LI Jian-jun1 ZHANG Hu1 WU Wei2 SHI Dan3▲
Objective
To explore the changes of renal function when the Vancomycin at a minimum concentration (C
min
)was first detected after intravenous administration of vancomycin, and to evaluate the correlation between blood concentration of Vancomycin and renal function indicators.
Methods
A total of 61 patients with severe brain injury and Gram-positive cocci (G
+
) pneumonia hospitalized in the Neuro-Intensive Care Unit of the 904
th
Hospital of the PLA(formerly 101 Hospital) from January 2015 to December 2017 were selected. The biochemical indicators including blood urea nitrogen (BUN), serum creatinine (SCr), β
2
microglobulin (β
2
-MG), α
1
microglobulin (α
1
-MG), cystatin C(Cys-C) before Vancomycin and 48 hours after medication were tested. The estimated glomerular filtration rate (eGFR)and Vancomycin concentration were calculated using Modification of Diet in Renal Disease. Statistical analysis was performed on the renal function indicators before and after medication, and the correlation between Vancomycin plasma concentration and renal function indicators was analyzed using multiple correlation analysis and multiple linear regression analysis.
Results
After treatment, the levels of β
2
-MG and Cys-C were higher than those before treatment, and the differences were statistically significant (P<0.05). There were no statistical differences in the levels of BUN, SCr, and α
1
-MG before and after treatment (P>0.05). After treatment, the eGFR level was lower than that before treatment, and the difference was statistically significant (P<0.05). The blood concentration of Vancomycin was positively correlated with the levels of BUN, SCr, β
2
-MG and Cys-C (r=0.468, 0.612, 0.733, 0.751, P<0.05), negatively correlated with the level of eGFR (r=-0.575, P<0.05), and not correlated with the level of α
1
-MG (P>0.05).
Conclusion
Forty-eight hours after using Vancomycin, indicators such as β
2
-MG and Cys-C increases significantly and eGFR decreases, suggesting that β
2
-MG, Cys-C, and eGFR are early sensitive indicators. The blood concentration of Vancomycin is positively correlated with the levels of BUN, SCr, β

DU Xiao-zhi WANG Le
Objective
To investigate the predictive value of high sensitivity C-reactive protein and albumin ratio and plasma copeptin (CPP) on major adverse cardiovascular events (MACE) in patients with coronary heart disease and chronic heart failure.
Methods
Eighty-six hospitalized patients with coronary heart disease and chronic heart failure in People′s Hospital of Huaiyin Ji′nan from March 2018 to June 2019 were selected. According to the occurrence of MACE within 6 months, the patients were divided into the MACE group (36 cases) and the non-MACE group (50 cases). The data of all patients were collected, the serum brain natriuretic peptide (BNP), CPP levels, high-sensitivity C-reactive protein/albumin ratio and left ventricular ejection fraction (LVEF) were detected, and receiver operating characteristic (ROC) curve was used to evaluate the predictive value of high-sensitivity C-reactive protein/albumin ratio and CPP for MACE in patients with coronary heart disease and chronic heart failure.
Results
The high-sensitivity C-reactive protein/albumin ratio, plasma BNP and CPP levels of the MACE group were higher than those of the non-MACE group, and the LVEF was lower than that of the non-MACE group, the differences were statistically significant(P<0.05). The area under the curve (AUC) of MACE in patients with coronary heart disease and chronic heart failure predicted by the high-sensitivity C-reactive protein/albumin ratio was 0.826, P=0.000, 95%CI (0.735-0.917), which had certain diagnostic significance. The AUC of MACE in patients with coronary heart disease and chronic heart failure predicted by the plasma CPP was 0.714, P=0.000, 95%CI (0.604-0.824), which had certain diagnostic significance. The AUC was 0.865 in the occurrence of MACE in patients with coronary heart disease and chronic heart failure predicted by combination of high-sensitivity C-reactive protein/albumin ratio and plasma CPP, which was better than the above two independent indicators.
Conclusion
The high-sensitivity C-reactive protein/albumin ratio and CPP can be used as biomarkers for early prediction of MACE in patients with coronary heart disease and chronic heart failure.

Molecular mechanism and research progress of cell pyrolysis
LIN Wen1,2 CAO Jia-qi1,2 ZHANG Yan1,2 DING Lei1,2 CUI Qing-hua1,2▲
Pyroptosis is a pattern of atypical cell death driven by inflammasome, mediated by Caspase-1/-11/-4/5.Gasdermin family proteins are identified as pore-forming proteins acting as the "executor" of pyroptosis, which usually remain in self-suppressed state. Gasdermin D and Gasdermin E are the most key palyers in pyroptosis. In the pathway of pyroptosis, GSDMD/GSDME is cleaved into N-terminal pore-forming fragments and C-terminal inhibitory fragments by activated Caspases, membrane perforation is initiated in turn, and cell contents such as inflammatory factors are released to trigger pyroptosis. Emerging evidences support that pyroptosis plays an important role in the development of various diseases, especially in tumors. This article summarizes the underlying molecular mechanism of pyroptosis and reviews the research progress in cancer.

Research progress of the effect of orthokeratology on the accommodation function of myopic patients
WEI Jia-qi1 WANG Hui-yun2 LU Qin-kang2▲
With the development of country′s economy and changes in living habits, the prevalence of myopia in adolescents is increasing year by year, seriously endangering the physical and mental health of adolescents. China has a high prevalence of myopia, and it is urgent to prevent and control myopia among young people. For patients with rapid myopia progression, orthokeratology can effectively reshape the cornea, reduce refractive errors and inhibit the elongation of eye axis. Moreover, orthokeratology has been clinically proved to be effective in controlling the progression of myopia among young people, and its safety and reversibility have been widely recognized by scholars. However, the mechanism of orthokeratology in controlling myopia has not been clarified. Currently, peripheral refraction theory is widely accepted while some others have pointed out that orthokeratology may control myopia by improving the eye accommodation and reducing retina defocusing. This article reviews the changes in the accommodation function of myopia patients after wearing orthokeratology.

A total of 105 patients with type 2 diabetes mellitus who were treated in our hospital from January 2017 to October 2019 were selected as the research objects, 5 cases fell out and 100 cases were eventually enrolled. They were divided into three groups by the random number table method, namely Metformin alone group (group A, n=30), Metformin+Acarbose group (group B, n=35), and Metformin+Sitagliptin group (group C, n=35).The glycated hemoglobin (HbA1c), 1,5-anhydroglucose (1,5-AG), malondialdehyde (MDA), stromal cell-derived factor-1α (SDF-1α) before and after treatment were compared among the three groups, and the incidence of adverse reactions was recorded in the three groups.
Results
After treatment, the HbA1c and MDA in group B and C were lower than those in group A, and the differences were statistically significant (P<0.01). There were no significant differences in HbA1c and MDA between group B and group C after treatment (P>0.05). The 1,5-AG in group B and C was higher than that in group A at different visit periods after treatment, and the difference was statistically significant (P<0.05).The 1,5-AG in group C was higher than that in group B in visit period 2 and visit period 3, and the difference was statistically significant (P<0.05). There was no significant difference in SDF-1α in group A and group B before and after treatment (P>0.05). The SDF-1α in group C after treatment was higher than that before treatment and in group A and group B, and the difference was statistically significant (P<0.01). The total incidence of adverse reactions in group A and group C was lower than that in group B, and the difference was statistically significant (P<0.05).
Conclusion
When blood glucose is poorly controlled with Metformin alone, the combination of Sitagliptin or Acarbose both can reduce the level of HbA1c in a tantamount effect; however, the combination of Sitagliptin has smaller blood glucose fluctuations and can reduce oxidative stress and cellular damage, and can improve atherosclerosis, which is an optimal glucoselowering program.
