Progress in the treatment of contrast nephropathy
CHEN Yu QIN Yan ZENG Qing
Contrast-induced nephropathy (CIN) is a common reversible acute renal failure in clinical practice. The incidence rate of CIN is increasing annually with the widely use of contrast agents such as angiography, coronary angiography, cerebral angiography, or other medical related radiography (urography and cholangiography). It has been an important cause of acute kidney injury after ischemia and drug injury. Nowadays, the mechanism of CIN induced by contrast agent is not completely clear, mainly including oxidative stress and apoptosis. In addition, many studies have also found that renal ischemia and tubular toxic injury are also the key pathological mechanisms of CIN. The occurrence of CIN is not only related to the increased medical expenses, but also leads to a significant increase in the risk of adverse outcomes (such as acute myocardial infarction, acute heart failure, dialysis and death). Therefore, early identification of risk factors for CIN and appropriate clinical intervention will help high-risk CIN patients benefit from early renal protection strategies and improve prognosis. This study reviews the diagnosis of CIN and the progress of current treatment measures, so as to provide basis for clinical practice.

Research progress on application of three skin expanding methods in peripherally inserted central catheter
WANG Hejin1 HE Jin′ai2▲ HE Jinwei1 LI Xiaoling1
Peripherally inserted central catheter (PICC) has the obvious advantages of reducing the pain caused by repeated venipuncture, protecting the patient′s blood vessels, avoiding drug extravasation caused by stimulating drug infusion through peripheral blood, and reducing the workload of nursing staff. At present, there are three methods of dilatation in clinical practice, they are, traditional dilatation knife method, blunt separation method and small incision combined with dilator blunt separation method. Thrombocytopenia is relatively contraindicated for PICC catheterization. Therefore, in the nursing process after catheterization, it is necessary to focus on key issues such as local bleeding and petechiae at the puncture point in patients with less platelets. This paper reviews the research progress of the application of three skin expansion methods in peripherally inserted central catheter, compares and analyzes their operation steps and the amount of bleeding, puts forward the enlightenment of PICC catheterization in patients with thrombocytopenia. Small incision combined with blunt separation of expander can effectively reduce the injury and bleeding of catheterization, it can play a better therapeutic effect and has important clinical application and research significance. However, in order to make the catheterization method widely used, for the special group of people with thrombocytopenia in vivo, more in-depth clinical exploration, improvement and optimization of the operation method are needed. This paper is expected to provide reference for further clinical research.

HUANG Shuang; HAO Jiping; GAO Yuqin; LIANG Oujing;TIAN Feng
Objective To investigate the influencing factors and the changes of ultrasonic blood flow parameters of renal segmental artery before and after coronary angiography. Methods A total of 46 patients undergoing coronary angiography or percutaneous coronary intervention (PCI) in Ninth Hospital of Xi′an from July 2020 to April 2021 were selected as the study subjects. The peak systolic velocity (PSV),end diastolic velocity (EDV)and resistance index (RI) of bilateral renal segmental arteries were measured before angiography and the 3rd day and the 10th day after angiography. The influencing factors of renal blood flow parameters after radiography were also discussed by multiple linear regression analysis. Results The PSV on the 3rd day and 10th day after angiography were higher than that before angiography, the EDV on the 10th day after angiography was higher than that before angiography and on the 3rd day after angiography, the RI on the 3rd day after angiography was higher than that before angiography, the differences were statistically significant (P<0.05). On the 3rd day after angiography, PSV was related to age and gender, EDV was correlated with age and the using of Calcium Channel Blockers (CCB). While on the 10th day after angiography, PSV was only related to age,EDV was correlated with age, the dose of contrast medium and body mass index. On the 3rd day and 10th day after angiography, RI was related to age and diabetes. Conclusion PSV, EDV and RI of segmental renal arteries show a trend of deterioration and subsequent improvement. After angiography, age and gender are the influencing factors of PSV.Age, CCB, contrast agent volume and body mass index are the influencing factors of EDV, then age and diabetes are the influencing factors of RI. Among them, age is the most significant influencing factor for renal blood flow parameters, regardless of the contrast agent itself.

The two groups of patients were compared with the numerical rating scale (NRS) score, the number of burst pain, Pittsburgh sleep quality index (PSQI) score, cancer fatigue scale (CFS) score and the satisfaction rate of pain control. Results There were no statistically significant differences in NRS score, burst pain time, PSQI score, and CFS score between the two groups before intervention (P>0.05). The NRS score of the two groups after intervention was lower than those before intervention, the burst pain time was less than that before intervention, and the PSQI and CFS scores were lower than those before intervention, the differences were statistically significant (P<0.05). The NRS score of the observation group after intervention was lower than that of the control group, the burst pain time was less than that of the control group, the PSQI score and CFS score were lower than those in the control group, and the total pain control satisfaction rate was higher than that of the control group, the differences were statistically significant (P<0.05). Conclusion The systematic pain nursing model can effectively reduce the pain degree and burst pain frequency of patients with cancer pain at home, reduce the degree of cancer-related fatigue, and improve the patient′s sleep quality and pain control satisfaction rate.

GU Xiaoyan;YI Chaoqun
Objective To explore the effect of early enteral nutrition nursing in diabetic patients with severe cerebral infarction. Methods A total of 78 patients with severe diabetic cerebral infarction admitted to the Fifth Affiliated Hospital of Guangzhou Medical University from July 2020 to June 2021 were selected as research objects. They were divided into control group (routine nursing) and research group (early enteral nutrition) by drawing lot method, with 39 cases in each group. The control group using routine nursing method and research group using early enteral nutrition method. The adverse reactions, nutritional status (arm circumference [AC], serum albumin [ALB], arm muscle circumference[AMC] and triceps skinfold thickness [TST] of upper limb on non paralyzed side) and nursing satisfaction of two groups were observed. Results After treatment, AC and AMC in the study group were longer than those in the control group,ALB was higher than that in the control group, and TST was thicker than that in the control group, the differences were statistically significant (P<0.05). The nursing satisfaction rate of the study group was higher than that of the control group, the difference was statistically significant (P<0.05). The total incidence of the study group was lower than that in the control group, the difference was statistically significant (P<0.05). Conclusion The application of early enteral nutrition in severe diabetic cerebral infarction has a high value, can improve the satisfaction and nutritional status of patients, reduce the total incidence of adverse reactions, and it′s worthy of promotion and application.
