发展性照顾在早产儿护理中的应用

摘要目的探究发展性照顾在早产儿护理中的应用效果。方法选取2016年8月～2017年2月我院新生儿科收治的124例早产儿为研究对象，根据出生顺序平均分为观察组和对照组，每组各62例。于纠正胎龄40周后的第12～14、26～28天行新生儿神经行为测试（NBNA），比较两组患儿日摄奶量、睡眠时间、日体重增长、恢复正常体质量时间、住院时间及不良反应发生率。结果观察组患儿NBNA评分、日摄奶量、日睡眠时间、日体质量增长均高于对照组，恢复正常体质量时间早于对照组，总住院时间短于对照组，差异均有统计学意义（P＜0.05）。出生1个月后，观察组患儿体质量、身长和头围均大于对照组，差异有统计学意义（P＜0.05）。观察组患儿不良反应发生率为8.1%（5/62），其中医院感染1例、喂养不耐受4例，对照组患儿不良反应发生率为19.4%（15/62），其中医院感染2例、神经行为障碍3例、喂养不耐受10例，差异有统计学意义（χ2=12.367，P＜0.05）。结论发展性照顾能够有效提高早产儿摄奶量、延长睡眠时间、加速体质量恢复，进而促进神经及生长发育，减少患儿不良反应发生率，缩短住院时间，值得在早产儿的护理中推广应用。

Abstract：ObjectiveTo explore the application of developmental care in the nursing of preterm infants.Methods124 premature infants admitted to our department from August 2016 to February 2017 were selected as the research objects.They were divided into observation group and control group according to birth order,62 cases in each group.In twelfth-40 weeks after correcting gestational age of 14 days,26-28 days of neonatal neurobehavioral test(NBNA),the incidence rate of two groups were compared on the intake of milk,sleep time,daily weight growth and quality of recovery time,hospitalization time and normal body adverse reaction.ResultsThe NBNA score，daily milk intake，daily sleep time and daily weight gain of the observation group were all higher than those of the control group.The time of returning to normal body weight was earlier than that of the control group,and the total hospitalization time was shorter than that of the control group.The differences between the two groups were statistically significant(P＜0.05).1 months after the birth,the observation group with body weight,length and head circumference were higher than the control group,the differences were statistically significant(P＜0.05).The observation group with adverse reaction incidence rate was 8.1% （5/62）,of which 1 cases of hospital infection and feeding intolerance in 4 cases of adverse reactions in the control group,the incidence rate was 19.4% （15/62）,the hospital infection in 2 cases,3 cases of neurobehavioral disorders 10 cases,feeding intolerance,the difference between the two groups was statistically significant(χ2=12.367,P＜0.05).ConclusionDevelopmental care can effectively improve the milk intake,prolong sleep time and speed up the recovery of body weight,thereby promoting nerve growth and development,reducing the incidence of adverse reactions and shortening the length of stay.It is worthy of popularization and application in the nursing of premature infants.
