LI Shiqing; SONG Zengliang
Objective To investigate the diagnostic value of CT perfusion imaging (CTP) combined with mean arterial pressure (MAP) in patients with severe craniocerebral trauma and its relationship with prognosis. Methods A total of 90 patients with severe suspected craniocerebral trauma who were admitted to the Department of Neurosurgery of the Third Hospital of Nanchang from April 2019 to February 2021 were selected as the research objects. All patients underwent routine head CTP examination after admission, and the final clinical examination results were used as "golden standard". The diagnostic value was analyzed and the MAP level was monitored. The patients were followed up for 6 months, and the Glasgow coma index score (GOS) was used as the prognostic scoring standard. The patients were evaluated at 1, 6 months after treatment, and the correlation analysis was completed. Results Among 90 patients with severe suspected craniocerebral trauma, 62 patients were finally diagnosed by clinical examination, with a diagnosis rate of 68.89%. 57 patients were finally diagnosed by head CTP examination, and the coincidence rate with the "gold standard" diagnosis was 85.56% [(53+24)/90]. The diagnostic sensitivity was 85.48% (53/62), the specificity was 85.71% (24/28). The levels of cerebral blood perfusion rate (CBF), cerebral blood volume (CBV) and MAP in severe craniocerebral trauma were lower than those in non-severe craniocerebral trauma, and the differences were statistically significant (P<0.05). The mean transit time (MTT) and the time to peak contrast (TTP) were longer than those of non-severe craniocerebral trauma, and the differences were statistically significant (P<0.05). The average GOS score of patients with severe craniocerebral trauma was (3.13±0.63). The results of correlation analysis showed that the GOS score of patients with severe craniocerebral trauma was negatively correlated with the levels of CBF, CBV and MAP, and positively correlated with MTT and TTP (P<0.05). Conclusion CTP has high diagnostic value in patients with severe craniocerebral trauma. Combined examination with MAP can guide clinical treatment and has a strong correlation with the prognosis of patients.

Research progress in diagnosis and differential diagnosis of spinal tuberculosis
LIU Jian; WANG Li′nan; MA Hongbao; LOU Caili; NIU Ningkui
Spinal tuberculosis is the most common extrapulmonary and osteoarticular tuberculosis caused by Mycobacterium tuberculosis. Its onset is hidden, early clinical symptoms and signs are not obvious, similar to other infectious diseases of the spine, it is difficult to diagnose, often lead to missed diagnosis, misdiagnosis, and even mistreatment. Patients cannot be diagnosed and treated in time. With the aggravation of the disease, they often visit the doctor when nerve injury, bone destruction, spinal instability, kyphosis and other complications, which increases the difficulty of clinical treatment and seriously affects the quality of work and life of patients. Early diagnosis and treatment are the key factors affecting the prognosis of spinal tuberculosis. With the rapid development of diagnostic technology and the continuous updating of treatment regimens, the level of diagnosis and treatment has improved significantly. However, the number of patients with spinal tuberculosis has increased in recent years due to drug resistance, infectious disease transmission and other factors. In order to improve the diagnostic level of clinicians for spinal tuberculosis, this paper reviewed the research progress of typical clinical symptoms and signs, auxiliary examination and differential diagnosis of spinal tuberculosis in recent years.

Research progress on the role of sputum eosinophils in airway inflammation in bronchial asthma
DENG Niangen; WANG Jun
Bronchial asthma is one of the common specific diseases. It is often accompanied by symptoms such as asthma, shortness of breath, chest tightness and cough. Some patients will also develop chronic obstructive pulmonary disease. With prolonged disease duration, patients will experience irreversible airflow limitation and airway remodeling. Therefore, early diagnosis of the disease is necessary to avoid missed optimal treatment opportunities due to missed diagnosis and misdiagnosis. Sputum eosinophils (EOS) are inflammatory effector cells in bronchial asthma, and their changes are closely related to airway inflammation and airway remodeling in patients, so they are important in the diagnosis, treatment, and assessment of disease progression in patients with bronchial asthma. In order to clarify its value in the progress of airway inflammation in bronchial asthma, this paper discusses the research progress of the role of sputum eosinophils in airway inflammation in bronchial asthma. Workers in the same industry provide references.

Objective To study the common etiology and infection characteristics of community-acquired pneumonia in elderly patients in emergency department. Methods A total of 79 elderly patients who were diagnosed with community-acquired pneumonia in the emergency department of Beijing Jingmei Group General Hospital from March 2020 to October 2021 were retrospectively analyzed. Qualified sputum samples were taken, common etiology was analyzed, the clinical characteristics of patients with mixed infection and single bacterial infection were compared. The variables with statistically significant difference in univariate analysis were included in logistic multivariate regression analysis. Results The detection rate of Gram-negative bacilli such as Klebsiella pneumoniae and Pseudomonas aeruginosa were higher in elderly patients with community-acquired pneumonia. Acinetobacter baumannii has a high drug resistance rate. The results of univariate analysis showed that mixed infection was related to dependence on gastric tube, urinary tube, bedridden status, history of antibiotic abuse, respiratory disease, type 2 diabetes, and infection rate of multi-drug resistant bacteria, and the differences were statistically significant (P<0.05). Multivariate analysis results of logistic regression model showed that type 2 diabetes (β=2.166, OR=8.723, 95%CI=2.195-34.671), multi-drug resistant bacteria infection (β=1.749, OR=5.746, 95%CI=1.166-28.323) were independent risk factors for mixed infection (P<0.05). Conclusion The bacteria isolated from community-acquired pneumonia in elderly patients gradually appear to have a bacterial spectrum similar to that of hospital-acquired pneumonia bacteria. Type 2 diabetes and multi-drug resistant bacteria infection are independent risk factors for mixed infection.

Evaluation value of combined monitoring of bispectral index and neuron-specific enolase in the prognosis of patients with traumatic brain injury
QIAN Wuqiang; ZHAO Ping; SHI Xiaofen; TANG Jianlei; CUI Weiyan
Objective To explore the diagnostic value of combined monitoring of bispectral index (BIS) and neuron-specific enolase (NSE) in the prognosis of patients with traumatic brain injury. Methods A total of 60 patients with raumatic brain injury from March 2020 to July 2021 in Wujin Hospital Affiliated to Jiangsu University were selected as the research subjects. According to the Glasgow outcome scale (GOS), they were divided into the good prognosis group (n=23) and the poor prognosis group (n=37). All patients received BIS monitoring and NSE level detection. The BIS value and NSE level of the two groups of patients were compared, and the correlation between BIS value and NSE index and prognostic score, as well as the predictive value of BIS value and NSE index on the prognosis of traumatic brain injury were analyzed. Results The BIS level in the good prognosis group was higher than that in the poor prognosis group, while the NSE level in the good prognosis group was lower than that in the poor prognosis group, and the differences were statistically significant (P<0.05). The results of correlation analysis showed that the BIS value was positively correlated with the prognostic GOS score (r=0.712, P=0.001), and the NSE level was negatively correlated with the prognostic GOS score (r=-0.606, P=0.001). Combined monitoring of BIS value and NSE index (AUC=0.93) had higher sensitivity and specificity than those of single BIS value (AUC=0.81) and NSE index (AUC=0.77) in predicting poor prognosis of traumatic brain injury. Conclusion Both BIS value and serum NSE level can reflect the prognosis quality of patients with traumatic brain injury. The lower the BIS value, the higher the NSE index, the worse the prognosis quality, and the combined monitoring is more conducive to improving the monitoring sensitivity and specificity, which is worthy of clinical promotion.

WANG Jingrong
Objective To analyze the effect of Green′s model-based health education on social avoidance in patients with colostomy. Methods A total of 80 patients with enterostomy admitted to the Department of Oncology, Ji′an Central People′s Hospital from October 2019 to June 2021 were selected as the research objects, and divided into the control group and the observation group by random number table method, with 40 cases in each group. The patients in the control group were given routine health education, and the patients in the observation group were given the Green′s model-based health education. The social avoidance, stoma self-care ability and the incidence of stoma complications within 3 months after discharge were compared between the two groups. Results Eight weeks after the intervention, the social avoidance and distress scale (SADS) score of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). Eight weeks after the intervention, the stoma self-care scale-general version (SSCS-GV) score of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). Within 3 months after discharge, the total incidence of stoma complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion Green′s model-based health education can reduce the social avoidance of colostomy patients, improve self-care ability, and reduce the occurrence of complications.

LI Jinlong; LI Huan
Objective To explore the application effect of the concept of rapid rehabilitation surgery in thoracoscopic surgical nursing of lung cancer. Methods A total of 80 patients with thoracoscopic lung cancer surgery admitted to the Affiliated People′s Hospital of Fujian University of Traditional Chinese Medicine from March 2019 to November 2020 were selected as the study subjects, and they were divided into control group and observation group by random number table method, with 40 cases in each group. The control group received routine nursing, and the observation group received rapid rehabilitation surgical concept intervention on the basis of routine nursing. The incidence of complications, perioperative indexes and lung function indexes were compared between the two groups. Results The total incidence of complications in observation group was lower than control group, the difference was statistically significant (P<0.05). The time of operation, time of getting out of bed, time of urinary tube extubation, time of chest tube extubation and length of hospital stay in the observation group were shorter than those in the control group, and the hospitalization cost in the observation group was lower than that in the control group, the differences were statistically significant (P<0.05). There were no significant differences in preoperative peak expiratory flow (PEF), 6 min walking distance and resting oxygen saturation (SpO2) between two groups (P>0.05). PEF, 6 min walking distance and SpO2 at rest in the observation group were higher than those in the control group, the differences were statistically significant (P<0.05). Conclusion The intervention of patients undergoing thoracoscopic lung cancer surgery with the concept of rapid rehabilitation surgery can significantly reduce the rate of infection and complications, and promote improved lung function and postoperative recovery.
