关节镜下有限清理术配合中药熏洗治疗早中期膝骨性关节炎的应用效果

摘要目的探讨关节镜下有限清理术配合中药熏洗治疗早中期膝骨性关节炎的应用效果。方法选取莆田学院附属医院2017年1月至2019年1月收治的90例膝关节早中期骨性关节炎患者为研究对象，根据随机数字表法分为对照组和观察组，每组各45例。对照组采用关节镜有限清理术进行治疗，观察组采用关节镜有限清理术配合中药熏洗进行治疗。比较两组患者术前和术后2、4、8、12周的疼痛视觉模拟评分（VAS）、膝关节活动度（ROM）、Lysholm膝关节功能评分以及术后12周的中医临床疗效。结果观察组术后12周的中医临床总有效率为95.56%，高于对照组的82.22%，差异有统计学意义（P＜0.05）；两组患者术前、术后2周时的VAS疼痛评分、膝关节ROM、Lysholm膝关节功能评分比较，差异无统计学意义（P＞0.05）；观察组术后4、8、12周时的VAS疼痛评分低于对照组，差异有显著统计学意义（P＜0.01），观察组术后4、8、12周时的膝关节ROM、Lysholm膝关节功能评分高于对照组，差异有显著统计学意义（P＜0.01）；同组患者术后12周时的VAS疼痛评分、膝关节ROM、Lysholm膝关节功能评分与术前、术后2周时比较，差异有显著统计学意义（P＜0.01）。结论关节镜下有限清理术配合补肾活血汤中药熏洗能有效缓解膝关节疼痛，改善膝关节功能，对早中期膝骨性关节炎具有良好的疗效。

Abstract：Objective To investigate the application effect of arthroscopic limited debridement combined with Chinese medicine fumigation in the treatment of early and middle knee osteoarthritis.Methods A total of 90 patients with early and middle knee osteoarthritis admitted to Affiliated Hospital of Putian University from January 2017 to January 2019 were selected as the research objects.According to the random number table methods,they were randomly divided into the control group and the observation group,45 patients in each group.The control group was treated with arthroscopic limited debridement,and the observation group was treated with arthroscopic limited debridement combined with traditional Chinese medicine fumigation.The score of pain visual analogue scale (VAS),knee range of motion(ROM),Lysholm knee function score before and 2,4,8 and 12 weeks after surgery and the clinical efficacy of traditional Chinese Medicine at 12 weeks after surgery were compared between the two groups.Results The total effective rate of clinical efficacy of traditional Chinese medicine in the observation group at 12 weeks after surgery was 95.56%,which was significantly higher than that in the control group (82.22%),with statistical significance (P＜0.05).Before and 2 weeks after surgery,there were no significant differences in the VAS pain score,knee ROM and Lysholm knee function score between the two groups(P＞0.05).The VAS pain score of the observation group at 4,8 and 12 weeks after surgery were lower than those of the control group,and the differences were statistically significant(P＜0.01).The Knee ROM and Lysholm knee function score of the observation group were higher than those of the control group at 4,8 and 12 weeks after surgery,and the differences were sta-tistically significant(P＜0.01).The VAS pain score,knee ROM and Lysholm knee function score of patients in the same group at 12 weeks after surgery were compared with those before surgery and 2 weeks after surgery,and the differences were statistically significant (P＜0.01).Conclusion Arthroscopic limited debridement combined with fumigation of Bushen Huoxue decoction can effectively relieve the pain of knee joint,improve the function of knee joint,and have a good effect on early and middle knee osteoarthritis.
