背部膀胱经叩击联合穴位按摩预防老年卧床患者肺部感染的效果

摘要目的观察老年卧床患者肺部感染预防中背部膀胱经叩击联合穴位按摩的效果。方法选取2019年10月—2020年10月赣州市中医院收治的100 例老年卧床患者作为研究对象，依据随机数字表法将患者分为实验组和对照组，每组各50 例。两组患者均实施常规预防肺部感染措施，在此基础上，对照组采取背部膀胱经叩击，实验组采取背部膀胱经叩击联合穴位按摩。统计分析两组患者的肺部感染发生情况；比较两组患者治疗前后的肺功能、血气分析指标；比较两组肺部感染患者治疗结束后1 周内排痰量、体温、血常规恢复正常时间。结果实验组患者的肺部感染发生率为18.0%（9/50），低于对照组的42.0%（21/50），差异有统计学意义（P＜0.05）。治疗后，实验组患者的第1 秒用力呼气容积（FEV1）、用力肺活量（FVC）、呼气峰值流速（PEF）、每分钟通气量（MMF）均高于对照组，差异有统计学意义（P＜0.05）。两组患者治疗前后的动脉血氧饱和度（SaO2）比较，差异无统计学意义（P＞0.05）。治疗后，实验组患者的氧分压（PaO2）高于对照组，二氧化碳分压（PaCO2）低于对照组，差异有统计学意义（P＜0.05）。实验组肺部感染患者治疗结束后1 周内排痰量多于对照组，体温恢复正常时间和血常规恢复正常时间均短于对照组，差异有统计学意义（P＜0.05）。结论老年卧床患者肺部感染预防中背部膀胱经叩击联合穴位按摩的效果好。

Abstract：Objective To observe the effect of back bladder percussion combined with acupoint massage in the prevention of pulmonary infection in elderly bedridden patients. Methods A total of 100 elderly bedridden patients admitted to Ganzhou Hospital of traditional Chinese Medicine from October 2019 to October 2020 were selected as the research objects. The patients were divided into experimental group and control group according to random number table method, with 50 cases in each group. Both groups were given routine measures to prevent pulmonary infection. On this basis, the control group was treated with back bladder meridian percussion, while the experimental group was treated with back bladder meridian percussion combined with acupoint massage. The incidence of pulmonary infection in two groups was statistically analyzed. Lung function and blood gas analysis indexes were compared between the two groups before and after treatment. The sputum output within 1 week after treatment, body temperature and normal time of blood routine were compared among the patients with pulmonary infection of the two groups. Results The incidence of pulmonary infection in the experimental group was 18.0% (9/50), lower than 42.0% (21/50) in the control group, and the difference was statistically significant (P＜0.05). After treatment, the forced expiratory volume in the first second(FEV1), forced vital capacity (FVC), peak expiratory flow rate (PEF) and ventilated volume per minute (MMF) in experimental group were higher than those in control group, with statistically significant differences (P＜0.05). There was no significant difference in arterial oxygen saturation (SaO2) between the two groups before and after treatment (P＞0.05).After treatment, the partial pressure of oxygen (PaO2) in the experimental group was higher than that in the control group, and the partial pressure of carbon dioxide (PaCO2) in the experimental group was lower than that in the control group, with statistically significant differences (P＜0.05). The sputum output within 1 week after treatment of patients with pulmonary infection in the experimental group was more than that in the control group, and the time for the body temperature to return to normal and blood routine return to normal time in the experimental group were shorter than those in the control group, with statistically significant differences (P＜0.05). Conclusion The effect of back bladder percussion combined with acupoint massage in the prevention of pulmonary infection in elderly bedridden patients is good.
