CHEN Yanquan;Li Jia;Deng Minjun
Objective To investigate the relationship between ultrasonic features, clinicopathological and biological indicators and lymphovascular invasion of invasive ductal breast carcinoma. Methods A total of 108 patients with invasive ductal breast carcinoma treated in the Department of Breast of Guangdong Second Traditional Chinese Medicine Hospital from January 2017 to April 2021 were selected retrospectively as the study subjects. They were divided into group with lymphovascular invasion (63 cases) and group without lymphovascular invasion (45 cases) according to the postoperative pathological results. The relationship between ultrasonic features, clinicopathological and biological indicators of invasive ductal breast carcinoma and lymphovascular invasion of invasive ductal breast carcinoma were analyzed. The value of four ultrasonic features of abnormal axillary lymph nodes in diagnosis of lymphovascular invasion in invasive ductal breast carcinom was also assessed. Results Univariate analysis of ultrasonic features showed that invasive ductal breast carcinomas showed more irregular shape, non-parallel orientation, internal microcalcification, posterior echo attenuation and abnormal axillary lymph nodes in the lymphovascular invasion group, with statistically significant differences (P<0.05). Multivariate logistic regression analysis showed that irregular morphology (β=2.093, OR=8.106, 95%CI=1.634-40.207) and abnormal axillary lymph nodes (β=2.509, OR=12.287, 95%CI=4.464-33.822) were independent risk factors for lymphovascular invasion in invasive ductal breast carcinoma respectively (P<0.05). Univariate analysis of clinicopathological and biological indicators showed that more proliferating cell nuclear antigen-67 (Ki-67) ≥20% and TNM stage 3 were found in the lymphovascular invasion group of invasive ductal breast carcinoma, with statistically significant differences (P<0.05). There were no statistically significant differences in the area under the ROC between the four features of ultrasonographic abnormal axillary lymph nodes and their combined diagnosis (P>0.05). Conclusion Irregular morphology of mass and abnormal axillary lymph nodes are independent risk factors for lymphovascular invasion in invasive ductal breast carcinoma respectively. Combined with clinicopathological and biological indicators, it is meaningful to assess lymphovascular invasion of invasive ductal breast carcinoma, and provides a useful basis for prognostic assessment and adjuvant treatment of breast cancer.

XU Panfeng;WEI Yanguo▲;LIANG Jiongwen;YANG Shangrong;ZHOU Yuanlin
Knee osteoarthritis (KOA)is one of the common diseases in the field of orthopedics, which seriously affects the quality of life of patients. At present, the commonly used knee sparing surgical treatment schemes include arthroscopic surgery, high tibial osteotomy (HTO), unicondylar joint replacement (UKA) and proximal fibula osteotomy (FO).Each surgical procedure has its advantages and disadvantages, and the choice of surgical method is a topic of discussion in the medical field recently. In order to explore the clinical efficacy of four kinds of knee sparing surgery for knee osteoarthritis, this study is to summarize and conclude some recent methods of knee sparing surgery in KOA. The advantages and disadvantages, indications and complications of four types of KOA knee sparing surgery were reviewed.According to the analysis of its curative effect, each surgical technique has its advantages and limitations. If there is little trauma, quick recovery, low cost, less damage to other bone tissues, and no impact on other surgical plans in the future, peroneal osteotomy and knee arthroscopy can be given priority. For early and young patients, high tibial osteotomy can be used in pursuit of greater mobility. In elderly patients, in order to reduce postoperative complications and revision rate, knee joint single condyle replacement can be selected. At the same time of choosing surgical plan, we should consider many factors to make an appropriate surgical plan for patients.

ZOU Yun;YUAN Hua;FU Ronghua;JIN Pingliang
Objective To observe the clinical effect of auricle appliances in the treatment of congenital auricle deformities in children. Methods A total of 32 infants with congenital auricle deformities treated with auricle appliances in the Jiangxi Provincial Children′s Hospital from May 2018 to May 2019 were selected. According to the Byrd classification, there were cryptotia (n=14), lop ears (n=8), cup ear (n=2), protruding ears (n=2), helix deformities (n=4), and mixed deformities (n=2). They were divided into the younger group (0-3 months) and the elder group (3-6 months) according to age, with 16 cases in each group. Photographs were taken before and after treatment, with a period of 2 to 4 weeks. The curative effects of children after wearing were observed. Results After the treatment, the auricle morphologies were evaluated by family members and doctors jointly, which were divided into excellent (same as normal ears), good (basically normal), and poor (no change or slight improvement). All auricle deformities were improved, 29 cases (90%) were evaluated as excellent, and 3 cases (10%) were good. Two of the children had skin eczema, and one had small skin ulcers, which improved 3 days after topical application of erythromycin ointment. No obvious complications were observed in the remaining ears. After 1 year, there was no recurrence in the re-examination. Conclusion Auricle appliances have an obvious effect on non-invasive treatment of auricle deformities, and the earlier the correction is implemented, the better the effect is. The auricle appliances have a low incidence of adverse reactions and a short wearing time, which can avoid later surgical treatment and psychological hazards. The auricle appliances are easy to operate and promote, becoming the first choice for the treatment of auricle deformities.
