4 Effect of Valproic Acid on proliferation and apoptosis of esophageal cancer cells and study on its antitumor activity mechanism

Objective To explore the effect of Valproic Acid (VPA) on proliferation and apoptosis of esophageal cancer cells,and expression of Bcl-2,Caspase protein,as well as molecular mechanisms of signal transduction pathways. Methods ECa-109 cell line of esophageal cancer was cultured with PRMI1640 medium, and grouped according to solvent control and solubility gradient:blank control and VPA 0.25 mmol/L, 0.5 mmol/L, 1.0 mmol/L, 2.0 mmol/L, 4.0 mmol/L.After treating cells for 24, 48 and 72 hours according to different drug concentrations, the changes of esophageal cancer ECa-109 cells were observed, and different detection methods were used to detect the changes of cell viability, apoptosis and cycle of esophageal cancer ECa-109 cells.Western Blot was used to detect CyclinD1, p21, Survivin, Bcl-2,Caspase protein and PI3K/Akt and MAPK signal transduction pathway changes. Results VPA showed different extent changes on vitality, proliferation, differentiation of ECa-109 cells, could induce cell apoptosis by inducing cell cycle arrest.Western Blot experiment results showed that VPA could affect the expression of Bcl-2, Survivin and Caspase proteins in ECa-109 cells.VPA could significantly reduce the expression level of CyclinD1 protein in ECa-109 cells and increase the expression of p21 protein, which influenced the phosphorylation of key proteins such as AKT and MAPK in the signaling pathway PI3K/Akt and MAPK pathway. Conclusion VPA has an effect on the proliferation and apoptosis of esophageal cancer cells and it is concentration-dependent, which can affect the growth of esophageal cancer cells through the signaling pathways of multiple protein kinase pathways, the expression of survival proteins, the expression of PI3K/Akt and MAPK signaling pathways.

10 Effect comparison between ultrasonic bone knife combined with high speed turbine and traditional bone chisel crown method in the extraction of difficult impacted wisdom teeth of jaw

Objective To compare the effect between ultrasonic bone knife combined with high speed turbine and traditional bone chisel crown method in the extraction of difficult impacted wisdom teeth of jaw. Methods A total of 120 patients with difficult impacted wisdom teeth of jaw admitted to our hospital from January 2017 to October 2018 were selected as the research objects, and they were divided into the observation group (n=60) and the control group (n=60)according to the random number table method.The observation group was treated with ultrasonic bone knife combined with high speed turbine, while the control group was treated with traditional bone chisel and crown method.The general conditions of surgery (operation time, amount of intraoperative blood loss) of the two groups of patients were compared,and the visual analogue scale (VAS) was used to evaluate the pain degree of the two groups of patients during the operation and at 2 and 7 days after the operation.At the same time, the degree of swelling and the restriction degree of mouth opening were evaluated, and the incidence of postoperative complications of the two groups was counted. Results The operation time of the observation group was shorter than that of the control group, and the difference was statistically significant (P<0.05).The amount of intraoperative blood loss in the observation group was less than that in the control group, and the difference was statistically significant (P<0.05).The VAS scores of the observation group during the operation and at 2 and 7 days after the operation were lower than those of the control group, the differences were statistically significant (P<0.05).The degree of swelling and the restriction degree of mouth opening in the observation group were lighter than those in the control group at 2 days after operation, and the differences were statistically significant (P<0.05).The total incidence rate of postoperative complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).Conclusion Compared with the traditional bone chisel crown method, ultrasonic bone knife combined with high speed turbine has the advantages of minimal invasion, efficiency and safety in removing difficult impacted wisdom teeth.

15 Influencing factors analysis of infiltrative lesion and anaplastic lymphoma kinase gene mutation in patients with lung ground glass nodule

Objective To analyze the influencing factors of infiltrative lesion and anaplastic lymphoma kinase (ALK)gene mutation in patients with lung ground glass nodule (GGN). Methods A total of 80 lung GGN patients suspected with ALK gene mutation admitted to oncology department of our hospital from January 2015 to January 2018 were selected as the research objects, including 30 cases with pre-infiltration lesions and 50 cases with infiltration lesions, 36 patients in normal ALK gene group, 44 patients in the gene mutation group.The pathological results were used as the gold standard to analyze the differences between the pathological status of patients with pre-infiltration lesions and infiltration lesions, as well as ALK gene mutations, and to study the influencing factors of GGN invasion and ALK gene mutations. Results There were statistically significant differences in morphology (hair prick sign, lobular sign, pleural indentation or stretch sign, bronchial inflation sign or vacuole sign), size, relative CT value, CT value, volume and mass between pre-infiltration lesions and infiltration lesions (P<0.05).Logistic regression analysis showed that GGN mass(β=2.190, OR=8.935, 95%CI:7.446-10.722), volume (β=1.781, OR=5.936, 95%CI:4.841-7.278), CT value (β=1.709,OR=5.523,95%CI:4.694-6.499),relative CT value(β=1.684,OR=5.387, 95%CI:3.690-7.864), size (β=1.762, OR=5.824,95%CI:5.264-6.324),hair prick sign(β=1.594,OR=4.923,95%CI:4.184-5.793),lobular sign(β=1.665,OR=5.286,95%CI:4.153-6.727), pleural indentation or stretch sign (β=1.606, OR=4.983, 95%CI:4.413-5.627), bronchial inflation sign or vacuole sign (β=1.794, OR=6.013, 95%CI:5.191-6.966) were influencing factors of infiltrative lesions (P<0.05).The ROC curve showed that the sensitivity of lesions ≥0.276 g to diagnose GGN infiltration was 80.0% (24/30),the specificity was 94.0% (47/50), and the area under the ROC curve was 0.872.There were statistically significant differences in GGN size, volume, mass, and invasion between the gene mutation group and the normal gene group (P<0.05).Logistic regression analysis showed that mass (β=2.059, OR=7.838, 95%CI:5.380-11.419), volume (β=1.936, OR=6.931, 95%CI:5.036-9.540), size (β=1.781, OR=5.936, 95%CI:4.450-7.918), and invasion (β=1.635, OR=5.129, 95%CI:4.031-6.528) were the influencing factors of ALK gene mutation (P<0.05).The optimal cut-off values for ALK gene mutations diagnosed by GGN mass, volume and size were 0.256 g, 565.6 mm3 and 10.4 mm, respectively, and the areas under the ROC curve were 0.803, 0.781 and 0.755. Conclusion The mutation of ALK gene is an important factor affecting the invasion of GGN.High-resolution CT (HRCT) signs are of great value in the prediction of the invasion of GGN and the mutation of ALK gene.

20 Clinical effect of Fat-Soluble Vitamin Injection combined with Adenosine Butadionate Methionine in the treatment of alcoholic liver disease with intrahepatic cholestasis

Objective To evaluate the clinical effect of Fat-Soluble Vitamin Injection combined with Adenosine Butadionate Methionine in the treatment of alcoholic liver disease with intrahepatic cholestasis. Methods Clinical data of 126 cases of alcoholic liver disease with intrahepatic cholestasis patients in our hospital during December 2016 to March 2019 were retrospectively analyzed.According to the treatment methods they were divided into control group(68 cases), observation group(58 cases).Patients in control group were treated with Adenosine Butadionate Methionine,those in study group used Fat-Soluble Vitamin Injection combined with Adenosine Butadionate Methionine treatment.Short-term efficacy, liver functionindexes (glutamic oxaloacetic transaminase [AST], alanine aminotransferase [ALT], total bilirubin [TBil], alkaline phosphatase [ALP]), coagulation indexes (prothrombin time [PT], thrombin time [TT], activated partial thromboplastin time [APTT], fibrinogen [FIB]and thromboplastin function [PT]), soluble adhesion molecule-1 (sVCAM-1) and phospholipid transporter (PLTP) were compared between two groups.During treatment period, the incidence of adverse drug reactions were recorded. Results 4 weeks after treatment, the total effective rate in study group was higher than that in control group (χ2=4.645,P=0.031); levels of AST, ALT, TBil and ALP in study group were lower than those in control group (t=15.964, 19.451, 18.656, 9.130,P<0.05); the PT, TT and APTT in study group were shorer than those in control group, FIB was higher than that in the control group (t=3.334、5.520、4.873、6.704,P<0.05); levels of sVCAM-1 and PLTP in the control group were lower than those in the control group (t=12.543、6.717, P<0.05).During treatment, there was no significant difference in the total incidence of adverse drug reactions between two groups (P>0.05). Conclusion The effect of Fat-Soluble Vitamin Injection combined with Adenosine Butadionate Methionine in patients with alcoholic liver disease and intrahepatic cholestasis is reliable.At the same time, it has good therapeutic safety.

24 Application of fluorescence PCR-probe melting curve analysis in the rapid diagnosis of deletion α-thalassemia

Objective To explore the application of fluorescence PCR-probe melting curve analysis (PMCA) in the rapid diagnosis of deletion α-thalassemia. Methods A total of 2261 blood samples and 35 amniotic fluid samples were collected in Zhuhai Municipal Maternity and Child Healthcare Hospital from October to December 2019.Through double-blind method, PMCA and single-tube multiplex PCR (gap-PCR) were used to detect three common deletion αthalassemia genes in these 2296 clinical samples.The samples inconsistent with results were confirmed by DNA sequencing analysis. Results In these 2296 samples, except for one unknown mutation gene detected by PMCA in one sample, the results of PMCA in others were consistent with those of gap-PCR, with a coincidence rate of 99.9%.DNA sequencing of the only inconsistent sample showed that a single base mutation (A>G) occurred in the probe coverage area. Conclusion PMCA can quickly and accurately detect three common deletion α-thalassemia genes, which can be used as an alternative or verification method of gap-PCR technology, as well as for large-scale population screening and prenatal diagnosis.

28 Effect of cerebellar fastigial nucleus electrical stimulation treating chronic subjective dizziness

Objective To investigate the effect of cerebellar fastigial nucleus electrical stimulation treating chronic subjective dizziness. Methods Fifty-eight patients with chronic subjective dizziness treated in our hospital from November 2018 to December 2019 were enrolled in the study as subjects.They were divided into the study group (n=29) and the control group (n=29) according to the random number table method.Conventional antidepressant treatment was performed in the control group and the study group was given cerebellar fastigial nucleus electrical stimulation based on the above measures.And all the patients in the study were assessed by dizziness handicap inventory (DHI)and hospital anxiety and depression scale (HADS). Results There were no significant differences between the two groups in any of the assessments before treatment (P>0.05).After 30 days of treatment, the HADS score, scores of DHI total index, body, function, emotion in the two groups were lower than those before treatment, and the differences were statistically significant (P<0.05).After 30 days, the HADS score, scores of DHI total index, body, function, emotion in the study group were lower than those of control group, and the differences were statistically significant (P<0.05).Conclusion On the basis of conventional antidepressant drugs, combined with cerebellar crest nuclear stimulation therapy may be more beneficial to improving the dizziness symptoms of patients with chronic subjective dizziness, and can effectively improve the symptoms of anxiety and depression.

31 Content determination of o/p-toluenesulfonamide in saccharin sodium by the gas chromatography

Objective To establish the gas chromatography for the determination of o/p-toluenesulfonamide in saccharin sodium. Methods DB-17MS chromatographic column was applied, the initial temperature was set at 180 ℃for 20 min, and then heated up to 260℃at 10℃/ min and maintained for 32 min. Results The peak area of o-toluenesulfonamide in 11.80-223.60 μg/ml (r=1) showed a good linear relationship with the concentration, the average sample recovery rate was 96.58%, and the RSD was 5.73% (n=6).The peak area of p-toluenesulfonamide in 10.15-203.00 μg/ml(r=1) showed a good linear relationship with the concentration, the average sample recovery rate was 103.99%, and the RSD was 2.83% (n=6).The content of o/p-toluenesulfonamide in saccharin sodium in 6 batches was not detected because the results were lower than the detection limit, which met the requirements of the pharmacopoeia. Conclusion The GC for the determination of toluenesulfonamide in saccharin sodium is simple and accurate, and can be used as a quality evaluation standard for saccharin sodium.

36 Research advances in the value of noninvasive markers in the early diagnosis of early endometriosis

Endometriosis, as a common gynecological disease, has affected many women of childbearing age.In recent years, because of its high incidence, high recurrence rate, and younger tendencies, much more attention has been paid,and because of its invasion and implantation behavior similar to malignant tumors, it has been nicknamed the "benign carcinoma", which has caused serious impact on life quality of women.The "gold standard" for the diagnosis of endometriosis is laparoscopy, which is not only expensive, requires general anesthesia, and may even cause complications such as pelvic and abdominal adhesions.At present, CA125 is the most commonly used clinical indicator to predict endometriosis, but it is not a reliable tool for early diagnosis.Due to the lack of specific clinical symptoms and noninvasive early detection methods, the diagnosis of endometriosis is often delayed or even missed.Therefore, it is very important to seek a noninvasive biological index with high specificity and sensitivity to diagnose early endometriosis.This paper mainly expounds the value of noninvasive biological index in the early diagnosis of endometriosis.

48 Progress on application of vacuum sealing drainage technology in traumatic orthopedics

Vacuum sealing drainage is a new technology in the 1990s, pioneered by Dr.Fleischman of traumatology orthopedics, Ulm University in Germany.It has provided a new direction and revolutionary change for the treatment of various types of clinical trauma.Traditional model of passive healing in the past has been changed, and new development of clinical treatment for trauma has been promoted.With the development and application of vacuum sealing drainage, this new technology has gradually been introduced into China.Secretion or necrosis tissue in cavity and wound can be removed effectively with vacuum sealing drainage to help wound healing.In recent years, vacuum sealing drainage has been developed and used in various clinical fields resulting in excellent clinical effect and economic value.Based on this, this paper mainly reviews the application of vacuum sealing drainage in traumatology orthopedics, in order to facilitate the clinical promotion and development of the technology.

52 Clinical effect of CT-guided percutaneous microwave ablation on treatment of early primary hepatic carcinoma

Objective To study the clinical effect of CT-guided percutaneous microwave ablation (MWA) on treatment of early primary hepatic carcinoma. Methods A total of 86 early primary hepatic carcinoma patients treated in our hospital from January 2013 to July 2016 were selected as research subjects.According to the computer random method,patients were assigned to intervention group (43 cases) and control group (43 cases).The intervention group was treated with CT-guided percutaneous MWA, the control group was treated with conventional transcatheter arterial chemoembolization.The short-term efficacy, long-term efficacy and adverse reactions were compared. Results The total effective rate in intervention group was 81.40%, which was higher than that of control group (55.81%), with statistically significant difference (P<0.05).The total incidence of adverse reactions between groups showed no statistically significant difference (P>0.05).One-year survival rate between groups showed no statistically significant difference (P>0.05).Twoyear and three-year survival rates in intervention group were higher than those of control group, with statistically significant differences (P<0.05). Conclusion For early primary hepatic carcinoma patients, CT-guided percutaneous MWA has better short-term and long-term efficacy, and lower adverse event rate.

56 Clinical effect of Atorvastatin combined with Trimetazidine in the treatment of angina pectoris of coronary heart disease

Objective To investigate the clinical effect of Atorvastatin combined with Trimetazidine in the treatment of angina pectoris patients with coronary heart disease. Methods A total of 100 patients with coronary heart disease and angina pectoris admitted to our hospital from January 2018 to March 2019 were selected as the research objects.According to different treatment schemes, they were divided into the experimental group (50 cases) and the control group(50 cases).The experimental group was treated with Atorvastatin combined with Trimetazidine, while the control group was treated with Trimetazidine.The treatment effect, blood lipid levels before and after treatment, angina pectoris frequency and duration were compared between the two groups. Results The total effective rate of the experimental group was higher than that of the control group, and the difference was statistically significant (P<0.05).The levels of lowdensity lipoprotein cholesterol (LDL-C), triglyceride (TG) and total cholesterol (TC) in the two groups after treatment were lower than those before treatment, and the levels of high-density lipoprotein cholesterol (HDL-C) in the two groups were higher than those before treatment, with statistically significant differences (P<0.05).After treatment, the levels of LDL-C, TG and TC in the experimental group were lower than those in the control group, and the level of HDL-C was higher than that in the control group, with statistically significant differences (P<0.05).After treatment, the number of angina attacks in the two groups was fewer than that before treatment, and the attack time was shorter than that before treatment, with statistically significant differences (P<0.05).After treatment, the number of angina attacks in the experimental group was fewer than that in the control group, and the duration of the attacks was shorter than that in the control group, with statistically significant differences (P<0.05). Conclusion Patients with coronary heart disease and angina pectoris receive Atorvastatin and Trimetazidine in combination, the effect is good, and it can improve the blood lipid treated with of patients.

59 Clinical effect of Clopidogrel combined with Urokinase thrombolysis in the treatment of patients with ST-elevation myocardial infarction

Objective To explore the clinical effect of Clopidogrel combined with Urokinase thrombolysis in the treatment of patients with ST-elevation myocardial infarction. Methods Fifty patients with ST-segment elevation myocardial infarction treated in our hospital from August 2015 to August 2019 were selected as subjects.By a random number table method, they were divided into the control group (n=25) and the observation group (n=25).Patients in the control group were treated with thrombolytic therapy with Urokinase thrombolysis.Patients in the observation group were treated with Clopidogrel combined with Urokinase thrombolytic therapy.The levels of inflammatory factors, myocardial function indicators, myocardial injury and cardiovascular events were compared between the two groups. Results After treatment, the levels of interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), and left ventricular end-diastolic dimension(LVEDD) decreased and the level of left ventricular ejection fraction (LVEF) increased, the levels of serum IL-6, TNFα, and LVEDD in the observation group were lower than those of the control group, while LVEF level was higher than that of the control group, and the differences were statistically significant (P<0.05).After treatment, the levels of cardiac troponin-T (cTnT), creatine kinase-MB (CK-MB), and brain natriuretic peptide (BNP) of patients in the two groups decreased, the levels of cTnT, CK-MB, and BNP in the observation group were lower than those in the control group, with statistically significant differences (P<0.05).The total incidence of cardiovascular disease in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The application of Clopidogrel combined with Urokinase thrombolysis in patients with ST-segment elevation myocardial infarction can effectively reduce patients′ inflammatory factors and myocardial damage, improve myocardial function, and effectively decrease the incidence of recurrent cardiovascular disease.The therapeutic effect is safe and effective, and it is worth popularizing and applying.

Objective To explore the effect of transcranial magnetic stimulation in the treatment of schizophrenia.Methods From March 2016 to March 2018, 112 patients with schizophrenia treated in our hospital were selected as the research objects.They were randomly divided into control group and study group by double color sphere method, with 56 patients in each group.Patients in the control group were given drug therapy, and patients in the study group were treated with transcranial magnetic stimulation on the basis of the control group.The clinical efficacy of the two groups after treatment was compared and analyzed.And the cognitive function and mental symptoms before and after treatment were compared between the two groups. Results The total effective rate of clinical treatment in the observation group was 98.21%, which was higher than that in the control group (78.57%), the difference was statistically significant (P<0.05).Before treatment, there were no significant differences in the scores of positive symptoms, negative symptoms,general psychopathological symptoms and the total score of positive and negative syndrome scale (PANSS) between the two groups (P>0.05).After treatment, the scores of positive symptoms, negative symptoms, general psychopathological symptoms and the total score of PANSS in the two groups were lower than those before treatment, and the scores of positive symptoms, negative symptoms, general psychopathological symptoms and the total score of PANSS in the observation group were lower than those of the control group, the differences were statistically significant (P<0.05).Before treatment, there were no significant differences in invisual spatial working memory (VSWM) and vocabulary fluency(VFT) scores between the two groups (P>0.05).After treatment, the VSWM and VFT scores of both groups were higher than those before treatment, and the VSWM and VFT scores in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). Conclusion The effect of transcranial magnetic stimulation on schizophrenia is remarkable, which can obviously improve the mental symptoms and cognitive function of schizophrenia, which is worth popularizing in clinic.

66 Effect of muscle relaxation therapy on post-treatment pain of patients with depression non-convulsive electric shock

Objective To analyze the effect of muscle relaxation therapy on the post-treatment of non-convulsive electric shock (MECT) in patients with depression. Methods From January 2017 to January 2019, 60 patients with depression treated with MECT in our hospital were selected as subjects, and the patients were divided into the control group and the treatment group according to the random digital method, 30 cases in each group.Patients in the control group were given routine nursing, and patients in the treatment group combined with the muscle relaxation therapy.The visual analogue scale (VAS) scores, Hamilton depression scale (HAMD) scores, Pittsburgh sleep quality index (PSQI) scores,and total nursing satisfaction were compared between the two groups. Results There were no significant differences in VAS, HAMD and PSQI scores between the two groups before intervention (P>0.05).The VAS, HAMD and PSQI scores of the two groups after intervention were lower than those before intervention, and the differences were statistically significant (P<0.05).The VAS scores at 1, 6, 12 and 24 h after intervention in the treatment group were lower than those in the control group, with statistically significant differences (P<0.05).After intervention, HAMD and PSQI scores of patients in the treatment group were lower than those in the control group, with statistically significant differences (P<0.05).The total nursing satisfaction of patients in the treatment group was higher than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Muscle relaxation therapy after MECT treatment can effectively improve postoperative pain, depression and sleep quality of patients with depression, and improve the clinical treatment effect, which is worth promoting in the clinical application.

70 Effect comparison of skull puncture drainage and borehole drainage treating hypertensive cerebral hemorrhage

Objective To explore the effect of skull puncture drainage and borehole drainage treating hypertensive cerebral hemorrhage. Methods Totally 40 patients with hypertensive cerebral hemorrhage treated in the department of neurosurgery of our hospital from March 2015 to June 2019 were selected as research subjects.They were divided into observation group(n=20)and control group(n=20)by the random number table method.Patients in the control group were treated with borehole drainage, and patients in the observation group were treated with skull puncture drainage.The total incidence of bleeding, surgical parameters, neurological impairment score, daily living ability score and therapeutic effect were compared between the two groups. Results The total incidence of bleeding in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).The operation time, hospitalization time and hematoma clearance time of the observation group were shorter than those of the control group, with statistically significant differences(P<0.05).There were no statistically significant differences in the scores of neurological impairment and the scores of daily living ability before treatment between the two groups (P>0.05).On the 7th day after treatment, the scores of neurological impairment in both groups were lower than those before treatment, and the scores of daily living ability were higher than those before treatment,with statistically significant differences (P<0.05).On the 7th day after treatment, the neurological impairment score of the observation group was lower than that of the control group,and the daily living ability score was higher than that of the control group,with statistically significant differences(P<0.05).The total effective rate of the observation group (95.00%) was higher than that of the control group (70.00%), and the difference was statistically significant (P<0.05). Conclusion For patients with hypertensive cerebral hemorrhage, skull puncture drainage can significantly reduce the bleeding rate, shorten the hospital stay, reduce the hematoma clearance time, alleviate neurological impairment and improve quality of life.It is worth promoting in the clinic.

74 Application effect of transcranial magnetic stimulation combined with antipsychotics in vagrant psychosis

Objective To analyze the application effect of transcranial magnetic stimulation combined with antipsychotics in vagrant psychosis. Methods A total of 114 vagrant psychosis patients admitted to our hospital from June 2018 to May 2019 were selected as the research objects.According to the method of random number table method, they were divided into the control group (57 cases) and the observation group (57 cases).The control group was treated with Olanzapine, and the observation group was treated with Olanzapine and transcranial magnetic stimulation.The clinical efficacy, positive and negative symptom scale (PANSS), Pittsburgh sleep quality scale (PSQI), personal and social function scale (PSP) and adverse reactions were compared between the two groups. Results The total effective rate of the observation group was 75.44%, which was higher than that of the control group (56.14%), the difference was statistically significant (P<0.05).There were no significant differences in PANSS score, PSQI score and PSP score between the two groups before treatment (P>0.05).The PANSS and PSQI scores of the observation group after treatment were (57.62±3.91) and (5.08±1.92) points, which were lower than those of the control group ([76.54±3.15]and [8.67±2.04]points),the differences were statistically significant (P <0.05).The PSP score of the observation group after treatment was(63.46±7.05) points, which was higher than that of the control group ([50.89±6.21]points), the difference was statistically significant (P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion The combination of transcranial magnetic stimulation and antipsychotic drugs is safe and effective in the treatment of vagrant psychosis, which is beneficial to relieving mental symptoms and sleep disorders, and can restore social functions.

77 Application effect of Methylprednisolone in the treatment of acute multiple sclerosis

Objective To analyze the clinical effect and safety of Methylprednisolone in the treatment of acute multiple sclerosis. Methods A total of 94 patients with acute multiple sclerosis admitted to our hospital from January 2018 to August 2019 were selected as the research subjects.They were divided into the reference group (n=47) and the study group (n=47) by the random number table method.Dexamethasone combined with Methylprednisolone treatment was applied in the reference group, 20 mg of Dexamethasone dissolved in 500 ml of 0.9% sodium chloride was injected intravenously for 5 days, followed by oral administration of 10 mg of Methylprednisolone daily for 90 days.In the study group, patients were treated with Methylprednisolone, 1000 mg of Methylprednisolone dissolved in 500 ml of 0.9%sodium chloride was injected intravenously for 5 days, followed by oral administration of 60 mg of Methylprednisolone daily for 90 days.The levels of CD4+, CD8+, CD4+/CD8+ and the total incidence rate of adverse reaction were compared between the two groups. Results There was no significant difference in the level of CD4+ between the two groups after treatment (P>0.05).The level of CD8+ in the study group after treatment was higher than that of the reference group,and the difference was statistically significant (P<0.05).There was no significant difference in the level of CD4+/CD8+between the two groups after treatment (P>0.05).The total incidence rate of adverse reaction in the study group was lower than that of the reference group, and the difference was statistically significant (P<0.05). Conclusion Methylprednisolone applied in the treatment of acute multiple sclerosis can effectively regulate the balance of T lymphocyte subsets and reduce patients′ adverse reactions, which is worthy of promotion.

80 Clinical effect of Donepezil Hydrochloride combined with Nimodipine in the treatment of vascular dementia

Objective To evaluate the clinical effect and safety of Donepezil Hydrochloride combined with Nimodipine in the treatment of vascular dementia, and to provide reference for clinicians to choose a reasonable drug regimen.Methods A total of 86 patients with vascular dementia diagnosed and treated in Shenyang Red Cross Hospital from January 2018 to July 2019 were selected as the study objects.According to the method of random number table, they were divided into control group and experimental group, 43 cases in each group.The control group was given Nimodipine treatment.The experimental group was treated with the combination of Donepezil Hydrochloride and Nimodipine.the clinical efficacy and safety of the two groups were compared. Results The total clinical effective rate in the experimental group (93.02%) was higher than that in the control group (76.74%), the difference was statistically significant(P<0.05).There were no significant differences between the two groups in the mini-mental state examination scale(MMSE) score and the ability of daily life (ADL) score before treatment (P>0.05).After treatment, MMSE score and ADL score of the two groups were higher than those before treatment, the differences were statistically significant (P<0.05), and the MMSE score and ADL score of the experimental group were higher than those of the control group, the differences were statistically significant (P<0.05).The total incidence of adverse reactions of the experimental group and control group was compared (9.30% vs.4.65%), the difference was not statistically significant (P>0.05). Conclusion In the treatment of vascular dementia patients, the combination of Donepezil Hydrochloride and Nimodipine has significant clinical effect, can improve the cognitive status of patients, improve their living ability, and has good safety,it is worth recommending.

Objective To analyze the clinical effect of low dose Tirofiban combined with dual antiplatelet therapy in the treatment of progressive cerebral infarction. Methods A total of 46 patients with progressive cerebral infarction admitted to our hospital from January to December in 2018 were selected as the research objects, and they were divided into the control group (23 cases) and the observation group (23 cases) according to the random digital table method.The control group was treated with dual antiplatelet program, and the observation group was treated with low-dose Tirofiban combined with dual antiplatelet program.The scores of national institutes of health stroke scale (NIHSS),Barthel index, hypersensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), modified Rankin scale (mRS) and the incidence of adverse reactions were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05).The NIHSS score of the observation group at 7 days after treatment was lower than that of the control group, the difference was statistically significant (P<0.05).The Barthel index of the observation group at 7 days after treatment was higher than that of the control group, the difference was statistically significant (P<0.05).The levels of hs-CRP and IL-6 in the observation group at 14 days after treatment were lower than those in the control group, the difference was statistically significant (P<0.05).The mRS score of the observation group after treatment was lower than that of the control group, and the difference was statistically significant (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Low dose Tirofiban combined with dual antiplatelet regimen can achieve good therapeutic effect in the treatment of progressive cerebral infarction.

86 Clinical effect of laparoscopic rectal cancer transanal total mesorectal excision and laparoscopic radical surgery in the treatment of rectal cancer

Objective To investigate the clinical effect of laparoscopic rectal cancer transanal total mesorectal excision(TaTME) and laparoscopic radical surgery in the treatment of rectal cancer. Methods A total of 90 patients with rectal cancer who were admitted and treated in our hospital from February 2017 to May 2019 were selected as subjects.According to the random number table method, they were divided into two groups, with 45 cases in each group.Group A was given laparoscopic rectal cancer TaTME method.Group B was given conventional laparoscopic radical resection of rectal cancer.Surgery-related indicators, postoperative anal function, and overall incidence of complications were compared between the two groups. Results There were no statistically significant differences in the surgery time, number of lymph node dissections, and length of surgical specimen removal in group A compared with those in group B (P>0.05).The postoperative exhaust time, the first time for off-bed activity, and the length of hospital stay in group A were shorter than those in group B, the postoperative recovery rate of anal function was higher than that in group B, the blood loss during operation was less than that in group B, and the overall incidence of postoperative complications was lower than that in group B, the differences were statistically significant (P<0.05). Conclusion Compared with laparoscopic radical surgery of rectal cancer, the application of laparoscopic rectal cancer TaTME in the treatment of rectal cancer is safer,which can reduce blood loss during surgery, shorten postoperative recovery time, and promote anal function recovery.

90 Application comparison of laparoscopic cholecystectomy and open surgery in senile patients with acute cholecystitis

Objective To compare and analysis the effect of laparoscopic cholecystectomy and open surgery on the length of stay and complications in senile patients with acute cholecystitis. Methods A total of 58 senile patients with acute cholecystitis who were treated in our hospital from July 2017 to June 2019 were divided into observation group(n=40) and control group (n=18) according to random number table method.Observation group was treated with laparoscopic cholecystectomy, and control group was treated with open surgery.The inflammatory factors before and after surgery, postoperative surgical indexes and complications were compared between two groups. Results After surgery,the level of C-reactive protein (CRP) in observation group was lower than that in the control group, the difference was statistically significant (P<0.05).The recovery time of gastrointestinal function, hospital stays, operation time, postoperative recovery time of bowel sound in observation group were shorter than those in control group, with statistically significant differences (P<0.05).The total incidence of postoperative complications in observation group was lower than that in control group, the difference was statistically significant (P<0.05). Conclusion Compared with conventional open surgery, laparoscopic cholecystectomy can decrease the levels of inflammatory factors, shorten postoperative recovery time, reduce complications, and improve curative effects.

93 Clinical effect of laparoscopic transabdominal preperitoneal hernia repair treating inguinal hernia

Objective To study the effect of laparoscopic transabdominal preperitoneal hernia repair (TAPP) treating inguinal hernia. Methods The clinical data of 147 patients with inguinal hernia admitted to our hospital from January 2016 to January 2019 were analyzed retrospectively.According to different treatment methods, the patients were divided into the endoscopic group (69 cases) and the open group (78 cases).The endoscopic group was treated with TAPP operation, while the open group was treated with tension-free herniorrhaphy with hernia ring filling.The operative time,intraoperative blood loss, postoperative hospital stay, postoperative pain and total incidence of postoperative complications were compared between the two groups. Results There was no significant difference in operative time and intraoperative blood loss between the two groups (P>0.05).The length of postoperative hospital stay in the endoscopic group was shorter than that in the open group, with statistically significant difference (P<0.05).The total incidence of postoperative pain and postoperative complications in the endoscopic group was lower than that in the open group, and the difference was statistically significant (P<0.05). Conclusion Laparoscopic TAPP can be used as the first choice for treatment because of its rapid recovery and fewer complications.

Objective To explore the clinical effect of vagina tightening to preserve vaginal mucosa in the treatment of vaginal relaxation. Methods From July 2017 to July 2019, 60 cases of vaginal relaxation treated in our hospital were selected for retrospective study, they were divided into two groups according to the surgical methods, 30 cases in each group.The control group was treated with traditional vaginal tightening, and the observation group was treated with vaginal tightening to preserve the vaginal mucosa.The operation time, intraoperative bleeding, total incidence of postoperative complications, vaginal relaxation, sexual function score and quality of life score were compared between the two groups. Results There was no significant difference in operation time between the two groups (P>0.05).The amount of bleeding in the observation group was less than that in the control group, the difference was statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was 0.00%, which was lower than 13.33% in the control group, the difference was statistically significant (P<0.05).After treatment, the relaxation degree of vagina in the two groups was better than that before treatment, the difference was statistically significant (P<0.05),and the relaxation degree of vagina in the observation group was better than that in the control group, the difference was statistically significant (P<0.05).After treatment, the scores of sexual function and quality of life in the two groups were higher than those before treatment, with statistically significant differences (P<0.05), the scores of sexual function and quality of life in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). Conclusion The vagina tightening to preserve the vagina mucosa can reduce the surgical trauma of the patients with vagina relaxation, reduce the postoperative complications, improve the vagina relaxation, improve the sexual function, and improve the quality of life.

100 Therapeutic effect of Alendronate Sodium combined with traditional Chinese medicine and physical therapy on osteoporosis

Objective To explore the curative effect of Alendronate Sodium combined with traditional Chinese medicine and physical therapy on osteoporosis. Methods A total of 68 osteoporosis patients in our hospital from January 2017 to February 2019 were selected as the research objects.According to the patient′s medical record tail number, they were divided into the observation group and the control group, 34 cases in each group.The control group was given Calcium Carbonate D3 combined with Alendronate Sodium Tablets for oral treatment.The observation group was given traditional Chinese medicine and physical therapy on the basis of the control group, and the bone turnover, the level of biochemical markers, bone mineral density and the total effective rate of treatment of the two groups were compared. Results After treatment, the level of biochemical markers of bone turnover in the observation group was lower than that in the control group, while the level of bone density was higher than that in the control group, the total effective rate of clinical treatment in the observation group (97.06%) was higher than that in the control group (82.35%), the differences were statistically significant (P<0.05). Conclusion The clinical application of Alendronate Sodium in combination with traditional Chinese medicine and physical therapy for osteoporosis patients has significant effect.It can reduce the level of biochemical markers of bone turnover and improve the level of bone mineral density, improve the quality of life of patients, which is worthy of clinical promotion.

Objective To explore the application effects of closed negative pressure suction technique in the treatment of patients with orthopedic wound and infection wound. Methods From June 2015 to June 2019, 60 patients with orthopedic wound and infection wound were selected and divided into control group and experimental group by computer randomization, 30 cases in each group.The control group was treated by conventional dressing change, and the experimental group was treated by closed negative pressure suction technique.The treatment effect, dressing change, wound healing time and the total incidence rate of complication of the two groups were compared. Results The total effective rate of the experimental group was 93.33%, which was higher than that in the control group (70.00%), the difference was statistically significant (P<0.05).The frequency of dressing changes in the experimental group was less than that in the control group, and wound healing time in the experimental group was shorter than that in the control group, the differences were statistically significant (P<0.05).The total incidence rate of complications in the experimental group(6.67%) was lower than that in the control group (26.67%), and the difference was statistically significant (P<0.05).Conclusion The effect of closed negative pressure suction technique in the treatment of patients with orthopedic wound and infection wound is better, which can shorten the recovery time and reduce the incidence of complications, which is worthy of promoting.

Objective To investigate the clinical effect of locking plate and anatomical plate in the treatment of distal femoral fractures. Methods Sixty-six patients with distal femoral fractures treated in our hospital from November 2016 to November 2018 were selected as the research subjects.They were divided into group Ⅰ(n=33) and group Ⅱ(n=33)according to the coin tossing.In group Ⅰ, locking plate was used, while in group Ⅱ, anatomical plate was used.The total therapeutic effectiveness rate, and surgical indicators were compared between the two groups. Results The total therapeutic effectiveness rate in group Ⅰwas higher than that in group Ⅱ, and the difference was statistically significant (P<0.05).There was no significant difference in the operation time between the two groups (P>0.05).The hospital stay and fracture healing time in group Ⅰwere shorter than those in group Ⅱ (P<0.05).The intraoperative blood loss in group Ⅰwas less than that in group Ⅱ, and the difference was statistically significant (P<0.05).The scores of mental state, activity ability and daily life of group Ⅱwere higher than those of group Ⅰ, and the differences were statistically significant (P<0.05). Conclusion Compared with the anatomical plate, the locking plate is more effective for the treatment of distal femoral fractures.The surgical trauma is milder and the hospital stay is much shorter, which is conducive to rapid recovery after surgery.

Objective To explore the clinical effect of Platelet-rich Plasma Injection in the treatment of heel pain.Methods Fifty patients with heel pain treated in Shenzhen People′s Hospital from October 2017 to October 2019 were selected and divided into two groups according to the random lottery method, 25 cases in each group.The control group was treated with a needle knife and the observation group added Platelet-rich Plasma Injection therapy on the basis of the control group, and the patient′s treatment effect, visual analogue score (VAS) score and ankle-hind foot score were compared between two groups. Results The total effective rate of treatment in the observation group (96.00%) was higher than that in the control group (76.00%), the difference was statistically significant (P<0.05).There was no statistical difference in the VAS score of the two groups of patients before treatment, and the VAS score of the observation group was lower than that of the control group after treatment, the difference was statistically significant (P<0.05).There was no significant difference in the ankle posterior foot system (AOFAS) score in two groups (P>0.05).After treatment, the scores of pain, ground walking, maximum walking distance (blocks),functions and autonomous activities,support,forward and backward activities,abnormal gait in the observation group were higher than those in the control group(P<0.05). Conclusion Platelet-rich Plasma Injection has a good effect in treating heel pain, which can effectively alleviate pain and improve the ankle-hindock function of patients, which is suitable for clinical promotion.

Objective To explore the clinical effect of biomimetic electrical stimulation technology in the treatment of perimenopausal syndrome. Methods Totally 90 patients with perimenopausal syndrome diagnosed and treated in our outpatient clinic from May 2016 to June 2018 were selected as research subjects.According to the method of drawing lots, they were divided into three groups:experimental group, control group and blank control group, with 30 cases in each group.The experimental group was treated with biomimetic electrical stimulation, the control group was treated with hormone replacement therapy, and the blank control group was treated with expectant treatment.The levels of follicle-stimulating hormone (FSH), estradiol (E2) and anti-Mullerian hormone (AMH) before and after treatment and the clinical efficacy were compared among the three groups. Results The hormone levels (FSH, E2 and AMH) of the three groups before treatment were compared, and the differences were not statistically significant (P >0.05).After treatment, the FSH levels in the experimental group and the control group were lower than those before treatment, while the E2 level was higher than that before treatment with statistical significances (P<0.05).After treatment, the levels of FSH and E2 in patients of the blank control group were not significantly different from those before treatment (P>0.05).The difference in the AMH level of the experimental group and the control group before and after treatment had no statistical significance (P>0.05).In the blank control group, the AMH level after treatment was lower than that before treatment, and the difference was statistically significant (P<0.05).After treatment, the FSH level in the experimental group and the control group was lower than that of the blank control group, and the levels of E2 and AMH were higher than those of the blank control group with statistical significances (P<0.05).The differences in the FSH, E2, and AMH levels after treatment between the experimental group and the control group had no statistical significance (P>0.05).The total therapeutic effectiveness rates in the experimental group and the control group were both higher than those of the blank control group, and the differences were statistically significant (P<0.05).The difference in the total therapeutic effectiveness rate between the experimental group and the control group had no statistical significance (P>0.05).Conclusion The biomimetic electrical stimulation technology used in the treatment of perimenopausal syndrome has the same effect as the hormone replacement therapy.It is non-invasive and painless, which is worthy of clinical promotion.Due to limited study time,long-term effect needs to be tracked.

117 Application of Z-type injection in pregnant women with subcutaneous injection of insulin

Objective To investigate the application of Z-type injection in pregnant women who injected insulin subcutaneously. Methods Thirty pregnant women with gestational diabetes mellitus who had been in the department of outpatient of our hospital for delivery and hospitalization for more than 28 weeks from May 2018 to January 2019 were selected as the study subjects.Pregnant women were randomly divided into control group and study group according to parity, with 15 cases in each group.In the control group, traditional injection was used, while in the study group, a new Z-type injection method was adopted.The pain level, heart rate, and total incidence of adverse reactions (effusion,redness) were compared between the two groups. Results The pain in the study group was milder than that in the control group, the heart rate was slower than that in the control group, the differences were statistically significant (P<0.05).The total incidence of adverse reactions in the study group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Subcutaneous injection of insulin to pregnant women by Ztype injection can alleviate the pain at the injection site, reduce the heart rate of pregnant women and the local adverse reaction rate, and improve the quality of clinical care.

120 Application effect research of cervical dilatation balloon in term pregnancy induction

Objective To investigate the application effect of cervical dilatation balloon in term pregnancy induction.Methods Fifty pregnant women undergoing term pregnancy induction from January 2016 to December 2017 were selected as subjects.By the random number table method, they were divided into the control group and the observation group, with 25 cases in each group.In the control group, a small dose of Oxytocin was used to induce labor by intravenous drip, and in the observation group, a cervical dilation balloon was used.The labor stage, neonatal Apgar score,and bleeding at 24 hours after surgery were compared between the two groups.In addition, Bishop′s cervical maturity score, the rates of vaginal delivery and cesarean delivery were compared between the two groups. Results In the observation group, the first and total stages of labor were shorter than those in the control group, the neonatal Apgar score was higher than that in the control group, the amount of bleeding at 24 hours after delivery was less than that in the control group, the differences were statistically significant (P<0.05).The vaginal delivery rate in the observation group was higher than that in the control group, the cesarean delivery rate was lower than that in the control group, and Bishop′s cervical maturity score was higher than that in the control group, the differences were statistically significant (P<0.05). Conclusion The use of cervical dilatation balloon in induction of labor in term pregnancy can reduce the cesarean section rate, significantly shorten the labor stage, and can greatly improve the vital signs of newborns.The effect is favorable, and it is worth popularizing and applying.

123 Application of health education measures of Maslow′s hierarchy of needs theory in patients with secondary cesarean section

Objective To explore the application of health education measures of Maslow′s hierarchy of needs theory in patients with secondary cesarean section. Methods Sixty patients with secondary cesarean section admitted to our hospital from February 2017 to February 2019 were selected as study subjects.According to the random number table method, the patients were divided into study group (30 cases) and control group (30 cases).Control group was given routine nursing, while study group was given health education measures of Maslow′s hierarchy of needs theory.The psychological status (Chinese version of profile of mood states [POMS]), cognitive status (Montreal cognitive assessment scale [MoCA]) and self-esteem level (Rosenberg self-esteem scale [SES]) were compared between the two groups before health education and at 5 d after operation. Results At 5 d after operation, the POMS score in study group was lower than that before education and that in control group, the differences were statistically significant (P<0.05).At 5 d after operation, the MoCA scores in the two groups were higher than those before education, and the MoCA score in study group was higher than that in control group, the differences were statistically significant (P<0.05).The SES score in study group at 5 d after operation was higher than that before education and that in control group, the differences were statistically significant (P<0.05). Conclusion Health education measures of Maslow′s hierarchy of needs theory can effectively improve psychological status and enhance the cognition and self-esteem level.

126 Clinical application effect comparison of hysteroscopy combined with Dydrogesterone or Mirena in the treatment of perimenopausal abnormal uterine bleeding

Objective To explore the clinical application effect of hysteroscopy combined with Dydrogesterone or Mirena in the treatment of perimenopausal abnormal uterine bleeding. Methods A total of 102 patients with perimenopausal abnormal uterine bleeding who were treated in our hospital from January 2016 to June 2018 were selected as study subjects, and they were divided into Mirena group and Dydrogesterone group according to the random number table method, with 51 cases in each group.Dydrogesterone group was given hysteroscopy combined with Dydrogesterone while Mirena group was given hysteroscopy combined with Mirena.The disease-related indexes (endometrial thickness,hemoglobin concentration, pictorial blood loss assessment chart [PBAC]score) before and after treatment and incidence rate of adverse reactions during treatment were compared between the two groups, and the long-term efficacy after 1 year of follow-up was recorded in the two groups. Results Before treatment, there were no significant differences in the endometrial thickness, hemoglobin concentration and PBAC score between the two groups (P>0.05).After 3 months of treatment, the endometrial thickness and PBAC score in the two groups were lower than those before treatment, the difference was statistically significant (P<0.05), and the endometrial thickness and PBAC score in the Mirena group were lower than those in Dydrogesterone group, the differences were statistically significant (P<0.05).After 3 months of treatment, the hemoglobin concentrations in the two groups were higher than those before treatment, the differences were statistically significant (P<0.05), and the hemoglobin concentration in the Mirena group was high than that in Dydrogesterone group, the difference was statistically significant (P<0.05).The incidence rate of vaginal spotting bleeding in Mirena group was 15.7%, which was higher than that in Dydrogesterone group (2.0%), the difference was statistically significant (P<0.05), and there were no statistically significant differences in the incidence rates of other adverse reactions and the total incidence (P>0.05).After 1 year of follow-up, the clinical efficacy of the Mirena group was better than that of the Diquprogesterone group, and the total effective rate of the Mirena group was higher than that of the Diquprogesterone group, with statistically significant differences (P<0.05).Conclusion Hysteroscopy combined with Mirena is better than Dydrogesterone in the treatment of menopausal dysfunctional uterine bleeding, and it is prone to vaginal spotting bleeding symptoms

130 Effect of midwife-led pregnancy care services on maternal weight and maternal and child outcomes during pregnancy

Objective To explore the effect of midwives-led prenatal health care services on maternal weight during pregnancy and delivery outcomes. Methods A total of 120 cases of puerpera who had regular maternity examination in our hospital from June 2017 to June 2018 were selected and divided into two groups by random number table method,60 cases in each group.The control group received routine prenatal health guidance and accompanying care, while the research group received midwives-led prenatal health care services.The maternal weight control during pregnancy and delivery outcomes were compared between the two groups. Results The weight before delivery and the mid-term increase in the third trimester of pregnancy in the research group were lower than those in the control group, the differences were statistically significant (P<0.05); the rate of cesarean section and total complications of mothers and infants in the research group were lower than those of the control group, and the rate of natural delivery was higher than that of the control group, the differences were statistically significant (P<0.05). Conclusion Midwives-led pregnant health care services can enhance the effect of maternal weight control during pregnancy, help to reduce the incidence of maternal and infant complications, and improve the birth rate.

133 Effect of nutrition and health education combined with dietary guidance on maternal and infant outcomes in single neonates during pregnancy

Objective To explore the effect of nutrition and health education combined with dietary guidance on the outcome of single birth primipara. Methods From August 2018 to July 2019, 90 cases of single birth pregnant women in our hospital were selected as the study objects, and divided into control group and observation group according to random number table method, 45 cases in each group.The control group received routine nutritional guidance before pregnancy, and the observation group received nutritional health education combined with dietary guidance.The weight growth, delivery mode, pregnancy complications and newborn conditions of the two groups were compared. Results The body mass index (BMI) of pregnant women in the two groups was higher than that before pregnancy, the differences were statistically significant (P<0.05).The total incidence of pregnancy complications (13.3%) and cesarean section rate(11.1%) in the observation group were lower than those in the control group (22.2%, 51.1%), and the differences were statistically significant (P<0.05).There was no significant difference in the incidence of low-weight infants in the observation group (15.6%) compared with the control group (20.0%) (P>0.05); the incidence of preterm infants in the observation group (8.9%) was lower than that in the control group (20.0 %), the difference was statistically significant (P<0.05). Conclusion Carrying out nutrition and health education and dietary guidance during pregnancy has a good effect on weight control of single parturient women and improves maternal and infant outcomes, effectively preventing adverse pregnancy outcomes.

136 Effect of intervention in the second and third trimester of gestational diabetes mellitus on the outcome of mother and infant

Objective To explore the effect of intervention in the second and third trimester of gestational diabetes mellitus (GDM) on the outcome of mother and infant. Methods From July 1, 2017 to June 1, 2019, 600 patients with gestational diabetes were enrolled in our hospital.According to the gestational group, the second trimester group (28-32 weeks, 300 cases) and the third trimester group (>32 weeks, 300 cases) took targeted interventions and followed up the patients to production.The complications, neonatal complications, Apgar score and delivery mode were compared between the two groups. Results The total incidence of complications in the intervention period of patients in the second trimester group was 14.00%, lower than 41.00% in the third trimester group, the total incidence of neonatal complications in the second trimester group was 3.33%, the conversion rate was 5.00%, lower than 23.00% and 20.00% in the third trimester group, and the Apgar score of newborns in the second trimester group was higher than that in the third trimester group, the natural delivery rate of patients in the second trimester group was 60.00%, higher than that in the third trimester group, the differences were statistically significant (P<0.001). Conclusion Health education, diet, medication and other interventions in the second trimester of pregnancy can significantly reduce the complications after intervention, improve the outcome of mother and infant, improve the natural delivery rate and Apgar score of newborn,and reduce the rate of neonatal transfer.The effect is ideal.

139 Effect of predictive nursing on reducing postpartum hemorrhage of parturients in vaginal delivery and the influence on maternal quality of life score

Objective To investigate the effect of predictive nursing on reducing postpartum hemorrhage in vaginal delivery and the influence on maternal quality of life (QOL) score. Methods A total of 60 vaginal delivery women who were admitted to our hospital from April to June in 2019 were selected and randomly divided into the control group and the observation group according to odd even groupind method, and 30 patients in each group.The control group was given routine nursing.On this basis, the observation group adopted predictive nursing.The postpartum at 2, 24 h bleeding, QOL score and total nursing satisfaction of the two groups of women were compared. Results The neonatal asphyxia score in the observation group was (9.65±0.36) points, which was higher than that of the control group ([9.02±0.28]points), the difference was statistically significant (P<0.05).The postpartum hemorrhage volume at 2 and 24 hours in the observation group were (134.58±16.35) and (181.76±16.67) ml, which were less than those in the control group([261.24±22.19], [302.34±34.75]ml), the differences were statistically significant (P<0.05).The cognitive function score of the observation group was (84.31±5.49) points, and the role function score of the observation group (79.91±6.48)points, the score of physical functions of the observation group was (78.09±4.72) points, and the social function score of the observation group was(82.51±5.56) points, which were higher than those of the control group ([66.58±4.62], [63.42±5.67], [64.59±4.61], [68.62±5.32]points), the differences were statistically significant (P<0.05).The total nursing satisfaction of the observation group was 100.00%, which was higher than that of the control group (80.00%), the difference was statistically significant (P <0.05). Conclusion The application of predictive nursing can effectively reduce the amount of postpartum hemorrhage in vaginal delivery, reduce the risk of neonatal asphyxia, and improve the quality of maternal life.

142 Analysis of the influencing factors of poor drug response of benign epilepsy of childhood with centrotemporal spikes

Objective To analyze the clinical features and electroencephalogram (EEG) characteristics of benign epilepsy of childhood with centrotemporal spikes (BECT), and to discuss the influencing factors of poor drug response. Methods A retrospective analysis was conducted on the clinical data (including low age of onset, frequency of seizures, type of seizures, time of seizures, history of fever seizure, and family history of convulsions) and EEG characteristics (including spur/spike [spine/slow complex wave]discharge, discharge period, discharge generalization) of 101 children newly diagnosed with BECT in our hospital from January 2016 to December 2018.All subjects were routinely treated with antiepileptic drugs, and followed up for at least one year, including control of seizure, new form of seizure and evolution of EEG.The subjects were then divided into seizure free group (83 cases) and poor drug response group (18 cases) according to whether there were seizures after one year of treatment.The differences in clinical characteristics and EEG characteristics before treatment between the two groups as well as the factors influencing poor drug response were analyzed. Results There were statistically significant differences in the low age of onset, frequency of seizures, and type of seizures between the two groups (P<0.05).There were no statistically significant differences in time of seizures, history of fever seizure, and family history of convulsions between the two groups (P>0.05).Multivariate Logistic regression analysis showed that the frequency of seizures was the influencing factor of poor drug response (P<0.05). Conclusion BECT generally has a good prognosis, and its drug response is related to low age of onset (<5 years old), the frequency of seizures, and the type of seizures, among them, the frequency of seizures is an influential factor for poor drug response.

146 Application effect of head and face touch intervention on reducing neonatal crying

Objective To explore the effect of head and face touch intervention on reducing neonatal crying. Methods A total of 200 full-term newborns delivered by cesarean section in our hospital from February to June 2019 were selected as the research objects, and they were divided into four groups of A, B, C and D group by random number table method, 50 cases in each group.Foot touch intervention was used in group A, chest touch intervention was used in group B, hand touch intervention was used in group C, and head and face touch intervention was used in group D.Each part was touched for 8 times, with about 10 s each time.The changes of heart rate and respiratory rate before and after intervention and the total effective rate were compared. Results After touching intervention, the heart rate and respiratory rate of neonates in group D were lower than those before the intervention, and the differences were statistically significant (P<0.05).After touching intervention, the heart rate and respiratory rate of neonates in group D were lower than those in groups A, B and C, with statistically significant differences (P<0.05).There were no statistically significant differences in the heart rate and respiratory rate of neonates in groups A, B and C after touching intervention compared with those before the intervention (P>0.05).After intervention, the total effective rate of neonatal touching in group D was higher than that in group A, B and C, and the differences were statistically significant (P<0.05). Conclusion Touch intervention on the head and face can quickly and effectively reduce crying for neonates, generate a good physiological response with easy operation, so it is worthy of promotion and application.

150 Effect of low dose Methylprednisolone combined with Azithromycin in the treatment of refractory Mycoplasma pneumoniae pneumonia in children

Objective To investigate the effect and safety of low dose Methylprednisolone combined with Azithromycin in the treatment of refractory Mycoplasma pneumoniae pneumonia (RMPP) in children. Methods A total of 84 children with RMPP treated in our hospital from December 2014 to August 2018 were selected as the study subjects.According to the random number table method, the children were divided into observation group (n=43) and control group (n=41).The children in the two groups were given expectoration, oxygen inhalation, maintenance of water electrolyte and acid-base balance.Azithromycin was given intravenously in the control group, and low dose Methylprednisolone was given in the observation group on the basis of the control group for 7 d.The time of hospitalization, the recovery time of body temperature, the level of C-reactive protein (CRP) before and after treatment and effect in the two groups were compared. Results The time of hospitalization and recovery of body temperature in the observation group were shorter than those in the control group, and the differences were statistically significant (P<0.05).There was no significant difference in CRP level between two groups before treatment (P>0.05); the CRP level in the observation group was lower than that in the control group after one week of treatment, and the difference was statistically significant (P<0.05).The total effective rate in the observation group was 93.0% (40/43), which was higher than 75.6% (31/41) in the control group (χ2=4.87, P<0.05).In the observation group, there was one case of facial flushand and two cases of mild euphoria, but no obvious adverse reactions were observed.In the control group, there was no significant facial flush and euphoria, and the difference between the two groups was not statistically significant (χ2=2.97, P>0.05). Conclusion Lowdose Methylprednisolone combined with Azithromycin in the treatment of children with RMPP can effectively shorten the recovery time of body temperature and improve the effect without obvious adverse reactions.Therefore, it is recommended to use in children with RMPP.

153 Clinical effect comparison between single-port laparoscopy and small inguinal incision in the treatment of pediatric hydrocele

Objective To compare the clinical effect between single-port laparoscopy and small inguinal incision in the treatment of pediatric hydrocele. Methods The clinical data of 633 children with hydrocele admitted to our hospital from June 2014 to May 2018 were retrospectively analyzed.According to the inclusion and exclusion criteria,546 children were finally included.They were divided into laparoscopic group (303 cases) and open operation group (243 cases) according to different treatment methods.The single-port laparoscopic high ligation of double-line sheath process was performed in the laparoscopic group, while the small inguinal incision was performed in the open operation group.The operation time, hospitalization time, intraoperative blood loss, hospitalization expense, total incidence rate of postoperative complications and recurrence rate of the two groups were compared. Results The operation time (unilateral [17.9±2.5]min, bilateral [27.7±2.6]min) of the laparoscopic group was shorter than that of the open operation group (unilateral [24.9±3.3]min, bilateral [40.8±3.1]min), the hospitalization time ([2.6±0.7]d) was shorter than that of the open operation group ([3.9±1.6]d), and the intraoperative blood loss ([1.2±0.4]ml) was less than that of the open operation group([2.2±1.0]ml),the hospitalization expense([6076.5±496.6]yuan)was higher than that of the open operation group([3665.1±444.3]yuan), and the total incidence rate of postoperative complications (0.7%) was lower than that of the open operation group (11.9%), the differences were statistically significant (P<0.05).After 1 year of follow-up, the recurrence rate in the laparoscopic group (0.7%) was not significantly different from that in the open operation group (2.1%) (P>0.05).Conclusion The single-port laparoscopic high ligation of double-line sheath process has obvious advantages over small inguinal incision in terms of effect and safety for pediatric hydrocele, and it is worthy of clinical promotion.

157 Clinical effect of Ferrous Lactate Syrup in the treatment of children with iron deficiency anemia

Objective To observe the clinical effect of Ferrous Lactate Syrup in the treatment of iron deficiency anemia(IDA) in children. Methods A total of 76 children with IDA treated in our hospital from January 2018 to March 2019 were divided into two groups by the random number table method, 38 cases in each group.The control group received Ferrous Succinate Tablets, the observation group received Ferrous Lactate Syrup treatment.The clinical efficacy, parameters of peripheral red blood cells (hemoglobin [Hb], mean corpuscular volume [MCV]), iron metabolism indexes(serum ferritin [SF], serum iron [SI]), total incidence of adverse reactions and recurrence rate were compared between the two groups. Results The total effective rate of the observation group (92.10%) was higher than that of the control group (73.68%), the recurrence rate (5.26%) was lower than that of the control group (21.05%), the differences were statistically significant (P<0.05).There were no significant differences in the levels of Hb, SF, MCV and Si between the two groups before treatment (P>0.05).After treatment, the Hb in observation group was (119.69±6.28) g/L, SF was(38.97±4.08) μg/L, MCV was (90.36±8.35) fl and Si was (15.16±2.38) μmol/L, higher than the control group of (102.64±5.49) g/L, (30.75±3.51) μg/L, (73.84±8.72) fl, (10.94±3.81) μmol/L, the differences were statistically significant (P<0.05).There was no significant difference in the total incidence of adverse reactions between the observation group and control group (5.26% vs.10.53%) (P>0.05). Conclusion Ferrous Lactate Syrup treatment is safe and feasible for IDA children, which is beneficial to improving the level of Hb, SF, MCV, Si, correcting anemia symptoms, and has a low recurrence rate.

Objective To explore the clinical effect of tympanic injection of Ambroxol Hydrochloride in the treatment of secretory otitis media. Methods A total of 120 patients with secretory otitis media treated in our hospital from June 22, 2017 to June 22, 2018 were selected as the research subjects.They were divided into observation group (n=60) and control group (n=60) according to the randomized method of drawing lots.Patients in the control group were treated with tympanic injection of dexamethasone, and in the observation group, they were treated with tympanic injection of Ambroxol Hydrochloride.The pure tone hearing threshold to measure the improvement of air conduction hearing level,speech frequency air conduction threshold, tympanic pressure, total complication rate, and total effective rate were compared in the two groups. Results The pure tone hearing thresholds to measure the improvement of air conduction hearing level in the observation group at 0.5, 1 and 2 kHz were higher than those of the control group, and the differences were statistically significant (P<0.05).The speech frequency air conduction threshold and tympanic pressure of the observation group after treatment were higher than those of the control group, and the differences were statistically significant (P<0.05).The total therapeutic effective rate in the observation group was higher than that of the control group,and the total incidence of complications was lower than that of the control group with statistical significances (P<0.05).Conclusion The treatment of Ambroxol Hydrochloride in patients, with secretory otitis media has a remarkable effect.It can increase the pure tone hearing threshold to measure the level of air conduction hearing and reduce the incidence of complications, and is worthy of widespread promotion.

Objective To analyze the clinical effect of Tobramycin Dexamethasone Eye Drops combined with Yupingfeng Granules in treating children with allergic conjunctivitis. Methods A total of 80 children with allergic conjunctivitis who were treated in our hospital from October 2017 to November 2018 were selected as the research objects.They were divided into control group(40 cases)and experimental group(40 cases),according to the random number table method.The control group was treated with Tobramycin Dexamethasone Eye Drops, and the experimental group was treated combined with Yupingfeng Granules.The treatment efficacy, recurrence rate, disease symptom and signs score,quality of life, and complications were compared between the two groups of children. Results The total clinical treatment efficiency of the experimental group was higher than that of the control group, and the difference was statistically significant (P<0.05).The recurrence rate of the experimental group was lower than that of the control group at one week(P<0.05).The symptoms and signs score, quality of life score, and total rate of complications in the experimental group were better than those in the control group, and the differences were statistically significant (P<0.05). Conclusion The combination of Tobramycin Dexamethasone Eye Drops combined with Yupingfeng Granules has a clear effect on allergic conjunctivitis, and has good clinical efficacy.At the same time, fewer adverse reactions occur, and children have better compliance.The combination method can be used as the first choice for clinical treatment of allergic conjunctivitis.

Objective To explore the application of Estradiol Valproate Tablets/Escitalopram Tablets and Escitalopram in the treatment of perimenopausal depression. Methods A total of 60 patients with perimenopausal depression in Ganzhou Third People′s Hospital from January 2016 to December 2017 were selected as the subjects of the study.According to the random number table method, the patients were divided into the study group (30 cases) and the control group (30 cases).The study group was treated with Estradiol Valproate Tablets/Escitalopram Tablets + Escitalopram Tablets, while the control group was treated with Escitalopram.The Hamilton depression scale (HAMD), therapeutic effect and adverse reactions were compared between the two groups. Results The HAMD scores of the study group at the 4th, 8th and 12th weeks after treatment were lower than those of the control group, the differences were statistically significant (P<0.05).The total effective rate of the study group was higher than that of the control group, the difference was statistically significant (P<0.05).The total incidence of adverse reactions in the study group was lower than that in the control group, the difference was statistically significant (P<0.05). Conclusion Estradiol Valerate Tablets/Escitalopram Tablets combined with Escitalopram are effective in the treatment of permenopausal depression, which can reduce the depression status of patients, improve the therapeutic effect, and have a lower incidence of adverse reactions.

169 Comparative analysis on distribution and drug sensitivity of pathogenic bacteria in our hospital for four years

Objective To understand the distribution of pathogenic bacteria and the change of drug-resistant bacteria in our hospital, and sort out the monitoring data of antimicrobial resistance in the past four years, so as to provide the basis for clinical rational selection of antimicrobial drugs. Methods The monitoring data of drug resistance every quarter from 2016 to 2019 in our hospital were collected, including the distribution of main pathogenic bacteria, the drug sensitivity of main Gram-negative bacteria, the detection of multiple drug-resistant bacteria, etc.The annual data of the detection were sorted out and statistically analyzed to find out the changes of bacterial drug resistance rate, included main Enterobacteriaceae bacteria, Extended-Spectrum β-Lactamases (ESBL), and the drug sensitivity of main Gram-negative bacteria. Results A total of 24 430 strains were collected in the past four years, the main Gram-positive bacteria accounted for 26.52%, the first three were Coagulase negative staphylococcus, Enterococcus and Staphylococcus aureus.The main Gram-negative bacteria accounted for 42.40%, the first three were Escherichia coli, Pseudomonas aeruginosa and Klebsiella pneumonia.The detection rate of ESBL produced by Klebsiella pneumoniae increased significantly.The sensitivity of Imipenem-Cilastatin increased in Pseudomonas aeruginosa significantly, with 97.75% in recent years.However, in Enterobacteriaceae bacteria producing ESBL and Acinetobacter baumannii, the sensitivity of Imipenem-Cilastatin decreased significantly.In Pseudomonas aeruginosa, the sensitivity of Imipenem-Cilastatin increased significantly. Conclusion The detection rate of drug-resistant bacteria in hospital is still increasing, especially the drug resistance of Acinetobacter baumannii can′t be ignored.It is necessary to strengthen the monitoring of bacterial drug resistance, improve the management measures of drug-resistant bacteria such as hand hygiene management, establish multi-disciplinary professional cooperative management, and promote the rational application of antibiotics.

173 Application effect of Atorvastatin Calcium combined with Clopidogrel in patients with acute cerebral infarction

Objective To explore the application effect of Atorvastatin Calcium combined with Clopidogrel in the treatment of patients with acute cerebral infarction. Methods A total of 120 patients with acute cerebral infarction treated in our hospital from June 2017 to March 2018 were selected as research subjects.They were divided into the control group (n=60) and the observation group (n=60) by the random number table methods.Patients in the control group were treated with Clopidogrel, and in the observation group, they were treated with Atorvastatin Calcium combined with Clopidogrel.The inflammatory factors, the degree of neurological impairment and the total effectiveness rate were compared between the two groups. Results After two weeks of treatment, the levels of interleukin-6 (IL-6), tumor necrosis factor-α ( TNF-α) and high-sensitivity C-reactive protein ( hs-CRP) in the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05).The neurological impairment score in the observation group was lower than that of the control group, the difference was statistically significant (P<0.05).The total effectiveness rate in the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The combination of Atorvastatin Calcium and Clopidogrel in the treatment of patients with acute cerebral infarction can significantly reduce inflammatory factors and alleviate neurological impairment,which is worthy of clinical promotion.

176 Application effect of Huatan Tiaojing Decoction combined with Western medicine therapy in polycystic ovary syndrome with kidney deficiency and phlegm-dampness syndrome

Objective To investigate the application effect of Huatan Tiaojing Decoction combined with Western medicine therapy in polycystic ovary syndrome (PCOS) with kidney deficiency and phlegm-dampness syndrome.Methods A total of 70 patients with PCOS who visited our hospital from March 2017 to June 2019 were divided into two groups according to random number table method, 35 cases in each group.The control group was treated with Ethinylestradiol Cyproterone Tablets, and the test group was treated with Huatan Tiaojing Decoction based on the control group.The clinical effect, sex hormone level, polycystic change of ovary and the occurrence of adverse reactions were compared between the two groups. Results The total effective rate of treatment in the experimental group was 94.29%, which was higher than that of control group (74.29%), the difference was statistically significant (P<0.05).The traditional Chinese medicine score of the experimental group was (4.13±1.21) points, which was lower than that of the control group for (8.36±2.27) points (P<0.05).There was no significant difference in the level of follicle stimulating hormone (FSH) between the two groups after treatment (P>0.05).The level of estradiol (E2) in the experimental group was higher than that in the control group, while the levels of luteinizing hormone (LH) and testosterone (T) in the experimental group were lower than those in the control group, the differences were statistically significant (P<0.05).No significant adverse reactions were observed in the two groups. Conclusion The treatment of PCOS patients with oral Huatan Tiaojing Decoction combined with Western medicine is safe, can effectively improve the clinical symptoms and signs of patients, regulate hormone levels, promote the recovery of the disease, it is worth clinical promotion.

179 Clinical study of Tongluo Lotion bath treating limb dysfunction in patients with ischemic stroke

Objective To investigate the clinical effect of Tongluo Lotion bath treating limb dysfunction in patients with ischemic stroke. Methods A total of 80 cases with limb dysfunction after ischemic stroke admitted to our hospital from January 2018 to January 2019 were selected and divided into two groups with 40 cases in each group according to the random number table method.The control group received rehabilitation treatment and mainly intervened by placing good limbs, while the observation group received intervention combined with Tongluo Lotion.All the participants were followed up for 6 months, and the ankle joint and shoulder joint mobility after 6 months of intervention were compared between the two groups.The muscle strength of upper and lower limbs of the two groups were compared after 6 months of intervention.The joint-related complications 6 months after intervention were compared between the two groups.The changes of modified Edinburgh-Scandinavian stroke scale (MESSS) score and Barthel index of overall quality of life after intervention were compared. Results After 6 months of follow-up, the ankle joint activity in the observation group was higher than that in the control group, and the internal rotation, external rotation and lifting angles of the shoulder joint were all higher than those in the control group (P<0.05).The proportion of upper and lower limb muscle strength in the observation group was higher than that in the control group (P<0.05).The proportions of shoulder subluxation,hip subluxation, joint contracture and joint ankylosis in the observation group were all lower than those in the control group (P<0.05).The MESSS score in observation group was lower than that in control group (P<0.05), the Barthel index was higher than that in control group (P<0.05). Conclusion Bath with Tongluo Lotion on the basis of rehabilitation therapy for hemiplegic patients with ischemic stroke can effectively improve the patient′s function, improve limb muscle strength, reduce joint-related complications, promote the recovery of neurological function and improve the quality of life of patients.

183 Value of microcolumn gel immunoassay technology in the identification of clinical blood types and ensuring the safety of blood transfusion

Objective To explore the value of microcolumn gel immunoassay for identifying clinical blood type and ensuring blood transfusion safety. Methods A total of 100 patients who underwent transfusion therapy from February 2017 to February 2019 in Fengcheng People′s Hospital and Zhangshu People′s Hospital were enrolled in the study.According to the random number table method, they were divided into the saline test tube method group (50 cases) and the microcolumn gel immunoassay group ( 50 cases).The saline test tube method was detected by saline test tube method group, and the microcolumn gel immunoassay method was detected by microcolumn gel immunoassay group.The positive and negative stereotypes, positive and negative stereotypes, cross-matching success rate, cross-matching failure rate, false positive rate, detection sensitivity, specificity, transfusion accident rate and blood transfusion service quality score data indicators were compared between the two detection methods. Results There were no significant differences in the positive and negative conformity rates and positive and negative stereotypes of the two detection methods (P>0.05).The cross-matching success rate of microcolumn gel immunoassay of the microcolumn gel immunoassay method was higher than that of the saline test tube method, there was statistically significant difference (P<0.05).The crossmatching failure rate of the microcolumn gel immunoassay method was lower than that of the saline test tube method,the difference was statistically significant (P<0.05).There was no significant difference in the detection sensitivity and specificity of the examination method between the microcolumn gel immunoassay method and saline test tube method(P>0.05).The blood transfusion service quality score of the microcolumn gel immunoassay method was higher than that of the saline test tube method, and the difference was statistically significant (P<0.05).The incidence of transfusion accidents in the microcolumn gel immunoassay method was lower than that in the saline test tube method, and the difference was statistically significant (P<0.05). Conclusion Microcolumn gel immunoassay identification method can effectively reduce the incidence of transfusion accidents and improve the quality of blood transfusion services.

186 Correlation analysis of serum estradiol level with total cholesterol and triglyceride level in perimenopausal women

Objective To explore the expression and clinical significance of serum estradiol (E), total cholesterol (TC)2 and triacylglycerol (TG) in perimenopausal women. Methods A total of 125 perimenopausal women admitted to the Fourth Hospital of Xinyu City in Jiangxi Province from January to July in 2019 were selected as the observation group,and 100 healthy women who had physical examination in the Fourth Hospital of Xinyu City in Jiangxi Province in the same period were selected as the control group.The levels of E2, TC and TG were compared between the two groups,and the correlation between E2 and TC, TG was analyzed. Results The E2 level of the observation group was (54.7±9.5)ng/L, which was lower than that of the control group ([97.8±10.6]ng/L), the difference was statistically significant (P<0.05).The level of TC of the observation group was (5.4±0.6) mmol/L, the level of TG of the observation group was (1.2±0.4)mmol/L, which were higher than those of the control group ([4.8±0.7], [0.9±0.5]mmol/L), the differences were statistically significant (P<0.05).The level of E2 and TC、TG were significant negative correlation (r=-0.812, -0.513,P<0.05).Conclusion The E2 level and TC, TG levels of women in perimenopause are significant negative correlation, which could provide basis for guiding clinical treatment.

189 Analysis of influence factors of biochemical pre-test process on test quality

Objective To explore the influence of pre-clinical biochemical examination process on test quality and analyze its main influencing factors. Methods The clinical data of 368 patients undergoing blood biochemical test in our hospital from February 2017 to March 2018 were analyzed retrospectively.the deviation of test results, the incidence of errors and the main influencing factors of errors were observed and analyzed. Results After 368 patients underwent blood biochemical test, 46 patients had errors, the incidence rate was 12.50%, there was no significant difference in sex between the two groups (P>0.05).The smoking history, posture (standing posture), sample hemolysis, collection time(feeding time≤12 h) and drug use in the error group were higher than those in the error-free group, the differences were statistically significant (P<0.05).Logistic regression analysis showed that smoking history, sample hemolysis, collection time (feeding time≤12 h), drug use and posture (station) were the influencing factors of biochemical test results(OR>1, P<0.05). Conclusion The pre-biochemical process has a great influence on the quality of clinical examination,and the quality of clinical examination should be controlled actively, which is beneficial to improving the accuracy of clinical examination results.

192 Application of PDCA cycle in the management of adverse events of nondrug therapy in traditional Chinese Medicine

Objective To explore the application effect of PDCA cycle in the management of adverse events of nondrug therapy in traditional Chinese Medicine. Methods The adverse events of non-drug therapy of traditional Chinese medicine in the Third People′s Hospital of Jiangmen City from January to June 2017 were analyzed retrospectively, and PDCA cycle was applied for management from January to June 2018.After PDCA management, the incidence of adverse events of non-drug therapy in traditional Chinese medicine was analyzed and compared with that before PDCA management.At the same time, the total satisfaction of patients before and after PDCA management was counted. Results The incidence rate of adverse events of non-drug therapy of traditional Chinese medicine after PDCA management was lower than that before PDCA management, and the difference was statistically significant (P<0.05).The patients′ total satisfaction after PDCA management was higher than that before PDCA management, and the difference was statistically significant (P<0.05). Conclusion PDCA cycle management method can reduce the incidence of adverse events in non-drug therapy of traditional Chinese medicine, and improve satisfaction of patients.

Objective To explore the application of test performance and quality control of diagnostic items in the physical examination of our hospital. Methods The data of 1000 physical examination people admitted to our hospital clinical laboratory physical examination from June 2017 to June 2018 were retrospectively statistically analyzed.The 500 cases from June to December 2017 before quality control management were set as the control group, the 500 cases from January to June after quality control management were set as the study group, The total satisfaction, disease detection accuracy, nosocomial infection rate, unqualified rate of physical examination report and the complaints of nursing skills, service attitude, equipment, medical supplies, medical environment. Results The total satisfaction rate and disease detection accuracy of the study group were higher than those of the control group, and the rate of nosocomial infection rate and the rate of unqualified physical examination report were lower than those of the control group, with statistically significant differences (P<0.05).The complaint rate of the study group was lower than that of the control group, with statistically significant difference (P<0.05). Conclusion Strengthening the quality control of the test performance and items in the physical examination of our hospital can improve the satisfaction and the accuracy of disease detection, and reduce the rate of infection in the hospital, the rate of unqualified physical examination report and the rate of complaint, so it should be popularized and applied.

Objective To analyze and study the situation of irrational drug use in the pharmacy intravenous admixture service of our hospital, in order to strengthen the intravenous drug intervention in our hospital and provide a reference basis for rational clinical drug use. Methods A total of 526 unreasonable medical advices from the pharmacy intravenous admixture service of our hospital from January 2017 to December 2018 were collected, and their unreasonable conditions were analyzed. Results There were irrational medication types in the pharmacy intravenous admixture service of our hospital, including menstruum selection, drug dosage, frequency of medication, route of administration, incompatibility, contraindications, repeated medication, and other reasons, among which inappropriate menstruum and irrational drug dosage were the most common. Conclusion There is some irrationality on the medical advices in the pharmacy intravenous admixture service of our hospital.It is important to study and analyze the reasons and explore solutions to reduce the irrational medical advices in our hospital, play the importance in pharmacists in promoting clinical rational drug use, and improve the quality of medical services.

Objective To explore the influence of story theory nursing mode on the negative emotion and quality of life of gout patients. Methods Retrospective analysis of 60 patients with gout admitted to our hospital from June 2017 to July 2018 was made.According to the different nursing methods, they were divided into control group (n=30) and observation group (n=30).The control group used routine nursing, the observation group used story theory nursing on that basis, and the negative emotions and quality of life of the two groups were compared. Results The scores of anxiety and depression in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05); the scores of physical function, psychological function, material life state, social function and the excellent rate of nursing compliance in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). Conclusion The nursing mode based on story theory can effectively improve the patients′ negative emotions and improve the patients′ quality of life, which is worthy of clinical application.

Objective To investigate the effect of multi-directional full-intervention nursing mode on patients in intensive care unit. Methods Sixty patients with severe cases admitted to our hospital from January to September 2019 were enrolled in the study.According to the random number table method, they were divided into the control group (n=30)and the observation group (n=30).The control group received routine nursing, the observation group used a multi-directional full-course intervention nursing method.The incidence of ICU catheter-related infection, adverse reactions and time indicators were compared between the two groups. Results The incidence of ICU catheter-related infection in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).The hospitalization time and catheter indwelling time in the observation group were shorter than those in the control group, and the differences were statistically significant (P<0.05). Conclusion The implementation of multi-directional full-intervention nursing mode in the nursing of critically ill patients can effectively reduce the incidence of catheter-related infections, shorten the length of hospital stay, and reduce the incidence of adverse reactions.

Objective To explore the influence of training on functional styles in the ward on mental status of patients with industrial injury rehabilitation. Methods A total of 76 cases of industrial injury rehabilitation patients admitted to our hospital from January 2018 to March 2019 were selected as the research objects.According to the random grouping method, they were divided into experimental group (n=38) and control group (n=38).Under the premise of taking routine rehabilitation treatment, the patients in the experimental group were added to the department to carry out the training method of ward style and function, while the patients in the control group were given routine rehabilitation.The scores of symptom checklist 90 (SCL-90), self-rating anxiety scale (SAS), self-rating depression scale (SDS), Hamilton depression scale (HAMD), Hamilton anxiety scale (HAMA) and quality of life questionnaire (QOL) were compared between the two groups. Results After intervention, the scores of SCL-90, SAS, SDS, HAMD and HAMA in the two groups were lower than those before intervention, and QOL was higher than that before intervention, with statistical differences (P<0.05).After intervention, the scores of SCL-90, SAS, SDS, HAMD, HAMA and QOL in the experimental group were lower than those in the control group, and QOL was higher than that in the control group, with statistical differences (P<0.05). Conclusion The training of recreational and sports function in the ward can help the injured patients significantly, promote the rehabilitation of the injured patients effectively, improve the mental health of the injured patients, and improve the overall ability of the injured patients to return to society, which has important practical significance for their prognosis.

216 Application effect of multi-sensory wake-up nursing mode in patients with coma after hypertensive intracerebral hemorrhage

Objective To explore the effect of multi-sensory wake-up nursing mode in patients with coma after hypertensive intracerebral hemorrhage (HICH). Methods Sixty-two patients with coma after HICH admitted to our hospital from January 2018 to February 2019 were selected and divided into two groups according to the random number method, 31 cases in each group.The control group was given routine nursing mode, and the study group was given multiple-sensory wake-up nursing mode.The wakefulness of the two groups, the Glasgow coma scale (GCS), the dysfunction scale (DFS) scores, acetylcholine (Ach), norepinephrine (NE), dopamine (DA), and complications were compared. Results The recovery of the study group was better than that of the control group, and the difference was statistically significant (P<0.05).The GCS score, Ach, NE, and DA levels in the study group were higher than those in the control group, and the DFS score was lower than that in the control group, the differences were statistically significant (P<0.05).The incidence rates of constipations, such as limb paralysis, pressure sore, and lower extremity venous thrombosis were lower in the study group than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Patients with coma after HICH use multi-sensory wake-up nursing mode can regulate the release of related neurotransmitters, accelerate patient recovery, and reduce the incidence of complications.

219 Effect of pain control nursing on negative emotion and exercise compliance after knee fracture surgery

Objective To explore the effect of pain control nursing on negative emotions and exercise compliance after knee fracture surgery. Methods Retrospective analysis of the clinical data of 60 patients with knee joint fracture admitted to our hospital from March 2017 to November 2018 was made, according to different nursing methods, they were divided into control group (n=30) and observation group (n=30).The control group adopted routine nursing methods, the observation group received pain control nursing methods.The mental state, total exercise compliance,pain and total satisfaction of nursing of the two groups were compared. Results After intervention, the self-rating anxiety scale (SAS)and self-rating depression scale (SDS), visual analogue scale (VAS) scores of the observation group were lower than those of the control group, with statistical differences (P<0.05); after the intervention, total exercise compliance and total satisfaction of nursing of the observation group were higher than those of the control group, with statistical differences (P<0.05). Conclusion The application of pain control nursing in patients with knee fracture can effectively alleviate their negative psychology and improve their exercise compliance.

223 Application effect of clinical nursing pathway in cataract patients treated by intraocular lens implantation combined with phacoemulsification

Objective To analyze the application value of clinical nursing pathway in cataract patients treated with intraocular lens implantation and phacoemulsification. Methods A total of 62 cataract patients treated with intraocular lens implantation and phacoemulsification in our hospital from June 2017 to June 2018 were selected as the research objects.They were divided into observation group (n=31) and reference group (n=31) according to random parallel method.The observation group adopted clinical nursing path method, and the reference group adopted general nursing method.The total nursing satisfaction, total complication rate and hospital stay, hospital expenses of the two groups were compared. Results The nursing total satisfaction of the observation group was higher than that of the reference group, the difference was statistically significant (P<0.05).The total incidence of complications in the observation group was lower than that in the reference group (P<0.05).The time of hospitalization in the observation group was shorter than that in the reference group, hospital expenses was less than that of the reference group, the differences were statistically significant (P<0.05). Conclusion The implementation of clinical nursing pathway in cataract patients treated with intraocular lens implantation+phacoemulsification can help improve clinical outcomes, obtain patient recognition of nursing work, and reduce the length of hospital stay, reducing hospital costs, the overall incidence of complications was also reduced, so it is suitable for clinical application and promotion.

226 Application effect of parents-involved nursing in postoperative rehabilitation of children undergoing developmental dislocation of hip surgery

Objective To explore application effect of parents-involved nursing in postoperative rehabilitation of children undergoing developmental dislocation of hip (DDH)surgery. Methods A total of 120 children who underwent DDH surgery in our hospital from January 2017 to January 2019 were enrolled in the study.They were divided into control group and observation group by random number table method, 60 cases in each group.The control group was given routine nursing.On this basis, observation group was additionally given parents-involved nursing.Harris hip function scale (Harris) and Fugl -Meyer assessment (FMA) were performed to measure hip function and lower limb motor function of the two groups before and after nursing.Quality of life scale (SF-36) was performed to evaluate quality of life in both groups before and after nursing.The scores of satisfaction for nursing were compared between the two groups. Results After nursing, scores of Harris and FMA in observation group were higher than those in control group, with statistical differences (P<0.05).The scores of SF-36 in 8 dimensions such as physiological function, physiological role, physical pain, general health status, energy, social function, emotional role and mental health of observation group were higher than those of control group, with statistical differences (P<0.05).The scores of nursing cognition, nursing comfort, nursing communication and nursing synthesis of observation group were higher than those of control group, with statistical differences (P<0.05). Conclusion Parents-involved nursing can effectively improve recovery of postoperative hip and motor function of lower limbs in DDH children, improve their quality of life.And nursing satisfaction is high.

229 Application effect of enhanced recovery after surgery in the perioperative nursing of artificial hip replacement

Objective To investigate the application value of enhanced recovery after surgery (ERAS) in the perioperative nursing care of artificial hip replacement (THA), and provide new methods and new ideas or clinic. Methods A total of 60 patients with THA treated in our hospital from January 2017 to January 2019 were selected and divided into experimental group and control group according to the time sequence, 30 cases in each group.The experimental group received perioperative nursing with the concept of ERAS, while the control group received traditional nursing methods.The hip function, satisfaction, nutritio status, daily living ability were compared between two groups. Results The Harris scores of hip joints in the experimental group at 1, 2, and 4 weeks after surgery were all higher than those in the control group,with statistical differences(P<0.05);the nutritional scores of the experimental group on the day of operation,2 days and one week after operation were all higher than those of the control group, with statistical differences (P<0.05); the scores of activities of daily life function(ADL) scale (Barthel) scores in the experimental group were higher than those in the control group 1, 2 and 4 weeks after operation, and the differences were statistically significant (P<0.05); the total scores of conscious recovery, nursing attitude, technical operation, quality of rehabilitation guidance,other scores and satisfaction of the experimental group were higher than those of the control group, with statistical differences (P<0.05). Conclusion ERAS is obviously better than the traditional nursing.It not only promotes the rehabilitation of patients and improves the quality of life, but also significantly promotes and improves the quality of nursing.It has practical application value.

233 Application of holistic nursing in patients with ulcerative keratitis

Objective To explore the application of holistic nursing in patients with ulcerative keratitis. Methods A total of 84 patients with ulcerative keratitis admitted to our hospital from June 2015 to June 2017 were selected as the study subjects.According to the sequence before and after admission, the patients were divided into the control group(n=42) and the observation group (n=42).The control group was given routine nursing method, while the observation group was given holistic nursing method.Psychological status, quality of life before and after nursing and satisfaction were compared between the two groups. Results There were no statistically significant differences between the two groups in individual scores and total scores of psychological state and quality of life before nursing (P>0.05).There were statistically significant differences between the two groups in psychological status and quality of life scores before and after nursing (P<0.05).The psychological status score after nursing in the observation group was lower than that in the control group, and the individual score and total score of quality of life and satisfaction were higher than those of the control group, with statistical differences (P<0.05). Conclusion Holistic nursing can alleviate the negative emotions of patients with ulcerative keratitis, promote the degree of satisfaction and the improvement of life quality, and has a high application value.

236 Application effect of holistic rehabilitation nursing intervention for patients with coronary heart disease

Objective To analyze the application effect of holistic rehabilitation nursing intervention for patients with coronary heart disease (CHD). Methods A total of 50 patients with CHD treated in our hospital from February to December 2018 were selected as subjects of study and divided into routine group (n=25) and intervention group (n=25) according to lottery method.The routine group was given general nursing in cardiology department, and the intervention group was given holistic rehabilitation nursing on this basis.The anxiety, depression, quality of life scores and total nursing satisfaction of the two groups were compared. Results After intervention, the anxiety and depression scores of the intervention group were lower than those of the conventional group, and the differences were statistically significant(P<0.05).Before intervention, there were no significant differences in the physical function, psychological status, independent ability, social relationship, living environment and mental support scores between the two groups (P>0.05).After intervention, the physical function and psychology status, independence, social relations, living environment, and spiritual sustenance scores were higher than those before the intervention, and the differences were statistically significant (P<0.05).After intervention, the intervention group′s physical function, psychological status, independence, social relations, the living environment and mental sustenance scores were higher than those of the conventional group, and the differences were statistically significant (P<0.05).After intervention, the nursing total satisfaction of the intervention group was higher than that of the conventional group, and the difference was statistically significant (P<0.05). Conclusion The holistic nursing of patients with CHD can effectively alleviate their anxiety, depression, improve the quality of life, and thus improve the satisfaction of nursing.It is worth clinical promoting.

240 Application effect of special breathing exercise nursing before operation on lung cancer patients

Objective To explore the application effect of preoperative breathing nursing on lung cancer patients before surgery and its effect on postoperative lung function and prognostic factor levels of lung cancer. Methods A total of 80 patients with lung cancer treated in our hospital from April 2017 to April 2019 were selected and divided into the control group (n=40) and the study group (n=40) according to the random number table method.The control group was given routine nursing intervention.Respiratory training ass used 1-2 weeks before the operation in the study group.One week after operation, the forced vital capacity (FVC%), forced expiratory volume in one second (FEV1%), vascular endothelial cell growth factor receptor 2 (VEGFR2), and vascular endothelial growth factor (VEGF) and total complications between two groups were compared. Results The FVC% and FEV1% in the study group were higher than those in the control group after surgery, and the differences were statistically significant (P<0.05).The levels of VEGFR2 and VEGF in the study group after surgery were lower than those in the control group, and the differences were statistically significant (P<0.05).The total incidence of postoperative complications (2.50%) in the study group was lower than that in the control group (15.00%), and the difference was statistically significant (P<0.05). Conclusion Preoperative application effect of respiratory nursing special care can effectively improve lung function and lung cancer prognostic factor levels in patients with lung cancer, and reduce the incidence of postoperative complications.

243 Effect analysis of nursing intervention based on information-motivationbehavior skill model in patients with urinary retention after cervical cancer operation

Objective To explore the application effect of nursing intervention based on information-motivation-behavioral skill (IMB) model in patients with urinary retention after cervical cancer surgery. Methods From June 2017 to June 2019, 98 patients with cervical cancer who were hospitalized in our hospital were selected and divided into control group (n=49) and intervention group (n=49) according to random number table method.Routine nursing intervention was used in the control group and IMB nursing intervention was used in the intervention group.The residual urine volume, total nursing efficacy, incidence of urinary tract infection and nursing satisfaction score of the two groups were observed. Results The residual urine volume in the intervention group was less than that in the control group ([75.51±14.61 vs.131.07±31.29]ml) (P<0.05); the nursing efficacy rate in the intervention group was higher than that in the control group (95.92% vs.79.59%) (P<0.05); the total incidence of urinary tract infection in the intervention group was lower than that in the control group (8.16% vs.30.61%) (P<0.05); and scores of standardization of nursing behavior,problem-solving ability and health education ability of nurses in the intervention group were higher than those of control group, the differences were statistically significant (P<0.05). Conclusion Nursing intervention based on IMB model for patients discharged from hospital with urinary retention tube after cervical cancer surgery can significantly reduce residual urine volume, improve treatment efficiency, reduce the risk of urinary tract infection, and improve the satisfaction of nursing service, which is worthy of clinical promotion.

246 Application effect of VR glasses on primiparas′ childbirth

Objective To explore the application effect of VR glasses on primiparas′ childbirth. Methods A total of 71 primiparas treated in our hospital from November 2018 to May 2019 were selected as subjects.They were divided into observation group (35 cases) and control group (36 cases) by random number table method.Control group was given routine nursing intervention, experimental group was given VR glasses during parturition on this basis.The degree of pain during childbirth (visual analogue scale [VAS], verbal response scale [VRS]and real pain intensity scale [PPI]were used for evaluation), mental state (self-rating anxiety scale [SAS]and self-rating depression scale [SDS]were used for evaluation), sense of birth control (labour agentry scale [LAS]was used for evaluation) and total degree of parturition satisfaction were recorded and compared between the two groups. Results The VAS, VRS and PPI scores of primiparas in the observation group were lower than those in the control group, with statistically significant differences (P<0.05).The SAS and SDS scores of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05).The LAS score of primiparas in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05).The total parturition satisfaction of primiparas in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05).Conclusion Using VR glasses can reduce primiparas′ pain during parturition, improving their mental state and increasing childbirth experience, it is worth popularizing in clinical work.

250 Effect of postpartum rehabilitation nursing on lactation and uterine involution of parturients undergoing cesarean section caesarean women

Objective To discuss the effect of postpartum rehabilitation nursing on lactation and uterine involution of parturients undergoing cesarean section. Methods A total of 60 parturients who underwent cesarean section delivery in our hospital from October 2018 to August 2019 were selected as the research objects, and they were divided into control group and observation group according to the random number table method, with 30 cases in each group.The parturients in the control group were given routine postpartum nursing, and those in the observation group were given postpartum rehabilitation nursing intervention on the basis of the control group.The postpartum lactation, uterine involution and incidence of complications were compared between the two groups. Results The lactation time of the observation group was shorter than that of the control group, and the difference was statistically significant (P<0.05).The lactation condition of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).The proportions of the observation group with the height of uterine fundus≤4 transverse fingers and the thickness of endometrium≤10 mm were higher than those of the control group, and the incidence rates of postpartum hemorrhage and serous lochia were lower than those of the control group, with statistically significant differences (P<0.05).The total incidence rate of postpartum complications in the observation group was lower than that in the control group,and the difference was statistically significant (P<0.05). Conclusion Postpartum rehabilitation nursing can improve the lactation of caesarean section parturients, promote the uterine involution, and reduce the incidence of complications.
