基于慢性疾病轨迹模式的护理干预在血液透析患者中的应用

摘要目的探讨基于慢性疾病轨迹模式护理干预在血液透析（HD）患者的应用效果。方法选取2019年7月—2020年6月佛山市南海区第四人民医院HD 治疗的120 例终末期肾病患者，使用随机数字表法分为对照组和观察组，每组各60 例。对照组实施常规护理，观察组实施基于慢性疾病轨迹模式护理干预；比较两组干预前后的焦虑、抑郁、应对方式及生活质量情况。结果干预前两组的焦虑、抑郁、应对方式各维度和生活质量各维度评分比较，差异无统计学意义（P＞0.05）；干预后两组的焦虑、抑郁、回避和屈服维度评分低于干预前，除对照组生活质量生理职能维度外，两组的面对维度及生活质量其他维度评分高于干预前，差异均有统计学意义（P＜0.05）；干预后观察组的焦虑、抑郁、回避和屈服维度评分低于对照组，面对维度及生活质量各维度评分高于对照组，差异均有统计学意义（P＜0.05）。结论基于慢性疾病轨迹模式护理干预可明显缓解HD 患者的焦虑和抑郁心理，使患者积极应对疾病和治疗，提高生活质量。

Abstract：Objective To explore the application effect of nursing intervention based on chronic disease locus model in hemodialysis (HD) patients. Methods A total of 120 patients with end-stage renal disease who underwent HD in Nanhai Fourth People′s Hospital of Foshan from July 2019 to June 2020 were selected and divided into the control group and the observation group according to random number table method, 60 cases in each group. The control group was given routine nursing, and the observation group was given nursing intervention based on chronic disease track mode;the changes of anxiety, depression, coping style and quality of life of the two groups before and after intervention were compared. Results Before the intervention, there were no significant differences in the scores of anxiety, depression,coping style and quality of life between the two groups (P＞0.05). After the intervention, the scores of anxiety, depression, avoidance dimension and yield dimension of the two groups were lower than those before the intervention, except for the physiological function dimension of the quality of life of the control group, the scores of the face dimension and other dimensions of the quality of life of the two groups were higher than those before the intervention, the differences were statistically significant (P＜0.05). After the intervention, the scores of anxiety, depression, avoidance and yield in the observation group were lower than those in the control group, while the scores of face dimension and quality of life in the observation group were higher than those in the control group, the differences were statistically significant (P＜0.05). Conclusion The nursing intervention based on chronic disease trajectory model can significantly alleviate the anxiety and depression of HD patients, promote the patients to actively deal with the disease and treatment, and improve the quality of life.
