Clinical effect of laparoscopic minimally invasive surgery in the treatment of acute appendicitis and its influence on stress hormones and inflammatory factors
Abstract: Objective To investigate the clinical effect of laparoscopic minimally invasive surgery in the treatment of acute appendicitis and its influence on stress hormones and inflammatory factors. Methods A retrospective study of 87 patients with acute appendicitis admitted to Wuhan Seventh Hospital from June 2019 to June 2020, they were divided into laparoscopic group (45 cases) and open group (42 cases) according to the operation methods. The laparoscopic group received laparoscopic appendectomy, while the control group received open appendectomy. The perioperative indexes and postoperative complications, gastrointestinal function [gastrin (MTL) and motilin (gas)], cortisol (Cor), interleukin-6 (IL-6) and C-reactive protein (CRP) levels were compared between the two groups. Results There was no significant difference in operation time between the two groups (P>0.05). The anal exhaust time, ambulation time and postoperative hospital stay of laparoscopic group were shorter than those of open group, the differences were statistically significant (P<0.05). The serum levels of MLT and GAS at 3 days after operation in two groups were lower than before operation, the differences were statistically significant (P<0.05). The total complication rate of laparoscopic group was lower than those of open group, the difference was statistically significant (P<0.05). The levels of serum MLT and gas in laparoscopic group were higher than those in open group, the differences were statistically significant (P<0.05). The serum levels of Cor, IL-6 and CRP in the two groups were higher than those before operation, the differences were statistically significant (P<0.05). The serum Cor, IL-6 and CRP of laparoscopic group were lower than those of open group 3 days after operation, the differences were statistically significant (P<0.05). Conclusion Laparoscopic appendectomy has less postoperative complications in patients with acute appendicitis, can promote the recovery of gastrointestinal function faster, and has little impact on stress-induced inflammatory response.
