拖线置管垫棉综合疗法治疗高位复杂性肛瘘的有效性和安全性评价

摘要目的评价拖线置管垫棉综合疗法治疗高位复杂性肛瘘的有效性和安全性。方法选取我院2016年10月～2017年9月收治的80例高位复杂性肛瘘患者，随机分为对照组（n=40）和观察组（n=40）。对照组给予切开挂线术治疗，观察组给予拖线置管垫棉综合疗法治疗。比较两组的临床治愈率、疼痛程度、创面愈合时间及肛门直肠压力，并观察复发率及不良反应。结果 ①两组的临床治愈率及复发率比较，差异无统计学意义（P＞0.05）；②观察组治疗后第10、14天的疼痛值明显低于对照组（P＜0.05）；③观察组创面愈合时间显著短于对照组（P＜0.01）；④两组愈合后3个月的20 mm收缩压与静息压较术前降低（P＜0.01），观察组愈合后3个月的20 mm收缩压与静息压较对照组均升高，差异均有统计学意义（P＜0.05）；⑤两组在治疗过程中均未发生不良反应。结论拖线置管垫棉综合疗法可有效、安全地治疗高位复杂性肛瘘。

Abstract：Objective To evaluate the efficacy and safety of the tube-padding combined therapy in the treatment of high complex anal fistula.Methods A total of 80 patients with high complex anal fistula in our hospital from October 2016 to September 2017 were selected and they were randomly divided into control group(n=40)and observation group(n=40).The control group was treated by incision-thread-drawing procedure,and the observation group was treated with the combined therapy of tubing and cotton.Clinical cure rate,pain level,wound healing time and anorectal pressure were compared,meanwhile recurrence rate and adverse reaction were observed.Results①There was no significant difference of the clinical cure rate and recurrence rate between the two groups(P＞0.05);②The pain value of the observation group was significantly lower than that of the control group on the 10 th and 14 th day after treatment(P＜0.05）;③The wound healing time in the observation group was significantly shorter than that of the control group (P＜0.01）;④The 20 mm systolic pressure and resting pressure of the two groups were significantly lower than before operation at 3 months after healing(P＜0.01）,the systolic and resting pressure of 20 mm in the observation group were significantly higher than those of the control group at 3 months after healing,the differences were statistically significant(P<0.05）.⑤There were no adverse reactions in both groups during the course of treatment.Conclusion The combined therapy of tubing and cotton with towed thread can be effective and safe in the treatment of high complex anal fistula.
