综合老年评估护理干预模式对我国住院老年冠心病干预效果的Meta分析

摘要目的系统评价综合老年评估护理干预模式对我国住院老年冠心病干预的效果。方法检索中国知网、万方、维普、PubMed、CINAHL等国内外常用医学数据库，检索时间为建库到2018年5月，按照一定的纳入标准和排除标准选取文献，应用Review Manager 5.3统计软件对选取的文献进行Meta分析。结果共纳入17篇文献，共有1873例老年冠心病患者。Meta分析结果显示，实施综合老年评估护理干预模式患者的住院老年冠心病生活质量得分明显高于实施心内科常规护理，差异有统计学意义［MD=14.54，95%CI（13.60～15.48），P＜0.00001］；实施综合老年评估护理干预模式患者的住院不良事件发生率显著低于实施心内科常规护理，差异有统计学意义［相对危险度（RR）=0.39，95%CI（0.29～0.52），P＜0.00001］；实施综合老年评估护理干预模式患者的住院天数显著短于实施心内科常规护理，差异有统计学意义［MD=-4.26，95%CI（-4.94～-3.59），P＜0.00001］。结论综合老年评估护理干预模式可以提高住院老年冠心病患者的医疗服务效果，节省医疗卫生资源。

Abstract：Objective To systematically evaluate the effect of comprehensive geriatric assessment nursing intervention model on the elderly inpatients with coronary heart disease in China.Methods Relevant literatures of common medical database at home and abroad such as CHKI，Wanfang，Weipu，PubMed and CINAHL etc.were searched，and the search date was from database construction to May 2018.Documents were selected according to certain inclusion criteria and exclusion criteria and Meta analysis was performed using Review Manager 5.3 statistical software.Results A total of 17 articles were included which contained 1873 elderly inpatients with coronary heart disease.Meta analysis results showed that，the scores of life quality of hospitalized elderly patients with coronary heart disease who were given the comprehensive geriatric assessment nursing intervention were significantly higher than those of the implementation of cardiology routine nursing care，and the differences were statistically significant（MD=14.54，95%CI ［13.60-15.48］，P＜0.00001）.The incidence rate of adverse events of hospitalized elderly patients with coronary heart disease who were given the comprehensive geriatric assessment nursing intervention was significantly lower than that of the implementation of cardiology routine nursing care，and the differences were statistically significant（relative risk ［RR］=0.39，95%CI［0.29-0.52］，P＜0.00001）.The hospital stay of hospitalized elderly patients with coronary heart disease who were given the comprehensive geriatric assessment nursing intervention was significantly shorter than that of the implementation of cardiology routine nursing care，and the differences were statistically significant（MD=-4.26，95%CI ［-4.94--3.59］，P＜0.00001）.Conclusion The comprehensive geriatric assessment nursing intervention model can improve the medical service effect of hospitalized elderly patients with coronary heart disease and save medical and health resources.
