高通量血液透析治疗尿毒症的临床效果

摘要目的观察高通量血液透析治疗尿毒症的临床效果。方法选取2016年12月～2019年2月我院收治的118例尿毒症患者作为研究对象，按照随机抽样的方式将其分为参照组（59 例）与研讨组（59 例）。参照组患者采用常规血液透析治疗，研讨组患者采用高通量血液透析治疗。比较两组患者的治疗总有效率及治疗前后肾功能指标（C反应蛋白、三酰甘油、肌酐、尿微量蛋白、尿素氮）、免疫球蛋白（IgA、IgM、IgG）水平。结果研讨组患者的治疗总有效率为93.22%，高于参照组的64.41%，差异有统计学意义（P＜0.05）。两组患者治疗前的肾功能指标水平比较，差异无统计学意义（P＞0.05）；治疗后两组患者的肾功能指标水平均低于治疗前，差异有统计学意义（P＜0.05）；研讨组患者治疗后的肾功能指标水平低于参照组，差异有统计学意义（P＜0.05）。两组患者治疗前的免疫球蛋白水平比较，差异无统计学意义（P＞0.05）；治疗后两组患者的免疫球蛋白水平均高于治疗前，差异有统计学意义（P＜0.05）；研讨组患者治疗后的免疫球蛋白水平高于参照组，差异有统计学意义（P＜0.05）。结论高通量血液透析应用于治疗尿毒症患者的效果相对于常规血液透析效果更优，各项临床症状得以明显改善。

Abstract：Objective To observe the clinical effect of high-flux hemodialysis in the treatment of uremia.Methods A total of 118 patients with uremia admitted to our hospital from December 2016 to February 2019 were enrolled in the study.According to the random sampling method,they were divided into reference group (59 cases) and study group(59 cases).Patients in the reference group were treated with conventional hemodialysis,and patients in the study group were treated with high-flux hemodialysis.The total effective rate of treatment and renal function indexes (C-reactive protein,triacylglycerol,creatinine,urine microprotein,urea nitrogen),and immunoglobulin (IgA,IgM,IgG) levels of the two groups before and after treatment were compared.Results The total effective rate of treatment in the study group was 93.22%,which was higher than 64.41% in the reference group,and the difference was statistically significant (P＜0.05).There were no significant differences in the levels of renal function indexes before treatment between the two groups (P＞0.05).After treatment,the levels of renal function indexes of the two groups were lower than those before treatment,and the differences were statistically significant (P＜0.05).The levels of renal function indexes of patients in the study group after treatment were lower than those in the reference group,and the differences were statistically significant (P＜0.05).There were no significant difference in the immunoglobulin levels before treatment between the two groups (P＞0.05).After treatment,the levels of immunoglobulin of the two groups were higher than those before treatment,and the differences were statistically significant (P＜0.05).The levels of immunoglobulin of patients in the study group after treatment were higher than those in the reference group,and the differences were statistically significant (P＜0.05).Conclusion The effect of high-flux hemodialysis in the treatment of uremia patients is better than that of conventional hemodialysis,and the clinical symptoms are significantly improved.
