Relationship between MTHFR C677T gene polymorphism and homocysteine and coronary heart disease

Objective
To investigate the relationship between C677T polymorphism of methylenetetrahydrofolate reductase (MTHFR) and the level of plasma homocysteine (Hcy) as well as coronary heart disease (CHD).
Methods
From December 2016 to October 2019, 153 patients with CHD from Fuxin Fifth People′s Hospital and the First Affiliated Hospital of Jinzhou Medical University were selected as CHD group, and 149 subjects for healthy examination were selected as the control group at the same period. The clinical data of biochemical indexes such as blood lipid and Hcy were collected. The plasma Hcy level was measured by enzyme circulation method. The blood samples were taken to extract genomic DNA, and the C677T polymorphism of MTHFR gene was detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP).
Results
The Hcy level was the lowest in CC genotype and the highest in TT genotype in CHD group (P<0.05). In the control group, the Hcy level in different genotypes showed no significant difference (P>0.05). The proportions of CC genotype and C allele frequency in the CHD group were lower than those in the control group, and the differences were statistically significant (P<0.05). The proportions of TT genotype and T allele frequency in the CHD group were higher than those in the control group, and the differences were statistically significant (P<0.05). TT genotype was an independent risk factor for CHD (OR=4.388, 95%CI: 2.091-5.695, P=0.001), and CC genotype was an independent protective factor for CHD (OR=0.590, 95%CI: 0.393-0.921, P=0.001).
Conclusion
TT genotype in MTHFR C677T polymorphism is a risk factor for CHD. The plasma Hcy level is closely related to it, the increase of plasma Hcy level is related to TT genotype mutation and T allele. Hcy level in plasma and MTHFR C677T polymorphism may be an important reference indexes to study the etiology, pathogenesis and genetic mechanism of CHD.

Prediction value of acute gastrointestinal injury grade combined with multifactorial evaluation on death in critical patients

Objective
To explore the value of a new rating system for acute gastrointestinal injury (AGI) grade combined with multiple factors to predict death in critically ill patients.
Methods
A single-center retrospective cohort study was used, 658 critically ill patients admitted to ICU at the Second People′s Hospital of Zhaoqing from January 2016 to March 2020 were enrolled. They were divided into death group (112 cases) and survival group (546 cases) according to 28 days survival. The highest grade of AGI within 1 week of admission was evaluated, according to the highest grade to give the score(0-4 points),as the AGI score.Physiological chronic health score(APACHE Ⅱ),sequential organ failure (SOFA) score, MODS score, and MEWS score which of within 24 h of admission in both groups were recorded. Data of two groups were analyzed by one-way analysis, and further Logistic regression analysis was performed factors, screening independent risk factors of death in critically ill patients, and drawing the receiver operating characteristic (ROC) curve of subjects to evaluate the predictive value of a new rating system for acute gastrointestinal injury grading combined with multifactorial critical patient death.
Results
The age,the AGI score in the first week,APACHE Ⅱ,SOFA score in the death group were higher than those in the survival group, and the differences were statistically significant (P<0.05). According to the ROC curve, the best diagnostic thresholds for AGI, APACHE Ⅱ, SOFA,and AGE were AGI ≥2 points, APACHE Ⅱ≥21 points, SOFA ≥7 points, and age ≥71 years old. Results of multivariate analysis showed that AGI≥2 points (β=1.608, OR=4.994, 95%CI=3.258-7.654),APACHE Ⅱ≥21 points(β=0.762, OR=1.587, 95%CI=1.328-1.896), SOFA≥7 points (β=1.590, OR=4.904, 95%CI=3.354-7.171), age≥71 years (β=0.715, OR=1.122, 95%CI=1.063-1.183) were independent risk factors for poor prognosis in critically ill patients (P<0.05). The ROC curve was drawn for the death warning score, AGI, APACHE Ⅱ, and SOFA score, the death warning score AUC value was 0.909, which was higher than the AUC value of the AGI score, APACHE Ⅱscore and SOFA score alone (0.796, 0.715, 0.805), the differences were statistically significant (P<0.05).
Conclusion
The new scoring system of acute gastrointestinal injury rating combined with multi-factor evaluation can warn the death risk of critically ill patients, and the predictive value of critical patients′ death is better than that of any single score.

Application of modified micro fenestration in lower maxillary sinus floor elevation

Objective
To investigate the clinical application effect of modified micro fenestration in lower maxillary sinus floor elevation.
Methods
From January 2015 to September 2019, 30 patients admitted to the oral implantology department of our hospital were selected as the research subjects (70 implantation sites, the remaining alveolar bone height below the maxillary sinus measured by cone beam CT was 1-3 mm). Patients were randomly assigned to the experimental group (15 cases) and the control group (15 cases). The experimental group used modified micro fenestration to elevate the maxillary sinus floor, and the control group used conventional micro fenestration to elevate the maxillary sinus floor. Guided bone regeneration technology was applied in both groups after Bio-Oss bone powder was implanted at either the same period or at different periods. The diameter and area of open windows, operation time, post-operation pain, time of maxillary sinus bottom mucosa dissection and bone meal filling, degree of pain and facial swelling on 3 and 7 days after operation, bone height at the bottom of the maxillary sinus before and after operation, and the width of coronal bone around the sinus floor implants after operation were compared between the two groups.
Results
The diameter and area of the open windows in the experimental group were smaller than those in the control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the operation time between the two groups (P>0.05). In the process of bone window opening, the time used in the experimental group was shorter than that in the control group, and the difference was statistically significant (P<0.01). In the process of maxillary sinus bottom mucosa dissection and bone meal filling, the time used in the experimental group was longer than that in the control group, and the difference was statistically significant (P<0.01). There was no statistically significant difference in the degree of pain between the two groups at 3 and 7 days after operation (P>0.05). The degree of facial swelling in the experimental group was lower than that in the control group at 3 days after operation, with statistically significant difference (P<0.05). The bone height at the bottom of the maxillary sinus after operation in the two groups was higher than that before operation, with statistically significant difference (P<0.05). There was no significant difference in the width of coronal bone around the sinus floor implants between the two groups immediately after operation and 6 months after operation(P>0.05).
Conclusion
For the patients with severe alveolar bone atrophy in the maxillary molar area, the application of modified micro fenestration maxillary sinus floor elevation surgery can effectively increase bone height in the maxillary molar area, reduce the incidence risk of surgical complications.

Research of tumor necrosis factor-α gene polymorphisms associated with first-episode depression and the efficacy of Sertraline

Objective
To investigate the association between tumor necrosis factor-α (TNF-α) gene polymorphism with first-episode depression and efficacy of Sertraline.
Methods
A total of 265 inpatients and out-patients with firstepisode depression diagnosed and treated in Binzhou Youfu Hospital from June 2011 to June 2018 were selected as the case group. In addition, 380 healthy blood donors from Binzhou City and surrounding counties were selected as the control group. Each genotype polymorphism loci of patients with first-episode depression and healthy subjects was analyzed by the polymerase chain reaction-restriction fragment length polymorphism. SHEsis online software was used to analyze the differences in genotype distribution and allele distribution of TNF-α -G238A (rs361525)、-G308A (rs 1800629)、-C857T (rs1799724)、-C863A (rs1800630). Curative effect of 265 patients with first-episode depression treated with Sertraline after eight weeks was evaluated with Hamilton rating scale for depression (HAMD).
Results
Significant difference was shown in frequency distribution of genotypes and alleles of TNF-α gene rs1800629 between case group and control group (P<0.05); after Bonferroni correction, difference in frequency distribution of genotype was still significant (P<0.05). There was no significant difference in the frequency distribution of genotypes and alleles of rs361525 and rs1799724, rs1800630 between two groups (P>0.05). There was no significant difference in frequency of haploid type between case group and control group (P>0.05). After 8 weeks of Sertraline treatment, the chi-square test was used to analyze the relationship between the frequency distribution of alleles and genotypes of the 4 SNPS polymorphisms of TNF-α gene and the efficacy of Sertraline in patients with first-episode depression, the results showed that there was no significant difference in frequency distribution of genotype in rs1800629 between effective group and ineffective group (P>0.05); significant difference was shown in frequency distribution of allele in rs1800629 between two groups (P<0.05). After Bonferroni correction, no significant difference was shown in frequency distribution of allele in rs1800629 between effective group and ineffective group (P>0.05). Significant difference was shown in frequency distribution of genotypes and alleles of TNF-α gene rs361525 between effective group and ineffective group (P<0.05),after Bonferroni correction, difference in frequency distribution of genotypes and alleles in rs361525 was still significant(P<0.01). There was no statistically significant difference in genotype and allele frequency distribution of rs1800630 and rs1799724 between the effective group and the ineffective group (P>0.05). There was no significant difference in frequency of haploid type between effective group and ineffective group (P>0.05).
Conclusion
TNF-α gene rs1800629 polymorphism may be associated with the susceptibility of first-episode depression, rs361525 polymorphism may be associated with the efficacy of Sertraline.

Inflammatory bowel disease (IBD) is a kind of chronic idiopathic inflammatory diseases, its pathogenesis is not clear. The treatments including 5-aminosalicylic acid, adrenocortical hormone, biological agents, immunosuppressor, etc. However, adrenocortical hormone and immunosuppressor increase the risk of opportunistic infection in patients.Epstein-Barr virus infection is an opportunistic infection of IBD. In recent years, the reports about EBV infection in IBD led to refractory IBD, EBV-related lymphoproliferative diseases, lymphoma, and haemophilic cell syndromewere increasing. The progress of the epidemiology, clinical features, endoscopic and histological detection and treatment of EBV infection in IBD was presented.

Research progress of PD-1/PD-L1 blocker in immunotherapy of colorectal cancer

Programmed death receptor 1 and its ligand (PD-1/PD-L1) check point can inhibit the function of immune cells, especially in patients with tumor, the number of PD-1 expression increased significantly. In recent years, PD-1 has been used in the treatment of many advanced tumors, and has achieved certain effect, but it has not been ideal in colorectal cancer, especially in patients with the precise mismatch repair gene (pMMR). The influence factors also include related substances in the local immune microenvironment, such as β

-microprotein, which may play a role in the treatment and prognosis of the disease. This paper summarizes the clinical evidence of immunotherapy for colorectal cancer, reviews the current clinical challenges and new treatment methods, and it is found that some combination therapy and improvement of local immune microenvironment may be helpful for immunotherapy of this tumor.

Application of circular site-directed mutagenesis in vector construction

Objective
To explore the application of circular site-directed mutagenesis technology in single base mutation, restriction site modification and fragment deletion of the target plasmid.
Methods
The primer pairs were designed including target mutation, partial complementation, and 5’ end contains 5-8 bp protruding. The circular plasmid was used as template for amplification and digested and removed by methylase enzyme DpnI. Transform DH5α for gap repair then site-directed mutation plasmid was obtained.
Results
Through the validation by enzyme cutting and gene sequencing, circular site-directed mutagenesis technology could be efficiently used in single base mutation, modification of enzyme site and deletion of fragment.
Conclusion
The circular site-directed mutagenesis technology is simple, efficient and quick to realize single base mutation, restriction site modification and fragment deletion of the target plasmid,which can basically meet the needs of molecular construction in the process of drug development.

Comparative study of mouse model of blood deficiency pattern caused by comprehensive bloodletting

A total of 103 cases inpatients who underwent the

C-UBT and gastric tissue biopsy within a week in Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from January 1,2017 to December 31, 2018 were included in the study. The severity of antral mucosal lesions was graded pathologically, and the correlation between

C-UBT values and the five different histopathological grades was analyzed.
Results
The quantitative value of the

C-UBT was positively correlated with the number of Helicobacter pylori (Hp), the degree of chronic inflammation of the gastric antral mucosa, and the activity of the gastric antrum (r=0.257, 0.229, 0.238, P<0.05), but was not related to the degree of antral atrophy and intestinal metaplasia (P>0.05).
Conclusion
Clinically, the quantitative value of the non-invasive

Effect and safety of Fuzheng Xiaoji Recipe combined with FOLFIRI chemotherapy in the treatment of advanced rectal cancer

Objective
To investigate the effect of Fuzheng Xiaoji Recipe combined with FOLFIRI chemotherapy in the treatment of advanced rectal cancer.
Methods
From April 2016 to April 2019, 40 patients with advanced rectal cancer were selected. The patients were randomly divided into control group (20 cases) and study group (20 cases) according to the odd and even number of the patient′s hospitalization number. The study group accepted Fuzheng Xiaoji Recipe combined with FOLFIRI chemotherapy regimen while the control group accepted FOLFIRI only. Clinical treatment effects, quality of life, and adverse drug reactions of the patients within 1 month after 3 cycles of treatment in two groups were evaluated as clinical outcome measures.
Results
The total effective rate and disease control rate of the study group after treatment were higher than those of the control group, and the total incidence of adverse drug reactions was lower than that of the control group, the differences were statistically significant (P<0.05); after treatment, the effective rate of improvement of the quality of life of the study group was higher than that of the control group group, the difference was statistically significant (P<0.05).
Conclusion
Fuzheng Xiaoji Recipe combined with FOLFIRI chemotherapy can significantly alleviate the condition of patients with advanced rectal cancer, improve patient survival rate and quality of life, and reduce toxic and side effect.

Application effect of multidisciplinary collaborative diagnosis and treatment process in portal hypertension varicose veins bleeding patients in the second-class hospitals

Objective
To approach application effect of multidisciplinary collaborative diagnosis and treatment process in portal hypertension varicose veins bleeding patients in the second-class hospitals.
Methods
The 60 clinical data of portal hypertension varicose veins bleeding patients in People′s Hospital in Fengshun County, Guangdong Province from January 2017 to February 2019 were retrospectively analyzed, who were divided into two group by different diagnostic procedure, each had 30 cases. The first hemostasis successful rate, waiting time for diagnosis and treatment, life quality, satisfaction with the process, clinical effect of two group were compared.
Results
The first hemostatic success rate (86.7%) of the observation group was higher than that of the control group (66.7%), and the waiting time for diagnosis and treatment ([20.4±6.2]min) was shorter than that of the control group ([39.5±9.8]min). The quality of life score ([20.8±3.9]points), the satisfaction of diagnosis and treatment process (96.7%) in the observation group were higher than those in the control group ([16.7±4.2]points, 73.3%), the total clinical effective rate of the observation group (100.0%) was higher than that of the control group (83.3%), the differences were statistically significant (P<0.05).
Conclusion
The multidisciplinary collaborative diagnosis and treatment process is applied to patients with portal hypertension and varicose bleeding in the second-class hospitals, the diagnosis and treatment efficiency, quality of life and patient satisfaction are improved, and the bleeding effect is good.

Application of the management mode of medical and nursing combined with anti-aspiration in stroke patients

Objective
To explore the application effect of medical and nursing combined with anti-aspiration management model in stroke patients.
Methods
A total of 120 cases of stroke patients admitted to our hospital′s neurosurgery department from June 2018 to June 2019 were selected as the research objects, and were divided into an observation group (60 cases) and a control group (60 cases) according to the random number table method. The control group adoped conventional nursing intervention measures, and the observation group adopted a management model of combined with medical care and anti-aspiration. The incidence of aspiration, incidence of aspiration pneumonia, and enthusiasm for medical cooperation between the two groups were compared.
Results
The incidence of aspiration in the observation group was 3.33%, lower than that in the control group for 15.00%, the difference was statistically significant(P<0.05); the incidence of aspiration pneumonia in the observation group was 1.66%, which was lower than that in the control group for 11.67%, the difference was statistically significant (P<0.05); after implementation, the enthusiasm of medical cooperation in the observation group was better than that of the control group, and the difference was statistically significant (P<0.05).
Conclusion
In the clinical care of stroke patients, the use of medical care combined with anti-aspiration management model is conducive to reducing the incidence of aspiration of patients and the incidence of aspiration pneumonia, so that patients can recover as soon as possible, so it has high application value.

Application of cognitive behavior combined with diet intervention in patients with leukemia

Objective
To explore the application effect of cognitive behavior combined with diet intervention in patients with leukemia.
Methods
The clinical data of 80 leukemia patients admitted to our hospital from January 2014 to December 2019 were retrospectively analyzed, and they were divided into two groups according to the different intervention methods. Among them, the clinical data of 39 patients who were given diet intervention were classified in the control group, and the clinical data of 41 patients with cognitive behavior combined with diet intervention were included in the observation group. The quality of life and nursing satisfaction were compared between the two groups.
Results
After intervention, the scores of overall health, physical function, physiological function, body pain, emotion, energy, mental health and social function in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The nursing satisfaction in the observation group was higher than that in the control group,the difference was statistically significant (P<0.05).
Conclusion
Cognitive behavior combined with diet intervention can increase the quality of life in patients with leukemia, and increase patients′ satisfaction with nursing care.

Application of electrocardiogram monitoring in PICC catheterization

Objective
To explore the application of electrocardiogram (ECG) positioning during ECG monitoring in peripherally inserted central catheter (PICC) catheterization.
Methods
A total of 100 patients who underwent PICC catheterization in our hospital from March to December 2016 were selected as objects. They were divided into control group (50 cases) and observation group (50 cases) according to the random number table method. The control group used B-ultrasound guided conventional tube placement and external X-ray positioning, and the observation group used B-ultrasound to locate the puncture point and blood vessel, and connect to the ECG monitor. The PICC catheterization effect (one-time catheterization success rate, catheter tip position accuracy rate) and complications (phlebitis, thrombosis) during catheterization were compared between the two groups.
Results
The success rate of one-time catheterization and the accuracy rate of catheter tip position in the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).
Conclusion
ECG positioning in ECG monitoring has a high application value in PICC catheterization, which can increase the success rate of one-time catheterization and the accuracy rate of catheter tip position, and reduce the occurrence of phlebitis and thrombosis.

Application effect of posture cushion in lateral decubitus placement in urological surgery

Objective
To explore the application effect of posture cushion in lateral decubitus placement in urological surgery.
Methods
A retrospective study of 60 urological surgery patients admitted to our hospital from July 2018 to December 2019 was selected. According to the different treatment methods, they were divided into control group (30 cases, placed in the traditional lateral position) and experimental group (30 cases, placed in the lateral position with a posture cushion). The comfort scores, skin compression and total incidence of complications were compared between the two groups.
Results
After intervention, the comfort score of the experimental group was higher than that in the control group; the incidence of skin compression in the experimental group was lower than that in the control group; the total incidence of complications in the experimental group was lower than that in the control group, the differences were statistically significant (P <0.05).
Conclusion
The application of posture cushion in the lateral position of urological surgery is beneficial to improve the comfort of patient, significantly improve the pressure on the skin, and reduce the occurrence of complications.

Effect of 3.0% hypertonic saline in treatment of hypovolemic shock caused by multiple fractures of limbs

Objective
To explore the effect of 3.0% hypertonic saline in treatment of hypovolemic shock caused by multiple fractures of limbs.
Methods
A total of 86 cases with hypovolemic shock caused by multiple fracture of limbs admitted in our hospital from February 2019 to January 2020 were selected as the research objects. They were divided into two groups according to the random number table method, with 43 cases in each group. The control group was given Hydroxyethyl Starch 130/0.4 Injection and Lactated Ringer′s solution immediately, the observation group was given 3% hypertonic saline supplement. The electrolyte levels, plasma osmotic pressure and cerebral oxygen partial pressure levels during the treatment, and the mini-mental state examination (MMSE) scores within 12 hours after operation were compared between the two groups.
Results
After treatment, the sodium ion and chloride ion levels in the observation group were higher than those in the control group, and the potassium ion level was lower than that in the control group, the differences were statistically significant (P<0.05); at 30 min, 2 h and 12 h after treatment, the plasma osmotic pressure and cerebral oxygen partial pressure in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). At 12 h after treatment, the MMSE score in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05).
Conclusion
To treat patients with hypovolemic shock caused by multiple fractures of limbs with 3.0% hypertonic saline is of great value to maintain plasma electrolyte balance, increase plasma osmotic pressure, ensure cerebral oxygen supply, improve nerve function to a certain extent.

Effect of psychological intervention on negative emotions and prognosis of patients after emergency percutaneous coronary intervention

Objective
To explore the influence of psychological intervention on negative emotions and prognosis of patients after emergency percutaneous coronary intervention (PCI).
Methods
A total of 134 cases acute ST-segment elevation myocardial infarction patients who received emergency PCI from May 2017 to June 2019 were divided into intervention group and control group according to the random number table method, with 67 cases in each group. The control group was given conventional coronary heart disease-related knowledge education and secondary prevention education, and the intervention group received combination of individual psychological intervention and group intervention for 2 months. The changes in the scores of the self-rating anxiety scale (SAS) and self-rating depression scale (SDS) of the two groups before and after intervention were compared. At the same time, patients were followed up for 6 months to observe the difference in the incidence of major adverse cardiovascular events (MACE) and the hospitalized again rate due to suspected illness between the two groups.
Results
There were no significant differences in SAS and SDS scores between the two groups before intervention (P>0.05); and the SAS and SDS scores in the intervention group were lower than those in the control group after 2 months of intervention, the differences were statistically significant (P<0.01). After 6 months of follow up, the total incidence of MACE in intervention group was 5.97%, which was lower than that in control group of 19.40%, the difference was statistically significant (P<0.05); and the hospitalized again rate due to suspected illness in the intervention group was 10.45%, which was lower than that in the control group of 28.36%, the difference was statistically significant (P<0.01).
Conclusion
Patients after emergency PCI are prone to negative emotions such as anxiety and depression, standard psychological intervention can help relieve negative emotions, improve the clinical prognosis of patients, and reduce unnecessary medical waste.

Analysis on pathogenic bacteria and drug resistance of infective endocarditis in a third grade hospital

Objective
To explore the pathogenic bacteria and drug resistance of infective endocarditis, and to provide references for clinicians to apply antibiotics rationally.
Methods
A total of 107 inpatients with infective endocarditis(infective endocarditis) in a third-class hospital from June 1, 2014 to May 31, 2019 were selected for retrospective analysis, and 79 cases were selected for blood bacterial culture and drug sensitivity test. The pathogenic bacteria composition and drug resistance distribution in clinical data were analyzed.
Results
In 107 patients with infective endocarditis, 79 strains of non-duplicative pathogens were detected, including 75 strains of gram-positive bacteria(94.94%), including 31 strains of streptococcus (accounting for 39.24%), 22 strains of Staphylococcus aureus (accounting for 27.85%), and 4 strains of gram-negative bacteria (accounting for 5.06%), no cases of fungal infection were detected. Streptococcus was still the most common pathogenic bacteria (39.24%). Drug resistance analysis was showed that the drug resistance rate of Streptococcus bacteria to Penicillin was 90.32%. Of the 22 strains of Staphylococcus bacteria, 20 strains were resistant to Penicillin, and the drug resistance rate of Benzoxicillin was 81.82%. However, it was sensitive to antibiotics of special use grade, such as Vancomycin, Imipenem and Linezolid, and had no resistance.
Conclusion
The pathogenic bacteria of infective endocarditis are mainly gram-positive bacteria, and Streptococcus is still the most common. Gram-positive cocci are generally resistant to Penicillin. In order to ensure the efficacy and safety of antiinfective treatment, antibacterials should be selected empirically in combination with the local etiological data, and the drug regimen should be adjusted in time according to the results of drug susceptibility test.

Therapeutic effect comparison of open reduction and internal fixation with titanium plate and interlocking intramedullary nail for tibia and femur fractures

Objective
To explore the therapeutic effect of tibia and femur fractures through open reduction and titanium plate internal fixation and interlocking intramedullary nail internal fixation.
Methods
A total of 60 cases of tibia and femur fracture from June 2017 to June 2019 in our hospital were randomly divided into two groups according to random draw. All 30 patients in the control group were treated with open reduction and internal fixation with titanium plate, and 30 patients in the observation group were treated with internal fixation with interlocking intramedullary nail.The healing time, knee motion, weight-bearing time, ankle motion, swelling, pain, symptom score of dysfunction and complications were observed and compared in the two groups.
Results
Compared with the control group, the healing time and weight-bearing time of the observation group were shorter, the knee joint motion and ankle joint motion were lower, the differences showed statistical significance (P<0.05). There were no statistical significances in the scores of swelling, pain and dysfunction between the two groups before treatment (P>0.05);after treatment, the scores of swelling,pain and dysfunction in the observation group were lower than those in the control group, the differences showed statistical significance (P<0.05). The total incidence of complications in the observation group was 10.00%, lower than that in the control group (30.00%), the difference showed statistical significance (P<0.05).
Conclusion
The treatment of tibia and femur fractures with interlocking intramedullary nail can effectively improve the clinical symptoms of patients,promote the healing of fracture ends, and have fewer complications, which is worth popularizing in clinical treatment.

Application exploration of anal endoscopy combined with laparoscopic total mesorectal excision in low rectal cancer

Objective
To explore the application effect of anal endoscopy combined with laparoscopic total mesorectal excision in low rectal cancer.
Methods
The clinical data of 72 patients with low rectal cancer treated in our hospital from August 2016 to October 2019 were retrospectively analyzed. They were divided into the conventional group and the combined group according to different surgical methods, 36 cases in each group. In the conventional group, laparoscopic total mesorectal excision was used, and in the combined group, anal endoscopy combined with laparoscopic total mesorectal excision was used. The surgical indexes, postoperative recovery, surgical specimen indexes and postoperative complications of the two groups were compared.
Results
The length of surgical incision, the time of getting out of bed of the patients in the combined group were shorter than those of the conventional group, the amount of intraoperative bleeding in the combined group was less than that of the conventional group, the differences were statistically significant (P<0.05). The total incidence of complications in the combined group was 5.26%, which was lower than that in the conventional group (25.00%), the difference was statistically significant (P<0.05). The satisfaction rate of incision beauty of the combined group was 94.44%, which was higher than that of the conventional group (75.00%), the difference was statistically significant (P<0.05). There were no significant differences between the two groups in the distal margin,proximal margin, number of lymph node dissections, 1ow anterior resection syndrome score, and mesorectal integrity(P>0.05).
Conclusion
Anal endoscopy combined with laparoscopic total mesorectal excision is effective in the treatment of low rectal cancer. It can effectively remove the lesions, and has mild surgical trauma, few complications, and good cosmetic effects, which can speed up the postoperative recovery of patients.

Clinical features of patients with different pathological types of lung cancer and brain metastases and the risk factors of cerebral hemorrhage

Objective
To investigate the clinical features of different pathological types of lung cancer and brain metastases, and to analyze the risk factors of cerebral hemorrhage.
Methods
The clinical data of 370 patients with lung cancer and brain metastases who were treated in the General Hospital of Pingxiang Mining Group Co., Ltd. from May 2017 to May 2019 were retrospectively analyzed. The clinical features of different pathological types of lung cancer and brain metastases and the influencing factors of cerebral hemorrhage were analyzed.
Results
There were significantly statistical differences in the distribution of intracranial lesions, combined hemorrhage and density of lesion among the patients with different pathological types (squamous cell carcinoma, adenocarcinoma, small cell carcinoma, large cell carcinoma, adenosquamous carcinoma) of lung cancer and brain metastases (P<0.05). The patients were divided into the hemorrhage group (n=154) and the non-hemorrhage group (n=216) according to the hemorrhage of patients. The results of single-factor analysis showed that the proportions of high density or mixed density of multiple lesions and tumor in the hemorrhage group were higher than those in the non-hemorrhage group, the differences were statistically significant(P<0.05). Multiple-factor analysis showed that the high density or mixed density of multiple lesions (β=1.959, OR=7.091, 95%CI=4.442-11.320) and tumor (β=0.740, OR=2.095, 95%CI=1.376-3.190) were the risk factors of cerebral hemorrhage in patients with lung cancer and brain metastases (P<0.05).
Conclusion
The lung cancer patients with different pathological types have different clinical features of brain metastases, and the risk of cerebral hemorrhage can be estimated based on the clinical features of patients.

Influence of laparoscopic radical resection of rectal carcinoma with pelvic autonomic nerve preservation on patients′ urodynamic indexes

Objective
To explore the influence of laparoscopic radical resection of rectal carcinoma with pelvic autonomic nerve preservation on patients′ urodynamic indexes.
Methods
Totally 80 patients with rectal carcinoma admitted to our hospital from February 2017 to May 2019 were selected as research subjects. They were divided into group A(n=40) and group B (n=40) by a random number table method. Group A adopted laparoscopic radical resection of rectal carcinoma with pelvic autonomic nerve preservation, and group B used laparoscopic radical resection of rectal carcinoma. The incidence of postoperative complications and the urodynamics indexes, ejaculation function and erectile function after operation for six months were compared between the two groups.
Results
There was no significant difference in the total incidence of postoperative complications between the two groups (P>0.05). The maximum urinary flow rate(Q
max
), urine output, and maximum bladder detrusor systolic pressure (Pdet Q
max
) in group A were (17.60±2.19) ml/s,(296.31±34.68) ml, and (25.62±3.24) ml, respectively, which were higher than those in group B ([13.01±1.89]ml/s,[230.49±30.18]ml, [20.12±2.37]ml), respectively, and the differences were statistically significant (P<0.05). The residual urine volume (PVR) in group A was (33.48±5.21) ml, which was lower than that of group B ([60.14±7.59]ml), and the difference was statistically significant (P<0.05). Postoperative ejaculation function and erectile function in group A were better than those of the group B, with statistical significances (P<0.05).
Conclusion
Applying laparoscopic radical resection of rectal carcinoma with pelvic autonomic nerve preservation to treat patients with rectal carcinoma can effectively improve patient′s urinary function, ejaculation function and erectile function without increasing the incidence of complications. It has high safety and is worthy of clinical promotion.

Clinical effect of Xuanyu Tongjing Decoction combined with Levonorgestrel intrauterine system in the treatment of adenomyosis with dysmenorrhea

Objective
To explore the effect and safety of Xuanyu Tongjing Decoction combined with Levonorgestrel intrauterine system in the treatment of adenomyosis with dysmenorrhea.
Methods
A total of 120 adenomyosis patients with dysmenorrhea recruited in Jiangxi Provincial Maternal and Child Health Hospital from February to August 2018 were selected as the study objects and divided into control group and observation group, with 60 cases in each group based on random number table method. The observation group was treated with Xuanyu Tongjing Decoction combined with Levonorgestrel intrauterine system, and the control group was only treated with Levonorgestrel intrauterine system. The symptoms of dysmenorrhea, the degree of pain, traditional Chinese medicine (TCM) syndrome integral, the volume of uterus, the level of serum carbohydrate antigen-125 (CA-125) and the occurrence of the adverse reaction were compared after 6 months of treatment.
Results
The symptom score of dysmenorrhea, visual analogue scale (VAS) score,TCM syndrome integral score, uterine volume and the level of serum CA-125 level in both two groups after 6 months of treatment were higher than those before treatment, and the differences were statistically significant (P<0.05). The symptom score of dysmenorrhea, VAS score, TCM syndrome integral score, uterine volume and the level of serum CA-125 level in observation group after 6 months of treatment were lower than those in control group, and the differences were statistically significant (P<0.05). The total incidence of adverse reactions in the observation group and the control group had no statistically significant difference (P >0.05).
Conclusion
Xuanyu Tongjing Decoction combined with Levonorgestrel intrauterine system can effectively improve the clinical symptoms of adenomyosis with dysmenorrhea, and worthy to be promoted and applicated in clinical.

Application effect of COOK cervical balloon in induced labor of pregnant women and its influence on pregnancy outcome

Objective
To explore the application effect of COOK cervical balloon in induced labor of pregnant women and its influence on pregnancy outcome.
Methods
A total of 98 full-term pregnant women who were expected to give birth in our hospital from June 2018 to January 2020 were selected as the research objects, they were divided into two groups by random number table method, with 49 cases in each group. The control group was given Oxytocin to induce labor, and the observation group was given COOK cervical balloon in induced labor. The cervical maturity, newborn conditions and maternal delivery were compared between two groups,and the occurrence of maternal adverse reactions was counted.
Results
Before intervention, there was no significant difference in the cervical Bishop score of pregnant women of the two groups (P>0.05); after intervention, the cervical Bishop scores of pregnant women of two groups were higher than those in this group before intervention, and the observation group was higher than that in the control group, the differences were statistically significant (P<0.05). The total labor time in the observation group was shorter than that in the control group, the natural delivery rate in the observation group was higher than that in the control group, the cesarean section rate and the total incidence of adverse reactions in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). There were no significant differences in the neonatal Apgar score, neonatal asphyxia rate and postpartum bleeding volume between the two groups (P>0.05).
Conclusion
The use of COOK cervical balloon in induced labor of pregnant women can increase cervical Bishop score, promote cervical maturity, improve pregnancy outcome, reduce cesarean section rate, and shorten the stages of labor, which is worthy of clinical application and promotion.

Application of project management in the decrease of the infection rate of#br# Klebsiella pneumoniae in premature infants

Objective
To explore the application of project management in reducing the infection rate of Klebsiella pneumoniae in premature infants, so as to reduce the incidence of Klebsiella pneumoniae infection in premature infants.
Methods
A total of 119 premature infants who received ventilator assisted respiratory therapy in Department of Neonatology of Yue Bei People′s Hospital from April to October 2015 were selected as study subjects. According to the implementation of project management, they were divided into the intervention group (58 cases) and the control group(61 cases). The control group was before the implementation of project management, and the intervention group was after the implementation of project management. The positive rate and the infection rate of Klebsiella pneumoniae in premature infants in the two groups were compared.
Results
The positive rate and infection rate of Klebsiella pneumoniae in the intervention group were lower than those in the control group, and the differences were statistically significant(P<0.05). The unqualified rate in the implementation of endotracheal suction, sputum suction bottle and sputum washing fluid replacement, hand hygiene management, oral care, instrument disinfection, and children′s posture management of the intervention group were significantly lower than those of the control group, and the differences were statistically significant (P<0.05). Duration of ventilator treatment and hospitalization of the intervention group were shorter than those of the control group, the differences were statistically significant (P<0.05).
Conclusion
Project management can reduce those the nosocomial infection rate of Klebsiella pneumoniae in the quality management of premature infants and provide a new operation mode for nursing quality managers.

Clinical observation on the prevention of preterm delivery by the treatment of bacterial vaginosis during pregnancy with Vaginal Active Bacilli Capsules

Objective
To investigate the clinical effect of Vagina Active Bacilli Capsules in the treatment of bacterial vaginosis (BV) during pregnancy on preterm delivery.
Methods
A total of 100 patients with BV during pregnancy recruited in our hospital from January 2018 to September 2019 were selected as the study objects and divided into control group and observation group with 50 cases in each group by random number table method. The patients in the control group were treated with conventional therapy. The patients in the observation group were treated with Vaginal Active Bacilli Capsules. All patients were treated for 10 days. The premature rupture of membranes occurred, curative effect and adverse pregnancy outcome in the two groups were compared.
Results
The incidence of premature rupture of membranes in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). The total incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).
Conclusion
The treatment of BV during pregnancy with Vagina Active Bacilli Capsules has a positive effect on the prevention of preterm delivery, and can reduce the incidence of premature rupture of membranes, improve the total effective rate of treatment,reduce the incidence of abortion, preterm delivery, neonatal asphyxia, etc., and has a high application value.

Effect of acupoint massage combined with oral exercise intervention on oral feeding of premature infants

Objective
To explore the effect of acupoint massage combined with oral exercise intervention on preterm infants′ oral feeding.
Methods
A total of 50 premature infants admitted to our hospital from January to December 2019 were selected as the research objects, they were divided into observation group (n=25) and control group (n=25)according to the birth time. The control group was given conventional feeding and nursing methods, and the observation group was given acupoint massage combined with oral exercise intervention on the basis of control group, the effect of oral feeding of premature infants in the two groups were observed.
Results
After treatment, the total oral feeding time in the observation group was shorter than that in the control group, the feeding efficiency and feeding effect were higher than those in the control group, the differences were statistically significant (P <0.05). The body weight in the observation group was greater than that in the control group, and the hospitalization time was shorter than that in the control group, the differences were statistically significant (P<0.05). There was no complication in premature infants of both groups.
Conclusion
Acupoint massage combined with oral exercise intervention can improve the complete oral feeding of premature infants, improve the feeding efficiency and feeding effect, benefit the healthy growth of preterm infants, and has high clinical application value.

Application of ligation of ascending branch of uterine artery combined with row suture of lower uterus in cesarean section

Objective
To investigate the application effect of ligation of ascending branch of uterine artery combined with row suture of lower uterus in cesarean section.
Methods
A total of 56 parturient who received cesarean section treatment in our hospital from May 2019 to January 2020 were selected as the research objects. They were divided into control group (n=28) and observation group (n=28) by random number table method. The control group was given routine hemostasis during cesarean section, and the observation group used ligation of ascending branch of uterine artery combined with row suture of lower uterus. The postpartum hemorrhage, surgical indicators, complications and quality of life of the two groups were compared.
Results
The amount of blood loss and intraoperative blood transfusion during operation, 3 hours and 24 hours after operation in the observation group were less than those in the control group, the differences were statistically significant (P<0.05). The operation time, hospitalization time and uterine contraction time in the observation group were shorter than those in the control group, and the differences were statistically significant(P<0.05). The physical function, emotional function, cognitive function, social function and overall health scores of the observation group were higher than those in the control group, the differences were statistically significant (P<0.05).There was no significant difference in the total incidence of complications between the two groups (P>0.05).
Conclusion
The application of ligation of the ascending branch of the uterine artery combined with row suture of lower uterus technique during cesarean section can reduce surgical bleeding and postoperative blood transfusion, shorten the postoperative recovery time of parturient, and improve the quality of life of the parturient.

From June 2017 to September 2019, 100 cases of puerpera with postpartum urinary incontinence and constipation treated in Maternal and Child Health Hospital of Fuzhou City in Jiangxi Province were selected as the research objects. According to the random number table method, they were divided into observation group and control group, with 50 cases in each group. The control group was given routine health education, and the observation group was given rehabilitation treatment with the biostimulus feedback instrument on the basis of the control group. The effect of treatment and adverse reactions were compared between the two groups, and the tension intensity of pelvic floor muscle tension before and after treatment of the two groups was recorded.
Results
The total effective rate of patients in the observation group was 100.00%, which was higher than that in the control group of 52.00%, the difference was statistically significant (P<0.05). There was no significant difference in the pelvic floor muscle tension score between the two groups before treatment (P>0.05). After treatment, the pelvic floor muscle tension score of the two groups was higher than that before treatment, and the of pelvic floor muscle tension score of the observation group after treatment was higher than that in the control group, the differences were statistically significant (P<0.05). There were no obvious adverse reactions occurring in the two groups during treatment.
Conclusion
The biostimulus feedback instrument can significantly improve the treatment effect of postpartum urinary incontinence and constipation patients, improve the tension intensity of pelvic floor muscle tension, and will not increase adverse reactions, which is worthy of clinical promotion.

Clinical effect of transumbilical single port laparoscopic myomectomy for the treatment of uterine fibroids

Objective
To explore the clinical effect of transumbilical single port laparoscopic myomectomy for the treatment of uterine fibroids.
Methods
A total of 80 patients with uterine fibroids who were treated in our hospital from February 2018 to March 2019 were selected as the research objects. They were divided into control group and observation group by a random number table method, with 40 cases in each group. The control group was treated with porous laparoscopic myomectomy, and the observation group was treated with transumbilical single-port laparoscopic myomectomy. The clinical indicators, postoperative follow-up, pain degree, satisfaction, complications, and pelvic-abdominal implantation were compared between the two groups.
Results
The operation time in the observation group was longer than that in the control group, the amount of intraoperative blood loss was less than that in the control group, and the anal exhaust time was shorter than that in the control group, the differences were statistically significant (P<0.05). There was no statistical difference in the length of hospitalization time between the two groups (P>0.05). After 6 months of follow-up, there was no significant difference in normal menstrual rate between the two groups (P>0.05); the recurrence rate of fibroids in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05); the pain visual analogue scale (VAS) score and the body image scale (BIS) score in the observation group were lower than those in the control group, the cosmetic score (CS) was higher than that in the control group, the differences were statistically significant (P<0.05). There were no significant differences in the total incidence of postoperative complications and the rate of pelvic-abdominal implantation between the two groups (P>0.05).
Conclusion
The effect of transumbilical single port laparoscopic myomectomy for patients with uterine fibroids is significant, which can effectively reduce the postoperative pain and reduce the possibility of intraperitoneal implantation of fibroids or sarcomas, and the postoperative incision is beautiful and does not increase complications occur.

Application effect of Mifepristone combined with Ethacridine Lactate Injection amniotic cavity injection on induced labor in the second trimester of placenta previa

Objective
To study the application effect of Mifepristone combined with Ethacridine Lactate Injection amniotic cavity injection on induced labor in the second trimester of placenta previa.
Methods
A total of 74 patients in Shenyang Maternity Hospital who received induced labors in the second trimester placenta previa from November 2013 to September 2018 were selected as subjects and divided into control group (n=37) and observation group (n=37) according to the random numbers table method. The control group received the amniotic cavity injection Ethacridine Lactate Injection treatment, the obeservation group received the Mifepristone combined with Ethacridine Lactate Injection amniotic cavity injection. The labor outcome, clinical indicators and adverse reactions of induction were compared between the two groups.
Results
The induction time in the observation group was shorter than that of the control group,the amount of postpartum blood loss in the observation group within 2 and 24 hours after delivery were less than those of the control group, the success rate of labor induction in the observation group was higher than that of the control group, and the differences were statistically significant (P<0.05). The quality of the curettage of the palace in the observation group ([24.80±1.50]mg) was lower than that of the control group ([54.56±3.05]mg), and the difference was statistically significant (t=27.431,P=0.000). The total effective rate of cervical maturity in the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). The total rate of placental fetal membrane residue and adhesion in observation group was lower than that in control group, and the difference was statistically significant (P<0.05). There were no statistically significant differences between the observation group and the control group in the incidence rates of gastrointestinal side effect and soft birth canal damage (P>0.05).
Conclusion
Mifepristone combined with Ethacridine Lactate Injection amniotic cavity injection is effective in the second trimester induction labor of placenta previa excluding placental implantation, softens the cervix obviously, shortens the labor course, reduces the maternal pain, reduces the palace to clear the probability and is worthy of clinical application.
