Abstract:Objective To explore the clinical characteristics of type 2 diabetes mellitus(T2DM)first diagnosed with acute pancreatitis(AP),and to improve the diagnosis and treatment of such patients.Methods A retrospective analysis was made of 14 patients with initial T2DM who were first diagnosed with AP in Yanqing District Hospital of Beijing from October 2012 to October 2018.20 patients with AP who were not combined with DM in the same period were selected as control group.The clinical characteristics of initial T2DM who were first diagnosed with AP were summarized.Results The body mass index(BMI)of the observation group was significantly higher than that of the control group,the difference was significant(P<0.05).The incidence of non-induced pancreatitis in the observation group was 0.00%,which was lower than 25.00%in the control group (P<0.05).There was no significant difference in the incidence of severe pancreatitis,cholelithiasis,severe hypertriglyceridemia(HTG),alcoholism and overeating between the two groups(P>0.05).The levels of triglyceride(TG),fasting blood glucose(FBG)and glycosylated hemoglobin(HbA1c)in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in total cholesterol(TC),serum amylase(AMY)and lipase(LIPA)levels between the two groups(P>0.05).In the observation group,6 patients(42.86%)had diabetic ketosis or diabetic ketoacidosis(DKA),6 patients were overweight(42.86%)and 5 patients were obese(35.71%).Conclusion Type 2 diabetes mellitus patients with AP are more likely to be overweight,complicated with HTG and cholelithiasis,complicated with acute metabolic disorders of diabetes mellitus,which can aggravate organ damage,and have a high probability of severe pancreatitis,which increases the difficulty of diagnosis and treatment.It should be paid attention to in clinical work.
