MDT护理干预在老年腹腔镜下胆总管切开取石术患者中的应用价值

摘要目的探讨多学科协作诊疗（MDT）护理干预在老年腹腔镜胆总管切开取石术患者的应用价值。方法选取2019年10月至2020年10月宜春市人民医院收治的84 例老年腹腔镜胆总管切开取石术患者作为研究对象，采用随机数字表法分为对照组（42 例）和观察组（42 例）。对照组采用常规护理，观察组采用MDT护理干预。比较两组的胃肠功能恢复情况、疼痛程度、生活质量、护理满意度及术后并发症总发生情况。结果观察组的术后进食、首次肛门排气、首次排便及肠鸣音恢复时间短于对照组，差异均有统计学意义（P＜0.05）。干预前，两组的视觉模拟评分法（VAS）、生活质量调查问卷（QOL-30）评分比较，差异无统计学意义（P＞0.05）；干预后，观察组的VAS 评分低于对照组，QOL-30 评分高于对照组，差异均有统计学意义（P＜0.05）。观察组的护理总满意度高于对照组，观察组的术后并发症总发生率低于对照组，差异均有统计学意义（P＜0.05）。结论 MDT护理干预应用于老年腹腔镜胆总管切开取石术患者可促进其胃肠功能恢复，降低术后并发症发生，改善生活质量水平，提高护理满意度。

Abstract：Objective To explore the application value of multi-disciplinary team(MDT)nursing intervention in elderly patients undergoing laparoscopic common bile duct incision and stone removal.Methods A total of 84 elderly patients undergoing laparoscopic choledochectomy and lithotripsy who were admitted to Yichun People′s Hospital from October 2019 to October 2020 were selected as the research subjects.They were divided into the control group(42 cases)and the observation group(42 cases)by using random number table.Patients in the control group received routine nursing care,and patients in the observation group used MDT nursing intervention.The recovery of gastrointestinal function,pain degree,quality of life,nursing satisfaction and total occurrence of postoperative complications were compared between the two groups.Results The observation group′s postoperative food intake,first anal exhaust,first defecation,and bowel sound recovery time were shorter than those of the control group,the differences were statistically significant(P＜0.05).Before the intervention,the visual analogue scoring(VAS),the quality of life questionnaire(QOL-30)score comparison showed no significant differences(P＞0.05).After the intervention,the VAS score of the observation group was lower than that of the control group,and the QOL-30 score was higher than that of the control group,the differences were statistically significant(P＜0.05).The total nursing satisfaction of the observation group was higher than that of the control group,the total incidence of postoperative complications in the observation group was lower than that of the control group,the differences were statistically significant(P＜0.05).Conclusion The application of MDT nursing intervention in elderly patients undergoing laparoscopic common bile duct incision and stone removal can promote the recovery of gastrointestinal function,reduce postoperative complications,improve the quality of life,and increase nursing satisfaction.
