医院-社区-家庭一体化管理对慢性阻塞性肺疾病高龄患者生命质量的影响

摘要目的探讨医院-社区-家庭一体化管理对慢性阻塞性肺疾病高龄患者生命质量的影响。方法选取2016年5～12月我中心及建设街社区卫生服务中心接诊的80例老年慢性阻塞性肺疾病患者作为研究对象，采用随机数字表法分为对照组和研究组，每组各40例。对照组给予常规护理干预，研究组在对照组基础上给予医院-社区-家庭一体化管理，比较两组患者的血气指标、肺功能指标和生命质量评分。结果两组患者实施后的动脉血氧分压（PaO2）高于实施前，二氧化碳分压（PaCO2）及pH低于实施前，差异均有统计学意义（P＜0.05）；研究组患者实施后的PaO2高于对照组，PaCO2及pH低于对照组，差异均有统计学意义（P＜0.05）。两组患者实施后的1秒用力呼气容积（FEV1）、用力肺活量（FVC）、FEV1/FVC 的比值（FEV1%）、峰值呼气流速（PEF）、以及 FEV1/FVC 高于实施前，差异均有统计学意义（P＜0.05）；研究组实施后的各肺功能指标高于对照组，差异均有统计学意义（P＜0.05）。两组患者实施后的生命质量总均分（TMS）及各维度得分低于实施前，差异均有统计学意义（P＜0.05）；研究组患者实施后的生命质量总均分（TMS）及各维度得分低于对照组，差异均有统计学意义（P＜0.05）。结论医院-社区-家庭一体化管理在慢性阻塞性肺疾病高龄患者中应用，能有效改善肺功能及血气指标，提高患者的生命质量，具有较高的临床价值。

Abstract：]Objective To explore the effect of Hospital-Community-Family integrated management on the quality of life of elderly patients with chronic obstructive pulmonary disease.Methods A total of 80 elderly patients with chronic obstructive pulmonary disease(COPD)admitted to our center and Jianshe Community Health Service Center From May to December 2016 were selected as the study subjects,they were divided into control group and research group by random number table method,with 40 cases in each group.The control group was given routine nursing intervention,while the research group was given Hospital-Community-Family integrated management on the basis of the control group.Blood gas index,lung function index and quality of life score were compared between the two groups.Results The arterial partial pressure of oxygen (PaO2)after implementation was higher than before,and the partial pressure of carbon dioxide(PaCO2)and pH were lower than before,the differences were statistically significant(P＜0.05).The PaO2of the study group was higher than that of the control group,and the PaCO2and pH of the study group were lower than that of the control group,the differences were statistically significant(P＜0.05).The forced expiratory volume(FEV1),forced vital capacity (FVC),the ratio of FEV1to FVC (FEV1%),peak expiratory flow (PEF)and FEV1/FVC were significantly higher in the two groups than before,the differences were statistically significant(P＜0.05).The pulmonary function indexes in the study group were higher than those in the control group,the differences were statistically significant(P＜0.05).The total average score of quality of life(TMS)and each dimension score of the two groups after implementation were lower than those before implementation,the differences were statistically significant(P＜0.05).The total average score of quality of life (TMS)and the scores of each dimension in the study group were lower than those in the control group,the differences were statistically significant(P＜0.05).Conclusion The application of Hospital-Community-Family integrated management in elderly patients with chronic obstructive pulmonary disease can effectively improve pulmonary function and blood gas index,and improve the quality of life of patients with high clinical value.
