医院与社区联合进行慢性心力衰竭中西医结合专病管理模式的探索

摘要目的探讨医院和社区联合进行慢性心力衰竭（CHF）中西医结合专病管理的效果。方法选取2015年5月～2016年5月我院接收的150例CHF患者，随机分为对照组（n=75）与观察组（n=75），对照组给予现行的医疗模型，观察组给予医院和社区联合进行CHF中西医结合专病管理，观察两组管理效果。结果两组管理前后适当运动、低盐饮食、戒烟、服药依从性比较，差异有统计学意义（P＜0.05）；管理后，观察组在适当运动、低盐饮食、戒烟、服药依从性均显著高于对照组，差异有统计学意义（P＜0.05）。观察组心功能总有效率为90.7%，高于对照组的73.3%，差异有统计学意义（P＜0.05）。管理后，观察组QOL积分及LVEF均显著优于对照组，差异有统计学意义（P＜0.05）。观察组总死亡率、再住院率、急诊率、恶化率均显著低于对照组，差异有统计学意义（P＜0.05）。结论医院与社区联合进行CHF的中西医结合专病管理，可提高患者的依从性，有利于提高治疗效果及患者的预后，提高生活质量，具有推广价值。

Abstract：ObjectiveTo study the clinical effect of combination of hospital and community special disease management mode of TCM and Western medicine for chronic heart failure (CHF).Methods150 patients with CHF treated in our hospital from May 2015 to May 2016 were selected and randomly divided into two groups,75 cases in each group.Patients in control group adopted the existing medical mode,patients in observation group adopted hospital and community special disease management mode of TCM and Western medicine.The clinical effect between two groups was observed.ResultsThe difference of proper exercise,low salt diet,quiting smoking and medication compliance before and after administration in two groups had statistical significance(P＜0.05);after administration,proper exercise,low salt diet,quiting smoking and medication compliance in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P＜0.05).The total effective rate of cardiac function in the observation group was 90.7%,higher than 73.3%in the control group,and the difference was statistically significant(P＜0.05).After management, QOL integral and LVEF of the observation group was significantly better than that of the control group,and the difference was statistically significant(P＜0.05).The total mortality rate,rehospitalization rate,emergency rate and deterioration rate in the observation group was significantly lower than that in the control group,and the difference was statistically significant (P＜0.05).ConclusionFor patients with CHF,hospital and community special disease management mode of TCM and Western medicine can improve the patient′s compliance,treatment effect,prognosis and life quality.It is worthy of promotion.
