摘要目的 探讨七氟醚静吸复合麻醉对宫颈癌患者免疫功能、血流动力学的影响。方法 选取2017年4月~2020年4月大连金普新区妇幼保健院收治的54例宫颈癌患者作为研究对象,按照随机数字表法分为对照组(27例)与试验组(27例),对照组患者采用丙泊酚持续麻醉方法,试验组患者采用七氟醚静吸复合麻醉方法。比较两组患者的免疫功能水平、血清神经生长因子(NGF)、脑源性神经营养因子(BDNF)水平及不同时间段血流动力学指标。结果 两组术后48 h CD4+CD25+T、CD4+OX40+T 水平低于术前,差异有统计学意义(P<0.05);且试验组术后48 h 的CD4+CD25+T、CD4+OX40+T 水平低于对照组,差异有统计学意义(P<0.05)。两组术后48 h 的NGF、BDNF 水平低于术前,差异有统计学意义(P<0.05);且试验组术后48 h 的NGF、BDNF 水平高于对照组,差异有统计学意义(P<0.05)。两组麻醉前5 min 的心率(HR)、平均动脉压(MAP)、血氧饱和度(SpPO2)比较,差异无统计学意义(P>0.05);试验组麻醉后5 min、手术开始时的HR、MAP 低于对照组,SpO2 高于对照组,差异有统计学意义(P<0.05)。结论 七氟醚静吸复合麻醉用于宫颈癌手术,有助于减轻免疫反应,维持血流动力学稳定。
Abstract:Objective To analyze the effect of Sevoflurane intravenous inhalation combined anesthesia on the immune function and hemodynamics of patients with cervical cancer.Methods A total of 54 patients with cervical cancer admitted to Maternal and Child Health Hospital of Dalian Jinpu New District from April 2017 to April 2020 were selected as the research objects.According to the random number table method,they were divided into a control group(27 cases)and an experimental group (27 cases).Patients in the control group were treated with Propofol continuous anesthesia,and patients in the experimental group were treated with Sevoflurane intravenous inhalation combined anesthesia.The perioperative immune function levels,serum nerve growth factor (NGF),brain-derived neurotrophic factor (BDNF) levels and hemodynamic levels in different time periods were compared between the two groups of patients.Results The levels of CD4+CD25+T and CD4+OX40+T in the two groups at 48 hours after operation were lower than those before operation,the differences were statistically significant(P<0.05); and the levels of CD4+CD25+T and CD4+OX40+T in the experimental group at 48 hours after operation were lower than those in the control group,the differences were statistically significant (P<0.05).The levels of NGF and BDNF at 48 hours after operation were lower than those before operation,the differences were statistically significant (P<0.05); and the levels of NGF and BDNF in the experimental group at 48 hours after operation were higher than those in the control group,the differences were statistically significant (P<0.05).There were no significant differences in heart rate (HR),mean arterial pressure (MAP),blood oxygen saturation(SpO2) between the two groups at 5 minutes before anesthesia (P>0.05); at 5 minutes after anesthesia and the beginning of the operation,the HR and MAP of the experimental group were lower than those of the control group,and SpO2 was higher than that of the control group,the differences were statistically significant (P<0.05).Conclusion Sevoflurane intravenous inhalation combined anesthesia is used for the surgical treatment of cervical cancer,it helps to reduce immune response and maintain hemodynamic stability.
