单硝酸异山梨酯治疗N端脑钠肽异常的慢性阻塞性肺疾病的临床效果

摘要目的分析单硝酸异山梨酯治疗N端脑钠肽（BNP）异常的慢性阻塞性肺疾病（COPD）的临床效果。方法选取2018年7～12月我科收治的60例BNP异常的COPD患者作为研究对象，按照随机数字表法分为对照组（30例）与观察组（30例），对照组行抗感染、化痰等常规治疗，观察组在对照组治疗基础上加单硝酸异山梨酯治疗，比较两组的治疗效果。结果治疗后两组的BNP水平明显降低，且观察组明显低于对照组（P＜0.05）；观察组治疗后血氧饱和度、血氧分压明显高于对照组，血二氧化碳分压明显低于对照组（P＜0.05）；观察组的治疗总有效率为93.33%，明显高于对照组的73.33%（P＜0.05）；观察组的住院时间明显短于对照组（P＜0.05）；对照组治疗期间均未见明显不良反应，观察组不良反应总发生率为6.67%，差异无统计学意义（P＞0.05）。结论 BNP异常的COPD患者实施单硝酸异山梨酯治疗，可明显提高治疗效果，降低BNP水平，且可改善患者血气分析指标，缩短患者病程。

Abstract：Objective To analyze the clinical effect of Isosorbide mononitrate in the treatment of patients with chronic obstructive pulmonary disease(COPD)with abnormal N-terminal brain natriuretic peptide(BNP).Methods From July to December in 2018,60 patients with COPD with abnormal BNP treated in our department were selected as the subjects,and divided into control group(n=30)and observation group(n=30)by random number table method.The control group was treated with routine treatment such as anti-infection and phlegm resolving,and the observation group was treated with Isosorbide mononitrate on the basis of control group.The treatment effects were compared.Results After treatment,the level of BNP in the two groups was decreaced,and the observation group was significantly lower than that in the control group (P＜0.05).The blood oxygen saturation and partial pressure of oxygen in the observation group were significantly higher than those in the control group,and the partial pressure of blood carbon dioxide was significantly lower than that in the control group (P＜0.05).The total effective rate of treatment in the observation group was 93.33%,which was higher than that in the control group(73.33%)(P＜0.05).The hospitalization time of the observation group was significantly shorter than that of the control group (P＜0.05).There were no obvious adverse reactions in the control group.The incidence of adverse reactions in the observation group was 6.67%,and there was no significant difference between the two groups(P＞0.05).Conclusion Isosorbide mononitrate therapy for patients with COPD with abnormal BNP can significantly improve the therapeutic effect,decrease the level of BNP,improve the blood gas analysis index and shorten the course of disease.
