Clinical effect of whole-lung lavage in the treatment of pneumoconiosis

摘要目的 观察大容量全肺灌洗(WLL)治疗肺尘埃沉着病的临床效果。方法 纳入2016年1月~2019年4月我院收治的64 例肺尘埃沉着病患者作为研究对象,所有患者均接受大容量全肺灌洗术治疗,评价患者灌洗前后症状评分、动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)、每分钟最大通气量(MVV)、用力肺活量(FVC)、第一秒末用力呼气量(FEV1)。结果 患者灌洗后7 d 的咳嗽症状评分[(0.98±0.10)分]、咳痰评分[(1.05±0.11)分]、活动受限评分[(1.02±0.10)分],均低于灌洗前[(2.05±0.18)、(2.61±0.34)、(2.10±0.19)分],差异有统计学意义(P<0.05)。患者灌洗后7 d PaO2为(12.37±1.13)kPa,高于灌洗前的(9.49±1.25)kPa,差异有统计学意义(P<0.05);患者灌洗后7 d PaCO2 为(4.68±0.20)kPa,低于灌洗前的(5.19±0.40)kPa,差异有统计学意义(P<0.05)。患者灌洗后7 d MVV 为(86.10±1.10)L、FVC 为(2.99±0.16)L、FEV1 为(2.89±0.06)L,均高于灌洗前的(79.69±1.20)、(2.80±0.10)、(2.67±0.10)L,差异有统计学意义(P<0.05)。结论 肺尘埃沉着病病采用WLL 治疗疗效乐观,可有效改善患者症状、肺功能、血气值。
Abstract:Objective To observe the clinical effect of whole-lung lavage (WLL) in the treatment of pneumoconiosis.Methods A total of 64 pneumoconiosis patients underwent WLL from January 2016 to April 2019 in our hospital were included. The symptom scores as well as the values of partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2), maximal ventilatory volume (MVV), forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) were appraised. Results After 7 d of lavage, the symptom scores of cough, expectoration and limitation of motion were (0.98±0.10), (1.05±0.11) and (1.02±0.10) points, which were lower than those before lavage ([2.05±0.18],[2.61±0.34], [2.10±0.19]points), the differences were statistically significant (P<0.05). The value of PaO2 after 7 d of lavage was (12.37±1.13) kPa, which was higher than that before lavage ([9.49±1.25]kPa) (P<0.05). The value of PaCO2 after 7 d of lavage was (4.68±0.20) kPa, which was lower than that before lavage ([5.19±0.40]kPa) (P<0.05). The MVV([86.10±1.10]L), FVC ([2.99±0.16]L) and FEV1 ([2.89±0.06]L) were higher than those before lavage ([79.69±1.20),[2.80±0.10]and [2.67±0.10]L) (P<0.05). Conclusion WLL is proven to be effective, which can improve patients′symptoms, pulmonary function and blood gas indicators.
