Abstract:Objective To observe and analyze the influence of position management assisted by bare hand rotation of fetal head on the pregnancy outcome of primipara. Methods A total of 1080 primiparas admitted to our hospital from June 2018 to May 2019 were selected as the study objects. According to the random number table method they were divided into the control group (n=540)and the observation group (n=540).The control group used conventional midwifery,and the observation group used body position management to assist in freehand rotation of fetal head. Methods of delivery,duration of labor and complications of delivery were compared between the two groups. Results The rate of natural delivery in the observation group was higher than that in the control group,and the rate of instrument delivery and cesarean section was lower than that in the control group (P<0.05).In the observation group,the rate of second degree laceration and perineal incision was lower than that in the control group,and the rate of perineal completion was higher than that in the control group (P<0.05). The first,second and total labor stages of the observation group were shorter than those of the control group (P<0.05). The incidence of postpartum hemorrhage,secondary uterine inertia and postpartum urinary retention in the observation group were lower than those in the control group (P<0.05). There was no significant difference in the incidence of neonatal asphyxia,cervical laceration and amniotic fluid pollution between the two groups (P>0.05). Conclusion During the stage of labor,the body position management and the free-hand rotation of the fetal head can effectively reduce the risk of midwifery of the cesarean section and the device,shorten the time of first and second labor,reduce the postpartum hemorrhage rate,reduce the degree of perineal injury,and reduce the occurrence of secondary uterine contraction and post-partum urine retention. And the delivery safety is improved,and the effect is remarkable.
