-organs in patients with type 2 diabetic peripheral neuropathy(DPN). Methods A retrospective analysis of the clinical data of 100 patients with DPN admitted to Shenzhen Hospital of Guangzhou University of Chinese Medicine from December 2018 to August 2019 were selected. They were divided into two groups according to different interventions, 50 cases in each group. In the control group, patients were routinely given blood glucose control, circulation improvement,nerve nourishment, and health education and guidance on the protection of DPN. In the observation group, nursing intervention of regulating the meridian and the internal organs was added on the basis of the control group. The motor nerve conduction velocity (MNCV) and sensory nerve conduction velocity (SNCV) of the median nerve and common peroneal nerve of the two groups before and after treatment were observed. The efficacy was determined by giving electromyography examination and clinical symptom observation. Results The total effectiveness rate of the observation group after intervention was higher than that of the control group, and the difference was statistically significant (P<0.05). The MNCV and SNCV of the superficial peroneal nerve and median nerve after nursing intervention in the observation group and the control group were higher than those of the intra-group intervention before, and the differences were statistically significant (P<0.05). The MNCV and SNCV of the superficial peroneal nerve and median nerve in the observation group after nursing intervention were higher than those of the control group, and the differences were statistically significant (P<0.05). Conclusion On the basis of conventional treatment, joint application of the meridian and Zang-organs regulating can alleviate the symptoms of patients with DPN, and can improve the peripheral nerve damage to a certain extent.

252 Clinical application of music therapy in treatment of Alzheimer′s disease

Objective To observe the effect of music therapy on behavioral and psychological symptoms of patients with Alzheimer's disease (AD). Methods Eighty patients with AD treated in Xinhua Hospital Affiliated to Dalian University from April 2017 to April 2020 were selected as research subjects, and were divided into the treatment group (n=40)and the control group (n=40) according to the lot drawing method. Music therapy plus memantine therapy was given to the treatment group, and memantine therapy was given to the control group, with the treatment course of 16 weeks. The decreased score of neuropsychiatric inventory (NPI) after the treatment and the mini-mental state examination (MMSE)score before and after the treatment in the two groups were compared. Results In the comparison of decreased scores of NPI after the treatment in the two groups, the decreased scores of delusion, hallucination, agitation/aggression, depression/dysphoria, anxiety, and apathy/indifference in the treatment group were higher than those in the control group, with statistical differences (P<0.05); there were no statistically significant differences in the decreased scores of elation/euphoria, disinhibition, irritability/lability, motor disturbance, nightime behaviors or appetite/eating between the two groups (P>0.05). The decreased score of MMSE (the difference score between before and after the treatment) in the treatment group was (0.931±0.101) points, higher than that in the control group of (0.272±0.081), with statistical difference(P<0.05). Conclusion Music therapy can improve behavioral and psychological symptoms of patients with AD especially in agitation/aggression, apathy/indifference, depression/dysphoria, delusion, hallucination, and anxiety, and can also improve cognitive function.

The control group was given routine nursing care, and the observation group was given positive psychological nursing on the basis of the control group. The positive and negative symptom scores (PANSS), mental state scores, and quality of life scores were compared between the two groups. Results The scores of PANSS, Hamilton anxiety scale (HAMA), Hamilton depression scale (HAMD) of the observation group after nursing were lower than those of the control group, the differences were statistically significant (P<0.05). The score of quality of life in the observation group after nursing was higher than that in the control group, the difference was statistically significant (P<0.05). Conclusion Positive psychological nursing can effectively improve the mental condition of patients with schizophrenia during the rehabilitation period, relieve their mental symptoms, and improve their quality of life.

The quality of life scale (QOL-C30) score, Parkinson′s motor function scale (MORSPD) score, daily life ability scale (ADL) score were compared between the two groups; self-rating anxiety scale (SAS) score, self-rating depression scale (SDS) score, Barthel index were compared. Results There were no significant difference in Barthel index and QOL-C30, MORSPD, ADL, SAS, SDS score between the two groups before intervention (P>0.05). After intervention, the Barthel index and QOL-C30, MORSPD, ADL score of the two group were higher than those before intervention, while the SAS, SDS score were lower than those before intervention; after intervention, the Barthel index and QOL-C30,MORSPD,ADL score in the observation group were higher than those in the control group, while the SAS, SDS score were lower than those in the control group, the differences were statistically significant (P<0.05). Conclusion Targeted nursing combined with rehabilitation exercise intervention can effectively alleviate Parkinson′s disease negative emotions, improve the quality of life, improve motor function, promote the rapid recovery of daily life ability, worthy of clinical wide promotion.

261 Application of predictive nursing on complications and wound healing in patients with major damage

 Objective To explore the effect of predictive nursing on complications and wound healing in patients with major trauma. Methods A total of 50 patients with major damage and trauma admitted to the Second Department of Orthopedics of Zhangzhou Municipal Hospital of Fujian Province from January to December 2019 were selected as the research objects and divided into observation group (25 cases) and control group (25 cases) according to random number table method. The control group was treated with routine nursing measures, and the observation group was treated with predictive nursing. The incidence of complications, wound healing and psychological state were compared between the two groups. Results The total incidence of complications in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). The wound healing time of the observation group was shorter than that of the control group, and the wound healing rate of the observation group was higher than that of the control group, the differences were statistically significant (P<0.05). After intervention, the scores of self-rating anxiety scale(SAS) and self-rating depression scale (SDS) in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). Conclusion Predictive care can reduce the incidence of complications in patients with major damage and trauma, play a positive role in wound healing, and can shorten the wound healing time of patients, improve the psychological state of patients, and promote recovery.

265 Effect of evidence -based nursing model on blood glucose level,foot function recovery and neuropathy in patients with diabetic foot

Objective To investigate the effect of evidence-based nursing model on blood glucose level,recovery of foot function and neuropathy in patients with diabetic foot(DF). Methods A total of 88 cases of patients with DF who were admitted to the Second Affiliated Hospital of Nanchang University from January 2018 to December 2019 were selected as the research subjects, and they were divided into two groups according to the random number table method, 44 cases in each group. The control group underwent routine nursing, while the observation group was given evidence-based nursing model. The blood glucose level, foot function, neuropathy and vascular disease of the two groups were compared. Results The fasting blood glucose, 2 h postprandial blood glucose, dorsalis pedis artery pulse, numbness of lower limb,foot pain and peripheral neuropathy in the observation group were (6.31±1.43)mmol/L, (7.50±1.71) mmol/L,(1.23±0.28) points, (0.85±0.17) points, (1.21±0.41) points, (4.74±0.85) points lower than those in the control group of(7.18±1.27) mmol/L, (8.89±1.38) mmol/L, (1.62±0.47) points, (1.22±0.40) points, (1.73±0.56) points, (5.59±1.37) points,after intervention for 1 month, and the differences were statistically significant (P<0.05). There were no statistically significant differences between the two groups in blood glucose level, foot function, neuropathy and vascular disease before intervention (P>0.05). After 1 month of intervention, the blood glucose level, foot function and neuropathy of the two groups were lower than that before intervention, and the ankle-brachial index was higher than that before intervention, with statistical differences (P<0.05). Conclusion The evidence-based nursing model can decrease the blood glucose level of patients with DF, improve the foot function, alleviate the degree of peripheral nerve and vascular disease, and promote the recovery of patients.

268 Application effect of improved nursing cooperation in perioperative period of patients with chronic appendicitis

 Objective To explore the application effect of improved nursing cooperation in the perioperative period of patients with chronic appendicitis. Methods A total of 100 patients with chronic appendicitis treated in Dangyang People′s Hospital in Hubei Province from January 2018 to January 2020 were selected as the research objects, and the patients were divided into the observation group and the control group by random number table method, 50 cases in each group. The control group was given routine nursing cooperation, and the observation group was given improved nursing cooperation on this basis. Anal exhaust time, ambulation time, operation time, hospitalization time, postoperative complications and nursing satisfaction were compared between the two groups. Results The anal exhaust time, ambulation time, operation time and hospitalization time of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). The nursing total satisfaction of the observation group was higher than that of the control group, the difference was statistically significant(P<0.05). Conclusion The improved nursing cooperation in the process of chronic appendicitis surgery can improve the operation efficiency and effect, and can also reduce the occurrence of postoperative complications, so that patients are satisfied with the treatment and nursing work, which is worthy of promotion.

271 Influence of nursing service mode of midwifery on episiotomy rate in new stage of labor

Objective To explore the influence of the new delivery midwifery nursing service model on the rate of perineal lateral resection. Methods From February to September in 2020, 80 parturients from Wuxi People′s Hospital in Jiangsu Province were selected as the research subjects. They were divided into the observation group and the control group by the method of touching the ball, 40 cases in each group. In the control group, routine nursing service model was used, and in the observation group, the new labor midwifery nursing service model was adopted. The volume of postpartum hemorrhage, second stage of labor, episiotomy, postpartum pain score and maternal satisfaction were compared between the two groups. Results The volume of postpartum hemorrhage in the observation group was less than that of the control group, the second stage of labor was shorter than the control group, with statistical differences (P<0.05). The episiotomy rate of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). The postpartum visual analog score (VAS) in pain of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). The maternal total satisfaction of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).Conclusion The new delivery process midwifery nursing service model has a very obvious effect on reducing the volume of postpartum hemorrhage, shortening the second stage of labor, reducing the rate of perineal incision, alleviating postpartum pain and increasing maternal satisfaction. It can be popularized in clinical practice.

274 Effect of psychological nursing in perioperative period of patients with intracranial tumor

 Objective To explore the effect of psychological nursing in the perioperative period of patients with intracranial tumor. Methods A total of 68 cases of intracranial tumor patients treated in Liaocheng Third People′s Hospital in Shandong Province from October 2018 to January 2020 were selected as the research objects, and they were divided into the control group (34 cases) and the experimental group (34 cases) according to the random number table method. The control group was given routine nursing methods, and the experimental group was given psychological nursing intervention. The clinical treatment effect, hospitalization time, anxiety and depression scores and nursing satisfaction of the two groups were compared. Results The total effective rate and nursing total satisfaction of the experimental group after nursing were higher than those of the control group, the differences were statistically significant (P<0.05). The anxiety and depression scores of the experimental group after nursing were lower than those of the control group, the differences were statistically significant (P<0.05). The hospitalization time of the experimental group after nursing was shorter than that of the control group, the difference was statistically significant (P<0.05). Conclusion In the perioperative period in patients with intracranial tumor, cooperate psychological nursing intervention can effectively improve patient satisfaction with care, alleviate the psychological pressure of patients, more conducive to improve the patients′ condition and the desired therapeutic effect, thus effectively improve the quality of life of patients, in the future clinical work more popularization and application value.
