硬通道穿刺引流术及术中冲洗治疗中老年慢性硬膜下血肿的效果观察

摘要目的探讨硬通道穿刺引流术及术中冲洗治疗中老年慢性硬膜下血肿（CSDH）的临床效果。方法选取上饶市立医院2018年1月1日～2020年1月10日收治的60 例中老年CSDH 患者作为研究对象，按照治疗方法不同分为对照组与观察组，每组各30 例。对照组采用传统钻孔引流术，观察组采用硬通道穿刺引流术及术中冲洗。比较两组的临床治疗总有效率、临床相关指标及并发症发生情况与复发率。结果两组的临床治疗总有效率比较，差异无统计学意义（P＞0.05）；观察组的并发症总发生率低于对照组，差异有统计学意义（P＜0.05）；两组的复发率比较，差异无统计学意义（P＞0.05）；观察组的手术时间、引流时间、住院时间均短于对照组，差异有统计学意义（P＜0.05）。结论中老年CSDH 患者采用硬通道穿刺引流术及术中冲洗治疗的临床治疗有效率较高，有助于缩短手术时间、引流时间、住院时间，且并发症发生率与复发率均较低。

Abstract：Objective To investigate the clinical effect of hard channel puncture drainage and intraoperative irrigation in the treatment of chronic subdural hematoma (CSDH) in middle-aged and elderly.Methods A total of 60 middleaged and elderly patients with CSDH admitted to Shangrao Municipal Hospital from January 1, 2018 to January 10,2020 were selected as the research objects.According to different treatment methods, they were divided into the control group and the observation group, 30 cases in each group.The control group used traditional drilling and drainage, and the observation group used hard channel puncture and drainage and intraoperative irrigation.The total clinical treatment efficiency, clinical related indicators, the incidence of complications and the recurrence rate were compared between the two groups.Results There was no significant difference in the total clinical treatment effective rate between the two groups (P＞0.05).The total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P＜0.05).There was no significant difference in the recurrence rate between the two groups (P＞0.05).The operation time, drainage time, and hospitalization time of the observation group were shorter than those of the control group, and the differences were statistically significant (P＜0.05).Conclusion The clinical effective rate of hard channel puncture and drainage and intraoperative irrigation in middle-aged and elderly patients with CSDH is higher, which helps to shorten the operation time, drainage time, hospitalization time, and the incidence of complications and recurrence rate are lower.
