股骨近端防旋髓内钉内固定对股骨粗隆间骨折老年患者术后髋关节功能及生活质量的影响

摘要目的探讨股骨近端防旋髓内钉内固定对股骨粗隆间骨折老年患者术后髋关节功能及生活质量的影响。方法选取2015年12月～2019年12月武汉科技大学附属孝感医院诊治的80例股骨粗隆间骨折老年患者为研究对象，按照随机数字表法分为对照组（40例）和实验组（40例）。对照组采取动力髋螺钉内固定治疗，实验组采取股骨近端防旋髓内钉内固定治疗。比较两组患者的手术指标、髋关节功能、生活质量。结果实验组手术时间、手术切口、骨折愈合时间均短于对照组，术中出血量少于对照组，差异有统计学意义（P<0.05）。实验组的髋关节功能优良率（92.50%）高于对照组（75.00%），差异有统计学意义（P<0.05）。实验组治疗后的生活质量评分（90.47±8.54）分高于对照组（75.45±7.22）分，差异有统计学意义（P<0.05）。结论对股骨粗隆间骨折老年患者应用股骨近端防旋髓内钉内固定治疗，效果显著。

Abstract：Objective To investigate the effect of proximal femoral nail antirotation on hip function and quality of life in elderly patients with intertrochanteric fracture.Methods A total of 80 elderly patients with intertrochanteric fracture treated in Xiaogan Hospital Affiliated to Wuhan University of Science and Technology from December 2015 to December 2019 were selected as the research objects.According to the random number table method,the patients were divided into the control group (40 cases) and the experimental group (40 cases).The control group was treated with dynamic hip screw internal fixation,while the experimental group was treated with proximal femoral antirotation intramedullary nail internal fixation.Surgical indicators,hip function and quality of life were compared between the two groups.Results The operation time,surgical incision and fracture healing time in the experimental group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences were statistically significant (P<0.05).The excellent and good rate of hip joint function in the experimental group (92.50%) was higher than that in the control group (75.00%),and the difference was statistically significant (P<0.05).The score of quality of life in the experimental group after treatment ([90.47±8.54]points) was higher than that in the control group([75.45±7.22]points),and the difference was statistically significant (P<0.05).Conclusion For the elderly patients with intertrochanteric fracture,the effect of proximal femoral nail antirotation is significant.
