伤椎固定保留后方韧带复合体联合后路伤椎植骨短节段固定在胸腰椎骨折患者中的应用效果

摘要目的探讨伤椎固定保留后方韧带复合体联合后路伤椎植骨短节段固定在胸腰椎骨折患者中的应用效果。方法回顾性分析2017年11月～2019年6月我院收治的32 例胸腰椎骨折患者的临床资料，根据治疗方式的不同将其分为研究组（16 例）与对照组（16 例）。对照组采用伤椎固定保留后方韧带复合体的治疗方法，研究组采用伤椎固定保留后方韧带复合体联合后路伤椎植骨短节段固定的治疗方法。比较两组的临床效果、术后并发症发生情况。结果研究组的治疗总有效率高于对照组，差异有统计学意义（P＜0.05）；研究组的术后并发症总发生率低于对照组，差异有统计学意义（P＜0.05）；研究组的手术时间、下床时间及住院时间短于对照组，术中出血量少于对照组，差异均有统计学意义（P＜0.05）。结论胸腰椎骨折患者采用经伤椎固定保留后方韧带复合结合后路伤椎植骨短节段固定手术治疗效果理想，可有效降低患者术后并发症发生率，减少术中出血量，缩短住院治疗的时间，临床可以推广应用。

Abstract：Objective To investigate the application effect of traumatic vertebral fixation with posterior ligament complex preservation combined with posterior vertebral bone graft and short segment fixation in patients with thoracolumbar fracture. Methods The clinical data of 32 patients with thoracolumbar fracture admitted to our hospital from November 2017 to June 2019 were retrospectively analyzed. According to different treatment methods, they were divided into study group (16 cases) and control group (16 cases). The control group was treated with injured vertebra fixation and preservation of posterior ligament complex. The study group was treated with injured vertebra fixation and preservation of posterior ligament complex. The treatment of syndesmosis combined with posterior vertebral bone graft and short segment fixation. The clinical effect and complications were compared between the two groups. Results The total effective rate of the study group was higher than that of the control group, and the difference was statistically significant (P＜0.05). The total incidence of postoperative complications in the study group was lower than that in the control group,and the difference was statistically significant (P＜0.05). The time of operation, time of getting out of bed and time of hospitalization in the study group were shorter than those in the control group, and the amount of intraoperative blood loss was less than that in the control group, with statistically significant differences (P＜0.05). Conclusion For patients with thoracolumbar fractures, the treatment effect is ideal, which can effectively reduce the incidence of postoperative complications, reduce the amount of intraoperative blood loss, and shorten the length of hospitalization, which can be popularized and applied clinically.
