Application of Tolvaptan combined with Furosemide in patients with heart failure complicated with renal insufficiency

Objective To investigate the effect of Tolvaptan combined with Furosemide in the treatment of heart failure complicated with renal insufficiency. Methods A total of 70 patients with heart failure complicated with renal insufficiency admitted to the Third Affiliated Hospital of University of South China and the Second Affiliated Hospital of University of South China from December 2019 to December 2020 were selected as the research subjects, and were divided into experimental group (35 cases) and control group (35 cases) by random number table method. The experimental group was treated with topaptan combined with furosemide, while the control group was treated with furosemide. The cardiac function classification, urine volume, body weight, blood pressure, N-terminal B-type natriuretic peptide (NTproBNP), electrolyte and renal function changes were compared between the two groups after treatment. Results The grading of cardiac function in the experimental group was better than that in the control group, and the difference was statistically significant (P<0.05). There were no significant differences in urine volume, body weight and blood pressure between the two groups before treatment (P>0.05). After treatment, the weight of the experimental group was lower than that of the control group, and the difference was statistically significant (P <0.05). In the experimental group, urine volume after treatment was higher than before treatment and body weight was lower than before treatment, with statistical significance (P<0.05). There was no significant difference in blood pressure between the two groups before and after treatment (P>0.05). There was no significant difference in NT-probNP and electrolyte between the two groups before treatment (P>0.05). After treatment,nT-probNP in the experimental group was lower than that in the control group, and blood potassium level was higher than that in the control group, with statistical significance (P<0.05). After treatment, nT-probNP in the experimental group was lower than before treatment, and blood potassium level in the control group was lower than before treatment,the difference was statistically significant (P<0.05). There were no significant differences in sodium and chlorine levels between the two groups before and after treatment (P>0.05). There was no significant difference in renal function indexes between the two groups before treatment (P>0.05). After treatment, the levels of urea nitrogen and creatinine in the experimental group were lower than those in the control group, and the glomerular filtration rate was higher than that in the control group, with statistical significance (P<0.05). Conclusion Tolvaptan can effectively improve the symptoms and biochemical indicators of patients with heart failure and renal insufficiency, increase urine output, improve fluid balance, and have a good overall tolerance. It has great clinical value in the treatment of refractory heart failure.

Predictive value of blood leukocyte and neutrophil levels for aggressive behavior in first-episode schizophrenic patients

Objective To explore the predictive value of white blood cell (WBC) combined with neutrophil (N) levels and the occurrence of aggressive behavior in patients with first-episode schizophrenia. Methods A total of 60 patients with first-episode schizophrenia admitted to the Third People′s Hospital of Ji′an from July 2020 to December 2020 were selected as the research subjects. All patients were hospitalized for treatment for the first onset. According to the modified overt aggression scale (MOAS), they were divided into aggressive behavior group (n=30) and non-aggressive behavior group (n=30). The blood WBC and N levels of the two groups were compared, and the Pearson correlation coefficient test was used to analyze the blood WBC and N levels and the MOAS scale.The correlation of the scores of each item was analyzed by logistic regression to analyze the independent risk factors for the occurrence of aggressive behavior, and the receiver operating characteristic (ROC) curve was used to evaluate the predictive value of blood WBC and N levels for the occurrence of aggressive behavior. Results WBC and N levels in the aggressive group were higher than those in the non-aggressive group. Pearson correlation analysis showed that WBC and N levels were positively correlated with the scores of verbal aggression, self-aggression, physical aggression and property aggression in MOAS.Logistic regression analysis showed that WBC (β=2.149, OR=8.551, 95%CI= 2.291-32.065) and N (β=1.961, OR=7.803,95%CI=1.208-15.318) levels were independent risk factors for aggressive behavior. The area under the curve (AUC) of attack behavior predicted by the two indexes was 0.906. Conclusion The blood WBC and N levels of first-episode schizophrenia patients with aggressive behavior are significantly increased. Both indicators are independent risk factors leading to the occurrence of aggressive behavior in patients. The combined evaluation of the two can accurately predict the occurrence of aggressive behavior, so that clinicians could conduct early intervention and treatment.

Clinical effect of Biejia Ruangan Tablets combined with Entecavir in the treatment of chronic hepatitis B

Objective To investigate the clinical effect of Biejia Ruangan Tablets (BRT) combined with Entecavir (ETV)in the treatment of chronic hepatitis B (CHB). Methods A total of 174 CHB patients treated in People′s Hospital of Yichun City from September 2016 to December 2018 were selected as the research objects and divided into the control group and the observation group by random number table method, with 87 cases in each group. The control group was treated with ETV, and the observation group was treated with ETV and BRT. Liver inflammation grade, liver fibrosis grade, HBV DNA copy amount, serum alanine aminotransferase (ALT) and liver fibrosis index (hyaluronic acid [HA],type Ⅲprocollagen [PCⅢ], type Ⅳcollagen [Ⅳ-C], laminin [LN]) levels in the two groups were compared. Results At the 48th week of treatment, the grade of liver inflammation in the two groups were better than those before treatment,and the differences were statistically significant (P<0.05). At the 48th week of treatment, there was no statistically significant difference in the grade of liver inflammation between the two groups (P>0.05). At the 48th week of treatment,the grade of liver fibrosis in the observation group was better than that in the control group, and the difference was statistically significant (P<0.05). Before treatment, there were no statistically significant differences in serum ALT, HA, PCⅢ, Ⅳ-C and LN levels between the two groups (P>0.05). At the 48th week of treatment, the levels of ALT, HA, PCⅢ,Ⅳ-C and LN in the two groups were lower than those before treatment, and the differences were statistically significant(P<0.05). At the 48th week of treatment, the levels of ALT, HA, PCⅢ, Ⅳ-C and LN in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). There were no statistically significant differences in HBV DNA copy between the two groups before treatment, at the 12th, 24th and 48th week of treatment (P>0.05). Conclusion BRT is an effective adjuvant therapy for CHB, and it is of great value in improving the process of liver fibrosis.

Efficacy analysis of transumbilical single-port and three-port laparoscopic cholecystectomy in the treatment of calculous cholecystitis

Objective To investigate the clinical efficacy of transumbilical single-port and three-port laparoscopic cholecystectomy in the treatment of calculous cholecystitis. Methods A total of 86 patients with calculous cholecystitis admitted to Affiliated Central Hospital of Shenyang Medical College from January 2018 to June 2019 were selected as the research objects, and divided into the observation group and the control group according to the random number table method,with 43 cases in each group.The observation group was treated with transumbilical single-port laparoscopic cholecystectomy, and the control group was treated with transumbilical three-port laparoscopic cholecystectomy. The surgical time and postoperative hospitalization time, incision pain score, analgesia medication rate, incidence of complications, and cosmetic incision satisfaction score were compared between the two groups. Results The surgical time of the observation group longer than that of the control group, and the postoperative hospitalization time of the observation group was shorter than that of the control group, the differences were statistically significant (P<0.05). The postoperative cosmetic satisfaction score of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). The postoperative incision pain score and analgesia medication rate of the observation group were lower than those of the control group, the differences were statistically significant (P<0.05).Incidence of postoperative complications was compared between the two groups, and the difference was not statistically significant (P>0.05). Conclusion Transumbilical single-port laparoscopic cholecystectomy is used in the treatment of calculous cholecystitis. Although the operation time is relatively long, it can obtain good curative effect and has more advantages, that is, postoperative pain is lighter and can be reduce the use of analgesics, and to improve patients′satisfaction with incision beauty. It is worthy to use.

Determination of five components in Shudan Tablets by method of quantitative analysis of multi-components with a single-marker

Objective To establish a method for the determination of aloe-emodin, rhein, emodin, chrysophanol and physcion in Shudan Tablets by quantitative analysis of multi-components with a single-marker (QAMS). Methods The determination was performed on a Shimadzu VP-ODS C18 column (4.6 mm×250 mm, 5 μm) with mobile phase consisted of methanol-0.1% phosphoric acid solution (85∶15) at the flow rate of 1.0 ml/min. The detection wavelength was set at 254 nm, and the column temperature was 35℃. Using emodin as internal reference, the relative correction factors for other four components were established, and then the content of each component was calculated according to relative correction factors. Moreover the determination results were compared with those obtained by external standard method(ESM). Results The liner range of aloe-emodin, rhein, emodin, chrysophanol and physcion fell within the ranges of 0.4968-9.9365, 0.4791-9.5824, 5.4137-108.2739, 2.1948-43.8950 and 0.9563-19.1268 μg/ml, respectively. The average recoveries (n=6) were 95.22%, 96.46%, 98.29%, 97.90% and 95.88%, respectively. The relative correction factors of aloe-emodin, rhein, chrysophanol and physcion were 0.7640, 0.9118, 0.7257 and 0.9830, respectively, with good repeatability under the experimental conditions such as different instruments and chromatographic columns. Compared with the ESM, the determination results of QAMS showed no significant differences. Conclusion The QAMS is simple,accurate and reliable. It can be used for the quality analysis of multiple components in Shudan Tablets.

Atopic dermatitis is a type of skin disease, which belongs to allergic skin disease with atopic characteristics.The clinical manifestations of atopic dermatitis mainly include a variety of skin lesions and itching of different degrees,and the course of the disease is long and prone to recurrent attacks, so it has a serious negative impact on patients'health and quality of life. Atopic dermatitis is a chronic, pruritus, inflammatory and recurrent skin disease. In recent years, there are more and more clinical research literatures about this disease, and the clinical diagnosis and treatment guidance are further standardized and standardized. However, more attention should be paid to the clinical diagnosis and treatment of atopic dermatitis in order to improve the prognosis of the disease and improve the quality of life of patients. This article reviews the progress of diagnosis and treatment of atopic dermatitis.

Research progress of spindle assembly checkpoint proteins in meiosis of oocytes

Spindle assembly checkpoint proteins play an important role in meiosis of oocytes. It is a highly fidelity system for monitoring the accurate separation of chromosome. It can delay the transition from metaphase to anaphase until chromosomes are accurately arranged on the equatorial plate. In the research of spindle assembly checkpoint, the most important is that spindle assembly checkpoint protein′s influence on meiosis. If the spindle assembly checkpoint protein is abnormal or mutated, it may produce cells with abnormal chromosome number or morphology. This article reviews the location and function of some spindle assembly checkpoint proteins in meiosis.

Clinical significance of detection of serum cystatin C and urine renal injury molecule-1 in patients with early renal injury in hypertensive disorder complicating pregnancy

Objective To explore the clinical detection of serum cystatin C (SCys C) and urine renal injury molecule-1(KIM-1) in patients with early renal injury in hypertensive disorder complicating pregnancy. Methods From January to December 2017, 30 cases of patients with early renal injury in hypertensive disorder complicating pregnancy admitted to Jiangxi Maternal and Child Health Hospital were selected as the experimental group, 30 cases of patients with hypertensive disorder complicating pregnancy during the same period without renal injury were selected as the control group. The urine N-acetylglucosamine (NAG), 24 h urine protein quantification, SCys C, KIM-1 and the incidence of adverse maternal and infant outcomes were compared between the two groups. Results Urine NAG ([8.91±1.24] U/L),24 h urine protein quantification ([272.08±26.88] mg), SCys C ([7.05±1.02] mg/L), KIM-1 ([3.26±0.44] μmol/L) in the experimental group were higher than those in the control group ([1.72±0.35] U/L, [118.13±15.39] mg, [0.86±0.12] mg/L,[0.86±0.13] μmol/L), and the differences were statistically significant (P<0.05). The incidence of fetal growth restriction,neonatal asphyxia, neonatal hypoglycemia and respiratory distress in the experimental group were higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusion SCys C and UKIM-1 levels detection have practical values for the diagnosis of early renal injury in hypertensive disorder complicating pregnancy.

Clinical effect of subcutaneous buried vertical pad tension reduction suture combined with 90Sr-90Y application in the treatment of keloid after operation

Objective To investigate the clinical effect of subcutaneous buried vertical pad tension reduction suture combined with 90Sr-90Y application after operation. Methods A total of 100 cases of keloid treated in Hongqi Hospital Affiliated to Mudanjiang Medical University from August 2018 to February 2020 were selected as the research subjects. They were divided into observation group (58 cases) and control group (42 cases) according to the method of coin tossing. The observation group was treated with subcutaneous buried vertical bedding compression suture combined with 90Sr-90Y application, the control group was treated with 90Sr-90Y application, and the curative effect and Vancouver scar scale (VSS) score were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After 1 year of treatment, the VSS score of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Postoperative subcutaneous burying vertical pad compression suture combined with 90Sr-90Y application is a simple treatment, short treatment period, good efficacy, less side effects, suitable for clinical promotion.

Clinical effect of 311 nm narrow band ultraviolet B combined with Gabapentin in the treatment of post-herpetic neuralgia

Objective To investigate the clinical effect of 311 nm narrow band ultraviolet B (NB-UVB) combined with Gabapentin in the treatment of post-herpetic neuralgia (PHN). Methods A total of 84 PHN patients admitted to Shangrao Dermatology Hospital from January to December 2020 were selected as the research subjects, and they were divided into the observation group (42 cases) and the control group (42 cases) by a random number table methods. Patients in the control group were treated with Gabapentin, while patients in the observation group were added with 311 nm NB-UVB on the basis of the control group. The treatment effect, pain degree and adverse reactions were compared between the two groups. Results The effective rate of the observation group was higher than that of the control group,and the pain score of the observation group was lower than that of the control group, the differences were statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion The effect of 311 nm NB-UVB combined with Gabapentin in the treatment of PHN patients is better than Gabapentin alone, which is beneficial to improve the pain degree and treatment effect, and does not increase the adverse reactions, and the prognosis of patients is better.

Application effect of S-shaped rotating hook in fixation of drainage tubes after emergency trauma surgery

Objective To investigate the application effect of S-shaped rotating hook in the fixation of various drainage tubes after emergency trauma surgery. Methods A total of 60 patients with emergency trauma surgery treated in the Department of Emergency of the First Affiliated Hospital of Gannan Medical University from April 2019 to January 2021 were selected by double-blind method.They were randomly divided into the control group(n=30)and the observation group(n=30). The control group was given conventional fixation method, and the observation group was treated with S-shaped rotating hooks. The rate of tube detachment, status of smooth drainage, comfort during indwelling, and quality of life were compared between the two groups. Results The incidence rates of tube detachment, drainage disorder, retrograde infection,and lung infection in the observation group were lower than those in the control group,the patency rate of drainage tube in the observation group was higher than that in the control group, with statistically significant difference (P<0.05).The comfort during indwelling in the observation group was better than that in the control group, with statistically significant difference (P<0.05).The quality of life scores of the two groups after 7 days of intervention were higher than those before intervention,with statistically significant differences(P<0.05).The scores of physiological role,physical pain,physiological function, mental health, and emotional intelligence in the observation group after 7 days of intervention were higher than those in the control group, with statistically significant differences (P<0.05). Conclusion The application of S-shaped rotating hook in the fixation of drainage tubes after emergency trauma surgery can reduce the rate of tube detachment, improve the patency of drainage tube, improve the comfort of patients during indwelling, and achieve a higher quality of life. It is worthy of promotion and application.

Curative effect comparison of bare stent combined with covered stent TIPS in the treatment of portal hypertension and upper gastrointestinal bleeding

Objective To explore the curative effect of bare stent combined with covered stent transjugular intrahepatic portosystemic shunt (TIPS) in the treatment of portal hypertension and upper gastrointestinal bleeding (UGIB). Methods Sixty patients with portal hypertension and UGIB who were admitted to the Fifth People′s Hospital of Ganzhou City,Jiangxi Province from January 2019 to January 2020 were selected as the research objects and divided into the control group and the observation group according to the alternate grouping method, with 30 cases in each group. The control group was treated with conventional comprehensive treatment, and the observation group was treated with bare stent combined with covered stent TIPS. The portal vein parameters (portal pressure [PVP], portal vein flow rate [PVV]) before treatment and 24 hours of treatment, the effective rate of hemostasis within 24 hours, complications, shunt restenosis rate, recurrence bleeding rate and case fatality rate were compared between the two groups. Results At 24 hours of treatment, the PVP levels of the two groups were lower than those before treatment, PVV levels were higher than those before treatment, and the PVP of the observation group was lower than that of the control group, the PVV was higher than that of the control group, the differences were statistically significant (P<0.05). The effective rate of hemostasis within 24 h of the observation group (96.67%) was higher than that of the control group (23.33%), and the difference was statistically significant (P<0.05). There was no statistically significant difference in total incidence of complications between the two groups (P>0.05). The shunt restenosis rate (3.33%) and recurrent bleeding rate (10.00%) in observation group were lower than those in control group (26.67%, 33.33%), and the differences were statistically significant (P<0.05). There was no statistically significant difference in mortality between the two groups (P>0.05). Conclusion Bare stent combined with covered stent TIPS has high hemostasis efficiency in the treatment of patients with portal hypertension and UGIB, and reduces portal pressure without increasing the risk of complications, long-term recurrence of bleeding, and low mortality.

Analysis of predictive efficacy of time-glucose area under the curve combined with coagulation function indicators on perinatal outcomes in patients with gestational diabetes mellitus

Objective To analyze the predictive efficacy of time-glucose area under the curve (AUC) combined with coagulation function indicators on perinatal outcomes of patients with gestational diabetes mellitus (GDM). Methods The clinical data of 78 patients with GDM admitted to the Third Affiliated Hospital of Gannan Medical College between March 2018 and March 2020 were retrospectively analyzed. According to different perinatal outcomes, they were divided into the case group (with adverse perinatal outcomes, 40 cases) and the control group (without adverse perinatal outcomes, 38 cases). The AUC of time-glucose and coagulation function indicators (R value and K value of thromboelastic graph) were compared between the two groups, and receiver operating characteristic (ROC) curve was drawn to analyze the predictive efficacy of AUC of time-glucose and coagulation function related indicators on perinatal outcome of GDM patients. Results The AUC of time-glucose of case group was higher than that of the control group, and the difference was statistically significant (P<0.05). The R value and K value of thromboelastogram in the case group were lower than those of the control group, and the differences were statistically significant (P<0.05). The AUC of time-glucose and AUC values of thromboelastogram R value, K value were larger than the reference line (P<0.05), and the cut-off values were 17.62 mmol·h, 6.76 min, 1.46 min. Conclusion AUC of time-glucose and coagulation function indicators can effectively predict the perinatal outcomes of patients with GDM, and have high clinical application value.

Value of multiple tumor-associated autoantibodies in the diagnosis of nonsmall cell lung cancer

Objective To analyze the value of multiple tumor-related autoantibodies in the diagnosis of non-small cell lung cancer (NSCLC). Methods A total of 57 patients with newly diagnosed NSCLC who underwent radical resection of lung cancer in the Affiliated Hospital of Medical College of Jiaying University were selected as the NSCLC group between January 2019 and July 2020. Meanwhile, 52 patients with benign lung diseases (benign group) and 58 healthy people (healthy group) were selected as the control group. Seven tumor-related autoantibodies, including, tumor suppressor gene p53 (p53), protein gene product 9.5 (PGP9.5), sex determining region Y-box2 (SOX2), G antigen 7(GAGE7), tumor antigen (GBU4-5), cancer associated gene (GAGE) and melanoma antigen 1 (MAGE A1) were detected by ELISA. The receiver operating characteristic (ROC) curve was used to analyze the value of above seven tumor-related autoantibodies in the diagnosis of NSCLC. Results The levels of p53, PGP9.5, SOX2, GAGE7, GBU4-5, GAGE and MAGE A1 in the NSCLC group were higher than those in the benign group and the healthy group, and the differences were statistically significant (P<0.05). The area under curve values of p53, PGP9.5, SOX2, GAGE7, GBU4-5, GAGE,and MAGE A1 alone for diagnosing NSCLC were all larger than 0.6 (P<0.05), indicating certain diagnostic value. According to the largest Youden index, the cut-off values determined were 1.57 U/ml, 12.92 U/ml, 12.67 U/ml, 14.64 U/ml,8.45 U/ml, 10.65 U/ml, and 7.68 U/ml, respectively. For single diagnosis, the sensitivity of p53 was the highest(91.20%), followed by GAGE (68.40%), and GP 9.5 and GAGE7 (both 59.60%). Conclusion All of the 7 tumor-associated autoantibodies have certain value in the diagnosis of NSCLC.

Application effect of restrictive fluid management in patients with chronic obstructive pulmonary disease complicated with type Ⅱrespiratory failure

Objective To study the clinical effect of restrictive fluid management in the treatment of patients with chronic obstructive pulmonary disease (COPD) complicated with type Ⅱrespiratory failure. Methods Totally 120 patients with COPD complicated with type Ⅱrespiratory failure admitted to Ji′an Hospital of Shanghai East Hospital from January 2019 to December 2020 were selected as the study subjects, and they were divided into observation group and control group according to the random number table method, with 60 cases in each group. The two groups were given conventional treatment, and patients in control group were given non-restrictive fluid management on the basis of conventional treatment, and patients in observation group were given restrictive fluid management on the basis of conventional treatment. The ventilation time and fluid replacement volume of the two groups were compared within 1 week of treatment. The blood gas indexes (partial pressure of carbon dioxide [PaCO2], arterial oxygen pressure [PaO2], blood oxygen saturation [SpO2], pH) and blood indexes (lactic acid [LA], N-terminal B-type natriuretic peptide [NT-proBNP])were compared between the two groups before treatment and after 1 week of treatment. Results Within 1 week of treatment, the ventilation time and fluid replacement volume in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After 1 week of treatment,the LA,NT-proBNP and PaCO2 of observation group were lower than those of control group, the differences were statistically significant (P<0.05). The pH value, PaO2 and SpO2 of observation group after 1 week of treatment were higher than those of control group, the differences were statistically significant (P<0.05). Conclusion Restrictive fluid management for patients with COPD complicated with type Ⅱrespiratory failure is beneficial to restore the normal blood gas levels and shorten the ventilation time.

Effect of nasal high flow humidification oxygen therapy on patients after tracheal intubation and extubation and its influence on blood gas analysis

Objective To explore the effect of nasal high flow humidification oxygen therapy on patients after tracheal intubation and extubation and its influence on blood gas analysis. Methods From January 2018 to June 2020, 100 patients with tracheal intubation and extubation in the Department of Respiratory and Critical Care Medicine of Lianjiang People′s Hospital were selected as the research objects, and they were divided into experimental group (50 cases)and control group (50 cases) according to the random number table method. The control group was treated with noninvasive positive pressure ventilation, and the experimental group was treated with nasal high flow humidification oxygen therapy. The total effective rate of airway humidification, clinical outcome (times of sputum suction under fiberoptic bronchoscope, autonomous breathing time, ICU residence time) and changes of blood gas analysis indexes (pH value, arterial osygen pressure [PaO2] value, partial pressure of carbon dioxide in artery [PaCO2] value) were compared between the two groups. Results The total effective rate of airway humidification in the experimental group was higher than that in the control group, the difference was statistically significant (P<0.05). The frequency of sputum suction in the experimental group was less than that in the control group, the difference was statistically significant (P<0.05). The time of spontaneous breathing in the experimental group was longer than that in the control group, the difference was statistically significant (P<0.05). The ICU residence time of the experimental group was shorter than that in the control group,the difference was statistically significant (P<0.05). There were no differences in blood gas analysis indexes between the two groups before treatment (P>0.05). There were no differences in blood gas analysis indexes between the two groups after treatment (P>0.05). Conclusion The effect of nasal high flow humidification oxygen therapy on patients after tracheal intubation and extubation is significant.

Effect of weight management combined with pain reduction during pregnancy on pregnancy outcome and labor process

Objective To investigate the effect of weight management combined with pain reduction on pregnancy outcome and labor process. Methods A total of 60 cases of puerpera admitted to Shenyang Women′s and Children′s Hospital from January 2020 to October 2020 were selected as the research object and divided into the study group the and control group according to random number table method, with 40 cases in each group. The study group carried out weight management combined with pain reduction during pregnancy, while the control group carried out routine intervention. The total incidence of adverse pregnancy outcomes, labor course, 2 h postpartum blood loss and the total incidence of neonatal adverse outcomes were compared between the two groups. Results The total incidence of adverse pregnancy outcomes in the study group was lower than that in the control group, and the difference was statistically significant (P<0.05). The duration of labor in the study group was shorter than that in the control group, and the amount of 2 h postpartum blood loss in the study group was less than that in the control group, and the differences were statistically significant (P<0.05). The total incidence of neonatal adverse outcomes in the study group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Pregnancy weight management combined with pain reduction method can effectively control the newborn weight, increase the rate of vaginal delivery and improve the outcome of mother and baby.

High-risk factors for patients with type Ⅱrespiratory failure using non-invasive mechanical ventilation to treat abdominal distension

Objective To investigate the high-risk factors of abdominal distension in patients with type Ⅱrespiratory failure using non-invasive mechanical ventilation. Methods The clinical data of 118 patients with type Ⅱrespiratory failure who underwent non-invasive mechanical ventilation in Beijing Fengtai Hospital from January 2019 to November 2020 were retrospectively analyzed, and the high-risk factors of abdominal distension in patients with type Ⅱrespiratory failure after non-invasive mechanical ventilation were analyzed. Results After non-invasive mechanical ventilation treatment, 52 cases (44.07%) of type Ⅱrespiratory failure patients with abdominal distension, 66 cases(55.93%) without abdominal distension; univariate analysis showed that age, self-feeding, gastrointestinal decompression,open-mouth breathing, 24 h total ventilation time, electrolyte changes and gastrointestinal function changes were associated with abdominal distension after non-invasive mechanical ventilation in patients with type Ⅱrespiratory failure, and the difference was statistically significant (P<0.05). Gender, nasal feeding, exhalation positive pressure support, inhalation positive pressure support, lung infection had no significant difference with abdominal distension after non-invasive mechanical ventilation (P>0.05). Multivariate analysis showed that age ≥60 years (β=1.059, OR=2.884, 95%CI=1.322-6.292), open-mouth respiration (β=2.216, OR=9.173, 95%CI= 3.939-21.364), 24 h total ventilation time ≥10 h (β=1.866, OR=6.462, 95%CI= 2.767-15.090), electrolyte changes (β=2.659, OR=14.280, 95%CI= 5.815-35.065), gastrointestinal function changes (β=1.427, OR=4.167, 95%CI=1.819-9.546) were associated with abdominal distension after non-invasive mechanical ventilation in patients with type Ⅱrespiratory failure (P<0.05).Conclusion Abdominal distension in patients with type Ⅱrespiratory failure after non-invasive mechanical ventilation is closely related to age, mouth-opening breathing, total time of 24 h ventilation, changes in electrolytes, and changes in gastrointestinal function, and clinical attention should be paid to it.

Analysis of the causes and influence factors of red blood cell spill-out caused by apheresis platelets in the voluntary blood donation at blood station

Objective To explore the reasons and related influence factors of red blood cell spill-out caused by apheresis platelets (PLT) in the voluntary blood donation at blood stations. Methods A total of 1532 voluntary blood donors who donated PLT at the blood station in Ji′ning Blood Center, Shandong Province from January 2020 to January 2021 were selected as the research subjects. The status of RBC spill-out caused by apheresis PLT was counted, and the related influence factors of red blood cell spill-out caused by apheresis PLT were analyzed. Results Among the 1532 cases of voluntary blood donors, a total of 60 cases had red blood cell spill-out phenomenon, and the red blood cell spill-out rate of apheresis PLT was 3.92%. Single factor analysis showed that the differences of deposited (HCT),count of red blood cells (RBC), the average amount of hemoglobin (MCH), average red blood cell volume (MCV), mean hemoglobin concentration (MCHC) level, blood fat ratio, stress ratio and supplies operating errors of the two groups were statistically significant (P<0.05). Multivariate analysis showed that HCT increased (β=2.735, OR=9.784, 95%CI=3.272-18.095), RBC increased (β=2.262, OR=5.611, 95%CI=1.818-12.487), MCH decreased (β=2.097, OR=8.143,95%CI=3.178-16.205), MCV decreased (β=2.219, OR=6.053, 95%CI=1.028-9.099), MCHC decreased (β=3.797, OR=34.571, 95%CI=14.47-107.231), fat blood (β=2.856, OR=23.063, 95%CI=10.028-69.033), mental stress (β=2.436, OR=17.053, 95%CI=9.028-57.059), consumable operation error (β=2.052, OR=7.053, 95%CI=2.028-15.091) were risk factors of the red blood cell spill-out of apheresis PLT (P<0.05). Conclusion The red blood cell spill-out caused by apheresis PLT in the voluntary blood donation of blood stations is closely related to the increase of HCT, the increase of red blood cell, the decrease of MCH, MCV and MCHC, fat blood in the blood of patients, mental stress, and consumable operation error. In clinical practice, targeted measures should be taken to eliminate the influence factors so as to reduce red blood cell spill-out caused by apheresis PLT.

Correlation analysis between the expression of procalcitonin, interleukin-6,C-reactive protein and pulmonary function in patients with chronic bronchitis

Objective To investigate the relationship between the expression of procalcitonin (PCT), interleukin 6 (IL-6),C-reactive protein (CRP) and pulmonary function in patients with chronic bronchitis. Methods Clinical data of 79 patients with chronic bronchitis in Changqing District People′s Hospital of Jinan City from October 2018 to October 2020 were retrospectively analyzed. According to the progress of the disease, they were divided into the acute exacerbation group (41 cases) and the remission group (38 cases). A total of 30 healthy people in the same period were selected as the control group. The serum PCT, IL-6, CRP levels and related pulmonary function indexes such as the forced vital capacity (FVC), forced expiratory volume in the first second (FEV1) and maximum ventilation volume (MVV) were compared among the three groups, and the correlation analysis was carried out. Results There were statistically significant differences in the levels of PCT, IL-6 and CRP among the three groups (P<0.05). The levels of PCT, IL-6 and CRP in the acute exacerbation group and the remission group were higher than those in the control group, and the differences were statistically significant (P<0.05). The levels of PCT, IL-6 and CRP in the acute exacerbation group were higher than those in the remission group, and the differences were statistically significant (P<0.05). There were statistically significant differences in the FVC, FEV1 MVV among the three groups (P<0.05). The levels of FVC, FEV1and MVV in the acute exacerbation group and the remission group were lower than those in the control group, and the differences were statistically significant (P<0.05). The levels of FVC, FEV1 and MVV in the acute exacerbation group were lower than those in the remission group, and the differences were statistically significant (P<0.05). Pearson correlation analysis showed that the levels of PCT,IL-6 and CRP were negatively correlated with FVC,FEV1 and MVV(r=-0.693,-0.724,-0.738, P<0.05). Conclusion The levels of PCT, IL-6 and CRP in patients with chronic bronchitis are correlated with FVC, FEV1 and MVV, which can provide reference for clinical treatment and disease monitoring.

Correlation between fasting triacylglycerol and atherosclerosis index at different times

Objective To investigate the correlation between fasting triacylglycerol (TAG) and atherosclerosis index (AI)at different times.Methods A total of 24 outpatients with type 2 diabetes mellitus(T2DM)in Traditional Chinese Medicine Hospital of Urumqi from January 2019 to December 2019 were selected. The patients were divided into fasting TAG increased group (n=10, fasting TAG>1.8 mmol/L) and fasting TAG normal group (n=14, fasting TAG≤1.8 mmol/L), according to fasting TAG level. The general data, incidence of atherosclerosis, peak time of TAG, lipids metabolism at fasting and 2, 4 and 6 hours after meal (TAG, total cholesterol [TC], low density lipoprotein [LDL-C] and high density lipoprotein [HDL-C]), blood glucose, insulin, glycosylated hemoglobin (HbA1c), C-Peptide, tumor necrosis factor-α (TNF-α)and malondialdehyde (MDA) were compared in the two group to analyze the correlation between TAG and AI. Results The peak time of both group was 4 hours after meal. The TAG and TC of fasting and postprandial 2, 4 and 6 hours in the fasting TAG increased group were higher than fasting TAG normal group with statistically significant difference (P<0.05). AI of postprandial 2, 4 and 6 hours in fasting TAG increased group was higher than fasting TAG normal group(P<0.05). Fasting TAG was positively correlated with fasting AI (r=0.552, P<0.05). Postprandial 2 hours TAG was positively associated with postprandial 2 hours AI (r=0.868, P<0.05). Postprandial 4 hours TAG was positively associated with postprandial 4 hours AI (r=0.710, P<0.05). Postprandial positive correlation of 6 hours TAG with postprandial 6 hours AI (r=0.767, P<0.05). Conclusion There is a positive correlation between TAG and AI in T2DM patients. Therefore, early detection of fasting and postprandial TAG levels in T2DM patients can provide data support for clinical evaluation of dyslipidemia and judgment of atherosclerosisThere is a positive correlation between TAG and AI in T2DM patients. Research on narrative medicine promoting psychological resilience of junior psychiatric nurses

Objective To explore the role of narrative medicine in promoting the improvement of psychological resilience of junior psychiatric nurses. Methods A total of 62 cases of N0-N1 junior nurses in the Department of Psychiatry of Jiangxi Kangning Hospital from August 2018 to August 2020 were selected as the study subjects and divided into the experimental group and the control group according to the random number table method, with 31 nurses in each group. The control group was given routine education, the experimental group was given narrative medicine intervention every 2 weeks on the basis of the control group, and both groups were intervened for 6 months. The mental health status and psychological toughness after 6 months of intervention were compared between the two groups. Results Before the intervention, there was no significant difference in the symptom checklist 90 (SCL-90) scores between the two groups (P>0.05). After 6 months of intervention, the scores of interpersonal sensitivity, depression, anxiety, hostility, and psychosis in the experimental group were lower than those in the control group, and the differences were statistically significant (P<0.05). Before the intervention, there was no significant difference in the resilience scale (CD-RISC)scores between the two groups (P>0.05). After 6 months of intervention, the CD-RISC score in the experimental group was (112.84±10.26) points, which was higher than that in the control group ([95.25±9.32] points), and the difference was statistically significant (P<0.05). Conclusion Narrative medicine intervention can improve the psychological pressure of junior psychiatric nurses and improve their psychological resilience.

Application effect of acupuncture combined with acupoint injection on foot varus after stroke

Objective To investigate the application effect of acupuncture combined with acupoint injection in the treatment of foot varus after stroke. Methods A total of 84 patients with foot varus after stroke admitted to the First People′s Hospital of Fuzhou from August 2018 to August 2020 were selected as the research objective, and divided into the control group (42 patients) and the observation group (42 patients) according to the random number table method.The control group were treated with acupuncture, and the observation group were treated with acupuncture combined with acupoint injection. The clinical efficacy, motor function (Fugl-Meyer motor function scale for lower extremity[FMA-L], modified Ashworth scale [MAS] and Berg balance scale [BBS]), living ability [modified Barthel index (MBI)]and quality of life (short form 36-item health survey [SF-36]) were compared between the two groups. Results The total clinical effective rate in the observation group was higher than that in the control group, with statistically significant difference (P<0.05). Before treatment, there were no statistically significant differences in the scores of FMA-L, MAS,BBS, MBI and SF-36 between the two groups (P>0.05). After treatment, the scores of FMA-L, BBS, MBI and SF-36 in the observation group were higher than those in the control group, and the MAS score in the observation group was lower than that in the control group, with statistically significant differences (P<0.05). Conclusion The application of acupuncture combined with acupoint injection can improve motor function, daily living ability, and quality of life in patients with foot varus after stroke, which benefits the prognosis.

Changes of anterior cingulate function in patients with different types of depressive disorders under resting-state functional magnetic resonance imaging and its differential diagnosis value

Objective To investigate the changes of anterior cingulate function in patients with different types of depressive disorders under resting-state functional magnetic resonance imaging (fMRI), and its differential diagnosis value. Methods The clinical data of 50 patients with unipolar depression (UD) and bipolar depression (BD) from the Affiliated Brain Hospital of Guangzhou Medical University were retrospectively analyzed. Meanwhile, a total of 50 healthy individuals were selected as the control group. The three groups all received resting-state functional magnetic resonance imaging scan, and the functional connectivity characteristics of anterior cingulate were compared among the three groups. Results The connectivity between left anterior cingulate and left triangle inferior frontal gyrus as well as left top-inferior angular gyrus, and the connectivity between right anterior cingulate and left triangle inferior frontal gyrus in UD group were stronger than those in the control group, the differences were statistically significant (P<0.05). The connectivity between left anterior cingulate and right middle temporal gyrus, the connectivity between right anterior cingulate and left central posterior gyrus, right superior temporal gyrus, and right middle temporal gyrus in BD group were weaker than those in the control group, the differences were statistically significant (P<0.05). The connectivity between left anterior cingulate and left triangle inferior frontal gyrus as well as right triangle inferior frontal gyrus, the connectivity between right anterior cingulate and left triangle inferior frontal gyrus, left top-inferior angular gyrus, and right top-inferior angular gyrus in UD group were stronger than those in BD group, the differences were statistically significant (P<0.05). Conclusion Compared with normal people, there are obvious changes of anterior cingulate function in patients with UD and BD in resting state. Patients with UD mainly manifest as enhanced functional connectivity of anterior cingulate, and patients with BD have weakened functional connectivity of anterior cingulate, which provides reference for distinguishing UD from BD and helps to guide treatment and medication of physicians.

Effect of Duloxetine combined with repetitive transcranial magnetic stimulation in the treatment of elderly patients with depressive disorder and its influence on the level of brain-derived neurotrophic factor

Objective To investigate the therapeutic effect of Duloxetine combined with repetitive transcranial magnetic stimulation (rTMS) in the treatment of elderly patients with depressive disorder and its influence on the expression of serum brain neurotrophic factor (BDNF). Methods A total of 60 elderly depressive patients in Qingdao Mental Health Center were selected from December 2016 to August 2017. All patients were divided into the true stimulation group and the pseudo stimulation group according to random number table methods, 30 cases in each group. Patients in the true stimulation group were treated with Duloxetine and combined with rTMS, while patients in the pseudo stimulation group were treated with Duloxetine and combined with rTMS pseudo stimulation. The 17-item Hamilton depression rating scale (HAMD-17) and the repeatable battery for the assessment of neuropsychological status (RBANS) were compared and analyzed before treatment and 8 weeks after treatment. Fasting blood samples were taken on the second day of admission and the morning of the 8th weekend of treatment to detect the level of serum BDNF. Results At the end of 8 weeks of treatment, the HAMD-17 scores of the two groups were lower than those before treatment, and the HAMD-17 score of the true stimulation group was lower than that of the pseudo stimulation group at the end of 8 weeks of treatment, the differences were statistically significant (P<0.05). The RBANS scores of the two groups at the end of 8 weeks of treatment were higher than those before treatment, and the RBANS scores of the real stimulation group at the end of 8 weeks of treatment were higher than those of the pseudo stimulation group, the differences were statistically significant (P<0.05). The levels of BDNF in blood samples of the two groups at the end of 8 weeks of treatment were higher than those before treatment, and the level of BDNF in blood samples of the real stimulation group at the end of 8 weeks of treatment was higher than that of the pseudo stimulation group, the differences were statistically significant (P<0.05). Conclusion Duloxetine combined with low rTMS can effectively improve cognitive function and the symptoms of depression in the treatment of depression in older patients.

Influence factors of cerebral vasospasm in patients with intracranial aneurysm after clipping

Objective To analyze the related factors of cerebral vasospasm in patients with intracranial aneurysm (IA)after clipping. Methods Clinical data of 65 IA patients admitted to Jieyang People′s Hospital of Guangdong Province from January 2018 to December 2020 were retrospectively analyzed. Baseline data of patients were collected, followed up for 3 months after clipping, and the occurrence of cerebral vasospasm during the follow-up period was recorded to analyze the factors leading to cerebral vasospasm in IA patients after clipping. Results According to whether cerebral vasospasm occurred during the follow-up period, the patients were divided into the occurrence group and the non-occurrence group. Among 65 IA patients, 16 patients had cerebral vasospasm, with an incidence of 24.62%. The incidence of hypertension, location of aneurysm, number of aneurysm rupture and Hunt-Hess grading were compared with those in the non-occurrence group, the differences were statistically significant (P<0.05). Multivariate logistic regression analysis showed that hypertension, aneurysm location in anterior cerebral artery, aneurysm rupture ≥1 time and Hunt-Hess grade >Ⅲwere the risk factors for cerebral vasospasm in IA patients after clipping (P<0.05). Conclusion Clinical intervention should be carried out according to the risk factors of cerebral vasospasm in IA patients after clipping, so as to effectively treat and prevent the occurrence of cerebral vasospasm.

Clinical effect of frontal keyhole endoscopy in the treatment of hypertensive basal ganglia intracerebral hemorrhage

Objective To investigate the effectiveness and practicability of neuroendoscopy through frontal keyhole craniotomy in the treatment of hypertensive basal ganglia intracerebral hemorrhage. Methods The clinical data of 40 patients with basal ganglia intracerebral hemorrhage treated in the Department of Neurosurgery of Xiehe Shenzhen Hospital of Huazhong University of Science and Technology from January 2019 to December 2020 were analyzed retrospectively. According to the treatment methods, they were divided into the observation group and the control group,with 20 cases in each group. The observation group was treated with neuroendoscope assisted transfrontal keyhole craniotomy, and the control group was treated with microscopical craniotomy. The perioperative indexes and complications of the two groups were recorded. The patients were followed up for 6 months, the prognosis was evaluated by Glasgow outcome scale (GOS). Results The operation time of the observation group was shorter than that of the control group,the amount of intraoperative blood loss and postoperative residual volume of the observation group were less than those of the control group, and the hematoma clearance rate of the observation group was higher than that of the control group, the differences were all statistically significant (P<0.05). The incidence of postoperative complications in the observation group was lower than that in the control group (5.0%[1/20] vs. 35.0%[7/20]), the difference was statistically significant(χ2=5.630, P<0.05). There was no death in the two groups at 6 months after operation, and the good prognosis rate in the observation group was higher than that in the control group (85.0%[17/20] vs. 60.0%[12/20]), the difference was statistically significant (χ2=7.35, P<0.05). Conclusion Neuroendoscopy through frontal keyhole craniotomy is effective and minimally invasive in the treatment of hypertensive basal ganglia intracerebral hemorrhage, with high hematoma clearance rate, and can improve the prognosis of patients, which is worthy of promotion.

Objective To investigate the clinical effect of Dipyridamole combined with Shuxuening in the treatment of acute cerebral infarction over thrombolysis time window. Methods A total of 96 patients with acute cerebral infarction over thrombolysis time window in Zhuanghe Central Hospital, Liaoning Province from September 2018 to March 2020 were selected as the research subjects. They were divided into a reference group (48 cases) and a joint group (48 cases)according to the random number table method. Patients in the reference group were treated with conventional symptomatic treatment + Dipyridamole via intravenous drips, while patients in the joint group were treated with conventional symptomatic treatment + Dipyridamole + Shuxuening via intravenous drips. Neurological function, limb function, daily live activity ability, efficacy, improvement of coagulation indexes and incidence of adverse reactions were compared between the two groups. Results After treatment, the scores of National Institutes of Health stroke scale (NIHSS) in the joint group were lower than those in the reference group, with statistically significant different (P<0.05). The scores of Fugl-Meyer assessment scale (FMAS) and activity of daily living scale (ADL) in the joint group were higher than those in the reference group, the differences were statistically significant (P<0.05). The total effective rate of the joint group was higher than that of the reference group, with statistically significant different (P<0.05). After treatment, the prothrombin time (PT), thrombin time (TT), activated partial thromboplastin time (APTT) in the joint group were longer than those in the reference group, and fibrinogen (FIB) in the joint group was lower than that in the reference group,the differences were statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion The application of dipyridamole combined with Shuxuening in the treatment of acute cerebral infarction patients over thrombolysis time window has a significant effect, and the neurological function, limb function, life activity ability and coagulation indexes have been significantly improved, with few adverse reactions, which is safe and reliable.

Clinical study of improved urinary catheterization injecting with tetracaine mucilage in male urinary catheterization

Objective To investigate the effect of improved urinary catheter injection of tetracaine mucilage in male catheterization. Methods A total of 85 patients undergoing percutaneous nephrolithotomy under general anesthesia in the Department of Anesthesiology, No. 908 Hospital of Joint Logistic Support Force of the People′s Liberation Army from January 2018 to July 2020 were selected as the research subjects. According to random number table method, they were divided into control group (42 cases) and treatment group (43 cases). The control group was lubricated with liquid paraffin oil, and the treatment group was lubricated with tetracaine hydrochloride mucilage. The incidence of adverse reactions between the two groups was compared, including average arterial pressure (MAP) and heart rate (HR) during catheterization (T1), resuscitation (T2), 10 min after tracheal catheter removal (T3), and agitation score during resuscitation. Results The total incidence of adverse reactions in the treatment group was 2.33%, lower than 33.33% in the control group, and the difference was statistically significant (P<0.05). At T1, there were no significant differences in MAP and HR between the two groups (P>0.05). At T2 and T3, MAP and HR of the treatment group were lower than those of the control group, with statistical significance (P<0.05). The resuscitation agitation score of the treatment group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The injection of tetracaine mucilage into the improved urethral catheter can effectively relieve the discomfort of the indwelling urethral catheter in men, improve the agitation during postoperative anesthesia recovery, stabilize postoperative MAP and HR,and facilitate postoperative recovery.

Effect of Dexmedetomidine and Remifentanil combined with Sevoflurane in endoscopic retrograde cholangiopancreatography

Objective To compare the efficacy of Dexmedetomidine and Remifentanil combined with Sevoflurane in endoscopic retrograde cholangiography (ERCP). Methods A total of 80 elective ERCP patients admitted to Fujian Provincial Hospital South Branch from March 2019 to February 2020 were selected as the study subjects, and divided into Dexmedetomidine (DEX) group (40 cases) and Remifentanil (REMI) group (40 cases) according to computer random number method.In DEX group,DEX 0.8 μg/kg was pumped 10 min before induction of anesthesia,DEX 0.2 g/(kg·h) was maintained during surgery,and pumping was stopped 10 min before the end of surgery.In the REMI group, 2.5 μg/(kg·h)REMI was pumped from 10 min before induction of anesthesia to the end of surgery. Postoperative pain intensity, postoperative analgesia needs, incidence of adverse reactions, postoperative recovery, extubation time, anesthesia time and Sevoflurane usage were compared between the two groups. The hemodynamic change of two groups were compared before drug pumping (T0), 10 min after drug pumping (T1), immediately after endotracheal intubation (T2), 1 min after intubation (T3), immediately after operation (T4), 15 min after operation (T5), when entering the post-anesthesia monitoring treatment room (T6), immediately after extubation (T7), 1 min after extubation (T8). Results There were no significant differences in postoperative pain intensity, analgesic need, incidence of adverse reactions, postoperative awakening, extubation time and anesthesia time between two groups (P>0.05). The Sevoflurane dose and dose per unit time in DEX group were lower than those in REMI group, and the differences were statistically significant (P<0.05). In terms of hemodynamic changes, blood pressure at T3 and T4 in DEX group was lower than that in REMI group, heart rate at T2-T8 in DEX group was lower than that in REMI group, and the differences were statistically significant (P<0.05). There were no significant differences in blood pressure and heart rate between the two groups at other time points (P>0.05). Conclusion Low-dose Dexmedetomidine is used for ERCP treatment and is equivalent to Remifentanil. However, Dexmedetomidine can reduce intraoperative stress induced hemodynamic fluctuations and the need for inhalation anesthesia, and can be safely used as an alternative adjuvant to Remifentanil.

Effect of transverse abdominal fascia block combined with Butorphanol preemptive analgesia in daytime laparoscopic surgery

Objective To observe effect of transverse abdominal fascia block combined with Butorphanol preemptive analgesia in daytime laparoscopic surgery. Methods A total of 90 patients who underwent daytime laparoscopic cholecystectomy in Ganzhou People′s Hospital from January 1, 2021 to April 10, 2021 were selected as the research subjects, and were divided into study group (45 cases) and control group (45 cases) according to random number table. In the study group, 0.01 mg/kg Butorphanol was given intranasally 10 min before induction of anesthesia, while in the control group, the same dose of normal saline was given intranasally. After surgery, bilateral transverse abdominal fascia was blocked under ultrasound guidance in both groups. The dosage of Remifentanil, operation time, anesthesia time, extubation time and stay time in resuscitation room were compared between the two groups. Postoperative analgesia scores were scored at 2, 4, 6, 12 h after surgery and at the time of discharge (T1-T5). The number of additional tramadol patients, the incidence of adverse reactions, analgesic satisfaction and the rate of untimely discharge during postoperative analgesia were analyzed. Results There were no significant differences in Remifentanil dosage, operation time, recovery time, extubation time and stay time in resuscitation room between two groups (P>0.05). The visual analog scale (VAS)score of the study group at T1-T4 were lower than those of the control group, and the differences were statistically significant (P<0.05). The additional number of Tramadol in the study was less than that in the control group, the satisfaction of study group was higher than that of the control group, and the incidence of postoperative nausea, vomiting and lethargy in the study group was lower than the control group, and the differences were statistically significant (P<0.05).There was no significant difference in the rate of untimely discharge between the two groups (P>0.05). Conclusion Ultrason-guided transverse abdominal fascia block combined with nasal preemptive analgesia of butorphano is suitable for analgesia in daytime laparoscopic cholecystectomy, which can provide a good analgesic effect and does not affect the recovery time of patients, with high safety.

Clinical effects of different doses of Remazolam in induction of general anesthesia

Objective To investigate the clinical effect of different doses of Remazolam on induction of general anesthesia. Methods A total of 100 patients undergoing elective tracheal intubation under general anesthesia admitted to the First People′s Hospital of Lianyungang from January to March 2021 were selected as the research subjects, and they were divided into group A (25 cases), group B (25 cases), group C (25 cases) and group D (25 cases) according to random number table method. Sufentanil and Cisatracurium were used for induction of general anesthesia in all four groups. On this basis, group A was treated with 2 mg/kg Propofol, group B was treated with 0.2 mg/kg Remazolam,group C was treated with 0.3 mg/kg Remazolam, and group D was treated with 0.4 mg/kg Remazolam. The mean arterial pressure (MAP) and heart rate (HR) of the four groups were compared at entry into the operating room (T0), bispectral index of electroencephalogram after drug administration (BIS) ≤60 (T1) and endotracheal intubation (T2). The onset time and times of additional administration were compared among the four groups. The incidence of adverse reactions in four groups was compared. Intraoperative awareness assessment was performed 1 day after surgery. Results At T1, MAP and HR of the four groups were lower than those of the same group at T0, the difference was statistically significant (P<0.05), but there was no significant difference between MAP and HR of the four groups at T0, T1 and T2 (P>0.05). The onset time of drugs in groups B, C and D was longer than that in group A, group B was longer than that in groups C and D, and group C was longer than that in group D, with statistical significance (P<0.05). The frequency of additional administration in group B was higher than that in groups A, C and D, and the difference was statistically significant (P<0.008). The total incidence of adverse reaction in group B, C and D was lower than that in group A, and the differences were statistically significant (P<0.008). None of the patients in the four groups had any known intraoperative occurrence. Conclusion Remazolam can be used effectively and safely in induction of general anesthesia with tracheal intubation, and the effect and safety of 0.3 mg/kg is better.

Clinical comparison of electrocardiogram and cardiac color Doppler ultrasound in diagnosis of hypertensive heart disease

Objective To compare the clinical value of electrocardiogram and cardiac color Doppler ultrasound in diagnosing hypertensive heart disease. Methods A total of 70 patients with suspected hypertensive heart disease who were examined in the Second People′s Hospital of Jingdezhen from October 2019 to May 2020 were selected as the research subjects. All patients underwent heart color ultrasound and electrocardiogram examination, and the comprehensive examination results of clinical history, physical examination and laboratory examination were taken as the gold standard,the value of two examination methods in the diagnosis of hypertensive heart disease were compared. Results Among 70 suspected hypertensive heart disease patients, 46 hypertensive heart disease patients were confirmed by the gold standard. The accuracy of hypertensive heart disease diagnosed by cardiac color Doppler ultrasound was 92.85% (65/70),the sensitivity was 93.48% (43/46), the specificity was 91.67% (22/24), and the positive predictive value was 95.56%(43/45), the negative predictive value was 88.00% (22/25), were all higher than those diagnosed by electrocardiogram[68.57% (48/70), 71.74% (33/46), 62.50% (15/24), 78.57% (33/42), 53.57% (15/28)], the difference was statistically significant (P<0.05). According to Kappa consistency measurement, the diagnosis of hypertensive heart disease by cardiac color Doppler ultrasound had good consistency with the comprehensive examination results (Kappa=0.843, P=0.000);the diagnosis of hypertensive heart disease by electrocardiogram had poor consistency with comprehensive examination results (Kappa=0.329, P=0.000). Conclusion Cardiac Color Doppler ultrasound applied to the clinical diagnosis of hypertensive heart disease has higher accuracy, sensitivity and specificity of clinical diagnosis than those of electrocardiogram, and the results have good consistency with the results of comprehensive examination, which can provide reliable and effective imaging basis for hypertensive heart disease, and the diagnosis effect is better.

Study on color Doppler ultrasonography of acquired immune deficiency syndrome complicated with cervical lymph node enlargement

Objective To investigate the characteristics of color Doppler ultrasonography of acquired immune deficiency syndrome (AIDS) complicated with cervical lymphadenopathy. Methods A total of 40 patients with AIDS and cervical lymphadenopathy admitted to Jiangxi Chest Hospital from January 2019 to December 2020 were selected as the research objects. All of them are diagnosed by color Doppler ultrasonography, and after ultrasound diagnosis are further pathological diagnosis by puncture (gold standard). Results The results of color Doppler ultrasonography diagnosis of AIDS complicated with cervical lymphadenopathy showed that in the classification of lesions, the coincidence rate of ultrasound diagnosis of inflammatory reactive lymph nodes was 91.30%, the coincidence rate of ultrasound diagnosis of tuberculous lymph nodes was 88.89%, and the diagnosis of metastatic lymph nodes was diagnosed by ultrasound. The coincidence rate was 83.33%, and the coincidence rate of ultrasound diagnosis of lymphoma was 100.00%. The sonogram of inflammatory reactive lymph nodes showed that the morphology of lymph nodes was relatively regular, mostly oblong or oval, most of the lesions had no adhesion or fusion, echoes was mostly hypoechoic and relatively uniform, and L/S≥2 , and it seed that the lymphatic hilum was slightly offset or localized in the center; color Doppler flow imaging(CDFI): the blood supply in some lymph nodes was slightly increased, and the dendritic blood flow signal detected at the lymphatic hilum. The sonogram of tuberculous lymph nodes showed that the shape of the lymph nodes was oval or irregular, the lesions was mostly beaded, and the echoes was mostly uneven and hypoechoic, L/S<2; CDFI: blood flow signal disorder, cut back. The sonogram of metastatic lymph nodes showed that: the lesions of the lymph nodes did not merge in the early stage, and the fusion phenomenon occurred in the late stage, and the shape of the lesion was nearly round and irregular, and the echo was mostly uneven and hypoechoic, L/S=1, and The lymphatic hilum was deformed,shifted, or disappeared. CDFI: The blood flow was abundant and unevenly distributed. The sonogram of lymphoma shows that the morphology of lymph nodes was usually round, and the lesions was mostly without fusion. The echo is uniform and mainly hypoechoic, L/S≤1, and the lymphatic hilum was offset; CDFI: lesion location exploration and the blood flow patterns were diverse, and the internal blood flow signals was abundant. The resistance index (RI), minimum blood flow velocity (Vmin) and maximum blood flow velocity (Vmax) of inflammatory reactive lymph nodes were lower than those of tuberculous lymph nodes, metastatic lymph nodes and lymphomas, and the difference was statistically significant (P<0.05). Conclusion Color Doppler ultrasonography of AIDS with cervical lymphadenopathy has the advantages of high resolution, non-invasiveness and painlessness, and can diagnose the degree of cervical lymphadenopathy and the relationship with blood vessels.

Analysis of the value of transvaginal three-dimensional ultrasound in quantitative assessment of decreased ovarian reserve and premature ovarian failure

Objective To explore the application value of transvaginal three-dimensional ultrasound quantitative assessment in decreased ovarian reserve (DOR) and premature ovarian failure (POF). Methods From January 2019 to November 2020, 68 patients with suspected DOR admitted to the Department of Ultrasound, Weifang Hospital of Traditional Chinese Medicine were selected as the DOR group, and 70 patients with suspected POF as the POF group. In addition,70 healthy subjects from Weifang Hospital of Traditional Chinese Medicine during the same period were selected as the control group. The three groups all used transvaginal three-dimensional ultrasound examinations, and the sex hormone examination results were used as the gold standard. The antral follicle count (AFC), ovarian vascularization index (VI),ovarian volume (OV), vascularized blood flow index (VFI) and blood flow index (FI) were compared between the left and right ovaries of DOR, POF groups and were compared among the three groups, and the diagnostic accuracy, sensitivity and specificity were analyzed of DOR and POF by transvaginal three-dimensional ultrasound in quantitative assessment. Results There were no significant differences in AFC, VI, OV, VFI or FI of the left and right ovaries between the DOR and POF groups (P>0.05). The levels of AFC, VI,OV, VFI and FI in the DOR and POF groups were lower than those in the control group, and the differences were statistically significant (P<0.05). The levels of AFC, VI,OV, VFI and FI in the POF group were lower than those in the DOR group, and the differences were statistically significant (P<0.05). The diagnostic accuracy, sensitivity and specificity of transvaginal three-dimensional ultrasound for DOR were 92.65%, 94.23%, and 87.50%, respectively. The diagnostic accuracy, sensitivity and specificity of transvaginal three-dimensional ultrasound for POF were 95.71%, 96.23%, and 94.23%, respectively. Conclusion Quantitative assessment of transvaginal three-dimensional ultrasound has important value in the diagnosis of DOR and POF, and it is an effective method for ovarian screening.

Diagnostic value of dynamic contrast enhanced magnetic resonance imaging combined with diffusion weighted imaging quantitative analysis in ductal carcinoma in situ with microinvasion
Objective To investigate the diagnostic value of dynamic contrast enhanced magnetic resonance imaging combined with diffusion weighted imaging quantitative analysis in ductal carcinoma in situ with microinvasion. Methods A total of 100 patients with early breast cancer who were diagnosed by DCE-MRI combined with DWI in Xinyu People′s Hospital from March 2018 to January 2020 were retrospectively analyzed, and divided into the control group(60 patients who were diagnosed as ductal carcinoma in situ, DCIS, by the pathological examination) and the observation group (40 patients who were diagnosed as DCIS-Mi by the pathological examination), according to the difference of diagnosis types in the pathological examination. The apparent diffusion coefficient (ADC), early-phase enhancement rate (EER) and the characteristics in different MRI enhancement types were compared between the two groups. Results The ADCMin of the observation group was lower than that of the control group, and the ADCDR and EER of the observation group were higher than those of the control group, with statistically significant differences (P<0.05), while the ADCMax difference between the two groups was not statistically significant (P>0.05). No significant differences were observed in the numbers of non-phymatoid enhancement cases and phymatoid enhancement cases between the two groups (P>0.05). The non-phymatoid enhancement characteristics was compared between the two groups, the internal enhancement characteristics was compared between the two groups, with statistically significant difference (P<0.05). The proportion of homogeneous enhancement cases in the observation group was lower than that in the control group, while the proportion of heterogeneous enhancement cases was higher than that in the control group, with statistically significant differences (P<0.05). The distribution characteristics was compared between the two groups, with statistically significant (P<0.05). The proportion of cases of focal distribution in the observation group was lower than that in the control group, and the proportion of cases of segment distribution in the observation group was higher than that in the control group, with statistically significant differences (P<0.05). The differences in the characteristics of phymatoid enhancement between the two groups were statistically significant (P<0.05). The proportion of homogeneous enhancement cases in the observation group was lower than that in the control group, while the proportion of ring enhancement cases in the observation group was higher than that in the control group, with statistically significant differences (P<0.05). The proportion of cases with unclear lesion edge in the observation group was higher than that in the control group, and the difference was statistically significant(P<0.05). No significant differences were observed between the two groups in all of the proportions of lesions in the shape (P>0.05). Conclusion DWI combined with DCE-MRI has a good diagnostic efficacy for DCIS-Mi, and the differential diagnosis of DCIS alone and DCIS- Mi can be performed based on lesion distribution and internal enhancement characteristics.

MRI evaluation of renal cortical perfusion level and its correlation with long-term prognosis in patients with diabetic nephropathy

Objective To investigate the role of MRI in evaluating renal cortical perfusion level and long-term prognosis in patients with diabetic nephropathy. Methods A total of 120 patients with type 2 diabetes admitted to Shenyang First People′s Hospital from August 2016 to August 2017 were selected as the research objects. According to whether they had diabetic nephropathy (DKD), they were divided into simple diabetes (SD) group (60 cases) and DKD group (60 cases), and DKD group was further divided into mild DKD subgroup (27 cases) and moderate and severe DKD subgroup(33 cases) according to the severity of their disease. Sixty healthy people who came to Shenyang First People′s Hospital during the same period were selected as the normal control group.The incidence of adverse events(death, ≥30%increase in serum creatinine, and end-stage renal disease) was recorded during follow-up. All subjects underwent arterial spin labeling (ASL) MRI scanning, and renal blood flow (RBF) was measured by two physicians. The consistency of the results was analyzed, and the correlation between RBF and glomerular filtration rate (eGFR) was analyzed. The predictive value of RBF for long-term prognosis was evaluated by ROC curve. Results The intra-group correlation coefficient(ICC) of RBF values in SD group, DKD group and normal control group was higher than 0.90, indicating a high consistency. RBF values of SD group, mild DKD group, moderate and severe DKD group and normal control group were significantly different (P<0.05). The RBF value of normal control group was higher than that of SD group, mild DKD group and moderate and severe DKD group. The RBF value of SD group was higher than that of mild DKD group and moderate and severe DKD group. The RBF value of mild DKD group was higher than that of moderate and severe DKD group(P<0.05). The RBF value of renal cortex was positively correlated with eGFR value in diabetic patients (r=0.664, P<0.05).Forty-four DKD patients were followed up for 23-38 months,with an average of(32.1±2.1) months, and 18 patients had adverse events during the follow-up. The area under ROC curve of RBF for predicting long-term prognosis was 0.821 (95%CI 0.742~0.901), and the sensitivity, specificity and accuracy were 78.2%, 85.5% and 83.2%, respectively.Conclusion ASL MRI can quantitatively assess the level of renal cortex perfusion in diabetic patients, and sensitively distinguish differences in patient prognosis, which provides a non-invasive and reproducible clinical monitoring method for the evaluation of renal function and long-term efficacy prediction of DKD patients.

Effect of third-line therapy of Apatinib combined with Capecitabine on advanced triple negative breast cancer

Objective To investigate effect of third-line therapy of Apatinib combined with Capecitabine on the advanced triple negative breast cancer (TNBC). Methods A total of 78 patients with advanced TNBC who failed secondline chemotherapy in Anshan Cancer Hospital from May 2018 to May 2019 were selected and randomly divided into study group and reference group, with 39 cases in each group. The reference group was treated with Capecitabine monotherapy for three-line treatment, and the study group was treated with Apatinib combined with Capecitabine for three-line treatment. The clinical efficacy of the two groups of TNBC patients after treatment was compared, including the objective response rate (ORR)and disease control rate (DCR),the survival rate of the two groups of patients within 1 year after treatment was followed up, and the occurrence of adverse reactions was analyzed. Results The ORR and DCR of the study group were higher than those of the reference group, and the differences were statistically significant (P<0.05). The survival rate of the study group within 1 year after treatment was higher than that of the reference group, and the difference was statistically significant (P<0.05). The incidence of hypertension in the study group was higher than that in the reference group, and the difference was statistically significant (P<0.05); there was no significant difference in other adverse reactions between the two groups (P>0.05). Conclusion During the third line treatment of advanced TNBC patients, the short-term curative effect of Apatinib combined with Capecitabine chemotherapy regimen is considerable, which can improve the survival rate of the patients to a certain extent, and the adverse reactions are in a tolerant state.

Objective To investigate the application effect of Jinlong Capsules in the recurrence and metastasis examined by intensity modulated radiation therapy (IMRT) after cervical cancer surgery. Methods A total of 60 patients with postoperative cervical cancer who were admitted to Ganzhou Cancer Hospital from January 2017 to December 2019 were selected as the study subjects. The patients were divided into an observation group and a control group by the random number table method, with 30 cases in each group. The control group was treated with IMRT regimen, and the observation group was treated with Jinlong Capsule on the basis of IMRT. The recent clinical treatment effects, Karnofsky functional status (KPS) score, immune cell indicators, and adverse reactions were compared between the two groups.Results The short-term clinical treatment effect of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Before treatment, there was no statistically significant difference in the KPS scores of the two groups of patients (P<0.05); after treatment, the KPS scores of the two groups of patients were higher than before treatment, and the difference was statistically significant (P<0.05); after treatment, The KPS score of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).Before treatment, there was no statistically significant difference in immune cell indicators between the two groups of patients (P<0.05); after treatment, the immune cell indicators in the observation group were better than those before treatment, and the differences were statistically significant (P<0.05); after treatment, The immune cell indexes of the observation group were better than those of the control group, and the differences were statistically significant (P<0.05).The incidence of acute radiation enteritis, radiation cystitis and bone marrow suppression in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Jinlong Capsule combined with IMRT in the treatment of patients with cervical cancer after surgery has the effects of improving short-term clinical treatment effects, en hancing patient immune function, reducing adverse reactions and improving quality of life. It is worthy of further promotion and application.

Effect of Hydroxycodone Hydrochloride Sustained-release Tablets combined with Morphine Sulfate Sustained-release Tablets on pain outbreak in patients with advanced cancer

Objective To investigate the effect of Hydroxycodone Hydrochloride Sustained-release Tablets combined with Morphine Sulfate Sustained-release Tablets on pain outbreak in patients with advanced cancer. Methods A total of 104 patients with cancer pain in the People′s Hospital of Jining Beihu Provincial Tourism Resort from December 2015 to December 2018 were selected and use random lotteny to divided into control group(52 cases)and observation group(52 cases). The control group was treated with oral morphine sulfate sustained-release tablets, and the observation group was treated with oral Morphine Sulfate Sustained-release Tablets combined with Hydroxycodone hydrochloride Sustainedrelease Tablets. The pain relief rate, serum index, pain stability time, number of outbreaks of pain, quality of life score and incidence of adverse reactions were compared between the two groups after treatment. Results After treatment,there was no statistically significant difference in the pain relief rate between the two groups(P>0.05).Serum tumor necrosis facctor-α (TNF-α), interleukin-6 (IL-6), substance P (SP) of observation group after treatment were lower than before treatment, 5-hydroxytryptamine (5-HT) and β-endorphin (β-EP) were higher than before treatment, the differences were statistically significant (P<0.05); serum of observation group after treatment TNF-α, IL-6, SP were lower than the control group, 5-HT, β-EP were higher than the control group, the differences were statistically significant (P<0.05).The number of burst pains in the observation group was less than that of the control group, and the pain stabilization time was shorter than that of the control group, the differences were statistically significant (P<0.05). The quality of life score of the observation group after treatment was higher than that of the control group, and the difference was statistically significant (P<0.05). The comparison of the total incidence of adverse reactions between the two groups was not statistically significant (P>0.05). Conclusion Hydroxycodone Hydrochloride Sustained-release Tablets combined with Morphine Sulfate Sustained-release Tablets can effectively reduce the number of pain outbreaks in patients with cancer pain, significantly increase the pain relief rate and improve the quality of life of patients, and are relatively safe.

Analysis of the rationality of proton pump inhibitors in the prevention of stress ulcers in the perioperative period in general surgery areas

Objective To analyze the rationality of proton pump inhibitors (PPIs) in the prevention of stress ulcers (SU)in general surgery areas. Methods The clinical data of 264 patients who used PPIs during the perioperative period in the general surgery area of Sihong County People′s Hospital in August 2020 were retrospectively selected, and the disease severity, basic patient information, surgical status, and PPIs use status were screened according to the selection criteria. The rationality of PPIs medication were comprehensively evaluate. Results Among the 128 selected PPIs medical records, 4 types of PPIs were mainly involved, including omeprazole (injection), esomeprazole (injection), rabeprazole (injection), pantoprazole (injection). A total of 128 cases of preventive use of PPIs during the perioperative period were screened. There were 61 cases of irrational use of drugs, accounting for 47.66%, of which 16 cases (26.23%) were problematic in the route of medication, 32 cases (52.46%) were used without indications, and the course of medication was used. 8 cases (13.11%) were too long, 2 cases (3.28%) were incorrect in usage and dosage, and 3 cases (4.92%)had drug interactions. Conclusion When PPIs were used to prevent SU in this ward, there are unindicated medications,unsuitable selection of varieties and routes of administration, and unreasonable treatments. Relevant departments of the hospital should attach great importance to standardizing the rational use of PPIs.

Application effect of kneading and traction method combined with traditional massage in children with congenital muscular torticollis

Objective To analyze the application effect of kneading and traction method combined with traditional massage in children with congenital muscular torticollis. Methods From February 2018 to February 2021, 66 children with congenital muscular torticollis admitted to the Department of Traditional Chinese Medicine of the Iron Coal General Hospital of Liaoning Health Industry Group were selected as the research objects, and they were divided into the control group and the observation group by lottery grouping method, 33 cases in each group. Children in the control group were treated with traditional massage therapy, and the children in the observation group were treated with kneading and traction method combined with traditional massage therapy. The clinical efficacy, the thickness of the mass before and after treatment and the elastic grading of the lesion tissue before and after treatment were compared between the two groups. Results The clinical efficacy of children in the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in the thickness of the tumor between the two groups (P>0.05). After treatment, the thickness of the tumor in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Before treatment, there was no statistically significant difference in the elastic grading of the lesion tissue of the two groups (P>0.05). After treatment, the elastic grading of the lesion tissue of the observation group was better than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The application of kneading and traction method combined with traditional massage is effective in children with congenital muscular torticollis, which can significantly reduce the thickness of the tumor and improve the total effective rate of treatment.

Objective To investigate the effect of moxibustion combined with self-made dysmenorrhea patch on pain degree and sleep quality in the clinical treatment of patients with cold-coagulation and blood stasis dysmenorrhea.Methods A total of 120 patients with cold-coagulation and blood stasis dysmenorrhea who were treated in Xuzhou Hospital of Traditional Chinese Medicine from March 2017 to February 2019 were selected as the research objects. The patients were divided into the observation group and the control group according to the random number table method,with 60 cases in each group. The control group was treated with conventional medications, and the observation group was treated with moxibustion combined with self-made dysmenorrhea patch. The pain degree, clinical symptoms and sleep quality of the two groups were compared. Results The visual analogue scale (VAS) scores of the two groups after treatment were lower than those before treatment, and the VAS score of the observation group was lower than that of the control group, the differences were statistically significant (P<0.05). The dysmenorrhea duration score and severity score of the two groups after treatment were lower than those before treatment, the dysmenorrhea duration score and severity score of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05). The Pittsburgh sleep quality index scale (PSQI) scores of the two groups after treatment were lower than those before treatment, and the PSQI score of the observation group was lower than that in the control group, the differences were statistically significant (P<0.05). Conclusion The application of moxibustion combined with self-made dysmenorrhea patch in the clinical treatment of patients with cold-coagulation and blood stasis dysmenorrhea can effectively shorten the duration of symptoms, reduce the severity, and help reduce pain and improve sleep quality.

Methods A total of 60 patients with pressure ulcers who were admitted to Ruichang Hospital of Traditional Chinese Medicine from August 2019 to December 2020 were selected as the research objects, and were divided into a control group and a test group using a random number table method, with 30 cases in each group. The control group received conventional surgical intervention, and the test group received hospital preparation compound comfrey burn oil plus iodophor combined with pressure ulcer nursing. The clinical efficacy, pressure ulcer healing score and wound healing in the two groups were compared. Results The total effective rate of treatment in the test group was higher than that in the control group, the difference was statistically significant (P<0.05); the pressure ulcer healing score of the test group 14 days after treatment was lower than that in the control group , the difference was statistically significant (P<0.05); the number of dressing change in the test group was less than that in the control group, and the number of healing days were less than those in the control group, the differences were statistically significant (P<0.05). Conclusion The hospital preparation compound comfrey burn oil plus iodophor and pressure sore nursing can reduce frequency of dressing change, shorten wound healing time, and promote the recovery of prognosis.

Objective To analyze the protective effect of graphene infrared ray and Traditional Chinese Medicine package integrated hot compresser on early dysfunction of internal fistula. Methods A total of 60 patients treated with autologous arteriovenous fistula plasty in Shangrao City People′s Hospital from October 2019 to October 2020 were selected and divided into observation group and control group by random coin tossing, with 30 in each group.Control group was treated with hirudoid cream, and observation group was treated with graphene infrared radiation combined with traditional Chinese medicine hot compress, and the observation time was 6 months. The autologous fistula vascular indicators and serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein-cholesterol(LDL-C), blood urea nitrogen (BUN) and serum creatinine (Scr) were compared between the two groups before and after treatment. Results The diameter of internal fistula, blood flow of internal fistula and blood flow during dialysis in observation group within 6 months after treatment were higher than those in control group, and the differences were statistically significant (P<0.05). Six months after treatment, the levels of TC, TG, LDL-C, BUN and Scr in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). Conclusion Graphene infrared radiation combined with traditional Chinese medicine hot compress has a protective effect on early dysfunction of arteriovenous fistula in patients with hemodialysis, and it can promote metabolism and accelerate blood circulation.

Clinical effect of Liangxue Xiaobi Decoction combined with Calcipotriol Betamethasone Ointment in the treatment of psoriasis vulgaris

Objective To study the curative effect of Liangxue Xiaobi Decoction combined with Calcipotriol Betamethasone Ointment in the treatment of psoriasis vulgaris. Methods A total of 100 patients with psoriasis vulgaris admitted to the Seventh People′s Hospital of Shenyang from October 2019 to October 2020 were selected as the research objects and randomly divided into the observation group(50 cases) and the control group(50 cases). The observation group was treated with Liangxue Xiaobi Decoction combined with Calcipotriol Betamethasone Ointment, and the control group was treated with Calcipotriol Betamethasone Ointment. The psoriasis severity index (PASI), visual analogue scale (VAS), C-reactive protein (CRP), serum interleukin-6 (IL-6), quality of life and total effective rate of treatment were compared between the two groups. Results The PASI score and VAS score of the observation group after treatment were lower than those of the control group, the differences were statistically significant (P<0.05); the levels of CRP and IL-6 in the observation group after treatment were lower than those in the control group, and the differences were statistically significant (P<0.05); the observation group′s physical domain, psychological domain, social domain,material life score and the total effective rate of treatment were higher than those of the control group, and the differences were statistically significant (P<0.05). Conclusion Liangxue Xiaobi Decoction combined with Calcipotriol Betamethasone Ointment has a significant effect in the treatment of psoriasis vulgaris, and can reduce the level of inflammatory factors, improve the treatment effect and improve the prognosis of patients.

Objective To screen out traditional Chinese medicine (TCM) nursing techniques with good applicability in TCM nursing programs for spleen and stomach diseases, to facilitated the application of TCM nursing programs for spleen and stomach diseases in the future. Methods The Delphi expert correspondence method, literature analysis method, and semi-structured interview method were used to conduct 2 questionnaires from April to August 2020 for 31 nursing experts. Experts were asked to discuss the professionalism of spleen and stomach diseases, the competence of nurses, 4 dimensions of applicability and safety are screened, and expert enthusiasm coefficient, expert authority, expert coordination coefficient, and Kendall harmony coefficient are used to analyze the results of expert correspondence. Results After two rounds of expert letter inquiries, the expert opinions were relatively consistent, and finally 17 items with better applicability in the TCM nursing plan for spleen and stomach diseases were screened out of 115 items and 4 dimensions, cupping-gastric ulcer, traditional Chinese medicine enema-ulcerative colitis, moxibustion-stomach pain,Suffocation, diarrhea, constipation, ear acupoint embedding-constipation, acupoint massage-fullness, accumulation,acupoint massage-epigastric pain, fullness, acupoint injection-abdominal pain, ulcerative colitis, acupoint applicationaccumulation, Abdominal pain, diarrhea, constipation. Objective To explore the effect of mobile phone live broadcast combined with Wenjuanxing (wjx.cn) assessment on the level training for the nurses in intensive care unit (ICU). Methods A total of 56 ICU nursing staff at N1,N2, N3, and N4 levels who were trained from November 2019 to November 2020 in Jiangxi Pingxiang People′s Hospital were selected as the study subjects. They were divided into an observation group (n=28) and a control group (n=28) according to the random number table method. The observation group was given mobile phone live broadcast combined with Wenjuanxing assessment for training, and the control group was given traditional centralized teaching combined with paper tests for training. The training coverage rate, test results and total training satisfaction rate of nursing staff were compared between the two groups. Results The training coverage rate of nursing staff in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). The overall average grades gained by the nursing staff in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). The total trainingsatisfaction rate of nursing staff in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion For ICU nurses, the application of mobile phone live broadcast combined with Wenjuanxing assessment has a better effect on level training of nurses, which can effectively improve training coverage rate, test grades, and nursing staff satisfaction.Therefore, this method is worthy of clinical popularization application.

Application of BOPPS teaching mode in orthopedics theory teaching

Objective To explore the practical application effect of BOPPPS teaching mode in orthopedics theory teaching. Methods A total of 107 five-year undergraduate clinical medicine students (class 3 and 4) majoring in 2016 from Dongfeng Hospital Affiliated to Hubei University of Medicine were selected as the research objects. And they were divided into experimental group (55 students) and control group (52 students) by class as the unit. BOPPPS teaching mode and traditional teaching mode were adopted respectively, and comprehensive evaluation was conducted through questionnaire survey and theoretical examination after the course. Results In the experimental group, the proportions of"strong interest in learning and initiative" "clear teaching purpose, prominent teaching focus" "vivid and intuitive content, strong participation" "increased learning pressure, curriculum tension" "teaching goal completion, ability improvement" were higher than those in the control group, the differences were statistically significant (P<0.05). The total scores of theory test, practical operation test and basic theory in the experimental group were higher than those in the control group, the differences were statistically significant (P<0.05). Conclusion The theoretical teaching activities designed according to the BOPPS model promote the student-centered teaching reform, that encourage students to take the initiative to participate in classroom learning and improve the teaching effect, which is worthy of popularization and application in orthopedics clinical teaching.

Application effect of SOP-based centralized training combined tutorial system training in ART new nurse training

Objective To explore the application effect of the combined training methods of centralized training based on standard operation procedure (SOP) in the training of new nurses in assisted reproductive technology (ART). Methods A total of 28 new nurses who entered the job to 73rd Army Hospital of PLA Army from January 2013 to December 2019 were selected as the research subjects. There were 15 people who entered the job from January 2016 to December 2019 were selected as the observation group according to the admission time. The rest 13 people who were admitted to the department in January 2013 to December 2015 were set as the control group. The participants in the control group were provided with the traditional empirical teaching, and in the observation group, they were applied with SOP-based centralized training combined tutorial system training. The ability of assessment results and comprehensive work of the two groups were compared. Results The scores of specialty theory, basic knowledge, rules and regulations, and specialty operation of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05). The abilities of problem analysis and solvement, emergency response, job competence and teamwork in the observation group were better than those in the control group, and the differences were statistically significant(P<0.05). Conclusion The development of SOP can effectively standardize the training content and process. On this basis, the combination of SOP-based centralized training and tutorial system training has a good application effect on training. However, due to the small sample size, the above conclusions need to be drawn cautiously and expanded gradually the scope of research.

Effect of intensive health education combined with individualized nutrition intervention on blood glucose and glycosylated hemoglobin levels of pregnant women with abnormal glucose metabolism during pregnancy

Objective To explore the effect of intensive health education combined with individualized nutrition intervention on the blood glucose and glycosylated hemoglobin levels of pregnant women with abnormal glucose metabolism during pregnancy. Methods A total of 200 cases of pregnant women diagnosed with abnormal glucose metabolism during pregnancy who were admitted to Maternal and Child Health Hospital of Yangjiang City from November 2019 to November 2020 were selected as the research objects. They were divided into routine groups (n=100) and the intervention group (n=100) according to the random envelope method. The routine group carried out routine diet management in outpatient clinics, and the intervention group adopted intensive health education combined with individualized nutrition intervention. Blood glucose and glycated hemoglobin levels were measured at 32 and 38 weeks of gestation in both groups, and knowledge score of abnormal glucose metabolism during gestation was investigated by questionnaire at 38 weeks of gestation in both groups. Results Before the intervention, there were no significant differences in fasting blood glucose, 2 h postprandial blood glucose and glycosylated hemoglobin levels between the two groups (P>0.05). The fasting blood glucose, 2 h postprandial blood glucose and glycosylated hemoglobin levels at 32 and 38 weeks of pregnancy of the two groups were lower than those before intervention, and the fasting blood glucose, 2 h postprandial blood glucose and glycosylated hemoglobin levels in the intervention group were lower than those in the routine group, the differences were statistically significant (P<0.05). Knowledge score of abnormal glucose metabolism during pregnancy in the intervention group was higher than that in the routine group, and the difference was statistically significant (P<0.05). Conclusion Intensive health education combined with individualized nutrition intervention has a significant effect on pregnant women with abnormal glucose metabolism during pregnancy, which can improving the blood glucose and glycosylated hemoglobin levels of pregnant women, and improving the knowledge score of abnormal glucose metabolism during pregnancy. It is worthy of application and promotion.

Effect of health education based on behavior change theory on weight management, diet and blood glucose control in patients with gestational diabetes mellitus

Objective To explore the effect of health education based on behavior change theory on weight management, diet and blood glucose control in patients with gestational diabetes mellitus (GDM). Methods A total of 86 patients with GMD who were treated in Shangrao Maternal and Child Health Hospital from May 2019 to August 2020 were selected as the research objects, and they were divided into control group and observation group according to the random number table method, with 43 cases in each group. The control group was given routine health education intervention, and observation group was treated with health education intervention based on behavior change theory. The weight gain during pregnancy and weight at childbirth, compliance rates of grain, bean product, milk product and salty food of the two groups were compared after 3 months of intervention. The glycosylated hemoglobin (HbA1c), fasting plasma glucose (FPG) and 2-hour postprandial blood glucose (2 h PG) before intervention and after 3 months of intervention of the two groups were compared. Results The weight gain during pregnancy and weight at childbirth in observation group were lower than those in control group (P<0.05). After 3 months of intervention, the compliance rates of grains, bean product, milk product and salty food in observation group were significantly increased compared to before intervention, and the compliance rates were higher in observation group than those in control group (P<0.05). The levels of HbA1c, FPG and 2 h PG in the two groups were reduced compared with those before intervention, and the levels of HbA1c, FPG and 2 h PG in observation group were lower than those in control group, the differences were statistically significant (P<0.05). Conclusion Health education based on behavior change theory can help control the weight change of patients with GMD, and can also improve the diet and blood glucose control, with a high clinical promotion value.

Analysis of the causes of intraocular pressure fluctuations caused by hemodialysis in uremic patients and research on nursing intervention

Objective To explore the application effect of comprehensive nursing intervention in patients with uremic hemodialysis. Methods A total of 64 patients (128 eyes) with uremia who underwent hemodialysis treatment for more than 3 months in Jiangxi Fifth People′s Hospital from July 2019 to October 2020 were selected and divided into the intervention group (n=32, 64 eyes) and the control group (n=32, 64 eyes) by random number table method. The patients in the intervention group were provided with comprehensive nursing care, and in the control group was provided with routine nursing. The fluctuations of intraocular pressure before and after hemodialysis, treatment compliance and nursing satisfaction between the patients of two groups were compared. Results There was no statistically significant difference in intraocular pressure (IOP) between patients of the two groups before nursing (P>0.05). The IOP of the intervention group after nursing was lower than that in the control group, and the difference was statistically significant (P<0.05). The treatment compliance of the intervention group was higher than that in the control group, the difference was statistically significant (P<0.05). The nursing total satisfaction of the intervention group was higher than that in the control group, the difference was statistically significant (P<0.05). Conclusion The application of comprehensive nursing intervention in patients with uremic hemodialysis can promote the improvement of intraocular pressure, improve treatment compliance, and increase satisfaction. It is worthy of clinical application.

Application effect of moderate perineal protection delivery method in natural childbirth

Objective To explore the application effect of moderate perineal protection delivery method in natural childbirth. Methods A total of 160 parturients who underwent natural childbirth in the Jiujiang University Affiliated Hospital from January 2019 to July 2020 were selected as the research subjects, they were divided into the control group and the observation group by random number table method, 80 cases in each group. The control group was given unprotected perineal delivery method, and the observation group was given moderate perineal protection delivery method. The degree of perineal laceration, postpartum 24 h pain (visual analog scale [VAS] score), the occurrence of perinea lateral incision, postpartum hemorrhage, and neonatal status (asphyxia rate, weight, Apgar score) were compared between the two groups. Results The rate of perineal laceration,postpartum 24 h VAS score, incidence rate of perineal lateral resection and incidence rate of postpartum hemorrhage in observation group were lower than those in control group, the differences were statistically significant (P<0.05). There were no significant differences in neonatal asphyxia rate and body weight between the two groups (P>0.05). The neonatal Apgar score of observation group was higher than that of the control group, the difference was statistically significant (P<0.05). Conclusion Moderate perineal protection delivery method has a significant application effect in natural childbirth. And it can effectively reduce the occurrence of maternal perineal laceration, perineotomy, postpartum hemorrhage and pain. It is helpful to improve the physiological status of newborns, thus it is recommended for clinical application.

Study on nursing effects of holographic copper bIanstone scraping combined with ear point pressing beans on perimenopausal hypertension patients

Objective To explore the effect of holographic copper-bian-stone scraping combined with ear-point pressing bean-pressing care for perimenopausal hypertensive patients. Methods A total of 70 perimenopausal hypertensive patients who were admitted in Jiangxi Provincial Hospital of Integrated Traditional Chinese and Western Medicine from November 2019 to January 2021 were selected as the research objects, and divided into the observation group and the control group by random number table method, 35 cases in each group. The control group was treated with antihypertensive treatment and routine nursing care, and the observation group was treated with holographic copper-bianstone scraping combined with ear acupoint pressing beans. The sleep quality, traditional Chinese medicine (TCM) syndrome scores,immediate and long-term blood pressure, adverse reactions and nursing satisfaction of the two groups were compared.Results The sleep quality scores and TCM syndrome scores of the two groups after intervention were lower than before the intervention, after the intervention, the sleep quality scores and TCM syndrome scores in the observation group were lower than those of the control group,the differences were statistically significant(P<0.05). The immediate blood pressure and long-term blood pressure in the observation group were lower than those of the control group,the systolic blood pressure of the two groups were 18∶00<12∶00<6∶00, and the diastolic blood pressure of the observation group were 18∶00<12∶00,6∶00, the differences were statistically significant (P<0.05). The total incidence of adverse reactions in the observation group was lower than the control group, the nursing total satisfaction in the observation group was higher than that of the control group, the differences were statistically significant (P<0.05). Conclusion Perimenopausal hypertensive patients with holographic copper bian-stone scraping combined with ear-point pressing bean-pressing care has a significant effect, which can relieve the clinical symptoms of patients and is worthy of clinical promotion.

EFfect study of Omaha system on continuous nursing for patients with rheumatoid arthritis

Objective To explore the application effect of continuation care based on Omaha system in patients with rheumatoid arthritis (RA). Methods From October 2018 to October 2020, 60 patients with RA admitted to Pingxiang People′s Hospital, Jiangxi Province were selected and divided into the control group and the observation group according to random number table method, 30 cases in each group. The control group took routine continuation care, and the observation group used the continuation care model constructed by the Omaha system. The clinical efficacy and nursing satisfaction of the two groups were compared, self-care ability, health behavior compliance, functional status, and quality of life before and at 3 months of intervention. Results The total effective rate of treatment in the observation group was 93.33%, which was higher than that of (73.33%) in the control group. After the intervention, ESCA was (96.85±6.05) points, health behavior compliance was (70.95±3.37) points, HAQ was (34.72±3.26) points, quality of life was(68.98±5.13)points,which were higher than those of the control group′s (76.63±6.02), (56.69±3.42), (25.67±3.24), (53.74±5.01) points, and the total nursing satisfaction was 93.33%, which was higher than that of the control group 70.00%,there were astatistical difference (P<0.05). Conclusion Continuing care based on the Omaha system can improve the clinical efficacy of RA patients, enhance the patients′self-care ability and health behavior compliance, promote the improvement of functional status and quality of life, and help improve nursing satisfaction.

Effect of nursing risk standardization on prevention of early infection after hematopoietic stem cell transplantation

Objective To analyze the prevention effect of early infection in patients with hematopoietic stem cell transplantation. Methods From March 2015 to March 2020, 40 patients who received hematopoietic stem cell transplantation in Zhongshan People′s hospital were selected as the research objects,and were divided into the control group (20 cases),and the observation group (20 cases) according to singular and plural by patients ward. The control group received routine nursing intervention, the observation group received standardized nursing intervention. The infection occurrence time and maintenance time, infection occurrence and nursing satisfaction of the two groups were compared. Results The duration of infection in the observation group was shorter than that in the control group, and the total incidence of infection in the observation group was lower than that in the control group,.the total nursing satisfaction in the observation group was higher than that in the control group, the differences were statistically significant (P<0.05). But there was no significant difference in the time of infection between the two groups (P>0.05). Conclusion The standardized intervention of nursing risk in patients received hematopoietic stem cell transplantation, which can effectively reduce the risk of early infection, improve its overall treatment and nursing effect, improve patient care satisfaction, and even if infection occurscan control the infection in a short time, and reduce the impact of infection on its treatment effect.

Application of continuous rehabilitation nursing after terrible triad of elbow joint

Objective To explore the application effect of continuous rehabilitation nursing after terrible triad of elbow joint. Methods A total of 60 patients with terrible triad of elbow joints treated in Foshan Traditional Chinese Medicine Hospital from August 2018 to August 2020 were selected as the research subjects. They were divided into the control group (n=30) and the observation group (n=30) according to the random number table method. The control group adopted conventional care, and the observation group used continuous rehabilitation nursing based on conventional nursing.The incidence of postoperative complications, the elbow range of motion and Mayo elbow performance score before and after nursing in two groups were compared. Results There were no significant difference in the elbow range of motion and Mayo elbow performance score between two groups′ patients before nursing (P>0.05). After nursing, the elbow range of motion and Mayo elbow performance score of the two groups were higher than those of the group before nursing, the elbow range of motion and Mayo elbow performance score of the observation group were higher than those of the control group, the differences were statistically significant (P<0.05). The total incidence of postoperative complications in the observation group was lower than that of the control group, the difference was statistically significant (P<0.05). Conclusion Rehabilitation nursing model can effectively improve the joint mobility after terrible triad of elbow joint, reduce the risk of complications. Therefore, this nursing mode can be popularized and applied in clinic.

Application of labor pain prescheme in relieving pain during active labor of puerperae

Objective To explore the application effect of labor pain prescheme on relieving pain during active labor of puerperae. Methods The clinical data of 80 puerperae who underwent vaginal delivery in the Fifth Hospital of Fuzhou City from June 2018 to August 2020 were retrospectively analyzed. Patient were divided into into the control group and the observation group by digfferen nursing method, 40 cases in each group .The control group was given routine labor nursing, the observation group was given labor pain prescheme nursing. The pain level during active labor (visual analogue scale [VAS] score), course of labor, labor bleeding, fetal distress and nursing satisfaction were compared between the two groups. Results The VAS score of observation group during active labor was lower than that of control group,the difference was statistically significant (P<0.05). There were no statistical significance in the course of labor, labor bleeding and fetal distress between the two groups (P>0.05). The nursing total satisfaction of observation group was better than that of control group, the difference was statistically significant (P<0.05). Conclusion Labor pain prescheme can effectively alleviate the pain of puerperae during active labor and improve the nursing satisfaction, with high clinical application value.

Effect of continuous nursing on reducing stoma-related complications in patients with temporary intestinal stoma

Objective To explore the effect of continuous nursing on improving the disease awareness and self-care ability of patients with temporary intestinal stoma and reducing complications related to the intestinal stoma. Methods A total of 80 cases of temporary intestinal stoma patients who were admitted to the Chaozhou People′s Hospital in Guangdong Province from February 2018 to June 2020 were selected and divided into the observation group and the control group by random number table method, 40 cases in each group. The patients in observation group were given continuous nursing, and in the control group, they were given regular nursing. The incidence of stoma-related complications, on-time return, patient self-care ability, quality of life, nursing satisfaction, and psychological status between the two groups were compared. Results The self-care ability, on-time return, quality of life and nursing satisfaction scores of patients in the observation group after nursing were higher than those in the control group, and the differences were statistically significant (P<0.05); the total incidence of stoma-related complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Continuous nursing has a good prognostic effect on improving patients′ quality of life, self-care ability, ensuring timely return of the intestinal stoma, and reducing complications. It is worthy of promotion in clinical applications.

Application effect of children general anaesthesia by evidence-based early-warning nursing intervention

Objective To approach effect of children general anaesthesia by evidence-based early-warning nursing intervention. Methods A total of 120 cases hypospadias children of Xiamen University Affiliated Women′s and Children′s Hospital from October 2019 to October 2020 were selected as research objects, they were divided into the control group and the detection group by random number table method, 60 cases in each group. The control group was given routine nursing, the observation group was given evidence-based early warning nursing on the basis of the control group. The indwelling catheter complications, pressure sore rate of two group children were detected, the life quality score, mental toughness score of two group children were detected, nursing satisfaction and treatment compliance of two group children were detected. Results The catheter blockage, tubulation, urinary fistula, urethral stricture, pressure sore rate of detection group children were lower than those of the control group, the difference were statistical significance(P<0.05).The life quality score, mental toughness score of two group children before intervention were compared, the difference were no statistical significance (P>0.05); after intervention, the life quality score, mental toughness score of two group were higher than before intervention, the life quality score, mental toughness scoreof the detection group were higher than those of the control group, the difference were statistical significance (P<0.05). The total satisfaction degree of nursing and the total compliance of the observation group were higher than that of the control group, the differences were statistically significant (P<0.05). Conclusion The application effect of children general anaesthesia by evidencebased early-warning nursing intervention, which could increase life quality and mental toughness, decrease complications and pressure sore rate, improve family membersof children satisfaction and treatment compliance.

Clinical effect of cognitive behavior intervention combined with traditional Chinese medicine rehabilitation nursing on stroke patients

Objective To explore the clinical effect of cognitive behavior intervention combined with traditional Chinese medicine rehabilitation nursing on stroke patients. Methods A total of 78 stroke patients who were admitted to the Fushun Hospital of traditional Chinese medicine from January 2019 to January 2020 were selected as the research subjects. According to the random number table method, they were divided into the control group and the study group,39 cases in each group. The control group adopted conventional nursing methods, and the study group adopted cognitive behavior intervention combined with traditional Chinese medicine rehabilitation nursing. The cognitive function, rehabilitation self-efficacy and limb function of the two groups were compared. Results The Montreal cognitive assessment(MoCA) scores and the mini-mental state examination (MMSE) scores in the study group were higher than those of the control group, and the differences were statistically significant (P<0.05). The score of the rehabilitation self-efficacy scale of the study group was (91.31±9.55) points, which was higher than that of the control group of (80.23±11.76)points, and the difference was statistically significant (P<0.05). The scores of Fugl-Meyer motor function assessment scale and Fugl-Meyer balance function scale of the study group were higher than those of the control group, and the differences were statistically significant (P<0.05). Conclusion Cognitive behavior intervention combined with traditional Chinese medicine rehabilitation nursing has a significant effect in the application of stroke patients, which can help improve their cognitive function, and promote rehabilitation self-efficacy and limb function.

Application effect of enteral nutrition support on the basis of evidencebased nursing care in Crohn′s disease patients

Objective To explore the application effect of enteral nutrition support on the basis of evidence-based nursing care in Crohn′s disease patients. Methods A total of 120 cases diagnosed with Crohn′s disease in the First Affiliated Hospital of Fujian Medical University from May 2019 to May 2020 were selected. Patients were divided into the control group (60 cases) and the observation group (60 cases) by random number table method. The control group received routine nursing and the observation group received enteral nutrition support based on evidence-based nursing.The nutritional status, quality of life and adverse reactions of the two groups were compared. Results The levels of serum prealbumin (PA), albumin (Alb) and hemoglobin (Hb) in the observation group were higher than those of in the control group, the weight loss in the observation group was less than that in the control group, the differences were statistically significant (P<0.05). The quality of life score in the observation group was higher than that in the control group, the total incidence of adverse reactions in the observation group was lower than that in the control group, the differences were statistically significant (P<0.05). Conclusion Enteral nutrition support on the basis of evidence-based nursing care in Crohn′s disease patients is effective, which improve the nutritional status of patients, reduce the risk of adverse reactions and improve the quality of life.
