4 Research the chemical composition of Ixeris sonchifolia（Bge.）Hance

Objective To research the chemical composition of one traditional Chinese medicine called Ixeris sonchifolia （Bge.）Hance.Methods Macroporous adsorption resin，polyamide column chromatography，silica column chromatography，sephadex LH-20 column chromatography，ODS column chromatography and preparative liquid chromatograph were used to separation and purification.Structure identification was conducted according to physical and chemical properties and spectroscopy of compounds.Results 8 compounds separated from the 70%ethanol extract of Ixeris sonchifolia（Bge.）Hance and were respectively reseda-7-diglucoside gentianae，luteolin-7-O-β-D-glucopyranosyl-（1→2）-β-D-glucopyranoside，5，7-2 hydroxy-4′-methoxy-flavonoids-7-glycosidic rutin，luteolin，mignonette element-7-O-β-D-pyran glycosidase，mignonette，7-O-β-D-pyran glucuronic acid glycosides，apigenin and 7β-hydroxy-beta sitosterol.Conclusion Compounds reseda-7-diglucoside gentianae，mignonette，7-O-β-D-pyran glucuronic acid glycosides and 7β-hydroxy-beta sitosterol were the first isolation from Ixeris genus.

8 Study the changes of cellular immunity and humoral immune function in children with hand-foot-mouth disease in EV71

Objective To study the changes of cellular immunity and humoral immune function in children with handfoot-mouth disease in EV71.Methods 60 hand-foot-mouth disease children with EV71 infection who received diagnostic hospital from January 2014 to June 2015 were randomly selected and divided into light（group A）and heavy （group B）according to"Hand-foot-mouth disease diagnosis and treatment guidelines　（2010 version）"，and then 30 healthy children to the hospital for medical examination from January 2014 to June 2015 were randomly selected as control group（group C）.The three sets of blood lymphocytes（Th cells，Ts cells，B cells），expression the level of serum inflammatory mediators（TNF-α，IL-6，IL-10）expression levels，differences in serum immunoglobulins（IgA，IgG，IgM）and complement levels（C3，C4）skill were observed.Results①Blood lymphocyte expression levels：the Th cells，Ts cells express terms in group A and group B was significantly lower than that of group C，in terms of expressing B cells was higher than that of group C，the difference was statistically significant（P＜0.05）.②The expression level of serum inflammatory mediators：the expression levels of TNF-α，IL-6，IL-10 in group A and group B was significantly higher than that of group C respectively，and group B was significantly higher than that of group A（P＜0.05）.③The level of serum immunoglobulin and complement levels：the level of IgA，IgG in group A and group B was higher than that in group C，and group B was significantly higher than that of group A（P＜0.05）.The difference of the terms IgM among the three groups was not statistically significant.The levels of C3，C4 in group A and group B was significantly higher than that of group C，and group B was significantly higher than that in group A（P＜0.05）.Conclusion EV71 hand-foot-mouth disease infection child presenting cell and humoral immune function abnormalities，and have some correlation with children′condition to detect serum markers associated help improve the quality of clinical prognosis.

11 Progress dietary intervention of nutritional disorders in patients with nasopharyngeal carcinoma during radiotherapy and chemotherapy

In the procedure of clinical treatment，radiotherapy and chemotherapy can lead to various complications of patients，and malnutrition is one of the complications.This paper reviewed the dietary intervention for the symptoms associated with nutritional disorders including gastrointestinal reaction，electrolyte disorder，oropharyngeal pain，weight losing and hypoproteinemia during radiotherapy and chemotherapy of patients with nasopharyngeal carcinoma；cleared the importance and necessity of dietary nursing for patients with nasopharyngeal carcinoma during radiotherapy and chemotherapy；and provided the evidence to apply dietary intervention to improving nutritional disorders and promoting patient recovery.

14 The clinical application of ibutilide in atrial fibrillation and flutter

Ibutilide is a new type of classⅢ anti-arrhythmic drugs，mainly by extending the action potential to work. Ibutilide can be applied to the new-onset atrial fibrillation and atrial flutter.It can be also applied to atrial fibrillation and atrial flutter after radiofrequency ablation or after cardiac surgery.Compared with other antiarrhythmic drugs it has quick effect and high curative effect.The most serious adverse reaction of ibutilideis cutting-edge reverse type of ventricular tachycardia.It can be reduced by giving magnesium sulfate or other measures.

17 Clinical effects of Harmonic Scalpel used in elderly thyroid surgery

Objective To study the clinical effects of harmonic scalpel used in elderly thyroid surgery，aimed at improving the therapeutic effect.Methods 84 elderly thyroid surgery patients treated in our hospital from June 2011 to June 2015 were randomly selected as subjects.All patients were randomly divided into scalpel group an control group according to treatment mode，42 in each group.Harmonic scalpel used in the scalpel group and control group treated with conventional surgery.The surgery status and recurrent laryngeal nerve complications in two groups were compared.Results The operation time，blood loss in harmonic scalpel group was（88.05±20.45）min and（23.05±5.95）ml respectively，significantly lower than those of（123.55±30.90）min and（42.52±9.87）ml in the control group，the differences between the two groups were statistically significant（P＜0.05），and the difference in postoperative drainage between two groups was not statistically significant（P＞0.05）.The incidence of permanent recurrent laryngeal nerve injury and transient recurrent laryngeal nerve injury in harmonic scalpel group was 2.38%，significantly lower than 7.14%in the control group.And the difference of overall complications incidence was not statistically significant（χ2=1.816，P=0.178）.Conclusions Harmonic scalpel used in elderly thyroid surgery can greatly shorten the operation time and has better surgical field.It can protect the recurrent laryngeal nerve.

20 Clinical curative effect of laser occlusion combined with atherectomy on the treatment of lower extremity varicose veins

Objective To discuss the clinical curative effect of laser occlusion combined with atherectomy on the treatment of lower extremity varicose veins.Methods 80 cases with lower extremity varicose veins，which admitted in our hospital during January 2013 to June 2016，were selected as the research object.All patients were diagnosed for lower extremity varicose veins by ultrasound and clinical examination and divided into the observation group and the control group according to the type of surgery.35 cases in the control group were given the traditional great saphenous vein high ligation，and 45 cases in the observation group were given the laser occlusion combined with atherectomy treatment.Treatment effect and index such as operation time，intraoperative blood loss and postoperative complications of the two groups were compared.Results The operation time，intraoperative blood loss，hospital stay，subcutaneous ecchymosis absorption time of the observation group was significantly shorter than that of control group，the difference was significant statistically significant（P＜0.05）.Incidence of postoperative complications of observation group was obviously lower than that of control group，and 6 months follow-up limb varicose recurrence rate of the observation group was significantly lower than that of control group，the difference was significant statistically significant（P＜0.05）.Conclusion Laser occlusion combined with atherectomy on the treatment of lower extremity varicose veins has exact curative effect，less iatrogenic trauma，postoperative recovery fast，small incision，less postoperative scar left and the operation security is relatively high.Compared with the traditional great saphenous vein ligation has obvious advantages，which has clinical application and popularization value.

23 The feasibility study of vacuum sealing drainage and skin grafting bioremediation deep burn wounds

Objective To investigate the feasibility of vacuum sealing drainage（VSD）and skin grafting bioremediation deep burn wounds provides a new approach for the clinical treatment of deep burns.Methods From February 2013 to February 2015，74 cases patients with deep burn in our hospital burn unit were studied.The treatment method based on the patient′s willingness were divided into two groups.The observation group（38 cases）adopted VSD technology and combined with skin grafting for treatment；After skin graft the control group（36 cases）were used a general dressing make the wounds closed.After treatment，analyzed the therapeutic effect and positive rate of the two groups and incidence of adverse reactions.The results observation group　（number of dressing changes and healing time and healing rate）was significantly better than that of control group（P＜0.05）；The pain score of observation group（4.15±1.37）scores was significantly lower than that of control group（6.25±2.19）scores.The data were statistically significant（P＜0.05）；The pain level good rate of observation group（78.9%）was significantly higher（61.1%）.The difference was statistically significant（P＜0.05）；After treatment observation group positive rate（7.89%）was significantly lower than that of control group patients（41.67%）.Compared the two groups have a more significant difference（χ2=11.4543，P=0.0007）；After treatment complication rate（7.89% vs 44.44%）.The results of the two groups no significant difference（χ2=12.9410，P=0.0003）.Conclusion Repair deep burn wounds taking method of VSD technology and skin grafting combined makes a more significant effect than the traditional skin grafting using the general dressing wound closure，which can reduce the patient′s pain time，dressing and shorten healing time and higher graft survival.It is well worth broad application in clinical treatment.

26 Observation on herpes zoster neuralgia after cryotherapy in patients with herpes zoster combined with HIV infection

Objective To observe the clinical effect and safety of liquid nitrogen cryotherapy in treatment of herpes zoster neuralgia in patients with herpes zoster combined with HIV infection.Methods 50 patients with herpes zoster combined with HIV infection treated in our hospital from March 2012 to December 2015 were selected and randomly divided into cryotherapy group and control group.Patients in two groups were given antiviral，analgesic，and neurotrophic basic therapy，and patients in the cryotherapy group were additionally given liquid nitrogen cryotherapy，once daily for two continuous days.The effects were analyzed at the 7thand 14thday of treatment.Results The effect was more favorable in the cryotherapy group than in the control group，with significant difference（P＜0.05）.Conclusion Liquid nitrogen cryotherapy has favorable effect and low incidence of adverse reactions in treatment of patients with herpes zoster combined with HIV infection，which is worthy to be promoted.

29 Impact of lateral position mechanical ventilation on blood oxygen and circulation of chest trauma patients with acute respiratory failure

Objective To explore the impact of lateral position mechanical ventilation on blood oxygen and circulation of chest trauma patients with acute respiratory failure.Methods 20 chest trauma patients with acute respiratory failure were conducted lateral position mechanical ventilation from January 2014 to October 2015.Arterial blood gas，cycle indicator changes liked HR，MAP，CVP，CO，CI，SV and SVV before lateral position，1 h after lateral position and 1 h after recovery lateral position were tested.Results The arterial blood PaCO2had no obvious changes after change position.1 h after lateral position，the PaO2and PaO2/FiO2obvious increased than before lateral position　（P＜0.05）.1 h after recovery lateral position，the PaO2and PaO2/FiO2still obvious increased　（P＜0.05）；Changed position had no impact on HR，MBP，CVP，CO，CI，SV and SVV　（P＞0.05）.Conclusion Lateral position mechanical ventilation can improve the oxygenation status of chest trauma patients with acute respiratory failure，but have any obvious impact on hemodynamics.

32 Clinical value of end-stage liver disease model combined with MELD-Na in prevention short-term prognosis in patients with decompensated liver cirrhosis

Objective To explore the clinical value of end-stage liver disease model combined with MELD-Na in prevention short-term prognosis in patients with decompensated liver cirrhosis.Methods 152 patients with decompensated liver cirrhosis from January 2012 to January 2015 of digestive system department in our hospital were selected and followed-up for 3 months.According to follow-up results，death group was 43 cases and surviving group was 109 cases.The first value of CTP，MELD and MELD-Na were respectively counted after admission.Receiver operating characteristic curve　（ROC）was used to further evaluate three scoring systems on the 3 months later predictive value of decompensated liver cirrhosis patients.Results Among 152 decompensated liver cirrhosis patients，43 death within 3 months followed-up.The CTP，MELD and MELD-Na score in surviving group were respectively　（10.93±2.72），（20.99±5.62）and （24.67±4.89），which were respectively（10.93±2.72），（20.99±5.62）and（24.67±4.89）in death group，with significant difference（P＜0.05）.In judging the prognosis of patients with ROC curve AUC in comparison，the MELD-Na score（0.876）＞MELD score（0.840）＞CTP（0.754，P＜0.05）.Conclusion All of CTP scoring，MELD scoring and MELD-Na scoring can be used to evaluate the short-term prognosis in patients with decompensated liver cirrhosis，while MELD-Na model scoring can more accuracy reflect the severity of the patients and has more application value in clinical.

35 Relationship between lower extremity vascular disease and blood uric acid level of type 2 diabetic patients

Objective To explore the relationship between lower extremity vascular disease and serum uric acid level of type 2 diabetic patients.Methods 384 type 2 diabetic patients from July 2013 to July 2014 of our hospital were selected.Patients were divided into experimental group　（with lower extremity vascular disease，76 cases）and control group （without lower extremity vascular disease，308 cases）.Patients in control group were given routine oral drug or insulin，and patients in experimental group were given alprostadil injection to improve lower extremity vascular disease on the basis of control group.The difference of blood uric acid level in two groups was compared.The relationship between blood uric acid level and other clinical index was tested by Spearman test.The related factors between blood uric acid level and lower extremity vascular disease of type 2 diabetic patients was analyzed by Logistic regression analysis.Results The blood uric acid level in experimental group was obvious higher than that of control group with statistical difference（P＜0.05）.The Spearman test results showed that blood uric acid level had positive relationship with BMI，SBP，DBP，TC and TG of type 2 diabetic patients，while had negative relationship with HDL-C（P＜0.05）.Logistic regression analysis results showed that blood uric acid，TC，TG and CRP were dependent related with lower extremity vascular disease of type 2 diabetic patients　（P＜0.05）.Conclusion Blood uric acid level has close relationship with lower extremity vascular disease of type 2 diabetic patients，which is the risk factor of occur of lower extremity vascular disease.In the treatment of diabetes，the level of blood uric acid should be strictly controlled.

38 Clinical effects observation of pioglitazone，sitagliptin and metformin combination in treating type 2 diabetes and obesity patients with poor glycemic control

Objective To explore the clinical effects of pioglitazone，sitagliptin combined with metformin in treating type 2 diabetes and obesity patients with poor glycemic control.Methods 100 type 2 diabetes and obesity patients with poor glycemic control from June 2013 to June 2014 of our hospital were selected.Patients were divided into study group and control group according to random number table，and each group was 50 cases.Patients in study group were used pioglitazone，sitagliptin and metformin.Patients in control group were only used pioglitazone and metformin.All patients in two groups were treated for 3 months.The clinical index of glycated glycosylated hemoglobin（HbA1c），2 hour postprandial blood glucose（2 hPG），fasting blood glucose（FPG）and body mass index（BMI）in two groups before and after treatment were compared.After treatment，the level of fasting insulin（FINS），fasting c-peptide（CP0），postprandial 2 h insulin（2 h/INS）and postprandial 2 h C peptide（CP2）in two groups were compared.The rate of adverse effects in two groups were compared.Results After treatment，the HbA1c，2 hPG，FPG and BMI in two groups improved；and the HbA1c，2 hPG，FPG and BMI in study group was obvious better than of control group respectively after treatment（P＜0.05）.After treatment，the level of FINS，CP0，2 h/INS and CP2in study group was obvious better than that of control group respectively（P＜0.05）.The rate of adverse effects such as nausea，edema and headache compared with no statistical significance（P＞0.05）.Conclusion Conducting pioglitazone，sitagliptin and metformin combination therapy for type 2 diabetes and obesity patients with poor glycemic control can obviously control blood glucose level，decrease body′s resistance to insulin，control body weight and with definite clinical effects.It is worth to popularize in clinic.

41 Effect observation of modified radical mastectomy for breast cancer by preserving intercostobrachial nerve

Objective　To　study　the　effect of modified　radical mastectomy　for breast cancer by　preserving intercostobrachial nerve.Methods Clinical data was from 65 patients undergone modified radical mastectomy for breast cancer from March 2012 to October 2014 in our hospital.According to randomly sealed envelope，they were divided into two groups of preserved group and non-preserved group.In the non-preserved group，intercostobrachial nerve was removed，while in the preserved group，intercostobrachial nerve was kept.The operation time，intraoperative amount of bleeding，number of lymph node dissection，abnormal sensation of skin area，and incidence of postoperative complications in the two groups were compared.Besides，the difference on quality of life after surgery was compared.Results In the preserved group，the operation time，intraoperative amount of bleeding，and number of lymph node dissection were similar to those in the non-preserved group，the difference was not statistically significant（P＞0.05）.In the preserved group，abnormal sensation of skin area was smaller than that in the non-preserved group，the difference was statistically significant（P＜0.01）.The incidence of postoperative complications in the preserved group was lower than that in the non-preserved group，the difference was statistically significant（P＜0.01）.The quality of life after surgery was much better in the preserved group than that in the non-preserved group，the difference was statistically significant（P＜0.01）. Conclusion Effect of modified radical mastectomy for breast cancer by preserving intercostobrachial nerve is definite，which can effectively avoid the occurrence of skin sensory disturbance in armpit and upper arm，and is beneficial to reduce the postoperative complications and promote quality of life after surgery，and is worthy of promotion.

44 A comparative study of clinical effectiveness of application of complete laparoscopy and laparoscopy-assisted radical distal gastrectomy in treatment of gastric carcinoma

Objective To make a comparative analysis of the clinical effectiveness of application of complete laparoscopy and laparoscopy-assisted radical distal gastrectomy in treatment of gastric carcinoma.Methods Altogether 90 cases of gastric carcinoma patients who had been treated from January 2012 to January 2015 in the hospital were selected and randomly divided into Control Group and Observation Group with 45 cases in each group.Control Group were treated with laparoscopy-assisted radical distal gastrectomy；while，the patients in Observation Group were treated with complete laparoscopy.The intra-operation and post-operation indexes of patients in both groups were compared. Results Observation Group showed significantly less intra-operative blood loss［（188.03±23.45）ml vs（234.86±32.45）ml］and shorter incision［（3.46±0.97）cm vs（6.37±1.14）cm］than Control Group with statistically significant（P＜0.05）. There were no statistical significance（P＞0.05）between Observation Group and Control Group in the operation duration and number of lymph nodes.The hospitalization time［（10.03±2.24）d］，postoperative exsufflation time［（2.34±0.67）h］，the first time of getting out of bed after surgery［（1.24±0.32）d］of Observation Group were significantly shorter than those of Control Group　［（15.78±3.62）d，（4.37±0.97）h and　（2.89±0.43）d］.Conclusion As a safe and reliable surgical method with fewer traumas，the complete laparoscopy shows remarkably greater effectiveness than the laparoscopy-assisted radical distal gastrectomy in treatment of gastric carcinoma，which deserves wider clinical promotion.

47 Application　effect comparison　of improved　FOLFOX6　neoadjuvant chemotherapy and postoperative chemotherapy in treating progressive stomach cancer

Objective To analyze the effect of improved FOLFOX6 neoadjuvant chemotherapy and postoperative chemotherapy in progressive stomach cancer.Methods 46 progressive stomach cancer patients from November 2009 to December 2012 of our hospital were selected as study object for this study.46 progressive stomach cancer patients were random divided into control group and experimental group.23 progressive stomach cancer patients in control group were used improve FOLFOX6 postoperative chemotherapy and 23 progressive stomach cancer patients in experimental group were used improve FOLFOX6 neoadjuvant chemotherapy.The followed up of progressive stomach cancer patients in two groups were 3 years.The short term effect，three-year survival rate，time to tumor progression and the incidence rate of adverse effects of progressive stomach cancer patients in two groups were analyzed and compared.Results The incidence rate of adverse effects of progressive stomach cancer patients in control group was 47.83%，obvious higher than that was 21.74%in experimental group，and the short term effect，three-year survival rate，time to tumor progression of progressive stomach cancer patients between two groups had statistical significance　（P＜0.05）.Conclusion Using improved FOLFOX6 neoadjuvant chemotherapy for progressive stomach cancer patients can enhance the survival time and quality of patients effectively，and with little adverse effects.

50 Clinical application of fast track theory during perioperative period of colorectal cancer

Objective To discuss the clinical application of fast track theory during perioperative period of colorectal cancer.Methods 110 cases of colorectal cancer patients from June 2012 to July 2015 in our hospital were chosen as the research object，and were randomly divided into two groups.Patients in the observation group were given fast track treatment，while patients in the control group were given conventional perioperative therapy.The indicators including operation time，blood loss during operation，time to first exhaust，and time to getting out of bed were recorded.And incidence rates of complications such as intestinal obstruction，incision infection，and pulmonary infection were compared between two groups.Results In the observation group，the blood loss during operation was（78.3±48.7）ml，and the length of incision was（10.2.±1.3）cm，which were significantly superior to those in the control group［（154.3±32.6）ml，and（12.3±1.5）cm］（t=9.6176，t=7.8461，P＜0.01）.There was no significant difference in operation time between two groups（t=1.2239，P＞0.05）.In the observation group，the time to first exhaust was（33.6±1.1）h，the time to first defecation was（55.6±11.2）h，and the time to first feeding was（10.5±0.1）h，the bed time was［（1.6±0.2）d］，the hospital stay after operation was（5.5± 1.5）d，which were significantly shorter than those in the control group　［（38.3±0.3），（69.7±12.4），（15.6±1.5）h，（3.8±0.8），（10.3±2.5）d］（P＜0.01）.There was no death in both groups after operation.There were significant differences in incidence rates of intestinal obstruction，incision infection，and pulmonary infection between two groups（χ2=4.7673，P＜0.05）.Conclusion The fast track theory has significant effects when applied during perioperative period of colorectal cancer，which can reduce the incidence rate of complications and accelerate the recovery of patients.

Objective To explore the change of blood lipid and blood glucose level in macrosomia born by women with gestational diabetes.Methods 42 cases of macrosomia born by women diagnosed with gestational diabetes in our hospital from September 2013 to September 2014 were selected as the observation group.At the same time，another 40 cases of macrosomia born by healthy women were selected as the control group.40 full-term newborns who had normal weight （2.5-4.0 kg）and were born by healthy pregnant women were selected as the health group.The level of fasting blood glucose，triglyceride　（TG），low density lipoprotein　（LDL），high density lipoprotein　（HDL），apolipoprotein A　（ApoA）and apolipoprotein B　（ApoB）in pregnant women and newborns were determined.The BMI before pregnancy and degree of weight gain during pregnancy of pregnant women in the observation group and the control group were statistically analyzed.Results There was no statistical difference in the level of LDL and HDL between pregnant women in the observation group and the healthy group（P＞0.05）.The level of fasting blood glucose and TG of pregnant women in the observation group was higher than that in the healthy group，with significant difference（P＜0.05）.There was no statistical difference in gestational weeks between pregnant women in the observation group and the control group（P＞0.05）.The value of BMI before pregnancy and degree of weight gain during pregnancy of pregnant women in the observation group was higher than that in the control group，with significant difference　（P＜0.05）.There were no statistical difference in LDL and HDL level between newborns in the observation group and in the control group　（P＞0.05）.The level of TG of newborns in the observation group was higher than that in the control group，the level of ApoA，ApoB and blood glucose of newborns in the observation group was lower than that in the control group，with significant difference（P＜0.05）.There were no statistical difference in LDL and HDL level between newborns in the observation group and in the healthy group（P＞0.05）.The level of TG of newborns in the observation group was higher than that in the healthy group，the level of ApoA，ApoB and blood glucose of newborns in the observation group was lower than that in the healthy group，with significant difference （P＜0.05）.Conclusion The blood lipid and blood glucose level in macrosomia who are born by women with gestational diabetes are relatively higher，which increase the incidence rate of complication like low blood glucose in newborns.

57 Clinical discussion of pregnancy combining with thrombocytopenia in 60 cases

Objective To explore the main cause of gestational thrombocytopenia and clinical treatment method during delivery.Methods Clinical data from 60 gestational patients with thrombocytopenia in our department of obstetrics from January 2008 to December 2013 were retrospectively analyzed.The medical history from each patient was sumed up，and all patients were performed with physical examination，laboratory test，and B ultrasound examination.The main reasons leading to gestational thrombocytopenia were summarized and treatment method in patients with gestational thrombocytopenia during childbirth was concluded.Results The incidence rate of gestational thrombocytopenia and idiopathic thrombocytopenic purpura was higher than that of thrombocytopenia caused by other causes，with significant difference （P＜0.05）.After providing with glucocorticoid and platelet preparations for treatment，there was no significant difference in the incidence rate of complication between vaginal delivery and caesarean section（P＞0.05）.One preterm neonate was dead，and the rest were survived.Conclusion The gestational thrombocytopenia and idiopathic thrombocytopenic purpura are the major factors causing thrombocytopenia in pregnancy，glucocorticoid and platelet preparations can be selected to improve the condition of platelets in clinic.

60 Clinical research of post abortion care patients selecting contraception measure

Objective To explore the present situation of post abortion care（PAC）patients selecting contraception measure，and to optimize the PAC service process.Methods 1013 gestational age women because of unintended pregnancy and voluntary conducted artificial abortion to terminate pregnancy in our hospital from January 2013 to February 2014 were selected as the research group，one of contraceptive measure was voluntary selected after PAC service，all patients were divided into four groups according to the results.Group A was given subcutaneous embedding method，and 29 cases was immediately used.Group B was given oral short acting contraceptives，and 717 cases was immediately used.Group C was given intrauterine device，and 201 cases was immediately used.group D was given condom，and 66 cases was immediately used.Constituent ratio changes in 2 years in the four groups were analyzed.The repregnancy rate，the incidence rate of adverse effect，premature termination rate and satisfaction rate in the four groups after contraception was compared.Results After PAC service，the immediate use rate in group A was 2.9%，and was 5.9%after 2 years later.The immediate use rate in group B was 70.8%，and was 19.5%after 2 years later.The immediate use rate in group C was 19.5%，and was 33.4%after 2 years later.The immediate use rate in group D was 6.5%，and was 41.3%after 2 years later.The incidence rate of adverse reaction in group A was the highest，and the satisfaction rate was the lowest.The premature termination rate in group B was the highest.The satisfaction rate in group C was the highest and the repregnancy rate in group D was the highest，and there was significant difference among the four groups（P＜0.05）.Conclusion After PAC services，the patients can voluntarily choose effective contraceptive measure.Subcutaneous embedding has the disadvantages of low awareness and satisfaction.Three months after PAC，condom is still the first choice for most patients，but with the disadvantage of higher unintended pregnancy rate.PAC follow up process should be optimized.

63 Clinical effect of laparoscopic ovarian benign tumor resection

Objective To explore the clinical application effect of laparoscopic ovarian benign tumor resection in women ovarian benign tumor.Methods 145 ovarian benign tumor patients from August 2014 to October 2015 in our hospital were selected as the observation group.120 ovarian benign tumor patients from August 2013 to June 2014 in our hospital were selected as the control group.The control group was treated with traditional open way for removing ovarian tumor，the observation group was treated with laparoscopic ovarian benign tumor resection.The treatment effect，operation time，the amount of bleeding，postoperative exhaust time，recovery time of bowel function，hospitalization time，the incidence rate of postoperative fever and complication in the two groups was compared respectively.Results The significant efficiency and total effective rate in the observation group was higher than that in the control group，with significant difference（P＜0.05）.The amount of bleeding in the observation group was less than that in the control group，the postoperative exhaust time，recovery time of bowel function and hospitalization time in the observation group was shorter than that in the control group，with significant difference　（P＜0.05）.The postoperative fever rate of the observation group was 1.38%，which was lower than 6.67%of the control group，with significant difference（P＜0.05）.There was no complication liked pelvic effusion and adhesion occurred in the observation group at 1，3，6 months after operation，but 6 cases occurred bladder adhesion in the control group and all of them needed operation.Conclusion Laparoscopic ovarian benign tumor resection has better treatment effect，and late recovery effect is better.The incidence rate of complication is low，and with good safety，it is worthy of clinical promotion and application.

66 Clinical feature and pregnancy outcome of premature rupture of the membrane of single fetal breech pregnancy

Objective To explore the clinical feature and pregnancy outcome of premature rupture of the membrane of single fetal breech pregnancy.Methods 60 puerperae of premature rupture of the membrane of single fetal breech pregnancy from January 2014 to December 2015 in our hospital were selected as the study group，and 60 puerperae of premature rupture of the membrane of single fetal head position pregnancy at the same period in our hospital were selected as the control group.The indicators of the pregnant week of pregnancy termination and premature rupture of membrane time，premature birth rate，pregnancy outcome and so on in the two groups was compared.Results The time of premature rupture of membrane in the study group was（33.8±2.3）pregnant weeks，which was shorter than　（35.4±1.7）pregnant weeks in the control group，with significant difference（P＜0.05）.Irregular contraction in the study group before premature rupture of the membrane was 21 cases（35%），which was higher than 9 cases（15%）in the control group，with significant difference　（P＜0.05）.The proportion of dexamethasone promoting fetal lung maturity time more than 48 hours in the study group was higher than that in the control group，with significant difference（P＜0.05）.The proportion of using tocolysis in the study group was higher than that in the control group，with significant difference（P＜0.05）.The rate of premature birth，cesarean section，neonatal asphyxia in the study group was higher than that in the control group，with significant difference（P＜0.05）.Neonatal weight in the study group was lower than that in the control group，with significant difference（P＜0.05）.The incidence rate of hypamnion in the study group was higher than that in the control group，with significant difference　（P＜0.05）.There was no significant difference in the incidence rate of chorioamnionitis between the two groups　（P＞0.05）.Conclusion The incidence rate of preterm，fetal distress and neonatal asphyxia，hypamnion in the breech pregnancy with premature rupture of the membrane is higher，which can increase the rate of cesarean section and neonatal prognosis is poor，it should be positive prevention and treatment.

69 Clinical effect of tonsillectomy combined with adenoidectomy under nasal endoscope in the treatment of children with snoring

Objective To explore the clinical effect of tonsillectomy combined with adenoidectomy under nasal endoscope in the treatment of children with snoring.Methods 100 children with snoring from January 2012 to December 2015 in our hospital were selected and randomly divided into the control group and the observation group，50 cases in each group.The control group was given tonsillectomy combined with adenoidectomy，the observation group was given tonsillectomy combined with adenoidectomy under nasal endoscope.The operation time，the amount of bleeding during operation，the effective rate of treatment and hospitalization time was compared between the two groups.Results The effective rate in the observation group was 96.00%，which was higher than 80.00% in the control group，with significant difference　（P＜0.05）.The average operation time and hospitalization time of the observation group was shorter than that of the control group，the average amount of bleeding during operation in the observation group was less than that in the control group，with significant difference（P＜0.05）.Conclusion Using tonsillectomy combined with adenoidectomy under nasal endoscope in the treatment of children with snoring can get more ideal effect.

72 Application effect of laparoscopic total hysterectomy surgery in the patients with previous history of abdominal surgery

Objective To explore the application effect of laparoscopic total hysterectomy surgery in the patients with previous history of abdominal surgery.Methods 61 patients with previous history of abdominal surgery underwent total hysterectomy surgery from January 2011 to March 2016 in our hospital were selected and divided into the laparotomy group（31 cases）and the laparoscopic group（30 cases）according to the different operation methods.The incidence rate of postoperative complication，intraoperative and postoperative related indicators，the incision healing rate of grade A in the two groups was compared.Results One case of laparoscopic group transferred to laparotomy due to the dense bowel adhesion，the rest were completed under laparoscopy.There was one case of bladder injury in the laparoscopic group，three cases in the laparotomy group.There was no bowel injury and ureteral injury in the both groups.The operation time of the laparoscopy group was shorter than that of the laparotomy group，the amount of bleeding in the laparoscopy group was less than that in the laparotomy group，the temperature within 72 hours in the laparoscopy group was lower than that in the laparotomy group，with significant difference（P＜0.05）.Postoperative recovery time of spontaneous activity and hospital stay in the laparoscopic group was shorter than that in the laparotomy group，with significant difference　（P＜0.05）.The incision healing rate of grade A in the laparoscopic group was 100.0%，which was higher than 80.6%in the laparotomy group，with significant difference　（P＜0.05）.Conclusion Laparoscopic total hysterectomy surgery can be done in the patients with previous history of abdominal surgery.The operation time，postoperative recovery time，hospital stay are shortened，abdominal incision healed well，and without increasing operative risk，the safety is good，it is worthy of clinical promotion and application.

75 Clinical study of dynamic blood gas monitoring on pediatric continuous blood purification

Objective To explore the way of dynamic blood gas monitoring and its influence on pediatric continuous blood purification.Methods Clinical data from 40 cases of pediatric continuous blood purification in our hospital from August 2012 to October 2015 were retrospectively analyzed.The standardized and close observation on dynamic blood gas monitoring was given.The change of blood gas indexes before and after therapy was compared and adverse reaction was analyzed.Results The level of aspartate aminotransferase　（AST），alanine aminotransferase　（ALT），creatine kinase （CK-MB），total bilirubin（TBiL），creatinine（Cr），blood urea nitrogen（BUN），activated partial thromboplastin time（APTT），and ammonia　（NH3）after treatment for 24，72 hours and the end of treatment was lower than that before treatment，the level of AST，ALT，CK-MB，TBiL，Cr，BUN，APTT，and NH3in the end of treatment was lower than that after treatment for 24 hours，with significant difference（P＜0.05）.There were 4 cases with abnormal decrease of blood pressure in the process of blood purification，which were timely detected by close observation on dynamic blood gas monitoring and controlled after taking measures of elevating blood pressure，undesirable interruption of blood purification was avoided，and life safety and final comprehensive purified effect were ensured.Fluctuation trend of blood pressure in a small range was existed at an early stage in other participants and was risen and returned to normal via regulation on blood flow rate and ultrafiltration volume at a later stage.Conclusion Dynamic blood gas monitoring can be used as flexible and accurate analysis basis of vital signs and therapeutic evaluation basis.It is recommended to use for cooperation in pediatric continuous blood purification.

78 Electroencephalogram characteristics in 30 cases of children with sepsis associated encephalopathy and its value to the evaluation of early-stage condition

Objectives To explore the electroencephalogram（EEG）characteristics of children with sepsis associated encephalopathy and its value to the evaluation of early-stage condition.Methods 30 cases of children with sepsis associated encephalopathy in our hospital from January 2014 to October 2015 were monitored using video-electroencephalogram（VEEG）to study the children′s EEG characteristics.The multiple organ function of the enrolled children was evaluated，and the children with sepsis associated encephalopathy were divided into the early-stage group and the advanced-stage group.Statistical analysis was performed on the EEG manifestation of the two groups.Results The EEG manifestation of children with sepsis associated encephalopathy showed that 7 cases were normal，the basic rhythm in the background activity during the awaking period was slower than that of the matched children，11 cases had excessive θ-rhythm，8 cases had δ-rhythm，2 cases were suppression or burst-suppression，the physiological sleep architecture existed，the epileptic waves occurred occasionally during the sleep period with no nonconvulsive status epilepticus.The same EEG findings of 30 patients with early and late sepsis associated encephalopathy were statistically analyzed，the results showed that the occurrence rate of normal status and excessive δ-rhythm in the EEG characteristics of earlystage group was higher than that in the EEG characteristics of the advanced-stage group，the occurrence rate of θrhythm and suppression or burst-suppression in the EEG characteristics of the advanced-stage group was higher than that in the EEG characteristics of the early-stage group，with significant difference（P＜0.05）.Conclusion The EEG characteristics of children with sepsis associated encephalopathy exhibit no specificity.The EEG manifestations of sepsis associated encephalopathy in the early and the advanced stages show statistically significant difference.

81 Clinical effect of traditional Chinese medicine hot package combined with retention enema in the treatment of chronic pelvic inflammation

Objective To explore the clinical effect of traditional Chinese medicine　（TCM）hot package combined with retention enema in the treatment of chronic pelvic inflammation.Methods 85 patients with chronic pelvic inflammation in our hospital from January 2012 to February 2015 were selected and divided into the control group（42 cases）and the observation group（43 cases）according to the dates of admission.The control group was given conventional drug therapy，the observation group was given TCM hot package combined with retention enema on the basis of the control group.The treatment effect was compared between the two groups and the symptom score before and after treatment was assessed in the both groups.Results The total effective rate in the observation group was 95.3%，which was higher than 76.2%in the control group，with significant difference　（P＜0.05）.The symptom score in the two groups after treatment was lower than that before treatment，with significant difference　（P＜0.05）.The symptom score in the observation group after treatment was lower than that in the control group，with significant difference　（P＜0.05）.Conclusion Additional TCM hot package combined with retention enema has significant effect in the treatment of chronic pelvic inflammation and the symptoms are evidently relieved，it is worthy of clinical promotion and application.

Objective To explore the clinical efficacy of different vaginal surgery methods in the treatment of cesarean scar pregnancy with exogenous type.Methods 92 patients with cesarean scar pregnancy with exogenous type from June 2012 to June 2015 in our hospital were selected and randomly divided into the control group（30 cases），the experimental group A（31 cases）and the experimental group B（31 cases）.The control group was given laparoscopic surgery，the experimental group A was given vaginal surgery combined with methotrexate，the experimental group B was given simple vaginal surgery.The operative time，amount of bleeding，β-HCG to normal time，hospitalization expenses，the incidence rate of adverse reaction and complication was compared between the three groups.Results The operative time in the experimental group A and the experimental group B was shorter than that in the control group，the amount of bleeding in the experimental group A and the experimental group B was less than that in the control group，hospitalization expenses in the experimental group A and the experimental group B was lower than that in the control group，with significant difference（P＜0.05）.Hospitalization expenses in the experimental group B was lower than that in the experimental group A，with significant difference　（P＜0.05）.There was no significant difference in the β-hCG to normal time among the three groups　（P＞0.05）.The incidence rate of adverse reaction and complication in the experimental group B was lower than that in the control group and the experimental group A，with significant difference（P＜0.05）.Conclusion Simple vaginal surgery in the treatment of cesarean scar pregnancy with exogenous type has high efficacy，high security，small economic burden，it is worthy of clinical promotion and application.

Objective To explore the effect of methotrexate combined with hysteroscopic electric resection in the treatment of cesarean scar pregnancy.Methods 72 cases diagnosed as cesarean scar pregnancy from February 2010 to May 2014 in our hospital were selected and divided into the observation group and the control group according to the random number table method，with 36 cases in each group.The treatment group was given methotrexate combined with hysteroscopic electric resection，the control group was treated with methotrexate combined with curettage treatment.Surgery related condition and prognosis in the two groups was compared.Results The amount of bleeding in the observation group was less than that in the control group，the operation time，the recovery time and recovery time of serum β-HCG in the observation group was shorter than that in the control group，with significant difference　（P＜0.05）.The total incidence rate of complication in the observation group was lower than that in the control group，with significant difference （P＜0.05）.The pregnancy rate，the rate of vaginal delivery in the observation group was higher than that in the control group，with significant difference　（P＜0.05）.Conclusion The effect of methotrexate combined with hysteroscopic electric resection in the treatment of cesarean section scar pregnancy is significant，which can promote the recovery of patients，significantly reduce complication，with good prognosis，it is worthy of clinical promotion and application.

90 Related risk factor analysis on incidence of pharyngeal fistula after hypopharyngeal carcinoma operation

Objective To discuss the related risk factors of pharyngeal fistula after hypopharyngeal carcinoma operation. Methods 43 patients with hypopharyngeal carcinoma treated in our hospital from May 2006 to December 2015 were selected.The related factors of pharyngeal fistula after operation including T stage，N stage，preoperative radiotherapy，preoperative tracheotomy，laryngeal function preservation，time duration of operation，flap repairing，and local infection were analyzed by Logistic analysis and single factor analysis.Results Pharyngeal fistula was observed in 9 patients of the 43 patients，and the incidence rate was 20.93%.The result of single factor analysis showed that the incidence of pharyngeal fistula was significantly higher in patients with preoperative radiotherapy，preoperative tracheotomy，laryngeal function resection，local infection，longer operation time，and flap repairing（P=0.0276，0.0096，0.0276，0.0296，0.0073，0.0113）；the incidence rate was significantly lower in patients at T1+T2 stages than in those at T3+T4 stages（P=0.0096）；and there was no significant difference between patients at N0+N1 stages and patients at N2+N3 stages　（P=0.3826）.The result of Logistic analysis showed that T stage　（OR＝5.376，P＝0.047）and local infection　（OR＝16.651，P＝0.002）were independent risk factors of pharyngeal fistula after operation.Conclusion Based on assurance of therapeutic effect，risk factors should be prevented in preoperative preparation，operation process，and postoperative care.Measures should also be taken to manage risk factors and to further reduce the incidence of pharyngeal fistula after hypopharyngeal carcinoma operation.

Objective To explore the effect comparison between Hexuemingmu tablet and compound Danshen tablet in the treatment of mild and moderate non-proliferative diabetic retinopathy（NPDR）.Methods From March 2014 to March 2015，80 patients　（142 eyes）with mild and moderate NPDR admitted into our hospital were selected and divided into research group and control group by a random number table.In the research group　（n=40 with 70 eyes），compound Danshen tablet combined with regular hypolipidemic was selected.In the control group　（n=40 with 72 eyes），Hexuemingmu tablet combined with regular hypolipidemic was selected.The vision，retinopathy and changes of blood lipid were observed and the therapeutic effect was compared between two groups.Results There were statistical differences in number of microangioma，area of hemorrhagic focus and seepage focus before and after treatment in two groups（P＜0.05）.There were statistical differences in these three indexes after treatment between two groups（P＜0.05）. There was no statistical difference in mild and moderate NPDR vision between two groups before treatment（P＞0.05）；In the research group，both mild and moderate NPDR vision after treatment were superior to those in the control group with statistical differences　（P＜0.05）.Conclusion Compared with Hexuemingmu tablet，compound Danshen tablet can more effectively treat mild and moderate NPDR，improve retinal microcirculation，inhibit retinal neovascularization and greatly enhance vision，which is worthy of promotion and application in clinic.

96 Effects of nasolacrimal duct retrograde tube insertion and rhinodacryocystostomy in treating chronic dacryocystitis

Objective To explore the effects of nasolacrimal duct retrograde tube insertion and rhinodacryocystostomy in treating chronic dacryocystitis.Methods 80　（96 eyes）patients with chronic dacryocystitis from October 2012 to October 2014 in our hospital were selected.Patients were divided into control group（40 cases，49 eyes）and observation group（40 cases，47 eyes）according to different operation method.Patients in control group were treated with rhinodacryocystostomy and patients in observation group were given nasolacrimal duct retrograde tube insertion，the treatment effects in two groups were compared.Results The perioperative status of patients in observation group were obviously better than those in control group（t=15.22，37.70，10.76，P＜0.05）.12 months followed-up after surgery，the eyes of cure and effective and effective rate in control group were respectively 45 and 91.84%，which were respectively 45 and 95.74%in observation group，and the difference had no statistical significance　（χ2=0.63，P＞0.05）.The incidence rate of complications in control group and observation group was respectively 22.50%and 7.50%，the difference was statistical significance　（χ2=3.53，P＞0.05）.Conclusion Nasolacrimal duct retrograde tube insertion in treating chronic dacryocystitis with the advantages of simple operation，safety and effective，little complications，and is worthy to popularize in clinic.

99 Application effect of Zirconium dioxide full porcelain tooth on anterior teeth restoration

Objective To discuss the clinical effect of Zirconium dioxide full porcelain tooth on anterior teeth restoration.Methods From January 2013 to March 2015，in stomatology department of our hospital were required for the repair of anterior teeth patients，100 patients followed up successfully with restoration anterior teeth were randomly selected as the research object and divided into control group and observation group by taking computer random sampling method，each group had 50 cases.In the control group Ni Cr alloy porcelain crown were used to repair，while in the observation group Zirconium dioxide full porcelain tooth were used to repair.The repair effect of two groups were compared.Results Compared with the control group，the shape score and color score of dental prosthesis in the observation group were significantly better（P＜0.05）.In the observation group，the oral comfort and intact crowns was respectively 94%，86%，significantly higher than 78%，68%in the control group　（P＜0.05）.Compared with the control group，ratio of 0 level of gingival index in the observation group was significantly higher（P＜0.05）.Compared with the control group，satisfaction in the observation group was significantly higher（P＜0.05）.Conclusion Application of Zirconium dioxide full porcelain tooth in anterior teeth restoration，its aesthetic effect is significant and can effectively reduce the impact on gums.

101 Clinical application of nitrofurazone combined with Kangfuxin in patients with severe mouth ulcers

Objective To investigate the clinical efficacy of nitrofurazone combined with Kangfuxin in patients with severe mouth ulcers.Methods 90 patients of severe mouth ulcers treated from January 2006 to December 2015 in our department were selected，the patients were divided into two groups by random digits table，with 45 cases in each group. The control group was only given nitrofurazone mouthwash，the treatment group were treated combined with Kangfuxin on the basis of control group.The efficacy and the content of secretory immunoglobulin A（sIgA）and immunoglobulin G （IgG）were compared between two groups before and after treatment.Results The symptoms were significantly improved in the treatment group，the effective rate and the total effective rate in treatment group was 28.89%，91.11%respectively，with15.56%and 80.00%respectively in control group，the ulcer healing time in the treatment group was significantly shorter than that in the control group［（3.2±0.5）d vs（4.9±1.3）d］，there were statistically significant difference between two groups（P＜0.05）.The content of saliva sIgA in the treatment group after treatment 7 d was significantly higher than that in control group［（65.7±10.5）g/L vs（52.0±9.4）g/L］，and the content of IgG was significantly lower than that in the control group　［（23.5±5.5）g/L vs（35.3±7.5）g/L］，there was statistically significant difference between two groups　（P＜0.05）.Conclusion Nitrofurazone combined with Kangfuxin in treatment of patients with severe mouth ulcers has a good clinical efficacy，high security，can reduce patients′suffering and improvie patients′satisfaction and quality of care，it can be used widely in clinic.

104 Study on the levels of glutathione peroxidase in gingiva crevicular fluid and serum of patients with periodontitis

Objective To study the levels of glutathione peroxidase　（GSH-Px）in gingival fluid and serum of patients with periodontitis.Methods 41 cases of patients with chronic periodontitis in the active period and 27 cases in the diastasis period and 30 patients received the same period of periodontal health　（control group），who were treated in our hospital from January 2015 to February 2016 and selected as the research objects.Then，the levels of GSH-Px in gingival fluid and serum of three groups were compared，and correlation among GSH-Px levels and gingival index，periodontal pocket depth，attachment loss were statistically analyzed.Results The gingival fluid volume，gingival index，periodontal pocket depth and attachment loss of patients with chronic periodontitis in the active period and diastasis period were significantly higher than those in the control group（P＜0.05）；The gingival fluid volume，gingival index，periodontal pocket depth and attachment loss of patients with chronic periodontitis in the active period were significantly higher than those in the diastasis period（P＜0.05）.The levels of GSH-Px in gingival fluid and serum of patients with chronic periodontitis in the active period and diastasis period were significantly higher than those in the control group　（P＜0.05）；The level of GSH-Px in gingival fluid and serum of patients with chronic periodontitis in the active period were significantly higher than those in the diastasis period　（P＜0.05）.In addition，there was negative correlation among GSH-Px and gingival index，periodontal pocket depth，attachment loss in gingival fluid（r values respectively was-0.627，-0.459，-0.563，P＜0.05），there was negative correlation among the serum GSH-Px and gingival index，periodontal pocket depth，attachment loss（r values respectively was-0.443，-0.618，-0.538，P＜0.05）.Conclusion The implementation of dynamic monitoring of GSH-Px in gingival fluid，which can provide a reliable reference for further understanding the pathogenesis of periodontal disease and assessing the extent of disease.

107 Clinical effect of photodynamic therapy combined with intravitreal injection of anti-VEGF drugs in treatment of age-related macular degeneration

Objective To discuss the clinical effect of photodynamic therapy　（PDT）combined with intravitreal injection of anti-VEGF drugs in treatment of age-related macular degeneration　（ARMD）and to provide theoretical basis for its clinical application.Methods 120 ARMD patients treated in our hospital from January 2013 to September 2015 were selected and randomly divided into observation group　（n=60）and control group　（n=60）.Patients in control group were given intravitreal injection of anti-VEGF drugs，while patients in observation group were treated by anti-VEGF drugs combined with PDT.The vision recovery，changes of best corrected visual acuity　（BCVA）before and after treatment，and postoperative complications in two groups were analyzed.Results The overall effective rate was 95.00%in observation group and 73.33%in control group，which was significantly different between two groups　（P＜0.05）.The vision recovery was more significant in observation group than that in control group，which was most evident in week 1 and week 12（P＜0.05）.The overall incidence rate of complications was 5.00%in observation group and 13.33%in control group，which was significantly lower in observation group　（P＜0.05）.Conclusion PDT combined with intravitreal injection of anti-VEGF drugs has significant effect on ARMD，with favorable vision recovery，best BCVA，and few postoperative complications，which is worthy to be promoted.

110 Clinical effect of implants and natural teeth supportably fixed partial denture

Objective To explore the clinical effect of implant supportably fixed partial denture with natural teeth. Methods 20 dental patients with tooth loss in our hospital from January 2012 to December 2014 were randomly selected as the research objects and used planting and natural teeth to joint support and fix partial denture.The patients were taken with a period follow-up for 6 months-2 years，the clinical discomfort complaints，the bone loss of implant and the natural tooth，probing depth were observed.Results During the follow-up period，20 patients had no symptoms，their implants using function was good，The appearance was beautiful，all had not appeared gingival congestion，implants and natural teeth loosening，X-ray projection；the average amount of bone loss about the implant adjacent surface near neutral point was（1.05±0.27）mm，the average amount of bone loss about adjacent surface far neutral point was（0.98±0.28）mm，there was no statistically significant difference　（P＞0.05）；Probing depth of natural tooth was　（2.35±0.91）mm，probing depth of the implant was　（2.43±0.87）mm，there was no statistically significant difference　（P＞0.05）.Conclusion Using natural teeth and implant supportably fixed partial denture to repair the tooth loss，has a significantly clinical effect.

113 Clinical treatment of malignant glaucoma contralateral glaucoma

Objective To analysis the method and clinical treatment effects of malignant glaucoma contralateral glaucoma and to provide reference for clinic.Methods 50 malignant glaucoma contralateral glaucoma patients from April 2013 to April 2014 in our hospital were selected as study object and randomly divided into experimental group and control group，each group had 25 cases.In control group were used trabeculectomy，while in experimental group were used joint operation method，which contained intraocular lens implantation，trabeculectomy and clear corneal incision ultrasonic emulsification cataract extraction.At the end of operation，intraocular pressure and anterior chamber depth of patients in two groups were compared and the complications were analyzed to explore the effects.Results The postoperative intraocular pressure and anterior chamber depth in experimental group was respectively（11.37±5.47）mmHg and（3.04± 0.49）mm，which had statistical significance compared with control group　［（39.68±10.27）mmHg，（1.44±0.63）mm］（P＜0.05）.50 cases after operation，2 cases with corneal edema and corneal linear opacity，3 cases with inflammatory reaction of aqueous humor cells，1 case with intraocular lens pupil clamping and 2 cases with choroidal detachment.Conclusion For malignant glaucoma contralateral glaucoma patients，conducting intraocular lens implantation，trabeculectomy and clear corneal incision ultrasonic emulsification cataract extraction can improve vision of patients effectively and with great clinical value.After operation，dealing with the complications as soon as possible had positive help in improvement the prognosis of patients.

116 Clinical effect observation of periodental non-surgical treatment in patients with aggressive periodontitis

Objective To study the clinical effect of periodental non-surgical treatment in patients with aggressive periodontitis.Methods 68 patients with aggressive periodontitis treated by periodetal non-suryery from August 2013 to August 2015 in our hospital were selected.The periodental index，gomphiasis as well as the level of TNF-α and IL-4 in serum and gingival crevicular fluid were observed.Results After treatment for 1 month，the level of probing depth（PD），bleeding index（BI），attachment loss（AL）was greatly reduced（P＜0.05）；After treatment for 3 months，the level of PD，BI，AL was significantly lower than before treatment（P＜0.05）；After treatment for 3 months，the rate of no gomphiasis was significantly better than before treatment（P＜0.05）.The level of serum tumor necrosis factor-α　（TNF-α）and interleukin-4　（IL-4）before treatment and after treatment for 3 months was not signifcant difference　（P＞0.05）；The level of TNF-α and IL-4 of gingival crevicular fluid before treatment and after treatment for 3 months was signifcant difference （P＜0.05）.Conclusion The periodental non-surgical treatment has an obvious effect on patients with aggressive periodontitis，and can greatly improve the periodental condition and gomphiasis.

119 Clinical observation of intravitreal ranibizumab injection combined with grid laser photocoagulation in the treatment of macular edema secondary to branch retinal vein occlusion

Objective To observe the effectiveness and safety of combined therapy of intravitreal ranibizumab with grid laser photocoagulation for macular edema secondary to branch retinal vein occlusion（BRVO）.Methods Thirty-two eyes of 32 patients with macular edema secondary to BRVO in People′s Hospital of Meizhou City from July 2014 to July 2015 were randomized into two groups：laser group　（16 eyes）were given grid laser photocoagulation and compound Xueshuantong capsule，combined group（16 eyes）were given a single dose of intravitreal ranibizumab（0.05 ml/0.5 mg）and followed by grid laser photocoagulation after 1 week，combined with compound Xueshuantongcapsule.Follow-up for 6 months，the changes of best-corrected visual acuity　（BCVA）checked bg early treatment diabetic retinopathy study（ETDRS）and central macular thickness（CMT）among before treatment and 1month，3，6 months after theatment were compared in the two groups，the ocular and systemic adverse reactions about treatments were also followed up.Results The BCVA before treatment in the laser group was　（21.8±8.63）letters，which was significantly improved to　（33.5±9.76）letters，（31.6±9.96）letters，（30.5±8.83）letters respectively，at 1 month，3，6 months after treatment（P＜0.05）.The BCVA before treatment in the combined group was　（21.4±9.12）letters，which was also significantly improved to　（40.4±10.01）letters，（39.8±9.91）letters，（39.6±9.24）letters respectively，at 1 month，3，6 months after treatment（P＜0.05）.The CMT before treatment in the laser group was　（549.2±120.4）μm，which was significantly reduced to（290.3±88.6），（268.9±78.5），（252.4±66.3）μm respectively，at 1 month，3，6 months after treatment（P＜0.05）.The CMT before treatment in the combined group was（550.6±114.8）μm，which was significantly reduced to　（206.2±54.7），（190.1±53.8），（180.5±34.6）μm respectively，at 1month，3，6 months after treatment（P＜0.05）.The BCVA was better and the CMT was thinner in the combined group than that in the laser group at 1 month，3，6 months after treatment respectively （P＜0.05）.No ocular or systemic adverse reactions was observed during follow-up period.Conclusion Intravitreal ranibizumab injection combined with grid laser photocoagulation，which can significantly improve vision and is a clinical effective and safe method for macular edema secondary to BRVO.The curative effect of combined treatment is better than that of grid laser photocoagulation alone.

123 Effectiveness comparison of application of rhTPO and HGG in the treatment of refractory ITP

Objective To compare the effectiveness of application of recombinant human thrombopoietin（TPO）and human gamma globulin（HGG）in the treatment of patients with refractory idiopathic thrombocytopenic purpura（ITP）.Methods 70 patients with refractory ITP who had been treated in department of hematology，the second hospital of Sanming city from July 2011 to June 2015 were randomly and evenly divided into group A and group B with 35 cases in each group.Group A were treated with rhTPO；Group B were treated with HGG.The therapeutic effect，adverse reaction rate，platelet change time，length of stay　（LOS），pre-and post-treatment platelet count and immune function of the two groups were compared.Results The therapeutic effect and adverse reaction rate of the two groups showed no significant difference（P＞0.05）；The bleeding stop time，platelet rise time，platelet back-to-normal time and LOS of group B was obviously shorter than that of group A respectively（P＜0.05）.The pre-treatment platelet count and immune function of the two groups showed no significant difference（P＞0.05）.The post-treatment platelet count of the two groups had no significant difference（P＞0.05）.The post-treatment immune function of group B was significantly higher than that of group A （P＜0.05）.Conclusion rhTPO and HGG are two safe and effective ways in the treatment of refractory ITP，but HGG demonstrates its advantages of rapid effectiveness and short recovery time，and contributes to the improvement of immune functions in patients with refractory ITP.

126 Clinical effect of carboprost tromethamine injection in the treatment of postpartum hemorrhage caused by uterine inertia

Objective To study the clinical effect of carboprost tromethamine injection in the treatment of postpartum hemorrhage caused by uterine inertia.Methods 83 cases of postpartum hemorrhage caused by uterine inertia in our hospital from April 2013 to February 2015 were selected and divided into group a and group b according to random number method.The group a was given regular treatment with oxytocin，the group b was treated with carboprost tromethamine injection on the basis of the oxytocin routine therapy.The hemostasis efficiency，drug side effect rate，the third labor time，the amount of 2 hours of postpartum haemorrhage and 24 hours postpartum haemorrhage in the two groups were observed respectively.Results The hemostasis efficiency in the group b was higher than that in the group a，with significant difference（P＜0.05）.The incidence rate of drug side effect in the group b was lower than that in the group a，with significant difference（P＜0.05）.The third labor time in the group b was shorter than that in the group a，with significant difference（P＜0.05）.The amount of 2 hours of postpartum haemorrhage and 24 hours postpartum haemorrhage in the group b was less than that in the group a，with significant difference（P＜0.05）.Conclusion The effect of carboprost tromethamine injection in the treatment of postpartum hemorrhage caused by uterine inertia is definite，which can effectively shorten the duration time，reduce the incidence rate of postpartum hemorrhage，achieve the purpose of effective hemostasis，and side effect is less，high safety is higher.It is worth promoting.

129 Feasibility and safety of dimethyldiguanide as an adjuvant therapy in the treatment of nonalcoholic fatty liver disease

Objective To explore the feasibility and safety of dimethyldiguanide as an adjuvant therapy in the treatment of nonalcoholic fatty liver disease.Methods From March 2012 to September 2014，180 patients with nonalcoholic fatty liver diseases admitted into our hospital were selected and evenly divided into observation group and control group one and control group two in random.In the control group one，polyene phosphatidyl choline was used，in the control group two，dimethyldiguanide was adopted，and in the observation group，these two drugs in combination were applied.The liver function，blood lipids，insulin resistance and degree of hepatic steatosis were compared among three groups.Results In the control group one，homeostasis model assessment of insulin resistance（HOMA-IR）and triglyceride（TG）in the control group two were compared with no statistical difference（P＞0.05）.The levels of aspartate aminotransferase（AST）and total cholesterol（TC）were not in statistical significance after therapy in inter-group comparison　（P＞0.05）.Level of alanine aminotransferase（ALT），AST，TC，and degree of hepatic steatosis were greatly decreased compared with prior treatment（P＜0.05）.The total effective rates in the control group one and two，and observation group were 86.67%，85.00%，and 91.67% respectively with statistical difference（P＜0.05）.Conclusion Dimethyldiguanide as an adjuvant therapy in the treatment of nonalcoholic fatty liver disease can obtain a definite effect in high safety，which is worthy of clinical promotion.

132 Clinical observation on treatment effect of Qiangjin Jiangu capsule combined with diclofenac sodium enteric-coated tablet to treat periarthritis humeroscapularis

Objective To investigate the clinical treatment effect of Qiangjin Jiangu capsule combined with diclofenac sodium enteric-coated tablet in the treatment periarthritis humeroscapularis.Methods 100 patients with scapulohumeral periarthritis in our hospital outpatient or hospitalization from January 2012 to September 2013 were selected and devided into treatment group（group A）and control group（group B）in random control method，each group had 50 cases.Patients in group A were treated with diclofenac sodium enteric-coated tablet combined with Qiangjin Jiangu capsule，took diclofenac sodium enteric-coated tablet by oral therapy for 50 mg per time and 3 times per day，took Qiangjin Jiangu capsule with yellow wine for 6 pills per time（elderly or infirm patients took 4 pills）and 2 times per day.Patients in group B were only treated with diclofenac sodium enteric-coated tablet，took diclofenac sodium enteric-coated tablet by oral therapy for 50 mg per time and 3 times per day.Both group A and group B were treated for 10 days as a treatment process，then made follow-up observation on the situation of periarthritis humeroscapularis after 3 treatment processes. Results After treatment，the pain of two groups patients with was significantly relieved respectively，VAS scores was significantly decreased respectively　（P＜0.05）；The VAS score of group A was lower than that of group B，and the difference was statistically significant（P＜0.05）.The total effective rate of group A was 94%，higher than that of B group（66%），and the difference was statistically significant（P＜0.05）.After treatment，the degree of activity of the shoulder joint in the two groups of patients were improved，and the recovery of shoulder abduction，internal and external rotation and upper lift inthe group A was better than that of the group B respectively，the difference was statistically significant（P＜0.05）.Conclusion Treatment on periarthritis humeroscapularis with Qiangjin Jiangu capsule combined with diclofenac sodium enteric-coated tablet is better than the treatment only with diclofenac sodium enteric-coated tablet，the untoward effect is less，which is a kind of safe and effective method.Meanwhile，it also showed that the clinical effect of treatment on periarthritis humeroscapularis combined with traditional Chinese medicine and Western medicine is better.

136 Analysis of adverse drug reaction induced by antibiotics from 2012 to 2015 in our hospital

Objective To analyse the characteristics and the rule of ADR induced by antibiotics from 2012 to 2015 in our hospital，and to provide reference for drug use in the clinic.Methods 190 cases of ADR reports in our hospital from January 2012 to December 2015 were selected and analyzed retrospectively in respect of the kinds of antibacterial drugs，administration route，time，clinical manifestations，and patients′age，sex，and so on.Results Among 190 cases of ADR，the elderly were involved in the most of cases；The intravenous drip infusion was the main route（94.74%）；The fastest ADR occurred after 2 minutes of drug intravenously，and the slowest ADR occurred after 10d medication；The highest proportion was quinolones（41.59%）；The ADRs manifestations mainly covered lesions of skin and its accessories （40.53%）.Conclusion The age and injection administration are the main factors leading to the adverse reactions，drug use 48 h most easily lead to adverse reaction，and great importance should be attached to the clinical medication.

139 Curative effects of escitalopram in the treatment of elderly depression and its effects on life quality

Objective To understand the effectiveness of escitalopram in the treatment of elderly depression and its effects on life quality.Methods 60 outpatients and inpatients with elderly depression from October 2010 to October 2015 were randomly assigned to two groups，with 30 patients in each group.The two groups were given the treatment of escitalopram and citalopram respectively.HAMD was applied before the treatment，at the end of the first week，second week，fourth week，eighth week and twelfth week after the treatment to evaluate the curative effects，GQOLI-74 was applied to evaluate patients′life quality，and adverse reactions were observed.Results The differences of total scores of HAMD before the treatment in escitalpram group and citalpram group were not statistically significant（P＞0.05）；At the end of the first week until the twelfth week after the treatment，the total score of HAMD in both groups was gradually reduced respectively，and the difference compared with that before the treatment was statistically significant（P＜0.05）；At the end of the twelfth week，compared with citalpram group，the total score of HAMD in escitalpram group was reduced more greatly，and the difference between the two groups was statistically significant（P＜0.05）；Compared with the situation before the treatment，at the end of the twelfth week，physical function，mental function and social function in both groups was significantly improved respectively（P＜0.01）；At the end of the twelfth week，the improvement of physical function and social function in escitalpram group was better than that in citalpram group（P＜0.01）；The adverse reactions in both groups were slight.Conclusion Escitalpram is helpful for enhancing long-term curative effects and improving life quality in patients with elderly depression.

142 Effect observation of syndrome differentiation and treatment in Chinese medicine combined with comprehensive therapy in treating herpes zoster

Objective To observe the effect of syndrome differentiation and treatment in Chinese medicine combined with comprehensive therapy in treating herpes zoster.Methods 156 herpes zoster patients of Chinese medicine dermatology from June 2012 to June 2015 were random divided into two groups.78 cases in control group were treated with comprehensive therapy included oral administration aciclovir tablet，stab bleb and released liquid，high-energy red glow and external application of Jidesheng Sheyao tablet.78 cases in treatment group were further given syndrome differentiation and treatment in Chinese medicine of Chinese medicinal decotion on the basis of control group.The course was limited in 10 days.The index of rash healing time，pain disappeared time and clinical effects at the end of treatment were observed.Results After treatment for 10 days，the total effective rate in treatment group was 94.87%，which was obvious higher than that was 82.05%in control group　（P＜0.05）.Any obvious adverse effects appeared in two groups after treatment.Conclusion Syndrome differentiation and treatment in Chinese medicine combined with comprehensive therapy in treating herpes zoster can obvious enhance the cure rate of herpes zoster，shorted disease course and alleviate pain.

145 Effect of Buyanghuanwu Decoction on collateral circulation in patients with acute cerebral infarction

Objective To discuss the clinical effect of Buyanghuanwu Decoction on collateral circulation in patients with acute cerebral infarction　（ACI）.Methods 40 ACI patients treated in our hospital from October 2014 to October 2015 were reviewed and randomly divided into study group and control group，each with 20 cases.Patients in the control group were given western drugs for lipid regulation，blood activation and stasis elimination，and vascular dilation，while patients in the study group were additionally given Buyanghuanwu Decoction.Levels of VEGF and bFGF were detected by ELISA and neurofunction deficits at day 1 and day 7 of hospitalization were observed.Results VEGF levels were enhanced in both groups after treatment.At day 7，the VEGF level was significantly higher in the study group than that in the control group　（P＜0.05）.At day 7，the bFGF level increased in the study group，while reduced in the control group，which showed significant difference　（P＜0.05）.Before treatment，there was no significant difference in neurofunction deficit between two groups　（P＞0.05）；while after treatment，there was significant difference in neurofunction deficit between two groups　（P＜0.01）.There were significant differences in neurofunction deficit scores in each group before and after treatment（P＜0.01）.There were significant differences in high blood viscosity，low blood viscosity，and red blood cells between two groups　（P＜0.05）.Conclusion Buyanghuanwu Decoction can effectively promote the expression of serum VEGF and bFGF，and improve blood supply in ACI patients，thus facilitate establishment of collateral circulation and improve and protect the function of brain tissues.

148 Effect observation of integrated Chinese and Western medicines in treating ulcerative colitis

Objective To explore the effect of integrated Chinese and Western medicines in treating ulcerative colitis. Methods 60 ulcerative colitis patients from June 2013 to June 2015 of our hospital were selected and were divided into observation group　（30 cases）and control group　（30 cases）according to random number table.Patients in control group were given sulfasalazine combined with ranitidine.Patients in observation group were further given oral administration and enema traditional Chinese medicine apozem on the basis of control group.The treatment effects in two groups were observed.Results Western medicine curative effect compared in two groups，the total effective rate in observation group and control group was 90.00%and 63.3%respectively，and the difference was statistical significant（P＜0.05）.The Chinese medicine certificate waits curative effect compared in two groups，the total effective rate in observation group and control group was 93.3%and 63.3%respectively，and the difference was statistical significant（P＜0.05）.The TCM score in two groups decreased obviously after treatment，and the observation group was more obvious than control group，and the difference was statistical significant（P＜0.05）.Before treatment，the light，medium and severe lesions number between two groups had no obvious difference　（P＞0.05）.After treatment，there was any severe lesions patients in observation group，and light，medium lesions number obvious decreased，which compared with control group，the difference was statistical significant（P＜0.05）.Before treatment，the IL-6 level between two groups had no obvious difference and improved after treatment（P＞0.05），but the improvement effects in observation group were more obvious and the difference was statistical significant（P＜0.05）.Conclusion Western medicine sulfasalazine，ranitidine combined with oral administration and enema traditional Chinese medicine apozem with satisfactory effects in treating ulcerative colitis.

152 Clinical effect of integration of traditional Chinese and Western medicine in the treatment of chronic congestive heart failure

Objective To explore the clinical effect of integration of traditional Chinese and Western medicine in the treatment of chronic congestive heart failure　（CHF）.Methods Sixty-six patients with chronic CHF from March to June in 2015 in our hospital were divided into rehabilitation group and control group by a random number table.In the control group，conventional western medicine was provided，while in the rehabilitation group，on the basis of the conventional western medicine，acupuncture and moxibustion，and rehabilitation exercise recipe were added.They were treated for 3 consecutive weeks in both groups.Outcomes of left ventricular ejection fraction（LVEF），level of brain natriuretic peptide （BNP），6-min walk test（6M WT），MET and MLHFQ before and after treatment were comparatively analyzed.Results After therapy，cardiac function indexes，6M WT and MLHFQ were all greatly improved in the two groups（P＜0.05），and the improvement degrees in the rehabilitation group was superior to that in the control group（P＜0.05）.Conclusion Integration of traditional Chinese and western medicine plays a positive significance on prognosis in patients with chronic CHF，and can improve patients′exercise tolerance，quality of life as well as indexes of cardiac function.

155 Clinical effect of chronic glomerulonephritis in the pattern of Spleen and Kidney Qi Deficiency treated by traditional Chinese medicine

Objective To explore the clinical effect of Qi-Boosting and Kidney-Tonifying decoction in traditional Chinese medicine（TCM）on treating chronic glomerulonephritis in the pattern of spleen and kidney Qi deficiency.Methods 560 patients with chronic glomerulonephritis in the pattern of Spleen and Kidney Qi Deficiency admitted into our hospital from May 2011 to May 2015 were selected.According to the sequence of admission to hospital，they were randomly divided into control group　（n=279）and observation group　（n=281）.In the control group，conventional western medicine was used，while in the observation group，on the basis of the western medicine treatment，Qi-Boosting and Kidney-Tonifying decoction was added.The therapeutic effect of chronic glomerulonephritis and symptom score of TCM were compared.Results In the observation group，the total effective rate of chronic glomerulonephritis was 75.1%，significantly higher than 63.4%in the control group with a statistical difference　（P＜0.05）.In the observation group，scores of symptoms including sourness and pain of waist，lassitude，edema and no appetite，frequent urination，and increased nocturnal enuresis were greatly lower than those in the control group with a statistical difference　（P＜0.05）.Conclusion Qi-Boosting and Kidney-Tonifying decoction has a good therapeutic effect on treating chronic glomerulonephritis in the pattern of Spleen and Kidney Qi Deficiency，and can effectively improve the relevant symptoms of Spleen and Kidney Qi Deficiency.It is safe and effective，and is worthy of promotion in clinical application.

158 Survey on oral health status of community residents in Bao′an district of Shenzhen city

Objective To investigate oral health status of community residents in Baoan district of Shenzhen city so as to provide information support and basis for the prevention and treatment of oral diseases.Methods In reference with National Oral Epidemiological Survey Program，a questionnaire survey on the oral health status of community residents in Baoan district of Shenzhen city was designed to make an investigation and analysis of 200 cases 5-65 years old dental patients who were Buxin community residents in Baoan district of Shenzhen city and treated in Buxin Community Health Service Center of Shenzhen Baoan district People′s Hospital.Results Among 200 cases who were surveyed，the correct rate of oral health care behavior of 5-18 years old residents was significantly higher than that of 19-45 years old and 46-65 years old residents with statistically significant difference　（P＜0.05）；The correct rate of oral health care behavior of male residents was significantly higher than that of the female with statistically significant difference （P＜0.05）；A total of 130 residents suffered from dental caries（the total caries rate was 65.00%）and distributed in three age groups，17 patients of 5-18 years old（73.91%），65 patients of 26-45 years old（68.42%），48 patients of 46-65 years old（58.54%），the caries rate of 5-18 years old patients was significantly higher than that of other age groups（P＜0.05），and the caries rate reduced with the increase of patient′age；The dental caries rate in female was higher than that in male（P＜0.05）.The rate of gingivitis in patients of 19-45 and 46-65 years old was significantly higher than that in 5-18 years old patients with statistically significant difference（P＜0.05）；The occurrence rate of dental tophi，gingivitis and periodontitis in male were significantly higher than those of the female　（P＜0.05）.The dentition defect rate in 5-18 years old patients was significantly lower than that of patients of 19-45 years old and 46-65 years old with statistical significant difference　（P＜0.05）；The dentition defect rate in male was significantly lower than that in female with statistical significant difference（P＜0.05）.Conclusion The oral health status of community residents in Baoan District of Shenzhen city is not in a good condition.In view of this condition，oral health education shall be carried out to community residents so as to popularize the oral health knowledge，practice oral health behavior and improve their oral health level.

165 Apply of health quotient management in patients with stable COPD

Objective To explore effect of health quotient based health management on elderly patients with stable chronic obstructive pulmonary disease　（COPD）.Methods 200 elderly with stable COPD admitted into our hospital from January to 2014 December 2015 were selected as research subjects and randomly divided into two groups，each group with 100 cases，patients in the control group only were given for routine care，while patients in observation group were given health manage of entire process on base of routine care.The change of health awareness，health knowledge and health capabilities was evaluabled to patients by health quotient questionnaire after health management，care satisfaction of patients was surveyed by use of homemade patient satisfaction questionnaire.Results Subscale scores of self-care，life-style，mental status and life skill of patients before and after the implementation of health management in the observation group was significantly better than that in the control group respectively，and the difference was statistically significant（P＜0.05）；Nursing satisfaction in the observation group were 96%was significantly higher 77%in the control group，and the difference was statistically significant（P＜0.05）.Conclusion Comprehensive health management can improve the health quotient index of patients with stable COPD and reduce monitoring indicators of risk factors.

168 Application effect of lateral decubitus position delivery in primipara with persistent occipitotransverse and occipitoposterior

Objective To explore the application effect of lateral decubitus position delivery in primipara with persistent occipitotransverse and occipitoposterior.Methods 100 single-fetus full-term primiparas with persistent occipitotransverse and occipitoposterior in our hospital from May 2013 to October 2015 were selected and randomly divided into the observation group and the control group，50 cases in each group.The observation group was managed by lateral decubitus position delivery in the second stage of labor，the control group was managed by conventional supine lithotomy position.The time of the second stage of labor，mode of delivery，the rate of lateral episiotomy，the rate of perineal laceration，correction of fetal position，the rate of neonatal asphyxia and satisfaction of patients towards delivery outcome was compared respectively.Results The time of the second stage of labor in the observation group was（46.52±11.56）hours，which was shorter than（62.33±14.56）hours in the control group，with significant difference（P＜0.01）.The proportion of correction from persistent occipitotransverse and occipitoposterior to normal fetal position in the observation group was 86.00%，which was higher than 64.00%in the control group，with significant difference　（P＜0.05）.The rate of vaginal delivery in the observation group was 88.00%，which was higher than 62.00%in the control group，with significant difference　（P＜0.01）.The rate of lateral episiotomy in the observation group was lower than that in the control group，with significant difference（P＜0.01）.There was no significant difference in the rate of neonatal asphyxia and the rate of perineal laceration between the two groups　（P＞0.05）.The overall satisfaction degree of patients towards delivery outcome was 98.00%in the observation group，which was higher than 78.00%in the control group，with significant difference　（P＜0.01）.Conclusion Lateral decubitus position can effectively correct the fetal position in primiparas with persistent occipitotransverse and occipitoposterior，shorten the second stage of labor，improve the rate of vaginal delivery and reduce the rate of lateral episiotomy，improve the satisfaction of patients towards delivery mode with significant clinical effect.

171 Influence of Lamaze childbirth combined with improved unprotected perineum delivery on puerperal quality

Objective To explore the influence of Lamaze childbirth combined with improved unprotected perineum delivery on puerperal quality.Methods 300 puerperae by vaginal trial childbirth in our hospital from January 2015 to October 2015 were selected and randomly divided into the control group（140 cases）and the observation group（160 cases）.The observation group was given Lamaze childbirth combined with improved unprotected delivered，the control group was given spontaneous breathing plus and traditional delivery method.The incidence rate of labor pain，intrapartum maternal transfer cesarean section，postpartum hemorrhage，perineum and soft birth canal injury in the two groups was compared.Results The incidence rate of moderate pain in the observation group was higher than that in the control group，the incidence rate of severe pain and extreme pain in the observation group was lower than that in the control group，with significant difference（P＜0.01）.The incidence rate of postpartum hemorrhage，the rate of intrapartum maternal transfer cesarean section and the rate of perineum incision in the observation group was lower than that in the control group，the incidence rate of perineum without laceration or laceration of gradeⅠ in the observation group was higher than that in the control group，and the incidence rate of perineum with laceration of gradeⅡ in the observation group was lower than that in the control group，with significant difference　（P＜0.05）.Conclusion Lamahze delivery method combined with improved unprotected perineum delivery can significantly reduce pain intensity，reduce postpartum hemorrhage，effectively reduce the perineum incision rate and intrapartum cesarean section rate，promote the inciadence rate of natural childbirth，conducive to the maternity ward nursing quality management.

174 Nursing care of post-operation early enteral mixed nutrition in patients with gastric cancer

Objective To explore the clinical effect of application of the nursing program of post-operation early enteral mixed nutrition in patients with gastric cancer.Methods A total of 64 cases of gastric cancer patients who had been treated in department of digestive internal medicine in the hospital from February 2015 to February 2016 were randomly divided into observation group and control group with 32 cases in each group.Observed group was treated with enteral nutrition；while，control group was treated with parenteral nutrition.The postoperative and preoperative nutrition indicators，hospitalization stay，postoperative ventilation time and complication incidence of two groups were compared and analyzed.Results The postoperative body mass index，blood albumin，albumin and serum albumin index of two groups were increasing in comparison with the preoperative indexes；while，each index in observation group was significantly higher than that of control group，with statistically significant difference（P＜0.05）.The hospitalization stay and postoperative defecation ventilation time of observation group and control group were（13.4±4.1）d and（3.0±1.2）d versus（18.5± 5.8）d and　（6.9±1.4）d respectively，with statistical significance　（P＜0.05）.The complication incidence rate of control group was 40.63%（13 cases）versus 9.38%（3 cases）in observation group with statistical significance（P＜0.05）.Conclusion Application of postoperative early enteral mixed nutrition in patients with gastric cancer shows a significant advantage over parenteral nutrition in a certain degree，which is conducive to the prognosis of patients and is recommended to be widely used.

177 Effect observation on improvement of postoperative functional exercise compliance of patients with breast cancer by performing breast classes

Objective To discuss the influence of performing breast classes on improvement of postoperative functional exercise compliance of patients with breast cancer.Methods 43 patients from January 2010 to December 2012 were selected as control group，and were given routine health education of written materials such as photo album，health education sheets，etc.45 patients from January 2013 to December 2015 were selected as observation group，and were given breast classes from preoperative intervention，including give lessons，role play，peer education and WeChat classes.The postoperative functional exercise compliance rates，and recovery degrees of shoulder joint and muscle force of two groups were evaluated and analyzed.Results By interactive learning of breast classes，the functional exercise compliance rate，and recovery rates of shoulder joint and muscle force were 95.6%，84.5%，and 91.0%，respectively，in the observation group，which were significantly higher than the 86.1%，72.1%，and 84.0%in the control group（P＜0.05）.Conclusion Performing breast classes can improve the postoperative functional exercise compliance of patients with breast cancer，reduce incidence of movement dysfunction of affected limb，and improve life quality of patients.

180 Effect of nursing process management on improving the quality of nursing in operation room

Objective To explore the effect of nursing procedure management on nursing quality of operation room. Methods 100 cases of surgical patients in our hospital were selected from December 2014 to December 2015，and they were randomly divided into observation group and control group，50 cases in each group.The control group was given routine operation room nursing，and the observation group was implemented the program management nursing intervention measures.The patients nursing satisfaction in the two groups was investigated，the score of nursing quality in the two groups was evaluated after nursing intervention.Results the scores of quality of environmental management，service quality and safety management in operation room in the observation group were higher than that of control group，and the difference was statistically significant（P＜0.05）.The nursing satisfaction in the observation group were better than that of control group，and the difference was statistically significant（P＜0.05）.Conclusion The nursing process management can improve the quality of nursing in operation room，improve the patient's nursing satisfaction，and the effect is remarkable.

183 Study on the effect of individualized audio-visual intervention on anxiety and vital signs of patients in the waiting area

Objective To explore the effect of individualized audio-visual intervention on anxiety and vital signs of patients in the waiting area.Methods Clinical data of 200 patients who were in the waiting area of our hospital pre-operatively between February 2015 and February 2016 were analyzed and divided into control group（100 cases）and observation group　（100 cases）based on whether the individualized audiovisual distraction intervention was implemented.Patients in control group who were in the preoperative waiting area were maintained in a comfortable position waiting on the lathe and no audiovisual materials were given after routine psychological counseling done by nurses；individualized audiovisual materials were given to observation group on the basis of control group.The difference of two groups in the degree of anxiety and vital signs in the preoperative waiting area were compared and analyzed statistically.Results After implementation of individualized audiovisual distraction intervention，the degree of anxiety in observation group was lower than that of control group，with statistical significance difference（P＜0.05）.The post-intervention heart rate and blood pressure in observation group were lower than those before intervention，with statistically significant difference（P＜0.05）. Conclusion Implementation of individualized audiovisual distraction intervention for patients in the preoperative waiting area shows great advantages in reduction of patients stress and anxiety，as well as stabilization of blood pressure and heart rate，which is conducive to the smooth progress of anesthesia and surgery and deserves wider clinical application.

186 Impact of humanized nursing on physiological stress of emergency operation patients

Objective To explore the impact of humanized nursing on physiological stress of emergency operation patients.Methods 80 emergency operation patients from November 2015 to October who conducted humanized nursing were selected as observation group，and 80 emergency operation patients from January 2015 to February who conducted routine nursing were selected as control group.Physiological stress state，psychology and pain in two groups were compared. Results The postoperative heart，systolic pressure，cortisol and adrenaline in observation group were respectively（85.23± 7.35）/min，（147.25±10.12）mmHg，（112.361±4.27）nmol/L and（110.26±11.26）mg，lower than that were respectively（89.72± 7.02）/min，（152.36±5.83）mmHg，（132.56±14.21）nmol/L and（154.03±20.3）mg in control group，and the difference was statistical significance　（P＜0.05）.The incidence rate of anxiety before operation and in operation in observation group were respectively10.0%and 12.5%，lower than that were respectively 22.5%and 25.0%in control group；The postoperative non pain ratio was 40.0%，higher than that was20.0%in control group，and the difference was statistical significance（P＜0.05）.Conclusion Humanized nursing can obvious alleviate the physiological stress states of emergency operation patients，decrease adverse emotion and enhance pain tolerance.

189 The application effect evaluation of health education in nursing clavide fracture

Objective To investigate the effect of health education in the nursing of clavide fracture clinical application. Methods 106 admitted cases of clavicle fracture patients during the year 2013～2015 in our hospital were selected and divided randomly into two groups.One group is taken as control group，the other group as the conventional health education group，each group 53 cases.The patients in the control group were treated with conventional routine nursing，while health education were implemented in the conventional health education group in the basis of the conventional routine nursing.Results The results demonstrate that 90.6%patients in the conventional health education group have successfully acquired the knowledge of clavide fracture，which is obviously higher than that of control group.Meanwhile，the incidence of complications and psychological problems are significantly decreased in the conventional health education group.Finally，the rehabilitation quality of patients in the conventional health education group are also better than that of control group，the difference was statistically significant（P＜0.05）.Conclusion The health education in nursing of clavide fracture can effectively promote the rehabilitation quality of patients after operation and could provide an innovative，humanistic，and high-quality nursing service.

192 Study on the influence of strengthen preoperative communication for the incidence of acute pressure ulcer and related knowledge cognition of patients in operation room

Objective To explore the influence situation of strengthen preoperative communication for the incidence of acute pressure ulcer and related knowledge cognition of patients in operation room.Methods 150 surgical patients in our hospital from January 2014 to December 2015 were selected as the study object，and they were randomly divided into control group and observation group，75 cases in each group.The control group were given to the routine preoperative intervention，the observation group were given to strengthen preoperative communication，then the incidence of acute pressure ulcer，related knowledge cognition and local microcirculation indexes before the intervention and at different time after the intervention of two groups were compared.Results The incidence of observation group′s acute pressure ulcer with different operation time were all lower than those of control group，the related knowledge cognition at different time after the intervention were all higher than those of control group，while the local microcirculation indexes were all better than those of control group，with significant differences（P＜0.05）.Conclusion The strengthen preoperative communication has active control effect for the incidence of acute pressure ulcer of patients in operation room，and it also has higher clinical application value for the improvement of related knowledge cognition.

195 Application effects of risk management and preventive measures in pediatric nursing

Objective To explore application effects of risk management and preventive measures in pediatric nursing. Methods 72 children patients from January 2015 to October 2015 of our hospital were selected.Patients were random divided into control group and experimental group，and each group was 36 cases.Patients in control group were given routine nursing management，and patients in experimental group were given risk management and preventive measures. The management effects in two groups were compared.Results The nursing management effect in experimental group that using related nursing management measures was obvious better than that of control group，the incidence rate of risk was 8.33%，the satisfaction degree reached to 97.22%，and the difference was statistical significance when compared with control group（χ2=4.600，3.956，P＜0.05）.Families anxiety levels in experimental group was（32.62±5.25）points，which was （43.61±5.65）points in control group，and the difference was statistical significance when compared with control group（t= 8.624，P＜0.05）.Conclusion Using risk management and preventive measures can better prevention the risks in pediatric nursing and enhance the department satisfaction degree.It is a positive and effective management method.
