以督脉大椎、百会为主穴治疗血管性轻度认知损害的临床效果

摘要目的探讨以督脉大椎、百会为主穴，采用针刺疗法治疗血管性轻度认知损害的临床效果。方法选取2017年1月～2020年1月就诊于门头沟区中医院的60 例血管性轻度认知损害的患者作为研究对象，采用随机数字表法将其分为针刺组和药物组，每组各30 例。两组患者均采用脑血管病二级预防及重复经颅磁刺激的治疗方法，针刺组选取督脉的大椎和百会为主穴，结合辩证选取配穴，治疗频率为3 次/周。药物组予盐酸多奈哌齐5 mg 口服，1 次/d。按照试验要求完成为期12 周的治疗后，观察两组患者的蒙特利尔认知评估量表（MoCA）、简明精神状态量表（MMSE）、画钟试验（CDT）评分变化，比较两组患者的治疗效果。结果治疗后，两组患者的MoCA、MMSE、CDT 评分均高于治疗前，差异有统计学意义（P＜0.05）。治疗后，针刺组患者的MoCA、MMSE、CDT 评分均高于药物组，差异有统计学意义（P＜0.05）；治疗后，针刺组患者的总有效率为93.33%，药物组患者的总有效率为60.00%，针刺组患者的总有效率高于药物组，差异有统计学意义（P＜0.05）。结论以督脉的大椎、百会为主穴，配合辩证取穴，可以有效改善血管性轻度认知损害患者的认知功能，值得推广。

Abstract：Objective To investigate the clinical effect of acupuncture at Dazhui and Baihui of Governor Vessel as the main points in the treatment of vascular mild cognitive impairment.Methods Sixty patients with vascular mild cognitive impairment admitted to Mentougou Hospital of Traditional Chinese Medicine from January 2017 to January 2020 were selected as subjects.They were divided into the acupuncture group and the medication group by the random number table method,with 30 patients in each group.The two groups were both treated with secondary prevention of cerebrovascular disease and repeated transcranial magnetic stimulation.The acupuncture group were given acupuncture at Dazhui and Baihui of Governor Vessel as the main points combined with dialectical selection of other points,at the frequency of 3 times per week.The medication group were given oral Donepezil Hydrochloride 5 mg,once a day.After completing the 12-week treatment according to the study requirements,the changes of the scores of Montreal cognitive assessment scale(MoCA),concise mental state scale(MMSE)and picture clock test(CDT)were observed in the two groups,and the therapeutic effect of the two groups was compared.Results After treatment,the scores of MoCA,MMSE,and CDT in the two groups were higher than those before treatment,with statistically significant differences(P＜0.05).After treatment,the scores of MoCA,MMSE,and CDT in the acupuncture group were higher than those in the medication group,with statistically significant differences(P＜0.05).After treatment,the total effective rate of the acupuncture group was 93.33%,and that of the medication group was 60.00%.The total effective rate of the acupuncture group was higher than that of the medication group,with statistically significant difference(P＜0.05).Conclusion Acupuncture at Dazhui and Baihui of Governor Vessel as the main points combined with dialectical selection of other points can effectively improve the cognitive function of patients with vascular mild cognitive impairment,which is worthy of promotion.
