腹腔镜腹膜前疝修补术与Lichtenstein平片疝修补术治疗腹股沟疝的临床效果对比

摘要目的比较腹腔镜腹膜前疝修补术（TAPP）与Lichtenstein 平片疝修补术治疗腹股沟疝的临床效果。方法选择2016年1月～2017年5月于我院就诊的86 例腹股沟疝患者，采用随机数字表法分为TAPP 组（43 例）和对照组（43 例）。TAPP 组行腹腔镜腹膜前疝修补术，对照组采用Lichtenstein 平片疝修补术，记录和比较两组患者的手术时间、术后住院时间、下床活动时间、术后并发症发生率及术后疼痛评分等。结果两组的平均手术时间比较，差异无统计学意义（P＞0.05）。TAPP 组的术后住院时间、下床时间短于对照组，差异有统计学意义（P＜0.05）。TAPP组的并发症总发生率（2.33%）低于对照组（18.60%），差异有统计学意义（P＜0.05）。TAPP 组术后1、3、7 d 的疼痛评分低于对照组，差异有统计学意义（P＜0.05）。结论与Lichtenstein 疝平片修补术相比，TAPP 术在术后住院时间、下床活动时间、术后并发症发生率及术后疼痛方面具有显著优势。

Abstract：Objective To compare the clinical effects of transabdominal preperitoneal prosthetic (TAPP) herniorrhaphy and Lichtenstein hernia repair in the treatment of inguinal hernia.Methods From January 2016 to May 2017, 86 patients with inguinal hernia unilateral treated in our hospital were selected and divided into the TAPP group and the control group according to random number table method, 43 cases in each group.Patients in the TAPP group were treated with laparoscopic transabdominal preperitoneal prosthetic, and patients in the control group were treated with Lichtenstein hernia repair.The operative time, average length of hospital stay, postoperative leaving bed time, the incidence of postoperative complications and postoperative pain score were compared and recorded between the two groups.Results There was no significant difference in mean operation time between the two groups (P＞0.05).The average length of hospital stay and postoperative leaving bed time of the TAPP group were significantly shorter than those of the control group, with significant difference (P＜0.05).The total incidence of postoperative complications of the TAPP group was significantly lower than that of the control group, with significant difference (P＜0.05).The pain score of TAPP group at 1, 3 and 7 days after operation was lower than that of control group, with significant difference (P＜0.05).Conclusion Compared with Lichtenstein hernia repair, TAPP have significant advantages in average length of hospital stay, postoperative leaving bed time, the incidence of postoperative complicoations and postoperative pain score.
