Impact of extended care on the disease control and life quality in elderly patients with chronic heart failure

Abstract:Objective To explore the impact of extended care on the disease control and life quality in elderly patients with chronic heartfailure(CHF).Methods 112 elderly patients with CHF from February 2013 to February 2015 in ourhospital were divided into observation group and control group,according to their hospital admission sequence,56 cases in each groups.Patients in observation group were added regular telephone follow-up and family follow-up on the basis of that with in 12 months,and patients in control group were received cycle of internal conventional western medicine and nursing.The readmission rate,mortality rate and changes of life quality of patients in two groups were compared.Results The readmission rate in observation group was 32.1%,obvious lower than that of 62.5%in control group,and the difference was significant(P<0.01).The mortality rate in observation group was 3.6%,obvious lower than that of 10.7%in control group,and the difference was significant(P<0.05).The life quality of the two groups were improved compared significantly with pre-treatment(P<0.05),but the improvement effects in observation group were more obvious(P<0.05). Conclusion The extended care can effectively control the progression of CHF in elderly patients,reduce the readmission rate,and obvious improve the life quality.
