腹主动脉旁淋巴引流区预防性放疗对中晚期合并盆腔淋巴结阳性宫颈癌的疗效

摘要目的探讨腹主动脉旁淋巴引流区预防性放疗对中晚期合并盆腔淋巴结阳性宫颈癌的疗效。方法选取2015年2月～2017年10月江西省妇幼保健院收治的81 例中晚期合并盆腔淋巴结阳性宫颈癌患者作为研究对象，按照随机数表法分为对照组（40 例）与研究组（41 例），对照组采用以铂类为基础的化疗方案，研究组在此基础上实施腹主动脉旁淋巴引流区预防性放疗，比较两组疾病控制率、急性放化疗不良反应发生情况及生存率。结果研究组疾病控制率高于对照组，差异有统计学意义（P＜0.05）。两组急性放化疗不良反应总发生率比较，差异无统计学意义（P＞0.05）。研究组2、3年累计生存率高于对照组，差异有统计学意义（P＜0.05）。结论中晚期合并盆腔淋巴结阳性宫颈癌临床治疗中，给予腹主动脉旁淋巴引流区预防性放疗的临床疗效较好，安全性较高，值得推广。

Abstract：Objective To investigate the effect of prophylaxis radiotherapy in para-aortic lymphatic drainage area on middle and advanced cervical cancer with positive pelvic lymph nodes.Methods A total of 81 patients with advanced cervical cancer with pelvic lymph node positive admitted to Jiangxi maternal and child health hospital from February 2015 to October 2017 were selected as the research subjects.According to random number table method,they were divided into control group(40 cases)and study group(41 cases).The control group received platinum-based chemotherapy,and the study group received prophylactic radiotherapy in the para-aortic lymphatic drainage area on this basis.The disease control rate,the incidence of adverse reactions of acute radiotherapy and chemotherapy and the survival rate were compared between two groups.Results The disease control rate in the study group was higher than that in the control group,and the difference was statistically significant(P＜0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P＞0.05).The 2- and 3-year cumulative survival rate in the study group were higher than those in the control group,and the differences were statistically significant(P＜0.05).Conclusion In the clinical treatment of middle and advanced cervical cancer complicated with positive pelvic lymph nodes,prophylactic radiotherapy in the para-aortic lymphatic drainage area has good clinical efficacy and high safety,which is worthy of promotion.
