Expression and clinical significance of HIF-1α and COX-2 in peripheral blood and mucosal tissues of patients with ulcerative colitis

Objective
To investigate the expression and clinical significance of hypoxia-inducible factor-1α (HIF-1α)and cyclooxygenase-2 (COX-2) in peripheral blood and mucosal tissues of patients with ulcerative colitis.
Methods
From January to June in 2019, 50 patients with ulcerative colitis who were admitted to the Department of Internal Medicine of People′s Hospital of Maonan District of Maoming City and Guangdong Provincial People′s Hospital were selected as the research subjects. According to histological classification, ulcerative colitis tissue were categorized into the experimental tissue group, and normal colon tissues from the same patients (surrounding ulcerative tissue) were used as the tissue control group. Based on the classification of serology, serum from 50 patients with colitis were classified into the experimental serum group, and serum from the 50 healthy subjects were classified into the serum control group. The expression of HIF-1α and COX-2 in the serum and colonic mucosa were detected, and relationships between two indicators and clinical pathological parameters were analyzed.
Results
The average number of HIF-1α and COX-2 positive cells in the experimental tissue group were higher than those in the control tissue group, with statistical significance (P<0.05). The levels of HIF-1α and COX-2 in the experimental serum group were higher than those in the control serum group, and the differences were statistically significant (P<0.05). The levels of HIF-1α and COX-2 in mucosal tissues were positively correlated with clinical severity and endoscopic grade (r=0.812, 0.758, 0.829, 0.764,P<0.05). The level of HIF-1α and COX-2 in serum were positively correlated with clinical severity and endoscopic grade (r=0.801, 0.749, 0.812, 0.758, P<0.05).
Conclusion
The expression of HIF-1α and COX-2 in peripheral blood and mucosal tissues of patients with ulcerative colitis remarkably increase, which are positively correlated with the severity of the disease, providing a reliable basis for clinical diagnosis and treatment assessment.

Meta-analysis of the effect of Compound Methenamin Capsules in the treatment of cough after respiratory infection

Objective
To evaluate the effectiveness and safety of Compound Methenamin Capsules in the treatment of cough after respiratory infection.
Methods
Databases such as PubMed, Wanfang, CQVIP, and China National Knowledge Infrastructure (CNKI) were searched by computer from the establishment to July 3, 2019. The randomized controlled trials involving Compound Methenamin Capsules in the treatment of cough after respiratory infection were included, and Revman 5.3 was used for data analysis.
Results
There were 9 studies with a total of 1340 patients. Metaanalysis results showed that the total effective rate of the treatment group was higher than that of the control group, and the difference was statistically significant (RR=1.15, 95%CI [1.11, 1.19], P<0.000 01). The cough score of the treatment group after treatment was lower than that of the control group, and the difference was statistically significant (MD=-0.34,95%CI [-0.41, -0.28], P<0.000 01). There was no statistically significant difference in the incidence rate of adverse reactions between the two groups (RR=0.73, 95%CI [0.37, 1.47], P=0.38).
Conclusion
Compound Methenamin Capsules are effective and safe in the treatment of cough after respiratory infection. However, due to the small sample size and low quality of existing studies,more large-sample,high-quality and multi-center clinical studies are needed to confirm it.

Study on the effect of Diphenoxylate on liver tissue metabolism in rats based on GC-MS technology

Objective
To study the effect of Diphenoxylate on liver tissue metabolism in rats.
Methods
Thirty-two SD rats were randomly divided into four groups: Diphenoxylate administration group (low, medium, and high dose groups)and control group, with 8 rats in each group. The gas chromatography-mass spectrometry (GC-MS) of the liver tissue was collected and analyzed by partial least-squares discriminant analysis (PLS-DA) to identify different metabolites.
Results
Compared with the control group, the levels of these metabonomics in liver tissue such as butyric acid, propionic acid, glycerin, leucine, glycine, L-aspartic acid, glutamine, xylulose, erythrose, 9H-purine, gluconic acid, D-mannitol, galactose, and arachidonic acid increased, while the level of inositol myo-inositol decreased (P<0.05).
Conclusion
The changes of these metabolites indicate abnormalities in amino acid metabolism, lipid metabolism, and energy metabolism in rats after administration. This study can provide useful information for liver metabolism of Diphenoxylate.

Research progress of molecular pathways in lung squamous cell carcinoma

The national smoking has been at a higher level for a long time and with the prevalence of electronic cigarette in the crowd of young people, due to the close relationship between lung squamous cell carcinoma (SQCC) and smoking, smoking is one of the most important risk factors for lung SQCC, therefore, lung SQCC will occupy a pivotal position among the lung cancer population in China. Through the analysis of recent studies on genes and molecular signaling pathways related to abnormal expression of lung epithelial squamous metaplasia and canceration in databases such as PubMed and Wiley, this paper elaborated a number of molecular pathway mechanisms related to the occurrence of lung SQCC, and summarized new and more valuable experimental studies on molecular pathways related to the occurrence of lung SQCC and the progress of corresponding pathway target drugs in recent years, aiming at introducing some new developments and possible drug research directions of the current lung SQCC generation mechanism and targeted therapy.

Study on the current situation of physical restraints in hospitalized senile patients with dementia

The number of senile patients with dementia has been increasing year by year. Due to their physiological and pathological changes, unsafe behaviors often occur during hospitalization. Therefore, nurses often impose physical restraints on senile patients with dementia to ensure their hospitalization safety. However, the standard for the use of physical restraints in clinical nursing is not clear, and some physical restraints cause psychological and physical harm to patients. Therefore, this article reviews the current situation of the use of physical restraints in hospitalized senile patients with dementia, the impact of physical restraints on patients, and the measures to reduce the use of physical restraints, so as to provide effective physical restraint guidance for clinical nursing work and help patients improve hospitalization safety and the quality of life.

Study on the mechanism of Human papillomavirus E6/E7 gene in the development of cervical cancer

At present, cervical cancer is still the most common reproductive system malignant tumor in women worldwide, and 90% of patients with cervical cancer are infected with Human papillomavirus (HPV), among which high-risk HPV types 16 and 18 are the main types. High-risk HPV E6/E7 gene is the main cause of cell immortalization and tumor progression. The integration of the viral genome into the host genome is a key step for oncogenes E6 and E7. A large number of studies have confirmed that E6 and E7 genes can lead to the inactivation of two important tumor suppressor genes, like p53 and pRb, the latest researches have shown these two genes can act on more cytokines, such as cell methylation status, PI3K AKT pathway, matrix metalloproteinases (MMPs), and interleukin-18 (IL-18), and different effect on these factors ultimately lead to the occurrence, metastasis and immune escape of cancer.

Research progress of optical coherence tomography angiography in the diagnosis and treatment of primary glaucoma

With the development of modern ophthalmic techniques, the understanding on ophthalmic diseases has become increasingly accurate, and the requirements for its testing method have continued rising. Optical coherence tomography angiography (OCTA) is a new non-invasive ophthalmic examination technique used for glaucoma in recent years, which can not only qualitatively analyze micro-flow and vascular structure, but also quantitatively calculate the blood flow in different layers of retina. In addition, OCTA can also be used to test and dynamically observe the vascular density in different areas around the optic disc and macula, which provides new methods and ideas for the understanding of glaucoma′s pathogenesis, as well as the diagnosis and monitoring of glaucoma. Studies on the application of OCTA in the diagnosis and treatment of primary glaucoma are summarized in this paper, so as to provide a reference for investigating the hemorrheology of primary glaucoma.

Application of high-flow oxygen humidification in patients with craniocerebral injury combined with gas-cut

Objective
To investigate the application of high-flow oxygen humidification in patients with craniocerebral injury combined with gas-cut.
Methods
From September 2018 to February 2019, 68 patients with head injury and tracheotomy treated in First Affiliated Hospital of Gannan Medical University were selected as subjects. The control group was treated with routine airway humidification and the study group was treated with high flow oxygen humidification.The humidification effect, complications and sputum aspiration times were compared between the two groups.
Results
The humidification effect in the study group was better than that in the control group, and the difference was statistically significant (P<0.05). The total incidence of complications of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The number of sputum aspiration in the study group was fewer than that in the control group was lower than that of the control group, and the difference was statistically significant (P<0.05).
Conclusion
High flow oxygen humidification is performed in patients with craniocerebral injury complicated with air resection, which can effectively improve airway humidification, reduce complications and reduce sputum aspiration.

Application effect of bedside indwelling nasal jejunum tube nutrition in critically ill patients

Objective
To study the application effect of bedside indwelling nasal jejunum nutrition in critically ill patients.
Methods
A total of 80 critically ill patients treated in our hospital from March 2017 to March 2019 were selected as the research subjects, they were divided into control group and experimental group according to random number table method, 34 cases in each group, the patients in the control group were treated with nasogastric tube nutrition, and the patients in the experimental group were given nasal jejunum tube nutrition therapy. The changes of nutritional indexes, immune function indicators at 1, 5, 10 d after treatment, incidence of postoperative complications and intubation time between the two groups were compared.
Results
After treatment, the levels of serum hemoglobin (Hb) and serum albumin (ALB) in the experimental group were higher than those in the control group, and the differences were statistically significant (P<0.05). There were no significant differences in IgM and IgG between the two groups at 1 and 5 days after treatment (P>0.05). IgM and IgG were higher than those before treatment at 10 days after treatment, and the experimental group was higher than the control group, the differences were statistically significant (P<0.05). The incidence rates of postoperative obstruction, diarrhea, gastrointestinal hemorrhage, and reflux in the experimental group were lower than those in the control group, and the intubation time was longer than that in the control group, and the differences were statistically significant (P<0.05).
Conclusion
Bedside indwelling nasal jejunum nutrition can effectively improve the nutritional status of critically ill patients, promote the recovery of immune function after treatment, and reduce the incidence of complications after intubation.

Application effect of internal and external application combined with medical glue in small wound healing after surgery

Objective
To investigate the application effect of internal and external application combined with medical glue on small wound healing after surgery.
Methods
A total of 168 patients with surgical procedures in our hospital from March 20 to March 2019 were selected as subjects. They were divided into experimental group and control group according to the random sampling principle of odd and even numbers, 84 cases in each group. The patients in the control group were given routine internal and external nursing care. The patients in the experimental group were combined with the medical glue method on the basis of routine nursing. The clinical effect of the two groups of patients after different nursing modes were compared.
Results
The recovery time of the experimental group was shorter than that of the control group, the total incidence of postoperative complications was lower than that of the control group, the differences were statistically significant (P<0.05). The total satisfaction of the patients in the experimental group was higher than that of the control group, the psychological scores of the experimental group were lower than those of the control group, the differences were statistically significant (P<0.05).
Conclusion
The internal and external application of medical glue nursing for patients with small wounds after surgery can significantly improve the healing effect of patients,reduce the complications of postoperative wound healing, improve the satisfaction of patients′ care, and also contribute to the positive development of patients′ psychology. The clinical effect is significant and worthy of promotion and application.

Application effect of posterior ligament complex preservation combined with posterior vertebral bone graft and short segment fixation in patients with thoracolumbar fracture

Objective
To investigate the application effect of traumatic vertebral fixation with posterior ligament complex preservation combined with posterior vertebral bone graft and short segment fixation in patients with thoracolumbar fracture.
Methods
The clinical data of 32 patients with thoracolumbar fracture admitted to our hospital from November 2017 to June 2019 were retrospectively analyzed. According to different treatment methods, they were divided into study group (16 cases) and control group (16 cases). The control group was treated with injured vertebra fixation and preservation of posterior ligament complex. The study group was treated with injured vertebra fixation and preservation of posterior ligament complex. The treatment of syndesmosis combined with posterior vertebral bone graft and short segment fixation. The clinical effect and complications were compared between the two groups.
Results
The total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P<0.05). The total incidence of postoperative complications in the study group was lower than that in the control group,and the difference was statistically significant (P<0.05). The time of operation, time of getting out of bed and time of hospitalization in the study group were shorter than those in the control group, and the amount of intraoperative blood loss was less than that in the control group, with statistically significant differences (P<0.05).
Conclusion
For patients with thoracolumbar fractures, the treatment effect is ideal, which can effectively reduce the incidence of postoperative complications, reduce the amount of intraoperative blood loss, and shorten the length of hospitalization, which can be popularized and applied clinically.

Clinical effect of whole-lung lavage in the treatment of pneumoconiosis

Objective
To observe the clinical effect of whole-lung lavage (WLL) in the treatment of pneumoconiosis.
Methods
A total of 64 pneumoconiosis patients underwent WLL from January 2016 to April 2019 in our hospital were included. The symptom scores as well as the values of partial pressure of oxygen (PaO
2
), partial pressure of carbon dioxide (PaCO
2
), maximal ventilatory volume (MVV), forced vital capacity (FVC) and forced expiratory volume in one second (FEV
1
) were appraised.
Results
After 7 d of lavage, the symptom scores of cough, expectoration and limitation of motion were (0.98±0.10), (1.05±0.11) and (1.02±0.10) points, which were lower than those before lavage ([2.05±0.18],[2.61±0.34], [2.10±0.19]points), the differences were statistically significant (P<0.05). The value of PaO
2
after 7 d of lavage was (12.37±1.13) kPa, which was higher than that before lavage ([9.49±1.25]kPa) (P<0.05). The value of PaCO
2
after 7 d of lavage was (4.68±0.20) kPa, which was lower than that before lavage ([5.19±0.40]kPa) (P<0.05). The MVV([86.10±1.10]L), FVC ([2.99±0.16]L) and FEV
1
([2.89±0.06]L) were higher than those before lavage ([79.69±1.20),[2.80±0.10]and [2.67±0.10]L) (P<0.05).
Conclusion
WLL is proven to be effective, which can improve patients′symptoms, pulmonary function and blood gas indicators.

Clinical effect of Nicorandil in the treatment of coronary slow flow

Objective
To investigate the clinical effect of Nicorandil in the treatment of coronary slow flow (CSF), thus providing reference for more effective treatment in CSF patients.
Methods
A total of 126 patients who underwent coronary angiography (GAG) and diagnosed with CSF in Wuhan Third Hospital from December 2016 to June 2018 were selected as the research objects, and they were divided into control groups (64 cases) and combined treatment group (62 cases) according to the random number table method. Patients in the control group were given conventional treatment,while patients in the combined treatment group were given Nicorandil treatment on the basis of conventional treatment.Patients were followed up once a month for 6 months. The changes of symptoms, blood routine, inflammatory factors,homocysteine, and platelet aggregation rate of the two groups of patients before and after treatment were observed, and the incidence of adverse reactions between the two groups of patients after treatment was compared.
Results
After treatment, the total cholesterol, triacylglycerol, high-sensitivity C-reactive protein, interleukin-1, interleukin-6, endothelin-1, homocysteine, and platelet aggregation rate in both groups were lower than those before treatment, nitric oxide was higher than that before treatment, and the differences were statistically significant (P<0.05). The total cholesterol, triacylglycerol, high-sensitivity C-reactive protein, interleukin-1, interleukin-6, endothelin-1, homocysteine, and platelet aggregation rate after treatment in the combined treatment group were lower than those in the control group, nitric oxide was higher than that in the control group, and the differences were statistically significant (P<0.05). After treatment, the incidence rates of chest pain, arrhythmia and chest tightness of the two groups of patients were lower than those before treatment,and the differences were statistically significant (P<0.05).The incidence rates of chest pain and chest tightness in the combined treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05). There was no significant difference in the total incidence rate of adverse reactions between the two groups (P>0.05).
Conclusion
The combination of Nicorandil and conventional treatment can improve symptoms and inflammatory status in CSF patients more significantly.

Clinical effect of Sacubatrol Valsartan Sodium in the treatment of heart failure with preserved ejection fraction

Objective
To explore the clinical effect of Sacubatrol Valsartan Sodium in the treatment of heart failure with preserved ejection fraction (HFpEF).
Methods
A total of 78 patients with HFpEF admitted to our hospital from March 2018 to May 2019 were enrolled as study subjects. The participants were divided into the control group (n=39) and the experimental group (n=39) according to the random number table method. The control group was given routine treatment, while the experimental group was given Sacubatrol Valsartan Sodium. The therapeutic effect, cardiac function indexes (early diastolic velocity of mitral valve [E], early diastolic velocity of mitral annulus [e′], E/peak diastolic velocity of mitral valve [E/A], E/e′), N-terminal B-type natriuretic peptide (NT-proBNP), 6 min walking test distance (6MWT),quality of life score before and after treatment and incidence of adverse reactions were compared between the two groups.
Results
The total effective rate of treatment in the experimental group was 92.31%, which was higher than that in the control group accounting for 79.49%, with statistical difference (P<0.05). There were no significant differences in cardiac function indexes, NT-proBNP, 6MWT and quality of life score between the two groups before treatment (P>0.05). The E, e′ and E/A of the two groups after treatment were higher than those before treatment, the 6MWT was farther than that before treatment, with statistical differences (P<0.05). The E/e′, NT-proBNP and quality of life scores in the two groups after treatment were lower than those before treatment, with statistical differences (P<0.05). The E, e′and E/A in the experimental group after treatment were higher than those in the control group, the 6MWT was farther than that in the control group, with statistical differences (P<0.05). The E/e′, NT-proBNP and quality of life scores in the experimental group after treatment were lower than those in the control group, with statistical differences (P<0.05). There was no significant difference in the total incidence rate of adverse reactions between the two groups (P>0.05).
Conclusion
The effect of Sacubatrol Valsartan Sodium is remarkable in the treatment of HFpEF. It can enhance cardiac function, and increase the exercise tolerance and quality of life in patients of HFpEF without increasing the incidence rate of adverse reaction, whcih is worthy of clinical application.

Therapeutic effect of senile hypertension under the assist of ambulatory blood pressure monitoring

Objective
To explore the therapeutic effect of senile hypertension under the assist of ambulatory blood pressure monitoring (ABPM).
Methods
A total of 133 senile patients with hypertension admitted to our hospital from January 2016 to December 2018 were selected as the research objects, and they were divided into the control group (66 cases)and the experimental group (67 cases) according to the random number table method. The patients in the control group were guided according to the blood pressure in the clinic, and the drugs were taken orally in the morning. The patients in the experimental group were guided according to the ABPM results, and the drug treatment plan was decided according to the patients′ ABPM results. The therapeutic effect of the two groups was compared.
Results
The blood pressure control rate of patients in the experimental group after 4 weeks of treatment was higher than that in the control group,and the difference was statistically significant (P<0.05). The proportion of morning peak blood pressure of patients in the experimental group after 12 weeks of treatment was lower than that in the control group, the proportion of dippershaped blood pressure was higher than that in the control group, and the differences were statistically significant (P<0.05). The daily mean systolic blood pressure (dMSBP), 24-hour mean systolic blood pressure (24 h MSBP) and night mean systolic blood pressure (nMSBP) of the patients in the experimental group after 12 weeks of treatment were all lower than those in the control group, and the differences were statistically significant (P<0.05).
Conclusion
The therapeutic effect of senile hypertension is better under the assist of ABPM.

Effect of kinesio taping combined with conventional rehabilitation training in the treatment of shoulder subluxation in stroke patients with hemiplegia and the influence of side upper limb motor function

Objective
To investigate the effect of kinesio taping combined with conventional rehabilitation training in treating shoulder subluxation of stroke patients with hemiplegia and the influence of affected side upper limb motor function.
Methods
A total of 80 cases of shoulder subluxation in stroke patients with hemiplegia admitted to our hospital from March 2016 to March 2019 were selected and divided into the observation group and the control group according to random number table method, 40 cases in each group. The control group was given placebo patch combined with conventional rehabilitation training, while the observation group was given kinesio taping combined with conventional rehabilitation therapy. The distance of acromio-humeral interval (AHI value) and simplified Fugl-Meyer Assessmentupper extremity(FMA-UE) were compared between the two groups.
Results
There were no significant differences in the AHI value between the control group after treatment and before treatment (P>0.05). The AHI value of the observation group after treatment was longer than that before treatment (P<0.05). The AHI value of the observation group after treatment was longer than that of the control group (P<0.05). The FMA-UE scores of the two groups after treatment were higher than those before treatment (P<0.05). The FMA-UE scores of the observation group after treatment were higher than those of the control group (P<0.05).
Conclusion
Kenesio taping combined with conventional rehabilitation training can improve the shoulder subluxation of stroke patients with hemiplegia, improve the upper limb motor function of the affected side, and improve the clinical treatment effect, which is worthy of clinical promotion and application.

Application effect of Urapidil combined with Esmolol for controlled hypotension in patients with intracranial aneurysm interventional surgery

Objective
To investigate the application effect of Urapidil combined with Esmolol for controlled hypotension in patients with intracranial aneurysm interventional surgery.
Methods
A total of 220 patients with intracranial aneurysms admitted to our hospital from January 2017 to January 2019 were selected as the research objects, and the patients were divided into observation group (110 cases) and control group (110 cases) according to the random number table method. Patients in the observation group were given Urapidil combined with Esmolol for controlled hypotension,and patients in the control group were given Sodium Nitroprusside for controlled hypotension. The intraoperative blood pressure stability, ventilation, oxygenation scores, total amount of Isoflurane inhalation and blood pressure control were compared, and the postoperative mean arterial pressure, hemorheology, normal electrocardiogram score, normal blood pressure monitoring score and end-tidal carbon dioxide partial pressure were compared between the two groups.
Results
The intraoperative blood pressure stability, ventilation and oxygenation scores in the observation group were higher than those in the control group, and the total amount of Isoflurane inhalation was less than that in the control group,with statistically significant differences (P<0.05). The postoperative mean arterial pressure, hemorheology and end-tidal carbon dioxide partial pressure in the observation group were lower than those in the control group, and the normal electrocardiogram and normal blood pressure monitoring scores were higher than those in the control group, with statistically significant differences (P<0.05). The stability score of blood pressure in the observation group was higher than that in the control group, the fluctuation range of blood pressure was smaller than that in the control group, and the dosage of antihypertensive drugs was less than that in the control group, with statistically significant differences (P<0.05).
Conclusion
Urapidil combined with Esmolol in interventional surgery for intracranial aneurysms has a good effect in controlled hypotension, and the stability of blood pressure is good, which is conducive to achieving good surgical results.

Application effect of different anterior communicating aneurysms direct resection scope in patients with anterior communicating aneurysms

Objective
To investigate the application effect of different anterior communicating aneurysms direct resection scope in patients with anterior communicating aneurysms.
Methods
A total of 52 patients with anterior communicating aneurysms admitted to our hospital from January 2017 to January 2018 were enrolled. The patients were divided into control group (22 cases) and study group (30 cases) according to different resection areas. The patients in the control group were treated with standard direct resection (resection range ≤1 cm

),and the patients in the study group were treated with a large range of direct resection (resection range >1 cm
3
). At the same time, 30 healthy people with normal physical examination results were selected as the healthy group. The probability of cognitive dysfunction between the control group and the study group was compared, and scores of mini-mental state examination (MMSE) and memory(simplified Rey-Osterreith complex figure test [ROCFT], World Health Organization-University of California Los Angeles auditory vocabulary learning test [WHO-UCLAAVLT]) were compared among the three groups.
Results
The probability of cognitive dysfunction in the study group was higher than that in the control group, and the difference was statistically significant (P<0.05). The recall ability, memory ability, language ability, computing ability and attention, orientation and total scores of MMSE in the study group were lower than those in the control group and in the healthy group, the recall ability, memory ability, language ability, computing ability and attention, orientation and total scores of MMSE in the control group were lower than those in the healthy group, and the differences were statistically significant(P<0.05). The simplified ROCFT scores such as word recognition, immediate memory and long-term delayed memory of the study group were lower than those of the control group and the healthy group, the simplified ROCFT scores of the control group such as word recognition, immediate memory and long-delayed memory were lower than those of the healthy group, with statistically significant differences (P<0.05). The WHO-UCLAAVLT scores such as short-term delayed memory, long-term delayed memory and immediate memory of the study group were lower than those of the control group and the healthy group, the WHO-UCLAAVLT scores such as short-term delayed memory, long-term delayed memory and immediate memory of the control group were lower than those of the healthy group, and the differences were statistically significant (P<0.05).
Conclusion
The influence of cognitive impairment is not obvious when the direct resection of the anterior communicating aneurysm scope is smaller. A larger scope direct anterior communicating aneurysm has a greater impact on the postoperative cognitive impairment. The scope of direct aneurysm resection has a certain impact on postoperative cognitive impairment.

Clinical effect of Mecobalamin in the treatment of cerebral infarction patients with hyperhomocysteinemia

Objective
To investigate the clinical effect of Mecobalamin in the treatment of cerebral infarction patients with hyperhomocysteinemia.
Methods
Sixty-four cerebral infarction patients with hyperhomocysteinemia who were treated in our hospital from February 2018 to February 2019 were selected as research subjects. They were divided into the control group and the observation group according to the random number table method, with 32 cases in each group. The patients in the control group were given conventional cerebral infarction therapy, and the patients in the observation group were given Mecobalamin on the basis of the control group. The plasma homocysteine (Hcy) concentrations and national institutes of health stroke scale (NIHSS) scores before and after treatment were observed, and the clinical efficacy and incidence of adverse reactions during treatment were compared in the two groups.
Results
The total effective rate of treatment in the observation group was 93.75%, which was higher than that in the control group accounting for 78.13%, and the difference was statistically significant (P<0.05). The plasma Hcy concentrations of the two groups of patients after treatment were lower than those before treatment, and the Hcy concentration of the observation group after treatment was lower than that of the control group, the differences were statistically significant (P<0.05). The NIHSS scores of the two groups of patients after treatment were lower than those before treatment, and the NIHSS score of the observation group after treatment was lower than that of the control group, the differences were statistically significant (P<0.05). There was no significant difference in the total incidence rate of adverse reactions between the two groups (P>0.05).
Conclusion
Methylcobalamin can remarkably improve the clinical effect in the treatment of cerebral infarction patients with hyperhomocysteinemia, greatly reduce plasma Hcy concentration, and effectively improve the neurological function.

Influence of modified electroconvulsive therapy combined with Risperidone on patients with schizophrenia

Objective
To investigate the influence of modified electroconvulsive therapy combined with Risperidone on patients with schizophrenia.
Methods
A total of 80 patients with schizophrenia who admitted in the hospital from January 2017 to June 2019 were selected as research subjects, and they were divided into the control group (40 cases) and the observation group (40 cases) according to random number table methods. The control group orally took Risperidone,and the observation group was treated with modified electroconvulsive therapy on the based of the control group. The positive and negative syndrome scale (PANSS) and Wechsler memory scale (WMS) scores of the two groups before treatment and after treatment for 28 d were compared.
Results
After treatment, the PANSS positive score and negative score of the observation group were lower than those of the control group, and the WMS score of the observation group was higher than that of the control group, and the differences were statistically significant (P<0.05).
Conclusion
Modified electroconvulsive therapy combined with Risperidone in the treatment of patients with schizophrenia can decrease PANSS score and increase WMS score, which is worthy of clinical promotion and application.

Clinical effect of Memantine Hydrochloride combined with Donepezil Hydrochloride in the treatment of psychobehavioral symptoms in patients with Alzheimer′s disease

Objective
To explore the clinical effect of the combination of Memantine Hydrochloride and Donepezil Hydrochloride in the treatment of psychobehavioral symptoms in patients with Alzheimer′s disease.
Methods
A total of 70 patients with Alzheimer′s disease admitted to Shenyang Red Cross Hospital from January 2017 to July 2019 were selected as the study objects. They were divided into the control group and the observation group according to the method of random number table, 35 cases in each group. The control group was treated with Memantine Hydrochloride, and the observation group was treated with the combination of Memantine Hydrochloride and Donepezil Hydrochloride. The psychobehavioral symptoms, the total clinical effective rate and the total incidence of adverse reactions were compared between the two groups.
Results
The psychobehavioral symptoms of the observation group were lighter than those of the control group, the differences were statistically significant (P<0.05). The total clinical effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05).
Conclusion
The combination of Memantine Hydrochloride and Donepezil Hydrochloride has a significant clinical effect in the treatment of psychobehavioral symptoms in patients with Alzheimer′s disease. It can not only improve the psychobehavioral symptoms, but also improve the treatment effect, and will not increase the adverse reactions. Therefore, the drug safety is high, which is worthy of clinical promotion.

Influence of exercise therapy on body weight and blood glucose in patients with schizophrenia complicated with diabetes mellitus

Objective
To explore the influence of exercise therapy on body weight and blood glucose in patients with schizophrenia complicated with diabetes mellitus, in order to provide new ideas for clinical treatment.
Methods
From October 2017 to October 2018, 60 patients with schizophrenics admitted to our hospital were selected as the research objects, and they were divided into the control group (n=31) and the observation group (n=29) according to the method of drawing lots. The control group was treated with routine treatment of schizophrenia complicated with diabetes mellitus, the observation group was treated with exercise therapy on the basis of the control group. The body mass index(BMI), blood glucose Glu level and blood lipid index were compared between the two groups.
Results
There were no significant difference in the levels of BMI and Glu between the two groups before treatment(P>0.05).The levels of BMI and Glu after 6 months of intervention in the two groups were lower than that before treatment(P<0.05).The levels of BMI and Glu after 6 months of intervention in the obsrevation group were lower than those of the control group (P<0.05). There were no significant difference in the levels of total cholesterol (TC) and triglyceride (TG), high-density lipoprotein(HDL) and low-density lipoprotein (LDL) between the two groups before treatment (P>0.05). The levels of TC, TG and LDL in the two groups after 6 months of treatment were lower than those before treatment, and the level of HDL in the two groups after 6 months of treatment was higher than that before treatment (P<0.05). The levels of TC, TG and LDL in the observation group after 6 months of treatment were lower than those in the control group, and the level of HDL in the observation group after 6 months of treatment was higher than that before treatment (P <0.05). The total compliance rate of the observation group was higher than that of the control group, the difference was statistically significant(P<0.05).
Conclusion
Exercise therapy can effectively reduce the body weight of patients with schizophrenia,improve the blood lipid level, and stabilize the blood glucose of diabetic patients at the normal level, and improve the treatment compliance.

Effect of Atorvastatin combined with Danhong Injection in the treatment of cerebral insufficiency caused by vertebrobasilar arteriosclerosis

Objective
To analyze the effect of Atorvastatin combined with Danhong Injection in the treatment of cerebral insufficiency caused by vertebrobasilar arteriosclerosis.
Methods
A retrospective study was conducted in 106 patients with vertebrobasilar arteriosclerosis insufficiency treated in our hospital from June 2018 to October 2019. The patients were divided into control group and study group according to different treatment methods, among them, 53 patients in the control group were treated with Difenidol Hydrochloride Tablets, and 53 patients in the study group were treated with Atorvastatin combined with Danhong Injection, the maximal blood flow velocity and total adverse reaction rate of right basilar artery and left and right vertebral artery during systole were further analyzed.
Results
After treatment, the maximal arterial blood flow velocity of the study group was higher than that of the control group, the difference was statistically significant (P<0.05). The total adverse reaction rate of the study group was lower than that of the control group, and the total effective rate of the study group was 94.34%, higher than that of the control group(66.02%), the differences were statistically significant (P<0.05).
Conclusion
The effect of Atorvastatin combined with Danhong Injection in the treatment of vertebrobasilar arteriosclerosis insufficiency is ideal, it can obviously improve the clinical symptoms such as visual acuity, dizziness, etc. , it is worth popularizing in clinic.

Effect of vocational rehabilitation case management on rehabilitation effect of community schizophrenia patients

Objective
To explore the effect of case management on the rehabilitation of community schizophrenia patients.
Methods
A total of 120 patients with schizophrenia who were cured and discharged from our hospital were selected. According to the hospitalization file number, the single number was the control group and the double number was the observation group, 60 cases in each group. The control group implemented community case management, the observation group implemented vocational rehabilitation case management, and the two groups′ daily living activity scale (ADL) scores, inpatient psychiatric patients′ rehabilitation effect assessment scale (IPROS) scores, disease recurrence rate, and reemployment rate were compared.
Results
There were no significant differences in ADL and IPROS scores between the two groups before intervention (P>0.05). At the end of 3 and 6 months of intervention, the ADL scores in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.01). At the end of 6 months of intervention, the total IPROS score and the scores of all factors in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.01). Comparing the recurrence rates between the two groups, the difference was not statistically significant at the end of 3 months of intervention (P>0.05); at the end of 6 months of intervention, the recurrence rate in the observation group was lower than that in the control group, and the reemployment rate was higher than the control group, the differences were statistically significant (P<0.05).
Conclusion
Occupational rehabilitation case management improves the ability of interpersonal communication and social work of discharged schizophrenia patients, improves the ability of patients to take care of themselves, stabilizes the disease, reduces the recurrence of the disease, and improves the reemployment rate of patients.

Feasibility of Holmium laser drainage under flexible ureteroscope in the treatment of renal cystic diseases

Objective
To explore the feasibility of Holmium laser drainage under flexible ureteroscope in the treatment of renal cystic diseases.
Methods
A total of 40 patients with renal cystic diseases treated in our hospital from March 24, 2018 to June 25, 2019 were selected as the research subjects. They were divided into the observation group (n=20)and the control group (n=20) according to the random number table method. In the observation group, Holmium laser drainage under flexible ureteroscope was used, and in the control group, laparoscopic cyst windowing decompression was applied. The surgical conditions, the decrease of hemoglobin, the increase of serum creatinine, and the total incidence of complications were compared between the two groups.
Results
In the observation group, the operation time,postoperative intestinal recovery time, postoperative hospital stay, postoperative time of getting out of bed were all shorter than those of the control group, and the differences were statistically significant (P<0.05). In the observation group,the hemoglobin decreased value, intraoperative blood loss less than that of the control group, and the serum creatinine increased value were both lower than those of the control group, with statistical significance (P<0.05). The total incidence of complications in the observation group was 5.00%, which was lower than that of the control group accounting for 30.00%, with significant difference (P<0.05).
Conclusion
Holmium laser drainage under flexible ureteroscope has good feasibility in treating renal cystic lesions. It can shorten the operation time, reduce the incidence of complications,and promote patient′s rehabilitation, which is worthy of clinical promotion.

Comparison of the effect of 14 -16F micro -channel percutaneous nephrolithotomy and traditional percutaneous nephrolithotomy in the treatment of upper urinary tract calculi

Objective
To explore the effect of 14-16F micro-channel percutaneous nephrolithotomy and traditional percutaneous nephrolithotomy in the treatment of upper urinary tract calculi.
Methods
From June 2017 to October 2019, 70 patients with upper urinary tract calculi were selected. According to the random number table method, they were divided into the study group (n=35) and the control group (n=35). The study group was treated with 14-16F microchannel percutaneous nephrolithotomy, while the control group was treated with traditional percutaneous nephrolithotomy. The operation time, intraoperative bleeding, postoperative bleeding, visual analogue scale (VAS), hospitalization time, stone clearance rate, postoperative blood transfusion rate and total complication rate were compared between the two groups.
Results
The operation time of the study group was longer than that of the control group ([95.63±11.25]min vs. [74.49±9.73]min), and the hospitalization time of the study group was shorter than that of the control group ([6.72±1.25]d vs. [8.85±1.64]d), the differences were statistically significant (P<0.05). The amount of intraoperative bleeding,the amount of postoperative bleeding and VAS score of the study group were lower than those of the control group([49.56±8.72]ml vs. [63.05± 8.54]ml, [22.74±6.42]ml vs. [28.437.15]ml, [2.96±0.77]points vs. [3.72±0.86]points),the differences were statistically significant (P<0.05). There were no significant differences in stone clearance rate(91.4% vs. 94.3%) and blood transfusion rate (2.9% vs. 8.6%) between the two groups (P>0.05). The total incidence of complications in the study group was lower than that in the control group (5.7%vs.17.1%),the difference was statistically significant (χ

Conclusion
14-16F micro-channel percutaneous nephrolithotomy have less trauma and lower incidence of postoperative complications, which is conducive to promoting postoperative recovery and shortening hospital stay.

Clinical effect comparison of Febuxostat and Allopurinol in the treatment of chronic kidney diseases combined with hyperuricemia

Objective
To compare the clinical effect of Febuxostat and Allopurinol in the treatment of chronic kidney disease (CKD) combined with hyperuricemia (HUA).
Methods
A total of 60 patients with CKD (stage 2-4) combined with HUA who were treated in our hospital from January 2017 to May 2019 were selected as research subjects. They were randomly divided into observation group (n=30) and control group (n=30) based on odd/even number. On the basis of routine treatment, patients in the observation group were treated with Febuxostat to reduce uric acid, and in the control group, patients were treated with Allopurinol to reduce uric acid. The treatment lasted for six months. The levels of blood uric acid, serum creatinine, blood urea nitrogen and the glomerular filtration rate (eGFR) before and after treatment were compared in the two groups. The total effective rate of treatment, incidence rates of eGFR decrease>10%and adverse reaction were compared between the two groups.
Results
The total effective rate of treatment in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). The incidence rate of eGFR decrease>10% of patients in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).The levels of blood uric acid,serum creatinine and blood urea nitrogen of the two groups after treatment were lower than those of their respective group before treatment, and the differences were statistically significant(P<0.05).After treatment,the levels of blood uric acid,serum creatinine and blood urea nitrogen in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). There was no statistically significant difference in eGFR between the two groups after treatment (P>0.05).There was no statistically significant difference in the total incidence rate of adverse reactions between the two groups(P>0.05).
Conclusion
Both Febuxostat and Allopurinol can effectively treat patients with mild or moderate renal insufficiency combined with HUA. They can markedly reduce blood uric acid levels and improve renal function. The clinical effect of Febuxostat is more obvious than that of Allopurinol. No serious adverse reactions occur in either group.

Correlation analysis of type 2 diabetic nephropathy with blood glucose fluctuation and oxidative stress

Objective
To explore the correlation between type 2 diabetic nephropathy and blood glucose fluctuation and oxidative stress.
Methods
A total of 90 patients with type 2 diabetes who were admitted to our hospital from January 2015 to December 2017 were selected. According to the 24-hour urine microalbumin (UAlb) determination results, they were divided into the normal UAlb group (43 cases) and the high UAlb group (47 cases). General clinical data, blood glucose, blood lipid and other biochemical indicators of patients in the 2 groups were recorded, and the levels of oxidative stress index superoxide dismutase (SOD) and malondialdehyde (MDA) were detected. The seven-stage self-monitoring of blood glucose was improved, and the standard deviation of blood glucose level (SDBG) was calculated.
Results
Fasting blood glucose (FBG), glycosylated hemoglobin (HbA1c), systolic blood pressure, diastolic blood pressure, course of diabetes, SDBG, SOD, MDA, and UAlb of the two groups were compared, and the differences were statistically significant (P<0.05). The results of correlation analysis showed that UAlb was positively correlated with the course of disease, SDBG and MDA in patients with type 2 diabetes (r=0.298, 0.435, 0.673, P<0.05). There was a negative correlation between UAlb and SOD (r=-0.716, P<0.05). SOD was negatively correlated with SDBG (r=-0.294, P<0.05). MDA was positively correlated with SDBG (r=0.282, P<0.05).
Conclusion
The course of diabetes, the fluctuation of blood glucose and the level of oxidative stress are correlated with the occurrence and development of diabetic nephropathy.

Clinical application effect of laparoscopic treatment of thyroid benign tumors by small cut diameter before neck

Objective
To investigate the clinical application effect of laparoscopic treatment of thyroid benign tumors by small cut diameter before neck.
Methods
A total of 72 patients with thyroid benign tumors admitted to our hospital from January 2017 to December 2018 were selected as the research subjects. According to the random number table method, they were divided into study group (36 cases) and control group (36 cases). The research group used the laparoscopic thyroid surgery by small cut diameter before neck. The control group used traditional thyroid surgery. The total effective rate, total incidence of complications, recurrence rate, operation time, hospital stay, intraoperative blood loss and overall satisfaction with incision scar were compared between the two groups.
Results
The total effective rate of treatment in the study group was higher than that in the control group, and the difference was statistically significant(P<0.05). The total incidence of complications and recurrence rate of patients in the study group were lower than those in the control group, and the differences were statistically significant (P<0.05). The overall satisfaction with incision scar of patients in the study group was higher than that in the control group, and the difference was statistically significant (P<0.05). The operation and hospitalization time of patients in the study group were shorter than those in the control group, and the differences were statistically significant (P<0.05). Intraoperative blood loss in the study group was less than that in the control group, and the difference was statistically significant (P<0.05).
Conclusion
The clinical treatment of patients with thyroid benign tumors, the laparoscopic thyroid surgery by small cut diameter before neck,can shorten the operation and the length of hospital stay, with small wound, less bleeding, the patient satisfaction of incision scar is higher, and the treatment effect is obvious, the postoperative complications and the recurrence rate is low,more safe and reliable, so it is the value of clinical application of promotion.

Risk factors and correlation of type 2 diabetes mellitus complicated with hyperuricemia

Objective
To investigate the risk factors and correlation of type 2 diabetes mellitus (T2DM) complicated with hyperuricemia.
Methods
A total of 40 cases with T2DM complicated with hyperuricemia from July 2017 to October 2019 in our endocrinology department were selected as the hyperuricemia group, while 40 cases of T2DM patients without hyperuricemia at the corresponding period were selected as the normal uric acid group. The related data in the two groups were analyzed. Multivariate Logistic regression was used to analyze the independent risk factors of T2DM complicated with hyperuricemia, and the correlation between the prevalence rate of T2DM complicated with hyperuricemia and the independent risk factors was analyzed.
Results
Univariate analysis showed that the proportion of the age >50 years old, the course of disease >5 years, smoking history, drinking history and T2DM family history, and the incidence rates of diabetic foot, diabetic nephropathy, diabetic peripheral neuropathy, hyperlipidemia in the hyperuricemia group were higher than those of the normal uric acid group, the differences were statistically significant (P<0.05). There was no significant difference in gender between the two groups (P>0.05). Multivariate Logistic regression analysis showed that the influencing factors of T2DM complicated with hyperuricemia included smoking history (β=1.83, OR=1.96, 95%CI=1.03-7.45), drinking history (β=2.92, OR=6.86, 95%CI=1.59-9.24), diabetic foot (β=2.34, OR=2.81, 95%CI=1.21-4.40), diabetic nephropathy (β=0.89, OR=2.44,95%CI=1.38-4.31) and diabetic peripheral neuropathy (β=1.71, OR=5.52, 95%CI=3.10-9.83) were independent risk factors for T2DM complicated with hyperuricemia (P<0.05). The prevalence of T2DM complicated with hyperuricemia were positively correlated with smoking time, drinking time, diabetic foot course, diabetic nephropathy course and diabetic peripheral neuropathy (r=0.893, 0.839, 0.718,0.925, 0.623, 0.541, P<0.05).
Conclusion
For T2DM patients with smoking, drinking, diabetic foot, diabetic nephropathy, diabetic peripheral neuropathy, the possibility of hyperuricemia should be alerted.

Correlation between the ratio of triglyceride to high density lipoprotein cholesterol and patients with type 2 diabetes complicated with osteoporosis

Objective
To investigate the correlation between the ratio of triglyceride (TG) to high density lipoprotein cholesterol (HDL-C) and patients with type 2 diabetes complicated with osteoporosis.
Methods
A total of 186 hospitalized patients with type 2 diabetes in our hospital from April 2018 to April 2019 were selected as subjects. According to the differences in bone mineral density of lumbar spine and proximal end of left femur detected by dual energy x-ray absorptiometry, the patients were divided into normal bone mass group (64 cases), osteopenia group (77 cases) and osteoporosis group(45 cases).Venous blood was collected from patients in the three groups,and fasting blood glucose(FBG),uric acid (UA), TG, total cholesterol (TC), HDL-C, low-density lipoprotein cholesterol (LDL-C), cystatin C (CysC), and serum glycosylated hemoglobin (HbA1c) were detected. The correlation between the ratio of TG/HDL-C and bone mineral density was analyzed by One-way analysis of variance and Pearson correlation.
Results
The body mass index(BMI), UA, TG and TG/HDL-C of the osteoporosis group were lower than those of the osteopenia group and the normal bone mass group, and the differences were statistically significant (P<0.05). The duration of diabetes in the osteoporosis group was longer than that in the osteopenia group and the normal bone mass group, HDL-C, LDL-C and CysC were higher than those in the osteopenia group and the normal bone mass group, and the differences were statistically significant (P<0.05). BMI, UA, TG and TG/HDL-C in the osteopenia group were lower than those in the normal bone mass group, with statistically significant differences (P<0.05). The duration of diabetes in the osteopenia group was longer than that in the normal bone mass group, and HDL-C, LDL-C and CysC were higher than those in the normal bone mass group,with statistically significant differences(P<0.05).Pearson correlation analysis results showed that TG/HDL-C was positively correlated with bone mineral density of lumbar L
1-4
, left femoral neck, femoral trochanter, wards triangle and all femur in patients with type 2 diabetes (r=0.275, 0.281, 0.276, 0.268, 0.287, P<0.001).
Conclusion
Low TG/HDL-C is related to the increased risk of type 2 diabetes complicated with osteoporosis.

Clinical effect of acupoint bloodletting therapy in children with asthmatic lung disease

Objective
To explore the clinical effect of acupoint bloodletting therapy on children with asthmatic lung disease.
Methods
A total of 120 children with asthmatic lung disease in the pediatric inpatient area of our hospital from December 2018 to August 2019 were selected and divided into study group and control group according to the random number table method, 60 cases in each group. For treatment, the study group combined with blood bleed at the Shao Shang acupoint on the basis of conventional Western medicine treatment, the symptom relief, length of stay and cost of the two groups were compared.
Results
The total effective rate of early treatment in the study group was 83.33%, which was higher than that of the control group of 66.67%, the difference was statistically significant (P<0.05).The total effective rate of treatment in the study group at discharge was 100.00%, which was higher than that in the control group of 83.33%, and the difference was statistically significant (P<0.05). TCM symptom scores before treatment between the two groups was not statistically significant (P>0.05); traditional Chinese medicine (TCM) symptom scores in the study group were higher than those in the control group after treatment, the length of hospital stay in the study group was shorter than that in the control group, and the hospitalization cost was lower than that in the control group,the differences were statistically significant (P<0.05).
Conclusion
On the basis of conventional Western medicine treatment, the addition of Shao Shang acupoint bloodletting therapy can significantly improve clinical symptoms and improve clinical efficacy.

Analysis of pregnancy outcome of gestational diabetes mellitus patients with increased fasting blood glucose level in oral glucose tolerance test

Objective
To analyze of pregnancy outcome of gestational diabetes mellitus (GDM) patients with increased fasting blood glucose level in 75 g oral glucose tolerance test OGTT.
Methods
A total of 450 cases of pregnant women with increased fasting blood glucose of GDM who were received standard prenatal examination and delivered in our hospital from January 2015 to January 2018 were selected, and grouped according to the results of fasting, 1 and 2 hours after taking the glucose in OGTT test, only fasting blood glucase increased to GDM group Ⅰ, the fasting combined with elevalted blood glucose at any other time point as the GDM group Ⅱ, all three time points were elevated as GDM group Ⅲ. A total of 450 healthy pregnant women who were examined at the same time were selected for a control group, the pregnancy outcomes of the two groups were compared. Spearman correlation was used to analyze the relationship between different phase blood glucose and neonatal weight.
Results
The incidences of premature delivery,large for gestational age (LGA), cesarean section, and hypertensive disorder complicating pregnancy in the study group were higher than those in the control group (P<0.05); there were significant differences in the comparison of LGA, insulin usage and cesarean section (P<0.05), that was the complications increased with the growing abnormal blood glucose levels. There were no differences in premature delivery and hypertensive discorders comlicating pregnancy in the three groups (P>0.05). The incidence of LGA in GDM group Ⅲwas higher than that of GDM group Ⅰ, and the difference was statistically significant (P<0.05). There was no significant difference compared with GDM group Ⅱ(P>0.05).Comparison of insulin usage rate, GDM group Ⅲwas higher than that of GDM groupⅠ, and the difference was statistically significant (P<0.05). Spearman correlation analysis result showed that fasting plasma glucose was positively correlated with neonatal weight (r=0.122, P<0.01), that neonatal weight was on the rise with the increase of fasting blood glucose.
Conclusion
The incidence of adverse pregnancy outcome is significantly increased in GDM patients with OGTT multipoint abnormal blood glucose. The fasting blood glucose level is important for the prediction of pregnancy outcome and neonatal weight. Clinical management should be individualized based on OGTT abnormalities status .

Clinical effect of squeezing embryo extraction for tubal pregnancy combined with Pituitrin in the treatment of tubal pregnancy

Objective
To explore the clinical effect of squeezing embryo extraction for tubal pregnancy combined with Pituitrin in the treatment of tubal pregnancy.
Methods
A total of 60 cases of fallopian tube pregnancy who were admitted and treated in the in our hospital from January 2019 to November 2019 were selected as the research subjects.They were divided into the control group (n=30) and the observation group (n=30) according to odd even number method. The control group was treated with squeezing embryo extraction for tubal pregnancy, and the observation group was treated with squeezing embryo extraction for tubal pregnancy combined with Pituitrin. The surgery time, intraoperative blood loss, total incidence of postoperative complications, tubal patency, tubal retention rate, and human chorionic gonadotropin β-subunit (β-HCG) levels were compared between the two groups.
Results
There was no statistically significant difference in surgical time between the two groups (P>0.05); the intraoperative blood loss in the observation group was less than that in the control group, and the difference was statistically significant (P<0.05); after surgery, the total incidence rate of postoperative complications in the observation group was lower than that in the control group, the tubal patency rate and tubal retention rate in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05); there were no statistically significant differences in β-HCG levels between the two groups before surgery and one day, one and two weeks after surgery (P>0.05).
Conclusion
In the squeezing embryo extraction for the patients with tubal pregnancy, the administration of Pituitrin can reduce the amount of intraoperative bleeding and have a small effect on β-HCG levels.

Influence of placing different intrauterine devices immediately after induced abortion on pregnant women with artificial abortion

To investigate the influence of different intrauterine device (IUD) on pregnant women with artificial abortion.
Methods
A total of 78 pregnant women who underwent artificial abortion in our hospital from July 2016 to December 2018 were selected and divided into the control group and the observation group by random number table method, 39 cases in each group. The control group was placed with a copper-containing IUD immediately after the abortion, and the observation group was placed with Man Yuele immediately after the villi were sucked out after the abortion. rate. The use of contraceptives, infection of reproductive tract and the total incidence of adverse reactions were compared between the two groups at 6 months after operation.
Results
The abnormal use rate of contraceptive device in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). There was no significant difference in reproductive tract infection between the two groups (P>0.05). The total incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).
Conclusion
Compared with copper-containing IUDs, placing Man Yuele immediately after artificial abortion can effectively reduce the abnormal use of IUD, reduce the incidence of adverse reactions, and does not increase the incidence of reproductive tract infections. It can be widely used in clinical practice.

Clinical effect of laparoscopic high ligation of hernial sac in the treatment of indirect inguinal hernia in children

Objective
To explore the clinical effect of laparoscopic high ligation of hernial sac in the treatment of indirect inguinal hernia in children.
Methods
A total of 80 cases with indirect inguinal hernia in our hospital from October 2018 to September 2019 were selected as the research objects, and they were divided into the observation group (40 cases) and the control group (40 cases) according to the random number table method. The control group was received routine open high ligation of hernial sac, the observation group was received laparoscopic high ligation of hernial sac.The operation and anesthesia related indexes (operation time, intraoperative blood loss and anesthesia recovery time),postoperative recovery (recovery time of bowel sounds, anal exhaust time and anal defecation time), perioperative complications (anesthesia recovery delay, postoperative bleeding, postoperative infection and postoperative severe pain) were compared between the two groups, the clinical effects in the two groups was counted.
Results
The operation time in the observation group was shorter than that in control group, the intraoperative blood loss in the observation group was less than that in the control group, the anesthesia recovery time in the observation group was shorter than that in the control group, the differences were statistically significant (P<0.05). The recovery time of bowel sounds, the exhaust time and the defecation time in the observation group were shorter than those in the control group, the differences were statistically significant (P<0.05). The incidence of anesthesia recovery delay, postoperative bleeding, postoperative infection and postoperative severe pain in the observation group were lower than those in the control group, the differences were statistically significant (P <0.05). The hernial sac disappeared in both groups after operation, and they recovered smoothly and were discharged from hospital.
Conclusion
Laparoscopic high ligation of hernial sac for indirect inguinal hernia in children have the advantages of short operation time, small trauma and quick recovery, which is worthy of clinical promotion.

To explore application effect of Lamaze non-drug pain-reducing labor in primiparous women.
Methods
Sixty primiparas treated in our hospital from May 2016 to January 2018 were selected as the research objects.They were divided into the control group (n=30) and the observation group (n=30) through the coin toss. In the control group, regular delivery was used, while in the observation group, Lamaze non-drug pain-reducing labor was used. The pain score, delivery time, and newborn health score (Apgar) were compared between the two groups.
Results
The pain score of the observation group was lower than that of the control group, and the difference was statistically significant(P<0.05). In the observation group, the delivery time was shorter than that of the control group, and the difference was significant (P<0.05). The grade of Apgar score in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05).
Conclusion
The administration of Lamaze non-drug pain-reducing labor in primiparas may help shorten the stages of labor, reduce the pain during delivery, and improve the health of the newborns.

Clinical effect of continuous positive airway pressure non-invasive ventilator in the treatment of severe pneumonia complicated with acute respiratory failure in children

Objective
To evaluate the effect of continuous airway pressure non-invasive ventilator (CPAP) in patients with severe pediatric pneumonia complicated with acute respiratory failure (ARF).
Methods
Seventy children with severe pediatric pneumonia complicated with ARF admitted to our hospital from October 2013 to August 1919 were selected as research objects. They were divided into the control group (n=35) and treatment group (n=35) according to computer randomization. The control group was treated with nasal cannula oxygenation. The treatment group was treated with continuous positive airway pressure CPAP. The blood gas index, respiratory rate and heart rate, time to symptom relief, and total effective rate were compared between the two groups.
Results
The arterial partial pressure of oxygen in the treatment group was higher than that in the control group, the partial pressure of arterial blood carbon dioxide was lower than that of the control group (P<0.05). The respiratory rate and heart rate of the treatment group were lower than those of the control group (P<0.05). The symptom relief time of the treatment group was lower than that of the control group, the difference was statistically significant (P<0.05). The total effective rate of the treatment group was higher than that of the control group, the difference was statistically significant (P<0.05).
Conclusion
In the clinical treatment of severe pediatric pneumonia complicated with ARF, continuous positive airway pressure CPAP has a significant effect on improving blood gas index, respiratory rate and heart rate, shortening symptom relief time and improving clinical efficacy. The clinical application value is high and worth promoting.

Objective
To analyze the changes and significance of procalcitonin (PCT), C-reactive protein (CRP) and prealbumin (PA) levels in children with bacterial infection.
Methods
A total of 120 children with infectious diseases treated in Changsha Fourth Hospital from March 2016 to March 2019 were analyzed retrospectively, included 60 cases in the bacterial infection group and 60 cases in the viral infection group. Meanwhile, 60 children who underwent physical examination in Changsha Fourth Hospital were selected as the control group. To test the levels of PCT, CRP and PA in the three groups.
Results
The positive rates of PCT, CRP and PA levels in the bacterial infection group and their combined detection were higher than those in the viral infection group and the control group, the differences were statistically significant (P<0.05). The positive rate of all indicators in the viral infection group was compared that in the control group, the difference was not statistically significant (P>0.05). Serum PCT and CRP levels in the bacterial infection group were higher than those in the viral infection group and the control group (P<0.05). PA level was lower than that of the viral infection group and control group, the difference was statistically significant (P<0.05). However,the levels of PCT, CRP and PA in the viral infection group were not significantly different from those in the control group (P >0.05).
Conclusion
The detection of PCT, CRP and PA levels have certain clinical value in identifying whether infectious diseases in children are bacterial or viral infections.

Results
The operation time of the observation group was shorter than that of the control group, intraoperative bleeding volume, ventilation fluid amount, the days of menstruation after operation of the observation group were less than those of the control group, the rate of secondary operation and the incidence of postoperative intrauterine adhesions of the observation group were lower than those of the control group, the differences were statistical significance (P<0.05). There was no significant difference in the rate of cure between the two groups (P>0.05).
Conclusion

Clinical effect of endoscopic tympanoplasty for chronic otitis media

Objective
To analyze the clinical effect of endoscopic tympanoplasty on chronic otitis media.
Methods
A total of 74 patients with chronic otitis media treated in our hospital from April 2018 to April 2019 were selected as the research objects. They were divided into control group (n=37) and observation group (n=37) by the random number table method. The patients in the control group were treated by routine operation, while those in the observation group were treated by endoscopic tympanoplasty. The dry ear rate, cure rate, disease recurrence and postoperative complications of the two groups were compared, and the bone conduction hearing threshold and air conduction hearing threshold of the two groups were observed.
Results
The dry ear rate and cure rate of the patients in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). The bone conduction hearing threshold, air conduction hearing threshold and disease recurrence rate of the patients in the observation group were lower than those in the control group, with statistically significant differences (P<0.05). There was no statistical significance between the two groups in the total incidence of postoperative complications (P>0.05).
Conclusion
Endoscopic tympanoplasty is a clinically effective treatment for chronic otitis media. It not only has a high clinical cure rate, but also can prevent recurrence of the disease and has the value of further promotion and implementation.

Application effect of dacryocystorhinostomy under nasal endoscope in patients with chronic dacryocystitis

Objective
To analyze the application effect of dacryocystorhinostomy under nasal endoscope in patients with chronic dacryocystitis.
Methods
A retrospective analysis was conducted on 68 patients with chronic dacryocystitis treated in our hospital from June 2017 to July 2019. They were divided into observation group and control group according to the choice of treatment selected by the patients or their family, 34 patients in each group. Patients in the control group were treated with lacrimal passage probing and lacrimal drainage tube implantation. In the observation group, they were treated with dacryocystorhinostomy under nasal endoscope. The total therapeutic effectiveness rate and postoperative complications were compared between the two groups.
Results
The total effective rate (94.12%) of the observation group was higher than that of the control group (76.47%), and the difference was statistically significant (P<0.05). The overall incidence of complications in the observation group (5.88%) was lower than that in the control group(26.47%), and the difference was statistically significant (P<0.05).
Conclusion
Treating chronic dacryocystitis with dacryocystorhinostomy under nasal endoscope has the advantages of mild injury, fast recovery, remarkable therapeutic effect and high safety. It has a high value of clinical application.

Effect of decompression ball relaxation therapy on tooth extraction in patients with anxiety

Objective
To explore the effect of decompression ball relaxation therapy on tooth extraction in anxious patients.
Methods
A total of 68 patients with dental anxiety who had tooth extraction from April 2018 to April 2019 in our hospital were selected as the research objects, and they were divided into control group and observation group by random number table method, 34 cases in each group. The control group adopted jaw extraction and routine dental care, and the observation group used a decompression ball relaxation nursing method based on the control group. The modified dental anxiety assessment scale (MDAS) and clinical behavior rating scale (FRANKL) scores of the two groups of patients before and after systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), and adverse reactions rate were compared.
Results
There were no significant differences in MDAS scores between the two groups of patients before nursing (P>0.05). The MDAS score of the observation group after nursing was lower than that in the control group, the difference was statistically significant (P<0.05), and the FRANKL scale score of the observation group after nursing was higher than that in the control group, the difference was statistically significant (P<0.05). The SBP, DBP, HR scores of the two groups of patients before and after nursing were compared, and the differences were not statistically significant (P>0.05). The incidence of adverse reactions in the observation group (11.76%) was lower than that in the control group (2.94%), the difference was statistically significant (P<0.05).
Conclusion
Decompression ball relaxation therapy can reduce the anxiety level of patients with dental anxiety, promote their active cooperative treatment, reduce the incidence of adverse reactions, and are worthy of clinical application.

To study the effect of Sufentanil combined with Remifentanil in anesthesia for emergency brain trauma surgery.
Methods
Sixty patients with emergency brain trauma received in our hospital from March 2016 to September 2019 were selected as research subjects. They were divided into the reference group and the Sufentanil+Remifentanil group by random number table method. Patients in the reference group (n=30) were anesthetized with Remifentanil alone. In the Sufentanil+Remifentanil group (n=30), Sufentanil combined with Remifentanil for anesthetization was applied. The heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) levels, and the blood glucose changes before and after anesthesia between the two groups were compared.
Results
The HR after 10 min of anesthesia in the reference group was higher than before anesthesia, and the difference was statistically significant(P<0.05). The levels of SBP and DBP after 10 min of anesthesia in the reference group were lower than before anesthesia, with statistically significant differences (P<0.05). There was no significant difference in HR, SBP, and DBP before and after 10 min of anesthesia in the Sufentanil+Remifentanil group (P>0.05). The HR level of patients in the Sufentanil+Remifentanil group was lower than that of the reference group after 10 min of anesthesia, and the difference was statistically significant (P<0.05). The SBP and DBP levels of patients in the Sufentanil+Remifentanil group were higher than those in the reference group after 10 min of anesthesia, and the differences were statistically significant (P<0.05).The blood glucose after 10 min of anesthesia in the Sufentanil+Remifentanil group was lower than that of the reference group, and the difference was statistical significance (P<0.05).
Conclusion
Sufentanil combined with Remifentanil for anesthetization has a remarkable effect on anesthesia in emergency brain trauma surgery, and can be promoted in clinical practice.

Effect of ultrasound-guided intercostal nerve block on patients with postoperative breast cancer

Objective
To explore the influence of ultrasound-guided intercostal nerve block on patients with postoperative breast cancer.
Methods
From October 2016 to September 2019, 50 cases of breast cancer surgery patients in our hospital were selected as the subjects. According to the random number table method, the patients were divided into the observation group and the control group, 25 cases in each group. The patients in both groups underwent radical mastectomy under general anesthesia. The observation group used ultrasound-guided intercostal nerve block to achieve postoperative analgesia, while the control group used patient-controlled intravenous analgesia (PCIA) to achieve postoperative analgesia. After the operation, the time of tracheal extubation, the time of spontaneous respiration and the time of complete awake of the two groups were recorded, and the scores of analgesia and sedation at 2, 4, 8, 12, 24 h after surgery were compared between the two groups.
Results
The time of tracheal extubation, the time of spontaneous respiration and the time of complete awake in the observation group were shorter than those in the control group, the differences were statistically significant (P<0.05). The visual analogue scale (VAS) scores of the observation group 2 and 4 h after surgery were lower than those of the control group, and the differences were statistically significant (P<0.05). VAS scores at 8, 12 and 24 h after surgery showed no significant difference between the two groups (P>0.05).The sedation scores of the observation group at different postoperative periods (2, 4, 8, 12 and 24 h after surgery) were all lower than those of the control group, and the differences were statistically significant (P<0.05).
Conclusion
Ultrasound-guided intercostal nerve block can shorten the time of tracheal extubation, the time of spontaneous respiration and the time of complete awake, and play a better role in sedation and analgesia.

Perioperative airway management strategy in patients with maxillofacial space infection

Maxillofacial infection is one of the common emergency cases in oral and maxillofacial surgery. It has the characteristics of fast onset, severe illness, more complications and difficult airway management. Timely and effective surgical intervention can effectively control the progress of infection. Therefore, the anesthesiologists are highly required for the ability of airway management during the anesthesia period. Appropriate ventilation management strategy can effectively reduce the damage to patients and avoid respiratory failure or even cardiac arrest caused by ventilation disorders to the greatest extent. This article describes the pre anesthesia evaluation, the selection of airway management methods, the common and special problems of various airway management methods and the prevention measures. It provides strategies and methods for peri anesthesia period airway management of patients with maxillofacial space infection.

Comparative ultrasound study of normal fetal umbilical blood flow and fetal umbilical blood flow around the neck after 37 weeks of gestation

Objective
To explore the clinical value of ultrasonography in diagnosing umbilical blood flow around neck and umbilicus of fetus after 37 weeks of pregnancy.
Methods
From March 2017 to March 2019, 30 pregnant women with umbilical cord around the neck after 37 weeks of pregnancy were selected as the observation group, 30 cases of normal pregnant women after 37 weeks of gestation in the same period were selected as the control group. The clinical data of the patients were analyzed retrospectively.All the patients were examined by umbilical cord blood flow ultrasound.The pulsation index(PI),resistance index(RI),the ratio of fetal umbilical artery systolic blood pressure and diastolic blood pressure (S/D) were measured. The results of ultrasound diagnosis of fetal umbilical cord around the neck were analyzed.
Results
The PI, RI and S/D in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). The sensitivity, specificity, accuracy, missed diagnosis rate and misdiagnosis rate of fetal umbilical cord around the neck were 96.67%, 93.33%, 95.00%, 3.33% and 6.67% respectively.
Conclusion
The RI, PI, S/D of umbilical cord blood flow around the neck of the fetus after 37 weeks of gestation are higher than those of the normal fetus. According to the results of ultrasonic examination of umbilical cord blood flow, the umbilical cord blood flow around the neck of the fetus can be effectively identified, which has important clinical application value.

Diagnostic value of 64 slice spiral CT low dose chest scan in lung cancer patients

Objective
To explore the diagnostic value of 64 slice spiral CT low dose chest scan in lung cancer patients.
Methods
A total of 100 patients suspected with lung cancer admitted to our hospital from February 2017 to February 2019 were selected as the research objects. All patients were examined by direct digital X-ray radiography system (DR) and 64 row spiral CT low-dose chest scanning. The detection rate, lung sign detection, sensitivity, specificity and image quality of the two methods were compared.
Results
Of the 100 patients, 96 were diagnosed as lung cancer by pathology and 4 were non lung cancer. A total of 85 cases of lung cancer were confirmed by DR chest film. 95 cases of lung cancer were diagnosed by low dose chest scan with 64 slice spiral CT. The sensitivity (98.96%) and accuracy (98.00%)of 64 row spiral CT low-dose chest scanning were higher than those of DR chest film examination (88.54% and 86.00% ), the differences were statistically significant (P <0.05); the detection rate of hilar and mediastinal lymphadenopathy of 64 row spiral CT low-dose chest scanning was higher than that of DR chest film examination, the difference was statistically significant (P<0.05). There were no significant differences in the detection rates of pleural effusion, vacuole sign and bronchial symbol, pleural indentation sign, thick wall cavity, burr sign and lobulation sign between the two methods (P>0.05). The excellent and good image quality rate of 64 slice spiral CT low-dose chest scan(98.00%) was higher than that of DR chest film (85.00%), the difference was statistically significant (P <0.05).
Conclusion
Low dose chest scan with 64 slice spiral CT can significantly improve the accuracy of lung cancer diagnosis, and the image quality is high, which is worth learning.

Clinical value of T1WI and T1 mapping in the diagnosis of mild neonatal hyperbilirubinemia with 3.0T MRI

Objective
To investigate the clinical value of T1WI and T1 mapping in the diagnosis of mild neonatal hyperbilirubinemia with 3.0T MRI.
Methods
From July 2018 to January 2019, 13 neonates with mild hyperbilirubinemia in the Department of Radiology of People′s Hospital of Longhua District in Shenzhen City were selected as the hyperbilirubinemia group and 17 normal neonates in the same period were selected as the normal group. T1WI and T1 mapping were performed on both neonates in the hyperbilirubinemia group and the normal group by Philips 3.0T MRI. T1 signal intensity of the bilateral globus pallidus was measured at the post-processing workstation and the mean value was calculated. T1 mapping was obtained by automatic operation using IR-TSE sequences with 8 different inversion times. T1 value of the bilateral globus pallidus were measured and the average was calculated. The difference of T1 signal intensity and T1 value between the two groups were compared, and the correlation between T1 signal intensity and T1 value of the globus pallidus with peak value of total bilirubin were analyzed separately.
Results
The peak value of serum bilirubin in the hyperbilirubinemia group was higher than that in the normal group, and the difference was statistically significant (P<0.05). The T1 signal intensity in the hyperbilirubinemia group was higher than that in the normal group, and the difference was statistically significant (P<0.05). T1 value of globus pallidum in the hyperbilirubinemia group was lower than that in the normal group, and the difference was statistically significant (P<0.05). The T1 signal intensity of globus pallidum was positively correlated with the peak value of total bilirubin (r=0.675, P=0.000). T1 value of globus pallidum was slightly negatively correlated with the peak value of serum total bilirubin (r=-0.455, P=0.012).
Conclusion
The T1 signal intensity and T1 value of globus pallidus in patients with mild neonatal hyperbilirubinemia are different from those in normal neonates, and have implications for the diagnosis of early acute bilirubin encephalopathy.

Effect of acupuncture combined with massage on pain, limited function and quality of life in patients with lumbar disc herniation

Objective
To explore the effect of acupuncture combined with massage in the treatment of lumbar disc herniation (LDH) pain and its influence on functional limitation and quality of life.
Methods
A total of 84 patients with LDH who were treated in our hospital from January to September 2019 were selected as subjects, and were divided into the control group and the observation group according to the random number table method, 42 cases in each group. The control group was treated with acupuncture. The observation group was treated with acupuncture combined with massage. The clinical efficacy, pain, function limitation, quality of life and adverse reactions were observed.
Results
The total clinical effective rate in the observation group was 95.24%, which was higher than that in the control group of 73.81%, the difference was statistically significant (P<0.05). After treatment, the visual analogue scale scores and Oswestry dysfunction indexes in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). Waist activity and SF-36 quality of life questionnaire scores were higher than those in the control group, the differences were statistically significant (P<0.05). The effect time of conscious pain relief in the observation group was shorter than that in the control group, and the duration of analgesia was longer than that in the control group, with statistically significant differences (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).
Conclusion
The clinical effect of acupuncture combined with massage for the treatment of LDH is remarkable, and the safety is good. It can effectively alleviate the symptoms such as pain and function limitation of patients and improve the quality of life of patients.

Clinical effect of Zhuang medical thread moxibustion in the treatment of vascular dementia

Objective
To investigate the clinical effect of Zhuang medical thread moxibustion in the treatment of vascular dementia.
Methods
A total of 60 patients with vascular dementia who treated in our hospital from February 2016 to July 2016 were selected as subjects. The patients were divided into treatment group (30 cases) and control group(30 cases) on the basis of random number table method. Patients in the treatment group were treated with Zhuang medical thread moxibustion on the basic treatment, while the patients in the control group were treated with Piracetam Tablets, 30 days were a course of treatment. The treatment effect, score changes of mini mental state examination(MMSE), the Barthel index of ADL (Barthel) and the National Institutes of Health stroke scale (NIHSS) of the two groups were compared.
Results
The total effective rate of the score of MMSE in the treatment group was 86.67% after treatment, which was higher than 63.33% of the control group, and the difference was statistically significant (P<0.05). The total effective rate of the score of NIHSS in the treatment group (90.00%) was higher than that in the control group(66.67%), and the difference was statistically significant (P<0.05). After treatment, the MMSE and Barthel scores of the two groups were higher than those before treatment, and the NIHSS scores were lower than those before treatment, the differences were statistically significant (P<0.05). The scores of MMSE and Barthel in the treatment group were higher than those of the control group, while NIHSS score was lower than that of the control group, with significant differences(P<0.05).
Conclusion
Zhuang medical thread moxibustion can effectively improve the cognitive function and behavior ability of patients and reduce neurological deficit. It is worthy of clinical application and promotion.

Clinical effect of Shenling Baizhu Powder combined with Ear Acupressure Pills on allergic rhinitis

Objective
To explore the clinical effect of Shenling Baizhu Powder combined with Ear Acupressure Pills on allergic rhinitis.
Methods
A total of 100 patients with allergic rhinitis treated in our hospital from January 2018 to May 2019 were selected as research subjects. They were divided into Chinese medicine group (n=50) and Western medicine group (n=50) by the random number table method. Patients in the Chinese medicine group were treated with Shenling Baizhu Powder combined with Ear Acupressure Pills, and in the Western medicine group, they were treated with Cetirizine Hydrochloride. The differences in comprehensive effect, adverse reactions and quality of life between the two groups were compared.
Results
The total effectiveness rate of the Chinese medicine group was 96.00%, higher than that of the Western medicine group, accounting for 84.00%, and the difference was statistically significant (P<0.05). The incidence of adverse reactions in the Chinese medicine group was 2.00%, lower than that in the Western medicine group,accounting for 16.00%, and the difference was statistically significant (P<0.05). Before treatment, the scores of each item from quality of life scale including physical pain, physiological function, social function, mental health, and general health status between the two groups had no significant differences (P>0.05). After treatment, the scores of these items in the quality of life scale including physical pain, physiological function, social function, mental health, and general health status in the Chinese medicine group were higher than those of the Western medicine group, and the differences were statistically significant (P<0.05). After treatment, the scores in the quality of life scale including physical pain, physiological function, social function, mental health, and general health status of the two groups were higher than those before treatment, with statistical significance (P<0.05).
Conclusion
Treatment of allergic rhinitis with Shenling Baizhu Powder combined with Ear Acupressure Pills has a remarkable therapeutic effect and it′s safe and reliable,which is worthy of promotion.

Non-immune fetal edema caused by perforin 1 complex heterozygous mutation: a case report and literature review

The clinical, laboratory and genetic data of non-immune fetal edema caused by perforin 1 (PRF1) complex heterozygous mutation were analyzed. A literature review was conducted on PRF1 mutation and non-immune fetal edema, familial hemophagocytic lymphohistiocytosis and non-immune fetal edema related articles. A new heterozygous combination of PRF1 genes c.503G>A (p.S168N) and c.1070G>A (p.R357Q) was found in a recurrent non-immune fetal edema. Literature retrieving shows that PRF1 pathogenic gene mutations can cause fetal non-immune edema. Familial hemophagocytic lymphohistiocytosis should be considered in the differential diagnosis of fetal edema and early diagnosis of multiple organ failure in the newborns, especially lack of reasons for infection or metabolism. Pathogenic genes related to hemophagocytic lymphohistiocytosis should be included as candidate genes for non-immune fetal edema. This paper reported the clinical characteristics of non-immune fetal edema caused by PRF1 complex heterozygous mutation and analyzed its etiology.

Diagnostic value of different test methods in bacterial vaginitis

Objective
To analyze the diagnostic value of different test methods in bacterial vaginitis.
Methods
A total of 150 patients with suspected bacterial vaginitis admitted to the Department of Obstetrics and Gynecology, People′s Hospital of Luohu District in Shenzhen City, from January to June, 2018, were selected as research objects. Bacterial vaginitis rapid detection method, lactic acid bacillus detection method, pH detection method and amine test detection method were used to detect the vaginal secretions of patients. The detection rate, sensitivity and specificity of the four methods were compared.
Results
The detection rate, sensitivity and specificity of the bacterial vaginitis rapid detection method were higher than those of lactic acid bacillus detection method, pH detection method and amine test detection method, the differences were statistically significant (P<0.05).
Conclusion
In the bacterial vaginitis test, the bacterial vaginitis rapid detection method should be given priority, the clinical test effect is remarkable and has high clinical application value.

Analysis of detection results of TORCH antibody and treponema pallidum antibody in progestational women

Objective
To analyze detection results of TORCH antibody and treponema pallidum antibody (anti-TP) in progestational women.
Methods
A retrospective analysis was performed on the data of 100 progestational women who were treated in Dalian Maternal and Child Health Hospital from January to December 2018. TORCH infection conditions were compared among the progestational women in different seasons, different living environments and different age groups.
Results
There were no significant differences in positive rates of toxoplasma IgM antibody (TOX-IgM),rubella virus IgM antibody (RV-IgM), cytomegalovirus IgM antibody (CMV-IgM), herpes simplex virus IgM antibody(HSV-IgM) and anti-TP among spring, summer, autumn and winter group (P>0.05). The positive rates of TOX-IgM,RV-IgM and CMV-IgM in winter and spring group were higher than those in summer and autumn group, the positive rate of HSV-IgM in spring group was higher than that in autumn group, the differences were statistically significant (P<0.05). There were no significant difference in positive rates of TOX-IgM, RV-IgM, CMV-IgM, HSV-IgM and anti-TP among between urban and rural group (P>0.05). There were no significant difference in positive rates of TOX-IgM,RV-IgM, CMV-IgM or HSV-IgM among 20-25 years old group, >25-32 years old group and >32-38 years old group(P>0.05). The positive rate of anti-TP in >32-38 years old group was higher than that in >25-32 years old group and 20-25 years old group, the differences were statistically significant (P<0.05).
Conclusion
There are certain infection rates of TORCH and syphilis in progestational women. There are seasonal features of TORCH. TOX, RV and CMV infection should be prevented in spring and winter. HSV infection should be prevented in spring. The infection rate of syphilis is increased with age growing. Older women should pay attention to syphilis screening and infection prevention before pregnancy.

Application of improved Braden scale and acute physiology and chronic health evaluation Ⅱin patients with high risk of pressure ulcer

Objective
To explore the effect of improved Braden scale (BRADEN) and acute physiology and chronic health evalution Ⅱ (Apache Ⅱ) on the early warning of pressure ulcer in critically ill patients.
Methods
A total of 60 critically ill patients admitted to our hospital from January to March in 2019 were selected. All the patients were evaluated with BRADEN score, improved BRADEN score and ApacheⅡto predict the occurrence of pressure ulcer.Combining with the actual number of patients who got pressure ulcer, the specificity, sensitivity and accuracy of the two scoring methods were compared.
Results
Totally 12 of the 60 critically ill patients got pressure ulcer. The BRADEN score indicated 50 cases of high risk of pressure ulcer, and the improved BRADEN and Apache Ⅱhinted 13 cases of high risk of pressure ulcer. The early warning′s specificity and accuracy of the improved BRADEN score and ApacheⅡwere 95.83% and 95.00%, higher than those of the BRADEN score, which accounting for 18.75% and 33.33% with statistical differences (P<0.05). The difference in the early warning′s sensitivity of the two scoring methods had no statistical significance (P>0.05).
Conclusion
The improved BRADEN and ApacheⅡscoring methods are reliable for the early warning of pressure ulcer in high-risk patients and have significant application value.

Application of case management mode in the transitional care of tumor patients discharged from hospital with PICC

Objective
To explore the application and effect of case management mode in the transitional care of tumor patients discharged from hospital with peripherally inserted central catheter (PICC).
Methods
A total of 216 patients with newly inserted PICC catheters who were admitted to our hospital from January 2018 to January 2019 were selected as the study subjects. According to the random number table method, the patients were divided into experimental group(n=108) and control group (n=108). The control group received routine continuous nursing. The experimental group implemented case management. The duration of PICC, the rate of unplanned extubation, the compliance of timely maintenance and the complications of PICC were compared between the two groups.
Results
The total incidence of complications and unplanned extubation in the experimental group were lower than those in the control group, and the compliance of timely maintenance was higher than that in the control group, the differences were statistically significant (P<0.05). The duration of PICC in the experimental group was longer than that in the control group, the difference was not statistically significant (P>0.05).
Conclusion
The case management mode can provide the whole course and high quality continuous nursing for the patients with PICC discharged from the hospital, improve the compliance of patients′catheter maintenance, reduce the incidence of catheter-related complications, and prolong the retention time of PICC,which is worthy of clinical application.

Application of hospital-community coordinated care model in patients with mild cognitive impairment

Objective
To explore the effect of hospital-community care model on cognitive ability, daily activity ability and compliance behavior of patients with mild cognitive impairment (MCI).
Methods
Fifty-two patients with MCI admitted to our hospital from March to December 2018 were selected as the research objects. They were divided into experimental group (n=26) and control group (n=26) according to the community. The control group adopted the conventional health education management method, and the experimental group adopted the MCI care mode of hospital-community interaction. Montreal cognitive assessment scale (MoCA) and Barthel index (BI) scale were used to evaluate the cognitive level and daily living ability of patients in the two groups at discharge, 1, 3 and 6 months after intervention,and the changes of patients′ compliance behavior were also analyzed.
Results
There were no significant differences in MoCA and BI scores between the two groups 1 month after intervention (P>0.05). The MoCA and BI scores of the experimental group were higher than those of the control group at 3 and 6 months after intervention, with statistical differences (P<0.05). The compliance behavior of the experimental group was higher than that of the control group at 6 months after intervention, with statistical difference (P<0.05).
Conclusion
The implementation of MCI care mode under the hospital-community interaction can effectively improve patients′ cognitive function, daily living ability and compliance behavior.

Application of global nursing model in interventional treatment of coronary heart disease

Objective
To explore the effect of global nursing model in patients with coronary artery disease (CHD) undergoing interventional therapy.
Methods
80 patients with CHD admitted to our hospital from September 2018 to April 2019 were selected and divided into two groups according to the random number table method, 40 cases in each group.The control group adopted the routine nursing mode of cardiology, while the observation group adopted the global nursing mode. The nursing effect, complications, nursing satisfaction, anxiety and depression of the two groups were compared.
Results
The total effective rate of nursing in the observation group (95.00%) was higher than that in the control group (85.00%), with statistical difference (P<0.05). The scores of anxiety, depression and complications in the observation group were lower than those in the control group, with statistical differences (P<0.05). The nursing total satisfaction of the observation group (92.50%) was higher than that of the control group (77.50%), with statistical difference (P<0.05).
Conclusion
For patients undergoing interventional therapy for CHD, the effect of global nursing intervention is remarkable. It can not only improve bad mood, but also reduce complications, improve nursing satisfaction,and the clinical application value is higher.

Effect of diet nursing care on lactation and gastrointestinal function of parturient women after cesarean section

Objective
To discuss the effect of diet nursing care on lactation and gastrointestinal function of parturient women after cesarean section.
Methods
Eighty patients undergoing cesarean section in our hospital from January to April 2019 were selected as research subjects and were divided into the control group (n=40) and the observation group(n=40) by the random number table method. The conventional nursing method was adopted for the control group, and the conventional nursing care+diet nursing intervention method was adopted for the observation group. The lactation and the recovery of gastrointestinal function in the two groups were compared.
Results
The start time of lactation and the adequate time of lactation in the observation group were shorter than those in the control group, with statistical differences (P<0.05); the adequacy ratios of milk at 24 and 48 hours in the observation group were higher than those in the control group, with statistical differences (P<0.05); the first time of flatus, the first time of defection and the appear time of bowel sounds in the observation group were shorter than those in the control group, with statistical differences(P<0.05).
Conclusion
Diet nursing intervention can promote the postpartum milk secretion and shorten the recovery time of gastrointestinal function in parturient women after cesarean section, which has significant overall advantages and is worthy of popularization and application in clinical practice.

Effect of detailed nursing on perioperative nursing quality and satisfaction of thyroid tumor patients

Objectives
To investigate the effect of detailed nursing on perioperative care quality and satisfaction of patients with thyroid tumors.
Methods
A total of 120 patients with thyroid tumor treated in our hospital from June 2018 to June 2019 were selected. They were divided into study group and control group according to random number table method, 60 cases in each group. Patients in the control group underwent general nursing, and the study group performed detailed nursing. The quality of perioperative nursing and total satisfaction between the two groups were compared.
Results
The quality of nursing in the study group was higher than that in the control group, the total satisfaction rate of the study group was 96.6%, which was higher than that of the control group (78.3%), the differences were statistically significant (P<0.05).
Conclusion
The application of detailed nursing to patients with thyroid tumor perioperative period can improve the quality of nursing services and improve patient satisfaction.

Effects of comprehensive nursing intervention on psychological status and complications of elderly patients after cataract surgery

Objective
To explore the effects of comprehensive nursing intervention on psychological status and complications of elderly patients after cataract surgery.
Methods
A total of 150 cases of elderly cataract patients (150 eyes)from June 2018 to June 2019 were selected. They were divided into two groups:routine nursing group (n=75, 75 eyes)and comprehensive nursing group (n=75, 75 eyes) according to the random table method. Routine nursing group received routine nursing, comprehensive nursing group received comprehensive nursing intervention, the psychological status and complications of the two groups of patients were compared.
Results
The differences of self-rating depression scale scores, before nursing intervention between the two groups were not significant (P>0.05). The SAS and SDS score after nursing intervention of the comprehensive nursing group ([27.0±4.5], [(29.1±4.0]scores) were lower than the routine nursing group ([39.0±4.0], [38.7±3.6]scores), the differences were statistically significant (P<0.05), the total incidence of postoperative complications of comprehensive nursing group was lower than the routine nursing group, the difference was statistically significant (P<0.05).
Conclusion
Comprehensive nursing intervention is more effective than routine nursing intervention in improving the psychological status and reducing the incidence of complications of elderly patients after cataract surgery.

Application of targeted nursing in patients with ascending aortic aneurysm with aortic regurgitation

Objective
To investigate the application of targeted nursing in patients with ascending aortic aneurysm and aortic regurgitation.
Methods
Sixty patients with ascending aortic aneurysm and aortic regurgitation who were admitted to Jiangxi Provincial People′s Hospital from September 2017 to September 2018 were selected. The patients were divided into control group (n=30) and intervention group (n=30) according to the time of admission. The control group received routine nursing, and the intervention group used targeted nursing. The total satisfaction of the two groups of patients with the incidence of postoperative complications were compared.
Results
The total satisfaction of nursing in the intervention group (93.33%) was higher than that in the control group (66.67%), the difference was statistically significant (P<0.05); the total incidence of complications in the intervention group (3.33%) was lower than that of the control group (20.00%), and the difference was statistically significant (P<0.05).
Conclusion
Targeted nursing in patients with aortic aneurysm with aortic regurgitation in the perioperative period to obtain better results and patient satisfaction, reduce the incidence of complications, it is worth promoting.

Application effect of programmed emergency care in first aid of patients with acute myocardial infarction

Objective
To explore the application effect of programmed emergency care in the first aid of patients with acute myocardial infarction (AMI).
Methods
A total of 144 patients with AMI admitted to our hospital from January 2018 to April 2019 were selected and randomly divided into the control group (n=72) and the study group (n=72). The control group was given routine nursing intervention and the study group was given programmed emergency care intervention. The overall rescue time, hospitalization time and nursing satisfaction of the two groups were compared and analyzed.
Results
In the control group, the overall rescue time was (55.26±18.23) min, the hospitalization time was(11.71±2.10) d, and the acute balloon dilation time was (78.67±12.60) min. In the study group, the overall rescue time was (40.73±11.26) min, the hospitalization time was (10.84±3.15) d, and the acute balloon dilation time was (71.32±15.20) min. There were no significant differences in the hospitalization time and the acute balloon dilation time between the two groups (P>0.05). The overall rescue time of the study group was shorter than that of the control group,and the difference was statistically significant (P<0.05). There was no significant difference in nursing total satisfaction between the two groups (P>0.05).
Conclusion
The programmed emergency care has a significant effect for patients with AMI. It can shorten the overall rescue time and improve the quality of nursing service. Therefore, it is worthy of promotion and application.

Application value of programmed oral care in preventing pulmonary infection of patients with severe craniocerebral injury

Objective
To explore the application value of programmed oral care in preventing pulmonary infection of patients with severe craniocerebral injury.
Methods
A total of 80 cases of patients with severe craniocerebral injury hospitalized in Hanyang Hospital Affiliated to Wuhan University of Science and Technology during the period of January 2017 to August 2019 were selected as the research objects, these patients were divided into two groups according to random number table method, 40 cases in each group. The control group was intervented with conventional oral nursing, and the observation group was intervented with procedural oral nursing. The nursing efficacy, incidence of pulmonary infection and degree of bad breath before and after intervention between the two groups were compared.
Results
There were no significant differences about the degree of oral malodor in the control group before and after nursing intervention (P>0.05), and the degree of oral malodor in the observation group after nursing intervention was lower than that of before nursing intervention and the control group (P<0.05). The effect of oral care in the observation group was better than that in the control group (P<0.05). There was no significant difference about the bacterial count culture between the two groups before oral care (P>0.05), and the number of bacteria cultured of the two groups after oral care was lower than that of before nursing intervention (P<0.05), and that of the observation group was lower than that of the control group (P<0.05). The incidence of pulmonary infection in the observation group (15.0%) was lower than that in the control group (37.5%) (P<0.05).
Conclusion
The programmed oral care can obviously reduce the bad breath of patients with severe craniocerebral injury, inhibit the growth of bacteria in the mouth, improve the curative effect of oral care and reduce the incidence of pulmonary infection.

Application effect of individualized nursing scheme in the treatment of infantile pneumonia by oxygen-driven atomization inhalation of Budesonide

Objective
To explore the application effect of individualized nursing in the treatment of pediatric pneumonia with oxygen-driven atomization inhalation of Budesonide.
Methods
From March 2017 to December 2018 treated in our hospital 70 cases of children with infantile pneumonia were selected. By random sampling method, childrens were divided into observation group (n=35) and control group (n=35). Two groups were driven oxygen atomization inhalation Budesonide treatment, the control group was given the conventional nursing method, the observation group was given individualized care plan on the basis of the control group, the clinical efficacy, symptoms improved and nursing satisfaction between two groups were compared.
Results
The clinical efficacy and parental satisfaction of the observation group were higher than those of the control group, and the symptom improvement time was shorter than that of the control group, with statistical differences between the groups (P<0.05).
Conclusion
In the course of receiving oxygen-driven atomization inhalation of Budesonide in hospital for pediatric pneumonia, the individualized nursing program has a good effect, which is worthy of promotion and application in clinical practice.

Application effect of modified 3M elastic tape fixation in patients with oral tracheal intubation in intensive care unit

Objective
To explore the application effect of modified 3M elastic tape fixation method in patients with tracheal intubation through mouth in intensive care unit.
Methods
From December 2017 to January 2019, 100 patients with endotracheal intubation were selected as the study objects. According to random number table method, they were divided into study group (n=50) and control group (n=50). The control group used silk tape+dental pad+inch belt fixed endotracheal intubation method, and the study group used modified 3M tape +dental pad+inch belt fixed endotracheal intubation method, the degree of displacement of endotracheal intubation, facial skin injury, angle injury, oral ulcer and odor, comfort and completion time of oral care between the two groups were compared.
Results
The incidence of catheter displacement in the study group was lower than that in the control group (P<0.05). The incidence of facial skin injury, angle of mouth injury, oral ulcer and oral odor in the study group was lower than that in the control group, with statistical differences (P<0.05). The comfort of the patients in the study group was higher than that in the control group,and the oral care time was shorter than that in the control group (P<0.05).
Conclusion
The improved 3M adhesive tape fixation method can reduce the incidence of catheter displacement, reduce the incidence of pressure injury to patients′facial skin caused by adhesive tape fixation, improve the oral situation, ensure the safety of patients, increase the comfort of awake patients, reduce the workload of nurses, improve the quality of nursing.

Role of Doula accompanying delivery in improving the quality of obstetrics

Objective
To explore the role of Doula accompany delivery in improving the quality of obstetrics.
Methods
From June 2015 to June 2017, 94 cases of maternal treated in our hospital were selected as the research objects, according to the hospital before and after the order, they were divided into the control group (n=47) and observation group(n=47). The control group adopts full responsibility midwifery, the observation group used doula to accompany delivery,the satisfaction of the two groups, cesarean section rate, incidence of postpartum hemorrhage, neonatal asphyxia, perineum infection, labor, and the bleeding were compared between two groups.
Results
The total satisfaction of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). The incidence of cesarean section, postpartum hemorrhage, neonatal asphyxia and perineal infection in the observation group was lower than those in the control group, with statistical differences (P<0.05). The total labor stage of the observation group was shorter than that of the control group, and the amount of bleeding was less than that of the control group, the differences were statistically significant (P<0.05).
Conclusion
The use of Doula accompanying delivery can promote the satisfaction degree of parturients, shorten the labor process, reduce the amount of bleeding, and reduce the risk of adverse pregnancy outcome.

Application of cluster nursing in maintenance of central venous catheter in department of cardiology

Objective
To explore the application value of cluster nursing in the maintenance of central venous catheter(CVC) in the department of cardiology.
Methods
A total of 258 patients diagnosed and treated in our hospital from August 2016 to August 2018 were selected as subjects. The patients were divided into the observation group and the control group according to the order of diagnosis and treatment. Among them, 138 patients diagnosed and treated from August 2016 to August 2017 served as the control group, and 120 patients from September 2017 to August 2018 served as the observation group. The control group was given routine catheterization nursing measures, and the observation group was given cluster nursing on the basis of the control group. The CVC catheter-related blood flow infection rate, unplanned extubation rate, catheterization and hospitalization days were observed and compared between the two groups.
Results
The infection rate of CVC related blood flow in the observation group was lower than that in the control group(P<0.05), and the rate of unplanned extubation in the observation group was lower than that in the control group, with statistical differences (P<0.05). In addition, the days of catheterization and hospitalization in the observation group were also shorter than those in the control group, with statistical differences (P<0.05).
Conclusion
Cluster nursing can effectively reduce the CVC catheter-related blood flow infection rate and unplanned extubation rate, and reduce the catheterization time and hospitalization days of patients.

Application effect of humanized nursing in prosthodontics

Objective
To explore the application value of humanized nursing in prosthodontics.
Methods
A total of 100 patients with prosthodontics admitted to our hospital from September 2018 to September 2019 were selected as the research objects. According to the random non double blind method, they were divided into control group (n=50) and observation group (n=50). The control group adopted the routine nursing method, and the observation group adopted the humanized nursing method. The scores of anxiety, depression self-assessment scale, nursing quality and patients′ nursing satisfaction were compared before and after nursing.
Results
There was no significant difference in self-rating anxiety scale (SAS) and self-rating depression scale (SDS) scores between the two groups (P>0.05). The scores of SAS and SDS in the observation group after nursing were lower than those in the control group, the differences were statistically significant (P<0.05). The total satisfaction score of the observation group was higher than that of the control group, with statistical difference (P<0.05). The score of nursing service quality in the observation group was higher than that in the control group (P<0.05).
Conclusion
Humanized nursing has high application value in the patients with prosthodontics.It is helpful to improve the negative emotions of patients, improve the quality of nursing service, and improve the patients′ satisfaction with nursing, which is worthy of clinical promotion.
