步长脑心通胶囊联合尼莫地平对脑出血恢复期患者神经营养状态及神经损伤程度的影响

摘要目的探讨步长脑心通胶囊联合尼莫地平对脑出血恢复期患者神经营养状态及神经损伤程度的影响。方法选取我院2017年1月～2018年1月收治的176 例脑出血恢复期患者作为研究对象，按照随机、双盲、对照原则分为观察组（88 例）与对照组（88 例。观察组采用步长脑心通胶囊联合尼莫地平治疗，对照组采用尼莫地平治疗。比较两组的疗效、血清神经营养状态指标[神经营养因子（NGF）、脑源性神经营养因子（BDNF）、胰岛素样生长因子-1（IGF-1）、胰岛素样生长因子-2（IGF-2）]含量及神经损伤程度。结果观察组的总有效率高于对照组，差异有统计学意义（P＜0.05）。两组的神经功能缺损评分（NHISS）比较，差异无统计学意义（P＞0.05）。观察组治疗后的NHISS评分低于对照组，差异有统计学意义（P＜0.05）。两组治疗前的NGF、BDNF、IGF-1、IGF-2 含量比较，差异无统计学意义（P＞0.05）。观察组患者治疗后的NGF、BDNF、IGF-1、IGF-2 含量明显增加且高于对照组，差异有统计学意义（P＜0.05）。结论脑出血恢复期患者接受步长脑心通胶囊联合尼莫地平治疗，不仅在一定程度上提高了治疗效果，而且能够促进神经营养状态改善，减少神经功能缺损，效果显著，值得临床应用。

Abstract：Objective To investigate the influence of Buchang Naoxintong Capsules combined with Nimodipine on neurotrophic status and neurological injury in patients with cerebral hemorrhage in convalescent stage. Methods A total of 176 patients with cerebral hemorrhage in convalescent period from January 2017 to January 2018 in our hospital were selected as the research objects. According to the principle of randomization, double blindness and control, they were divided into the observation group (88 cases) and the control group (88 cases). The observation group was treated with Buchang Naoxintong Capsules combined with Nimodipine, while the control group was treated with Nimodipine. The curative effect, serum neurotrophic status index (neurotrophic factor [NGF], brain-derived neurotrophic factor [BDNF],insulin-like growth factor-1 [IGF-1], insulin-like growth factor-2 [IGF-2]) and the degree of nerve injury were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group, with statistical significance (P ＜0.05). There was no significant difference in neurological deficit score(NHISS) between the two groups (P＞0.05). The NHISS score of the observation group after treatment was lower than that of the control group, with statistical significance (P＜0.05). There was no significant difference in the contents of NGF, BDNF, IGF-1 and IGF-2 between the two groups before treatment (P＞0.05). After treatment, the contents of NGF, BDNF, IGF-1 and IGF-2 in the observation group increased significantly and were higher than those in the control group (P＜0.05). Conclusion Buchang Naoxintong capsule combined with Nimodipine can not only improve the therapeutic effect to some extent, but also promote the improvement of neurotrophic status and reduce neurological deficits in convalescent patients with cerebral hemorrhage. The effect is remarkable and worthy of clinical application.
