临床药师对骨科住院患者辅助用药合理使用的干预效果分析

摘要目的分析临床药师对骨科住院患者辅助用药合理使用的干预效果，促进我院骨科住院患者辅助用药的合理使用。方法选取我院骨科2017年1～6月（干预前）的住院病历，对其中的辅助用药医嘱进行专项点评，将发现的典型问题进行归纳总结，采取措施进行干预。收集2017年7～12月（干预后）的骨科辅助用药住院医嘱进行点评，以辅助用药合理使用率、适应证合理率、用法用量合理率、溶媒合理率、疗程合理率、联合用药合理率作为考察指标，分析干预前后上述指标的变化，分析是否存在统计学意义，以此评估干预效果。结果干预前骨科使用辅助用药的住院病历共489份，干预后骨科使用辅助用药的病历共492份，干预后的用药合理使用率（85.37%）、适应证合理率（93.09%）、用法用量合理率（95.93%）、溶媒合理率（93.90%）、疗程合理率（89.84%）、联合用药合理率（94.30%）均分别高于干预前的41.72%、81.60%、83.23%、85.89%、83.64%、85.89%，差异有统计学意义（P＜0.05）。结论通过临床药师干预后，我院骨科住院患者辅助用药不合理使用情况得到明显改善，减少了临床用药风险及患者经济负担，用药更加合理化。

Abstract：Objective To analyze the interaction effect of clinical pharmacists on rational use of adjuvant drugs in orthopedic inpatients,and to promote the rational use of adjuvant drugs for orthopedic inpatients in our hospital.Methods The medical records of the orthopaedics department of our hospital from January to June 2017 (before the intervention) were selected,and the auxiliary medication orders were specially reviewed.The typical problems found were summarized and measures were taken to intervene.The hospital orders of orthopaedic auxiliary drugs from July to December 2017 (after the intervention) were collected and reviewed.The reasonable utilization rate of auxiliary drugs,the reasonable rate of indications,the reasonable rate of usage and dosage,the reasonable rate of solvent,the reasonable rate of course of treatment and the reasonable rate of combined drugs were used as inspection indicators.The changes of the above indicators before and after the intervention were analyzed,and whether there was statistical significance was analyzed,so as to evaluate the intervention effect.Results Before the intervention,489 medical records of orthopaedics patients were treated with adjuvant drugs,492 medical records of orthopaedics patients were treated with adjuvant drugs after the intervention.After the intervention,the reasonable utilization rate of drugs (85.37%),the reasonable rate of indications (93.09%),the reasonable rate of usage and dosage (95.93%),the reasonable rate of menstruum (93.90%),the reasonable rate of course of treatment (89.84%) and the reasonable rate of combined drugs (94.30%)were all higher than those before the intervention (41.72%,81.60%,83.23%,85.89%,83.64%,85.89%),respectively,with statistically significant differences (P＜0.05).Conclusion After the intervention of clinical pharmacists,the irrational use of auxiliary drugs for orthopedic inpatients in our hospital has been significantly improved,reducing the risks of clinical drug use and the economic burden of patients,and making drug use more rational.
