早期护理在子痫期间伴血栓形成倾向孕妇中的应用效果

摘要目的探讨早期护理在子痫期间伴血栓形成倾向孕妇中的应用效果。方法选取2015年1月～2017年12月我院收治的68例子痫期间伴血栓形成倾向孕妇作为研究对象，采用随机分配原则将其分为两组，每组各34例。对照组给予常规护理，观察组给予早期护理干预，对两组的凝血指标、血栓标志物、血小板活化指标及并发症发生率进行比较分析。结果两组均顺利生产，无母婴死亡。护理后，观察组的凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）均明显长于对照组，观察组的纤维蛋白原（FBG）明显低于对照组，差异有统计学意义（P＜0.05），两组的凝血酶时间（TT）比较，差异无统计学意义（P＞0.05）；护理后，观察组的纤溶酶原活性（PLG）、抗凝血酶活性（AT）均明显高于对照组，差异有统计学意义（P＜0.05），观察组的D二聚体（D-D）明显低于对照组，差异有统计学意义（P＜0.05）；护理后，观察组的溶酶体膜糖蛋白（CD63）、血小板α颗粒膜糖蛋白（CD62p）均明显低于对照组，差异有统计学意义（P＜0.05）；观察组的产后并发症总发生率明显低于对照组，差异有统计学意义（P＜0.05）。结论对子痫期间伴血栓形成倾向孕妇实施早期护理，可优化患者的各项凝血指标、血栓标志物和血小板活化指标，产后并发症发生率低，值得在临床上进一步推广应用。

Abstract：Objective To explore the application effect of early nursing on pregnant women with eclampsia and thrombosis tendency.Methods A total of 68 pregnant women with thrombophilia who admitted to our hospital from January 2015 to December 2017 were enrolled in this study.The patients were divided into two groups,34 patients each group.In control group,routine nursing was used,while in observation group,early nursing intervention was adopted.The coagulation index,thrombus marker,platelet activation index and complication rate of the two groups were compared and analyzed.Results Participants were all delivered successfully without maternal and child death.After nursing,the prothrombin time(PT)and activated partial thromboplastin time(APTT)of the observation group were significantly longer compared with those in the control group.The fibrinogen (FBG)level of the observation group was significantly lower than that of the control group with statistical significance(P＜0.05).There was no significant difference in thrombin time(TT)between the two groups(P＞0.05).After nursing,the plasminogen activity(PLG)and antithrombin activity(AT)of the observation group were both significantly higher than those of the control group with significant differences (P＜0.05).The D-dimer(D-D)level of the observation group was much lower than that of the control group,with significant difference(P＜0.05).After nursing,lysosomal membrane glycoprotein(CD63)and platelet alpha granule membrane glycoprotein (CD62p)were significantly lower in the observation group than that in the control group (P＜0.05).The total incidence of postpartum complications in the observation group was significantly lower than that in the control group(P＜0.05).Conclusion Early care for pregnant women with eclampsia and thrombosis tendency can optimize the patient′s coagulation indexes,thrombus markers and platelet activation indexes in a low incidence of postpartum complications,which is worthy of further clinical application.
