Significance of β-type brain natriuretic peptide,D-dimer and procalcitonin in the assessment of condition and near-and long-term treatment prognosis of elderly patients with chronic obstructive pulmonary disease

Abstract:Objective To investigate the significance of β-type brain natriuretic peptide,D-dimer and procalcitonin in the assessment of condition and near-and long-term treatment prognosis prognosis of elderly patients with chronic obstructive pulmonary disease(COPD).Methods A total of 100 patients with COPD admitted to our hospital from January 2016 to June 2017 were enrolled as the observation group.100 healthy subjects were selected as the control group according to the 1:1 pairing principle.the Pulmonary function index (first forced forced expiratory volume[FEV1],forced vital capacity[FVC],one second rate[FEV1/FVC])and the biochemical indicators(beta-type brain natriuretic peptide[BNP],D-dimer[DD],procalcitonin[PCT])were performed.The lung function and biochemical indicators in observation group and control group were compared,the patients in the observation group were divided into 1,2,3,and 4 groups according to the estimated FEV1%of lung function,then the biochemical indicators between different subgroups were compared.The first hospitalization time (recent prognosis index)and the number of rehospitalization(long-term prognostic indicators)were recorded in the statistical observation group,and the correlation between biochemical indicators and prognosis was analyzed.Results The pulmonary function indexes FEV1,FVC and FEV1/FVC in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The PCT,D-D,BNP of the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).There were significant differences in PCT,D-D and BNP levels among patients with grades 1,2,3 and 4 (P<0.01),the PCT,D-D and BNP levels in grades 3 and 4 were higher than grades 1 and 2 (P<0.05).The first hospitalization time in CysC,D-D,BNP positive group was significantly longer than that in the negative group (P<0.05).The number of re-hospitalization during the follow-up period in CysC,D-D,BNP positive group was significantly more than that in the negative group(P<0.05).The PCT,DD,BNP were positively correlated with the time of first hospitalization (r=0.393,0.457,0.504,P<0.05),and the PCT,D-D,BNP were positively correlated with the number of rehospitalization(r=0.308,0.286,0.488,P<0.05).Conclusion BNP,D-D and PCT can be used as sensitive indicators for the evaluation of disease status and prognosis of elderly patients with COPD,whic have high clinical value.
