Clinical effect of operation under the guidance of theory of damage control in the treatment of pelvic fracture combined with skin and soft tissue injury

Abstract:Objective To study the clinical effects of operation under the guidance of theory of damage control (TDC)in the treatment of pelvic fracture combined with skin and soft tissue injury.Methods The clinical data of 350 cases of pelvic fracture combined with skin and soft tissue injury in our hospital from February 2016 to December 2018 were analyzed retrospectively.According to different treatment methods,the patients were divided into control group(175 cases)and observation group (175 cases).The patients in the control group were treated by conventional operation, while those in the observation group were treated by operation under the guidance of TDC.The operation time, intraoperative bleeding volume, total hospitalization time, treatment effect and complications between the two groups were compared.Results The operation time and hospitalization time of the patients in the observation group were shorter than those in the control group, and the amount of bleeding in the operation was less than that in the control group, the differences were statistically significant (P<0.05).The excellent and good rate of treatment in the observation group was 84.4%,higher than 53.7% in the control group, the difference was statistically significant (P<0.05).The total incidence of complications in the observation group was 4.0%, lower than 35.4% in the control group, the difference was statistically significant (P<0.05).Conclusion Under the guidance of TDC, the operation for pelvic fracture with systemic skin and soft tissue injury has a significant effect.Compared with conventional operation, it can significantly shorten the operation time, reduce the amount of intraoperative bleeding, shorten the length of stay and reduce complications, which is worth popularizing and applying in clinical practice.
