4 Exploration of the mechanism of regulating uterus peristaltic wave by Shoutai Sijunzi Decoction based on network pharmacology

Objective To explore the potential target and mechanism of the regulation of uterus peristaltic wave by Shoutai Sijunzi Decoction based on network pharmacology and molecular docking techniques. Methods Based on Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and Traditional Chinese Medicines Integrated Database (TCMID), the active ingredients and targets of Shoutai Sijunzi Decoction were screened. At the same time, Gencards and DrugBank databases were used to mine the relevant targets of uterus peristaltic waves, and the potential targets of regulating abnormal endometrial peristalsis waves were obtained after the intersections were taken. The protein interaction network of potential target was constructed using the STRING platform, and the "drug-active ingredients-intersection target-disease" network was constructed using Cytoscape software. The "clusterProfiler" package was used for gene ontology (GO) functional enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis. PyMOL and AuTo Dock software were used to verify the molecular docking between the core target and the active component monomer. Results A total of 594 targets of Shoutai Sijunzi Decoction were retrieved, 90 targets of uterus peristaltic wave correlation, and 33 intersection targets were obtained. Kaempferol, β-sitosterol, quercetin, isorhamnetin and 7-methoxy-2-methyl isoflavones were the main active monomers of tetraphanoides, and nitric oxide synthase 3 (NOS3), transient receptor potential vanilloid subtype (TRPV1), prostaglandin-endoperoxide synthase 2 (PTGS2), caspase 3 (CASP3) and prostaglandin-endoperoxide synthase 1 (PTGS1) were the core action targets. The KEGG pathway was mainly concentrated in neuroactive ligand-receptor interaction, calcium signaling pathway, 5-hydroxytryptaminergic synapse, renin secretion, inflammatory mediators regulation of transient receptor potential (TRP) channels, cyclic guanosinc monophosphate-protein kinase G (cGMP-PKG) signaling pathway and relaxin signaling pathway. Molecular docking showed that kaempferol, β-sitosterol, quercetin, isorhamnetin and 7-methoxy-2-methyl isoflavone could stably bind to the core target. Conclusion Shoutai Sijunzi Decoction may target core genes such as NOS3, TRPV1, PTGS2, CASP3 and PTGS1 to regulate the neuroactive ligand-receptor interaction, calcium signaling pathway, 5-hydroxytryptaminergic synapse, renin secretion, inflammatory mediators regulation of TRP channel, cGMP-PKG signaling pathway and relaxin signaling pathway. It plays the role of regulating the uterus peristaltic wave.

10 Molecular mechanisms of LncRNA-CASC2 competitively binding miR-193a-5p regulating GSDMD scorch death pathway reversing Cisplatin resistance in non-small cell lung cancer

Objective To explore the molecular mechanism of the competitive binding of LncRNA-CASC2 to miR-193a-5p in regulating GSDMD focal death pathway to reverse cisplatin resistance in non-small cell lung cancer. Methods Non-small cell lung cancer (NSCLC) cell line A549 was selected to establish a drug-resistant strain A549/DDP, and the sensitivity of the cell to cisplatin was detected by CCK8 to verify whether the drug-resistant cell line was successfully established. The expression of CASC2 in A549 and A549/DDP cells and its effect on the expression of GSDMD2 and miR-193a-5p were detected by PCR. The binding sites of LncRNA-CASC2 and miR-193a-5p, miR-193a-5p and GSDMD were predicted by bioinformatics software. The target relationship between CASC2 and miR-193a-5p and miR-193a-5p and GSDMD was analyzed by luciferase reporter gene, and the competitive binding of LncRNA-CASC2 to miR-193a-5p was detected by Western blot. Results The semi-inhibitory concentration of A549/DDP cells was more than 10 times that of A549 cells, indicating that the drug-resistant cell line A549/DDP had been successfully constructed. The expression level of CASC2 in A549/DDP was lower than that in A549 (P<0.05). CASC2 mimics inhibited the expression of miR-193a-5p in cells, while GSDMD expression was up-regulated (P<0.05). There was a significant targeting relationship between CASC2 and miR- 193a-5p, and between miR-193a-5p and GSDMD (P<0.05). Conclusion The low expression of LncRNA-CASC2 and miR-193a-5p in NSCLC cells and the high expression of GSDMD are closely related to the chemotherapy resistance of NSCLC. The interaction of LncRNA-CASC2 and miR-193a-5p regulates GSDMD and promotes the drug resistance of tumor cells.

15 Efficacy observation of different cut-off values of tumor abnormal protein in screening lung cancer

Objective To explore the efficacy of different cut-off values of tumor abnormal protein (TAP) in screening lung cancer. Methods A total of 66 lung cancer patients and 66 non-lung cancer patients admitted to Jiangxi Chest Hospital from January 2021 to January 2022 were selected as the research objects. The area difference of TAP aggregate between lung cancer patients and non-lung cancer patients was calculated to determine whether TAP can be used to diagnose lung cancer. The optimal cut-off value of TAP was obtained by drawing the ROC curve to diagnose lung cancer and determine the diagnostic efficiency. The diagnostic efficiency was compared with the clinical common TAP normal reference range (<121 μm2). Results TAP condensed area of lung cancer group is greater than the lung cancer group, and the difference was statistically significant (P<0.05). By drawing the ROC curve, the highest cut-off value>170 μm2 was determined, and the diagnostic efficacy was compared with the normal reference range of TAP (<121 μm2). The results showed that there were no significant differences in the positive detection rate, sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic coincidence rate between the two groups (P>0.05). Conclusion Different cut-off values of TAP are used to screen suspected lung cancer patients, and the screening effect is better with the cut-off value of 170 μm2, which is worthy of clinical application.

19 Influencing factors on postoperative hospital acquired pneumonia in surgical patients

Objective To explore the relevant influencing factors of postoperative hospital acquired pneumonia (HAP) in surgical patients. Methods The clinical data of 2 824 patients undergoing surgical treatment in Jiujiang First People's Hospital from January 2020 to February 2023 were retrospectively selected. The patients were divided into HAP group (31 cases) and non HAP group (n=2 793 cases) according to whether they had HAP after surgery. Results The results of single factor analysis showed that there were significant differences in the incidence between patients with different age, nature of surgery, mode of anesthesia, diabetes, smoking history, tracheotomy, indwelling gastric tube, and operation time >3 h (P<0.05); Sex, type of Surgical incision, amount of intraoperative bleeding, and hypertension were not related to postoperative HAP in surgical patients, and the difference was not statistically significant (P>0.05); According to logistic regression analysis, age ≥ 60 years old (β=0.899, OR=2.458, 95%CI=1.205-5.011), emergency surgery ( β=1.985, OR=7.279, 95%CI=3.488-15.188), general anesthesia (β=2.739, OR=15.464, 95%CI=2.106-113.569), combined with diabetes (β=0.983, OR=2.672, 95%CI=1.310-5.450), smoking history (β=1.150, OR=3.157, 95%CI=1.537-6.484), tracheotomy (β=1.043, OR=2.837, 95%CI=1.331-6.047), indwelling gastric tube (β=0.753, OR=2.123, 95%CI=1.035-4.355), surgery time>3 h (β=2.198, OR=9.006, 95%CI=4.384-18.501) were high-risk factors for postoperative HAP in surgical patients (P<0.05). Conclusion Surgical patients are prone to postoperative HAP, which is influenced by multiple factors. Corresponding prevention and treatment strategies can be developed for each factor in clinical practice, which may reduce the occurrence of HAP.

23 Effect of rapid initiation of highly active antiretroviral therapy on treatment compliance and prognosis of human immunodeficiency virus infected acquired immune deficiecy syndrome patients with high CD4+T lymphocytes

Objective To investigate the effect of rapid initiation of highly active antiretroviral therapy (HAART) on the treatment compliance and prognosis of human immunodeficiency virus infected acquired immune deficiecy syndrome (HIVAIDS) patients with high CD4+T lymphocytes. Methods From July to December 2021, 61 HIVAIDS patients hospitalized in Department of Infectious Diseases, Jiangxi Xinyu People's Hospital were selected. 32 patients who went to the hospital in time within 7 days after diagnosis and started antiviral treatment with informed consent were set as the fast start group. 29 patients who did not go to the AIDS clinic in time after diagnosis or did not receive the fast start HAART after transfer were set as the fast start group. The baseline viral load (HIV-RNA) and CD4+T lymphocyte count of two groups of patients, as well as the viral load and CD4+T lymphocyte count were recorded after 6 and 12 months of antiviral treatment, and the treatment compliance, virus inhibition rate, and follow-up loss rate of the enrolled patients were recorded. Results The the CD4+T lymphocyte count in the rapid start group was higher than that in the non rapid start group at 6 and 12 months of treatment, with statistically significant difference (P<0.05). After 6 and 12 months of treatment, the HIV-RNA level of the two group was lower than before treatment, while the HIV-RNA level in the rapid start group was lower than those in the non rapid start group at 6 and 12 months of treatment, with astatistically significant difference (P<0.05). The treatment compliance of the rapid start group was 100.00% (32/32), and the virus inhibition rate was 93.75% (30/32), which were higher than 82.76% (24/29) and 68.97% (20/29) of the non rapid start group, the differences were statistically significant (P<0.05). The loss of follow-up rate was 0, which was lower than 17.24% (5/29) of the non rapid start group, the difference was statistically significant (P<0.05). Conclusion Rapid initiation (within 7 days of diagnosis) of HAART can maximize the adherence of HIV/AIDS patients with high CD4+T lymphocytes to antiviral treatment, increase the virus inhibition rate, CD4+T lymphocyte count level, and reduce the loss of follow-up rate, which is worthy of clinical promotion.

27 Influencing factors of neovascular glaucoma in patients with proliferative diabetic retinopathy after pars plana vitrectomy

Objective To investigate the related factors of neovascular glaucoma (NVG) after pars plana vitrectomy (PPV) for proliferative diabetic retinopathy (PDR). Methods The clinical data of 65 PDR patients who underwent PPV in Nanchang Bright Eye Hospital from January 2021 to January 2023 were retrospectively analyzed. According to whether NVG occurred after surgery, they were divided into the occurrence group (n=18) and the non-occurrence group (n=47). The general data of the patients were collected, including gender, age, body mass index, duration of diabetes, PDR stage, insulin injection, cardiovascular and cerebrovascular diseases, preoperative intraocular pressure, fasting blood glucose, anemia, retinal detachment, combined cataract surgery, preoperative anterior chamber angle or iris neovascularization. The general data were analyzed by univariate analysis, and then the factors with statistically significant differences were analyzed by multivariate logistic regression analysis. Results Among the 65 PDR patients treated with PPV, 18 cases of NVG occurred, with an incidence of 27.69%. Univariate analysis showed that there were no significant differences in body mass index, duration of diabetes, PDR stage, insulin injection, preoperative intraocular pressure, retinal detachment, and combined cataract surgery between the two groups (P>0.05). There were statistically significant differences between the two groups in gender, age, whether complicated with cardiovascular and cerebrovascular diseases, fasting blood glucose, anemia, and whether there were anterior chamber angle or iris neovascularization (P<0.05). Multivariate logistic regression analysis showed that male gender (β=1.567, P=0.024, OR=4.792, 95%CI: 1.224-18.766), age<60 years (β=2.380, P=0.003, OR=10.800, 95%CI: 2.225-52.413), cardiovascular and cerebrovascular diseases (β=1.707, P=0.003, OR=5.814, 95%CI: 1.791-18.870), fasting blood glucose ≥7.0 mmol/L (β=1.655, P=0.006, OR=5.231, 95%CI: 1.623-16.853), anemia (β=1.892, P=0.002, OR=6.635, 95%CI: 2.010-21.900), preoperative or iris neovascularization corner of the eye (β=1.905, P=0.004, OR= 6.720, 95%CI: 1.808-24.981) were the independent risk factors (P<0.05 and OR>1). Conclusion There are many influencing factors for NVG after PPV in PDR patients, including male gender, age<60 years old, combined with cardiovascular and cerebrovascular diseases, fasting blood glucose≥7.0 mmol/L, etc. Clinical attention should be paid to relevant conditions and prevention work should be done.

31 Clinical effect of endobronchial tube pump in the treatment of thick-wall cavity pulmonary tuberculosis

Objective To explore the clinical effect of endobronchial tube pump in the treatment of thick-wall cavity pulmonary tuberculosis. Methods A total of 60 patients with thick-wall cavity pulmonary tuberculosis admitted to the Fifth People's Hospital of Ganzhou from October 2019 to December 2020 were selected as the study objects, and they were divided into observation group and control group according to lottery method, with 30 cases in each group. The control group was treated with conventional antituberculosis chemotherapy, and the observation group was treated with endobronchial tube pump on the basis of conventional antituberculosis. The negative conversion rate of sputum tuberculosis, cavity closure rate, recurrence rate and adverse reactions were compared between the two groups. Results There was no statistically significant difference in the negative conversion rate of sputum tuberculosis between the two groups after 1 month of treatment (P>0.05). After 2 months of treatment, the negative conversion rate of sputum tuberculosis in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). After 6 months of treatment, the sputum tuberculosis in the two groups turned negative, and the difference was not statistically significant (P>0.05). After 1 month of treatment, there was no statistically significant difference in the cavity closure rate between the two groups (P>0.05). After 2 and 6 months of treatment, the cavity closure rates of the observation group were 90.00% and 93.33%, which were higher than 43.33% and 56.67% of the control group, and the differences were statistically significant (P<0.05). After 1 year of follow-up, there was no significant difference in the recurrence rate of thick-walled cavitary tuberculosis between the two groups (P>0.05). There was no statistically significant difference in the total incidence of adverse reaction between the two groups (P>0.05). Conclusion The effect of endobronchial tube pump drug in the treatment of thick-wall cavitary pulmonary tuberculosis is significant, which can promote the sputum to find tuberculosis bacteria to turn negative, improve the cavity closure rate, the recurrence rate is low, and there is no obvious adverse reaction.

35 Effect of cardiac rehabilitation mode based on six minutes walking training on exercise tolerance in vulnerable period of chronic heart failure patients

Objective To investigate the effect of cardiac rehabilitation model based on six-minute walk training on exercise tolerance in vulnerable period of patients with chronic heart failure. Methods Forty-nine patients with heart failure with preserved ejection fraction meeting the criteria in the cardiovascular department of the First Branch of the First Affiliated Hospital of Chongqing Medical University from July 2020 to April 2021 were selected as the study objects, and were divided into the experimental group (n=25) and the control group (n=24) by random number table method. The control group was given routine discharge cardiac rehabilitation guidance, and the experimental group was instructed to carry out six-minute walking test training every day on the basis of the control group for 12 weeks. The re-hospitalization rate, six-minute walking distance (6MWD), and blood biochemical indexes of the two groups were compared. Results There was no significant difference in the readmission rate between experimental group and control group (P>0.05). There was no significant difference in 6MWD between experimental group and control group (P>0.05). However, after 12 weeks of follow-up, 6MWD was longer in both groups than before discharge, and the difference was statistically significant (P<0.05). There were no significant differences in blood biochemical indexes between experimental group and control group (P>0.05). Conclusion The cardiac rehabilitation model based on six-minute walking training can partially improve the exercise tolerance of patients with chronic heart failure, but the difference is not statistically significant, and larger sample studies are needed to confirm. At the same time, it is also confirmed that appropriate exercise rehabilitation is safe and feasible in the vulnerable period of chronic heart failure.

39 Application of total thoracoscopic minimally invasive in cardiac surgery

Objective To analyze the application effect of total thoracoscopy minimally invasive cardiac surgery. Methods From April 2022 to April 2023, 60 patients of undergoing the cardiopulmonary bypass cardiac valve surgery in Huizhou Municipal Central Hospital and the People's Hospital of Gaozhou were chosen as the study subjects, according to the random number table method, they were divided into group A (30) and group B (30 cases). Group A received total thoracoscopy minimally invasive heart surgery, and group B received traditional median thoracotomy. The relevant surgical indicators, postoperative pain scores, and incidence of complications between two groups were compared. Results The surgical time, extracorporeal circulation time, and aortic occlusion time in group A were longer than those in group B, while the blood loss and transfusion volume were lower than those in group B, with statistically significant differences (P<0.05). The postoperative drainage volume in group A was lower than that in group B, and the postoperative ventilator use time and ICU stay time were shorter than those in group B, with statistically significant differences (P<0.05). The wound pain score on the first and third postoperative days in group A was lower than those in group B, with statistically significant difference (P<0.05). The total incidence of postoperative complications in group A was lower than that in group B, with a statistically significant difference (P<0.05). There were no statistically significant differences in the comparison of physical stress response indicators between the groups before operation (P>0.05). The levels of adrenaline, norepinephrine and cortisol of the two groups 72 hours after operation were higher than before operation, and the norepinephrine and cortisol in the group A were lower than those in the group B, with statistically significant differences (P<0.05). Conclusion Compared with median open heart surgery, total thoracoscopy minimally invasive cardiopulmonary bypass heart valve surgery has the advantages of low trauma, rapid postoperative recovery, and fewer postoperative complications.

43 Comparison of effect of different doses of PCSK9 inhibitors on patients with coronary atherosclerosis

Objective To compare the effect of different doses of PCSK9 inhibitors on coronary atherosclerosis. Methods Sixty-six patients with coronary atherosclerosis admitted to Nanchang Third Hospital from August 2021 to August 2022 were selected as the study objects, and they were divided into high-dose group (n=33) and low-dose group (n=33) according to random number table method. Both groups received basic statin therapy: the high-dose group was given a PCSK9 inhibitor 450 mg once every 4 weeks, and the low-dose group was given a PCSK9 inhibitor 300 mg once every 4 weeks for 24 weeks. The clinical efficacy, blood lipid index and Gensini score were compared between the two groups. Results The total effective rate of high dose group was higher than that of low dose group, and the difference was statistically significant (P<0.05). After treatment, the levels of triglyceride (TG), low-density lipoprotein cholesterol (LDL-C) and total cholesterol (TC) in two groups were lower than before treatment, and the levels of TG, LDL-C and TC in high-dose group were lower than those in low-dose group, with statistical significances (P<0.05). After treatment, the Gensini score of the two groups was lower than that before treatment, and the Gensini score of the high-dose group was lower than that of the low-dose group, with statistical significances (P<0.05). Conclusion The curative effect of high-dose PCSK9 inhibitor is significant on patients with coronary atherosclerosis, which can regulate blood lipid metabolism and improve coronary stenosis.

47 Application of flexible ureteroscope combined with flexible ureteral access sheath in patients with renal calculi

Objective To explore the application value of flexible ureteroscope combined with flexible ureteral access sheath in patients with renal calculi. Methods A total of 40 patients with renal calculus admitted to the Third Affiliated Hospital of Gannan Medical University from January to December 2022 were selected as research objects. According to random number table method, they were divided into control group and observation group, with 20 cases in each group. Both groups were treated with flexible ureteroscopic lithotripsy. The control group was treated with non-flexible ureteral access sheath, while the observation group was treated with flexible ureteral access sheath. The stone clearance rate, surgical indicators, pain degree, negative emotions, quality of life, and incidence of complications were compared between the two groups. Results The stone clearance rate of the observation group at 1 day and 1 month after operation were higher than those of the control group, and the difference were statistically significant (P<0.05). The operation time of the observation group was shorter than that of the control group, and the intraoperative blood loss was less than that of the control group, and the differences were statistically significant (P<0.05). Before operation, there were no statistically significant differences in pain degree, negative emotion and quality of life scores between the two groups (P>0.05). The pain visual analogue scale (VAS) score of the observation group was lower than that of the control group 3 days after operation, the scores of self-rating depression scale (SDS) and self-rating anxiety scale (SAS) were lower than those of the control group 1 month after operation, and the scores of social relationship, physical health, mental health and surrounding environment in the World Health Organization quality of life brief assessment (WHOQOL-BREF) 1 month after operation were higher than those in the control group, and the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Flexible ureteroscope combined with flexible ureteral access sheath has obvious advantages in shortening the operation time and reducing intraoperative blood loss compared with non-flexible ureteral access sheath, which is conducive to improving the stone clearance rate, reducing the risk of complications, reducing the pain and negative emotions of patients, and improving the quality of life.

51 Clinical study of improved arteriovenous anastomosis and cephalic vein dilation on maturation of small vascular fistula

Objective To investigate the effect of improved arteriovenous anastomosis and cephalic vein dilation on the maturation of small intravascular fistula in end-stage renal failure hemodialysis patients. Methods A total of 43 maintenance hemodialysis patients with small forearm vessels admitted to the Department of Nephrology, Lianzhou People's Hospital from January 2014 to April 2022 were selected as the research objects, and they were divided into observation group (n=21) and control group (n=22) by random number table method. The observation group was treated with improved arteriovenous anastomosis and cephalic vein dilation to establish internal fistula, while the control group was treated with traditional internal fistula surgery. The diameter of cephalic vein, radial artery flow velocity, blood flow of internal fistula, the time of first use of internal fistula, the success rate of operation, the maturity rate of internal fistula, and the patency rate of 1 month and 1 year after operation were compared between the two groups. Results The diameter of cephalic vein in the observation group was larger than that in the control group, the flow velocity of radial artery and the flow rate of internal fistula were higher than those in the control group, and the time of first use of internal fistula was shorter than that in the control group, and the differences were statistically significant (P<0.05). The success rate of fistula operation, maturity rate, patency rate of 1 month and 1 year after operation in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusion The improved surgical method of arteriovenous anastomosis and cephalic vein expansion is beneficial to improve the success rate, maturity rate and patency rate of forearm small vascular internal fistula surgery, which is worthy of clinical application.

55 Clinical effect of different sequential Azithromycin treatments on Mycoplasma pneumoniae pneumonia in children

Objective To investigate the effect of different sequential treatments of Azithromycin on Mycoplasma pneumoniae pneumonia (MPP) in children. Methods A total of 68 children with MPP admitted to Maternity & Child Care Center of Xinyu from September 2020 to September 2022 were selected as the research objects, and they were divided into control group (34 cases) and observation group (34 cases) according to random number table method. Children in both groups were treated with Azithromycin. The control group received intravenous injection of Azithromycin for 3 days, withdrawal for 4 days, and then oral Azithromycin dry suspension for 7 days. Observation group was given intravenous Azithromycin for injection for 5 days, drug withdrawal for 4 days, and then oral Azithromycin dry suspension for 5 days. Both groups were treated for 14 days. The clinical efficacy, duration of clinical symptoms, length of hospital stay, immune function, lung function and adverse reactions were compared between the two groups. Results The total clinical effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). There was no significant difference in the time of fever reduction between the two groups (P>0.05). The disappearance time of lung rales, cough and hospital stay in observation group were shorter than those in control group, and the differences were statistically significant (P<0.05). There were no significant differences in immunoglobulin E (IgE), interleukin-3 (IL-3) and eosinophils (EOS) between the two groups before treatment (P>0.05). The IgE, IL-3 and EOS of the two groups after treatment were lower than those before treatment, and the IgE, IL-3 and EOS of the observation group were lower than those of the control group, with statistical significances (P<0.05). There were no significant differences in forced expiratory volume in the first second (FEV1), forced vital capacity (FVC) and FEV1/FVC in the first second before treatment between the two groups (P>0.05). After treatment, FEV1, FVC and FEV1/FVC in the two groups were higher than those before treatment, and FEV1, FVC and FEV1/FVC in the observation group were higher than those in the control group, with statistical significances (P<0.05). No obvious adverse reactions were observed in the two groups. Conclusion Using Azithromycin for 5 consecutive days under sequential therapy can effectively improve the clinical symptoms and immune function of MPP children, shorten the hospital stay of children, and is safe, worthy of promotion.

59 Application of fluorescence quantitative PCR in prenatal diagnosis

Objective To investigate the effect of quantitative fluorescent PCR (QF-PCR) combined with chromosome karyotype analysis in prenatal diagnosis. Methods A total of 388 pregnant women in the second trimester of pregnancy who underwent amniocentesis at the Prenatal Diagnosis Center, Huizhou Second Maternal and Child Health Hospital from September 2022 to May 2023 were selected as the study objects. Chromosome karyotype analysis and QF-PCR were performed on all the enrolled women. Using chromosome karyotype analysis as the standard, the application value of QF-PCR in prenatal diagnosis was analyzed. Results In 388 cases of amniotic fluid, QF-PCR detected one case of maternal blood contamination. Among the 388 subjects, 17 cases of chromosome aneuploidy were detected by QF-PCR, including 10 cases of trisomy 21, 2 cases of trisomy 18, 1 case of trisomy 13, and 4 cases of sex chromosome abnormality. It was consistent with the results of chromosome karyotype analysis. Chromosome karyotype analysis of 13 cases of structural abnormalities, QF-PCR technology did not detect. Conclusion QF-PCR can identify maternal blood contamination and detect it quickly, which can make up for the lack of long karyotype analysis time. Chromosome karyotype analysis can supplement the deficiency of QF-PCR technology for the detection of other chromosome abnormalities, and the combination of the two methods can play a complementary role, which is conducive to reducing birth defects.

64 Etiology and cytological analysis of bronchoalveolar lavage fluid in children with pulmonary infection

Objective To analyze the pathogenic and cytological components of bronchoalveolar lavage fluid (BALF) in children with pulmonary infection and to provide evidence for clinical diagnosis and treatment. Methods A total of 42 children hospitalized in Quanzhou Children's Hospital from August 2021 to October 2022 were selected as the research objects. Among them, 23 children diagnosed with bacterial pneumonia by BALF culture were in the bacterial pneumonia group, and 19 children without bacterial growth cultured by BALF after foreign body removal were in the bronchial foreign body group. The pathological analysis of BALF in the two groups was carried out by cytological sedimentation method. Results The main bacterial pneumonia in 23 cases was Streptococcus pneumoniae infection, lung CT was easy to be complicated with atelectasis. The most common bronchial foreign bodies in 19 cases were in the left main bronchus, and nut foreign bodies were the main ones. The percentage of neutrophil and C-reactive protein (CRP) in bacterial pneumonia group were higher than those in bronchial foreign body group, and the differences were statistically significant (P>0.05). The percentage of lymphocyte in bacterial pneumonia group was lower than that in bronchial foreign body group, the difference was statistically significant (P>0.05). The percentage of neutrophils in BALF in bacterial pneumonia group was higher than that in bronchial foreign body group, and the difference was statistically significant (P<0.05). There were no significant differences in the percentage of lymphocytes, macrophages and ciliary columnar epithelial cells in BALF between the two groups (P>0.05). Conclusion Streptococcus pneumoniae is the most common bacterial pneumonia in children, and the cytological composition of BALF is mainly neutrophils, which provides a basis for clinical diagnosis and treatment, and avoids clinical antibiotic abuse.

68 Effect of ultrasound-monitoring ovulation and guiding pregnancy preparation on postoperative pregnancy outcome of patients undergoing laparoscopic tubal pregnancy conservative surgery

Objective To explore the effect of ultrasound monitoring ovulation and guiding pregnancy preparation on the pregnancy outcome of patients undergoing laparoscopic tubal pregnancy conservative surgery. Methods A total of 80 patients diagnosed with tubal pregnancy and conservative laparoscopic tubal pregnancy surgery in the gynecology department of Jieyang People's Hospital from January 2019 to January 2021 were selected as the study objects. They were divided into test group and control group by random number table method, with 40 cases each. The control group had natural pregnancy after operation, and the test group had ultrasound monitoring of ovulation to guide pregnancy after operation. The lowest function of the affected and contralateral fallopian tubes and ovaries of the two groups were compared, and the intrauterine pregnancy rate and repeated ectopic pregnancy rate were followed up for 1 year. Results There was no significant difference between the two groups in the lowest functional score of the affected side and the lowest functional score of the contralateral side (P>0.05). The intrauterine pregnancy rate in the test group was higher than that in the control group, and the rate of recurrent ectopic pregnancy was lower than that in the control group, the differences were statistically significant (P<0.05). Conclusion Ultrasound monitoring ovulation can guide the pregnancy after conservative operation of laparoscopic tubal pregnancy, which can improve the rate of intrauterine pregnancy and reduce the rate of repeated ectopic pregnancy.

72 Efficacy and safety of Guizhi Fuling Capsules combined with laparoscopic surgery in the treatment of endometriosis

Objective To investigate the efficacy and safety of Guizhi Fuling Capsules combined with laparoscopic surgery in the treatment of endometriosis. Methods A total of 90 patients with endometriosis admitted to Weihai Second Hospital Affiliated to Qingdao University from January 2020 to January 2022 were selected as research objects, they were divided into observation group and control group according to random number table method, with 45 cases in each group. The observation group was treated with Guizhi Fuling Capsules combined with laparoscopic surgery, and the control group was treated with laparoscopic surgery alone. The effects, adverse reactions, sex hormones, inflammatory factors, T lymphocytes, visual analogue scale (VAS) and endometriosis health scale (EHP-30) were compared between the two groups. Results The total effective rate of observation group was higher than that of control group, the difference was statistically significant (P<0.05). There were no significant differences in gastrointestinal symptoms, liver and kidney function injury, bleeding and wound pain complications between two groups (P>0.05). After treatment, estradiol (E2), luteinizing hormone (LH) and progesterone (P) in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). After treatment, interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), CD3+, CD4+ and CD8+ in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). VAS and EHP-30 score of observation group were lower than those of control group after treatment, and the differences were statistically significant (P<0.05). Conclusion Guizhi Fuling Capsules combined with laparoscopic surgery can improve the sex hormone, inflammatory response and T lymphocyte immunity in patients with endometriosis, and has the advantages of high efficiency and safety, its efficacy is higher than that of laparoscopic surgery, and it is worthy of widespread promotion in clinical practice.

76 Appearance and MRI findings of hepatic perivascular lucency in different liver diseases

Objective To describe the pathological mechanism and MRI findings of PVL, discuss its association with the occurrence of the same liver diseases, and evaluate the clinical significance of PVL. Methods A total of 197 patients with liver-related diseases admitted to the Third People's Hospital of Jiujiang from January 2019 to January 2020 were retrospectively analyzed, and according to the disease type, they were divided into chronic hepatitis B group (111 cases), cirrhosis group (57 cases), and liver tumor group (29 cases). The MRI manifestations of PVL in the three groups were observed, and the MRI plain scan and enhanced image data of the upper abdomen of the patients were studied, and different liver diseases and different levels of PVL were analyzed. Results The PVL detection rate was 32.4% in patients with chronic hepatitis B, 31.0% in patients with liver tumor, and 40.4% in patients with cirrhosis. The incidence of PVL in patients with cirrhosis did not change with the increase of cirrhosis grading score (P>0.05). The incidence of PVL in patients with hilar obstruction was higher than that in patients with uninvaded hilar area, and the difference was statistically significant (P<0.05). Conclusion The MRI distribution and manifestations of PVL in different liver diseases have corresponding characteristics, which has reference value for the evaluation of the severity and prognosis of chronic hepatitis B disease, as well as the diagnosis of liver tumor invading the hepatic portal region.

81 Study of contrast-enhanced ultrasound combined with Chinese-thyroid imaging, reporting and data system in the diagnosis and biopsy efficiency of thyroid nodules

Objective To investigate the value of Chinese-thyroid imaging, reporting and data system (C-TIRADS) combined with contrast-enhanced ultrasound in the diagnosis of thyroid nodules. Methods A total of 413 patients with thyroid nodules (413 nodules) who received ultrasound examination in the Department of Ultrasound Medicine, the First Affiliated Hospital of Guangxi Medical University from January to June 2022 were selected as the study objects. With pathological findings as the "gold standard", all patients were classified by C-TIRADS and combined with contrast-enhanced ultrasound. The sensitivity, specificity and accuracy of C-TIRADS classification and C-TIRADS classification combined with contrast-enhanced ultrasound were compared. The recommended thyroid biopsy was performed according to the C-TIRADS guideline and the C-TIRADS guideline combined with contrast-enhanced ultrasound, and the biopsy yield of malignant tumors, the biopsy rate of malignant tumors and the rate of unnecessary biopsies were calculated and compared respectively. Results The results of C-TIRADS showed 363 benign nodules and 50 malignant nodules. C-TIRADS combined with contrast-enhanced ultrasound were used to diagnose 267 benign nodules and 146 malignant nodules. The sensitivity, specificity and accuracy of C-TIRADS combined with contrast-enhanced ultrasound in the diagnosis of thyroid cancer were higher than those of C-TIRADS, and the differences were statistically significant (P<0.05). For all nodules, 1.0-1.5 cm nodules, the biopsy rate of malignant tumors with C-TIRADS combined with contrast-enhanced ultrasound was lower than that of C-TIRADS alone. For nodules>1.5 cm, the biopsy rate of malignant tumor and unnecessary biopsy rate of C-TIRADS combined with contrast-enhanced ultrasound were lower than that of C-TIRADS alone, the differences were statistically significant (P<0.05). Conclusion The combination of C-TIRADS and contrast ultrasound can improve the ultrasound diagnostic efficiency of thyroid nodules, and contrast ultrasound combined with C-TIRADS can also improve the reliability of recommending thyroid nodules biopsy, increase the biopsy yield of malignant nodules, and reduce the unnecessary biopsy rate of benign thyroid nodules.

85 Effect on preoperative anxiety, vital signs, and sleep quality in patients with lumbar compression fractures by thumbtack needles

Objective To explore the effect of thumbtack needles on improve preoperative anxiety, vital signs, and sleep quality in patients with lumbar compression fractures. Methods A total of 82 patients with lumbar compression fractures treated in Jiangxi Hospital of Integrated Chinese and Western Medicine from January 2021 to November 2022 were selected, and randomly divided into the control group and the observation group, with 41 cases in each group. The control group received the conventional intervention, while the observation group was given intervention with thumbtack needles. The self-rating anxiety scale (SAS) scores of the two groups on the day of admission, 1 day before operation, the day of operation and 3 days after operation were evaluated. Systolic blood pressure (SAP), diastolic blood pressure (DAP) and heart rate (HR) in the two groups before operation and on the day of operation were analyzed. Results The SAS scores of the two groups of patients were higher than those of the group before surgery on the 1 day and the day of surgery. The SAS scores of the two groups of patients on the 3 day after surgery were lower than those of the group before surgery, and the differences were statistically significant (P<0.05). The SAS scores of the observation group patients were lower than those of the control group at all times, and the difference was statistically significant (P<0.05). There was no statistically significant difference in HR, SAP, and DBP levels between the two groups of patients before surgery (P>0.05); on the day of surgery, the levels of HR, SAP, and DBP in both groups of patients were higher than before surgery, and the differences were statistically significant (P<0.05). The levels of HR, SAP, and DBP in the postoperative observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). The difference in PSQI scores between the two groups of patients before surgery was not statistically significant (P>0.05). The postoperative PSQI scores of the observation group were lower than those of the control group in all dimensions, and the differences were statistically significant (P<0.05). Conclusion For patients with lumbar compression fracture, treatment with thumbtack needles can effectively relief preoperative anxiety and ensure stable vital signs.

89 Application effect of thunder fire moxibustion combined with Chinese herbal bath in postpartum rehabilitation

Objective To explore the application effect of thunder fire moxibustion combined with Chinese herbal bath in postpartum rehabilitation. Methods A total of 62 parturients in Xingguo Hospital Affiliated to Gannan Medical College from November 2020 to October 2022 were selected as the study objects, they were divided into control group (n=31) and study group (n=31) by random number table method. The control group was treated with thunder fire moxibustion, and the study group was combined with Chinese herbal bath on the basis of the control group. The postpartum recovery effect, contraction duration, first urination time, postpartum lochia volume, lactation initiation time, postpartum bleeding volume, lactation volume and uterine relapse were compared between the two groups. Results The total effective rate of postpartum rehabilitation in the study group was higher than that in the control group, the difference was statistically significant (P<0.05). The duration of postpartum contraction in the study group was shorter than that in the control group, the first urination time and lactation start time were earlier than that in the control group, and the lochia volume 3 days after delivery was lower than that in the control group, the differences were statistically significant (P<0.05). The blood loss on day 3 after delivery was lower than that on day 1 after delivery, and the milk yield was higher than that on day 1 after delivery, the differences were statistically significant (P<0.05). The postpartum blood loss at 3 days in the study group was lower than that in the control group, and the milk yield was higher than that in the control group, the differences were statistically significant (P<0.05). The product of three diameters of uterus in the study group was lower than that in the control group at the time of discharge, and the average descending speed of fundal was higher than that in the control group, the differences were statistically significant (P<0.05). The total complication rate of the two groups was lower in the study group than in the control group, the difference was statistically significant (P<0.05). Conclusion The use of thunder fire moxibustion combined with Chinese herbal bath in postpartum rehabilitation can shorten the contraction time of parturients, promote urination and lochia, increase the start time and milk volume of lactation, help the uterus to recover early, and speed up the postpartum rehabilitation process.

Zhang Zhongjing and Huangfu Mi lived in a very close age. The former's Treatise on Febrile and Miscellaneous Diseases became the "ancestor of the prescription book", while the latter's A and B Classics of Acupuncture and Moxibustion also became the classic of acupuncture and moxibustion. The publication of the two monographs jointly promoted the formation and development of traditional Chinese medicine clinical therapy. Huangfu Mi believes that the medical path is divided into two systems: acupuncture and decoction. Decoction medicine has been perfected by Zhang Zhongjing and Wang Shuhe. However, in the circulation of acupuncture theory, the existing acupuncture classics have been incomplete, and there are many errors, which need to be further compiled and sorted out. On the basis of this medical research and analysis, Huangfu Mi wrote A and B Classics of Acupuncture and Moxibustion. Therefore, Huangfu Mi's medical academic thought system is not limited to acupuncture and moxibustion, people should not ignore the fact that Huangfu Mi is also good at decoction, especially the use of Zhongjing classical prescription. Through the analysis of Huangfu Mi's relevant writings and relevant historical materials, we can know that Huangfu Mi highly respected Zhang Zhongjing, and conducted in-depth research on Zhongjing classical prescription, set a precedent for the study of Zhongjing classical prescription, clarified the origin, function, usage and adverse reactions of Hanshi Powder, introduced the provisions of classical prescription into the A and B Classics of Acupuncture and Moxibustion and improved the theory of acupuncture and moxibustion. The medical thought of Huangfu Mi attaching equal importance to acupuncture and medicine is worth inheriting and developing.

Objective To investigate the clinical effect of modified Huangqi Jianpi Decoction in treating recurrent diarrhea in chronic colitis of dual deficient of spleen and stomach type. Methods A total of 60 cases of patients with chronic colitis admitted to the Department of Traditional Chinese Medicine (TCM) Internal Medicine of Dalian Central Hospital from December 2020 to August 2022 were selected as subjects. The patients were divided into the treatment group and the control group by random number table method, with 30 cases in each group. The treatment group was given modified Huangqi Jianpi Decoction. The control group was treated with Live Combined Bacillus Subtilis and Enterococcus Faecium Enteric-coated Capsules. The levels of T-lymphocyte subpopulations (CD3+ and CD4+) and TCM symptom scores were recorded before treatment and 2, 4, 8 weeks after treatment. The clinical efficacy and recurrence rate after one month of drug cessation in the two groups were compared. Results The levels of blood T-lymphocyte subpopulations (CD3+ and CD4+) in the treatment group were higher than those in the control group at 2, 4 and 8 weeks after treatment, and the differences were statistically significant (P<0.05). The total clinical symptom scores of patients in the treatment group were lower than those of the control group at 2, 4 and 8 weeks after treatment, and the differences were statistically significant (P<0.05). The total effective rate of treatment in the treatment group was higher than that of the control group, and the difference was statistically significant (P<0.05). The recurrence rate of patients in the treatment group was lower than that of the control group at one month of drug cessation, and the difference was statistically significant (P<0.05). Conclusion Modified Huangqi Jianpi Decoction in the treatment of recurrent diarrhea of dual deficiency of spleen and stomach type of chronic colitis obtains a significant effect. It can improve the clinical symptoms of the patients, and enhance their immune function.

103 Clinical significance of N-terminal pro-B-type natriuretic peptide and hypersensitive C-reactive protein in predicting immunoglobulin A nephropathy with cardiac insufficiency

Objective To explore the clinical significance of N-terminal pro-B-type natriuretic peptide (NT-proBNP) and hypersensitive C-reactive protein (hs-CRP) in the predicting of immunoglobulin A nephropathy (IgAN) with cardiac insufficiency. Methods A total of 60 patients with IgAN admitted to Jiujiang Traditional Chinese Medicine Hospital from January to December 2022 were selected as the study subjects, and 60 normal examinees during the same period were selected as the control group. By measuring creatinine levels in patients, glomerular filtration rate (GFR) in IgAN patients was calcuiated, the staging of IgAN related chronic kidney disease (CKD) in patients was evaluated, and cardiac ultrasound examinations on all patients was conducted, the level of cardiac dysfunction was determined based on left ventricular ejection fraction (LVEF) and clinical manifestations. According to the level of cardiac dysfunction, patients were divided into group A (n=20), group B (n=17), group C (n=13), and group D (n=10), the differences in serum NT-proBNP and hs-CRP levels among five groups were detected and compared, and Spearman correlation analysis was used to examine the relationship between NT-proBNP, hs-CRP, CKD stage, LVEF, and level of cardiac dysfunction. Results The GFR levels among four groups was compared, the difference was statistically significant (P<0.05) between group D<group C<group B<group A; the CKD staging of the four groups was compared, and the staging of groups C and D was higher than that of groups A and B, the differences were statistically significant (P<0.05); the LVEF levels of 5 groups were compared, the difference were statistically significant (P<0.05) between group D<group C<group B<group A<control group; The levels of NT-proBNP and hs CRP in 5 groups were compared, with group D>group C>group B>group A>control group, and the differences were statistically significant (P<0.05). Spearman correlation analysis showed that the serum NT-proBNP and hs-CRP levels in IgAN patients were significantly positively correlated with CKD stage and heart failure grade, and significantly negatively correlated with LVEF (P<0.05). Conclusion NT-proBNP and hs-CRP can both serve as predictive indicators for patients with IgAN complicated with heart failure, and NT-proBNP and hs-CRP can reflect CKD staging and level of heart failure. Early detection and analysis have important clinical significance for preliminary diagnosis and treatment policy formulation.

Methods From January to December 2021, 200 children who were treated in the Department of Dermatology, Child Health Care, Traditional Chinese Medicine and Pediatrics of Xinyu Maternal and Child Health Hospital were selected as the study objects, and 200 healthy people were selected during the same period as the healthy control group. Serum IgG antibodies of 14 kinds of food were detected in the subjects, and the detection results were analyzed. Results Food-specific IgG was detected in 172 cases, the total positive rate was 86.00% (172/200). The positive rate of specific IgG in milk, egg and wheat was the highest. In addition, all other foods except cod caused the increase of IgG concentration, mainly mild sensitive cases, moderate sensitive cases and high sensitive cases. The proportion of male and female children was 50.50% and 49.50%, respectively. The top 3 IgG positive percentages were milk, egg and wheat, and the IgG positive percentage of male children shrimp was higher than that of female children, the difference was statistically significant (P<0.05). There was no significant difference between genders in beef, chicken and other foods (P>0.05). Among 172 specific IgG positive patients, digestive system and skin system diseases accounted for the most positive patients, 90.63% and 92.39%, respectively. The serum specific IgG antibody concentrations of 14 foods in the children group were higher than those in the healthy control group, the differences were statistically significant (P<0.05). Conclusion Food-specific IgG antibody detection has high value in common chronic diseases in children, and provides new ideas for the treatment of some chronic diseases.

111 Comparison of Six Sigma theory applied to two biochemical detection systems

Objective To explore the application of Six Sigma (6σ) theory for comparison of two biochemical detection systems. Methods The indoor quality control test results of 32 items on the biochemical analyzer in the Department of Laboratory, Panjin Liaoyou Gem Flower Hospital from July 1 to December 31, 2022 were collected. The Sigma (σ) value and bias of each item were calculated from the internal quality control data of two instruments, and the bias of each item was calculated by the traditional comparison method. The Pearson correlation analysis between the σ value and bias calculated from the internal quality control data and the bias of the traditional method was carried out by using SPSS 26.0 statistical software. Results Among the 32 items, 15 items had a σ value ≥6 and did not need to be improved. 17 items were improved by calculating the results of quality goal index (QGI), among which, 15 items needed to improve the precision, and 2 items needed to improve the precision and accuracy. The calculated coefficient of variation (CV) of 17 items needed to be improved was compared with the allowable CV in the laboratory. The results showed that both were less than the allowable CV. According to the comparison of the two instruments, for the traditional method, the bias of the two instruments was also within the standard range. There was no significant correlation between the σ values of the two quality control levels and the bias of the traditional method (P=0.754, 0.779), but there was a positive correlation between the bias of the two quality control levels and the bias of the traditional method (P=0.041, 0.029). Conclusion Application of quality control data to calculate σ value and QGI for comparison between different detection systems is a relatively convenient and operable method, which can monitor the comparability between different detection systems in the laboratory at any time, and complement the traditional comparison method.

116 Application of information closed-loop management in the management of hospital infection

Objective To explore the application value of information closed-loop management in the management of hospital infection. Methods All the departments of the People's Hospital of Longhua, Shenzhen from December 2021 to November 2022 were retrospectively selected as the research objects, and were grouped according to the differences in different management modes of nosocomial infection. From December 2021 to May 2022, the People's Hospital of Longhua, Shenzhen implemented the traditional artificial hospital infection management mode, and from June 2022 to November 2022, the People's Hospital of Longhua, Shenzhen implemented the hospital infection information closed-loop management mode. A total of 100 cases of continuous improvement of hospital infection management quality that were supervised when the traditional artificial hospital infection management mode was implemented were included in the control group, and another 100 cases of continuous improvement of hospital infection management quality that were supervised when the hospital infection information closed-loop management mode was implemented were included in the observation group. The same 30 full-time infection management staff and part-time clinical infection control staff were investigated in the two periods. The effective implementation rate of nosocomial infection prevention and control measures, the efficiency score of hospital infection data recovery, the timely effectiveness score of hospital infection work, the compliance score of hand hygiene, and the satisfaction of medical staff in different periods were compared. Results The effective implementation rate of hospital infection prevention and control measures in the observation group was higher than that in the control group (99.00% vs. 90.00%), and the difference was statistically significant (P<0.05). The data recovery efficiency score and the timely effectiveness score of hospital infection work in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The self-evaluation scores of hand hygiene compliance and hand hygiene management standardization in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). The satisfaction of medical staff in observation group was higher than that in control group (100.00% vs. 73.33%), and the difference was statistically significant (P<0.05). Conclusion The closed-loop management mode of hospital infection information can improve the quality and efficiency of hospital infection management, and the satisfaction of medical staff is high.

126 Application effect of WeChat-assisted case teaching method in clinical practice teaching of cerebrovascular disease

Objective To explore the effectiveness of WeChat-assisted case-based learning (CBL) teaching method in the clinical practice teaching of cerebrovascular disease. Methods A total of 80 interns practiced in the Department of Neurology of the Second Affiliated Hospital of Soochow University between September and December 2022 were selected as study objects, and they were divided into the observation group and the control group by random number table method, with 40 students in each group. The observation group adopted the WeChat-assisted CBL teaching method, while the control group adopted the traditional teaching method. The theoretical knowledge scores, practical ability scores and comprehensive quality scores of the two groups of students were assessed. Results The theoretical knowledge scores, practical ability scores and comprehensive quality scores of interns in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). In addition, teaching satisfaction in the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion The WeChat-assisted CBL teaching method has a good application effectiveness in the clinical practice teaching of cerebrovascular diseases in the department of neurology, and this method is worth popularizing and applying in the clinical practice teaching of neurology department.

This article summarized the application effect of wound endoscopy in a case of patient with ischemic necrosis of stoma mucosa combined with separation of stoma skin mucosa. Key points include accurate evaluation of the depth of ischemia and necrosis of the stoma mucosa with wound endoscopy at an early stage, treatment of stoma mucosa ischemia with stoma skin care powder, elevation of the stoma skin mucosa with a raised chassis and belt, etc. At the same time, attention should be paid to patient's nutrition, psychology, family and social support, and strengthening health education and basic nursing. 140 Two cases of Chlamydia psittaci pneumonia and literature review

Two patients had a history of pigeon contact and presented with fever, fatigue and dyspnea. One patient developed acute respiratory distress syndrome (ARDS) on the fourth day of the disease and required non-invasive ventilator assisted ventilation. Leukocyte count was slightly increased in 2 patients, lymphocyte percentage was decreased, platelet count was normal, procalcitonin was increased, C-reactive protein was significantly increased, alanine aminotransferase and aspartate aminotransferase were increased to varying degrees, serum sodium, potassium and albumin were decreased, erythrocyte sedimentation rate (ESR) was normal in 1 patient and slightly increased in 1 patient. The pathogen could not be identified by routine culture and serological examination of blood and alveolar lavage fluid. Chest X-ray, computed tomography (CT) showed large exudation and consolidation of one lung lobe, rapid progression in severe cases, involving both lung lobes, respiratory failure, and poor effect of empirical antibiotic use. After timely application of metagenomic next-generation sequencing (mNGS) technology combined with avian contact history, the patient was finally diagnosed with Chlamydia psittaci pneumonia. After treatment with quinolones combined with tetracycline antibiotics, the symptoms, signs and imaging abnormalities of the patient were significantly improved, but 1 patient had poor imaging absorption and was further treated with macrolide antibiotics for 1 week. Follow-up examination of chest CT showed that the lesion was basically absorbed. By analyzing the clinical characteristics, diagnosis and treatment process of these two patients, it can provide reference for clinical diagnosis and treatment of these diseases.

146 A case of biotinidase deficiency with neuromyelitis optica-like manifestations

Biotinidase deficiency (BTD) is an autosomal recessive disease. It is caused by the biotinidase gene mutation encoding biotinidase, which causes the complete or partial deficiency of biotinidase activity, resulting in the decrease of biotin, the decrease of the activity of various biotin-dependent carboxylases, the disorder of mitochondrial energy synthesis, metabolic acidosis, organic aciduria and a series of neurological and skin system damage. The clinical manifestations of this disease are complex, individual differences are very large, which can easily lead to missed diagnosis and misdiagnosis. One 11-year-old male patient was admitted to Gansu Children's Hospital, Lanzhou University Second Hospital. The main symptoms were weakness of both upper limbs and spastic paralysis of both lower limbs, and decreased vision. The patient was initially diagnosed as a disease of optic neuromyelitis pedigree, but the effect of immunotherapy was not good. After blood and urine genetic metabolic disease screening and gene testing, the patient was finally diagnosed as biotinidase deficiency, and the symptoms of the patient were relieved after supplementation of biotinidase. Biotinidase deficiency often leads to serious nervous system damage, and biotin supplementation therapy can play a good role. 150 A case of erythrodermic psoriasis with latent tuberculosis infection and hepatitis B cirrhosis treated with Secukinumab and literature review

Due to the selective immunosuppressive effect of biological agents, long-term use in psoriasis patients may lead to reactivation of hepatitis B virus and mycobacterium tuberculosis. Clinical applications have found that Secukinumab is safer than other biological agents. This article retrospectively collected a case of erythrodermic psoriasis patient with latent tuberculosis infection and hepatitis B cirrhosis treated by Secukinumab, who suffered from bilateral femoral head necrosis due to irregular systematic use of hormones. After informed consent, the patient was given anti-hepatitis B virus and preventive antituberculosis treatment. After that, the patient was given the standard protocol of Secukinumab and then bilateral total hip arthroplasty. At the 24th week of treatment, the patient's psoriasis area and severity index (PASI) reached 90; and at the 40th week, the patient reached PASI100, at the 60th week of treatment, the patient still reached PSAI90, with only the scaly erythema scattered in front of both tibia. Up to now, no hepatitis B virus replication and active tuberculosis have been found during follow-up. Through the introduction of this case and review of relevant literature, this article shows the effectiveness and safety of Secukinumab, and promote dermatologists' awareness of biological agents in improvement of metabolic comorbidity, hoping to make more patients with psoriasis accompanied by hepatitis B and tuberculosis obtain better clinical prognosis.

155 Application of early diversified activity nursing combined with EGDS strategy in patients with multiple injuries caused by mechanical ventilation

Objective To explore the effect of early multiple activity nursing combined with early goal-directed sedation (EGDS) strategy in patients with multiple injuries caused by mechanical ventilation. Methods A total of 80 patients with multiple injuries undergoing mechanical ventilation admitted to the Second Affiliated Hospital of Nanchang University from January 2021 to January 2022 were selected as the study objects, and they were divided into control group (40 cases) and observation group (40 cases) according to random number table method. The control group used conventional ICU nursing combined with standard sedation strategy, while the observation group used early diversified activity nursing combined with EGDS strategy. The sedation level, clinical indicators, ICU acquired weakness, and delirium incidence rates of the two groups were compared. Results There was no statistically significant difference in the incidence of agitation and deep sedation between the two groups (P>0.05). The compliance rate of shallow sedation in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). The mechanical ventilation time, total hospitalization time, and ICU hospitalization time of the observation group were shorter than those of the control group, and the muscle strength score was higher than that of the control group, the differences were statistically significant (P<0.05). The incidences of acquired asthenia and delirium in ICU in observation group were lower than those in control group, with statistically significant differences (P<0.05). Conclusion Early multiple activity nursing combined with EGDS strategy can achieve the goal of shallow sedation, reduce the risk of acquired weakness and delirium in ICU, improve the patients' muscle strength and promote the recovery of patients with multiple injuries caused by mechanical ventilation.

159 Application effect of "Internet + home care" in volunteer nursing service

Objective To explore the application effect of "Internet+home nursing" in voluntary nursing services. Methods A total of 82 people in need of service by the "365 platform for party and mass service" from December 2020 to December 2021 were selected as the research subjects. They were divided into control group (41 cases) and observation group (41 cases) by random number table method. The control group was provided with routine medical services, and the observation group was provided with "Internet + home care" services. Both groups received continuous care for 12 months. The number of hospital visits, medical expenses, daily living ability, mental state, caregiver care skill level and life satisfaction of the two groups were compared. Results The number of hospital visits and medical expenses in observation group were lower than those in control group, and the differences were statistically significant (P<0.05). The score of activities of daily living (ADL) in the observation group was lower than that in the control group, and the score of mini-mental state examination (MMSE) and nursing skill level in the observation group were higher than those in the control group, with statistical significances (P<0.05). The scores of post-nursing life satisfaction rating (LSR) in the observation group were higher than those in the control group, with statistical significances (P<0.05). Conclusion Through internet platforms, nursing volunteers can provide home care to the service population, which can improve the mental state of the service population, promote their physical and mental health, improve their ability to take care of themselves, improve the nursing skills of caregivers, reduce the number of visits and medical expenses to a certain extent, and thus improve the satisfaction of service personnel.

163 Effect of group sandbox game intervention on occupational stress and mental health of operating room nursing staff

Objective To explore the application effect of group sandbox game intervention in operating room nursing staff. Methods A total of 90 nurses in the operating room of Yichun People's Hospital from August 2021 to August 2022 were selected as the study objects. They were divided into control group (45 cases) and observation group (45 cases) by random number table method. The control group received routine psychological intervention, while the observation group received group sandbox game intervention on top of the control group. Both groups continued to receive intervention for 3 months. Occupational stress, mental health and job quality were compared between the two groups before intervention and 3 months after intervention. Results The occupational stress scores of the observation group were lower than those of the control group after intervention, and the differences were statistically significant (P<0.05). The symptom checklist 90 (SCL-90) scores of the observation group after intervention were lower than those of the control group, and the differences were statistically significant (P<0.05). The observation group's work quality scores were higher than those of the control group, and the differences were statistically significant (P<0.05). Conclusion Group sandbox same intervention can reduce occupational stress among operating room nursing staff, promote their mental health, and respond to work with a more positive attitude, which helps to improve work quality.

167 Effect of pain care combined psychological nursing on lower limb motor exercise after congenital dislocation of hip

Objective To investigate the effect of pain nursing on congenital dislocation of hip (CDH). Methods A total of 40 children treated with CDH surgery in Ganzhou Maternal and Child Health Hospital from January 2022 to January 2023 were selected as the study objects, and were divided into control group (20 cases) and observation group (20 cases) according to random number table method. The control group was given routine nursing intervention, and the observation group was given pain nursing combined with psychological nursing intervention on the basis of the control group. The pain, lower limb motor function and sleep quality of the children in the two groups were compared. Results On the second day after surgery, there were no statistical significant differences in the children's hospital of Eastern Ontario pain scale (CHEOPS) score and Pittsburgh sleep quality index (PSQI) (P>0.05). The CHEOPS scores and PSQI of the two groups on the 8th and 15th day after surgery were lower than those on the 2nd day after surgery, and the CHEOPS scores and PSQI of the two groups on the 15th day after surgery were lower than those on the 8th day after surgery, with statistical significance (P<0.05). The CHEOPS scores and PSQI of the observation group on the 8th and 15th day after surgery were lower than those of the control group, and the differences were statistically significant (P<0.05). On the second day after surgery, there was no significant difference in the scores of the Fugl-Meyer motor function assessment (FMA) between the two groups (P>0.05). The FMA scores on the 8th and 15th day after surgery were higher than those on the 2nd day after surgery, and the FMA scores on the 15th day after surgery were higher than those on the 8th day after surgery, with statistical significance (P<0.05). The FMA scores of the observation group were higher than those of the control group at the 8th and 15th day after surgery, and the differences were statistically significant (P<0.05). Conclusion Pain nursing combined with psychological nursing can effectively improve the lower limb motor function of children with CDH surgery, reduce pain and improve sleep quality.

171 Application effect of group management combined with punch card training in rapid rehabilitation of stroke patients with hemiplegia

Objective To explore the application value of group management combined with card training in stroke patients with hemiplegia. Methods A total of 68 stroke patients with hemiplegia admitted to Jiujiang Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were selected. They were divided into the control group and the observation group by random number table method, with 34 cases in each group. The control group received routine rehabilitation care, while the observation group received group management combined with card training on the basis of the control group. Both groups were treated for 3 months. The rehabilitation training compliance, limb motor function, daily living ability and quality of life of the two groups were compared. Results There were no significant differences in evaluation activities questionnaire (EAQ) and modified Barthel index (BI) scores between the two groups before nursing (P>0.05). After nursing, the EAQ and BI scores of the two groups were higher than those before nursing, and the EAQ and BI scores of observation group were higher than those of control group, the differences were statistically significant (P<0.05). There was no significant difference in the scores of the Fugl-Meyer motor function assessment (FMA) before nursing between the two groups (P>0.05). After nursing, the FMA score of the two groups was higher than that before nursing, and the FMA score of the observation group was higher than that of the control group, with statistical significance (P<0.05). There was no significant difference in the scores of stroke specific quality of life (SS-QOL) before nursing between the two groups (P>0.05). After nursing, the SS-QOL score of the two groups was higher than that before nursing, and the SS-QOL score of observation group was higher than that of control group, the differences were statistically significant (P<0.05). Methods A total of 124 ICU patients admitted to the People's Hospital of Cenxi City from January 2022 to April 2023 were selected as the research objects, and they were divided into experimental group (63 cases) and control group (61 cases) according to coin toss method. The experimental group adopted improved positive pressure extubation technology, and the control group adopted traditional negative pressure extubation technology. The vital signs (including heart rate, blood pressure, respiration, blood oxygen saturation) before extubation, 1 min and 15 min after extubation, and the incidence of complications after extubation were compared between the two groups. Results The systolic blood pressure of the two groups at 1 min after extubation were higher than those before extubation, the mean arterial pressure of the experimental group at 15 min after extubation was lower than that before extubation and 1 min after extubation, the breathing of the experimental group at 15 min after extubation was slower than that at 1 min after extubation, while the breathing of the control group at 1 min after extubation was faster than that before extubation, the blood oxygen saturation of the two groups at 1 min after extubation were lower than those before extubation, and the differences were statistically significant (P<0.05). The incidence of hypertension, tachycardia, oxygen saturation decline, shortness of breath, severe cough, sore throat, and upper airway spasm in the experimental group 15 min after extubation were lower than those in the control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the incidence of extubation failure between the two groups (P>0.05). Conclusion Both of the two techniques can cause the increase of systolic blood pressure and the decrease of blood oxygen saturation at 1 min after the removal of oral tracheal intubation in ICU. However, the incidence of complications caused by the modified positive pressure extubation technique is lower, which is worthy of clinical application and promotion.

180 Research progress on factors affecting anxiety, depression negative emotions and health management in women of childbearing age with polycystic ovary syndrome

Polycystic ovary syndrome (PCOS) is a more common reproductive endocrine disease in clinical practice, and the cause of the pathogenesis has an important relationship with the hormone secretion disorder in the patients' body, resulting in female PCOS patients of childbearing age are more likely to show irritability, nervousness, emotional instability, anxiety, depression and other negative emotions than the normal population, and anxiety and depression are the most important psychological distress of PCOS patients of childbearing age, which have serious adverse effects on their fertility, quality of life and physical health. In recent years, a series of studies at home and abroad have shown that the factors affecting the negative emotions of anxiety and depression in women of childbearing age are mainly sociodemographic factors, sleep quality, complications and sexual dysfunction. The domestic and foreign PCOS clinical diagnosis and treatment guidelines point out that effective and standardized health management can significantly improve the treatment compliance of PCOS patients, and then improve their clinical prognosis. The manager-led multidisciplinary collaborative whole-process health management, family-participatory case health management and "Internet+" based mobile health management are the main management strategies for anxiety and depression in women of childbearing age with PCOS, which can effectively regulate negative emotions such as anxiety and depression. Based on this, this paper will review the status quo, influencing factors and health management strategies of anxiety and depression in female PCOS patients of childbearing age, hoping to arouse the attention of society to the anxiety and depression of PCOS patients of childbearing age and provide reference for helping them establish a healthy lifestyle with long-term compliance.

Multiple myeloma (MM) is a common blood system tumor characterized by the accumulation of clonal plasma cells in the bone marrow. Most MM will eventually evolve into recurrent/refractory multiple myeloma (RRMM), but for multidrug resistant RRMM patients, the efficacy of conventional cytotoxic drugs and targeted drugs are not ideal. In recent years, with the improvement of medical level and the deepening of clinical research, the clinical trial data of the new generation of original targeted drugs and new anti-MM drugs are continuously accumulated and updated, and they have shown good safety and encouraging activity, which can better relieve the symptoms of patients and prolong the survival time of patients. This review aims to collect, analyze and summarize the clinical data of iterative anti-MM targeted drugs and new anti-MM drugs, in order to understand the current development trend and provide reference for the formulation of treatment plans for RRMM patients.

In this review, intracellular calcium homeostasis and regulation, calcium overload in acinar cells, the role of Ca2+ overload in the pathogenesis of AP, and the clinical diagnosis and treatment of Ca2+ overload in acinar cells are reviewed.
