阿司匹林与氯吡格雷联合治疗急性心肌梗死患者的效果及对患者超敏C反应蛋白的影响

摘要目的探讨阿司匹林联合氯吡格雷在急性心肌梗死患者中的效果及对患者超敏C 反应蛋白（hs-CRP）水平的影响。方法选取2017年3月—2018年7月沈阳市医学院附属中心医院收治的92 例急性心肌梗死患者为研究对象，按照随机数字表法将其分为观察组和对照组，每组各46 例。两组入院后30 min 给予静脉泵入尿激酶治疗，对照组采用阿司匹林治疗，观察组在对照组基础上联合氯吡格雷治疗，治疗后对患者进行3 个月的随访。比较两组患者的凝血功能、hs-CRP 水平及心血管事件发生率。结果观察组患者治疗后1 个月的凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）均长于对照组，纤维蛋白原（FIB）水平低于对照组（P<0.05）；观察组患者治疗后1、2、3 个月的hs-CRP 水平均低于对照组（P<0.05）；观察组患者治疗期间的心肌梗死、心绞痛、心力衰竭、心律失常发生率及死亡率均低于对照组（P<0.05）。结论对急性心肌梗死患者采用阿司匹林联合氯吡格雷治疗，能显著改善患者的凝血功能，并显著降低hs-CRP 水平，使心血管事件发生风险下降，值得推广应用。

Abstract：Objective To investigate the effect of Aspirin combined with Clopidogrel in the treatment of patients with acute myocardial infarction and its influence on the level of hypersensitive C-reactive protein(hs-CRP).Methods Ninety-two patients with acute myocardial infarction admitted to the Central Hospital Affiliated to Shenyang Medical College from March 2017 to July 2018 were selected as the research subjects,and they were divided into observation group and control group according to random number table method,with 46 patients in each group.The two groups were treated with intravenous pumping Urokinase 30 minutes after admission,the control group was treated with Aspirin,and the observation group was combined with Clopidogrel on the basis of the control group.After treatment,the patients were followed up for 3 months.The blood coagulation function,hs-CRP levels and the incidence of cardiovascular events in the two groups were compared.Results The prothrombin time(PT)and activated partial thromboplastin time(APTT)of the observation group at 1 month after treatment were longer than those of the control group,and the fibrinogen(FIB)level was lower than that of the control group(P<0.05); the hs-CRP levels of the observation group at 1,2 and 3 months after treatment were lower than those of the control group(P<0.05); the incidence rates of myocardial infarction,angina pectoris,heart failure,arrhythmia and mortality in the observation group were lower than those in the control group(P<0.05).Conclusion Aspirin combined with Clopidogrel in the treatment of patients with acute myocardial infarction can significantly improve the coagulation function,significantly reduce the level of hs-CRP,and reduce the risk of cardiovascular events,which is worthy of popularization and application.
