急性重症胰腺炎早期行腹腔穿刺置管引流的价值分析

摘要目的分析急性重症胰腺炎（SAP）早期行腹腔穿刺置管引流（APD）的价值。方法选取2018 年1 月至2020年11 月宜春市人民医院收治的94 例SAP 患者作为研究对象，采用随机数字表法分为对照组（47 例）和观察组（47 例）。对照组采用常规治疗方案，观察组在对照组基础上早期行APD。比较两组患者ICU 住院时间、机械通气时间和实验室检查指标水平。结果观察组的ICU 住院时间和机械通气时间均短于对照组，差异有统计学意义（P＜0.05）；治疗后，观察组的血糖、降钙素原（PCT）、动脉血乳酸水平均低于对照组，C 反应蛋白（CRP）高于对照组，差异有统计学意义（P＜0.05）；治疗后，观察组的血钙水平高于对照组，差异有统计学意义（P＜0.05）。结论早期行APD 可显著缩短SAP 患者的ICU 住院时间和机械通气时间，改善实验室检查指标水平。

Abstract：Objective To analyze the application value of early abdominal paracentesis drainage (APD) in severe acute pancreatitis (SAP). Methods A total of 94 patients with SAP admitted to Yichun People′s Hospital between January 2018 and November 2020 were enrolled and divided into control group (47 cases) and observation group (47 cases) by simple number table method. The control group was given routine treatment regimens, the observation group underwent early APD on basis of control group. The stay time in ICU, mechanical ventilation time and levels of laboratory test indexes were compared between the two groups. Results The length of ICU hospitalization and mechanical ventilation in the observation group were shorter than those in the control group, and the differences were statistically significant (P＜0.05).After treatment,the observation group′s blood glucose, procalcitonin (PCT), the arterial blood lactic acid level were lower than those of the control group, C-reactive protein (CRP) was higher than that of the control group, the differences were statistically significant (P＜0.05). After treatment, the blood calcium level of the observation group was higher than that of the control group, the difference was statistically significant (P＜0.05). Conclusion Early APD can significantly shorten stay time in ICU and mechanical ventilation time in SAP patients, and improve levels of laboratory test indexes.
