子宫颈扩张球囊与缩宫素在足月妊娠促宫颈成熟及引产的应用观察

摘要目的比较子宫颈扩张球囊与缩宫素在足月妊娠促宫颈成熟及引产的效果与安全性。方法选择2016年1月～2017年6月我院收治的200例有引产指征的产妇作为研究对象，按引产方法不同将产妇随机分为观察组和对照组各100例。对照组产妇静脉点滴缩宫素引产，观察组使用COOK子宫颈扩张球囊引产，干预前后进行宫颈Bishop评分，评判两组促宫颈成熟效果，比较分娩情况及并发症发生的差异。结果两组干预后宫颈Bishop评分均较干预前明显升高（P＜0.05），观察组干预后宫颈Bishop评分明显高于对照组，差异有统计学意义（P＜0.05）。观察组促宫颈成熟总有效率及引产成功率明显高于对照组（P＜0.05）。观察组临产时间、总产程均明显短于对照组（P＜0.05），阴道分娩率明显高于对照组（P＜0.05），剖宫产率明显低于对照组（P＜0.05），并发症总发生率明显低于对照组，差异均有统计学意义（P＜0.05）。结论子宫颈扩张球囊是一种安全、有效、简便的促宫颈成熟与引产的方法，促宫颈成熟与引产的效果优于药物，且显著降低剖宫产率及并发症发生率，保证母婴安全。

Abstract：Objective To compare the efficacy and safety of of cook cervical ripening double balloon and Oxytocin for labor induction and cervical ripening in term gestational women.Methods 200 term pregnant women with induced labor indications from January 2016 to June 2017 in our hospital treated were selected as the research object,they were randomly divided into the observation group and the control group according to different abortion methods,100 cases in each group.The control group were received intravenous Oxytocin to promote cervical ripening and labor induction,the observation group were used cook balloon catheter for cervical ripening and labor induction,cervix bishop score were assessed before and after intervention,the effect of cervical ripening were evaluated,the delivery and complications were observed.Results The cervix Bishop score of the observation group and the control group after treatment were significantly that higher than those before intervention (P＜0.05),and after intervention,the cervix Bishop score of the observation group was higher than the control group,the difference was statistically significant(P＜0.05).The effect of cervical ripening and labor induction in the observation group was significantly higher than that the control group(P＜0.05).The labor time and production labor in the observation group were significantly shorter than those in the control group(P＜0.05)，the Vaginal delivery rate was significantly higher than that the control group(P＜0.05),the cesarean section rate was significantly lower than that the control group (P＜0.05)，the overall complication rate was significantly lower than the control group,the differences were statistically significant(P＜0.05).Conclusion The cook cervical ripening double balloon is safe,effective and simple method for cervical ripening and labor induction,The effect of cervical ripening and labor induction is better than that of drugs,and significantly reduce the incidence of cesarean section and complications,and ensure that mother and baby.
