5 Inhibitory effect of triterpenoid extract of Prunella vulgaris L. combined with Survivin-siRNA on esophageal cancer Eca-109 cells

Objective To observe whether Survivin-siRNA can enhance the proliferation inhibition and apoptosis induction of triterpenoid extract of Prunella vulgaris L. (P. vulgaris) on esophageal cancer Eca-109 cells, and to explore its mechanism. Methods The esophageal cancer Eca-109 cells were divided into blank control group, negative control group, P. vulgaris group, siRNA transfection group, and P. vulgaris+siRNA transfection group. After 48 h of esophageal cancer Eca-109 cells in each group, the expression levels of Survivin mRNA and Survivin protein were detected by RT-PCR and Western blot. The proliferation inhibition rate of esophageal cancer cells was detected by CCK-8, and the apoptosis rate was detected by flow cytometry. Results The expression levels of Survivin mRNA and Survivin protein in esophageal cancer Eca-109 cells in the P. vulgaris group, siRNA transfection group and P. vulgaris+siRNA transfection group were lower than those in the control group and negative control group, and the differences were statistically significant (P<0.05). The esophageal cancer Eca-109 cell proliferation inhibition rate and cell apoptosis rate in the P. vulgaris group, siRNA transfection group and P. vulgaris+siRNA transfection group were higher than those in the control group and negative control group, and the differences were statistically significant (P<0.05). The expression levels of Survivin mRNA and Survivin protein in esophageal cancer Eca-109 cells in the P. vulgaris+siRNA transfection group were lower than those in the P. vulgaris group and the siRNA transfection group, and the differences were statistically significant (P<0.05). The proliferation inhibition rate and cell apoptosis rate of esophageal cancer Eca-109 cells in the P. vulgaris+siRNA transfection group were higher than those in the P. vulgaris group and siRNA transfection group, and the differences were statistically significant (P<0.05). Conclusion Survivin-siRNA can enhance the effect of Chinese herbal medicine P. vulgaris on the proliferation inhibition and apoptosis induction of esophageal cancer Eca-109 cells.

10 Determination of Teicoplanin in plasma by high performance liquid chromatography

Objective To establish a method for detecting Teicoplanin plasma concentrations. Methods Plasma samples were pretreated by acetonitrile precipitation of protein and dichloromethane extraction, and Piperacillin sodium was used as internal standard. Diamonsil C18 chromatographic column (4.6 mm × 150 mm, 5 μm) was used as chromatographic column, acetonitrile and 0.01 mol/L potassium dihydrogen phosphate solution (25∶75) (adjust pH value to 2.8 with phosphoric acid solution) was used as mobile phase, the column temperature was 30℃, the flow rate was 1.2 ml/min, the detection wavelength was 215 nm, the injection volume was 20 μl. Results The peaks of Teicoplanin and internal standard were well shaped with retention times of about 7.3 min and 12.5 min, respectively. Linear regression equation was Y=11.295 386X-0.048 985 (r=0.9987), linear range was 3.125 to 100.000 μg/ml. The intra batch and inter batch accuracy were in the range of 85% to 115%. The relative standard deviation within and between batches was less than 15%. The recoveries of Teicoplanin and internal standard were in the range of 85% to 115%. Conclusion The plasma concentration of Teicoplanin can be determined by high performance liquid chromatography. The method is simple, feasible, specific and reproducible. Methodological indicators are in line with the requirements of Pharmacopoeia of the People′s Republic of China.

14 Clinical effect comparison of Sacubitril-Valsartan and Irbesartan in the treatment of chronic heart failure

Objective To compare the clinical effect of Sacubitril-Valsartan and Irbesartan in the treatment of chronic heart failure. Methods From June 2018 to December 2020, 100 patients with coronary heart disease complicated with chronic heart failure who were admitted to the Second Department of Circulatory Medicine, Changchun People′s Hospital were selected as the research subjects. They were divided into an observation group (50 cases) and a control group (50 cases) according to random number table method. Among them, 7 cases quit midway, and finally 50 cases in the observation group and 43 cases in the control group. Observation group was treated with conventional drugs + Sacubitril-Valsartan, and control group was treated with conventional drugs + Irbesartan. Both groups were followed up for 6 months. The left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), 6-min walking test distance and N-terminal probrain natriuretic peptide (NT-proBNP) were compared between the two groups before and 24 weeks after treatment. The evaluate glomerular filtration rate (eGFR), cardiac troponin T (cTnT) and growth stimulation expressed gene 2 (ST2) were compared between the two groups before treatment and 24 weeks after treatment. The treatment effect and incidence of adverse reactions were compared between the two groups. Kaplan-Meier survival analysis was used to plot survival curves to compare the rate of unplanned rehospitalization due to heart failure, cumulative risk of adverse cardiovascular events, cumulative ratio of rehospitalization costs, and all-cause mortality between the two groups. Results Before treatment, there were no statistical significances in 6-min walking test distance, NT-proBNP, LVEDD, LVEF, ST2, cTnT and eGFR between the two groups (P>0.05). After 24 weeks of treatment, the levels of NT-proBNP and LVEDD in both groups were lower than those before treatment, the levels of LVEF in both groups were higher than those before treatment, and the 6-min walking test distancese in both groups were longer than those before treatment, the differences were statistically significant (P<0.05). After 24 weeks of treatment, the levels of NT-proBNP and LVEDD in the observation group were lower than those in the control group, and the level of LVEF in the observation group was higher than that in the control group, the 6-min walking test distance in the observation group was longer than that in the control group, and the differences were statistically significant (P<0.05). After 24 weeks of treatment, ST2 and eGFR levels in observation group were higher than those in control group, while cTnT level in the observation group was lower than that in control group, the differences were statistically significant (P<0.05). After 24 weeks of treatment, the treatment effect of observation group was better than that in control group, and the total effective rate was higher than that in control group, the difference was statistically significant (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05). The rate of unplanned rehospitalization due to heart failure, cumulative risk of adverse cardiovascular events, cumulative ratio of rehospitalization costs and all-cause mortality in the observation group were lower than those in the control group (P<0.05). Conclusion Sacubitril-Valsartan can significantly improve the activity tolerance and clinical symptoms of patients with chronic heart failure, and improve the glomerular filtration function, which has certain improvement on patients′ long-term prognosis.

20 Predictive value of neutrophil to lymphocyte ratio in immune checkpoint inhibitor-related pneumonitis in lung cancer

Objective To investigate the predictive value of peripheral blood lymphocyte subsets and inflammation related indexes in the occurrence of immune checkpoint inhibitor associated pneumonia (CIP) in advanced lung cancer. Methods A retrospective analysis was performed on the clinical data of 77 patients with advanced lung cancer who received PD-1/PD-L1 inhibitor treatment and lymphocyte subsets detection in Qilu Hospital of Shandong University from February 2019 to November 2021. According to the occurrence of CIP, they were divided into CIP group (11 cases) and non-CIP group (66 cases). The levels of lymphocyte subsets, neutrophil to lymphocyte ratio (NLR), lymphocyte to monocyte ratio (LMR) and platelet/lymphocyte ratio (PLR) were compared between two groups, and the predictive value of each indicator for CIP occurrence was evaluated by ROC curve. Results During follow-up, CIP occurred in 11 patients, the incidence of 14.3%. The baseline NLR in CIP group was higher than that in non-CIP group, and the difference was statistically significant (P<0.05). However, there were no significant differences in lymphocyte subsets, LMR and PLR between two groups (P>0.05). ROC curve showed that the AUC predicted by NLR for CIP was 0.718 (95%CI: 0.502-0.933), the cut-off value was 4.50, the sensitivity was 75.8%, and the specificity was 72.7%. Conclusion NLR can be used as a predictor of CIP in advanced lung cancer patients before immunotherapy, but lymphocyte subsets, LMR and PLR have no significant correlation with CIP occurrence.

Pharmacy extension service is an important development trend under the situation of medical and health reform. The implementation plan of professional and systematic pharmacy extension service is of great significance to improve patients′ medication compliance and treatment effect and safety. Children is in the rapid stage of growth and development, and organ, immune, and nervous systems are not well developed, while have special sensitivity to drugs, so pharmacy extension service in pediatric outpatient medication needs to pay more attention to the medication compliance of patients and the whole process of rational drug use, in order to maximize the benefit of children′s medication in the process of disease diagnosis and treatment, improve the treatment effect of patients. This paper based on the key words of "pharmacy extension service" "paediatric drugs" "paediatric clinic" "rational drug use" "off-label drug use" published from January 2009 to February 2022 were searched in CNKI, Wanfang data, database of Chinese sci-tech periodicals (VIP), PubMed, GeenMedical and other databases, the relevant literatures were queried. According to the analysis of the current situation of drug use in pediatric outpatient department, the examples of rational drug use in children and the interpretation of the content of pharmaceutical care in hospital, the implementation plan of pharmaceutical extended service in pediatric outpatient department of hospital is summarized. In order to provide reference for promoting "patient-centered" hospital pharmaceutical care.

30 Research progress of Taraxerol in protecting gastric epithelial cells and anti-inflammatory effect

With the development of society and irregular sleep, gastric mucosal lesions caused by fast-paced life have become a very common situation. At present, various acid suppressors and mucosal protectants used in clinical treatment of gastric mucosal lesions have achieved certain effect, but there are often various adverse reactions in the treatment process. The results show that the Dandelion resource is very abundant in our country, contain a variety of active ingredients, has extensive pharmacological effect, including the Taraxerol in protecting gastric epithelial cells and anti-inflammatory has significant curative effect, especially in the eradication of Helicobacter pylori, treatment is more simple, and side effect of small, use it as a protection of gastric epithelial cells and anti-inflammatory effect of drugs has important clinical significance. Its effect and mechanisms are mainly accomplished by interfering cell signal transduction, inducing inflammation-related enzymes, regulating cytokines, inhibiting gastric acid and killing Helicobacter pylori. In order to provide a theoretical basis for the therapeutic effect of this drug, the protective effect and anti-inflammatory effect of this drug were systematically reviewed in this paper.

35 Advances in the treatment of acute myeloid leukemia in the elderly

Acute myeloid leukemia (AML) is a clinically and genetically heterogeneous myeloid primitive cell clonal malignant proliferative disease that tends to occur in the elderly. The treatment of AML mainly consists of initial induction of remission, followed by consolidation and maintenance therapy after complete remission (CR). Due to physiological age, physical status and other characteristics of elderly AML patients, as well as disease-related factors such as poor cytogenetics, elderly AML patients are characterized by low CR rate, poor prognosis and high early mortality. For elderly AML patients, doctors need to carry out individualized evaluation. In the stage of induced remission, some elderly people are suitable for intensive chemotherapy, while most elderly people are suitable for low-intensity chemotherapy. Some new advances have been made in the treatment of AML in the elderly. In this review, we reviewed the advances in the treatment of elderly AML in order to improve the quality of life of elderly PATIENTS with AML.

40 Research Progress on traditional Chinese medicine treatment of nephrotic syndrome

In recent years, the incidence of nephrotic syndrome has become higher and higher, because of its complex condition, repeated diseases and high difficulty of treatment, which has a serious impact on the daily life of patients and families. The disease is caused by increased permeability of the glomerular basal membrane and is attributed to "three highs and one low" according to the symptoms of nephrotic syndrome. In the past, Western medicine used hormones and other therapies for the treatment of this disease, although it can alleviate the condition, but the symptoms are repeated after stopping the drug, and there are more adverse reactions caused by it, and the long-term effect is not good. Chinese medicine believes that the occurrence and development of the disease is closely related to factors such as yang qi weakness, when the yang qi is weak, it is unable to promote the blood flow, or its ability is weakened, hematological stasis; or water stops qi obstruction, qi stagnation blood stasis; or due to crooked invasion, or the disease has been in the network for a long time and cause stasis. And because the disease is the evidence of the original false standard, it is necessary to adopt dialectical treatment in the treatment, and at the same time take the principle of "both symptoms and root causes" to obtain the best clinical effect. In view of this, this paper analyzes the etiology and pathogenesis of nephrotic syndrome, and analyzes the application effect of TCM dialectics and combination therapy in this disease, so as to provide a reliable reference for follow-up research.

44 Correlation analysis of tumor necrosis factor-α, cytokeratin 19 fragment antigen 21-1 expression and disease stage in non-small cell lung cancer

Objective The correlation between the expression of tumor necrosis factor-α (TNF-α) and cytokeratin 19 fragment antigen 21-1 (CYFRA21-1) in non-small cell lung cancer was analyzed. Methods Sixty patients with non-small cell lung cancer admitted to Jiujiang First People′s Hospital from October 2018 to November 2021 were selected as the research subjects. Cancer tissue and paracancer tissue (more than 5 cm away from cancer tissue) were collected for immunohistochemical staining. The expression levels of CYFRA21-1 and TNF-α were detected by chemiluminescence. Results The positive rate of TNF-α expression and CYFRA21-1 expression in cancer tissues were higher than those in adjacent tissues, and the differences were statistically significant (P<0.05). The positive rate of TNF-α and CYFRA21-1 expression in patients with pathological stage Ⅲ-Ⅳ were higher than those in patients with pathological stage Ⅰ-Ⅱ, and the differences were statistically significant (P<0.05). The positive expression of TNF-α in patients with pathological stage Ⅲ-Ⅳ was stronger than that in patients with pathological stage Ⅰ-Ⅱ, and the difference was statistically significant (P<0.05). The expressions of TNF-α and CYFRA21-1 were positively correlated with pathological stage (P<0.05). Conclusion TNF-α and CYFRA21-1 were abnormally high in non-small cell lung cancer tissues, which were correlated with the pathological stages of the disease.

48 Establishment of non-alcoholic fatty liver model in type 2 diabetic rats

Objective To establish a non-alcoholic fatty liver model in rats with type 2 diabetes, and to explore the effect of type 2 diabetes on the liver of rats. Methods Forty healthy male SD rats were randomly divided into normal control group (NC) and diabetic group (DM), with 20 rats in each group. Type 2 diabetic rats in DM group were induced by intraperitoneal injection of streptozotocin (STZ, 35 mg/kg) combined with high-glucose and high-fat diet. After modeling, blood glucose, triacylglyceride (TG), total cholesterol (TC), alanine aminotransferase (ALT), aspartate aminotransferase (AST), liver index, insulin sensitive index (ISI), Homeostasismodel assessment insulin resistance index (HOMA-IR) related indicators were detected. Liver tissue of rats was stained with HE and its pathological changes were observed. Results At 6, 7, 8, 9, 10 weeks after modeling, the body weight of DM group was lower than that of NC group, and blood glucose was higher than that of NC group, the differences were statistically significant (P<0.05). TG, TC, ALT and liver index in DM group were higher than those in NC group, the differences were statistically significant (P<0.05). There was no significant difference in AST between the two groups (P>0.05). 6, 10 weeks after modeling, the fasting insulin (FINS), HOMA-IR of DM group were higher than those of NC group, ISI was lower than that of NC group, the differences were statistically significant (P<0.05). Under the light microscope, the normal liver cells in NC group were orderly, the liver lobules were normal, the cytoplasm was round and uniform, and there was no infiltration of steatosis and inflammatory cells. In DM group, liver cells were improperly organized, fatty degeneration, liver cell edema, infiltration of inflammatory cells, partial spots and local necrosis. The activity score of non-alcoholic fatty liver disease (NAS) in DM group was higher than that in NC group, and the difference was statistically significant (P<0.05). Conclusion Intraperitoneal injection of low-dose STZ combined with high-sugar and high-fat feeding can induce a non-alcoholic fatty liver model of type 2 diabetes in rats.

52 Comparison of blood transfusion volume in patients with different clinical characteristics of liver diseases during liver transplantation

Objective To compare the blood transfusion volume in patients with different clinical characteristics of liver diseases during liver transplantation. Methods The clinical data of 96 patients with liver transplantation admitted to Beijing Youan Hospital Affiliated to Capital Medical University from October 2018 to October 2019 were analyzed retrospectively. The intraoperative blood transfusion volume of patients with different sex, age, basic liver disease and operation methods were compared. The preoperative coagulation indexes in patients with different basic liver diseases was compared. Results There were no significant differences in suspended red blood cells, plasma and platelets transfused in patients of different sex and age (P>0.05). There were significant differences in prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (Fib) of patients with different basic liver diseases before operation (P<0.05). The preoperative PT and APTT of patients with acute and chronic liver failure were longer than those of patients with primary liver cancer, hepatitis cirrhosis and other liver diseases, the preoperative TT of patients with acute and chronic liver failure was shorter than that of patients with primary liver cancer, hepatitis cirrhosis and other liver diseases, the preoperative Fib of patients with acute and chronic liver failure was lower than that of patients with primary liver cancer, hepatitis cirrhosis and other liver diseases, the differences were statistically significant (P<0.05). There were significant differences in suspended red blood cells and plasma transfusion in patients with different basic liver diseases (P<0.05). There was no significant difference in platelet transfusion between patients with different basic liver diseases (P>0.05). The suspended red blood cells and plasma transfusions in patients with acute and chronic liver failure were significantly higher than those in patients with primary liver cancer and hepatitis cirrhosis, the differences were statistically significant (P<0.05). The suspended red blood cells and plasma transfusions in patients with other liver diseases were greater than those in patients with primary liver cancer, the differences were statistically significant (P<0.05). There were no significant differences in the transfusion of suspended red blood cells, plasma and platelets in patients with different operation methods (P>0.05). Conclusion The blood demand of liver transplantation patients must be individualized by evaluating preoperative factors, reasonably grasping the indications of blood transfusion, ensuring adequate blood supply and reducing related complications.

55 Application effect of I PASS structured disease handover model in emergency critically ill patients

Objective To investigate the application effect of I PASS structured disease handover model in emergency critically ill patients. Methods A total of 42 nurses worked in emergency department of the Second Affiliated Hospital of Nanchang University from May to December 2021 were selected as the research subjects. Traditional bedside handover model was adopted in the emergency department from May to August 2021, and nurses adopted this model during the period were included in the control group. I PASS structured disease handover model was adopted in the emergency department from September to December 2021, and nurses adopted this model during the period were included in the observation group. The shift handover quality, shift handover time, completeness of handover contents and nurses′ satisfaction with the shift handover mode were compared between the two groups. Results The scores of handover environment, handover content, communication skills, humanistic care, organizational efficiency, clinical judgment and overall handover ability in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). The scores of shift environment, communication skills, humanistic care, organizational efficiency, clinical judgment and overall handover ability in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). The shift handover time of the observation group was shorter than that of the control group, and the completeness of the handover content in the observation group was higher than that in the control group, the differences were statistically significant (P<0.05). The nurses′ total satisfaction with the shift handover mode in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). Conclusion I PASS structured disease handover mode is applied to emergency critically ill patients, the shift handover quality, shift handover efficiency and handover contents completeness of nurses are improved, and they are more satisfied with the handover mode.

59 Effect of respiratory function training combined with bladder function training on urodynamic index and bladder function in patients with urinary incontinence after stroke

Objective To investigate the effects of respiratory function training combined with bladder function training on urodynamic index and bladder function in patients with urinary incontinence after stroke. Methods A total of 90 patients with urinary incontinence after stroke treated in Zhaoqing Second People′s Hospital from June 2020 to October 2021 were selected as subjects. They were divided into the control group (n=45) and the study group (n=45) by random number method. The control group received percutaneous electrical nerve stimulation combined with motor imagery therapy, and the study group received respiratory function training combined with bladder function training. The clinical treatment effect, urodynamic index and bladder function before and after treatment were compared between the two groups. Results The total effective rate in the study group was higher than that in the control group, and the difference was statistically significant (P<0.05). There were no significant differences in the bladder volume, detrusor pressure, bladder compliance and bladder pressure before treatment between the two groups (P>0.05). After treatment, the bladder volume of the two groups was greater than that before treatment, and the detrusor pressure, bladder compliance and bladder pressure of the two groups were lower than those before treatment, the bladder volume of the study group was greater than that of the control group, and the detrusor pressure, bladder compliance and bladder pressure of the study group were lower than those of the control group, the differences were statistically significant (P<0.05). There were no significant differences between the two groups in the 24 h urination times, 24 h urinary incontinence times, each urination volume and residual urine volume before treatment (P>0.05). After treatment, the 24 h urination times, 24 h urinary incontinence times and residual urine volume of the two groups were less than those before treatment, and each urination volume of the two groups was more than that before treatment, the 24 h urination times, 24 h urinary incontinence times and residual urine volume of the study group were less than those of the control group, and each urination volume of the study group was more than that of the control group, the differences were statistically significant (P<0.05). Conclusion In the clinical treatment of patients with urinary incontinence after stroke, respiratory function training combined with bladder function training has a particularly ideal clinical effect, which can effectively reduce clinical symptoms, improve urodynamic index, and promote the early recovery of bladder function, which is of popularization value.

63 Application effect of pediatric massage therapy in pediatric bronchial pneumonia

Objective To explore the application effect of pediatric massage therapy in pediatric bronchial pneumonia. Methods A total of 200 children with bronchial pneumonia and their parents who were hospitalized in the Department of Pediatrics of Dongguan Kanghua Hospital from January 2019 to August 2021 were selected as the research objects, they were divided into the control group (100 cases) and the observation group (100 cases) according to the random number table method. Both groups were treated with conventional symptomatic supportive treatment and nursing. 66 Changes and analysis of diagnostic value of serum fibrin degradation products and haptoglobin in elderly asthma

Objective To explore the changes of serum fibrin degradation products (FDP) and haptoglobin (HPT) in elderly asthma and their diagnostic value. Methods A total of 58 elderly patients with asthma diagnosed and treated in Jinzhou Central Hospital from March 2016 to March 2019 were selected as the asthma group, and 58 volunteers with normal physical examination during the same period were selected as the control group. Pearson method was used to analyze the correlation between FDP and HPT levels and lung function indicators. ROC curve was used to analyze the diagnostic value of FDP and HPT in elderly patients with asthma. Results The forced ventilation volume, forced expiratory volume in the first second and maximum respiratory peak flow rate in the asthma group were lower than those in the control group, the difference was statistically significant (P<0.05). The levels of serum FDP and HPT in the asthma group were higher than those in the control group, the difference was statistically significant (P<0.05). Pearson analysis showed that the levels of serum FDP and HPT in elderly patients with asthma were negatively correlated with the forced ventilation volume, forced expiratory volume in the first second and maximum respiratory peak flow rate (P<0.05). ROC curve showed that the area under the curve (AUC) of serum FDP and HPT levels in the diagnosis of elderly asthma was 0.830 and 0.833 respectively, and the AUC of combined diagnosis of elderly asthma was 0.911. Conclusion Serum FDP and HPT have the potential as early diagnosis and identification indicators for elderly asthma.

70 Application of medium length catheter in clinical anesthesia infusion

Objective To study the feasibility and value of medium length catheter in clinical anesthesia infusion. Methods A total of 40 patients with acute cholecystitis who underwent open cholecystectomy in Anesthesia Surgery Center of Tongzhou District Hospital of Traditional Chinese Medicine of Nantong City in Jiangsu Province from January 2020 to November 2021 were selected. According to random number table method, they were divided into group A and group B, 20 cases in each group. Group A was placed with medium length catheter as an intravenous infusion tool under ultrasound guidance. Peripheral short vein indwelling needle was routinely placed in group B. The puncture success rate, average catheter replacement time and catheter related complications were observed and compared between the two groups. Results Before anesthesia, the puncture success rate of group A was higher than that of group B, the difference was statistically significant (P<0.05). The average replacement time of catheter in group A was longer than that in group B, and the difference was statistically significant (P<0.05). During catheter indwelling, the total incidence of catheter-related complications in group A was less than that in group B, with statistically significant difference (P<0.05). Conclusion In clinical anesthesia, it is safe and feasible to use ultrasound visualization technology to place medium length catheter as infusion tool, with fewer complications. It is worthy of further study and generalization.

74 Diagnostic value of peripheral blood cystatin C, kidney injury molecule-1 and neutrophil gelatinase-associated lipocalin in patients with chronic hepatitis B and early renal injury

Objective To explore diagnostic value of peripheral blood cystatin C (Cys-C), kidney injury molecule-1 (KIM-1) and neutrophil gelatinase-associated lipocalin (NGAL) in patients with chronic hepatitis B and early renal injury. Methods A total of 56 patients with chronic hepatitis B and early renal injury admitted to Dalian Public Health Clinical Center from April 2018 to April 2021were enrolled as the observation group, while other 56 patients with matched gender and age, and simple chronic hepatitis B during the same period were enrolled as the control group. The levels of estimated glomerular filtration rate (eGFR), Cys-C, KIM-1 and NGAL between the two groups were compared. The detection value of Cys-C, KIM-1 and NGAL levels in patients with chronic hepatitis B and early renal injury and their correlation with eGFR were analyzed. Results The eGFR in the observation group was lower than that in the control group, and the levels of Cys-C, KIM-1 and NGAL in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05). The results of ROC showed that the AUC of serum Cys-C, KIM-1 and NGAL levels in patients with chronic hepatitis B and early renal injury was 0.771, 0.785 and 0.766 respectively, lower than that of combined detection (0.896, P<0.05). The cut-off values of Cys-C, KIM-1 and NGAL were 1.24 mg/L, 1.03 mg/L, 116.82 ng/ml. Cys-C, KIM-1, NGAL levels were negatively correlated with eGFR (P<0.05). Conclusion The levels of serum Cys-C, KIM-1, NGAL and their combined detection have diagnostic value for patients with chronic hepatitis B and early renal injury, which is worthy of clinical reference.

77 Research of effect of palmar approach percutaneous cannulated screw fixation on scaphoid fractures

Objective To investigate the effect of palmar approach percutaneous cannulated screw fixation for the treatment of scaphoid fractures. Methods A total of 76 patients with scaphoid fracture admitted to Guangdong Armed Police Corps Hospital from June 2018 to June 2020 were retrospectively selected as the study subjects. They were divided into control group (n=38) and study group (n=38) by the method of difference in surgical plan. The control group was treated with dorsal approach while the study group was treated with palmar approach. The efficacy, pain degree, clinical indicators and wrist function of the two groups were compared. Results The excellent and good rate of the study group was higher than that of the control group, with statistical significance (P<0.05). There was no significant difference in the pain score between the two groups before operation (P>0.05). The pain score of the control group at 3 and 6 months after operation were lower than those of study group, with statistical significances (P<0.05). The intraoperative blood loss and fracture healing time in the study group were less than those in the control group, with statistical significances (P<0.05). There were no significant differences between the two groups in the preoperative wrist self questionnaire (PRWE) score and Mayo wrist score (P>0.05). The PRWE score of the study group at 6 months after operation was lower than that of the control group, with statistical significance (P<0.05). The Mayo wrist score in the study group 6 months after operation was higher than that in the control group, with statistical significance (P<0.05). Conclusion The treatment of scaphoid fracture through palmar approach with percutaneous cannulated screw fixation can improve the curative effect and function of wrist joint, reduce postoperative pain, promote healing, and is worthy of application.

81 Comparison of the effect of fracture healing of intramedullary nail fixation and inverted cannulated compression screw in the treatment of femoral neck fracture

Objective To compare the effect and fracture healing of intramedullary nail fixation and inverted cannulated compression screw (CCS) in the treatment of femoral neck fracture. Methods A total of 85 patients with femoral neck fracture admitted to the Department of Orthopedics, Putian Medical Area of No.900 Hospital of the Joint Logistic Protection Force of the People′s Liberation Army from July 2019 to April 2021 were selected as research objects, they were divided into observation group (43 cases) and control group (42 cases) according to random number table method. Patients in the observation group were treated with inverted CCS, and those in the control group were treated with intramedullary nail fixation. Surgical conditions, fracture healing, hip function, postoperative complications and pain degree were compared between the two groups. Results The operation time, length of hospital stay, length of incision of observation group were shorter than those of the control group, the intraoperative blood loss of observation group was less than that of the control group, and the differences were statistically significant (P<0.05). The fracture healing time and weight-bearing time of observation group were shorter than those of the control group, and the differences were statistically significant (P<0.05). The hip function scores (Harris scores) of observation group were higher than those of the control group at 1, 3 and 6 months after operation, and the differences were statistically significant (P<0.05). The visual analogue scale (VAS) scores at 1, 3 and 6 months were lower than those of the control group, and the differences were statistically significant (P<0.05). The total incidence of postoperative complications in the observation group (6.98%) was lower than that in the control group (23.81%), and the difference was statistically significant (P<0.05). Conclusion Compared with intramedullary nail fixation, inverted CCS can effectively shorten the operation time, reduce intraoperative blood loss, accelerate bone healing, reduce postoperative complications, and promote recovery.

85 Application effect of arthroscopic anterior cruciate ligament residual reconstruction in anterior cruciate ligament injury

Objective To explore the application effect of arthroscopic anterior cruciate ligament residual reconstruction in the treatment of anterior cruciate ligament injury and its impact on the quality of life. Methods A total of 58 patients with anterior cruciate ligament injury treated in Shaoguan First People′s Hospital from January 2019 to December 2020 were divided into observation group and control group according to random number table method, with 29 cases in each group. Among them, the observation group underwent anterior cruciate ligament residual reconstruction, and the control group underwent anterior cruciate ligament residual reconstruction. The curative effect and improvement of quality of life were compared between the two groups. Results After treatment, the Lysholm score of the observation group was higher than that of the control group, and the objective scoring standard of knee injury in the International Knee Documentation Committee (IKDC) knee evaluation form was lower than that of the control group, the differences were statistically significant (P<0.05). Before treatment, there were no significant differences between the two groups in passive activity detection threshold and passive angle (P>0.05). After treatment, the passive activity detection threshold of the observation group was lower than that of the control group, and the passive angle was less than that of the control group, the differences were statistically significant (P<0.05). Before treatment, there was no significant difference in the quality of life scores between the two groups (P>0.05). After treatment, the score of quality of life in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). Conclusion The comprehensive effect of arthroscopic anterior cruciate ligament residual reconstruction is significant, the recovery effect of knee function is good, and the quality of life of patients is effectively improved, which is worthy of popularization and application.

Objective To explore the effect of Shenqi Yiliu Formula combined with radiotherapy on immune function and prognosis of patients with middle and advanced cervical cancer. Methods A total of 84 patients with advanced cervical cancer treated in Ganzhou Cancer Hospital from April 2015 to October 2018 were selected as the research subjects. According to random number table method, they were divided into control group (42 cases) and observation group (42 cases). The control group was treated with radiotherapy, and the observation group was treated with Shenqi Yiliu Formula combined with radiotherapy. The disease control rate, objective remission rate, immune function and prognosis were compared between the two groups. Results The objective remission rate of the observation group was 71.43%, higher than 47.62% of the control group, and the disease control rate of the observation group was 90.48%, higher than 69.05% of the control group, the differences were statistically significant (P<0.05). After treatment, CD3+, CD4+ and natural killer (NK) cells in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). The 3-year survival rate of the observation group was higher than that of the control group, and the overall incidence of tumor metastasis was lower than that of the control group, the differences were statistically significant (P<0.05). Conclusion Shenqi Yiliu Formula combined with radiotherapy has reliable efficacy in the treatment of patients with middle and advanced cervical cancer, which can improve immune function, reduce the total incidence of tumor metastasis and improve the 3-year survival rate.

92 Efficacy and safety of Capecitabine maintenance in elderly patients with advanced triple negative breast cancer

Objective To analyze the efficacy and safety of Capecitabine maintenance in elderly patients with advanced triple negative breast cancer. Methods A total of 100 elderly patients with advanced triple negative breast cancer treated in Shangrao People′s Hospital from July 2016 to July 2019 were selected as the research objects, and were divided into control group (50 cases) and observation group (50 cases) by random number table method. The control group was treated with Docetaxel+Doxorubicin+Cyclophosphamide, the observation group was additionally given Capecitabine maintenance therapy. Treatment effect, progression-free survival, median survival and total incidence of adverse reactions were compared between the two groups. Results The total effective rate of the observation group was 88.00%, higher than that of the control group 64.00%, the difference was statistically significant (P<0.05). The progression-free survival and median survival in the observation group were (7.26±1.42) and (21.32±2.33) months, respectively, longer than (5.62±1.52) and (16.59±2.31) months in the control group, the differences were statistically significant (P<0.05). The total incidence of adverse reactions in the observation group was 4.00%, lower than 18.00% in the control group, the difference was statistically significant (P<0.05). Conclusion Capecitabine maintenance therapy can effectively prolong the survival of patients, improve the treatment effect, significantly improve the prognosis of patients, and high safety, less adverse reactions, worthy of promotion.

Objective To investigate the Short term effect of Docetaxel combined with Carboplatin in the treatment of breast cancer patients with pulmonary metastasis and its impact on survival. Methods A total of 72 patients with lung metastasis of breast cancer admitted to the Sixth People′s Hospital of Huizhou City from January 2019 to January 2020 were selected as the research objects. They were divided into control group and research group by ball-touching method, with 36 patients in each group. The control group was treated with Carboplatin combined with Paclitaxel, and the study group was treated with Carboplatin combined with Docetaxel. The short-term efficacy, survival and adverse drug reactions of the two groups were compared. Results The objective effective rate of study group was higher than that in control group, and the difference was statistically significant (P<0.05). There was no significant difference in disease control rate between two groups (P>0.05). There was no significant difference in overall survival rate between the two groups (P>0.05). In terms of adverse reactions, there was no significant difference in the incidence of liver function damage, diarrhea, nausea and vomiting between the two groups (P>0.05). The incidence of hand-foot syndrome and leukopenia in the study group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Combined treatment of Carboplatin and Docetaxel in patients with lung metastasis of breast cancer can improve the short-term objective response rate of patients, with higher safety and fewer side effects.

Methods The clinical data of 60 patients with ovarian cysts treated in the Third People′s Hospital of Jiujiang City from January 2018 to October 2020 were retrospectively analyzed, the patients were divided into the observation group (30 cases) and the control group (30 cases) according to the operation method. The patients in the observation group were treated with laparoscopic ovarian cyst excision, and the patients in the control group were treated with traditional open ovarian cyst excision. The operation time, intraoperative bleeding volume, gastrointestinal function recovery time, first under bed time, hospitalization time and other surgical recovery indicators, the levels of luteinizing hormone (LH), serum follicle stimulating hormone (FSH), estradiol (E2) and other sex hormones, and the improvement of the hemodynamic indexes such as pulsatility index (PI), resistance index (RI) and peak systolic velocity (PSV) before treatment and 1 month after treatment of the two groups were compared. Results The intraoperative bleeding volume in the observation group was less than that in the control group, and the gastrointestinal function recovery time, the first under bed time and the hospitalization time in the observation group were shorter than those in the control group, the differences were statistically significant (P<0.05). After treatment, the levels of LH and FSH in the observation group were lower than those in the control group, and the level of E2 in the observation group was higher than that in the control group, the differences were statistically significant (P<0.05). After treatment, the levels of PI and RI in the observation group were lower than those in the control group, and the PSV level in the observation group was higher than that in the control group, the differences were statistically significant (P<0.05). Conclusion Laparoscopic ovarian cyst excision has a better effect on patients with ovarian cysts, it can reduce the influence on patients′ ovarian function and has higher application value.

Objective To investigate the application effect of Levonorgestrel Intrauterine Contraceptive System with Didroxyprogesterone in adenomyosis (AM). Methods A total of 70 cases of AM patients from the Department of Obstetrics and Gynecology of Yichun People′s Hospital from January 2019 to December 2021 were selected as the study subjects, and they were divided into the study group and the control group, with 35 cases in each group. The patients in the control group were treated with Levonorgestrel Intrauterine Contraceptive System, and the patients in the study group were treated with Levonorgestrel Intrauterine Contraceptive System combined with Didroxyprogesterone. The menstrual situation, uterine situation, sex hormone level, the rate of drip bleeding and accidental exfoliation and the total effective rate were compared between the two groups before and after treatment. Results Before treatment, there were no significant differences in the menstrual volume, pain degree, endometrial thickness and uterine volume between the two groups (P>0.05). The menstrual volume of the study group after treatment was less than that of the control group, the degree of pain of the study group after treatment was less than that of the control group, and the endometrial thickness and uterine volume of the study group after treatment were less than those of the control group, the differences were statistically significant (P<0.05). Before treatment, there were no significant differences in the levels of luteinizing hormone (LH), serum follicle stimulating hormone (FSH) and estradiol (E2) between the two groups (P>0.05). The level of E2 of the study group after treatment was higher than that of the control group, the difference was statistically significant (P<0.05). The drip bleeding rate and the accidental shedding rate in the study group were lower than those in the control group, the differences were statistically significant (P<0.05). The total effective rate of the study group was higher than that of the control group, the difference was statistically significant (P<0.05). Conclusion Levonorgestrel Intrauterine Contraceptive System combined with Didroxyprogesterone was effective in treating AM, which can effectively improve the clinical symptoms, prevent adverse events and improve the quality of life.

105 Application of abdominal aortic balloon occlusion in type Ⅲ cesarean scar pregnancy

Objective To retrospectively study the clinical efficacy and safety of abdominal aorta balloon occlusion pretreatment in the treatment of type Ⅲ cesarean scar pregnancy (CSP). Methods From January 2019 to December 2021, 54 patients with type Ⅲ CSP diagnosed by transvaginal ultrasound or MRI in Maternal and Child Health Care Hospital of Jiangxi Province were selected as the research objects. According to different pretreatment methods, they were divided into the experimental group (26 cases) and the control group (28 cases). The experimental group was pretreated with abdominal aorta balloon occlusion, and then underwent laparoscopic scar pregnancy excision. The control group were pretreated with high intensity focused ultrasound (HIFU) before laparoscopic scar pregnancy excision. The amount of intraoperative bleeding, operation time, postoperative anal exhaust time, hospitalization expenses, hospitalization time and menstrual rehydration time were compared between the two groups. Results There was no significant differences in operation time, postoperative anal exhaust time, hospitalization time and hospitalization expenses, menstrual rehydration time between the two groups (P>0.05). The amount of intraoperative bleeding in the experimental group was less than that in the control group, the difference was statistically significant (P<0.05). Conclusion Before operation, abdominal aortic balloon occlusion pretreatment can significantly reduce the amount of intraoperative bleeding of type Ⅲ uterine scar pregnancy. It is a safe and effective pretreatment method, which can preserve the uterus to the greatest extent and preserve the reproductive function.

109 Postoperative recovery of gastrointestinal function in children with perforated appendicitis and its risk factors

Objective To observe the postoperative recovery of gastrointestinal function in children with perforated appendicitis and analyze its risk factors. Methods A total of 160 children with perforated appendicitis underwent surgical treatment enrolled in Affiliated Hospital of Youjiang Medical University of Nationalities from January 2017 to August 2021 were selected retrospectively as research objects. The evaluation indexes of gastrointestinal function recovery (bowel sound recovery time, anal exhaust time, defecation time) were compared among children with different clinical characteristics. The multivariate logistic regression was used to analyze the risk factors in postoperative poor recovery of gastrointestinal function. Results There were 136 children with postoperative good recovery of gastrointestinal function and 24 children with postoperative poor recovery of gastrointestinal function in 160 children. Univariate analysis results showed that the postoperative recovery of gastrointestinal function was correlated with the time from onset to operation, operation method, operation time, CO2 pneumoperitoneum pressure, peritoneal washes, postoperative caregivers, postoperative complications, postoperative intervention, and the differences were statistically significant (P<0.05). Multivariate logistic regression model analysis showed that the laparotomy (β=0.274, OR=1.249, 95%CI=1.022-1.615), operation time≥60 min (β=0.285, OR=1.358, 95%CI=1.097-1.704), CO2 pneumoperitoneum pressure≥12 mmHg (β=0.382, OR=1.375, 95%CI=1.119-1.762), postoperative complications (β=0.241, OR=1.218, 95%CI=1.064-1.637), without postoperative intervention (β=0.338, OR=1.276, 95%CI=1.025-1.696) were the independent risk factors in postoperative recovery of gastrointestinal function (P<0.05). Conclusion There were many risk factors of postoperative poor recovery of gastrointestinal function in children with perforated appendicitis, so targeted intervention measures should be taken to promote gastrointestinal function recovery.

 Objective To study the influence of nasal irrigation concentration and timing after ultrasonic atomization through nose and mouth on nasal ventilation function and clinical curative effect after chronic sinusitis operation. Methods A total of 80 patients with chronic sinusitis from Pingxiang People′s Hospital from March, 2020 to November, 2021 were selected as subjects for retrospective analysis. They were divided into four groups, A, B, C and D according to the treatment concentration and timing, with 20 patients in each group. Group A was rinsed with 0.9% saline 10 min after atomization inhalation, group B was rinsed with 2.5% saline 10 min after atomization inhalation, group C was rinsed with 0.9% saline 30 min after atomization inhalation, and group D was rinsed with 2.5% saline 30 min after atomization inhalation. The clinical efficacy, hospitalization indexes, nasal ventilation function and total incidence of complications were compared among the four groups. Results The total effective rate of group B was higher than that of groups A, C and D, and the difference was statistically significant (P<0.008). The hospitalization time and respiratory improvement time of group B were shorter than those of groups A, C and D, and the hospitalization expense of group B was lower than that of groups A, C and D, with statistically significant differences (P<0.05). After intervention, the total nasal resistance (RT) and nasal resistance difference ratio (Rlr) in group B were lower than those in groups A, C and D, and the differences were statistically significant (P<0.05). The total incidence of complications in group B was lower than that in groups A, C and D, and the difference was statistically significant (P<0.008). Conclusion Nasal irrigation with 2.5% saline 10 min after ultrasonic atomization through nose and mouth is more beneficial to the improvement of nasal ventilation function and clinical efficacy in patients with chronic sinusitis.

118 Effect of Recombinant Bovine Basic Fibroblast Grow Factor combined with Sodium Hyaluronate in the treatment of senile patients with dry eye after cataract surgery and its effect on tear film function and inflammatory factors

Objective To investigate the effect of Recombinant Bovine Basic Fibroblast Grow Factor (rb-bFGF) combined with Sodium Hyaluronate in the treatment of senile patients with dry eye after cataract surgery and its effect on tear film function and inflammatory factors. Methods A total of 76 elderly patients (76 eyes) with dry eye who were underwent cataract surgery in the People′s Hospital of Xintai City from November 2020 to October 2021 were selected as the research subjects, and they were divided into the treatment group (38 cases, 38 eyes) and the control group (38 cases, 38 eyes) by random touching-ball method. The control group was treated with Sodium Hyaluronate, the treatment group was treated with rb-bFGF combined with Sodium Hyaluronate. The clinical efficacy of the two groups were compared, and the tear film function and inflammatory factor levels were compared between the two groups before and after treatment. Results The total effective rate in treatment group was higher than that in control group, and the difference was statistically significant (P<0.05). After treatment, the tear film break-up time (BUT) levles of the two groups after treatment were longer than those before treatment, and the Schirmer Ⅰ test (SⅠt) lengths of the two groups were higher than those before treatment, corneal fluorescein staining (FL) scores of the two groups were lower than those before treatment, and BUT in treatment group after treatment was longer than that in control group, SⅠt in treatment group after treatment was higher than that in control group, FL score in treatment group after treatment was lower than that in control group, the differences were statistically significant (P<0.05). After treatment, the levels of interleukin-1β (IL-1β) and interleukin-8 (IL-8) in the two groups were lower than those before treatment, and compared, the levels of IL-1β and IL-8 in the treatment group after treatment were lower than those in the control group, and the differences were statistically significant (P<0.05). Conclusion The treatment of rb-bFGF combined with Sodium Hyaluronate in elderly patients with dry eye after cataract surgery can improve the clinical efficacy, improve tear film function and regulate the level of inflammatory factors.

122 Diagnostic value of prenatal three-dimensional power Doppler ultrasound in placental implantation

Objective To analyze the effect of prenatal three-dimensional power Doppler ultrasound in evaluating the severity of placental implantation. Methods Through the convenient sampling method, 100 pregnant women with placenta previa and placenta accreta treated in Jiangxi Maternal and Child Health Hospital from January 2017 to December 2020 were selected as the research objects. According to the severity of placenta implantation, the patients were divided into severe group (placenta attached to lower uterine scar site, 32 cases) and mild to moderate group (placenta not attached to lower uterine scar site, 68 cases). Another 100 normal women with placenta in the anterior wall were selected as the control group. All maternal antenatal achieved three-dimensional power doppler ultrasound and cesarean section surgery and the postoperative pathological results as the gold standard. The placental flow index (FI), vascular flow index (VFI) and peak blood flow velocity (PSV) were compared among the three groups, and the diagnostic efficacy of FI, VFI and PSV for severe placental implantation was analyzed. Results FI, VFI and PSV of the severe group and mild-moderate group were higher than those of the control group, and FI, VFI and PSV of the severe group were higher than those of the mild-moderate group, and the differences were statistically significant (P<0.05). The sensitivity, specificity, Youden index and area under curve (AUC) of VFI in the diagnosis of severe placental implantation were higher than those of FI and PSV. Conclusion Three dimensional power Doppler ultrasound is an effective way to diagnose placental implantation. It has high application value in evaluating the severity of placental implantation and is worthy of clinical attention.

126 CT imaging characteristics of pulmonary carcinoid in different location of airway

Objective To analyze the CT imaging characteristics of pulmonary carcinoid in different locations of airway. Methods Thirty cases of pulmonary carcinoid admitted to the First Affiliated Hospital of Guangzhou Medical University from January 2015 to December 2020 were retrospectively selected as the research objects. All patients underwent the plain, arterial and venous CT scan. The location, size, margin, density, enhancement degree, area of necrosis, calcification and lymph node metastasis were observed. Results According to lesions′ location in the airway, the lesions were divided into the central type, the hilar type, and peripheral type. According to lesions′ location in the airway, total 30 cases included 7 cases of central type, 9 cases of hilar type, and 14 cases of peripheral type. In 7 cases of central type, 4 nodules and 3 masses mainly located in trachea or main bronchial lumen. Especially, all masses were accompanied by extratubular growth. Nine cases of hilar type included 8 nodules and 1 mass in lobar or segmental bronchus, whose accompanied symptoms included 2 cases of pulmonary atelectasis, 1 case of mucous embolus in distal bronchial lumen, and 6 cases of obstructive inflammation. 14 cases of peripheral type in small bronchus (that is below the segmental bronchus) had 12 nodules and 2 masses, in which 11 cases had lobulation sign. The solid part of 90% lesions had moderate or severe enhancement in arterial phase, and the solid part of 100% lesions had moderate or severe enhancement in the venous phase. In 30 cases of pulmonary carcinoid, 18 cases showed patchy low-density area in the lesion and 12 cases showed no low-density. Only six cases of Hilar and mediastinal lymphadenopathy metastases were observed. All cases were surgically removed or biopsied, which included 17 cases of typical and 13 cases of atypical carcinoid. Conclusion Pulmonary carcinoid has different typical imaging manifestations in different location of tracheal bronchial tree. The density and enhancement are also distinctive for pulmonary carcinoid.

130 Diagnostic value of contrast-enhanced ultrasound and transrectal real-time elastography for benign and malignant prostate diseases

Objective To explore the diagnostic value of contrast-enhanced ultrasound (CEUS) and transrectal real-time elastography (TRTE) for the benign and malignant prostate diseases. Methods The clinical data of 135 patients (146 lesions) who underwent rectal prostate biopsy in Taizhou Hospital of Traditional Chinese Medicine from June 2019 to December 2021 were retrospectively analyzed, and the diagnostic value of TRTE and CEUS in benign and malignant prostate lesions was compared with the pathological diagnosis results of biopsy as the gold standard. Results The strain ratio (SR) of benign lesions was lower than that of malignant lesions during TRTE examination, and the difference was statistically significant (P<0.05). The accuracy of CEUS combined with TRTE in diagnosing benign and malignant prostate lesions was 93.84% (137/146), the specificity was 96.92% (63/65), the sensitivity was 91.36% (74/81), the positive predictive value was 90.00% (63/70), and the negative predictive value was 97.37% (74/76). The area under curve (AUC) of combined diagnosis was higher than CEUS and TRTE alone, and the difference was statistically significant (P<0.05). Conclusion The combination of CEUS and TRTE can improve the diagnostic efficiency of benign and malignant prostate diseases, and has a high application value in prostate biopsy.

135 Inquire into the attitude and demand of community pharmacy in developing diabetes management from different angles

Objective To explore the attitudes and needs of diabetes management in community pharmacies from three perspectives: community pharmacy managers, licensed pharmacists and diabetic patients, and to provide reference for community pharmacies to develop diabetes management services. Methods Three questionnaires were designed for community pharmacy managers, licensed pharmacists and diabetic patients respectively, and questionnaires were conducted and statistically analyzed in selected pharmacies in Nanjing for their attitudes, perceptions and needs for diabetes management services. Results 98.8% of community pharmacy managers thought it was necessary to provide diabetes chronic disease management services, and 93.8% thought it was necessary to have a diabetes specialist pharmacist. All licensed pharmacists thought it was necessary to receive regular professional training on diabetes, and 87.8% thought it was necessary to make regular telephone follow-up visits or home follow-up visits to diabetes patients. 8.6% of diabetes patients thought it was necessary to make regular return visits to ask about their self-monitoring results of blood glucose, and none patients said that when their blood glucose was treated but did not reach the standard, the licensed pharmacists alway advised them to adjust their treatment plan and gave suggestions. The satisfaction rate was 6.3% for community pharmacy managers, 5.8% for licensed pharmacists and 21.6% for diabetic patients. Conclusion Chinese community pharmacies and licensed pharmacists showed a positive attitude towards developing diabetes management in China, but their satisfaction was low. As the main provider of diabetes management services, licensed pharmacists should constantly improve their professional skills to eventually gain patients′ trust, improve satisfaction, and provide more urgent and professional chronic disease management services for diabetic patients.

140 Analysis and intervention of unqualified medical orders in department of pediatrics in pharmacy intravenous admixture service of Zibo Central Hospital

Objective The unqualified prescriptions of pediatrics in the Department of Pharmacy of Zibo Central Hospital were statistically analyzed, and the problems existing in the medication process were summarized to sum up the experience for the rational use of pediatrics drugs. Methods Retrospective statistical analysis was conducted on unqualified prescriptions of pharmacy intravenous admixture services (PIVAS) of Zibo Central Hospital from January 1 to June 1, 2021. And many measures were taken to intervene the unqualified prescriptions. Results During the statistical period, PIVAS reviewed a total of 16 086 pediatric prescriptions, 1494 irrational dosage ranges, 388 compatible concentrations, 272 routes of administration, 45 drugs for children, 40 drug contraindications, 13 interactions and 2 repeated drugs. After the implementation of the intervention measures, the proportion of unqualified prescriptions decreased from 19.50% to 8.91%. The average success rate of intervention with unqualified prescriptions was 99.58%, and the failure of intervention mainly focused on the dose range. Conclusion Irrational drug use in pediatrics is a serious problem. PIVAS should strengthen prescription audit in this department, and prescription audit pharmacists should strengthen professional knowledge learning to promote the rational use of children′s drugs.

144 Application effect of outpatient and emergency prescription pre-audit system in ensuring drug safety of patients

Objective To explore the application effect of outpatient and emergency prescription pre-audit system in ensuring drug safety of patients. Methods A total of 1200 outpatient and emergency prescriptions implemented by the prescription pre-audit system from December 2021 to January 2022 in Loudi Central Hospital were selected as the study group, 1200 outpatient and emergency prescriptions not implemented by the prescription pre-audit system from October to November 2021 were selected as the control group, the qualified rate of prescription quality and the success rate of pharmacists′ intervention were compared between the two groups. Results The qualified rates of emergency prescription and outpatient prescription in the study group were 96.00% and 97.87% respectively, higher than those in the control group (92.50% and 93.13%), the differences were statistically significant (P<0.05). The intervention success rate of outpatient pharmacists (93.75%) and emergency pharmacists (91.43%) in the study group were higher than those in the control group, and the differences between the two groups were statistically significant (P<0.05). Conclusion The implementation of prescription pre-audit system in outpatient and emergency department can improve the success rate of prescription intervention by pharmacists, strengthen the qualified rate of prescription, and ensure the safety of medication for patients, which has certain clinical value.

147 Application effect of Buyang Huanwu Decoction combined with four foot needles in post-stroke ankle dysfunction

Objective To discuss the application effect of Buyang Huanwu Decoction combined with four foot needles in post-stroke ankle dysfunction. Methods A total of 80 stroke patients with ankle joint disorder admitted to Shangrao People′s Hospital from March to September 2021 was selected as the research objects. The patients was divided into the observation group and the control group according to random number table method, with 40 cases in each group. Patients in the control group was given routine rehabilitation training and treated with Buyang Huanwu Decoction, while patients in the observation group was given routine rehabilitation training and treated with Buyang Huanwu Decoction combined with four foot needles. The neurological function, activities of daily living, ankle function, walking function and adverse reactions were compared between the two groups. Results There was no statistically significant difference between the two groups in the National Institutes of Health stroke scale (NIHSS) score before treatment (P>0.05). After treatment, the NIHSS score of observation group was lower than those of control group, with statistically significant difference (P<0.05). There was no statistically significant difference between the two groups in the activity of daily living scale (ADL) score before treatment (P>0.05). The ADL score of observation group was higher than that of control group after treatment, with statistically significant difference (P<0.05). There was no statistically significant difference between the two groups in the American orthopedics foot and ankle score (AOFAS) before treatment (P>0.05). The AOFAS score of observation group was higher than that of control group after treatment, with statistically significant difference (P<0.05). There was no statistically significant difference between Hold-on functional ambulation category (FAC) score before treatment (P>0.05). The FAC score of observation group was higher than that of control group after treatment, with statistically significant difference (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion Buyang Huanwu Decoction combined with four-needle acupuncture has a good effect, and the patients′ nerve defect, mobility, ankle function and walking function have been significantly improved, and the patients′ prognosis is good.

Prudent nursing syndrome differentiation should root in the theoretical TCM classics. Therefore, full excavation of the nursing connotation in TCM classical works like Treatise on Cold Damage combining with modern nursing techniques of integrated traditional Chinese and Western medicine can enrich the more practical techniques in current clinical nursing, inspire new ideas, and develop research goals. In this article, we focused on the nursing content recorded in the classical formulas in Treatise on Cold Damage by following the principle of formula-syndrome correspondence and reference of syndrome differentiated system of formulas-like-formula and syndrome-primary syndrome and explored the differentiated methods of nursing by taking nursing content of Guizhi Decoction syndrome and Chaihu Decoction syndrome as examples. In addition, together with three methods of sweat promoting, harmonization, and purgation among eight medical therapeutic methods, attempted to probe into the Zhongjing′s nursing thought.

156 Prognostic value of neutrophil / lymphocyte ratio and platelet/lymphocyte ratio for progression-free survival in patients with multiple myeloma initially treated with a three-drug combination regimen

 Objective To explore the relationship between the efficacy and prognosis of neutrophil/lymphocyte ratio (NLR) and platelet / lymphocyte ratio (PLR) in progression-free survival (PFS) in patients with newly treated multiple myeloma (MM) who received three-drug regimen (RCD regimen). Methods Seventy newly diagnosed MM patients in Yichun People′s Hospital from January 2016 to June 2018 were selected as the research subjects. All patients were treated with RCD regimen. According to the efficacy, patients were divided into ≥ very good partial response (VGPR) group and < VGPR group, and the differences of NLR and PLR between the two groups were compared. The predictive value of serum NLR and PLR for curative effect was analyzed by ROC curve, and AUC, sensitivity and specificity were calculated. The 3-year PFS survival curve of MM patients was constructed by Kaplan-Meier method, and the survival curve was analyzed by log-rank test. Multivariate Cox regression analysis was performed on the factors affecting prognosis in univariate analysis. Results NLR and PLR in ≥VGPR group were lower than those in < VGPR group, and the differences were statistically significant (P<0.05). ROC curve analysis showed that the AUC, sensitivity and specificity of NLR for predicting post-treatment efficacy were 0.802, 70.5% and 82.7% respectively. The AUC of PLR was 0.794, the sensitivity was 66.7%, and the specificity was 84.6%. The AUC of NLR+PLR was 0.843, the sensitivity was 61.5%, and the specificity was 94.2%. The AUC, sensitivity and specificity of NLR+PLR were the highest. Univariate analysis showed that β2 microglobulin, NLR and PLR were the related factors affecting PFS 3 years after treatment (P<0.05). Log-rank test showed that the 3-year PFS of patients with low NLR and PLR was better than that of patients with high NLR and PLR. Multivariate Cox regression analysis showed that NLR <2.55 and PLR <143.88 were protective factors for PFS in MM patients within 3 years after treatment (P<0.05). Conclusion The NLR and PLR of MM patients are related to the curative effect. Higher NLR and PLR predicted shorter progression-free survival time in patients with MM. The combined application of NLR and PLR can better evaluate the prognosis of newly diagnosed MM patients, and has important guiding significance for the clinical treatment and curative effect of the patients.

161 Study on the value of combined detection of glycosylated hemoglobin, glycosylated serum protein and C-peptide in the treatment of patients with type 2 diabetes mellitus

Objective To analyze the application value of combined glycated hemoglobin (HbA1c), glycated serum protein (GSP), and C-peptide (C-P) testing in the diagnosis and treatment of patients with type 2 diabetes. Methods From January to December 2020, A total 50 suspected diabetic patients whose clinical test blood glucose results exceeded the highest value of the threshold in Nanchang First Hospital were selected as the case group, 50 healthy individuals with normal examination results at the same time were selected as the control group. The levels of HbA1c, GSP and C-P in the two groups were detected, and the differences of above indexes between the two groups were observed. The fasting blood glucose (FBG) and oral glucose tolerance test (OGTT) were used as the gold standard. The diagnostic efficacy of HbA1c, GSP, C-P single detection and combined detection were compared. Results The levels of HbA1c and GSP in the case group were higher than those in the control group, and the levels of C-P were lower than those in the control group, with statistically significant differences (P<0.05). There were significant differences in sensitivity, accuracy and negative predictive value among single detection of HbA1c, GSP, and C-P and combined detection (P<0.05). There were no significant differences in the specificity and positive value among the four detection methods (P>0.05). Conclusion The combined detection of HbA1c, GSP and C-P ensures the accuracy of diagnosis and treatment, and has the significance of monitoring and guiding the clinical treatment of type 2 diabetes, providing a strong basis for clinical diagnosis and treatment.

166 Influencing factors and diagnostic value of lung cancer related cerebral hemorrhage

Objective To study the influencing factors of lung cancer related cerebral hemorrhage (LCRCH) and the diagnostic value of the main factors. Methods Clinical data of 307 patients with lung cancer hospitalized in Dazhou Central Hospital from March 2014 to April 2020 were retrospectively analyzed, including 56 patients with LCRCH in the experimental group and 251 patients with lung cancer in the control group. Single factor analysis and propensity score matching (PSM) were used to screen out the suspected influencing factors. Logistic regression was used to analyze the independent risk factors. Receiver operating characteristic (ROC) curve was drawn and the diagnostic indexes were calculated. Results The results of univariate analysis and PSM showed that platelets (PLT), prothrombin time (PT) and serum tumor markers carcinoembryonic antigen (CEA), carbohydrate antigen (CA) 125 and CA199 were significantly different between the test group and control group (P<0.05). There were no statistically significant differences in gender, age, lung cancer type, activated partial thrombin time (APTT), international standardized ratio (INR), and serum tumor marker neuron specific enolase (NSE), cyto-keratin 19 fragment antigen 21-1 (CyFRA21-1) between the two groups (P>0.05). Logistic regression analysis showed that CEA level was increased (β=0.001, OR=1.001, 95%CI=1.000-1.002), CA199 level was increased (β=0.004, OR=1.004, 95%CI=1.003-1.004) and PT was prolonged (β=0.157, OR=1.170, 95%CI=1.084-1.263) were independent risk factors for LCRCH (P<0.05). The area under ROC curve (AUC) of PT for LCRCH diagnosis was 0.82 (95%CI=0.74-0.89). Conclusion Prolonged PT and elevated CEA and CA199 levels are independent risk factors for LCRCH, and PT can be used as a good indicator for the diagnosis of this disease.

170 Effect of early activity management program for kinesiophobia on postoperative rehabilitation of elderly patients with hip arthroplasty

Objective To explore the effect of early activity management scheme of kinesiophobia in elderly patients undergoing hip arthroplasty (THA). Methods A total of 60 patients with postoperative kinesiophobia after THA treated in Jiujiang Hospital of Traditional Chinese Medicine from October 2019 to October 2021 were selected as the research object. They were divided into control group (30 cases) and observation group (30 cases) by random number table method. The control group was given routine nursing care, and the observation group was given the early activity management plan for kinesiophobia on the basis of the control group. The level of kinesiophobia, pain, postoperative rehabilitation, hip function and the total incidence of complications were compared between the two groups. Results After nursing, the scores of tampa scale of kinesiophobia (TSK) and visual analogue scale in the observation group were lower than those in the control group, with statistical significances (P<0.05). The first time out of bed activity time and hospital stay in the control group were shorter than those in the control group, with statistical significances (P<0.05). The score of hip function scale (HHS) in the observation group was higher than that in the control group 3 months after operation, with statistical significance (P<0.05). The total incidence of complications was  lower than that in the control group, with statistical significance (P<0.05). Conclusion The early activity management scheme of kinesiophobia can reduce the kinesiophobia symptoms and postoperative pain of elderly THA patients, reduce complications, promote the postoperative rehabilitation of patients, and is conducive to the early recovery of hip function.

173 Effect of cluster nursing model on rehabilitation process and satisfaction of postoperative patients with ectopic pregnancy with hemorrhagic shock

Objective To explore the effect of cluster nursing mode on postoperative patients with ectopic pregnancy with hemorrhagic shock. Methods A total of 82 ectopic pregnancy patients with hemorrhagic shock admitted to the Yichun Second People′s Hospital from January 2020 to October 2021 were selected as the study subjects, and they were divided into control group (41 cases) and observation group (41 cases) according to random number table method. The control group received routine nursing, while the observation group received cluster nursing on the basis of the control group. The rehabilitation process, total incidence of complications and total nursing satisfaction were compared between the two groups. Results In the observation group, the postoperative catheter removal time ([5.69±1.27] h), anal exhaust time ([15.64±2.55] h), first time out of bed ([8.75±2.30] h), hospital stay ([5.26±0.54] d) were shorter than those in the control group ([25.53±2.64] h, [27.32±3.14] h, [26.12±3.23] h, [7.42±1.13] d], the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was 7.32%, lower than 24.39% in the control group, and the total nursing satisfaction was 95.12%, higher than 80.49% in the control group, the differences were statistically significant (P<0.05). Conclusion The cluster nursing model can accelerate the rehabilitation process of patients with ectopic pregnancy with hemorrhagic shock after surgery, reduce the incidence of complications, and improve patient satisfaction.

177 Effect of early activity management program for kinesiophobia on postoperative rehabilitation of elderly patients with hip arthroplasty

Objective To discuss the effect of narrative nursing combined with psychological nursing on psychological state and quality of life of patients undergoing interventional therapy for intracranial aneurysm. Methods From July 2020 to July 2021, 84 patients with intracranial aneurysm interventional therapy treated in the Department of Neurosurgery of Huidong People′s Hospital were selected as the research objects. Patients were divided into study group (42 cases) and reference group (42 cases) according to the order of treatment. The study group used narrative nursing combined with psychological nursing intervention, the reference group used traditional conventional nursing intervention. The changes of mental state, quality of life and complications were compared between the two groups. Results The anxiety score and depression score of the two groups after nursing were lower than those before nursing, and the anxiety score and depression score of the research group after nursing were lower than those in the reference group, the differences were statistically significant (P<0.05). The scores of social function, physical function, cognitive function and role function of quality of life in the two groups after nursing were higher than those before nursing, and the scores of quality of life in the study group after nursing were higher than those in the reference group, the differences were statistically significant (P<0.05). The total incidence of post-nursing complications in the study group was 2.38%, lower than that in the reference group (28.57%), and the difference was statistically significant (P<0.05). Conclusion The clinical application of narrative nursing combined with psychological nursing intervention in patients with intracranial aneurysm interventional therapy can effectively improve patients′ negative emotions such as anxiety, improve their confidence in treatment, control the incidence of complications and achieve the expected clinical efficacy and quality of life, with outstanding clinical application value.

181 Effect evaluation of standardized nursing process management in patients with prone position ventilation

Objective To explore the effect of standardized nursing process in prone position ventilation (PPV) patients. Methods A total of 60 patients conducted PPV in the Department of Critical Care Medicine of the Second People′s Hospital of Zhaoqing from January 1, 2019 to May 31, 2021 were selected as research objects. According to the time nodes, 30 patients who did not implement standardized care process management from 1 January, 2019 to 30 April, 2020 were served as control group, 30 patients with standardized care process management from 1 May, 2020 to 31 May, 2021 were served as observation group. The incidence of complications, target calorie rate, lymphocyte count and ventilation duration in prone position were compared between the two groups. Results The rates of unplanned extubation, facial edema, pressure ulcers, body variable hypotension and nasal feeding fluid reflux aspiration in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). The target heat compliance rate in the observation group was higher than that in the control group, the lymphocyte count in the observation group was more than that in the control group, and the prone ventilation in the observation group was longer than that in the control group, the differences were statistically significant (P<0.05). Conclusion Implementing standardized nursing process management can reduce the complications of prone ventilation patients and improve their tolerance, and reach the expected value of prone ventilation treatment to the maximum extent.

185 Application effect of drug-assisted Epley manipulation reduction therapy in patients with benign paroxysmal vertigo

Objective To investigate the application effect of drug-assisted Epley technique reduction therapy in patients with benign paroxysmal vertigo (BPPV). Methods A total of 62 patients with benign paroxysmal vertigo in Panyu District Central Hospital from October 2020 to October 2021 were selected as the research object, they were divided into study group (n=31) and control group (n=31) by random sampling method. Both groups were given drug-assisted Epley manipulation reduction therapy, the control group was given routine care, and the research group was given overall care. The scores of hospital anxiety and depression scale (HADS) and University of California Los Angeles (UCLA), reset success rate, the score of short form 36 questionnaire (SF-36) and nursing satisfaction were compared between the two groups. Results The UCLA and HADS scores of the study group were lower than those of the control group after 1 and 4 weeks of reduction, and the differences were statistically significant (P<0.05). There was no significant difference in the total effective rate of reduction between the two groups (P>0.05). The scores of various quality of life dimensions in the study group were higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusion BPPV patients combined overall treatment with Epley can improve the success rate of reduction and improve vertigo symptoms.

189 Role of BCW theory assisted health education in the process of operating room care for ophthalmic patients

Objective To study the effect of behavior change wheel (BCW) assisted health education in the process of operating room care for ophthalmic patients. Methods A total of 100 patients (100 eyes) who underwent surgical treatment in the Department of Ophthalmology of the Second Affiliated Hospital of Dalian Medical University from February 2020 to March 2021 were selected as the research subjects. They were divided into observation group (50 patients, 50 eyes) and control group (50 patients, 50 eyes) by computer random number table method. Patients in the observation group were carried out BCW theory assisted health education nursing intervention, patients in the control group were carried out traditional eye surgery nursing intervention. The awareness and mastery of disease, visual acuity, self-rating anxiety scale (SAS) and self-rating depression scale (SDS) scores were compared between the two groups before and after nursing. The length of stay and total satisfaction of nursing were compared between the two groups. Results The disease knowledge, examination guidance, disease observation, daily health care scores and total score of observation group were higher than those of control group after nursing, the differences were statistically significant (P<0.05). The length of hospitalization in observation group was shorter than that in control group, and the difference was statistically significant (P<0.05). The visual acuity level of observation group was higher than control group after nursing, the difference was statistically significant (P<0.05). The SAS and SDS scores of two groups after nursing were lower than those before nursing, and SAS and SDS scores of observation group after nursing were lower than those in control group, the differences were statistically significant (P<0.05). The total nursing satisfaction of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). Conclusion After BCW theory assisted health education is applied in the operating room nursing process of ophthalmic patients, it can effectively improve patients' cognition and grasp of eye diseases, shorten the length of hospital stay, improve nursing satisfaction and vision, and improve negative emotions, which is worth promoting.

193 Effect of evidence-based nursing on limb function of patients with breast cancer after operation

Objective To investigate the clinical effect of evidence-based nursing for breast cancer patients after surgery. Methods A total of 80 postoperative patients with breast cancer who were admitted to the Department of Breast Surgery of Fujian Cancer Hospital from June 2019 to June 2020 were retrospectively analyzed and divided into control group and observation group by their mode of nursing, with 40 cases in each group. The control group was given routine nursing and the observation group was given evidence-based nursing, compare the nursing outcomes of the two groups, such as lymphedema status, quality of life, upper extremity function. Results There were no statistical significances in elbow function score (Mayo) and total length of arm circumference summation (SOAC) between the two groups before nursing (P>0.05). The total length of SOAC was lower than that of the control group, the Mayo score was higher than that of the control group at 3 months after nursing in the observation group, and the differences were significant (P<0.05). There were no statistical significances in the scores of 36-item short form health survey (SF-36) before nursing between the two groups (P>0.05). The SF-36 scores of the observation group after nursing were higher than those of the control group, the differences were statistically significant (P<0.05). Conclusion Breast cancer patients receiving evidence-based nursing postoperatively can significantly improve their upper extremity function, reduce lymphedema status and improve their quality of life.
