Abstract:Objective To investigate the effect of arthroscopic patellofemoral denervation combined with high tibial osteotomy in the treatment of knee osteoarthritis. Methods A total of 100 patients with varus medial tibiofemoral arthritis with patellofemoral arthritis treated in the Nanchang First Hospital from January 2019 to December 2020 were selected as the research objects. According to the wishes of the patients, they were divided into arthroscopy group and total knee arthroplasty group, with 50 cases in each group. Arthroscopic radiofrequency denervation of patellofemoral joint combined with high tibial osteotomy was performed in the arthroscopic group, and total knee arthroplasty was performed in the artificial total knee arthroplasty group. The operation related conditions, hospital for special surgery(HSS)knee score,Lysholm knee score and visual analogue scale(VAS)were compared between the two groups at discharge,and the changes in hemoglobin levels before and after operation were counted. Results The intraoperative blood loss in the arthroscopic group was less than that in the total knee arthroplasty group, the difference was statistically significant(P<0.05). The perioperative blood transfusion in the arthroscopic group was less than that in the total knee arthroplasty group, the difference was statistically significant (P<0.05). The operation time and postoperative residence time in the arthroscopic group were shorter than those in the total knee arthroplasty group, the differences were statistically significant (P<0.05). The HSS knee score and Lysholm knee score in arthroscopy group were higher than those in total knee arthroplasty group, the differences were statistically significant (P<0.05). The VAS score in arthroscopy group was lower than that in total knee arthroplasty group, the difference was statistically significant (P<0.05). The hemoglobin level in arthroscopy group was higher than that in total knee arthroplasty group, the difference was statistically significant (P <0.05). Conclusion For patients with knee osteoarthritis who need knee arthroplasty, radiofrequency denervation of patellofemoral joint combined with high tibial osteotomy under arthroscopy has the advantages of less trauma, rapid postoperative recovery, effective improvement of joint function and less postoperative chronic pain.
