日间手术结合加速康复外科理念在腹腔镜胆囊切除术中的应用效果

摘要目的探讨日间手术结合加速康复外科（ERAS）理念在腹腔镜胆囊切除术中的应用效果。方法回顾性分析2020年1月至2021年1月贵州医科大学附属医院肝胆外科收治的988例腹腔镜胆囊切除术（LC）患者作为研究对象，采用手术方法差异将其分为日间手术组（n=514）和常规手术组（n=474）。日间手术组患者采用日间手术结合ERAS方案，常规手术组患者采用常规方案。比较两组患者手术时间、术中出血量、疼痛评分、住院费用、引流管放置时间以及对手术效果的满意度和术后并发症总发生率。结果两组患者的手术时间、术中出血量比较，差异无统计学意义（P＞0.05）；日间手术组患者疼痛评分、住院费用低于常规手术组，差异有统计学意义（P＜0.05）；日间手术组患者引流管放置时间短于常规手术组，差异有统计学意义（P＜0.05）。日间手术组患者对手术方案的总满意度高于对照组，差异有统计学意义（P＜0.05），两组患者术后并发症总发生率比较，差异无统计学意义（P＜0.05）。结论日间手术结合ERAS理念在LC手术中的应用安全有效，可降低LC术患者的住院费用，减轻术后疼痛，提高了患者对手术效果的总满意度，值得大力推广。

Abstract：objective To explore the application effect of day surgery combined with enhanced recovery aftersurgery(ERAS)concept in laparoscopic cholecystectomy.Methods The clinical data of 988 patients undergoing laparoscopic cholecystectomy (LC)in Department of Hepatobiliary Surgery,the Affiliated Hospital of Guizhou Medical University from January 2020 to January 2021 were retrospectively analyzed,they were divided into day surgery group (n=514)and conventional surgery group (n=474)by differences in surgical methods.Patients in the day surgery group were treated with day surgery combined with ERAS,and patients in the conventional surgery group were treated with the conventional plan.The operation time,intraoperative blood loss,pain score,hospitalization cost,drainage tube placement time,satisfaction with operation effect and total incidence of postoperative complications were compared between the two groups.Results There was no significant difference in operation time and intraoperative blood loss between the two groups of patients(P＞0.05);the pain score and hospitalization cost of patients in the day surgery group were lower than those in the conventional surgery group,and the differences were statistically significant(P＜0.05).The drainage tube placement time in the day surgery group was shorter than that in the conventional surgery group,and the difference was statistically significant(P＜0.05).The total satisfaction of patients in the day surgery group was higher than that of the control group,and the difference was statistically significant(P＜0.05).There was no statistically significant difference in the total incidence of postoperative complications between the two groups (P＜0.05).Conclusions Day surgery combined with ERAS concept can effectively save hospital costs and alleviate postoperative pain for patients undergoing laparoscopic cholecystectomy surgery,improved patients′overall satisfaction with the results of surgery,worthy of vigorous promotion.
