终末期肝病模型联合血清钠预测失代偿期肝硬化患者短期预后的临床价值

Abstract Objective To explore the clinical value of end-stage liver disease model combined with MELD-Na in prevention short-term prognosis in patients with decompensated liver cirrhosis.Methods 152 patients with decompensated liver cirrhosis from January 2012 to January 2015 of digestive system department in our hospital were selected and followed-up for 3 months.According to follow-up results，death group was 43 cases and surviving group was 109 cases.The first value of CTP，MELD and MELD-Na were respectively counted after admission.Receiver operating characteristic curve　（ROC）was used to further evaluate three scoring systems on the 3 months later predictive value of decompensated liver cirrhosis patients.Results Among 152 decompensated liver cirrhosis patients，43 death within 3 months followed-up.The CTP，MELD and MELD-Na score in surviving group were respectively　（10.93±2.72），（20.99±5.62）and （24.67±4.89），which were respectively（10.93±2.72），（20.99±5.62）and（24.67±4.89）in death group，with significant difference（P＜0.05）.In judging the prognosis of patients with ROC curve AUC in comparison，the MELD-Na score（0.876）＞MELD score（0.840）＞CTP（0.754，P＜0.05）.Conclusion All of CTP scoring，MELD scoring and MELD-Na scoring can be used to evaluate the short-term prognosis in patients with decompensated liver cirrhosis，while MELD-Na model scoring can more accuracy reflect the severity of the patients and has more application value in clinical.
