矽肺合并慢性阻塞性肺疾病患者的肺功能及血气分析

摘要目的了解矽肺合并慢性阻塞性肺疾病患者的肺功能及血气指标特点，并分析其临床意义。方法选取2016年8月～2017年8月在我院诊断治疗的100例矽肺患者作为研究对象，根据是否合并慢性阻塞性肺疾病进行分组，其中对照组50例为单纯矽肺患者，研究组50例为矽肺合并慢性阻塞性肺疾病患者。采用日本CHESTAC-50型肺功能测定仪测定两组患者肺功能。采集两组患者股动脉血并采用美国雅培i-STAT1血气分析仪进行血气指标分析。结果研究组1秒用力呼出量（FEV1）、用力肺活量（FVC）、FEV1/FVC以及一氧化碳弥散量（DLco）显著低于对照组，差异有统计学意义（P＜0.05）。研究组氧分压、血氧饱和度、血液酸碱度显著低于对照组，差异有统计学意义（P＜0.05）。研究组二氧化碳分压显著高于对照组，差异有统计学意义（P＜0.05）。结论矽肺合并慢性阻塞性肺疾病表现出其特有的临床病理特征，给予该类患者肺功能检查以及血气分析可为其病情及预后判断提供非常重要的参考依据。

Abstract：ObjectiveTo investigate the pulmonary function and blood gas indexes in patients with silicosis complicated with chronic obstructive pulmonary disease (COPD),and to analyze its clinical significance.Methods100 cases of silicosis patients treated in our hospital from August 2016 to August 2017 were selected as the study subjects.According to whether or not to merge chronic obstructive pulmonary disease,50 patients in the control group were simple silicosis patients,50 patients in the study group were silicosis combined with COPD.The lung function of two groups of patients was measured by the Japanese CHESTAC-50 lung function tester.Two groups of patients were collected for arterial blood,the blood gas analysis was carried out by the United States abbott i-STAT1 blood gas analyzer.ResultsOne second forced expiratory volume(FEV1),forced vital capacity(FVC),one second forced expiratory volume/forced vital capacity(FEV1/FVC)and carbon monoxide dispersion(DLco)in the study group were significantly lower than those in the control group(P＜0.05).The oxygen partial pressure(PaO2),oxygen saturation(SpO2)and blood acidity(pH)in the study group were significantly lower than those in the control group(P＜0.05).The partial pressure of carbon dioxide (PaCO2)in the study group was significantly higher than that in the control group (P＜0.05).ConclusionSilicosis combined COPD showed its unique clinical pathological features,the patients′lung function and blood gas analysis can provide very important for its condition and prognosis judgement of reference.
