Changes of interleukin-1β and tumor necrosis factor-α in serum of patients with intestinal obstruction before and after operation

摘要目的 观察白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)在手术肠梗阻患者中的变化并探讨其意义。方法 选取2016年8月~2018年8月于甘肃省陇南市第一人民医院住院的行手术治疗的40 例肠梗阻患者作为研究对象,按照术中是否有肠坏死情况分为两组:单纯性肠梗阻组18 例,绞窄性肠梗阻组22 例。收集患者入院时及术后3、7 d 清晨空腹外周静脉血,采用酶联免疫吸附测定(ELISA)检测血清中IL-1β、TNF-α 的含量。结果 两组患者的IL-1β 在入院时、术后3 d 及术后7 d 三个时间点比较,差异有统计学意义(P<0.05);单纯性肠梗阻组及绞窄性肠梗阻组的IL-1β 在每个时间点比较,差异有统计学意义(P<0.05);组别与时间的交互作用比较,差异有统计学意义(P<0.05)。两组患者的TNF-α 在入院时、术后3 d 及术后7 d 三个时间点比较,差异有统计学意义(P<0.05);单纯性肠梗阻组及绞窄性肠梗阻组的TNF-α 在每个时间点比较,差异有统计学意义(P<0.05);组别与时间的交互作用比较,差异有统计学意义(P<0.05)。结论 肠梗阻患者血清中的IL-1β、TNF-α 随着疾病的转归而下降,动态监测术后的IL-1β、TNF-α 有助于了解肠梗阻患者的转归,并提示可能是肠梗阻发生绞窄的早期炎症细胞因子。
Abstract:Objective To observe the changes of interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in patients with intestinal obstruction treated by operation and to explore the clinical significance.Methods A total of 40 patients with intestinal obstruction who were hospitalized in the First People′s Hospital of Longnan City,Gansu Province from August 2016 to August 2018 were selected as the research objects.According to whether there was intestinal necrosis during the operation,they were divided into two groups: simple intestinal obstruction group(18 cases)and strangulated intestinal obstruction group(22 cases).The fasting peripheral venous blood was collected at the time of admission and 3 and 7 days after the operation,and the serum levels of IL-1β and TNF-α were detected by enzymelinked immunosorbent assay(ELISA).Results The levels of IL-1β between the two groups were compared at the time of admission,3 d after operation,and 7 d after operation,the differences were statistically significant(P<0.05).The comparison of IL-1β between the simple intestinal obstruction group and the strangulated intestinal obstruction group at each time point was statistically significant(P<0.05).There was a statistically significant difference in the interaction between group and time in IL-1β(P<0.05).The levels of TNF-α between the two groups were compared at the time of admission,3 d after operation,and 7 d after operation,the differences were statistically significant(P<0.05).The comparison of TNF-α between the simple intestinal obstruction group and the strangulated intestinal obstruction group at each time point was statistically significant(P<0.05).There was a statistically significant difference in the interaction between group and time in TNF-α(P<0.05).Conclusion Serum IL-1β and TNF-α in patients with intestinal obstruction decrease with the disease progression.Dynamic monitoring of postoperative IL-1β and TNF-α helps to understand the outcome of patients with intestinal obstruction and suggests that they may be intestinal early inflammatory cytokines in strangulation intestinal obstruction.
