4 Study on intracranial pressure monitoring after ruptured cerebral aneurysm clipped at different timing

Objective To explore the characteristics of intracranial pressure (ICP) in patients after ruptured cerebral aneurysms clipped at different timing.Methods The clinical data of 49 adult patients with ruptured cerebral aneurysm in our hospital who met the inclusion and exclusion criteria from October 2014 to September 2016 were retrospectively analyzed.They were divided into the early surgery (≤3 days after onset)group (26 cases) and delayed surgery (4-21 days after onset) group (23 cases) according to the timing of clipping.The continuous ICP monitoring and ICP-oriented comprehensive treatment were performed after surgery in both groups.The characteristics and clinical significance of postoperative ICP of patients in two groups were compared and analyzed.The prognosis of the patients in two groups was evaluated by mortality and modified Rankin scale (mRS) score at 12-month follow-up.Results There were no significant differences in imaging characteristics and prognosis between the two groups of patients (P>0.05).The total average postoperative ICP of patients in the early surgery group was (15.21±3.71) mmHg, which was higher than that in the delayed surgery group for (14.12±4.13) mmHg, and the difference was statistically significant (P<0.05).The average postoperative ICP of patients in the early surgery group increased slowly and then decreased, average ICP on the third and fifth days after surgery was higher than that at the first day, with the highest on the fifth day and it decreased on the seventh day after surgery, and was lower than that on the first day after surgery, with statistically significant differences (P<0.05).In the delayed surgery group, the average ICP started to decrease on the second day after surgery, and the average ICP on the sixth and seventh days after surgery was lower than that on the first day, lowest on the seventh day after surgery, with statistically significant differences (P<0.05).There was no significant difference in the average ICP between the two groups on the seventh day after surgery (P>0.05).Conclusion The total postoperative ICP of patients with ruptured cerebral aneurysm who have been operated within 3 days after onset is higher than that of delayed surgery.The postoperative ICP of patients with either early or delayed surgery shows a tendency of increasing first and decreasing then.The ICP reaches the peak on the fifth day after early surgery while that on the second day after delayed surgery.The ICP of patients after clipping at different timing can be reduced to a lower level after treatment.This feature may be helpful for the clinical diagnosis and treatment of increased postoperative ICP in patients with ruptured cerebral aneurysm.

8 Ganoderma lucidum spore oil extraction by supercritical CO2 and refining

Objective To study the supercritical CO2 extraction process conditions and refining method of Ganoderma lucidum spore oil.Methods Taking the extraction rate as an indicator, three extraction factors (extraction pressure, extraction temperature, extraction time) were investigated, the best process for extracting Ganoderma lucidum spore oil was selected and refined by steam distillation.Results The optimal process was established with the pressure being 30 MPa, temperature being 50℃and extraction time being 3 h and the extraction rate was 20.68%.The peroxide value of Ganoderma lucidum spore oil was significantly reduced after the steam distillation.Conclusion The process of extracting Ganoderma lucidum spore oil has the characteristics of high extraction rate and stability, and is reasonable and feasible.The refining process is simple and reasonable, which can reduce the peroxide value of Ganoderma lucidum spore oil and meet the national standard.

12 Research on main active components of ″Fructus Ligustri Lucidi-Ecliptae Herba″ drug pair based on network pharmacology

Objective To explore the main active ingredients and targets of the ″Fructus Ligustri Lucidi-Ecliptae Herba″ drug pair using the network pharmacology method, so as to study the mechanism of pharmacological action,and provide a new clue for further in-depth study of its pharmacological effect.Methods By searching the TCMSP database, combined the oral bioavailability and chemochemical property analysis, the effective components and target information of limond-inkwort were screened, and Cytoscape software was used to construct the component-target network to obtain the target information.Metascape system was used to annotate and analyze gene function and pathway.Results A total of 24 candidate active molecules were screened from the ″Fructus Ligustri Lucidi-Ecliptae Herba″ drug pair, acting on 248 targets, mainly involving multiple signaling pathways in biological processes such as humoral regulation, oxygen response, cell apoptosis, and neural receptor interaction.Conclusion Network pharmacology method reveales the main pharmacological mechanism of ″Fructus Ligustri Lucidi-Ecliptae Herba″ drug pair.

16 Clinical effect of Shashen Yuzhu Decoction Plus Minus on the treatment of chronic cough

Objective To observe the clinical effect of Shashen Yuzhu Decoction Plus Minus on the treatment of chronic cough.Methods A total of 60 patients with chronic cough treated in the outpatients and inpatients of the Department of Pneumology, No.1 Traditional Chinese Medicine Hospital in Changde Affiliated to Hu′nan University of Chinese Medicine from January 2018 and January 2019 were selected as subjects.According to the random number table method, they were divided into the treatment group and the control group, with 30 cases in each group.The treatment group was treated with Shashen Yuzhu Decoction Plus Minus.The control group was treated with Compound Methoxyphenamine Capsules, two weeks for one course of treatment, two courses of treatment were used as the standard of observation.The efficacy, adverse reactions and cough symptom scores before and after treatment of the two groups were compared.Results There was no significant difference in the total effective rate between the two groups (P>0.05).Before treatment, there was no statistically significant difference in the total score of cough symptoms between the two groups (P>0.05).After treatment, there was no statistically significant difference in the total score of cough symptoms between the two groups (P>0.05).After treatment, the total score of cough symptoms in both groups was lower than that before treatment, and the difference was statistically significant (P<0.05).The total incidence of adverse reactions in the treatment group was lower than that in the control group, and the difference was statistically significant (P<0.05).Conclusion Shashen Yuzhu Decoction Plus Minus and Compound Methoxyphenamine Capsules have curative effects in the treatment of chronic cough, which can obviously alleviate the clinical symptoms of cough patients and alleviate the pain of patients, the incidence of adverse reactions treated by Shashen Yuzhu Decoction Plus Minus is lower, which is worth popularizing.

20 Application of ear acupuncture therapy in perimenopausal syndrome

Perimenopausal syndrome is a common condition in perimenopausal women, which may cause neuropsychiatric symptoms, vasomotor symptoms, cardiovascular symptoms and endocrine changes, etc., and even affect their lives.At present, the treatment of Western medicine is mostly related to hormone replacement therapy.Traditional Chinese medicine treatment includes traditional Chinese medicine and acupuncture treatment.Because of its convenience, economy, safety and no side effects, ear acupuncture therapy can effectively alleviate related symptoms.The combination of auricular point therapy with acupuncture, traditional Chinese medicine and psychological therapy can effectively improve the symptoms of perimenopausal syndrome, achieve the similar effect with hormone replacement therapy, and have no side effects related to hormone therapy.This article reviews the application of ear acupuncture therapy in perimenopausal syndrome and its related mechanisms.

24 Advances in the diagnosis and treatment of ischemic white matter lesions

Recently, with intensive studies of white matter lesions and development of imaging and biochemical detection techniques, the detection rate, prevention and treatment methods of white matter lesions have been remarkably improved.Ischemic white matter lesions are a neuroimaging definition and indicated by low-density spotted or patchy near the periventricular white matter and semi-oval center on CT images and hyperintensities on T2-weighted or fluidattenuated inversion recovery (FLAIR) images.Ischemic white matter lesions have a higher prevalence in the elderly and multiply double the risk of dementia and triple stroke, which result in heavy social burden as the aging of the population.This article reviews the advances in the diagnosis and treatment of ischemic white matter lesions.

29 Research progress on risk factors and preventive care of catheter-related bloodstream infection with extracorporeal membrane oxygenation support therapy

Extracorporeal membrane oxygenation (ECMO) has been widely used in the treatment of patients with circulatory failure and respiratory failure, and can effectively improve the prognosis and survival rate of patients.In the ECMO supportive therapy, it is inevitable to use the interventional central arteriovenous tube.The pipeline is thicker, if the management is improper, it is easy to cause catheter-related bloodstream infection.This article reviews the risk factors, prevention and nursing strategies of ECMO catheter-related bloodstream infection, so as to provide a basis for medical personnel to prevent and reduce the occurrence of ECMO catheter-related bloodstream infection.

33 Research advances in antibiotic-associated diarrhea therapy

Antibiotic-associated diarrhea is a common complication during the use of antibiotics in hospitalized patients, with an incidence rate of 5% to 39%.According to the different symptoms and signs, there are five types of this disease, among which simple diarrhea type and colitis type are more common, and pseudomembranous colitis type,hemorrhagic colitis type and fulminant colitis type are relatively rare.Conventional treatment programs, such as discontinuation of relevant antibiotics, combined use of probiotics or antidiarrheal drugs, can alleviate patients′ condition and improve their symptoms to some extent, but the curative effect is not satisfactory for some patients with critical condition.In recent years, the use of integrated Chinese and Western medicine to dialectically treat antibiotic-associated diarrhea has achieved certain results, providing new ideas for the treatment of antibiotic-associated diarrhea.This article briefly discusses the progress of research on Chinese and Western medicine for antibiotic-associated diarrhea.

38 Research progress on the expression of immediate early gene in hepatic ischemia reperfusion

Hepatic ischemia reperfusion injury is a common and important complications in clinical hepatobiliary surgery.During the process of ischemia and reperfusion early response protein-1, c-Jun, c-Fos of immediate early genes expression was up-regulated, regulating hepatic tissue inflammatory response, resulting in serious hepatic function abnormalities and organ dysfunction.Those immediate early genes also regulate hepatic tissue regeneration and reconstruction in convalescent stage, playing an important role in hepatic energy metabolism, which has an important impact on the survival and prognosis of patients.Immediate early genes are a kind of genes which can be induced quickly and briefly after stimulation, and their activation do not require synthesis of any new proteins.So far, more than 100 kinds of immediate early genes have been found in cells.In-depth understanding of the expression and regulation of immediate early genes in hepatic ischemia-reperfusion may provide a new direction for ameliorating hepatic ischemia reperfusion injury and hepatic recovery after injury.This paper reviews the research progress of early gene expression of liver ischemia-reperfusion.

42 Application of health guidance intervention in physical examinations in older adults

Objective To explore the application of health guidance intervention in the physical examinations in older adults.Methods A total of 150 older adults receiving physical examinations in our hospital from August 2018 to August 2019 were selected as the research subjects.They were divided into a test group (n=75) and a reference group (n=75) according to a randomized and parallel method.In the test group, health guidance was used, while in the reference group, routine nursing was adopted.The awareness of disease knowledge, self-health awareness, self-management ability score, and total nursing satisfaction in the two groups were compared.Results The disease knowledge awareness rate, self-health awareness rate, self-management ability score, and total nursing satisfaction in the test group were higher than those in the reference group with significant differences (P<0.05).Conclusion The effect of health guidance in the physical examination in older adults is remarkable, by increasing the awareness rate of disease knowledge, selfhealth awareness, self-management ability, and the satisfaction on nursing during physical examinations, and finishing the physical examination items as soon as possible, which is of great significance on clinical application and promotion.

45 Effect of family non-invasive ventilation on right heart function in patients with stable phase of chronic obstructive pulmonary disease complicated with cor pulmonale

Objective To investigate the effect of family non-invasive ventilation on right heart function in patients with stable phase of chronic obstructive pulmonary disease (COPD) complicated with cor pulmonale.Methods A total of 120 patients with stable phase of COPD complicated with cor pulmonale admitted to our hospital from January 2016 to June 2018 were enrolled in the study.According to the random number table method, they were divided into group A(60 cases) and group B (60 cases).Non-invasive bi-level positive pressure ventilation was used in group A and noninvasive ventilation in group B was adopted.Pulmonary arterial pressure, right ventricular Tei index, and blood gas indexes were compared before and after treatment.Results There were no significant differences in pulmonary arterial pressure and right ventricular Tei index between the two groups before treatment (P>0.05).After treatment, the pulmonary arterial pressure in group B was (53.89±0.05) mmHg, and the right ventricular Tei index was (0.72±0.01),which were lower than those in group A for (59.48±1.32) mmHg and (0.79±0.02), with statistically significant differences (P<0.05).There were no significant differences in blood gas indexes between the two groups of patients before treatment (P>0.05).After treatment, the carbon dioxide partial pressure in both groups was lower than that before treatment, arterial blood oxygen partial pressure in both groups was higher than that before treatment, and the carbon dioxide partial pressure in group B was lower than that in group A, arterial blood oxygen partial pressure in group B was higher than that in group A, the differences were statistically significant (P<0.05).Conclusion Family non-invasive ventilation can effectively improve the right heart function, reduce pulmonary arterial pressure in patients with stable phase of COPD complicated with cor pulmonale, and help to improve their respiratory function, thereby improving quality of life, which is worthy of clinical application and promotion.

49 Clinical effect of acupuncture and moxibustion combined with low frequency repetitive transcranial magnetic stimulation in the treatment of motor aphasia of cerebral infarction

Objective To explore the clinical effect of acupuncture combined with low frequency repetitive transcranial magnetic stimulation (rTMS) in the treatment of motor aphasia of cerebral infarction.Methods A total of 40 patients with motor aphasia of cerebral infarction admitted to our hospital from January 2018 to June 2019 were selected as the study subjects (course of disease 2 to 24 weeks).They were divided into observation group and control group according to the random number table method, 20 cases in each group.Both groups received routine Western medicine treatment and speech training, while the control group received acupuncture and moxibustion treatment on this basis and pseudolow frequency rTMS.The patients in the observation group were treated with low frequency rTMS on the basis of the acupuncture and moxibustion.The scores of aphasia index, speech function and China Rehabilitation Research Center aphasia examination (CRRCAE) before and after treatment were compared between the two groups.Results After treatment, the aphasia index score of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).The self-speech, retelling and naming scores of the observation group after treatment were higher than those of the control group, and the differences were statistically significant (P<0.05).There was no statistically significant difference in listening comprehension score between the two groups after treatment (P>0.05).After treatment, the CRRCAE score of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).Conclusion Acupuncture and moxibustion combined with low frequency rTMS can effectively increase the aphasia index score and improve the symptoms of aphasia in cerebral infarction patients with motor aphasia, which can be promoted in the clinic.

53 Effect on intestinal preparation during enteroscopy in elderly patients after taking Dimethicone Powder at different times

Objective To investigate the effect on intestinal preparation during enteroscopy in elderly patients after taking Dimethicone Powder at different times.Methods A total of 200 patients who underwent enteroscopy in the gastroenterology department of our hospital from June 2018 to June 2019 were selected as the subjects.According to the random number table method, they were divided into group A (65 cases), group B (70 cases) and group C (65 cases).The patient began to take the intestinal lavage 6 hours before the examination and finished within 2 hours.Patients in group A were given Dimethicone Powder immediately after taking the intestinal lavage solution.Patients in group B took Dimethicone Powder one hour before enteroscopy.Patients in group C were given Dimethicone Powder 0.5 hours before enteroscopy.The tolerance of enteroscopy was compared among the three groups, and the intestinal cleanliness of the three groups was scored by Boston intestinal preparation scale (BBPS), and the intestinal bubble content of the three groups was observed.Results The tolerance of enteroscopy in group B was better than that in group A and C, the difference was statistically significant (P<0.05).There were no significant differences in the intestinal cleanliness scores among the three groups (P>0.05).The content of intestinal bubbles in group B was lower than that in group A and group C, the difference was statistically significant (P<0.05).Conclusion The clinical effect of Dimethicone Powder taken by the elderly patients 1 hour before enteroscopy after taking intestinal lavage solution is ideal, which can effectively improve the effect of defoaming and the tolerance of patients to colonoscopy.

56 Effect of transcutanclus electrical acupoint stimulation pretreatment on intradialytic hypotension

Objective To observe the effect of transcutanclus electrical acupoint stimulation (TEAS) pretreatment on blood pressure during hemodialysis.Methods Eighty patients with end stage renal disease (ESRD) who underwent hemodialysis in Changzhou TCM Hospital from January 2018 to March 2019 were selected as the subjects, and divided into the control group and observation group, with 40 cases in each group.At 30 min before hemodialysis, the observation group received bilateral Neiguan points and Zusanli points for TEAS for 30 min.The control group attached electrode pads at the same site, but no electrical stimulation was given.The blood pressure and the incidence of hypotension during hemodialysis were compared between the two groups.Results During hemodialysis and after hemodialysis,the systolic blood pressure, diastolic blood pressure and mean arterial pressure of the observation group were higher than those in the control group, the differences were statistically significant (P<0.05).The incidence of intradialytic hypotension of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05).Conclusion The application of TEAS to Neiguan and Zusanli pretreatment before hemodialysis can effectively prevent hypotension during hemodialysis.

59 Clinical significance study of serum 4-hydroxynonenal, human cartilage glycoprotein-39 and renal injury molecular-1 in the diagnosis of early diabetic nephropathy

Objective To investigate the clinical value of serum 4-hydroxynonenal (4-HNE), serum human chondroglycoprotein-39 (YKL-40) and kidney injury molecule 1 (KIM-1) in early renal injury of diabetic nephropathy (DN), and to analyze the correlation of 4-HNE, YKL-40 and KIM-1 with renal injury index, blood glucose and lipid level.Methods A total of 89 patients with type 2 diabetes mellitus (T2DM) hospitalized in the Department of Endocrinology of Pingxiang People′s Hospital from January to July in 2019 were selected, and the patients were divided into normal albuminuria group or only diabetes group (group NA), microalbuminuria group (group MI) and macro albuminuria group(group MA) according to the 24 h urinary protein excretion rate (UAER).In addition, 53 healthy patients in the Physical Examination Department of Pingxiang People′s Hospital were selected as the normal control group (NC).Serum concentrations of 4-HNE, YKL-40 and KIM-1 were determined by enzyme linked immunosorbent assay (ELISA).Results The serum 4-HNE, YKL-40 and KIM-1 in the group MA were higher than those in the group MI, with statistically significant differences (P<0.05).The serum 4-HNE, YKL-40 and KIM-1 in the group MI were higher than those in the group NA, with statistically significant difference (P<0.05).The serum 4-HNE, YKL-40 and KIM-1 in the groups NA, MI and MA were higher than those in the group NC, and the differences were statistically significant (P<0.05).The fasting blood glucose (FPG), glycosylated hemoglobin (HbA1c), serum creatinine (SCr), blood urea nitrogen(BUN), UAER indexes in groups NA, MI MA were higher than those in group NC, with statistically significant differences (P<0.05).Correlation analysis showed that SCR, BUN, Urinary microprotein (UmALB), UAER, FPG and HbA1c were positively correlated with 4-HNE, YKL-40 and KIM-1 (r=0.428, 0.349, 0.390; 0.501, 0.446, 0.491; 0.478, 0.409,0.461; 0.414, 0.502, 0.497; 0.584, 0.558, 0.607; P<0.05).BUN was positively correlated with 4-HNE and YKL-40 (r=0.223, 0.176; P<0.05).Low density lipoprotein (LDL) was negatively correlated with 4-HNE, YKL-40 and KIM-1 (r=-0.169,-0.289,-0.247; P<0.05).Conclusion 4-HNE, YKL-40 and KIM-1 can be used as a sensitive indicator for the early diagnosis of DN, and is expected to provide a basis for the diagnosis of DN in clinical practice.Serum 4-HNE,YKL-40 and KIM-1 are positively correlated with renal injury index and blood glucose level.

64 Clinical effect of Probiotics combined with Rehabilitation New Fluid in the treatment of patients with active mild to moderate ulcerative colitis

Objective To investigate the clinical effect of Probiotics combined with Rehabilitation New Fluid in the treatment of patients with active mild to moderate ulcerative colitis.Methods From October 2016 to April 2019, 150 patients with active mild to moderate ulcerative colon admitted to our hospital were selected as the research objects.They were divided into study group and control group by random number table method, 75 cases in each group.The study group was treated with Probiotics combined with Rehabilitation New Fluid, and the control group was treated with Probiotics.After treatment, the total effective rate of treatment, the total score of disease activity, and the total incidence of adverse reactions were compared between the two groups.Results After treatment, the total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P<0.05).The total activity score of the lesion was lower than that of the control group, and the difference was statistically significant (P<0.05).The total incidence of adverse reactions was not statistically significant between the two groups (P>0.05).Conclusion Probiotics combined Rehabilitation New Fluid is effective in the treatment of patients with active mild to moderate ulcerative colitis, which can effectively reduce the total activity index of patients and promote the rehabilitation of patients, which is worthy of clinical promotion.

Objective To investigate the application of cardiac stenting and thrombolysis and reperfusion in patients with ST-segment elevation acute myocardial infarction (STEMI).Methods Forty-six patients with STEMI admitted to our hospital from December 2016 to October 2018 were enrolled in the study.They were divided into control group (n=23) and observed group (n=23) according to the random number table method.The patients in the control group were treated with cardiac stenting.The patients in the observation group were treated with thrombolysis and reperfusion.The serum myocardial necrosis markers, inflammatory factors and left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDD) and primary endpoint (mortality, rate of re-infarction) were observed in the two groups.Results Creatine kinase isoenzyme(CK-MB),cardiac troponin I(cTnI), serum high-sensitivity C-reactive protein(hs-CRP), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and interleukin-10 (IL-10) were lower than those of the control group, the differences were statistically significant (P<0.05).The LVEF of the observation group was higher than that of the control group and LVEDD was lower than the control group, the difference was statistically significant(P<0.05).The mortality and re-infarction rate of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05).Conclusion The effect of thrombolysis and reperfusion treating STEMI is more significant,and it can effectively improve the relevant indicators of patients.It is more advantageous than cardiac stenting and is suitable for clinical development and implementation.

Objective To explore the influence of optimized operation mode on myocardial reperfusion injury in patients with acute myocardial infarction undergoing emergency percutaneous coronary intervention(PCI).Methods From March 2018 to September 2019, 60 patients with acute myocardial infarction who underwent emergency PCI were selected as the study objects.All patients were divided into control group (30 cases) and optimization group (30 cases) according to random number table method.The patients in the control group were treated with conventional PCI, and the patients in the optimization group were treated with PCI which was suitable for comfort.The differences of creatine kinase isoenzyme (CK-MB) and amino terminal pro brain natriuretic peptide (NT-proBNP) between the two groups(△CK-MB, △NT-proBNP) were compared.The indexes of color Doppler echocardiography were compared between the two groups.Results The △CK-MB and △NT-proBNP in the optimized group were higher than those in the control group, the differences were statistically significant (P<0.05).Immediately after surgery,there were no significant differences between the two groups in left atrial diameter (LA), left ventricular diastolic diameter (LVDD), left ventricular systolic diameter (LVDS) and ejection fraction (EF) (P>0.05).One month after the operation, the LVDS and EF of the patients in the optimization group were higher than those in the control group, the differences were statistically significant (P<0.05).Conclusion The optimized PCI for patients with acute myocardial infarction can effectively improve the myocardial status and improve the level of cardiac function, so as to achieve better therapeutic effect.

74 Effect of laparoscopic sleeve gastrectomy in the treatment of obesity complicated with type 2 diabetes and its effect on gastrointestinal hormone levels

Objective To investigate the effect of laparoscopic sleeve gastrectomy (LSG) in the treatment of obesity complicated with type 2 diabetes and its effect on gastrointestinal hormone levels.Methods A total of 88 patients with obesity and type 2 diabetes treated in our hospital from January 2017 to January 2018 were selected as subjects, they were divided into control group and LSG group according to random number table method, with 44 cases in each group.Patients in the control group were given conventional hypoglycemic drugs and symptomatic treatment.The patients in the observation group were treated with LSG on the basis of the control group.The body mass index(BMI),serum glycosylated hemoglobin (HbA1c), insulin, glucagon-like polypeptide-1 (GLP-1), intestinal inhibitory peptide (GIP), and gastrin were compared between the two groups.Results After treatment, the BMI and HbA1c levels in the LSG group were lower than those in the control group, the differences were statistically significant (P<0.05).The insulin and gastrin levels in the LSG group were lower than those in the control group, and GLP-1 level was higher than that in the control group, the differences were statistically significant (P<0.05).There was no significant difference in GIP level between the two groups after treatment (P>0.05).Conclusion LSG is effective in the treatment of obesity complicated with type 2 diabetes, and significantly improves its gastrointestinal hormone levels, which is worthy of clinical application.

77 Clinical effect of laparoscopic retrograde subtotal cholecystectomy in the treatment of acute calculous cholecystitis

Objective To explore the clinical effect of laparoscopic retrograde subtotal cholecystectomy in the treatment of acute calculous cholecystitis.Methods A total of 80 patients with acute calculous cholecystitis treated in our hospital from January to December 2018 were selected as the research subjects, they were divided into the observation group and the control group by random number table method, with 40 cases in each group.The patients in the observation group were treated with laparoscopic retrograde subtotal cholecystectomy, and the patients in the control group were treated with laparoscopic retrograde subtotal cholecystectomy.The operative related indexes, postoperative complications and postoperative stress indexes between the two groups were compared.Results The time of operation in the observation group was shorter than that in the control group, and the amount of bleeding during operation and fluid replenishment after operation were less than those in the control group, the differences were statistically significant (P<0.05).There were no significant differences in temperature on the first day after operation, drainage time and hospitalization days between the two groups (P>0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05).Before operation,there were no significant differences between the two groups in C-reactive protein (CRP) and cortisol (P>0.05).After operation, the CRP and cortisol in the two groups were higher than those before operation, and the CRP and cortisol in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05).Conclusion Compared with total cholecystectomy, laparoscopic retrograde subtotal cholecystectomy for patients with acute calculous cholecystitis has the advantages of shorter operation time, less bleeding and fluid supplement, fewer complications and less stress response, which is worthy of clinical application.

80 Application of anti-rotation proximal femoral nail in elderly hip fracture

Objective To investigate the application of anti-rotation proximal femoral nail in elderly hip fracture.Methods A total of 90 elderly patients with hip fractures who were admitted to the Department of Orthopaedics of Suqian Workers Hospital of Jiangsu Province and Guangzhou Red Cross Hospital from January 2018 to February 2019 were selected as the study objects.They were divided into the control group (45 cases) and the observation group (45 cases) according to the isometric random sampling method.The control group was treated with proximal femoral locking plate, and the observation group was treated with anti-rotation proximal femoral nail.The perioperative indexes such as operative time, intraoperative blood loss, perspective time, hemoglobin loss, time of bed movement, fracture healing time, length of hospital stay of the two groups were compared.The Harris score, excellent and good rate, quality of life score, Barthel index and the total incidence of complications of the two groups were recorded.Results The operation time, time of bed movement, fracture healing time, and hospitalization time in the observation group were shorter than those in the control group, the intraoperative blood loss and hemoglobin loss in the observation group were less than those in the control group, and the fluoroscopy time in the observation group was longer than that in the control group,the differences were statistically significant (P<0.05).There were no significant differences between the two groups in theHarris score, quality of life score and Barthel index before operation (P>0.05).The Harris score, quality of life score and Barthel index of the two groups at 6 months after operation were higher than those before operation, the differences were statistically significant (P<0.05).The Harris score, quality of life score and Barthel index of the observation group at 6 months after operation were higher than those of the control group, the differences were statistically significant (P<0.05).The excellent and good rate of the observation group was higher than that of the control group, the total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).Conclusion In the treatment of hip fracture in the elderly, the proximal femoral nail of the anti-rotation femur is more conducive to the operation, promote the recovery of hip function, improve the quality of life and self-careability of daily life, and reduce the total incidence of complications.It is worthy of popularization and application.

84 Value of proximal femoral nail anti-rotation in the treatment of intertrochanteric fracture in the elderly

Objective To investigate the value of proximal femoral nail anti-rotation (PFNa) in the treatment of intertrochanteric fracture of femur in the elderly.Methods A total of 42 elderly patients with intertrochanteric fracture of femur admitted to our hospital from January 2016 to December 2018 were selected as the study subjects.They were divided into the observation group (n=21) and the control group (n=21) according to random sampling method.The control group was treated with dynamic hip screw (DHS), while the observation group was treated with PFNa.The clinical efficacy, surgical related indicators and the total incidence of adverse reactions were compared between the two groups.Results The intraoperative blood loss, postoperative drainage volume of the patients in the observation group were significantly less than those in the control group, the operation time of the patients in the observation group was significantly shorter than that in the control group, and the differences were statistically significant (P<0.05).The excellent and good rate of treatment in the observation group was 95.24%, which was significantly higher than that in the control group (66.67%), and the difference was statistically significant (P<0.05).The fracture healing time, total weight-bearing time and hospitalization time of the observation group were significantly shorter than those of the control group, with statistically significant differences (P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).Conclusion PFNa can effectively improve the clinical effect and reduce the risk of adverse reactions in the treatment of elderly patients with femoral intertrochanteric fracture, which is worthy of clinical reference and application.

87 Application effect of pre-hospital first aid in patients with pelvic fracture

Objective To explore the application effect of pre-hospital first aid in patients with pelvic fractures.Methods A total of 80 patients with pelvic fractures admitted to our hospital from January 2015 to December 2018 were selected as the in the study objects.They were divided into the CO group (40 cases) and the PR group (40 cases) according to different treatment methods.The patients in the CO group were treated with routine nursing methods, and the patients in the PR group were treated with pre-hospital first aid.The incidence of shock, the total incidence of complications, and the quality of fracture reduction were compared between the two groups.Results The incidence of shock was 27.50% (11/40) in the PR group, which was lower than 57.50% (23/40) in the CO group, the difference was statistically significant (P<0.05).The total incidence of complications in the PR group was 27.50%, which was significantly lower than that in the CO group (75.00%), the difference was statistically significant (P<0.05).The excellent and good rate of the PR group was higher than that of the CO group, the difference was statistically significant (P<0.05).Conclusion Pre-hospital first aid can promote the follow-up treatment of patients with pelvic fracture.The treatment effect of patients with pelvic fracture after pre-hospital first aid is better, and the damage control effect is better.

90 Clinical effect of aerosol inhalation of Acetylcysteine combined with Beclomethasone Dipropionate in the treatment of capillary bronchitis

Objective To investigate the clinical effect of aerosol inhalation of Acetylcysteine combined with Beclomethasone Dipropionate in the treatment of capillary bronchitis.Methods A total of 306 patients with capillary bronchitis treated in our hospital from February 2018 to February 2019 were selected as the research subjects.According to the sequence of electronic medical records,they were divided into treatment-A group(n=106),treatment-B group(n=100),and control group (n=100).All children were given routine treatment.In the treatment-A group, aerosol inhalation of Acetylcysteine combined with Beclomethasone Dipropionate was used.In the treatment-B group, aerosol inhalation of Salbutamol combined with Beclomethasone Dipropionate was used.In the control group, aerosol inhalation of Salbutamol alone was applied.The disappearance time of panting, wheezing sound and three concave signs in the three groups was compared.The treatment effect and the occurrence of adverse reactions were observed.Results The disappearance time of panting, wheezing sound and three concave signs in the treatment-A group was shorter than that of the treatment-B group and the control group, the disappearance time of panting, wheezing sound and three concave signs in the treatment-B group was shorter than that in the control group, the differences were statistically significant (P<0.05).The total effectiveness rate in the treatment-A group was higher than that in the treatment-B group and the control group,and the differences were statistically significant (P<0.05).There was no significant difference in the total effective rate between the treatment-B group and the control group (P>0.05).No serious adverse reactions occurred during the treatment of the three groups of children.Conclusion Aerosol inhalation of Acetylcysteine combined with Beclomethasone Dipropionate in the treatment of capillary bronchitis can shorten the disappearance time of clinical symptoms, improve the clinical treatment effect in few adverse reactions and good safety, which is worthy of clinical promotion and application.

Objective To explore the influence of midwife Doula delivery mode on pain degree and delivery quality of parturients.Methods A total of 100 parturients who gave birth naturally in our hospital from January to December in 2018 were selected as the study objects.All patients were divided into the observation group and the control group according to random number table method, 50 cases in each group.The control group was given routine perinatal nursing,while the observation group was given midwife Doula delivery mode nursing.The pain degree, duration of labor and the total incidence of complications were compared between the two groups.Results The pain level of the observation group was lighter than that of the control group, the difference was statistically significant (P<0.05).The first, second,third and total labor time of the observation group were (8.36±2.28), (0.58±0.17), (0.24±0.12), (9.29±2.27) h, which were shorter than those of the control group ([11.54±3.66], [0.81±0.29], [0.46±0.15], [13.53±3.39]) h, the differences were statistically significant (P<0.05).The total incidence of complications in the observation group was 10.00%, which was lower than 36.00% in the control group, the difference was statistically significant (P<0.05).Conclusion Doula delivery mode of midwife can effectively shorten the time of parturient, relieve the pain of parturient, improve the quality of parturient delivery, and benefit the prognosis of patients.

Objective To discuss the application effect of Doula labor analgesia with full-time accompanying painless delivery in elderly parturients.Methods All 316 elderly women in Guangzhou Panyu Shiqiao Hospital and Guangdong Maternal and Child Health Hospital from December 2017 to October 2018 were selected as research objects, they were divided into group A and group B according to random number table method,158 cases in each group.Group A were given birth in a traditional way without any pain or companionship, group B were given Doula labor analgesia combine with full-time accompanying painless delivery.The time of labor, analgesic effect, anxiety state, postpartum hemorrhage,mode of delivery and neonatal status were compared between two groups.Results The time of the first and second labor and the total duration of labor in group B were shorter than those in group A, the differences were statistically significant (P<0.05), but the difference of the third delivery time of the two groups was not statistically significant(P>0.05).The visual analogue scale (VAS), self-rating anxiety scale (SAS) scores of group B were lower than those of group A, and postpartum hemorrhage was less than that of group A, the differences were statistically significant (P<0.05).The natural delivery rate of group B was higher than that of group A,the difference was statistically significant(P<0.05).The difference between the two groups in neonatal asphyxia and newborn Apgar′s score were not statistically significant (P>0.05).Conclusion Doula labor analgesia+full-time accompanying painless delivery can significantly improve the analgesic effect and anxiety state, reduce postpartum hemorrhage, shorten the labor time and improve the natural delivery rate.

100 Application of community contracting model in postpartum visits and impact on maternal and child health

Objective To study the application of community contracting model in postpartum visits and the impact on maternal and child health.Methods Sixty women who were admitted to the Yingtan Maternal and Child Health Hospital from January to December 2018 were selected as subjects.The maternal women were divided into the control group and the observation group by random number table method, and 30 patients in each group.The pregnant women in the control group were studied with routine postpartum visits, and the observation group was followed by postpartum visits using the community contract model.The postpartum health status, neonatal health status, and total nursing satisfaction of the two groups of women were compared.Results The incidence rates of insufficient breast milk, poor wound healing, cracked nipples, postpartum bleeding and puerperal infection in the observation group were all lower than those in the control group, with statistically significant differences (P<0.05).The incidence rates of jaundice, eczema, oral mucosal infection and umbilical cord infection in the observation group were lower than those in the control group, with statistically significant differences (P<0.05).The total nursing satisfaction of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).Conclusion Through the application of community contract model in postpartum visit, the postpartum health status of mothers can be effectively improved, and at the same time, the health status of newborns and maternal nursing satisfaction can be effectively improved.

Objective To explore the application effect of sensory integration training in cognitive ability and motor development ability of autistic children.Methods A total of 58 children with autism who were treated in our hospital from January 2017 to December 2018 were selected as the study objects.They were divided into control group and observation group by random number table method, with 29 cases in each group.The control group was treated with psychotherapy and the observation group was treated with sensory integration training.The the changes of the autism behavior scale score, motor development ability and the gross motor factors in motor development ability were compared between the two groups.Results The score of autism behavior scale in the observation group after intervention was lower than that before intervention and the control group, and the difference was statistically significant (P<0.05).The scores of gross and fine movements in the observation group after intervention were higher than those before intervention and the control group, the differences were statistically significant (P<0.05).The scores of walking, running, jumping and throwing in the observation group after intervention were higher than those before intervention and the control group, the differences were statistically significant (P<0.05).Conclusion Sensory integration training has a positive effect on autistic children′s behavior and movement development ability.It can improve children′s sense, communication,body movement, language, life self-care ability and movement development ability.

107 Clinical effect of acupoint application therapy in the treatment of asthmatic bronchial pneumonia

Objective To investigate the clinical effect of acupoint application therapy in the treatment of asthmatic bronchial pneumonia.Methods From January 2016 to January 2018, 450 children with asthmatic bronchial pneumonia treated in our hospital were selected as the research subjects.They were divided into the experimental group (n=225)and the control group (n=225) according to envelope randomization.Patients in the control group were treated with conventional treatment, and in the experimental group, they were treated with acupoint application on the basis of conventional treatment.The therapeutic effect, time of symptoms disappearing, respiratory function and adverse reactions were compared between the two groups.Results The total effective rate of the experimental group was 95.56%, which was higher than that of the control group accounting for 81.78%, the difference was statistically significant (P<0.05).The disappearing time of lung rales, cough, and wheezing in the experimental group was shorter than that in the control group, the difference was statistically significant (P<0.05).The levels of volume to peak expiratory flow (VPEF)/ total expiratory volume (VE), time to reach peak tidal expiratory flow (TPTEF)/ total expiratory time (TE) after treatment in the two groups were higher than those before treatment, and the levels of VPEF/VE and TPTEF/TE after treatment in the experimental group were higher than those in the control group, the differences were statistically significant (P<0.05).The total incidence of adverse reactions in the experimental group was 7.11%, which was lower than that in the control group accounting for 18.22%, the difference was statistically significant (P<0.05).Conclusion Children with asthmatic bronchial pneumonia receiving acupoint application have good effect, with low incidence of adverse reactions,which can improve their respiratory function.

110 Application effect of free posture in labor process of parturient with abnormal fetal head position

Objective To investigate the application effect of free body position in labor process of parturients with abnormal fetal head position.Methods Fifty parturients with abnormal fetal head position delivered in our hospital from May 2018 to February 2019 were selected as study objects.All parturients were divided into control group and observation group according to random number table method, 25 cases in each group.The parturients in the control group were given traditional bladder stone cutting position and the guidance of screen delivery was given, while the parturients in the observation group were given free posture during delivery.The neonatal Apgar score,natural delivery rate,fetal head decline rate, total labor duration, postpartum bleeding volume and clinical nursing total satisfaction were compared between the two groups.Results The Apgar score of the observation group was (9.87±0.15) points, higher than that of the control group for(9.33±0.14)points,the difference was statistically significant(P<0.05).The natural delivery rate of the observation group (96.00%) was higher than that of the control group (76.00%), the difference was statistically significant (P<0.05).The rate of fetal head decline in the observation group was (1.01±0.06) cm/h, higher than that in the control group for (0.45±0.01) cm/h, and the total labor time of the observation group was (216.06.±7.18) min,shorter than that of the control group for (272.9±9.85) min, the amount of postpartum hemorrhage in the observation group was (185.26 ± 21.12) ml, less than (204.16±32.95) ml in the control group, the differences were statistically significant (P<0.05).The total satisfaction rate of clinical nursing in the observation group (100.00%) was higher than that in the control group (84.00%), the difference was statistically significant (P<0.05).Conclusion The free position is effective in correcting the abnormality of the fetal position.It can effectively shorten the labor process, increase the natural delivery rate, and effectively suppress the neonatal asphyxia.The safety is good.

Objective To investigate the application effect of laparoscopic surgery and traditional laparotomy in the treatment of ectopic pregnancy.Methods A total of 92 ectopic pregnant patients admitted to our hospital from July 2011 to July 2019 were selected as research subjects.By systematic sampling, they were divided into two groups: reference group underwent traditional laparotomy (n=46) and research group underwent laparoscopic surgery (n=46).The embryo clearance rate, surgical efficacy index, and postoperative satisfaction were compared between two groups.Results There was no significant difference in the embryo clearance rate between the two groups (P>0.05).The surgical time, off bed time after surgery, catheter indwelling time, and postoperative anal exhaust time in the research group were shorter than those in the reference group.The intraoperative bleeding volume of the research group was less than that of the reference group.The total satisfaction of the postoperative nursing of the research group was higher than that of the reference group, and the differences were statistically significant (P<0.05).Conclusion For patients with ectopic pregnancy, compared with traditional laparotomy, time index in laparoscopic surgery is much shorter, and the bleeding amount decreases greatly, and the total satisfaction of postoperative nursing increases significantly.

Objective To investigate the application effect of labor analgesia in primiparae with natural childbirth.Methods A retrospective analysis was performed on 102 primiparous women who gave birth naturally in our hospital from January 2018 to July 2019.They were divided into control group (n=51) and observation group (n=51) according to the different ways of delivery intervention.In the control group, non-drug analgesia was used, and in the observation group, non-drug analgesia combined with spinal anesthesia was adopted.The progress of labor, postpartum hemorrhage,pain, Apgar score and pregnancy outcome were compared between the two groups.Results There was no significant difference in the first, second, third, and total labor time between the two groups (P>0.05).The postpartum hemorrhage was (239.84±21.69) ml in the observation group, less than that in the control group for (287.04±22.31) ml.The visual analogue scale (VAS) score was (5.06±0.69) points, lower than that of the control group for (6.79±0.78) points.At 5 minutes, the Apgar score was (8.55±0.54) points, higher than that of the control group for (8.31±0.50) points.At 10 minutes, the Apgar score was (8.95±0.47) points, higher than that of the control group for (8.64±0.43) points, and the differences were statistically significant (P<0.05).The total incidence of adverse pregnancy outcomes in the observation group was 3.92%, lower than that in the control group accounting for 17.65%, with statistical difference (P<0.05).Conclusion Application effect of labor analgesic measures has less effect on the labor stages in primiparous women with natural childbirth, and can reduce postpartum hemorrhage, alleviate postpartum pain, and improve pregnancy outcomes.It is worthy of clinical promotion and application.

119 Effect of selective cervical cerclage combined with Phloroglucinol on pregnancy outcome and newborn of pregnant women with cervical insufficiency

Objective To investigate the effect of selective cervical cerclage combined with Phloroglucinol on pregnancy outcome and neonatal outcome of pregnant women with cervical insufficiency (CIC).Methods A total of 45 pregnant women with CIC admitted to our department from January 2015 to June 2018 were selected as the subjects, and divided into control group (n=22) and observation group (n=23) by random number table method.The control group used expectant therapy, and the observation group were given selective cervical cerclage combined with Phloroglucinol, the two groups were all followed up until the end of pregnancy.The neonatal prognosis and pregnancy outcomes were compared between the two groups.According to the period of operation, the pregnant women of observation group were divided into group A (12-16 weeks of gestation, 13 cases) and group B (more than 16 to 24 weeks of gestation, 10 cases).The neonatal prognosis and pregnancy outcomes were compared between the two groups.Results The neonatal survival rate,birth weight and 5 min Apgar score in the observation group were higher than those in the control group, and the asphyxia rate was lower than that in the control group, the differences were statistically significant (P<0.05).The gestational week and full-term rate of delivery in the observation group were higher than those in the control group, and the abortion rate and premature delivery rate were lower than those in the control group, the differences were statistically significant (P<0.05).The full-term rate of group A was higher than that of group B, while the premature rate was lower than that of group B, the differences were statistically significant (P<0.05).Conclusion Selective cervical cerclage combined with Phloroglucinol can prolong the gestational age of CIC patients, effectively improve the prognosis and pregnancy outcomes of neonates, and the effect of early pregnancy surgery is better.

122 Effect of transurethral bipolar plasma prostatectomy under combined spinal-epidural anesthesia

Objective To observe the effect of transurethral bipolar plasma prostatectomy under combined spinalepidural anesthesia.Methods A total of 60 cases of patients undergoing transurethral bipolar plasma prostatectomy in our hospital from March 2018 to June 2019 were selected and divided into the combined spinal-epidural anesthesia group (n=30) and the continuous epidural anesthesia group (n=30) according to the random number table method.Patients in the continuous epidural anesthesia group received continuous epidural anesthesia, patients in the combined spinal epidural anesthesia group received combined spinal epidural anesthesia.The block plane scores, onset time of anesthesia, perfect time of block and anesthetic drugs dosages, adverse reactions of the two groups were statistically analyzed.Results The onset time of anesthesia, perfect time of block of the combined spinal-epidural anesthesia group were significantly shorter than those of the continuous epidural anesthesia group (P<0.05), the dosage of anesthetics of the combined spinal-epidural anesthesia group was significantly less than that of the continuous epidural anesthesia group, the differences were statistically significant (P<0.05).But the difference of score of block plane between the two groups was not significant (P>0.05).The total incidence of adverse reactions of the combined spinal-epidural anesthesia group was 10.0% (3/30), which was significantly lower than 30.0% (9/30) of the continuous epidural anesthesia group,the difference was statistically significant (P<0.05).Conclusion The effect of combined spinal epidural anesthesia is better than that of continuous epidural anesthesia.

125 Effect of nebulized inhalation and intravenous Penehyclidine on preoperative lavage of side lung in patients with large volume lung lavage

Objective To investigate the effect of nebulized inhalation and intravenous Penehyclidine on preoperative lavage of side lung in patients with large volume lung lavage.Methods A total of 109 patients who needed to be treated with large volume lung lavage admitted to the First and Second ward of pneumoconiosis in our hospital from January 2018 to June 2019 were selected as the study objects, the patients were divided into the experimental group (50 cases)and the control group (59 cases) according to the order of treatment.The experimental group was given nebulized inhalation of Penehyclidine, and the control group was given intravenous injection of Penehyclidine.The peak airway pressure and pulmonary compliance of the two groups were observed at 30 min (T1), 60 min (T2) and 90 min (T3) after lavage.The pulmonary function (FVC, FEV1, PEF) and arterial blood gas (PaO2, PaCO2) of the two groups were compared before and after operation.Results The peak airway pressures at T2 and T3 in the two groups were lower than those at T1, the pulmonary compliance at T2 and T3 in the two groups were higher than those at T1, the differences were statistically significant (P<0.05).The peak airway pressure at T2 and T3 in the experimental group was lower than that in the control group, the difference was statistically significant (P<0.05).The pulmonary compliance of the experimental group at T2 and T3 was higher than that of the control group, the difference was statistically significant (P<0.05).The level of PEF, FVC, FEV1 in the experimental group after operation were higher than those in the control group, the differences were statistically significant (P<0.05).The level of PaO2 in the experimental group after operation was significantly higher than that in the control group, and the level of PaCO2 in the experimental group after operation was significantly lower than that in the control group, the difference was statistically significant (P<0.05).Conclusion The clinical treatment effect of atomization inhalation or intravenous injection of Penehyclidine in the lavage side of patients with large volume lung lavage before operation is ideal, which can improve the pulmonary compliance at different time points after operation, reduce the peak airway pressure and improve the pulmonary function.

Objective To investigate the clinical effect of Valsartan Hydrochlorothiazide in the treatment of patients with cerebral-cardiac syndrome.Methods A total of 90 cases of patients diagnosed with cerebral-cardiac syndrome admitted to our hospital from January 2016 to January 2019 were selected as the research objects, who were divided into treatment group (45 cases) and control group (45 cases) according to random number table method,the control group was treated by regular treatment,the treatment group was treated by regular treatment combined with Valsartan Hydrochlorothiazide.The clinical efficacy, electrocardiograph (ECG) improvement, C-reactive protein (CRP), tumor necrosis factor(TNF-α) and interleukin-6 (IL-6) levels before and after treatment were compared between the two groups.Results The total effective rate and the ECG effective rate of the treatment group were higher than those of the control group,and the levels of CRP, IL-6 and TNF-α in the treatment group were lower than those of the control group, the differences were statistically significant(P<0.05).Conclusion Valsartan Hydrochlorothiazide can significantly reduce the CRP,TNF-α and IL-6 levels, which can improve the clinical curative effect of patients with cerebral-cardiac syndrome.

132 Application of Ziprasidone for weight gain induced by Clozapine

Objective To investigate the application of Ziprasidone for weight gain induced by Clozapine in patients with schizophrenia.Methods A total of 80 patients with chronic schizophrenia were recruited from January 2017 to January 2018 in our hospital.Based on the random number table method, they were assigned to the study group (n=40)and the control group (n=40).The study group was treated with Ziprasidone combined with Clozapine, and the control group was treated with Clozapine.Body weight and body mass index were assessed between the two groups.Similarly,clinical efficacy and adverse drug reactions were assessed by using positive and negative syndrome scale (PANSS) and treatment emergent symptom scale (TESS), respectively.Results After 12 weeks of treatment, there was no significant difference in the rate of discontinuation between the two groups (P>0.05).After 12 weeks of treatment, the body weight and body mass index of the patients and the total score of PANSS in the study group were lower than those in the control group, and the differences were statistically significant (P<0.05).There was no significant difference in the total score of TESS between the two groups (P>0.05).Conclusion Ziprasidone as a supplemental treatment is associated with weight loss in patients with schizophrenia who receiving the treatment of Clozapine.However, evidence-based medicine is needed to recommend it to clinical practice.

136 Effect of Guipazide combined with Xuesaitong in the treatment of ischemic cerebrovascular disease

Objective To explore the effect of Guipazide combined with Xuesaitong in the treatment of ischemic cerebrovascular disease.Methods Ninety patients with ischemic cerebrovascular disease treated in Department of Neurology of our hospital from February 2018 to February 2019 were selected as subjects.They were divided into two groups according random number table method.The control group was treated with routine drugs.On this basis, the observation group was treated with Guipazide combined with Xuesaitong.The clinical efficacy, the changes of hemorheological indexes, the degree of neurological deficit and the ability of daily living and the levels of serum inflammatory factors in the two groups were compared.Results The total effective rate of treatment in the observation group was 95.56%,which was higher than 77.78% in the control group (P<0.05).After treatment, the plasma viscosity, whole blood high shear viscosity, whole blood low shear viscosity, fibrinogen and specific volume of blood cells in the observation group were lower than those in the control group (P<0.05).After treatment, the National Institute of Health stroke scale(NIHSS) score of the observation group was lower than that of the control group, while the Barthel index of the observation group was higher than that of the control group (P<0.05).The levels of hypersensitivity C-reactive protein (hs-CRP), homocysteine (Hcy), tumor necrosis factor-α (TNF-α) in the observation group were lower than those in the control group, the level of IL-10 (IL-10) was higher than that of the control group (P<0.05).Conclusion The clinical effect of Guipiazide combined with Xuesaitong in the treatment of ischemic cerebrovascular disease is remarkable, which can effectively improve blood viscosity, inhibit platelet aggregation, improve cerebral blood circulation, reduce the level of inflammatory factors and repair damaged nerve function.

139 Physical examination results analysis of cadres in Guangdong provincial units from 2016 to 2018

Objective To find out the detecting results of different diseases during the physical examination of cadres in Guangdong provincial units from 2016 to 2018 and to guide the prevention and control of chronic diseases in research subjects.Methods The data of physical examination results in the cadres in provincial units from 2016 to 2018 were collected, the detection rates of different diseases were calculated, and the differences of detection rates of the same disease in different years were detected using Pearson χ2 or linear trend χ2.Results The participationrates of physical examination from 2016 to 2018 were 78.6%, 78.5%, and 76.0% respectively.In addition to the detection rates of myoma of uterus and gallstones in 2016 and myoma of uterus in 2018,the three-year detection rates of the top 11 diseases by the detection rate were all higher than 10%.The top 6 diseases by the three-year detection rate were prostatic hyperplasia, breast hyperplasia, hypertension, hyperuricemia, fatty liver, and hyperlipidemia, and their detection rates were all higher than 29.9%.The top 2 diseases by the three-year detection rate were prostatic hyperplasia, breast hyperplasia, hypertension, and hyperuricemia, and their detection rates were all higher than 39.5%.The three-year detection rates of prostatic hyperplasia, breast hyperplasia, hyperlipidemia, and fatty liver decreased year by year (P<0.001),the three-year detection rates of elevated fasting blood glucose, arrhythmia, gallstones, tumor and suspicious tumor elevated year by year (P<0.001), and the three-year detection rates of hypertension, hyperuricemia, myoma of uterus, and urinary calculus were fluctuated irregularly.Conclusion The study population should be the key population for prevention and control of chronic diseases; the key preventing and controlling diseases are hyperuricemia, fatty liver, hyperlipidemia, hypertension, prostatic hyperplasia, and breast hyperplasia, and hypertension and hyperuricemia are top priorities for prevention and control; the monitoring of elevated fasting blood glucose, arrhythmia, and gallstones should be strengthened, and the priority levels for prevention and control should be appropriately raised; comprehensive prevention and control measures with rich content and various forms should be regularly implemented in the long term, with more focus on the pertinence.

143 Investigation on support status of family members of schizophrenic patients with poverty records in our hospital

Objective To investigate the support status of family members of schizophrenic patients with poverty records in our hospital.Methods A total of 260 family members of schizophrenic patients with poverty records who were admitted to Hebei Sixth People′s Hospital from January 2017 to December 2018 were selected as the survey objects.The self-made general situation questionnaire, family support questionnaire for patients, self-rating anxiety scale(SAS), and social support rating scale (SSRS) were used to investigate the family members of patients, and the results were compared and analyzed for correlation.Results The complete completion rate of the 6 items of support for the patients in family members was less than 50%, with a total average score of (4.11±1.03) points, which was at a lower level of support.The total average score of SAS was (65.45±5.34) points, which was at a moderate anxiety status, and the total average score of SSRS was (22.55±2.32) points, which was at a medium social support level.The support scores of young caregivers were higher than those of the elderly, the support scores of families with high education and economic income were higher than those with low education and low economic income, the support scores in patients with long course of illness and more hospital stays were lower than those of patients with short course of illness and fewer hospital stays, and the support score of female caregivers was higher than that of male caregivers, with statistically significant differences (P<0.05).Family support score was positively correlated with SSRS score (r=0.512, P<0.05) and negatively correlated with SAS score (r=-0.498, P<0.05), and SSRS score was negatively correlated with SAS score (r=-0.572, P<0.05).Conclusion The support level of family members of schizophrenic patients with poverty records in our hospital is low, which is related to the educational level of caregivers, economic income, anxiety level and social support.The hospital stays and course of illness also affect the support level of patients.

147 Investigation analysis of drug-resistant phenotypes of Escherichia coli clinically isolated from our hospital in 2016

Objective To analyze the distribution of drug-resistance phenotypes of clinically isolated Escherichia coli,and to provide clinicians with laboratory data to better guide antibiotic therapy.Methods The clinical isolates of 851 Escherichia coli in the First Affiliated Hospital of Xiamen University from January to December 2016 were retrospectively analyzed.Strain identification and drug sensitivity test were conducted by BioMerieux VITEK2 Compact automatic bacterial identification and drug sensitivity analysis system.Results A total of 851 strains of Escherichia coli were clinically isolated in 2016, mainly including 425 (49.9%) strains from urine, 157 (18.4%) strains from whole blood,132 (15.5%) sterile strains from body fluid (including bile, ascites, pleural effusion, joint fluid but not cerebrospinal fluid), and 50 (5.9%) strains from sputum.There were four drug-resistant phenotypes of Tetracycline antibiotics (Tetracycline and Tegacycline), among which Tetracycline (R/I) Tegacycline (S) was the dominant phenotype.There were four drug-resistant phenotypes of Quinolone antibiotics(Ciprofloxacin and Levofloxacin) and two major phenotypes were Ciprofloxacin (R/I) Levofloxacin (R/I) and Ciprofloxacin (S) Levofloxacin (S).There were six drug-resistant phenotypes of aminoglycoside antibiotics (Gentamycin,Tobramycin and Amikacin) and two major phenotypes were Gentamycin (S) Tobramycin (S) Amikacin (S) and Gentamycin (R/I) Tobramycin (R/I) Amikacin (S).There were five drug-resistant phenotypes of β-lactam enzyme inhibitor complexes (Amoxicillin/Clavulanic Acid, Ampicillin/Sulbactam and Piperacillin/Tazobatam) and Amoxicillin/Clavulanic Acid (S) Ampicillin/Sulbactam (R/I) Piperacillin/Tazobattan (S) was the dominant phenotype.There were eleven drugresistant phenotypes of β-lactam antibiotics (Ampicillin, Cefuroxime, Ceftaxime, Ceftadime, Cefepime, Aztreonam,Imipenem and Meropenem) and two major phenotypes were Ampicillin (R/I) Cefuroxime (S) Ceftaxime (S) Ceftadime (S)Cefepime (S) Aztreonam (S) Imipenem (S) Meropenem (S) and Ampicillin (R/I) Cefuroxime (R/I) Ceftaxime (R/I) Ceftadime (S) Cefepime (S) Aztreonam (S) Imipenem (S) Meropenem (S).There were some differences in the distribution of drug-resistant phenotypes among different specimens, but the distribution of major drug-resistant phenotypes was basically consistent.The resistance rate of Escherichia coli to Tetracycline, Quinolones and aminoglycosamines was more than or close to 50%, but Escherichia coli remained highly sensitive to Tegacycline, Imipenem and Meropenem.Conclusion Escherichia coli mainly causes urinary tract infection, blood stream infection, cholecystitis and abdominal infection.The clinical prevalence of bacterial resistance phenotypes is relatively stable and the main kind of resistant pattern is relatively obvious.There are some differences in the distribution of drug-resistant phenotypes among different specimens.Depending on the source of the specimen, the clinicians can select appropriate antibiotics for relevant treatment, which can provide more specific theoretical basis for clinical experience in drug selection.

151 Investigation and analysis of catheter exit-site care by peritoneal dialysis nurses

Objective To understand the training status of catheter exit-site care (hereinafter referred to as "exit-site care") in peritoneal dialysis (PD) patients and the condition of exit-site care by PD nurses, so as to provide basis for the establishment of management standards and continuous improvement on quality of PD exit-site care.Methods The nursing version of Peritoneal Dialysis Catheter Exit-site Care Questionnaire was designed by researchers.From April to May 2018, 26 PD nurses in 10 hospitals with PD outpatient clinics in a coastal area of Guangdong Province were investigated through Questionnaire Star and WeChat APP.Results The average incidence of PD catheter-related infections in 10 hospitals with PD clinics in the area was about 0.15 times per year.Five of the ten hospitals had full-time and part-time nurses, and the other five hospitals had only part-time nurses (concurrently with ward nursing job).Among the 26 PD nurses, 9 cases (34.62%) were full-time PD nurses, the ratio of full-time nurses to patients was approximately 1∶95.67, and 17 cases (65.38%) were part-time PD nurses, the ratio of part-time nurses to patients was approximately 1∶37.76.The average training time of each patient was (15.39±5.06) hours by full-time nurses, and that by part-time nurses was (11.13±5.85) hours.All the PD nurses would retrain patients, and among them the retraining frequency of 13 PD nurses was fixed.A total of 92.31% of the PD nurses required post-shower exit-site care, 84.62% of the PD nurses required patients to change dressing once a day when the exit-site infections occurred, or even more frequently in other dressing changes.Only 2 PD nurses asked patients to routinely apply antibacterial ointment to prevent infections.Altogether 92.31% of the PD nurses could complete wound evaluation as suggested, 73.08% of the PD nurses used normal saline and iodine for disinfection and dressing change, and all the PD nurses could use dressings to cover the exit-site before maturity and during infections.Conclusion At present, half of the hospitals in this region that carry out PD treatment are in the initial development stage, and the ratio of nurses to patients among the hospitals varies greatly.The three aspects on training of patients, wound evaluation before dressing change and care points during dressing change are basically consistent with the International Society for Peritoneal Dialysis Guidelines.However, there is a difference in the application of antibacterial ointment from the guideline, which needs to be further studied.

156 Quality of life investigation of type 2 diabetes patients in a primary hospital in Zhuhai and analysis of its influencing factors

Objective To investigate the quality of life of patients with type 2 diabetes and analyze the influencing factors.Methods A total of 80 patients with type 2 diabetes who were treated in a primary hospital in Zhuhai from January to October 2019 were selected as the research objects.The quality of life of patients was assessed by the diabetes specific quality of life scale (DSQL), and the influencing factors were analyzed.Results In this study, the total score of DSQL of type 2 diabetic patients was 33-98 points, and the average score was (58.77±12.04) points.The scores of psychological/mental dimension, physiological function dimension, treatment dimension and social relationship dimension were (2.53±0.47), (2.19±0.44), (1.87±0.44) and (1.66±0.42) points respectively, of which the psychological/mental dimension score was the highest, and followed by physiological function dimension score.The DSQL scores of patients with more than 3 complications, unreasonable dietary control, unsatisfactory blood glucose control, poor mastery of diabetes knowledge, low personal income (≤2000 and 2001-4000 yuan)and intermittent drug use were higher than those of patients with less than 3 complications, reasonable dietary control, ideal blood glucose control, good mastery of diabetes knowledge, high personal income (4001-6000 and >6000 yuan) and regular drug use respectively, the differences were statistically significant (P<0.05).Conclusion The quality of life of patients with diabetes mellitus is general, effected by more than 3 complications, unreasonable diet control, unsatisfactory blood glucose control, poor knowledge of diabetes,low personal income and nonstandard drug use.The clinical intervention can be based on this to improve the quality of life of patients.

Objective To explore the application effect of basic public health services on diabetes management.Methods From January 2017 to January 2019, 100 patients with type 2 diabetes mellitus in Community Health Service Center, Guicheng Street, Nanhai District of Foshan City were selected as the research objects.They were randomly divided into two groups using the random number table, with 50 patients in each group.In the control group, patients did not receive basic public health services, and in the intervention group, participants received basic public health services.The physical examination rate was compared between the two groups.Blood glucose, physical signs and body mass index (BMI) control were compared between the two groups.Changes in lifestyle and drug-related indicators were compared between the two groups.Results The active examination rates of the blood glucose, blood lipid, urine routine, renal function, foot, fundus, and electrocardiogram of patients in the intervention group were higher than those in the control group, and the differences were statistically significant (P<0.05).In the intervention group, the blood glucose control rate, the touching rate of arteria dorsalis pedis, and the normal BMI rate were higher than those in the control group, and the differences were statistically significant (P<0.05).In the intervention group, the proportions of exercise,staple food ≤400 g/d, good compliance behavior, good medication rate, and good medication compliance among patients were higher than those of the control group, and the differences were statistically significant (P<0.05).There were no significant differences in smoking, alcohol drinking, and psychological status between the two groups (P>0.05).Conclusion When applying basic public health service diabetes mellitus management to diabetic patients, it can significantly increase the pouplarizaiton rate of patients, greatly improve blood glucose and physical signs, can markedly improve medication regularity and compliance, and change lifestyles.This method is worthy of application and promotion.

163 Correlation analysis between social depression and professional identity,turnover intention of nurses in a tertiary hospital

Objective To analyze the correlation between social depression and professional identity, turnover intention of nurses in a tertiary hospital.Methods From September to October 2019, a questionnaire survey was conducted among 588 nurses in a tertiary hospital in Quanzhou using the social depression scale, nurses′ occupational identity rating scale, and turnover intention scale.The status of nurses′ social depression, professional identity and turnover intention was analyzed, the impact of demographic characteristics on social depression was investigated and the correlation between social depression and professional identity, turnover intention of nurses was analyzed.Results The senior depression score of 588 nurses was (20.19±9.51) points, the colleague depression score was (19.72±8.27) points, the total score of professional identity was (100.52±22.94) points, and the total turnover intention score was (15.09±4.68) points.There were statistically significant differences in the scores of senior depression and colleague depression among nurses in different educational backgrounds, titles, and years of work (P<0.05).There were statistically significant differences in the colleague depression scores among nurses under different employment conditions (P<0.05).Both senior depression and colleague depression were negatively related to professional identity but positively related to the turnover intention (P<0.01).Conclusion The nurses in the tertiary hospital have suffered from moderate social depression.The nurses′ professional identity is at a medium level but the turnover intention is higher, which is obviously related to social depression.Hospital administrators should actively take effective measures to reduce social depression in nurses′work and create a good working environment for nurses to improve their professional identity and reduce their turnover intention, thus stabilizing the hospital nursing team.

168 Application of intelligent management system in the management of drugs for termination of pregnancy

Objective To explore the application effect of intelligent management system in the management of drug for termination of pregnancy.Methods An intelligent management system of drugs for termination of pregnancy compatible with the hospital information system (HIS) of our hospital was built from January to March 2018, and the registration error rate before the application of the system (January 1-March 12, 2018) and the registration error rate after the application (March 13-31, 2018) and the time taken by pharmacists to deploy the prescription for terminating pregnancy drugs were compared.Results After the application of intelligent management system, the registration error rate of terminating pregnancy drugs was fewer than that before the application of intelligent system and the average time spent by pharmacists in dispensing termination drugs was shorter than that before the application of intelligent system, the differences were statistically significant (P<0.01).Conclusion The application of the system promotes the transformation of drug management from extensive to information-based fine management mode, and the data are safe and easy to operate.

176 Application effect of nursing identifier management in strengthening nursing quality in neonatal intensive care unit

Objective To discuss the applicative effect of nursing identifier management in strengthening nursing quality in neonatal intensive care unit (NICU).Methods A total of 73 premature infants admitted to NICU department from January to December 2018 were selected as the observation group, and 73 premature infants admitted to NICU department from January to December 2017 were selected as the control group.In the observation group, the nursing identifier management was implemented, while in the control group, the traditional nursing management was implemented.The incidence rate of nursing error, parents′ tatal satisfaction and nursing quality scores of the two groups were compared.Results The incidence rate of nursing error in the observation group was 1.37%, which was significantly lower than that of the control group (10.96%), with statistically significant difference (P<0.05).The parents′ total satisfaction in the observation group was 97.26%, which was significantly higher than that of the control group (78.08%), with statistically significant difference (P<0.05).The assessment scores of identifier management, risk evaluation, checking works and safety management in the observation group were higher than those of the control group, with statistically significant difference (P<0.05).Conclusion The application of nursing identifier management in NICU can strengthen nursing quality, improve nursing satisfaction and reduce nursing errors accidents.

Objective To explore the influence of the hierarchical management of responsibility system on the nursing effect in emergency observation room.Methods A total of 60 patients admitted to Department of Emergency in our hospital from January to December 2018 were selected and divided into the observation group (n=30) and the control group(n=30) according to random number table method.Patients in the control group received nursing under the general public hospital management system, and the observation group received nursing under the hierarchical management of responsibility system.The quality of nursing including health education awareness score, nursing score, basic nursing implementation score, and score of accurate admission evaluation, and nursing satisfaction under different management systems were compared in the two groups.Results The health education awareness score, excellent and good nursing score, basic nursing implementation score, and accurate admission evaluation score, the patient′s satisfaction of the observation group were higher than those of the control group, with significant differences (P<0.05).Conclusion The hierarchical management of responsibility system in the emergency observation room obtains a good effect, which can effectively improve the nursing quality and increase patient′s satisfaction on nursing.This method is worthy of application by hospitals at all levels.

182 Effect study on standardized training of traditional Chinese medicine nursing in different levels

Objective To explore the application effect of standardized training of traditional Chinese medicine nursing in different levels on newly recruited nurses.Methods A total of 60 newly recruited nurses from January 2016 to May 2017 in our hospital were selected and divided into the observation group and the control group according to odd and even number method.There were 30 cases in both groups.The control group was given routine unified content training,while the observation group was divided into seven layers according to different nursing specialties, and the standardized training of traditional Chinese medicine nursing was implemented.The scores of traditional Chinese medicine theory, operation skills and comprehensive abilities of the two groups were compared.Results After training, the score of traditional Chinese medicine theory was(90.36±5.43)points,the score of operation level was(92.35±2.28)points,the score of clinical practice ability was (21.36±1.14) points, the score of working attitude was (23.54±1.02) points, the score of communication skills was (20.69±2.06) points, and the score of emergency response ability was (22.45±2.09) points in the observation group, which were higher than those in the control group ([86.35±4.21], [87.24±2.97], [18.53±1.42],[19.84±1.67], [17.28±2.51], [18.01±2.75]points), and the differences were statistically significant (P<0.05).Conclusion The standardized training of layered traditional Chinese medicine nursing in different levels can effectively improve the theory and operation skills of newly recruited nurses, and improve their comprehensive ability, which is highly feasible.

189 Application of maternity simulation training in practice teaching

Objective To explore the application effect of simulation training in the teaching of clinical obstetrics interns.Methods Seventy interns from the obstetrics in our hospital from January to December in 2018 were selected as subjects.They were divided into the control group and the experimental group according to the random digital table method, 35 interns in each group.The control group used the organizational teaching, while the experimental group used the obstetric simulation training method, after 6 months internship, the learning effect, total teaching satisfaction were compared between the two groups.Results The scores of specialist theoretical knowledge, medical history collection skills, and clinical operation skills in experimental group were all higher than those of the control group, and the differences were all statistically significant (P<0.05).The total teaching satisfaction of the experimental group was 97.1%, higher than 88.6% of the control group, and the difference was statistically significant (P<0.05).Conclusion The obstetrics simulation training teaching method can deepen the interns′ understanding of the theoretical knowledge of obstetric nursing, improve the theoretical knowledge level, and enhance the practical ability of interns to solve clinical obstetric diagnosis and treatment problems independently, which is welcomed to interns.

196 Anticoagulant therapy analysis and pharmaceutical monitoring in an elderly patient with atrial fibrillation undergoing catheter ablation: a case report

In this paper, one elderly atrial fibrillation patient with anticoagulant therapy undergoing catheter ablation was given pharmacological monitoring and guidance from the causes of Warfarin international standardized ratio (INR)fluctuations, the perioperative period and the formulation of long-term anticoagulation programs, the conversion between traditional oral anticoagulants and new oral anticoagulants, and precautions for taking anticoagulants.Through active pharmacological monitoring and guidance by the clinical pharmacist, there were no thrombotic and hemorrhagic adverse events during catheter ablation, and the anticoagulation strategy was adjusted to a new oral anticoagulant: Dabigatran, to ensure sufficient anticoagulant efficacy and also reduce the risk of bleeding.The clinical pharmacist and the medical care providing proper pharmacological management of perioperative anticoagulation for such elderly patients,not only ensures the rationality of anticoagulation treatment, but also ensures the safety and continuity of anticoagulation.

199 Follicular dendritic cell sarcoma of nasopharynx: a case report and literature review

The clinical and pathologic characteristics of follicular dendritic cell sarcoma (FDCS) of nasopharynx were investigated, so as to improve the diagnosis and treatment level of the nasopharynx FDCS.The clinical features,histopathological charactersistics and immunophenotype of a case of FDCS in nasopharynx were retrospectively analyzed, the patient was followed-up for a long period of time and the relative literature at home and abroad was reviewed.A 36-year-old male patient was admitted to hospital due to "progressive nasal obstruction on the right side for more than two months".The largest diameter of the tumor was 2.5 cm.The tumor cells were diffusely arranged in a sheet shape, with obvious cell aberrations, oval-shaped, spindle-shaped, like "syncytium cells", and a small number of multinuclear tumor cells and a large number of mitotic figures.In the focal area, dilated pseudovascular spaces and perivascular lymphatic sheaths could be seen and coagulative necrosis could be seen at the periphery.Immunohistochemistry: CD21 and CD23 diffuse (+), CD35 focal (+), EMA(++), CK (-), Ki-67 index 80%, in situ hybridization EBV(-).The patient was closely followed up for 12 years without recurrence or metastasis.FDCS is very rare in nasopharynx,and definite diagnosis should be combined with histopathological morphology and immunohistochemical phenotype.The follow-up results of this case support that nasopharynx FDCS is an intermediate grade tumor, which can be cured by radical resection of the lesion.

203 Application of early nursing intervention in patients with severe traumatic brain injury undergoing early functional exercise combined with hyperbaric oxygen therapy

Objective To explore the application of early nursing intervention in patients with severe traumatic brain injury undergoing early functional exercise combined with hyperbaric oxygen therapy.Methods From August 2017 to February 2019, 60 patients with severe traumatic brain injury treated with early functional exercise combined with hyperbaric oxygen therapy were selected.They were divided into observation group (n=30) and control group (n=30) according to the different nursing methods.The control group was given routine nursing, while the observation group was given early nursing intervention.The total compliance of hyperbaric oxygen therapy and (FMA)score of limb motor function scale were compared between the two groups.Results The total treatment compliance of hyperbaric oxygen in the observation group was higher than that of the control group (P<0.05), and the score of FMA in the observation group was higher than that of the control group (P<0.05).The total satisfaction of the observation group was 96.67%, higher than that of the control group (66.67%), and the difference was statistically significant (P<0.05).Conclusion The early nursing intervention has a remarkable effect in the early functional exercises of the hemiplegic patients after the severe head injury and the application of the hyperbaric oxygen in the patients, can effectively improve the treatment compliance of the patients, improve the function recovery speed of the limbs, and improve the satisfaction of the patients on the nursing.

206 Application effect of predictive health education in outpatient dressing change nursing

Objective To explore the application effect of predictive health education in outpatient dressing change nursing.Methods From September 2017 to September 2018, 120 outpatient dressing change nursing were selected as the research objects, according to the method of the double chromospheres, they were divided into control group (n=60)and observation group (n=60).The control group used conventional nursing methods outpatient treatment, observation group used predictive health education on the basis of the control group.The self-care ability score, total incidence of complications and total nursing satisfaction were compared between two groups.Results After nursing, the self-care ability score of the observation group was higher than that of the control group, the difference was statistically significant(P<0.05).The total incidence of complications in the observation group (3.3%) was lower than that in the control group(16.7%), the difference was statistically significant (P<0.05).The nursing total satisfaction of the observation group(95.0%) was higher than that of the control group (78.3%), the difference was statistically significant (P<0.05).Conclusion Applying predictive health education in outpatient dressing changing nursing can improve patients′ self-care ability and nursing satisfaction and reduce the incidence of complications through health education guidance.

209 Clinical effect of predictive nursing on prevention of deep vein thrombosis after lower extremity fracture

Objective To explore the clinical effect of predictive nursing to prevent deep vein thrombosis (DVT) after lower extremity fracture.Methods A total of 100 patients with lower extremity fractures admitted to our hospital from July 2017 to June 2019 were enrolled in the study.According to the random number table method, they were divided into control group (n=50) and observation group (n=50).The control group adopted the general routine nursing method after the operation of lower limb fracture.The observation group adopted the predictive nursing method on the basis of general routine nursing.The effect of the velocity of lower extremity venous blood flow before and after surgery and total satisfaction were compared between the two groups.Results The venous blood flow velocity of the lower limbs of the observation group was faster than that of the control group, and the difference was statistically significant (P<0.05).The total nursing total satisfaction of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).Conclusion Predictive nursing has a significant effect in preventing the formation of DVT in patients with lower extremity fractures, and the patient′s satisfaction with the nursing has also improved, which has certain promotion value.

212 Application effect of Yangming moxibustion intervention in nursing of patients with lung distention and constipation

Objective To explore the application effect of Yangming moxibustion intervention in the nursing of patients with lung distention and constipation.Methods Retrospective analysis was conducted on 200 patients with lung distention and constipation who were admitted to our hospital from February 2016 to November 2018.The patients were divided into the control group (n=100) and the study group (n=100) according to their nursing intervention methods.The control group was given routine nursing, and the study group was given Yangming moxibustion intervention.The frequency of defecation, excrement status, and quality of life between two groups were compared.Results The first dejection time and the time for the excrement form to return to normal in the study group were shorter than those in the control group, and the scores of defecation times and excrement traits in the study group at 2 and 4 were lower than those in the control group, with statistical differences (P<0.05).Before nursing, there was no statistically significant difference in the St.George′s respiratory questionnaire (SGRQ) score between the two groups (P>0.05).After nursing, the SGRQ score in the study group was lower than that in the control group, with statistical difference (P<0.05).Conclusion In patients with lung distension, Ren meridian Yangming moxibustion intervention on the basis of abdominal massage and other routine nursing care can significantly relieve their defecation difficulties, constipation, and asthma symptoms, and improve their quality of life.

215 Application of protection motivation theory nursing in postoperative rehabilitation of patients with thoracolumbar burst fracture

Objective To explore application of protection motivation theory (PMT) nursing in postoperative rehabilitation of patients with thoracolumbar burst fractures.Methods A total of 60 patients who underwent thoracolumbar burst fracture treatment in Shangrao Fifth People′s Hospital from January 2016 to March 2019 were selected as study objects.According to random number table method, they were divided into observation group (n=30) and control group (n=30).The control group was given routine nursing.On this basis, observation group was additionally given PMT for nursing intervention.The degree of spinal cord injury, self-management ability, and degree of low back pain were compared between the two groups.Results At 1 and 3 months after intervention, scores of spinal cord injury (Frankel) in observation group were higher than those in control group, the differences were statistically significant (P<0.05).At 3 months after intervention, scores of daily life management, information management, emotion management, symptom management and self-efficacy management in observation group were higher than those in control group, the differences were statistically significant (P<0.05).At 1 and 3 months after intervention, scores of visual analogue scale (VAS) in observation group were lower than those in control group, the difference was statistically significant (P<0.05).Conclusion The application of PMT in postoperative rehabilitation of thoracolumbar burst fracture patients for nursing intervention can promote recovery of injured spinal cord, improve self-management ability, and decrease VAS score.

218 Application effect of Rosenthal effect nursing in elderly patients undergoing total hip arthroplasty

Objective To explore the application effect of Rosenthal effect nursing in elderly patients undergoing total hip arthroplasty (THA).Methods A total of 76 elderly patients undergoing THA in our hospital from July 2015 to December 2018 were selected and divided into the control group (n=38) and the observation group (n=38) by the random number table method.The control group was given routine nursing.On this basis, the observation group was given Rosenthal effect nursing intervention.The psychological status before and after intervention, self-management ability after intervention, quality of life before and after intervention were compared between the two groups.Results After intervention, Hamilton rating scale for depression (HAMD) and Hamilton anxiety scale (HAMA) scores of the observation group were (11.35±1.22) and (14.67±1.58) points, lower than those of the control group for (12.26±0.97) and (15.15±1.26) points, the differences were statistically significant (P<0.05).However, scores of disease monitoring, emotion management, self-care of pressure sores and medication compliance were (9.35±2.04), (12.28±1.61), (11.26±1.27) and(10.61±1.93) points higher than those of the control group for (7.23±1.16), (9.21±0.59), (8.45±0.88) and (8.12±1.16)points, the differences were statistically significant (P<0.05).After intervention, the overall health score of the observation group was (76.45±7.32) points, higher than that of the control group for (72.38±7.59) points, the difference was statistically significant (P<0.05).Conclusion Rosental effect nursing intervention can improve the psychological state,self-management ability and quality of life of elderly patients undergoing THA with definite effect.

Objective To explore the application of psychological intervention nursing in recovery period of schizophrenia patients.Methods Fifty patients with recovery period of schizophrenia admitted to our hospital from March 2016 to March 2018 were selected as study subjects.They were divided into observation group and control group according to the two-color ball drawing method, 25 cases in each group.The control group implemented general nursing measures and the observation group implemented psychological nursing intervention.The rehabilitation effect scores, disease symptoms, total compliance, and life hope scores were compared between two groups.Results The scores of the psychiatric rating scale (BPRS) and the rehabilitation efficacy rating scale (IPROS) after nursing in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05).The total compliance rate in the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05).There was no significant difference in the life expectancy scores between the two groups before nursing (P>0.05).The life hope scores of the two groups after nursing were higher than those before nursing, life expectancy score of the observation group after nursing was higher than that of the control group, and the differences were statistically significant (P<0.05).Conclusion Psychological nursing intervention for convalescent schizophrenia patients can improve their mental rehabilitation effect, ensure their psychological and emotional state to be significantly improved, and then improve their treatment compliance, which is worthy of wide clinical use.

225 Application effect of predictive rewarming nursing in patients with multiple trauma in emergency department

Objective To investigate the application effect of predictive rewarming nursing in patients with multiple trauma in emergency department.Methods A total of 205 patients with multiple trauma patients who were admitted to our hospital from January 2016 to December 2018 were selected as study subjects, the patients were divided into the experimental group and the control group according to the random number table method, 100 in the experimental group and 105 in the control group.The patients in the control group were given routine emergency nursing, and the experimental group was given predictive rewarming nursing on the basis of the control group.The changes of body temperature before and after admission and 1 hour after admission, rescue success rate, chills, arrhythmia rate and spontaneous hypothermia, spontaneous hypothermia duration and hypothermia time cumulative value between the two groups were compared.Results The body temperature and rescue success rate of the experimental group admission and 1 hour after admission were higher than those of the control group, the difference was statistically significant (P<0.05),the incidence of chills, arrhythmia and spontaneous hypothermia in the experimental group was lower than that in the control group (P<0.05), the duration of spontaneous hypothermia in the experimental group was shorter than that in the control group, and the cumulative value of hypothermia was smaller than that in the control group, and the differences were statistically significant(P<0.05).Conclusion Predictive rewarming therapy can effectively strengthen the emergency rescue process, reduce the incidence of spontaneous hypothermia in patients with multiple trauma, and improve the success rate of rescue.

228 Application effect of standardized emergency nursing process in patients with acute ST-segment elevation myocardial infarction

Objective To explore the clinical effect of standardized emergency nursing process in patients with acute ST-segment elevation myocardial infarction (STEMI) and its effect on the incidence of cardiac adverse events.Methods A total of 180 acute STEMI patients in our hospital from January to May 2019 were selected as the research subjects.They were divided into the control group (n=89) and the study group (n=91) by random number table method.After hospitalization, patients in the control group received regular emergency treatment, and in the study group, participants received standardized emergency nursing process.The opening time of the venous channel, the time of the initial diagnosis, the time of the first balloon dilation, the total hospital stay, the decrease in ST segment, the satisfaction of patients and their families, and the total incidence of cardiac adverse reactions were compared between two groups.Results The opening time, initial diagnosis time, first balloon dilation time, and total hospital stay of the study group were shorter than those of the control group after admission, and the differences were statistically significant (P<0.05).The satisfaction of patients and their families was higher than that of the control group, and the difference was statistically significant (P<0.05).The mortality and total adverse reaction rate in the study group were lower than those of the control group with statistical differences (P<0.05).There was no significant difference in heart rate failure and myocardial reinfarction rate (P>0.05).Conclusion Immediate application of the standardized emergency nursing process for patients with acute STEMI after admission can obtain a favorable therapeutic effect and patient′s satisfaction, and reduce the incidence of cardiac adverse reactions.

231 Application effect of comprehensive nursing of traditional Chinese medicine in patients with cervical spondylosis

Objective To explore the application value of comprehensive nursing of traditional Chinese medicine in patients with cervical spondylosis.Methods A total of 60 patients with cervical spondylosis admitted to our hospital from January 2018 to June 2019 were selected as the research objects, the patients were divided into control group (n=30) and observation group (n=30) by random number table method.The patients in the control group received routine nursing, while the patients in the observation group received comprehensive nursing of traditional Chinese medicine for 2 weeks.The activity of cervical vertebrae, visual analogue scale (VAS), Pittsburgh sleep quality index (PSQI), serum endothelin (ET) and prostacyclin (PGI2) were compared between the two groups.Results After 2 weeks of nursing, the cervical flexion activity, right flexion activity and left flexion activity in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05).After 2 weeks of nursing, the VAS and PSQI scores of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05).After 2 weeks of nursing, the scores of self-rating anxiety scale (SAS) and self-rating depression scale(SDS) in the observation group were lower than those in the control group, the differences were statistically significant(P<0.05).Conclusion Comprehensive nursing of traditional Chinese medicine can improve cervical spondylosis patients cervical range of motion, reduce pain, improve sleep quality, and reduce bad mood.

234 Application effect of self-management theory based on individual and family in leukemia patients

Objective To explore the application effect of self-management theory based on individual and family in leukemia patients.Methods A total of 102 leukemia patients who were treated in our hospital from January 2015 to December 2018 were selected as study subjects, and divided into the observation group and the control group according to random number table method, 51 cases in each group.All patients were treated with chemotherapy.The control group was given routine nursing on the basis of chemotherapy.The observation group was given intervention based on personal and family self-management theory on the basis of the control group.The quality of life scale (SF-36) and self-perceived burden scale (SPBS) of the two groups were compared before and after 3 months of nursing, and the self-management effect (self-management behavior scale, emotion regulation self-efficacy scale [SRESE]) of the intervention was recorded.Results There was no significant difference between the two groups in the quality of life score before nursing (P>0.05); the quality of life scores after nursing in the two groups were higher than those before nursing(P<0.05), and the quality of life scores after nursing in the observation group were higher than those in the control group the differences were statistically significant (P<0.05).After nursing, the mild rate of SPB in the observation group was 84.3%, higher than that in the control group for 64.7% (P<0.05); after 3 months of nursing, the level of each index of self-management behavior and the total score of scores in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05).Conclusion Self-management theory based on individual and family can obviously improve the self-care ability, management effect and life quality of leukemia patients.

238 Application effect of nursing based on rapid rehabilitation concept in perioperative management of intracranial aneurysm rupture

Objective To explore the application effect of nursing based on rapid rehabilitation concept (FTS) in patients with intracranial aneurysm rupture.Methods Eighty patients with aneurysm ruptures performed with craniotomy from November 2016 to August 2019 in our hospital were selected as the research objects.They were divided into observation group (n=40) and control group (n=40) according to random number table method.In the observation group,nursing based on FTS was used, and in the control group, conventional nursing was used.Preoperative preparation time,postoperative hospital stay, and complications during postoperative hospitalization, such as postoperative infection, postoperative pulmonary infection, postoperative pain, and incidence of postoperative low intracranial pressure syndrome were compared between the two groups.Results The preparation time of observation group was shorter than that of the control group, and the postoperative hospital stay was shorter than that of the control group, the differences were statistically significant (P<0.05).Postoperative infection, postoperative pulmonary infection, postoperative pain, postoperative infection and the ratio of postoperative low intracranial pressure syndrome in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05).The rate of improvement and discharge in the observation group was 90.0%, higher than that in the control group accounting for 50.0%, the difference was statistically significant (P<0.05).Conclusion Nursing based on FTS in patients with intracranial aneurysm rupture can significantly shorten the preoperative preparation time and postoperative hospital stay, reduce postoperative complications, and improve the prognosis of patients.

241 Application effect of graded care+self-management in patients with rectal cancer

Objective To observe the application effect of graded nursing+self-management in patients with rectal cancer.Methods Sixty patients with rectal cancer admitted to our hospital from January 2014 to March 2018 were selected.All patients underwent surgical treatment.They were divided into control group and observation group according to the blind selection method, 30 cases in each group.The control group received routine nursing.Based on this, the observation group adopted graded care+self-management, and self-care, fatigue and sleep quality were evaluated with self-care capacity scale (ESCA), cancer-related fatigue self-rating scale (PFS) and Pittsburgh sleep quality index scale(PSQI).Postoperative infection was counted.Results There were no significant differences in self-care ability score,PFS and PSQI scores between the two groups before treatment (P>0.05).After nursing, the self-care skills, self-care responsibility, health knowledge level and self-concept scores in the observation group were higher than those of the control group, the differences were statistically significant (P<0.05).The PFS and PSQI scores of the observation group were lower than those of the control group after treatment, the differences were statistically significant (P<0.05); the total postoperative infection rate of the observation group (6.67%) was lower than the control group (30.77%), and the difference was statistically significant (P<0.05).Conclusion The application of graded care+self-management in clinical nursing of patients with rectal cancer can effectively reduce the risk of infection, improve the quality of sleep, reduce the degree of fatigue and improve the self-protection ability of patients.

244 Application effect of continuous nursing on patients with lung cancer undergoing radical operation

Objective To explore the effect of continuous nursing on patients with lung cancer undergoing radical operation.Methods A total of 66 cases of patients with radical resection of lung cancer admitted to our hospital from December 2016 to December 2018 were selected as research objects.According to the random number table method, they were divided into the control group and the experimental group, 33 cases in each group.The control group was given routine nursing, and the experimental group was given continuous nursingnursing.The degree of cancer fatigue, psychological state, like self-rating depression scale (SDS), self-rating anxiety scale (SAS), symptom self-rating scale (SCL-90) scores and patients′ quality of life before and after nursing were compared between the two groups.Results The scores of cancer, psychological state and quality of life in the control group and experimental group were lower than those before nursing, with statistical differences (P<0.05).The scores of cancer, psychological state and quality of life in the experimental group were lower than those in the control group, with statistical differences (P<0.05).Conclusion The continuous nursing method can effectively improve the degree of cancer fatigue and improve the quality of life of patients after radical resection of lung cancer.

247 Application effect of hierarchical chain nursing in children with pneumonia

Objective To analyze the application effect of hierarchical chain nursing in pediatric pneumonia.Methods All 57 children with pneumonia admitted to our hospital from June to December 2017 were selected as control group,and 51 children with pneumonia admitted from January to July 2018 as experimental group.The control group received conventional nursing and the experimental group received hierarchical chainnursing.The effect of two groups of nursing mode on the pulmonary function and treatment compliance were compared.Results There were no significant differences in the 1s forced expiratory volume, forced vital capacity and 1s forced expiratory flow rate between the experimental group and the control group (P>0.05).After nursing, the 1 s forced expiratory volume, forced vital capacity and 1 s forced expiratory flow rate in the experimental group were (1.89±0.24), (2.21±0.23) L, (61.80±3.31) ml/s, respectively, which were higher than those in the control group for (1.47±0.22), (1.97±0.20) L, (54.22±3.14) ml/s, and the differences were statistically significant (P<0.05).The treatment compliance of the experimental group was 90.20%, higher than that of the control group (73.68 %), and the difference was statistically significant (P<0.05).The clinical cure rate of the observation group was higher than that of the control group and the difference was statistically significant (P<0.05).Conclusion The application of hierarchical chain nursing in pediatric pneumonia is superior to conventional care, can improve the lung function of children, improve the cure rate.

251 Application effect of traditional Chinese medicine appropriate nursing technique in patients with urinary retention after anorectal diseases surgery

Objective To explore the application effect of appropriate nursing techniques of traditional Chinese medicine in patients with urinary retention after anorectal diseases surgery.Methods Sixty patients with urinary retention after anorectal diseases surgery admitted to our hospital from October 2017 to November 2018 were retrospective analyzed.According to the difference of nursing methods, they were divided into control group and study group, 30 cases in each group.The control group took traditional nursing.The research group took appropriate nursing techniques of traditional Chinese medicine, and the nursing differences between the two groups were compared.Results The total effective rate of the study group was 96.6%, higher than 76.6% of the control group, with statistical difference (P<0.05).The residual urine volume of the patients in the study group after treatment was less than that of the control group, and the self-urination time was shorter than that of the control group, with statistical differences (P<0.05).Conclusion Appropriate nursing techniques of traditional Chinese medicine has significant clinical effect in patients with urinary retention after anorectal diseases surgery, can effectively improve urination of patients, and should be popularized in clinical practice.
