肾上腺色腙片联合氨甲环酸氯化钠注射液在咯血患者中的应用效果

摘要目的探讨肾上腺色腙片联合氨甲环酸氯化钠注射液在咯血患者中的应用效果。方法选取2016年1月～2018年12月萍乡市人民医院呼吸内科收治的70例咯血患者作为研究对象，并采用随机数字表法将其分为对照组和实验组，每组各35例。其中对照组患者采取肾上腺色腙片治疗，实验组患者采取肾上腺色腙片联合氨甲环酸注射液治疗。统计两组患者的临床治疗效果；比较两组患者治疗前后的日咯血量以及咳血停止时间；比较两组患者的凝血酶原时间（PT）、凝血酶时间（TT）、活化部分凝血活酶时间（APTT）以及纤维蛋白原（Fbg）凝血功能相关指标；比较两组患者治疗期间出现的不良反应发生率。结果实验组患者的治疗总有效率为91.43%，高于对照组的71.43%，差异有统计学意义（P＜0.05）；实验组患者治疗后的日咳血量少于对照组，咳血停止时间短于对照组，差异有统计学意义（P＜0.05）；实验组患者治疗后的PT 长于对照组，TT、APTT 短于对照组，Fbg 水平低于对照组，差异有统计学意义（P＜0.05）；实验组患者的不良反应总发生率为8.57%，对照组患者的不良反应总发生率为20.00%，两组患者的不良反应总发生率比较，差异无统计学意义（P＞0.05）。结论肾上腺色腙片联合氨甲环酸注射液治疗咯血的效果确切，能减少日咳血量，缩短咳血停止时间，改善凝血功能。

Abstract：Objective To explore application effect of Carbazochrome Tables combined with Tranexamic Acid Sodium Chloride Injection on hemoptysis patients.Methods A total of 70 hemoptysis patients who were admitted to the Department of Respiratory Medicine, Pingxiang People′s Hospital, during the period from January 2016 to December 2018 were selected as subjects.They were divided into control group and experimental group by random number table method, 35 cases in each group.The control group was treated with Carbazochrome Tables, while experimental group was treated with Carbazochrome Tables combined with Tranexamic Acid Sodium Chloride Injection.The clinical curative effect of the two groups was statistically analyzed.The daily hemoptysis volume and stop time of hemoptysis before and after treatment were compared between the two groups.The coagulation function related indexes such as prothrombin time (PT), thrombin time (TT), activated partial thromboplastin time (APTT) and fibrinogen (Fbg) were compared between the two groups.The incidence of adverse reactions during treatment was compared between the two groups.Results The total effective rate was 91.43% in the experimental group, higher than 71.43% in the control group, with statistically significant differences (P＜0.05).The daily hemoptysis volume of the experimental group after treatment was less than that of the control group, and the hemoptysis cessation time was shorter than that of the control group, the differences were statistically significant (P＜0.05).After treatment, PT in the experimental group was longer than the control group, the TT, APTT were shorter than those in the control group, and Fbg level was lower than the control group, with statistically significant differences (P＜0.05).The total incidence of adverse reactions was 8.57% in the experimental group and 20.00% in the control group.There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P＞0.05).Conclusion The curative effect of Carbazochrome Tables combined with Tranexamic Acid Sodium Chloride Injection is significant on hemoptysis, which can reduce daily hemoptysis volume, shorten stop time of hemoptysis, and improve coagulation function.
